Abstract:Objective To explore feasibility and safety of endoscopy combined with and laparoscopy in the treatment of cholecystoiithiasis and extrahepatic bile duct cholelithiasis.Methods A total of 82 patients with cholecystoiithiasis and extrahepatic bile duct cholelithiasis were randomly divided into two groups,including 41 cases of treatment group,41 cases of control group.The treatment group was treated with endoscopic retrograde cholangiopancreatography(ERCP),endoscopic sphincterotomy(EST) and laparoscopic cholecystectomy(LC).The control group was treated with traditional operation.The success rate of operation,complications,operation time,feeding time after operation,stay-in-hospital time after operation were compared.Results The success rate of operation in treatment group was 95.1% and 92.7% in control group,and the difference was not significant(P>0.05).The operation time,blood loss,feeding time after operation,stay-in-hospital time after operation in treatment group were significantly better than those in control group(P<0.05).There was no significant difference between the two groups in the rate of complication(P<0.05).Conclusion It is a safe,effective and feasible microtranma surgery for treating cholecystoiithiasis and extrahepatic bile duct cholelithiasis with endoscopy combined with and laparoscopy.
The comparative analysis between two methods for minimally invasive surgery in the treatment of cholecystolithiasis and choledocholithiasis

Abstract:Objective To evaluate the effects of laparoscopic common bile duct exploration (LCBDE) and endoscopic retrograde cholangiopancreaticography and endoscopic sphincterotomy(ERCP/EST) combined with laparoscopic cholecystectomy(LC) in the treat-ment of cholecystolithiasis and choledocholithiasis. Methods The clinical data of patients suffering from cholecystolithiasis and choledocholithiasis from Jan.2012 to Jul.2013 were retrospectively analyzed. LC+LCBDE was performed in 124 cases and ERCP/EST combined LC was performed in 123 cases. Parameters including common bile duct (CBD) diameter, stone size, hospitalization cost, hospital stay, postoperative complications, residual bile duct stone incidence were comparatively studied. Results There was no statistical difference in the number of stone , conversion rate and residual bile duct stones incidence. But there were significant dif-ference in CBD diameter, stone size, hospitalization cost, hospital stay, and early postoperative complication. Conclusion Both of LC+LCBDE and ERCP/EST+LC were safe and reliable to treat patients with cholecystolithiasis and choledocholithiasis. The treatment for patients must be individualized. LC+LCBDE was better than ERCP/EST combined LC.
Characteristics of serum lipids,leptin,cholecystokinin and bile lipids in patients with cholecystolithiasis

Abstract:Objective To investigate the association of serum lipid,lipoprotein,apolipoprotein,leptin,cholecystokinin(CCK)and bile lipid with cholesterol gallstone formation.Methods The patients with gallstone were divided into cholesterol(n=99)and non-cholesterol(n=57)gallstone groups by infrared spectometry.And 52 healthy volunteers were served as control group.The concentrations of serum cholesterol,triglyceride,high and low density lipoproteins,apolipoprotein(Apo),leptin and CCK were measured and compared among three groups.The levels of total bile cholesterol,bile acid and lecithin were also detected.Results The concentrations of serum triglyeeride and total cholesterol in two gallstone groups were higher than those in control group(P value all＜0.01).The level of Apo-B in cholesterol gallstone group was higher than that in control group(P=0.017).While the concentrations of high density lipoprotein and CCK were significantly lower in two gallstone groups than those in control group(P value all=0.000).Serum leptin was higher in male patients compared to controls(P＜0.05).The bile cholesterol saturation index in two gallstone groups was above 1.Conclusions The changes of serum CCK,triglyceride,total cholesterol,high density lipoprotein,Apo-B and leptin may be correlated to the formation of gallbladder gallstone.
Intraoperative ERCP and endoscopic sphincterotomy combined with laparoscopic cholecystectomy for treatment of cholelithiasis with choledocholithiasis

