Abstract:Post-infantile giant cell hepatitis (PIGCH), or syncytial giant cell hepatitis, is a rare but potentially devastating diagnosis in adults. Histology should show hepatocytes that develop into multinucleated giant cells with associated lobular disarray, inflammation, Kupffer cell hypertrophy and hepatocyte spotty necrosis. 1 Most common etiologies include autoimmune disorders, viral infections, medications, and malignancy. Here, we present a case of PIGCH in a patient with newly diagnosed systemic lupus erythematous (SLE).

Cutaneous Perivascular Hemophagocytosis: A Report of 2 Cases With Emphasis on Clinical and Histological Features

Abstract:Cutaneous perivascular hemophagocytosis (CH) is a histological manifestation that manifests as systemic hemophagocytic syndrome, also known as hemophagocytic lymphohistiocytosis, when accompanied by fever, hepatosplenomegaly, liver dysfunction, and cytopenia, and may rarely manifest independently of hemophagocytic lymphohistiocytosis. CH typically presents as purpuric or brownish macules and patches on the extremities, abdomen, and trunks. Histopathologically, the hallmark of CH includes extravasated erythrocytes and karyorrhectic debris phagocytized by histiocytes, associated with dermal capillary ectasia, perivascular infiltration of neutrophils, nuclear dust, and histiocytes without atypia. In this study, we report 2 cases of CH encountered in routine diagnostic practice and elucidate their significant clinical and histologic features. Our first patient had leukocytoclastic vasculitis with CH in the setting of Yersinia enterocolitis, and the second case represents CH in association with non-Hodgkin lymphoma. This study highlights the importance of considering CH as a potential indicator of underlying systemic pathology, including infectious and hematological disorders, in clinical practice.

Henoch-Schönlein purpura and membranoproliferative-like glomerulonephritis

Abstract:A 17-year-old man presented Henoch-Schönlein purpura with renal impairment, nephrotic syndrome needing transitory hemodialysis and hematuria. By light microscopy, the renal biopsy revealed membranoproliferative-like lesion associated with massive subendothelial deposits, some subepithelial deposits, hyalin thrombi and intracapillary neutrophils. By immunofluorescence, intense nodular and segmental deposits of IgG, IgM, IgA, C3, fibrinogen and C1q were found to be present in the intracapillary area and the mesangium. By electron microscopy, large subendothelial and rare subepithelial deposits were observed. A skin biopsy demonstrated leukocytoclastic skin vasculitis with IgA deposits in the arterial walls. Treatment with corticosteroids resulted in return renal to normal renal function but persistent proteinuria and hematuria. A second renal biopsy, performed after 2 months, showed a marked decrease in lesions and deposits. Fifteen months later, the patient presented normal serum creatinine level but proteinuria and hematuria persisted. At this time, a third renal biopsy was performed and showed segmental mesangial sclerosis and the decrease of disappearance of deposits. Two years after the first hospitalization, no abnormal serum creatinine or urinalysis were present. This report describes a detailed study of a case presented with Henoch-Schönlein purpura and morphologic features consistent with membranoproliferative-like lesion, who recovered normal renal function and urinalysis; repeat biopsies performed at intervals of 2 months and 1 year confirmed the disappearance of mesangial proliferation, double contours and deposits.
Bone turnover markers in serum and periodontal microenvironments.

Abstract:BACKGROUND Periodontitis is characterized by altered bone turnover, but local measurements are difficult. OBJECTIVES The objective of this study was to develop a method to measure multiple markers of bone turnover from single samples collected at various bone surfaces of the periodontium, and to test the ratios of these markers against more traditional serum and gingival crevicular fluid (GCF) samples. MATERIALS AND METHODS Fourteen subjects with untreated periodontitis were recruited for sampling serum, GCF (from sites > or = 5 mm probing depth that bled on probing) and washes of periodontal bone surfaces (adjacent interproximal, vestibular cortical and trabecular bone) with a novel irrigating device. All samples were analyzed for osteocalcin (OC, bone turnover marker; RIA), cross-linked N-telopeptide of type I collagen (NTx, bone resorption marker; ELISA) and albumin (Alb, serum protein; ELISA). Results were reported as ratios: OC/NTx to determine relative bone turnover, and OC/Alb or NTx/Alb to determine local OC or NTx production. RESULTS The OC/NTx ratio was significantly higher (p < or = 0.05) in serum vs. GCF (OC undetectable), interproximal bone and cortical vestibular bone, but significantly lower than in trabecular vestibular bone. The OC/Alb ratio for serum was also statistically lower than for vestibular trabecular bone. The NTx/Alb ratio for serum was statistically lower than for GCF and all the bone wash test sites. The results indicated considerable local production of both OC and NTx. CONCLUSIONS This system demonstrated that multiple markers of bone turnover can be harvested by irrigation from periodontal bone microenvironments. Bone turnover profiles from periodontal bone surfaces and GCF differed from systemic bone turnover profiles (serum) and may be valuable in tracking site-specific responses to disease or treatment.
Classic paroxysmal nocturnal haemoglobinuria presenting with intestinal malabsorption syndrome, acute abdomen and acute kidney injury

