Abstract:Abstract Microsatellite instability (MSI) is a hypermutator phenotype caused by DNA mismatch repair deficiency. MSI has been reported in various human cancers, particularly colorectal, gastric and endometrial cancers. MSI is a promising biomarker for cancer prognosis and immune checkpoint blockade immunotherapy. Several computational methods have been developed for MSI detection using DNA- or RNA-based approaches based on next-generation sequencing. Epigenetic mechanisms, such as DNA methylation, regulate gene expression and play critical roles in the development and progression of cancer. We here developed MSI-XGNN, a new computational framework for predicting MSI status using bulk RNA-sequencing and DNA methylation data. MSI-XGNN is an explainable deep learning model that combines a graph neural network (GNN) model to extract features from the gene-methylation probe network with a CatBoost model to classify MSI status. MSI-XGNN, which requires tumor-only samples, exhibited comparable performance with two well-known methods that require tumor-normal paired sequencing data, MSIsensor and MANTIS and better performance than several other tools. MSI-XGNN also showed good generalizability on independent validation datasets. MSI-XGNN identified six MSI markers consisting of four methylation probes (EPM2AIP1|MLH1:cg14598950, EPM2AIP1|MLH1:cg27331401, LNP1:cg05428436 and TSC22D2:cg15048832) and two genes (RPL22L1 and MSH4) constituting the optimal feature subset. All six markers were significantly associated with beneficial tumor microenvironment characteristics for immunotherapy, such as tumor mutation burden, neoantigens and immune checkpoint molecules such as programmed cell death-1 and cytotoxic T-lymphocyte antigen-4. Overall, our study provides a powerful and explainable deep learning model for predicting MSI status and identifying MSI markers that can potentially be used for clinical MSI evaluation.

A Novel and Reliable Method to Detect Microsatellite Instability in Colorectal Cancer by Next-Generation Sequencing.

Abstract:Two types of molecular tests have been established to assess the deficiency of the DNA mismatch repair (MMR) system: microsatellite instability (MSI) and immunohistochemical (IHC) analysis. We have developed a reliable method to analyze the MSI status by next-generation sequencing (NGS) based on read-count distribution. A total of 91 patients with primary colorectal cancer were recruited. These patients included 54 cases with loss of expression of any MMR protein in IHC, suggesting deficient MMR (dMMR), and 37 cases of colorectal cancer with staining of all four MMR proteins in IHC, suggesting proficient MMR in the sample after surgery. DNA was extracted from paired tumor-normal tissue for MSI detection by both the ColonCore NGS panel and PCR. The sequencing data from the NGS panel was processed using various MSI detection pipelines for a comparison with the ColonCore panel. Using the MSI-PCR test as the gold standard, MSI-ColonCore achieved 97.9% sensitivity (47 of 48) and 100% specificity (37 of 37) for the detection of MSI status. MSI-ColonCore also showed more efficient and robust performance compared with other NGS-based MSI detection algorithms. The concordance rate was 92.3% between MSI-ColonCore and IHC testing, and 93.4% between MSI-PCR and IHC testing. These results suggest that MSI-ColonCore is a reliable and robust method for MSI status detection by NGS based on read-count distribution.
scMSI: Accurately inferring the sub-clonal Micro-Satellite status by an integrated deconvolution model on length spectrum