Abstract:Objective: To evaluate the benefit of intraoperative ERCP and endoscopic sphincterotomy (EST) combined with laparoscopic cholecystectomy (LC) in the treatment of cholelithiasis with choledocholithiasis. Methods: Fifty-two patients with cholelithiasis and choledocholithiasis (as determined by intraoperative cholangiography) were randomly divided into 2 groups during LC. In group A (27 patients), common bile duct stones were extracted by intraoperative EST during LC. In group B(25 patients), common bile duct (CBD) stones were extracted by conversion to open CBD exploration and cholecyctectomy. Results: The success rate was 26/27 (96.3%) in group A and 25/25 (100%) in group B (0.25< P <0.5); The mean postoperative hospitalization was 3.32±0.56 days in group A and 17.5±4.61 days in group B ( P <0.001). In group A, two cases were complicated transient hyperamylasemia after the combined procedure. In group B, one case of bile leakage and one case of duodenal ulcer occurred after conversion to open cholecystectomy with CBD exploration. There were no retained stones in group A but 2 cases in group B. Conclusion: Intraoperative ERCP and endoscopic sphincterotomy combined with LC for treatment of cholelithiasis and chiledochlithiasis is safe, effective and results in shorter hospitalization and fewer complications than traditional open cholecystectomy with CBD exploration.

Abstract:Objective To evaluate the effects of Laparoscopic cholecystectomy (LC) combined with intraoperative cholanggiography (IOC)and intraoperative endoscopic sphincterotomy (IOEST) for the treatment of cholecystocholedocolithiasis. Methods 106 cases with cholecystocholedocolithiasis diagnosed and treated by LC combined with IOC and IOEST were reviewed retrospectively. Results 60 40%(64/106)patients diagnosed cholelithiasis pre-LC were found choledocholithisis; LC combined with IOC and IOEST was successfully performed on 99 of 106 cases(93 40%),and their CBD stones were completely cleared in 98 of 99 cases(99 00%). Six cases(6 1%) were complicated with mild acute pancreatitis, 1 case(0 01%) with duodenal perforation,1 case(0 01%) with bile leakage, and 2 cases(0 02%) with stomach stasis. Conclusions LC combined with IOC and IOEST was a safe ,effective approach to diagnosis and minimally invasive treatment of cholecystocholedocholithiasis.

Abstract:Objective To evaluate the effects of laparoscopic cholecystectomy(LC)combined with intraoperative endoscopic sphincterotomy(IOEST)for the treatment of acute cholangitis due to cholecystocholedocolithiasis.Methods Statistical analysis was carried out for 18 patients with acute cholangitis due to cholecystocholedocolithiasis diagnosed and treated by LC combined with IOEST.Therapeutic effects were evaluated on the basis of the cure rate,incidence of early complications and days of hospitalization.Results LC with IOEST was successfully performed in 17 of 18 cases(94.44%),and their CBD stones were cleared completely in 16/18(88.89%)cases.Two cases(11.12%)were undertaken extraction of retained common bile duct stones by second EST,1 case(5.56%)complicated with mild acute pancreatitis,1 case(5.56%)missed diagnosis with duodenal adenocarcinoma,and 1 case(5.56%) was complicated with stomach stasis.Conclusion LC combined with IOEST is a safe,effective approach to minimally invasive treatment of acute cholangitis due to cholecystocholedocholithiasis.
Effects of Danning Tablet on the Bile Components and the Lithogenic Tendency in Patients with Cholelithiasis

Abstract:Objective To study the influence of Danning tablet on the bile components and the lithogenic tendency in patients with cholelithiasis.Methods Sixty patients with cholelithiasis undergoing cholecystectomy were randomly divided into Danning tablet group,ursodeoxycholic acid(UDCA) group and control group,with corresponding medications after the operation.During and after operation,bile total bilirubin(TB),direct bilirubin (DB),cholesterot(CH),phospholipid(PHL) and total bile salt(TBS) were measured for each patient,and the lithogenic index(LI),percentage of unconjugated bilirubin(UCB%) and the Z value for estimating the bile litho- genic tendency were calculated.Results Five days after the operation,the contents of the bile components, UCB%,LI and the Z value were not significantly different between the 3 groups(all P0.05),and the Z value remained greater than the critical value(44.7).In UDCA group and the control group,the contents of the bile components,UCB%,LI and the Z value on day 19 after the operation exhibited no significant difference from the measurements on day 5(all P0.05).In Danning tablet group,the CH level in the bile and the UCB%showed significant reduction;TBS and PHL exhibited significant increase on postoperative day 12(all P0.05);and on day 19,the UCB%,LI and the Z value were all significantly decreased(all P0.05),with a Z value below the critical value.Conclusion Danning tablet can modulate the bile components,and thus may reduce the tendency of gallstone formation.(Shanghai Med J,2008,31:104-106)

