Abstract:Patients with inflammatory bowel disease (IBD) are at an increased risk of developing colitis-associated neoplasia (CAN), including colorectal cancer (CRC), through the inflammation-dysplasia-neoplasia pathway. Dysplasia is the most reliable, early and actionable marker for CAN in these patients. While such lesions are frequently encountered, adequate management depends on an accurate assessment, complete resection and close surveillance. With recent advances in endoscopic technologies and research in the field of CAN, the management of dysplastic lesions has significantly improved. The American Gastroenterology Association and Surveillance for Colorectal Endoscopic Neoplasia Detection (SCENIC) provide a guideline framework for approaching dysplastic lesions in patients with IBD. However, there are significant gaps in these recommendations and real-world clinical practice. Accurate lesion assessment remains pivotal for adequate management of CAN. Artificial intelligence-guided modalities are now increasingly being used to aid the detection of these lesions further. As the lesion detection technologies are improving, our armamentarium of resection techniques is also expanding and includes hot or cold polypectomy, endoscopic mucosal resection, endoscopic sub-mucosal dissection and full-thickness resection. With the broadened scope of endoscopic resection, the recommendations regarding surveillance after resection has also changed. Certain patient populations such as those with invisible dysplasia or with prior colectomy and ileal pouch anal anastomosis need special consideration. In the present review, we aim to provide a state-of-the-art summary of the current practice of endoscopic detection, resection and surveillance of dysplasia in patients with IBD and provide some perspective on the future directions based on the latest research.

Abstract:Inflammatory bowel disease (IBD)is an extremely common gastrointestinal disorder that can give rise to dysplasia and colorectal cancer (CRC). There are various diagnostic methods but endoscopy has proved to be the best in the diagnosis, monitoring, and treatment of IBD. The objective of this review is to evaluate the efficacy of endoscopy in detecting patients with IBD. A structured search strategy on PubMed, Science Direct, and Google Scholar was used, as well as formal inclusion or exclusion, data extraction, validity assessment, and meta-analysis. RevMan 5.4 (Review Manager (RevMan) (Computer program). Version 5.4. The Cochrane Collaboration, 2020) was used for the meta-analysis, and forest plots were generated for each outcome separately. All of these studies are prospective cohorts and 11 of these are randomized controlled trials (RCTs). In IBD, both chromoendoscopy and white light endoscopy are useful in detecting dysplasia and neoplastic lesions. Furthermore, narrow-band imaging is a less time-consuming option for endoscopic surveillance. The meta-analysis also showed that chromoendoscopy is superior to other methods.
Perception of Cancer Risk and Management Practice for Colitis-associated Dysplasia Is Influenced by Colonoscopy Experience and Workplace Affiliation: Results of an International Clinician Survey

Abstract:Background and aims: A successful colitis cancer surveillance programme requires effective action to be taken when dysplasia is detected. This is the first international cross-sectional study to evaluate clinician understanding of dysplasia-cancer risk and management practice since the most recent international guidelines were introduced in 2015. Methods: A 15-item international online survey was disseminated to gastroenterologists and colorectal surgeons. Results: A total of 294 clinicians [93.5% gastroenterologists] from 60 countries responded; 23% did not have access to high-definition chromoendoscopy. University hospitals were more likely than non-academic workplaces to provide second expert histopathologist review [67% vs 46%; p = 0.002] and formal multidisciplinary team meeting discussion [73% vs 52%; p = 0.001] of dysplasia cases. Perceptions of 5-year cancer risk associated with endoscopically unresectable low-grade dysplasia varied between 0% and 100%. Non-academic hospital affiliation was predictive of lower perceived cancer risks. Although most [98.4%] respondents advised a colectomy for endoscopically unresectable visible high-grade dysplasia, only 34.4% advised a colectomy for unresectable visible low-grade dysplasia. Respondents from university hospitals were more likely to consider colectomy for multifocal low-grade dysplasia (odds ratio [OR] 2.17). If invisible unifocal low-grade dysplasia was detected, continued surveillance over colectomy was the preferred management among clinicians working mainly in private clinics [OR 9.4] and least preferred in those who had performed more than 50 surveillance colonoscopies [OR 0.41]. Conclusions: Clinicians with less surveillance colonoscopy experience and from non-academic centres appear to have lower cancer risk perceptions and are less likely to advocate colectomy for higher-risk low-grade dysplasia. Further education may align current management practice with clinical guidelines.