Abstract:Microsatellite instability (MSI) is an important genomic biomarker for cancer diagnosis and treatment, and sequencing-based approaches are often applied to identify MSI because of its fastness and efficiency. These approaches, however, may fail to identify MSI on one or more sub-clones for certain cancers with a high degree of heterogeneity, leading to erroneous diagnoses and unsuitable treatments. Besides, the computational cost of identifying sub-clonal MSI can be exponentially increased when multiple sub-clones with different length distributions share MSI status. Herein, this paper proposes "scMSI", an accurate and efficient estimation of sub-clonal MSI to identify the microsatellite status. scMSI is an integrative Bayesian method to deconvolute the mixed-length distribution of sub-clones by a novel alternating iterative optimization procedure based on a subtle generative model. During the process of deconvolution, the optimized division of each sub-clone is attained by a heuristic algorithm, aligning with clone proportions that adhere optimally to the sample's clonal structure. To evaluate the performance, 16 patients diagnosed with endometrial cancer, exhibiting positive responses to the treatment despite having negative MSI status based on sequencing-based approaches, were considered. Excitingly, scMSI reported MSI on sub-clones successfully, and the findings matched the conclusions on immunohistochemistry. In addition, testing results on a series of experiments with simulation datasets concerning a variety of impact factors demonstrated the effectiveness and superiority of scMSI in detecting MSI on sub-clones over existing approaches. scMSI provides a new way of detecting MSI for cancers with a high degree of heterogeneity. Microsatellites are short, repetitive sequences of DNA, and their instability (MSI) is an important marker for cancer diagnosis and treatment. However, tumors often consist of diverse groups of cells, or sub-clones, and existing sequencing methods often fail to detect MSI that occurs only in some sub-clones. This can lead to incorrect diagnoses and prevent patients from receiving the most effective therapies. To solve this problem, we developed a new computational method named as scMSI to accurately identify MSI of sub-clones within a tumor. scMSI utilizes advanced statistical techniques to deconvolute the complex mixture of genetic mutations. As a result, we can use scMSI to detect sub-clonal MSI that other methods might miss. In the testing, we examined scMSI on samples from 16 patients with endometrial cancer, who had been incorrectly labeled as MSI-negative by existing methods. Our method successfully identified MSI in sub-clones, showing that scMSI outperforms existing tools. Additionally, simulation experiments under various conditions further confirmed the effectiveness of scMSI in detecting sub-clonal MSI. By improving the detection of MSI in cancers with a high degree of heterogeneity, scMSI can enhance cancer diagnosis and treatments more effectively.

Abstract:DNA mismatch repair deficiency (dMMR) testing is crucial for detection of microsatellite unstable (MSI) tumors. MSI is detected by aberrant indel length distributions of microsatellite markers, either by visual inspection of PCR-fragment length profiles or by automated bioinformatic scoring on next-generation sequencing (NGS) data. The former is time-consuming and low-throughput while the latter typically relies on simplified binary scoring of a single parameter of the indel distribution. The purpose of this study was to use machine learning to process the full complexity of indel distributions and integrate it into a robust script for screening of dMMR on small gene panel-based NGS data of clinical tumor samples without paired normal tissue.

Abstract:Background: Microsatellite instability (MSI) is a prognostic biomarker used to guide medication selection in multiple cancers, such as colorectal cancer. Traditional PCR with capillary electrophoresis and next-generation sequencing using paired tumor tissue and leukocyte samples are the main approaches for MSI detection due to their high sensitivity and specificity. Currently, patient tissue samples are obtained through puncture or surgery, which causes injury and risk of concurrent disease, further illustrating the need for MSI detection by liquid biopsy. Methods: We propose an analytic method using paired plasma/leukocyte samples and MSI detection using next-generation sequencing technology. Based on the theoretical progress of oncogenesis, we hypothesized that the microsatellite site length in plasma equals the combination of the distribution of tumor tissue and leukocytes. Thus, we defined a window-judgement method to identify whether biomarkers were stable. Results: Compared to traditional PCR as the standard, we evaluated three methods in 20 samples (MSI-H:3/MSS:17): peak shifting method using tissue vs. leukocytes, peak shifting method using plasma vs. leukocytes, and our method using plasma vs. leukocytes. Compared to traditional PCR, we observed a sensitivity of 100%, 0%, and 100%, and a specificity of 100.00%, 94.12%, and 88.24%, respectively. Conclusion: Our method has the advantage of possibly detecting MSI in a liquid biopsy and provides a novel direction for future studies to increase the specificity of the method.
Evaluating the Microsatellite Instability of Colorectal Cancer Based on Multimodal Deep Learning Integrating Histopathological and Molecular Data