Abstract:AIM:To observe the prevention and cure effects of Shu Dan Tong granules(SDT) on experimental cholelithiasis in guinea pigs.METHODS:62 DHA guinea pigs were randomly divided into normal group,model control group,positive control group,as well as SDT 5,10,20 g(crude drug,the same below)/kg dose group,feed with high cholesterol and low protein lithogenic diet in addition to the normal group,and all guinea pigs in different group were given corresponding drugs simultaneously by gavage administration for 70 days,and the general condition,stone formation,bile secretion and composition,blood biochemistry were observerd or detected.RESULTS:SDT can significantly improve the general condition of cholelithiasis in guinea pigs,lower lithogenesis rate and improve total bile acid(TBA) content in doses of 5,10,20 g/kg,and can significantly improve the bile secretion and plasma cholecystokinin(CCK) levels,lower serum total cholesterol(TC) levels in doses of 10,20 g/kg.CONCLUSION: SDT has the prevention and cure effects on cholelithiasis modle of guinea pigs.
Effects of Ursodeoxycholic Acid on Intrahepatic Cholestasis in Rats

Abstract:Objective To investigate the effects of treatment with ursodeoxycholic acid (UDCA) on intrahepatic cholestasis in rats, and to explore its mechanism.Method Rats suffering from intrahepatic cholestasis were treated with UDCA. Their serum alanine transaminase (ALT), alkaline phosphatase (ALP), total bilirubin (TBIL), direct bilirubin (DBIL), γ-glutamyl transpeptidase (γ-GT), total cholesterol (TCH), bile flow, total bile acid excretion, total Na+ and TCH of bile were measured before and after treatment. In addition, the changes of liver tissue under microscrope were observed and recorded.Results Compared with the control group, serum ALT, ALP, TBIL, DBIL, γ-GT and TCH of rats in the treatment group decreased, while bile flow, total bile acid excretion, total Na+ and TCH decreased significantly. Degeneration of hepatocytes, infiltration of inflamed cells and proliferation of small bile ducts in the treatment group were improved under microscope.Conclusion UDCA may have therapeutic effects on cholestatic hepatitis. The mechanism may involve in its hydrophilicity, choleretic effect and immune modulation.
A Comparative Clinical Study for the Relapse of Cholelithiasis after Gallbladder-preserving Lithotomy between Tauroursodeoxycholic Acid and Ursodeoxycholic Acid

Abstract:Objective To evaluate the clinical effect between tauroursodeoxycholic acid ( TUDCA) and ursodeoxycholic acid ( UDCA) on preventing the relapse of cholelithiasis after gallbladder-preserving lithotomy via choledochoscope . Methods A total of 313 patients of cholecystolithiasis in Peking University Shougang Hospital undergoing gallbladder -preserving lithotomy via choledochoscope were divided into TUDCA and UDCA group by odd and even numbers of their medical record .The TUDCA group took 500 mg TUDCA (commodity name: Taurolite, Bruschettini S.r.l., Italy, import approval number: H20070200/H20110233) once per night, for 5 days consecutively, and withdrew the drug for 10 days, 15 days was considered a cycle;the UDCA group took 500 mg UDCA ( commodity name:Ursofalk, Dr.Falk Pharma GmbH, German, import approval number: H20050181) once per night .The total course of treatment of the two groups was 180 d.Nursing and diet therapies were the same in the two groups .The cases were checked at regular intervals after surgery with B ultrasonography and were followed up .The recurrence rate of calculi , the change of the thickness of gallbladder wall and the rate of gallbladder contraction were recorded . Results The two groups were followed up for 12-30 months.The 2-year recurrence rates of cholecystolithiasis were 3.7% and 12.4% respectively in the two groups ( log-rankχ2 =5.304, P =0.021 ). Conclusion The effect of TUDCA is better than that of UDCA in preventing the relapse of cholecystolithiasis for patients with gallbladder-preserving lithotomy via choledochoscope .