Abstract:AIM Present study reports the development of divalent vaccine with enhanced protection, permeation and presentation following peroral immunization. MATERIALS & METHODS Layersomes were prepared by layer-by-layer tuning of polyelectrolytes on liposomes template. The developed system was evaluated for in vitro stability of antigen and layersomes, cell-based assays and immunization experiments in mice. RESULTS Layersomes exhibited enhanced stability in simulated biological fluids, still preserving the integrity, biological activity and conformational stability of toxoids. Layersomes also exhibited complete and protective (>0.1 IU/ml) immunostimulatory response include serum IgG titer, mucosal sIgA titer and cytokines (IL-2 and IFN-γ) levels following peroral administration. CONCLUSION The positive findings of proposed strategy are expected to contribute significantly in the field of stable liposomes technology and peroral immunization.

Abstract:Background and aims: Inflammatory bowel disease [IBD] is associated with high rates of post-colonoscopy colorectal cancer [PCCRC], but further in-depth qualitative analyses are required to determine whether they result from inadequate surveillance or aggressive IBD cancer evolution. Methods: All IBD patients who had a colorectal cancer [CRC] diagnosed between January 2015 and July 2019 and a recent [<4 years] surveillance colonoscopy at one of four English hospital trusts underwent root cause analyses as recommended by the World Endoscopy Organisation to identify plausible PCCRC causative factors. Results: In total, 61% [n = 22/36] of the included IBD CRCs were PCCRCs. They developed in patients with high cancer risk factors [77.8%; n = 28/36] requiring annual surveillance, yet 57.1% [n = 20/35] had inappropriately delayed surveillance. Most PCCRCs developed in situations where [i] an endoscopically unresectable lesion was detected [40.9%; n = 9/22], [ii] there was a deviation from the planned management pathway [40.9%; n = 9/22], such as service-, clinician- or patient-related delays in acting on a detected lesion, or [iii] lesions were potentially missed as they were typically located within areas of active inflammation or post-inflammatory change [36.4%; n = 8/22]. Conclusions: IBD PCCRC prevention will require more proactive strategies to reduce endoscopic inflammatory burden, and to improve lesion optical characterization, adherence to recommended surveillance intervals, and patient acceptance of prophylactic colectomy. However, the significant proportion appearing to originate from non-adenomatous-looking mucosa which fail to yield neoplasia on biopsy yet display aggressive cancer evolution highlights the limitations of current surveillance. Emerging molecular biomarkers may play a role in enhancing cancer risk stratification in future clinical practice.

Abstract:Inflammatory bowel disease-related colorectal cancer (IBD-CRC) is one of the most serious complications of IBD contributing to significant mortality in this cohort of patients. IBD is often associated with diet and lifestyle-related gut microbial dysbiosis, the interaction of genetic and environmental factors, leading to chronic gut inflammation. According to the "common ground hypothesis", microbial dysbiosis and intestinal barrier impairment are at the core of the chronic inflammatory process associated with IBD-CRC. Among the many underlying factors known to increase the risk of IBD-CRC, perhaps the most important factor is chronic persistent inflammation. The persistent inflammation in the colon results in increased proliferation of cells necessary for repair but this also increases the risk of dysplastic changes due to chromosomal and microsatellite instability. Multiple pathways have been identified, regulated by many positive and negative factors involved in the development of cancer, which in this case follows the 'inflammation-dysplasia-carcinoma' sequence. Strategies to lower this risk are extremely important to reduce morbidity and mortality due to IBD-CRC, among which colonoscopic surveillance is the most widely accepted and implemented modality, forming part of many national and international guidelines. However, the effectiveness of surveillance in IBD has been a topic of much debate in recent years for multiple reasons - cost-benefit to health systems, resource requirements, and also because of studies showing conflicting long-term data. Our review provides a comprehensive overview of past, present, and future perspectives of IBD-CRC. We explore and analyse evidence from studies over decades and current best practices followed globally. In the future directions section, we cover emerging novel endoscopic techniques and artificial intelligence that could play an important role in managing the risk of IBD-CRC.

