Abstract:Objective To investigate the value of fetal intelligent navigation echocardiography (FINE,5D Heart)in displaying the normal fetal heart structure during 20 to 38 weeks'gestation.Methods Three-dimensional data of fetal heart during 20 to 38 weeks'gestation were acquired using volume transducer prospectively.The STIC volume images were post-processed with 5D heart software by three physicians with different levels of echocardiographic examination and the quality of the section images were analyzed.The intra-and interobserver repeatability was analyzed for the qualification of section images.The correlation between the quality of image and gestational age,body mass index,the placenta position,fetal initial plane position were evaluated using univariate and multivariable logistic regression analysis.Results Two hundreds and five cases of pregnant women were included,and 9 cases were excluded.Volume data acquisition was completed in 180 pregnant women,and the image acquisition rate was 91.8%.The data were assessed twice by three observers and qualified 9 section images were gotten in 156-180 cases(degree of qualification 86.7%-100%).The screening images were satisfied in 152 cases(84.4%),dissatisfied with 28 cases(15.6%).The intra-observer agreement by three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in three vessel trachea view(3VT),five chamber view, left ventricular outflow tract view and superior and inferior vena cava view;good in right ventricular outflow tract view,ductal arch view and aortic arch view.The inter-observer agreement by each of three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in superior and inferior vena cava view;excellent to good in five chamber view,left ventricular outflow tract view,right ventricular outflow tract view;good in 3VT,ductal arch view and aortic arch view.In a univariate analysis,the quality of the image was associated with obesity(χ2=15.338,P =0.000),gestational age(χ2=23.356,P =0.000).Multivariable analysis showed that the obesity(OR=5.587,P =0.001)and gestational age (OR=8.843,P=0.000)were factors affecting the quality of the image.The average time spent in post-processing the images for first time by three observers was(2.77±1.39)min,and there was no significant difference among observers(P > 0.05).Conclusions Fetal intelligent navigation echocardiography (FINE,5D Heart)based on STIC volumes can successfully generate 9 standard fetal echocardiography views in 86.7%-100% of fetuses in the 2nd and 3rd trimesters.This method can simplify examination of the fetal heart and reduce operator dependency.

Abstract:Objective To explore the applicability of fetal intelligent navigation echocardiography (FINE, 5D Heart) in displaying the normal fetal heart structure by a new sonographer and assess the value of this technique in fetal cardiac ultrasonography. Methods Eighty cases of normal fetal heart at the second trimester were scanned by an echocardiographic expert. STIC volume datasets with satisfactory from apical four-chamber and common 2D modality with 9 standard views of fetal echocardiogram were achieved for each case. The STIC volume images were post-processed with 5D heart software by two physicians with different levels of echocardiographic examination. The quality of the section images of 9 standard views from 2D 5D heart and were analyzed. The intraobserver and interobserver with common 2D modality repeatability were analyzed for the qualification of section images. The time spent in post-processing the images was record and analyzed. Results The qualified 9 section images with 2D were 75~80 cases (degree of qualification 93.8%~100.0%). The data were assessed twice by two observers and qualified 9 section images were got in 73~80 cases (degree of qualification 91.3%~100.0%). The intra-observer agreement by two observers was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in three vessel trachea view (3VT, Kappa=0.851~0.882); good to excellent in five chamber view (Kappa=0.787~0.882); good in aortic arch view (Kappa=0.738~0.787), left ventricular outflow tract view (Kappa=0.709~0.787), right ventricular outflow tract view (Kappa=0.655~0.794), superior and inferior vena cava view (Kappa=0.647~0.649) and ductal arch view (Kappa=0.640~0.707). The inter-observer agreement by each two observers and 2D was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in five chamber view (Kappa=0.851~0.902); good to excellent in ductal arch view (Kappa=0.749~0.820), aortic arch view (Kappa=0.707~0.882), right ventricular outflow tract view (Kappa=0.661~0.851) and 3VT (Kappa=0.655~0.851); good in left ventricular outflow tract view (Kappa=0.737~0.749), superior and inferior vena cava view (Kappa=0.655~0.794). The average time spent in post-processing the images for first time by three observers was (2.64±1.35) min, and there was no significant difference during the each observers (P>0.05). Conclusions 5D Heart represents a single method in obtaining 9 fetal diagnosis planes, which can be used easily by a sonographer lack of skills, and has high reliability and repeatability. Abstract:A rare case of ectopic scrotum is described together with a review of the literature and a discussion of the embryological role of the gubernaculum in the formation and location of normal and ectopic scrota. We identified 16 reported cases of a suprainguinal ectopic scrotum, 4 cases of a femoral ectopic scrotum, 26 cases of penoscrotal transposition, and 19 cases of a perineal (accessory) scrotum. Although the gubernaculum is a prerequisite for the ultimate location of both the testis and scrotum, its role is complicated by the subsequent differential growth of the labioscrotal folds in which the gubernaculum is stabilized. If this interaction is disturbed, the result may be a suprainguinal ectopia, penoscrotal transposition or a perineal scrotum. A femoral ectopic scrotum, unlike the above, is the result of an aberrant gubernacular stabilization. While the etiology of these malformations is likely to be multifactorial, the existence of an inbred strain of rats characterized by a high incidence of an ectopic scrotum suggests a genetic component to this anomaly.
Improvement of Diagnostic Efficiency in Fetal Congenital Heart Disease Using Fetal Intelligent Navigation Echocardiography by Less-Experienced Operators