Abstract:A male patient in his 30s presented with complaints of acute abdominal pain, black stools and red-coloured urine. CT revealed thrombi in the splenic and left renal veins, leading to infarctions. An endoscopy displayed scalloping of the duodenal folds, indicative of intestinal malabsorption syndrome (IMS). Histopathological examination confirmed IMS. Due to the presence of intravascular haemolysis, haemoglobinuria and thrombotic complications, paroxysmal nocturnal haemoglobinuria (PNH) was suspected and subsequently confirmed by flow cytometry. Thus, a diagnosis of classic PNH with IMS and thrombotic complications was established. This unique case highlights the coexistence of PNH and IMS, resembling the complement hyperactivation, angiopathic thrombosis and protein-losing enteropathy disease, suggesting potential shared pathophysiology.
[Hypophysitis and retroperitoneal fibrosis associated with autoimmune polyglandular syndrome and IgG4-related disease. Report of one case]

Abstract:We report a 23 year old woman presenting with a nephrotic syndrome due to minimal change disease, central diabetes insipidus, primary hypothyroidism, vitiligo and universal alopecia. Eleven years later, she presented secondary amenorrhea due to hypogonadotropic hypogonadism, with mild hyperprolactinemia and central adrenal insufficiency. A magnetic resonance imaging of the sella turcica showed a pituitary mass with suprasellar extension that was resected using a transsphenoidal approach. Pathology confirmed the presence of a lymphoplasmacytic hypophysitis. She needed a second surgical resection due to mass growth and neuro-ophthalmologic impairment. One year later, systemic lupus erythematosus, arterial hypertension and type 2 diabetes mellitus were diagnosed. Two years later, due to back pain, constipation and renal failure, retroperitoneal fibrosis was found, satisfactorily treated with glucocorticoids and colchicine. Hence, this clinical vignette shows the coexistence of autoimmune polyglandular syndrome with retroperitoneal fibrosis and lymphoplasmacytic hypophysitis. Tissue analysis showed the presence of IgG4 producing plasma cells in the pituitary and retroperitoneum, which constitute a basis for the diagnosis of IgG4 related disease.
Apparent hemolysis following intravenous antithymocyte globulin treatment in a patient with marrow failure and a paroxysmal nocturnal hemoglobinuria clone

Abstract:Background: Antithymocyte globulin (ATG) is a commonly used medication in the treatment of aplastic anemia. Although serum sickness has been described with the use of ATG, few cases of acute intravascular hemolysis have been reported. We report a case of apparent ATG-related hemolysis in a patient with aplastic anemia and a paroxysmal nocturnal hemoglobinuria (PNH) clone. Case report: A 62-year-old, group A, RhoD+ man with aplastic anemia and an 11.6 percent glycosylphosphatidylinositol (GPI)-anchored protein-negative population of red cells (RBCs), representing approximately 190 mL of his RBC volume, and 90 percent GPI-negative neutrophils were scheduled to receive equine ATG at 40 mg per kg per day for 4 days. After the first infusion, he developed a 1.6 g per dL decline in hemoglobin concentration and an increase in serum lactate dehydrogenase (normal, 113-226 U/L) from 284 to 1127 U per L. The hemolytic process was complicated by acute renal failure characterized by an increase in serum creatinine from 0.9 to 4.2 mg per dL and the appearance of dark-colored urine. Pre- and post-ATG direct antiglobulin tests were negative. Conclusion: The temporal association of intravenous ATG to lysis of complement-sensitive RBCs suggests a causal relationship. Although intravascular hemolysis after ATG administration appears to be uncommon, the clinical consequences may be severe, and determining the pathophysiology may yield clues to the mechanism of intravascular hemolysis.
Waldenström's Macroglobulinemia and Cryoglobulinemic Glomerulonephritis: An Unusual Case of Monoclonal Gammopathy of Renal Significance