Abstract:One of the most feared complications of inflammatory bowel disease [IBD]-associated colitis is colorectal cancer. When considering the substantial increase in the prevalence of IBD, without any anticipated decline, coupled with decreasing colectomy rates for dysplasia and expanding medical options for effectively controlling inflammation, it is predicted that the pool of people living with-and ageing with-colonic IBD, who are recommended to undergo lifelong colonoscopic surveillance for colorectal neoplasia, will strain existing resources and challenge the sustainability of current guideline-based surveillance recommendations. At the same time, we are missing the opportunity for early detection in a group that is overlooked as high-risk, as a substantial proportion of colorectal cancers are being diagnosed in individuals with colonic IBD who have disease duration shorter than when guidelines recommend surveillance initiation. Here, we reappraise: 1] inflammation as a dynamic risk factor that considers patients' cumulative course; 2] time of screening initiation that is not based primarily on absolute disease duration; and 3] surveillance intervals as an iterative determination based on individual patient factors and consecutive colonoscopic findings. This Viewpoint supports a paradigm shift that will ideally result in a more effective and higher-value colorectal cancer prevention approach in IBD.

Abstract:IntroductionLong-standing inflammation of the colorectum in ulcerative colitis (UC) and Crohn’s disease (CD) has been associated with an increased risk of subsequent dysplasia and colorectal cancer. Historically, it was described that the neoplastic transformation in these inflammatory bowel diseases (IBDs) occurred via a different biologic pathway and not by the non-IBD polyp-cancer pathway and predictable lag time of progression. Therefore, prevention strategies have focused on the detection of dysplasia in flat mucosa, and existing guidelines have recommended performance of interval surveillance colonoscopies with random biopsies to identify such lesions with proctocolectomy when they are confirmed.DiscussionThe use of a new technology higher-resolution colonoscopies has led to the appreciation more recently that dysplasia in IBD may be visible with standard optical colonoscopy and can be identified in an even more sensitive manner using chromoendoscopy. Furthermore, emerging evidence favors the intuitive understanding that neoplastic transformation in IBD is linked to the degree of inflammation and that disease control may therefore modify this risk and its subsequent prevention approaches.ConclusionFuture IBD cancer prevention strategies and timing of surgery in at-risk patients will require a better understanding of this evolving field.