Abstract:Aim: Tauroursodeoxycholic acid (TUDCA) is a taurine conjugated form of ursodeoxycholic acid (UDCA) with higher hydrophility. To further evaluate the efficacy and safety of TUDCA for primary biliary cholangitis (PBC), we performed this study on Chinese patients.Methods: 199 PBC patients were randomly assigned to either 250mg TUDCA plus UDCA placebo or 250mg UDCA plus TUDCA placebo, 3 times per day for 24 weeks. The primary endpoint was defined as percentage of patients achieving serum alkaline phosphatase (ALP) reduction of more than 25% from baseline.Results: At week 24, 75.97% of patients in the TUDCA group and 80.88% of patients in the UDCA group achieved a serum ALP reduction of more than 25% from baseline (P=0.453). The percentage of patients with serum ALP levels declined more than 40% following 24 weeks of treatment was 55.81% in the TUDCA group and 52.94% in the UDCA group (P=0.699). Both groups showed similar improvement in serum levels of ALP, aspartate aminotransferase, and total bilirubin (P>0.05). The proportion of patients with pruritus/scratch increased from 1.43% to 10.00% in UDCA group, while there's no change in TUDCA group (P=0.023). Both drugs were well tolerated, with comparable adverse event rates between the 2 groups.Conclusions: TUDCA is safe and as efficacious as UDCA for the treatment of PBC, and may be better to relieve symptoms than UDCA.

Abstract:To explore the clinical efficacy of microscope combined with Wechat smart platform in patients with cholecystolithiasis and choledocholithiasis, this paper proposes the effect of microscope combined with Wechat smart platform intervention after laparoscopic lithotomy and choledocholithotomy on the clinical efficacy and gastrointestinal function of patients with cholecystolithiasis combined with choledocholithiasis. From February 2018 to March 2019, 78 patients with gastric cancer were selected by our clinic and included in the research team. Evaluate the efficacy of endoscopic biliary lithotomy (LBL) + bile duct lithotomy (TBL) + T-tube drainage therapy to provide reliable evidence for improved efficacy and efficacy in order to provide a strong reference for improving the effectiveness and safety of surgical treatment of choledocholithiasis. Safety of surgical treatment of diseases. Gallstone disease. The experiments did not show any significant differences between the two groups during surgery. Diabetes was lower in the control group, and hospital incidence was lower in the control group. There were no significant differences between the two groups for preoperative WHOQOL-100 scores. Two weeks and four weeks after surgery, the man-key-100 score was higher than that of the control group. Endoscopic cholecystectomy + choledocholithotomy + choledochoscopic lithotomy + T-tube fluid have been shown to be effective in promoting rapid intestinal function and improving patient quality of life and are appropriate for therapeutic use.

Abstract:Objective:Evaluate the influence of laparoscopy combined with choledochoscopy on operation-related indexes, serum total bilirubin (TBIL) level, and abdominal drainage tube extraction time within cases carrying cholecystolithiasis/choledocholithiasis. Methods:86 cases of cholecystolithiasis together with choledocholithiasis were chosen for this investigation, and cases were randomly segregated within the control cohort (43 cases, open surgery) and observation cohort (43 cases, laparoscopy combined with choledochoscopy).The operation-related indexes, complete stone clearance rate, postoperative visual analogue scale (VAS) scoring, serum TBIL level, and postsurgical complications/recovery incidence were observed and comparatively analyzed across cohorts. Results:Compared with the control cohort, the incision length, operation duration, postoperative exhaust duration, abdominal drainage tube extraction time, and postsurgery hospitalization in observation cohort were markedly reduced (P < 0.05), the intrasurgical hemorrhaging was markedly reduced (P < 0.05), and the postoperative complication incidences were markedly reduced (P < 0.05). Furthermore, the complete stone clearance rates in the observation cohort were elevated compared with control, but the difference was not statistically significant (P > 0.05).VAS scoring for the observation cohort at 6, 12, 24, and 48 hours postsurgery was markedly reduced (P < 0.05). On the first day after the operation, the serum TBIL levels for the two cohorts were very high and gradually decreased, and the serum TBIL levels in the observation cohort were markedly reduced during day 1, 3, and 5 postsurgery (P < 0.05). Conclusion:Laparoscopy combined with choledochoscopy surgical treatment might reduce the surgery duration, intrasurgery hemorrhaging, postsurgical pain, and liver function damage.
Unique Distal Enhancers Linked to the Mouse Tnfsf11 Gene Direct Tissue-Specific and Inflammation-Induced Expression of RANKL.