Abstract:Objective This study investigated the feasibility and accuracy of fetal intelligent navigation echocardiography (FINE) for the prenatal diagnosis of congenital heart disease (CHD) by inexperienced and experienced operators. Method In this prospective study, all volume data sets from 120 fetuses with a broad spectrum of CHD were acquired using spatiotemporal image correlation technology. The prenatal diagnostic procedures were performed by two operators with different experience (beginner: 1 year and expert: 15 years) using FINE and traditional fetal echocardiography. Data were analyzed on the time of examination and acquisition of results. Results Diagnoses made by FINE and traditional echocardiography were completely consistent with the final diagnosis of CHD in 98 (81.66%) versus 20 (16.66%) (P < 0.001) beginners and 87.50% (n = 105) versus 101 (84.16%) experts, respectively. On the contrary, there was significant difference using traditional echocardiography (16.66% versus 84.16%, P < 0.001) by two examiners. Furthermore, the examination time decreased when using FINE compared with using traditional echocardiography (beginner operators: 4.54 +/- 1.03 min versus 20.58 +/- 3.36 min, P < 0.001; expert operators: 3.89 +/- 0.96 min versus 12.73 +/- 1.62 min, P < 0.001). Conclusion Based on our results, a prenatal diagnosis of CHD can be made with high feasibility and accuracy using FINE compared with traditional fetal echocardiography for beginner operators.