Abstract:Microsatellite instability (MSI), an important biomarker for immunotherapy and the diagnosis of Lynch syndrome, refers to the change of microsatellite (MS) sequence length caused by insertion or deletion during DNA replication. However, traditional wet-lab experiment-based MSI detection is time-consuming and relies on experimental conditions. In addition, a comprehensive study on the associations between MSI status and various molecules like mRNA and miRNA has not been performed. In this study, we first studied the association between MSI status and several molecules including mRNA, miRNA, lncRNA, DNA methylation, and copy number variation (CNV) using colorectal cancer data from The Cancer Genome Atlas (TCGA). Then, we developed a novel deep learning framework to predict MSI status based solely on hematoxylin and eosin (H&E) staining images, and combined the H&E image with the above-mentioned molecules by multimodal compact bilinear pooling. Our results showed that there were significant differences in mRNA, miRNA, and lncRNA between the high microsatellite instability (MSI-H) patient group and the low microsatellite instability or microsatellite stability (MSI-L/MSS) patient group. By using the H&E image alone, one can predict MSI status with an acceptable prediction area under the curve (AUC) of 0.809 in 5-fold cross-validation. The fusion models integrating H&E image with a single type of molecule have higher prediction accuracies than that using H&E image alone, with the highest AUC of 0.952 achieved when combining H&E image with DNA methylation data. However, prediction accuracy will decrease when combining H&E image with all types of molecular data. In conclusion, combining H&E image with deep learning can predict the MSI status of colorectal cancer, the accuracy of which can further be improved by integrating appropriate molecular data. This study may have clinical significance in practice.

Abstract:The prediction of microsatellite instability (MSI) and microsatellite stability (MSS) is essential in predicting both the treatment response and prognosis of gastrointestinal cancer. In clinical practice, a universal MSI testing is recommended, but the accessibility of such a test is limited. Thus, a more cost-efficient and broadly accessible tool is desired to cover the traditionally untested patients. In the past few years, deep-learning-based algorithms have been proposed to predict MSI directly from haematoxylin and eosin (H&E)-stained whole-slide images (WSIs). Such algorithms can be summarized as (1) patch-level MSI/MSS prediction, and (2) patient-level aggregation. Compared with the advanced deep learning approaches that have been employed for the first stage, only the naïve first-order statistics (e.g., averaging and counting) were employed in the second stage. In this paper, we propose a simple yet broadly generalizable patient-level MSI aggregation (MAg) method to effectively integrate the precious patch-level information. Briefly, the entire probabilistic distribution in the first stage is modeled as histogram-based features to be fused as the final outcome with machine learning (e.g., SVM). The proposed MAg method can be easily used in a plug-and-play manner, which has been evaluated upon five broadly used deep neural networks: ResNet, MobileNetV2, EfficientNet, Dpn and ResNext. From the results, the proposed MAg method consistently improves the accuracy of patient-level aggregation for two publicly available datasets. It is our hope that the proposed method could potentially leverage the low-cost H&E based MSI detection method. Abstract:Abstract Motivation: Microsatellite instability (MSI) is a promising biomarker for cancer prognosis and chemosensitivity. Techniques are rapidly evolving for the detection of MSI from tumor-normal paired or tumor-only sequencing data. However, tumor tissues are often insufficient, unavailable, or otherwise difficult to procure. Increasing clinical evidence indicates the enormous potential of plasma circulating cell-free DNA (cfNDA) technology as a noninvasive MSI detection approach. Results: We developed MSIsensor-ct, a bioinformatics tool based on a machine learning protocol, dedicated to detecting MSI status using cfDNA sequencing data with a potential stable MSIscore threshold of 20%. Evaluation of MSIsensor-ct on independent testing datasets with various levels of circulating tumor DNA (ctDNA) and sequencing depth showed 100% accuracy within the limit of detection (LOD) of 0.05% ctDNA content. MSIsensor-ct requires only BAM files as input, rendering it user-friendly and readily integrated into next generation sequencing (NGS) analysis pipelines. Abstract:Microsatellite instability (MSI) is associated with several tumor types and its status has become increasingly vital in guiding patient treatment decisions. However, in clinical practice, distinguishing MSI from its counterpart is challenging since the diagnosis of MSI requires additional genetic or immunohistochemical tests. In this study, interpretable pathological image analysis strategies are established to help medical experts to automatically identify MSI. The strategies only require ubiquitous Haematoxylin and eosin-stained whole-slide images and can achieve decent performance in the three cohorts collected from The Cancer Genome Atlas. The strategies provide interpretability in two aspects. On the one hand, the image-level interpretability is achieved by generating localization heat maps of important regions based on the deep learning network; on the other hand, the feature-level interpretability is attained through feature importance and pathological feature interaction analysis. More interestingly, both from the image-level and feature-level interpretability, color features and texture characteristics are shown to contribute the most to the MSI predictions. Therefore, the classification models under the proposed strategies can not only serve as an efficient tool for predicting the MSI status of patients, but also provide more insights to pathologists with clinical understanding.