Abstract:Receptor activator of nuclear factor κB ligand (RANKL) is expressed by a number of cell types to participate in diverse physiological functions. We have previously identified 10 distal RANKL enhancers. Earlier studies have shown that RL-D5 is a multifunctional RANKL enhancer. Deletion of RL-D5 from the mouse genome leads to lower skeletal and lymphoid tissue RANKL, causing a high bone mass phenotype. Herein, we determine the physiological role and lineage specificity of 2 additional RANKL enhancers, RL-D6 and RL-T1, which are located 83 and 123 kb upstream of the gene's transcriptional start site, respectively. Lack of RL-D6 or RL-T1 did not alter skeletal RANKL or bone mineral density up to 48 weeks of age. Although both RL-D5 and RL-T1 contributed to activation induction of T-cell RANKL, RL-T1 knockout mice had drastically low lymphocyte and lymphoid tissue RANKL levels, indicating that RL-T1 is the major regulator of lymphocyte RANKL. Moreover, RL-T1 knockout mice had lower circulating soluble RANKL, suggesting that lymphocytes are important sources of circulating soluble RANKL. Under physiological conditions, lack of RL-D6 did not alter RANKL expression. However, lack of RL-D5 or RL-D6, but not of RL-T1, blunted the oncostatin M and lipopolysaccharide induction of RANKL ex vivo and in vivo, suggesting that RL-D5 and RL-D6 coregulate the inflammation-mediated induction of RANKL in osteocytes and osteoblasts while lack of RL-D6 did not alter secondary hyperparathyroidism or lactation induction of RANKL or bone loss. These results suggest that although RL-D5 mediates RANKL expression in multiple lineages, other cell type- or factor-specific enhancers are required for its appropriate control, demonstrating the cell type-specific and complex regulation of RANKL expression.
Efficacy and influencing factors of ursodeoxycholic acid in the treatment of cholesterol gallstone

Abstract:Objective:To study the efficacy and influencing factors of ursodeoxycholic acid (UDCA) in the treatment of cholesterol gallstone, so as to provide reference for the treatment of cholesterol gallstone by internal medicine.Methods:From March 1, 2017 to March 31, 2018, at outpatient department of gastroenterology of 9 Beijing medical centers including Peking University People′s Hospital, the Sixth Medical Center of PLA General Hospital, Beijing Huaxin Hospital, PLA Rocket Force Characteristic Medical Center, Peking University Aerospace Center Hospital, Beijing Youan Hospital of Capital Medical University and Beijing Tiantan Hospital of Capital Medical University, Beijing Tongren Hospital of Capital Medical University, and Beijing Shijitan Hospital of Capital Medical University, the data of patients with cholesterol gallstone treated by UDCA were collected. The inclusion criteria were that the largest diameter of stone was ≤10 mm and the stone was not detected under X-ray. The treatment plan was taking UDCA orally for 6 months at a dose of 10 mg·kg -1·d -1. The basic information of patients, the ultrasound examination results before treatment and 6 months after treatment, and scores of biliary abdominal pain and dyspepsia symptom were collected. Univariate and multivariate logistic regression were used to analyze the influencing factors of the efficacy in gallstrone dissolution by UDCA, and Wilcoxon signed rank test was used for statistical analysis. Results:A total of 215 patients were enrolled. The complete dissolution rate of gallstone was 19.5% (42/215) and partial dissolution rate was 50.7% (109/215), and the total effective rate was 70.2% (151/215). The complete dissolution rate of sandy stone was significantly higher than that of lumped stones (37.0%(17/46) vs. 14.8%(25/169); OR=3.377, 95% confidence interval (95% CI) 1.621 to 7.035, P=0.001). In lumped stones, the complete dissolution rate of the stones with diameter ≤5 mm was significantly higher than that of the stones with diameter >5 mm (37.5%(9/24) vs. 11.0%(16/145); OR=4.837, 95% CI 1.823 to 12.839, P=0.002). The complete dissolution rate of patients with higher body mass index ( OR=0.872, 95% CI 0.764 to 0.995, P=0.043) and longer disease course ( OR=0.942, 95% CI 0.912 to 0.973, P<0.001) was low. The results of multivariate logistic analysis indicated that long disease course of gallstone ( OR=0.940, 95% CI 0.908 to 0.974, P=0.001), rough gallbladder wall ( OR=0.438, 95% CI 0.200 to 0.962, P=0.040) and lumped stone ( OR=0.236, 95% CI 0.101 to 0.550, P=0.001) were independent risk factors of influencing the efficacy of stone dissolution by UDCA. As for lumped stones, the independent risk factors included long disease course of gallstone ( OR=0.926, 95% CI 0.877 to 0.978, P=0.006) and stone diameter >5 mm ( OR=0.142, 95% CI 0.043 to 0.470, P=0.001). After 6 months of UDCA treatment, score of biliary abdominal pain decreased from 0 (0 to 6) to 0 (0 to 0) and the score of dyspepsia symptom decreased from 1 (0 to 2) to 0 (0 to 0), and the differences between before treatment and after treatment were statistically significant ( Z=-8.50, and -9.13, both P<0.001). Conclusions:UDCA has a certain efficacy in cholesterol gallstone dissolution and can ease biliary abdominal pain and dyspepsia symptom. Long disease course of gallstone, rough gallbladder wall and stone diameter >5 mm are independent risk factors of poor efficacy in gallstone dissolution by UDCA.
Clinical efficacy of laparoscopic cholecystectomy plus cholangioscopy for the treatment of cholecystolithiasis combined with choledocholithiasis