Abstract:Objective:To investigate the feasibility of applying Fetal Intelligent Navigation Echocardiography (FINE) combined with Virtual Intelligent Sonographer Assistance (VIS-Assistance ?) in the prenatal screening of right aortic arch (RAA) with left-sided ductus arteriosus (LDA). Methods:A total of 32 fetuses with RAA and LDA during middle and late pregnancy in Zhejiang University Medical College Affiliated Sir Run Run Shaw Hospital from Jauary 2018 to Jauary 2020 were included in this retrospective study, and the datas about fetal cardiac three-dimensional volume were analyzed. The time-space correlation imaging (STIC) volume data were collected by using FINE by Senior doctor A. The two diagnostic elements of the transverse aortic arch on the right side of the trachea and the "U" -shaped vascular ring, as well as the three-vessel tracheal diagnostic section were obtained by low-aged doctor B and middle-aged doctor C by using VIS-Assistance ? technology. And then the detection rates of diagnostic sections and diagnostic elements for fetuses with RAA and LDA were calculated. The postpartum outcomes of fetuses with RAA and LDA were followed up. Results:Thirty-two fetuses were included in the study after excluding 2 cases due to the poor quality images, and the datas about fetal cardiac three-dimensional volume of the 32 fetuses were analyzed. The detection rate of one diagnostic element (the aortic arch on the right side of the trachea) were 84.4% vs 87.5% before VIS-Assistance ?, and 93.8% vs 93.8% after VIS-Assistance ? for each doctor B and doctor C , respectively. Another diagnostic element ( "U" -shaped vascular ring) were 78.1% vs 87.3% before VIS-Assistance ?, and 90.6% vs 90.6% after VIS-Assistance ? for doctor B and doctor C, respectively. But no significant difference was found before and after VIS-Assistance ? between the two each doctors(all P>0.05). The detection rate of three-vascular tracheal diagnosis view were 65.6% vs 71.9% before VIS-Assistance ? and 84.4% vs 87.5% after VIS-Assistance ? for doctor B and doctor C, respectively. There was significant difference before and after VIS-Assistance ? of doctor C( P<0.05). The scores of image quality after VIS-Assistance ? were significantly higher than that before VIS-Assistance ? for doctor B and doctor C, respectively [3(2.5, 3) vs 3.25(3.0, 3.5), and 3(2.5, 3.5) vs 3.5(3.0, 3.5)]. The agreement between two doctors performing VIS-Assistance ? was investigated using Bland-Altman analysis and the result showed that within 95% of the differences fall in the agreement interval. No obvious clinical symptoms of compression were found in 32 neonates after follow-up. Conclusions:The application of FINE combined with VIS-Assistance ? technology can easily and reliably obtain the key diagnostic view of RAA with LDA (three-vessel and tracheal view), and clearly display all diagnostic elements, having high repeatability and stability. VIS-Assistance ? technology can improve the detection rate and image quality even if the doctor was lack of experience. So it can be used as an effective supplementary means for prenatal screening of RAA and LDA.

Abstract:Objective:To evaluate the value of Fetal Intelligent Navigation Echocardiography (FINE) and Virtual Intelligent Sonographer Assistance (VIS-Assistance?) in the screening of fetal double-outlet right ventricle (DORV).Methods:This study included 30 pregnant women diagnosed with fetal DORV who underwent fetal echocardiography at the Sir Run Run Shaw Hospital from September 2018 to December 2019. Three to five sets of three-dimensional volume data of the heart were collected and the best image quality was selected using the FINE technique for off-line analysis. The diagnostic plane image was optimized by using VIS-Assistance? function, and nine standard diagnostic planes were obtained. The essential information of four diagnostic sections (three-vessel tracheal view, four chamber view, left ventricular outflow view , and right ventricle outflow view) was scored, the display rates of each diagnostic plane and its diagnostic factors were analyzed, and the scoring results obtained by the same observer in different periods and those obtained by different observers in the same period were compared.Results:FINE and VIS-Assistance? were applied to the fetal volume data in 30 cases, of which 25 were analyzed, with a success rate of 83.3%. The display rates of the four diagnostic sections were 84.0%, 88.0%, 72.0%,and 84.0%, respectively. There was no significant difference in the scores obtained by the same observer at different time points, the scores obtained by two relatively inexperienced physicians, and the scores obtained by two relatively inexperienced physicians and one experienced physician (P＞0.05).Conclusion:FINE combined with VIS-Assistance? demonstrates a high success rate in producing four specific cardiac diagnostic sections in fetuses with DORV, has good consistency and repeatability, and therefore can be used for prenatal screening of fetal DORV.

Abstract:Background: Prenatal ultrasound is the preferred modality for diagnosing fetal congenital heart disease. Given issues of physician proficiency and hospital distribution, we propose a dynamic sequential cross-sectional scanning (SCS) to explore the feasibility of cardiac screening by sonographers with less than 5 years of experience in ultrasound. Materials and methods: Twenty residents were randomly divided into two groups, receiving training in the American Institute of Ultrasound in Medicine (AIUM) fetal echocardiography and the SCS method. According to the needs of training, the professional staff developed the theoretical knowledge question bank, the CHD ultrasonic video disease bank, and the assessment scale. Trainees completed the pre-training examination, theory and skill operation training, and post-training assessment. For the two groups, the theoretical knowledge, skill operation and disease diagnosis were analyzed statistically before and after training. Results: After training, the trainees in both groups had significantly improved knowledge and diagnostic abilities, their diagnostic thinking about CHD was clear, and they could identify major or even all structural abnormalities and make a definite diagnosis. In terms of skill operation, both groups could complete all required scanning within the specified time. The scanning time of the SCS group was significantly lower than that of the AIUM group, and the effect of the receptor site in the AIUM group was significantly higher than that in the SCS group. Conclusion: SCS can be used as a new rapid fetal cardiac scanning method and try to popularize among echocardiographer.
Application and Analysis of Imaging Characteristics of Four-Dimensional Ultrasound in the Diagnosis of Fetal Cleft Lip and Palate