Abstract:Cryoglobulins are immunoglobulins that precipitate at temperatures below 37 °C and dissolve upon reheating. They can induce small-vessel vasculitis with renal involvement. Cryoglobulinemic glomerulonephritis is a rare manifestation that occurs in patients with monoclonal gammopathy, specifically Waldenström's macroglobulinemia. We present the case of a 52-year-old patient with a history of cutaneous vasculitis and hypothyroidism, who presented with generalized edema, moderate anemia, hypercholesterolemia, nephrotic range proteinuria of 12.69 g/day, microhematuria, arterial hypertension, and hypocomplementemia via the classical pathway, without acute kidney injury and with negative serological studies and positive cryoglobulins in the second determination. Serum and urine protein electrophoresis and immunofixation studies showed a monoclonal band of IgM and kappa light chain. Renal biopsy was consistent with cryoglobulinemic glomerulonephritis. In the context of dysproteinemia and cryoglobulinemic glomerulonephritis, bone-marrow aspiration and biopsy were performed, leading to the diagnosis of Waldenström's macroglobulinemia. Monoclonal gammopathies have been described in association with type I cryoglobulinemias. This described association is uncommon, which is why we present this case, along with a review of the literature.

Abstract:Optimizing fabrication parameters for rapid solidification of Re-Fe-B (Re = Rare earth) alloys can lead to nanocrystalline products with hard magnetic properties without any heat-treatment. In this work, we enhanced the magnetic properties of Ce17Fe78B6 ribbons by engineering both the microstructure and volume fraction of the Ce2Fe14B phase through optimization of the chamber pressure and the wheel speed necessary for quenching the liquid. We explored the relationship between these two parameters (chamber pressure and wheel speed), and proposed an approach to identifying the experimental conditions most likely to yield homogenous microstructure and reproducible magnetic properties. Optimized experimental conditions resulted in a microstructure with homogeneously dispersed Ce2Fe14B and CeFe2 nanocrystals. The best magnetic properties were obtained at a chamber pressure of 0.05 MPa and a wheel speed of 15 m·s−1. Without the conventional heat-treatment that is usually required, key magnetic properties were maximized by optimization processing parameters in rapid solidification of magnetic materials in a cost-effective manner.
A Case of Pyoderma Gangrenosum with A Predominant Infiltration of Plasma Cells

Abstract:A 55 old male patient was admitted to our hospital because of bilateral lower limb lesions. The lesions had lasted for over 10 years and aggravated for half a year which were manifested as scattered ulcers on both legs with clear demarcations and purple-red borders. Histopathology showed pauci-infiltration of lymphocytes around the vessels in the whole depth of dermis and a predominant infiltration of plasma cells. A combination of intravenous cyclophosphamide and oral DDX showed a better effect.
[Evaluation of quality assurance program in high dose rate remote after-loading brachytherapy].

Abstract:In the past 9 years, we have routinely used high-dose-rate remote afterloading systems, a Toshiba RAL-302-2 remote afterloading system and a self-fabricated Ir-192 system, for radiation therapy of various cancers, particularly cancers of the uterine cervix and nasopharynx. The frequency of using these two systems is more than 30 times per week. These machines can deliver high-dose radiation (300-1000 cGy) to a specifically planned target on the patient in a few minutes, therefore, the performance of the systems requires well planned dosimetry and quality control assurance in order to achieve effective therapeutic results and ensure safety of the patient under treatment. Any malfunction of the machines may cause over-radiation that exceeds a planned dose to the patient, and any distortion in the distribution of a planned dose may give an unexpected high dose of radiation to the therapist operating the machines. Hence, a quality assurance program must be enforced regularly to avoid or minimize any possible accidents. Based on information and recommendations reported in the literature, as well as from our own experience, we set up a program of routine checks to correct and adjust the machinery conditions and to ensure the optimum operational status of the equipment on a weekly, monthly, semiannual and annual basis. In checking the Co-60 machine, we found that the rate of occurrence for abnormal source position was 3.9% and that for the timer was 2.8%. In the Ir-192 machine checks, abnormal conditions were found only in the source position at the rate of 7.8%.