Abstract:Headings aims: To establish a microsatellite instability (MSI) predictive model in pan-cancer and compare the multi-omics characterization of MSI-related molecular features. Materials and methods: We established a 15-gene signature for predicting MSI status and performed a systematic assessment of MSI-related molecular features including gene and miRNA expression, DNA methylation, and somatic mutation, in approximately 10,000 patients across 30 cancer types from The Cancer Genome Atlas, Gene Expression Omnibus database, and our institution. Then we identified common MSI-associated dysregulated molecular features across six cancers and explored their mutual interfering relationships and the drug sensitivity. Key findings: we demonstrated the model's high prediction performance and found the samples with high-MSI were mainly distributed in six cancers: BRCA, COAD, LUAD, LIHC, STAD, and UCEC. We found RPL22L1 was up-regulated in the high-MSI group of 5/6 cancer types. CYP27A1 and RAI2 were down-regulated in 4/6 cancer types. More than 20 miRNAs and 39 DMGs were found up-regulated in MSI-H at least three cancers. We discovered some drugs, including OSI-027 and AZD8055 had a higher sensitivity in the high MSI-score group. Functional enrichment analysis revealed the correlation between MSI score and APM score, HLA score, or glycolysis score. The complicated regulatory mechanism of tumor MSI status in multiple dimensions was explored by an integrated analysis of the correlations among MSI-related genes, miRNAs, methylation, and drug response data. Significance: Our pan-cancer study provides a valuable predictive model and a comprehensive atlas of tumor MSI, which may guide more precise and personalized therapeutic strategies for tumor patients.