Abstract:Background: Currently, endoscopic retrograde cholangiopancreatography (ERCP) plus laparoscopic cholecystectomy (LC) is the main treatment for cholecystolithiasis combined with choledocholithiasis. However, the treatment is unsatisfactory, and the development of better therapies is needed. Aim: To determine the clinical efficacy of LC plus cholangioscopy for cholecystolithiasis combined with choledocholithiasis. Methods: Patients (n = 243) with cholecystolithiasis and choledocholithiasis admitted to The Affiliated Haixia Hospital of Huaqiao University (910th Hospital of Joint Logistic Support Force) between January 2019 and December 2023 were included in the study; 111 patients (control group) underwent ERCP + LC and 132 patients (observation group) underwent LC + laparoscopic common bile duct exploration (LCBDE). Surgical success rates, residual stone rates, complications (pancreatitis, hyperamylasemia, biliary tract infection, and bile leakage), surgical indicators [intraoperative blood loss (IBL) and operation time (OT)], recovery indices (postoperative exhaust/defecation time and hospital stay), and serum inflammatory markers [C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) were compared. Results: No significant differences in surgical success rates and residual stone rates were detected between the observation and control groups. However, the complication rate, IBL, OT, postoperative exhaust/defecation time, and hospital stays were significantly reduced in the observation group compared with the control group. Furthermore, CRP, TNF-α, and IL-6 Levels after treatment were reduced in the observation group compared with the levels in the control group. Conclusion: These results indicate that LC + LCBDE is safer than ERCP + LC for the treatment of cholecystolithiasis combined with choledocholithiasis. The surgical risks and postoperative complications were lower in the observation group compared with the control group. Thus, patients may recover quickly with less inflammation after LCBDE.

Abstract:Background: Ursodeoxycholic acid (UDCA), chenodeoxycholic acid (CDCA) plus UDCA (C&U), and terpene are widely administered to prevent common bile duct (CBD) stone recurrence and dissolve gallbladder (GB) stones. We evaluated and compared the combined effects of these agents on CBD stone recurrence and GB stone resolution. Methods: This study included patients who underwent endoscopic retrograde cholangiopancreatography (ERCP) at six referral centers, retrospectively. A total of 940 patients who underwent cholecystectomy before or after CBD stone removal by ERCP were evaluated to assess CBD stone recurrence (the CBD recurrence cohort), and 98 patients with GB stones were assessed by abdominal or endoscopic ultrasonography before and 6 months after ERCP to evaluate GB stone resolution (GB cohort). Patients were divided into no-medication, single-agent treatment (UDCA, C&U, or terpene), or dual-agent treatment (terpene plus UDCA or C&U) groups for the analysis. Results: In the CBD recurrence cohort, baseline characteristics were similar in the three groups. CBD stone recurrence rates were 41.5%, 12.7%, and 9.8% in the no-medication, single-agent, and dual-agent groups, respectively (p 30% were observed in 5.3%, 14.3%, and 34.8% of patients in the no-medication, single-agent, and dual-agent groups, respectively (p = 0.028). Conclusions: C&U plus terpene was significantly more effective for preventing CBD stone recurrence and achieving GB stone resolution than no medication or single agents.