Abstract:Many different materials and treatment options are available in esthetic and restorative dentistry. Various newer products, such as pressed ceramics, offer enhanced functionality; however, in thinner dimensions, they lack the inherent esthetic beauty of traditional materials such as feldspathic porcelain. As patient demands for better esthetics have increased in recent years, so too has the need for restorative materials that closely mimic the patient's natural dentition. Initially used for the creation of porcelain dentures, feldspathic porcelain has emerged as the premier esthetic material for custom veneer restorations. In recent years, the use of hand-layered powder/liquid feldspathic porcelain has been revived based on its highly esthetic values and little-to-no preparation requirements. By keeping preparation to a minimum, less tooth structure is lost and procedures are much less invasive, which is exactly what patients desire.

Abstract:PurposeThe United States Immunodeficiency Network (USIDNET) patient registry was used to characterize the presentation, genetics, phenotypes, and treatment of patients with Hyper IgM Syndrome (HIGM).MethodsThe USIDNET Registry was queried for HIGM patient data collected from October 1992 to July 2015. Data fields included demographics, criteria for diagnosis, pedigree analysis, mutations, clinical features, treatment and transplant records, laboratory findings, and mortality.ResultsFifty-two physicians entered data from 145 patients of ages 2 months to 62 years (median 12 years); 131 were males. Using patients’ age at last entry, data from 2072 patient years are included. Mutations were recorded for 85 subjects; 82 were in CD40LG. Eighteen subjects had non-X-linked HIGM. 40 % had a normal serum IgM and 15 %, normal IgA. Infections were reported for 91 %, with pulmonary, ear, and sinus infections being the most common. 42 % had Pneumocystis jirovecii pneumonia; 6 % had Cryptosporidium. 41 % had neutropenia. 78 % experienced non-infectious complications: chronic diarrhea (n = 22), aphthous ulcers (n = 28), and neoplasms (n = 8) including colon cancer, adrenal adenoma, liver adenocarcinoma, pancreatic carcinoid, acute myeloid leukemia, hepatoma, and, in a female with an autosomal dominant gain of function mutation in PIK3CD, an ovarian dysgerminoma. Thirteen patients had a hematopoietic marrow or stem cell transplant; three had solid organ transplants. Thirteen were known to have died (median age = 14 years).ConclusionsAnalysis of the USIDNET Registry provides data on the common clinical features of this rare syndrome, and in contrast with previously published data, demonstrates longer survival times and reduced gastrointestinal manifestations.

Membranous-like glomerulopathy with masked monotypic IgG deposits: A single center case series

Abstract:Background: Membranous-like glomerulopathy with masked monoclonal IgG deposits (MGMID) is a newly recognized condition predominantly observed in young females, and its understanding in the pediatric population remains limited. Materials and methods: Four cases of MGMID are reported, including three pediatric patients. Results: All patients were female, with ages ranging from 12 to 26 years. None of the patients had malignancies. They presented with kidney dysfunction, proteinuria, or hematuria. Kidney biopsies of all cases exhibited a membranous pattern of injury with monoclonal IgG-κ restriction, "unmasked" by pronase digestion. Pediatric cases were treated conservatively, while the adult case underwent immunosuppressive treatment. All patients had favorable outcomes, and none reached end stage kidney disease (ESKD). Conclusion: MGMID can affect both adult and pediatric patients. Further studies are needed to fully characterize its risk factors, optimal therapy, and outcomes.
Paroxysmal Cold Hemoglobinuria: Mild to Catastrophic-Spectrum of a Rare Hemolytic Anemia of Childhood

Abstract:Paroxysmal cold hemoglobinuria (PCH) is among the rarest forms of autoimmune hemolytic anemia, most often seen in young children. PCH is caused by a biphasic immunoglobulin G antibody that binds to red cells at low temperatures and causes complement-mediated lysis as the temperature is raised. Diagnosis is based on high clinical suspicion followed by confirmation of the presence of Donath-Landsteiner antibodies. We have described 3 cases diagnosed with PCH over a span of 1 year, 2 cases presented with acute kidney injury with variable severity and needed hemodialysis. Another case showed prompt recovery with supportive treatment, suggesting variable severity of PCH. This report intends to generate awareness of this rare condition which is often misdiagnosed as nonspecific autoimmune hemolytic anemia and leads to unnecessary prolonged immunosuppressive therapy. It also emphasizes the rare possibility of the need for prompt renal replacement therapy in an otherwise benign self-limiting disorder.