Abstract:Abstract Purpose: Microsatellite instability (MSI) was the FDA-approved companion diagnostic biomarker for using pembrolizumab in solid tumors and the biomarker for Lynch Syndrome. Next-generation sequencing (NGS) based method was recommended by NCCN and ESMO for detecting MSI status due to rich mutation information it could provide. However, current NGS based methods could be discordant on some cases, triggering confusion in clinical practice. To address this concern, we developed a novel and highly accurate MSI detection method, 3D-MSI, with a set of well-selected loci and compared its performance with other 3 public methods (MANTIS, MSISensor2, mSINGS) Materials and Methods: 100 MSI loci were selected from 2539 candidates and then sequenced on 145 FFPE samples as the training dataset. A binomial distribution model was built on each loci to predict PCR-validated MSI statuses of all samples in the training dataset and then all models were ordered by their prediction accuracy. 3D-MSI method determined MSI status by the percentage of top 30 loci models which predict the sample as MSI-H (>40%: MSI-H; otherwise MSS). Performances of 3D-MSI and other 3 methods were compared on 485 MSI-validated sample. Their discordance were further investigated on a large cohort of 10399 Chinese cancer patients. Detection limits of these 4 methods were determined by 4 serial-diluted tumor samples. We also observed concordance between 3D-MSI detected statuses and immunotherapy response on 27 advanced colorectal cancer (CRC) patients. Results: In the dataset of 485 samples, 3D-MSI had the highest accuracy (All: 96.7%, Colorectal cancer CRC: 98.9%, Gastric cancer GC: 97.0%, other cancer: 86.4%), sensitivity (All: 93.2%, CRC: 97.0%, GC: 94.4%, Other: 80.8%) and specificity (All: 98.2%, CRC: 99.5%, GC: 98.8%, Other: 90.0%) among all 4 methods. It could detect correct MSI status under low tumor content down to 5%, but other methods required much higher tumor content to work (MSISensor2: ≥15%, mSINGS: ≥20%, MANTIS: ≥40%). Study on 10399 Chinese cancer patients confirmed superiority of 3D-MSI: compared with other 3 methods, it yielded the fewest discordant and unreliable MSI statuses questioned by PCR (3D-MSI: 1, MSISensor2: 36, mSINGS: 36, MANTIS: 50). Loci selection was the key point for 3D-MSI to outrival other 3 methods, revealed by comparing their scoring distribution. 3D-MSI detection predicted significantly prolonged PFS (log-rank P<0.0001) and OS (log-rank P<0.0001) of the MSI-H group in 27 CRC patients who received immunotherapy, proving its validity in clinical practice. Conclusions: Our study offered an improved and clinically validated method for NGS-based MSI detection and stressed importance of loci selection in MSI detection, which would have crucial implications for future research and clinical management. Radiogenomics for Predicting Microsatellite Instability Status and PD-L1 Expression with Machine Learning in Endometrial Cancers: A Multicenter Study

Abstract:Purpose: To evaluate the effectiveness of machine learning model based on magnetic resonance imaging (MRI) in identifying microsatellite instability (MSI) status and PD-L1 expression in endometrial cancer (EC). Methods: This retrospective study included 82 EC patients from 2 independent centers. Radiomics features from the intratumoral and peritumoral regions, obtained from four conventional MRI sequences (T2-weighted images; contrast-enhanced T1-weighted images; diffusion-weighted images; apparent diffusion coefficient), were combined with clinicopathologic characteristics to develop machine learning model for predicting MSI status and PD-L1 expression. 60 patients from center 1 were used as the training set for model construction, while 22 patients from center 2 were used as an external validation set for model evaluation. Results: For predicting MSI status, the clinicopathologic model, radscore model, and combination model achieved area under the curves (AUCs) of 0.728, 0.833, and 0.889 in the training set, respectively, and 0.595, 0.790, and 0.848 in the validation set, respectively. For predicting PD-L1 expression, the clinicopathologic model, radscore model, and combination model achieved AUCs of 0.648, 0.814, and 0.834 in the training set, respectively, and 0.660, 0.708, and 0.764 in the validation set, respectively. Calibration curve analysis and decision curve analysis demonstrated good calibration and clinical utility of the combination model. Conclusion: The machine learning model incorporating MRI-based radiomics features and clinicopathologic characteristics could be a potential tool for predicting MSI status and PD-L1 expression in EC. This approach may contribute to precision medicine for EC patients.

Abstract:The molecular‐based detection of microsatellite instability (MSI) in endometrial cancer is complex, due to the low sensitivity of PCR and a lack of standardization in next‐generation sequencing (NGS) methods. In the present study, sequenced data were obtained from an NGS panel following the addition of five commonly used microsatellite loci. Subsequently, a novel algorithm, namely MSIPeak, was developed for data analysis. Results of the present study demonstrated that MSI data obtained using MSIPeak were presented in a peak, using a threshold of 1.10 to distinguish stable and unstable loci. MSIPeak was further validated using synthetic DNA samples and endometrial cancer tissue and the results were compared with the immunohistochemical analysis‐determined mismatch repair status. The PCR results demonstrated a 3‐base‐pair (bp) deletion in synthetic DNA samples, compared with 1‐ and 2‐bp deletion controls. Results obtained using MSIPeak demonstrated notable differences in peak profiles and positive scores in synthetic DNA samples with 1‐, 2‐ and 3‐bp deletions, compared with controls. Thus, the results of the present study demonstrated that NGS‐based MSI detection exhibited a higher sensitivity compared with PCR. In addition, NGS‐based MSI detection exhibited higher levels of repeatability and applicability compared with other MSI‐NGS‐based methods, such as MSISensor2 and MANTIS. Collectively, the results of the present study highlighted that the combination of MSIPeak and NGS exhibits potential in the detection of cancer.