Abstract:Objective : In this paper, four-dimensional (4D) ultrasound scanning technology and level-set-based image segmentation algorithm are used to diagnose fetal cleft lip and palate to improve the detection rate and diagnostic accuracy of fetal cleft lip and palate cleft. Methods : Fifty-six fetuses were collected, and their type-B ultrasonic examination was cleft lip and palate. Also, they were identified as cleft lip and palate deformities after delivery or induced labor. Two-dimensional (2D) and 4D ultrasound scans were performed, and an ultrasound image of the fetal face was obtained using a level set image segmentation algorithm. The characteristics of 2D and 4D ultrasound images of cleft lip and palate fetuses were compared and analyzed. After the postpartum results, the comparison and analysis of the detection rate and diagnostic accuracy of fetal cleft lip and palate with 2D and 4D ultrasound were performed. Results : (1) The level set image segmentation algorithm can obtain a clear fetal facial image; (2) Fetal cleft lip and palate 2D and 4D have different imaging performances, and 4D imaging characteristics are more prominent and easier to distinguish; (3) The accuracy rate of 4D ultrasound for simple cleft lip is 100%, and that of 2D is 88.2%. The accuracy rate of 4D for cleft lip with complete cleft palate is 77.8%, and that of 2D is 69.4%. The detection rate and diagnostic accuracy of fetal cleft lip and palate in 4D ultrasound are higher. Conclusion : The level set image segmentation algorithm can well achieve the segmentation of the fetal face and improve the accuracy of ultrasound diagnosis. 4D ultrasound has good application value in the diagnosis of fetal cleft lip and palate, but the accuracy rate is less than 100%. Therefore, further research is needed to improve its accuracy rate.

Abstract:Objectives Well‐established clinical practice to assess progress in labor involves routine abdominal palpation and vaginal examination (VE). However, VE is subjective, poorly reproducible and painful for women. In this study, our aim is to evaluate the feasibility of systematically integrating transabdominal and transperineal ultrasound assessment of fetal position, psAOP, HPD and SCD to monitor labor progress in women undergoing induction of labor (IOL). We also aim at determining if ultrasound can reduce women's pain during examinations. Methods Women were recruited as they presented for IOL in three maternity units. Ultrasound assessments were performed in 100 women between 37+0 and 41+6 weeks' gestation. A baseline combined transabdominal and transperineal scan was performed, including the assessment of fetal biometry, umbilical artery and middle cerebral artery Dopplers, amniotic fluid index (AFI), fetal spine and occiput positions, psAOP, HPD, SCD, and cervical length. Intrapartum scans were performed instead of VEs according to protocol. Participants were asked to indicate their level of pain by verbally giving a pain score from 0 – 10 (with 0 representing no pain) during assessment. The repeated measures data were analyzed by mixed effect models to identify the significant factors that affected the relationship between psAOP, HPD, SCD and mode of delivery. Results 223 intrapartum ultrasound scans with a median of 2 scans per participant (interquartile range (IQR) = 1 – 3), and 151 VEs were performed with a median of 1 per participant (IQR = 0 – 2). There were no adverse fetal or maternal outcomes. After excluding those with epidural anesthesia during examination, median pain score for intrapartum scan was 0 (IQR = 0 – 1) and 3 for VE (IQR = 0 – 6). Cesarean delivery and epidural anesthesia were significantly associated with slower rate of change in psAOP, HPD and SCD. Maternal height, parity and neonatal birth weight did not affect ultrasound measurements of labor progress. Conclusions Comprehensive transabdominal and transperineal ultrasound assessment can be successfully used to assess progress in labor and can reduce the level of pain experienced during examination. Ultrasound assessment may be able to replace some transabdominal and VE examinations during labor. This article is protected by copyright. All rights reserved.