Abstract:Hypereosinophilic syndrome (HES) is a rare haematologic disorder characterised by unexplained, persistent eosinophilia with organ involvement. We report the case of a 74-year-old patient who presented with a pemphigoid-like bullous rash as the only manifestation of an HES. This report highlights the importance of the dermatological manifestations of this potentially life-threating condition that may initially present as a skin-limited disease. Bullous pemphigoid (BP) is a very common autoimmune blistering dermatosis that may share common clinical and histological features or associate with an HES. HES should be considered when patients diagnosed with BP demonstrate atypical course under corticosteroids and/or recalcitrant blood eosinophilia.

Abstract:Abstract Urgent dermatology review was sought for a neonate born at term via emergency caesarean section for reduced movements. The pregnancy was complicated by gestational diabetes and infection with suspected coxsackie virus in the second trimester. There were 15 purple/blue nodules consistent with “blueberry muffin syndrome” and associated anaemia (118 g/L). Due to hypoxia, she was given antibiotics for suspected sepsis. TORCH screen was negative and USS abdomen and cranium were normal. A blood film showed B cell aplasia and LDH was raised (710 unit/L). CXR showed a mediastinal mass, left pleural effusion and left lung opacity. A skin biopsy was taken, chest drain placed and the baby transferred to Great Ormond Street. She was subsequently diagnosed with acute myeloid leukaemia following bone marrow and lumbar puncture. All three skin biopsies showed a malignant infiltrate with one displaying positivity for CD12 and CD33. CT and MRI scans have shown widespread nodules in the lungs, liver, kidneys, bones and intra-cranially. She is being treated with chemotherapy which has been complicated by a cardiac arrest and severe mucositis leading to colostomy. She is also NG fed and has sensorineural hearing loss. The nodules seen in “blueberry muffin syndrome” are most commonly extramedullary erythropoiesis but can also represent purpura or metastases. The causes are congenital infections, blood disorders or tumours. There is frequently associated anaemia (as in our case) and hepatosplenomegaly. This case demonstrates a rare and concerning cause of the syndrome and highlights the role dermatologists can play in diagnosis.

Abstract:A 25-year-old male patient presented erythema with desquamation on the scalp for 1 year.He had a 1-month history of pruritic skin,lesion on trunk and limbs.Skin examination showed widespread annular lesions with polycyclic arrangement on the trunk and limbs with development of peripheral vesicular lesions.Nikosky′s sigh was negative.Skin biopsy showed intraepidermal vesicules containing eosinophils,no acantholytic cells,papillary oedema and dermal inflammatory infiltrate containing eosinophils,lymphocytes and histiocytes.Directimmunofluorescence staining showed the cell surface C3 deposits throughout the epidermis.

Abstract:The samples were collected from 168 unrelated Turkish individuals randomly selected from criminal cases. The DNA was extracted from fresh blood leucocytes and blood stains following the published methods (1,2). For each PCR mix 2-25 ng of DNA was added to the PCR mix. All the CTTV loci were amplified in one reaction tube. Amplification of multiplex loci was done with the help of GenePrint STR system CTTV kit in accordance of the manufacturer's (Promega Corp., Madison, WI) recommendations. Separations of the amplified alleles were performed by ABI Prism 310 Genetic Analyzer. The exact test (3) for the presence of HardyWeinberg equilibrium was carried out by the TFPGA (4) program. Abstract:An 8-year-old girl with prior respiratory syncytial virus infection presented to the emergency department with lethargy, weakness, pallor, and yellow discoloration of the eyes. Examination revealed pallor, jaundice, and splenomegaly. Laboratory investigations revealed hemoglobin 4.0 g/dL, white cell count 14 000/μL, neutrophils 11 200/μL, platelet count 418 000/μL, mean corpuscular volume 103 fL, reticulocyte count 19.7%, total bilirubin 3.9 mg/dL, unconjugated bilirubin 3.7 mg/dL, lactate dehydrogenase 660 units/L, and haptoglobin <0.01 mg/dL. The fibrinogen was mildly increased at 4.4 g/dL. Direct Coombs test was positive for immunoglobulin G. The blood smear showed that 10% of red cells were doublet spherocytes (arrows; 40× lens objective). Strikingly, single spherocytes were absent. The diagnosis was postinfectious warm antibody autoimmune hemolytic anemia, which was treated with red cell transfusion and prednisolone, after which her anemia resolved, mean corpuscular volume normalized, and doublet spherocytes disappeared from the blood smear.