Abstract:Gastric cancer can be classified into different subtypes according to their genetic expression. Microsatellite instability (MSI) is one of these subtypes and an important clinical marker for prognosis and consideration for immunotherapy. Since genetic testing is relatively expensive and laborious, this study tackles the challenge of using deep neural networks (DNNs) to identify MSI based on analyzing histomorphologic features of gastric whole-slide images (WSIs). A two-stage patch-wise framework was proposed, which first differentiates the tumor regions from normal, then predicts MSI status from the tumorous patches. The proposed deep learning architecture enhances the residual attention network with non-local modules and visual context fusion modules, thereby allowing both local fine-grained details and coarse long-range dependencies to be captured. Image post-processing procedures were also proposed to better align the region segmentation with pathologist annotations. The model was applied to a three-way classification task, namely normal tissue, microsatellite stable (MSS), and MSI, using a private dataset gathered by Chang Gung Memorial Hospital and achieved 91.95% slide-wise accuracy. We also studied the feasibility of transfer learning by fine tuning on the TCGA-STAD public dataset, where we attained a high accuracy of 96.53% and an AUC of 0.99, outperforming previous literature.

Abstract:Motivation: Microsatellite instability (MSI) is a cancer biomarker associated with cancer prognosis and chemotherapy sensitivity. Since the discovery of MSI, polymerase chain reaction (PCR)-based testing has been considered the gold standard for MSI detection. However, with the decrease in sequencing costs, software that calculates MSI based on next-generation sequencing (NGS) data has been widely applied. Results: In this study, we evaluated the performance of the MSIsensor detection software, focusing on the limitations of the chi-square test algorithm in determining microsatellite stability under high-depth sequencing data. We demonstrated that the chi-square test algorithm is insufficient for accurately as-sessing microsatellite stability in this context. Furthermore, we explored the application of downsampling techniques to enhance the accuracy of MSIsensor detection. Our findings provide insight into the limita-tions of current methods and offer potential improvements for more reliable MSI detection based on NGS data.

Abstract:Background Microsatellite instability (MSI) represents the first pan-cancer biomarker approved to guide immune checkpoint blockade (ICB) treatment. However its widespread testing, especially outside of gastrointestinal cancer, is hampered by tissue availability.Methods An algorithm for detecting MSI from peripheral blood was established and validated using clinical plasma samples. Its value for predicting ICB efficacy was evaluated among 60 patients with advanced gastrointestinal cancer. The landscape of MSI in blood was also explored among 5138 advanced solid tumors.Results The algorithm included 100 microsatellite markers with high capture efficiency, sensitivity, and specificity. In comparison with orthogonal tissue PCR results, the method displayed a sensitivity of 82.5% (33/40) and a specificity of 96.2% (201/209), for an overall accuracy of 94.0% (234/249). When the clinical validation cohort was dichotomized by pretreatment blood MSI (bMSI), bMSI-high (bMSI-H) predicted both improved progression-free survival and overall survival than the blood microsatellite stable (bMSS) patients (HRs: 0.431 and 0.489, p=0.005 and 0.034, respectively). Four patients with bMSS were identified to have high blood tumor mutational burden (bTMB-H) and trended towards a better survival than the bMSS-bTMB-low (bTMB-L) subset (HR 0.026, 95% CI 0 to 2.635, p=0.011). These four patients with bMSS-bTMB-H plus the bMSI-H group collectively displayed significantly improved survival over the bMSS-bTMB-L patients (HR 0.317, 95% CI 0.157 to 0.640, p<0.001). Pan-cancer prevalence of bMSI-H was largely consistent with that shown for tissue except for much lower rates in endometrial and gastrointestinal cancers, and a remarkably higher prevalence in prostate cancer relative to other cancer types.Conclusions We have developed a reliable and robust next generation sequencing-based bMSI detection strategy which, in combination with a panel enabling concurrent profiling of bTMB from a single blood draw, may better inform ICB treatment.
Development and Validation of a Deep Learning-Based Microsatellite Instability Predictor from Prostate Cancer Whole-Slide Images