Enhancing trainee performance in obstetric ultrasound through an artificial intelligence system: randomized controlled trial

Abstract:Objective: Performing obstetric ultrasound scans is challenging for inexperienced operators; therefore, the prenatal screening artificial intelligence system (PSAIS) software was developed to provide real-time feedback and guidance for trainees during their scanning procedures. The aim of this study was to investigate the potential benefits of utilizing such an artificial intelligence system to enhance the efficiency of obstetric ultrasound training in acquiring and interpreting standard basic views. Methods: A prospective, single-center randomized controlled study was conducted at The First Affiliated Hospital of Sun Yat-sen University. From September 2022 to April 2023, residents with no prior obstetric ultrasound experience were recruited and assigned randomly to either a PSAIS-assisted training group or a conventional training group. Each trainee underwent a four-cycle practical scan training program, performing 20 scans in each cycle on pregnant volunteers at 18-32 gestational weeks, focusing on acquiring and interpreting standard basic views. At the end of each cycle, a test scan evaluated trainees' ability to obtain standard ultrasound views without PSAIS assistance, and image quality was rated by both the trainees themselves and an expert (in a blinded manner). The primary outcome was the number of training cycles required for each trainee to meet a certain standard of proficiency (i.e. end-of-cycle test scored by the expert at ≥ 80%). Secondary outcomes included the expert ratings of the image quality in each trainee's end-of-cycle test and the discordance between ratings by trainees and the expert. Results: In total, 32 residents and 1809 pregnant women (2720 scans) were recruited for the study. The PSAIS-assisted trainee group required significantly fewer training cycles compared with the non-PSAIS-assisted group to meet quality requirements (P = 0.037). Based on the expert ratings of image quality, the PSAIS-assisted training group exhibited superior ability in acquiring standard imaging views compared with the conventional training group in the third (P = 0.012) and fourth (P < 0.001) cycles. In both groups, the discordance between trainees' ratings of the quality of their own images and the expert's ratings decreased with increasing training time. A statistically significant difference in overall trainee-expert rating discordance between the two groups emerged at the end of the first training cycle and remained at every cycle thereafter (P < 0.013). Conclusion: By assisting inexperienced trainees in obtaining and interpreting standard basic obstetric scanning views, the use of artificial intelligence-assisted systems has the potential to improve training effectiveness. © 2024 International Society of Ultrasound in Obstetrics and Gynecology.
Application of the formative assessment in nonprocedural laparoscopic simulating training for surgeons

Abstract:Objective To explore the feasibility and effect of the application of the formative assessment in nonprocedural laparoscopic simulating training for surgeons in different seniorities.Methods A total of 55 surgeons in different seniorities were selected from Peking University Third Hospital as research object,including 42 refresher doctors on surgery and 13 graduate students on surgery for master degree.For them,a 4-week nonprocedural laparoscopic simulating training was carried out,in which the formative assessment was set up for 29 trainees as experimental group,and the summative assessment for the other 26 trainees as control group.The laparoscopic skills before and after training were evaluated in scores and compared statistically in both groups.Results All trainees had higher laparoscopic skills post-test scores than their pretest's,but the enhancement in suturing and knotting test for refresher doctors (17.1±2.9) was statistically higher than that for graduate students(14.2±3.5) (t =2.97,P=0.004).In graduate students,statistically higher scores in the basic skills post-test was obtained in experimental group[(12.6±0.8)vs(9.0±0.6),t =-8.91,P =0.000].In refresher doctors,statistically higher scores in suturing-knotting post-test was obtained in experimental group [(35.2±2.5) vs(33.0± 2.5),t =-2.83,P =0.007].Formative assessment showed different trends in the curves of skill enhancement between surgical refresher doctors and graduate students.Condusions Nonprocedural laparoscopic simulating training can feasibly improve laparoscopic skills,with variety between surgical refresher doctors and graduate students.Formative assessment is an effective method for surgeons in different seniorities during laparoscopic simulating training.