Abstract:Introduction: Choledocholithiasis is usually treated with endoscopic sphincterotomy (EST), laparoscopic common bile duct exploration (CBDE) (LCBDE), or laparoscopic transcystic CBDE (LTCBDE). However, the comparative long-term prognosis of these methods in terms of adverse outcomes remains uncertain. Methods: In an ambispective, multicenter, observational, open cohort study, patients with cholecystocholedocholithiasis underwent one of three treatments (EST+laparoscopic cholecystectomy (LC), LCBDE+LC, and LTCBDE+LC).The primary outcome was adverse outcomes. Results: At a median follow-up period of 2.72 years among 2708 patients, the adverse outcome rate is 9.3%. Compared with LCBDE and LTCBDE groups, EST group displayed an increased risk for adverse outcomes (HR 1.25, 95% CI 0.93-1.69, P=0.146; HR 1.77, 95% CI 1.30-2.43, P<0.001) and stone-related outcomes (HR 1.43, 95% CI 0.99-2.06, P=0.054; HR 4.36, 95% CI 2.71-7.02, P<0.001) in the unadjusted Cox model. After adjustment in the multivariable analysis, EST group still displayed an increased risk of adverse outcomes (HR 1.43, 95% CI 1.05-1.95, P=0.021; HR 1.87, 95% CI 1.35-2.59, P<0.001) and stone-related outcomes (HR 1.64, 95% CI 1.13-2.37, P=0.010; HR 4.88, 95% CI 2.99-7.97, P<0.001). Compared with LTCBDE group, LCBDE group displayed an increased risk for a stone-related outcome in both the unadjusted Cox model (HR 3.05, 95% CI 1.89-4.90, P<0.001) and the adjusted multivariable analysis (HR 2.98, 95% CI 1.83-4.86, P <0.001). Conclusion: Compared with EST, the laparoscopic technique was associated with a lower risk of adverse outcomes and stone-related outcomes after adjustment for confounding factors. In laparoscopic techniques, the risk of stone-related outcomes after LTCBDE is reduced 66.4% than LCBDE. Clinical Trial Registration: CREST Choles ClinicalTrials.gov number, NCT02554097. Intestinal microbes affect bile acid metabolism involved in gallstone formation

Abstract:Abstract Background: The gut microbiota participates in the metabolism of substances and energy, promotes the development and maturation of the immune system, forms the mucosal barrier, and protects the host from pathogen attacks. Although the pathogenesis of cholesterol gallstones is still not clear, studies have suggested that gut microbiota dysbiosis plays an important role in their formation. Methods: Microbial DNA from faeces of normal control patients and those of patients with calculi was subjected to 16S rRNA gene sequencing to detect gene expression changes in intestinal microbes. ELISA kits were used to measure free bile acids, secondary bile acids and coprostanol according to the manufacturer’s instructions. The relationship between flora and their metabolites was then analysed. Results: In the gallstone group, the diversity of intestinal bacteria and the abundances of certain phylogroups were significantly decreased (p<0.05), especially Firmicutes (p<0.05), the largest phylum represented by the gut microbiota. This study found an increase in free bile acids (p<0.001) and secondary bile acids (p<0.01) in the enterohepatic circulation. Bile salt hydrolase activity was not related to the abundances of BSH-active bacteria. 7a-dehydroxylating gut bacteria were significantly increased (p<0.01), whereas cholesterol-lowering bacteria were significantly reduced (p<0.05). The Ruminococcus gnavus group could be used as a biomarker to distinguish the gallstone group from the control group. Conclusion: Substantial changes in the intestinal flora of patients with gallstones were observed, which affect cholesterol and bile acid metabolism and can lead to gallstones.