Abstract:Microsatellite instability-high (MSI-H) is a tumor agnostic biomarker for immune checkpoint inhibitor therapy. However, MSI status is not routinely tested in prostate cancer, in part due to low prevalence and assay cost. As such, prediction of MSI status from hematoxylin and eosin (H&E) stained whole-slide images (WSIs) could identify prostate cancer patients most likely to benefit from confirmatory testing and becoming eligible for immunotherapy. Prostate biopsies and surgical resections from de-identified records of consecutive prostate cancer patients referred to our institution were analyzed. Their MSI status was determined by next generation sequencing. Patients before a cutoff date were split into an algorithm development set (n=4015, MSI-H 1.8%) and a paired validation set (n=173, MSI-H 19.7%) that consisted of two serial sections from each sample, one stained and scanned internally and the other at an external site. Patients after the cutoff date formed the temporal validation set (n=1350, MSI-H 2.3%). Attention-based multiple instance learning models were trained to predict MSI-H from H&E WSIs. The MSI-H predictor achieved area under the receiver operating characteristic curve values of 0.78 (95% CI [0.69-0.86]), 0.72 (95% CI [0.63-0.81]), and 0.72 (95% CI [0.62-0.82]) on the internally prepared, externally prepared, and temporal validation sets, respectively. While MSI-H status is significantly correlated with Gleason score, the model remained predictive within each Gleason score subgroup. In summary, we developed and validated an AI-based MSI-H diagnostic model on a large real-world cohort of routine H&E slides, which effectively generalized to externally stained and scanned samples and a temporally independent validation cohort. This algorithm has the potential to direct prostate cancer patients toward immunotherapy and to identify MSI-H cases secondary to Lynch syndrome.

Abstract:Determining tumor microsatellite status has significant clinical value because tumors that are microsatellite instability-high (MSI-H) or mismatch repair deficient (dMMR) respond well to immune checkpoint inhibitors (ICIs) and oftentimes not to chemotherapeutics. We propose MSI-SEER, a deep Gaussian process-based Bayesian model that analyzes H&E whole-slide images in weakly-supervised-learning to predict microsatellite status in gastric and colorectal cancers. We performed extensive validation using multiple large datasets comprised of patients from diverse racial backgrounds. MSI-SEER achieved state-of-the-art performance with MSI prediction, which was by integrating uncertainty prediction. We achieved high accuracy for predicting ICI responsiveness by combining tumor MSI status with stroma-to-tumor ratio. Finally, MSI-SEER's tile-level predictions revealed novel insights into the role of spatial distribution of MSI-H regions in the tumor microenvironment and ICI response.

Abstract:Abstract Microsatellite instability (MSI) is an indispensable biomarker in cancer immunotherapy. Currently, MSI scoring methods by high-throughput omics methods have gained popularity and demonstrated better performance than the gold standard method for MSI detection. However, the MSI detection method on expression data, especially single-cell expression data, is still lacking, limiting the scope of clinical application and prohibiting the investigation of MSI at a single-cell level. Herein, we developed MSIsensor-RNA, an accurate, robust, adaptable, and standalone software to detect MSI status based on expression values of MSI-associated genes. We demonstrated the favorable performance and promise of MSIsensor-RNA in both bulk and single-cell gene expression data in multiplatform technologies including RNA sequencing (RNA-seq), microarray, and single-cell RNA-seq. MSIsensor-RNA is a versatile, efficient, and robust method for MSI status detection from both bulk and single-cell gene expression data in clinical studies and applications.