Abstract:Objective:To explore the methods of ultras on ography scanning for fetal lip, palate by three planes of verticality and to enh ance the detective rate of cleft lip or complicated cleft palate. Methods:Determine fetal position, then scanned fetal face on coronal, sagittal and transverse planes, to diagnose cleft lip, or complicate d cleft palate. Results:Of 67 cases of cleft lip or complicated cleft pala te validated by postpartum and induced labor observation, 62 fetus were detected prenatally (92.54%), 5 fetus were missed diagnosis (7.46%), no fake positive case. Abstract:Objective:To investigate the application value of WeChat-based problem-based learning teaching method combined with micro-classroom in the teaching of contrast-enhanced gastrointestinal ultrasound imaging for residents in standardized residency training.Methods:A total of 40 trainees who underwent the standardized residency training in the Department of Ultrasound, The First Affiliated Hospital of Chongqing Medical University from January 2019 to June 2021 were selected as the research objects. They were randomly divided into the experimental group (WeChat-based PBL teaching method combined with micro-classroom) and the control group (traditional teaching method). The teaching content was the diagnosis of gastrointestinal ultrasound imaging. After the theoretical learning and practice, assessments of contrast-enhanced gastrointestinal ultrasound imaging including theory and operating practice were performed to all students. SPSS 22.0 was used for t test and rank sum test.Results:After training, the theoretical test scores of the experimental group were higher than those of the control group [(93.09±2.31) vs. (90.63±2.26)], and the difference was statistically significant ( P<0.05). However, there was no significant difference between the two groups in the number distribution of students at all levels of operational practice ( P>0.05). Conclusion:The WeChat-based PBL teaching method combined with micro-classroom is beneficial to improve students' knowledge of contrast-enhanced gastrointestinal ultrasound imaging, and is worth being popularized and applied in clinical teaching.
Effectiveness of a modified doctor-patient communication training Programme designed for surgical residents in China: a prospective, large-volume study at a single Centre

Abstract:Background: Effective doctor-patient communication (DPC) is important to improve the quality of care and treatment outcomes. This study aimed to evaluate the effectiveness of a modified DPC training programme designed for surgical residents in China. Methods: A total of 210 surgical residents from 7 specialties were recruited to participate in a communication skills training programme. The extended SEGUE scale was administered to supervisors, and a visual analogue scale (VAS) was administered to standardized patients (SPs) and trainees. Evaluations were conducted by comparing the pre-post scale scores (before, immediately after and 1 month after the programme). Training effects were assessed using the Friedman test and the intraclass correlation coefficient (ICC). Results: Compared to the extended SEGUE scale total scores before the programme, the scores both immediately after and 1 month after the program increased significantly (15.88 ± 1.93 vs. 26.40 ± 1.47 and 26.15 ± 1.36, respectively, p < 0.001). The scores of five of the six dimensions in the extended SEGUE scale significantly increased (p < 0.001), except for the patient's perspective dimension score, which had no change (p = 0.162). With respect to this dimension, a subgroup analysis by gender indicated an increase among females (p < 0.001) and a decrease among males (p < 0.001). The VAS scores, which were evaluated for the SPs and trainees, increased significantly compared to the scores before the program, both immediately after and 1 month after the programme (4.31 ± 1.35 vs. 7.36 ± 1.62 and 7.34 ± 1.24, p < 0.001; 7.31 ± 1.25 vs. 8.39 ± 1.32 and 8.30 ± 1.24, p < 0.001, respectively). The consistency of the VAS between the SPs and surgical residents was 0.26 (p < 0.001), 0.70 (p < 0.001), and 0.70 (p < 0.001) before, immediately after and 1 month after the programme, respectively. Conclusion: This training programme improved the DPC competency of surgical residents, effectively increased the levels of satisfaction of both SPs and surgical residents, and improved the consistency of evaluation between SPs and surgical residents during doctor-patient encounters. Abstract:Loading of HAuCl4 in poly(amido amine) G4 dendrimers having poly(ethylene glycol) (PEG) grafts at all chain ends and subsequent reduction with NaBH4 yielded PEG-modified dendrimers encapsulating gold nanoparticles (Au NPs) of ca. 2 nm diameter. The Au NPs held in the dendrimers were stable in aqueous solutions and dissolved readily, even after freeze-drying. Despite their small particle size, the heat-generating ability of Au NPs held in the dendrimer was comparable to that of widely used Au NPs with ca. 11 nm diameter under visible light irradiation. The observed excellent colloidal stability, high heat-generating ability and their biocompatible surface confirm that the PEG-modified dendrimers encapsulating Au NPs are a promising tool for photothermal therapy and imaging.

Abstract:SCOPE Cereal arabinoxylan (AX) is one of the main dietary fibers in a balanced human diet. To gain insight into the importance of structural features of AX for their prebiotic potential and intestinal fermentation properties, a rat trial was performed. METHODS AND RESULTS A water unextractable AX-rich preparation (WU-AX, 40% purity), water extractable AX (WE-AX, 81% purity), AX oligosaccharides (AXOS, 79% purity) and combinations thereof were included in a standardized diet at a 5% AX level. WU-AX was only partially fermented in the ceco-colon and increased the level of butyrate and of butyrate producing Roseburia/E. rectale spp. Extensive fermentation of WE-AX and/or AXOS reduced the pH, suppressed relevant markers of the proteolytic breakdown and induced a selective bifidogenic response. Compared with WE-AX, AXOS showed a slightly less pronounced effect in the colon as its fermentation was virtually complete in the cecum. Combining WU-AX and AXOS caused a striking synergistic increase in cecal butyrate levels. WU-AX, WE-AX and AXOS together combined a selective bifidogenic effect in the colon with elevated butyrate levels, a reduced pH and suppressed proteolytic metabolites. CONCLUSION The prebiotic potential and fermentation characteristics of cereal AX depend strongly on their structural properties and joint presence.
Development and validation of an artificial intelligence assisted prenatal ultrasonography screening system for trainees

Abstract:Objective: Fetal anomaly screening via ultrasonography, which involves capturing and interpreting standard views, is highly challenging for inexperienced operators. We aimed to develop and validate a prenatal-screening artificial intelligence system (PSAIS) for real-time evaluation of the quality of anatomical images, indicating existing and missing structures. Methods: Still ultrasonographic images obtained from fetuses of 18-32 weeks of gestation between 2017 and 2018 were used to develop PSAIS based on YOLOv3 with global (anatomic site) and local (structures) feature extraction that could evaluate the image quality and indicate existing and missing structures in the fetal anatomical images. The performance of the PSAIS in recognizing 19 standard views was evaluated using retrospective real-world fetal scan video validation datasets from four hospitals. We stratified sampled frames (standard, similar-to-standard, and background views at approximately 1:1:1) for experts to blindly verify the results. Results: The PSAIS was trained using 134 696 images and validated using 836 videos with 12 697 images. For internal and external validations, the multiclass macro-average areas under the receiver operating characteristic curve were 0.943 (95% confidence interval [CI], 0.815-1.000) and 0.958 (0.864-1.000); the micro-average areas were 0.974 (0.970-0.979) and 0.973 (0.965-0.981), respectively. For similar-to-standard views, the PSAIS accurately labeled 90.9% (90.0%-91.4%) with key structures and indicated missing structures. Conclusions: An artificial intelligence system developed to assist trainees in fetal anomaly screening demonstrated high agreement with experts in standard view identification.

Abstract:Background: Following the development of the Royal Australian College of Obstetricians and Gynaecologists Intrapartum Fetal Surveillance Guideline in 2003, an education program was developed to support guideline implementation and clinical practice. It was intended that improved clinician knowledge, particularly of cardiotocography, would reduce rates of intrapartum fetal morbidity and mortality. The program contains a multiple-choice assessment, designed to assess fetal surveillance knowledge and the application of that knowledge. We used the results of this assessment over time to evaluate the impact of the education program on clinicians' fetal surveillance knowledge and interpretive skills, in the immediate and longer-term. Methods: We undertook a retrospective analysis of the assessment results for all participants in the Fetal Surveillance Education Program, between 2004 and 2018. Classical Test Theory and Rasch Item Response Theory analysis were used to evaluate the statistical reliability and quality of the assessment, and the measurement invariance or stability of the assessments over time. Clinicians' assessment scores were then reviewed by craft group and previous exposure to the program. Results: The results from 64,430, broadly similar assessments, showed that participation in the education program was associated with an immediate improvement in clinician performance in the assessment. Performance improvement was sustained for up to 18 months following participation in the program and recurrent participation was associated with progressive improvements. These trends were observed for all craft groups (consultant obstetricians, doctors in training, general practitioners, midwives, student midwives). Conclusions: These findings suggest that the Fetal Surveillance Education Program has improved clinician knowledge and the associated cognitive skills over time. The stable difficulty of the assessment tool means any improvement in clinician's results, with ongoing exposure to the program, can be reliably assessed and demonstrated. Importantly this holds true for all craft groups involved in intrapartum care and the interpretation of cardiotocography.
Application of blended teaching to forceps simulation training

Abstract:Objective:To apply and evaluate a blended teaching mode in forceps simulation training.Methods:A total of 28 obstetricians from an advanced training program of obstetricians in obstetrics Department of Third Hospital, Peking University joined the course of forceps simulation training from October to December, 2021.The trainees were randomly divided into experiment group and control group with 14 in each. The blended teaching mode defined by combination of online and offline teaching with simulation training was applied in experiment group and the trainees of control group was trained with traditional method characterized by off line teaching plus simulation technology assistant training. The training effect was evaluated by clinical skill assessment and questionnaire survey. Fisher' s exact test was used for data analysis. Results:Except for catheterization and prepare remedial plan after failure of forceps delivery, the proportion of trainees with high score in examinations of theoretical knowledge and clinical skill in experimental group was significantly higher than that in control group. For example, the proportion of trainees got 5 points in the informed consent, operation, timing and direction of forceps traction in experiment group reached 14/14 and 13/14 respectively while the proportion found in control group was 5/14 and 3/14 respectively( P<0.001). The results of questionnaire survey showed that the proportion of high score in the level of theoretical knowledge, clinical skill, learning interest, the ability of autonomous learning and approbation of teaching mode in the experiment group was significantly higher than that in the control group. For example, the proportion of trainees who got 5 points in approbation of teaching mode reached 12/14 in experimental group and 0/14 in the control group ( P<0.001). Conclusions:The blended teaching mode improves the effect of forceps simulation training, learning interest, autonomous learning ability of trainees of post-graduate education program. The method is well acknowledged and accepted by obstetricians.
Quality Control of Fetal Images in the Assessment of Trainee Competence at Second-Trimes-ter Structure Screening

Abstract:Objective:To improve the accuracy of fetus structure screening in ultra-sound training,combined quality control of image scoring and cumulative sum( CUSUM) analy-sis in second-trimester prenatal structure screening were carried out. Methods:A total of 28 standardized ultrasound planes of fetal organs,such as central nervous system,heart,digestive system were chosen. All images obtained by 3 ultrasound doctors at mid-trimester scan were stored. These images were analyzed by two reviews,according to predefined criteria. The score difference among three doctors and the total score was delineated using CUSUM,and the failure rate was set 10%. Interreviewer and Intrareviewer agreement was assessed using intraclass cor-relation coefficient ( ICC ) . Results:The images obtained by three doctors scoring above 234 were more than 80 . The ICC in image scoring showed good inter-and intrareviewer ( A1 and B 0. 985,A2 and B 0. 977,A1 and A2 0. 992). The CUSUM plots showed that screening compe-tence was achieved by 74、89 and 61 tests for three doctors,respectively. Conclusion:With the combined quality control methods, we effectively improved the quality of fetus structure ultra-sound screening and certified the competence of prenatal ultrasound screening in ultrasound training.