Abstract:Abstract Background Patients with IBD have increased risk for developing colorectal cancer (CRC), with poorer survival, compared to those without IBD. Despite surveillance colonoscopy, IBD post-colonoscopy CRC (PCCRC) rates are unacceptably high at 28-45%. We define surveillance colonoscopy quality, IBD-dysplasia and IBD-CRC prevalence, IBD-CRC characteristics, and IBD PCCRC rates at a tertiary IBD centre. Methods The GI Reporting Tool (Unisoft Medical Systems) was searched for IBD surveillance colonoscopy reports from April 2008–December 2018. Electronic medical records were reviewed. IBD phenotype and diagnosis date was confirmed on the Lothian IBD Registry. Follow-up was until September 2022. Results 1892 colonoscopies from 1262 patients were included. Median follow-up was 7.1 (4.4, 9.5) years. 885/1262 (70.1%) had UC, 344/1262 (27.3%) had Crohn’s, and 33/1262 (2.6%) had IBD-U. Median disease duration at colonoscopy was 16.8 (11, 24.6) years. 1825/1892 (96.5%) colonoscopies achieved caecal intubation, 1735/1840 (94.3%) adhered to biopsy guidelines and 704/1892 (37%) used chromoendoscopy. Histological inflammation was identified in 873/1823 (47.9%) cases; 57.2% in the right colon. Bowel preparation was poor in 194/1892 (10.3%) procedures and the most documented reason for not performing chromoendoscopy. Poor bowel preparation was associated with 23/60 (38.3%) poorly tolerated and 29/52 (55.8%) incomplete colonoscopies. Moviprep was superior to Picolax (p<0.001). GI physicians were more likely to use chromoendoscopy (p<0.001), biopsy per guideline (p<0.001), and identify dysplasia (p=0.012) compared to surgical colleagues. Dysplasia was identified in 145/1892 (7.7%) colonoscopies: 84/1892 (4.4%) sporadic, 22/1892 (1.2%) IBD-associated, and 39/1892 (2.1%) unspecified. IBD-CRC is rare, at 0.8 cases per 1000 patient years. Within the study period, 20/1262 (1.6%) patients had CRC diagnosed. 17/20 (85%) had UC and 3/20 (15%) had Crohn’s. 15/17 (88%) UC patients had E3 disease. 11/20 (55%) presented with lymph node or distant metastasis. 8/20 (40%) tumours had mucinous differentiation; only 1/15 (6.7%) was well differentiated. IBD-CRC patients had longer disease duration (28.3 (19.7, 37.8) years) compared to patients without CRC (22.25 (16.42, 29.85) years) at follow-up (p=0.028). The 3-year PCCRC rate was 15%. Conclusion IBD-CRC is rare however IBD PCCRC rates are unacceptably high. While this may reflect disease biology, a new approach to IBD surveillance is urgently needed. Interventions could include dedicated IBD surveillance lists, patient education around bowel preparation, and pre-screening for inflammation (e.g. faecal calprotectin). Cancer registry database linkage is now essential for us to identify IBD-CRC cases not identified by surveillance.

Abstract:Ulcerative colitis (UC) is a risk factor for the development of inflammation-associated dysplasia or colitis-associated neoplasia (CAN). This transformation results from chronic inflammation, which induces changes in epithelial proliferation, survival, and migration via the induction of chemokines and cytokines. There are notable differences in genetic mutation profiles between CAN in UC patients and sporadic colorectal cancer in the general population. Colonoscopy is the cornerstone for surveillance and management of dysplasia in these patients. There are several modalities to augment the quality of endoscopy for the better detection of dysplastic or neoplastic lesions, including the use of high-definition white-light exam and image-enhanced colonoscopy, which are described in this review. Clinical practice guidelines regarding surveillance strategies in UC have been put forth by various GI societies, and overall, there is agreement between them except for some differences, which we highlight in this article. These guidelines recommend that endoscopically detected dysplasia, if feasible, should be resected endoscopically. Advanced newer techniques, such as endoscopic mucosal resection and endoscopic submucosal dissection, have been utilized in the treatment of CAN. Surgery has traditionally been the mainstay of treating such advanced lesions, and in cases where endoscopic resection is not feasible, a proctocolectomy, followed by ileal pouch-anal anastomosis, is generally recommended. In this review we summarize the approach to surveillance for cancer and dysplasia in UC. We also highlight management strategies if dysplasia is detected.

Abstract:Opinion statementPatients with ulcerative colitis (UC) and Crohn’s disease (CD) colitis are at significantly higher risk for the development of colitis-associated colorectal cancer (CAC), a risk associated with increased duration and extent of disease [1]. CAC in patients with ulcerative colitis and Crohn’s colitis arises from dysplastic tissue in the background of mucosal inflammatory changes. Regular surveillance by colonoscopy is a recommended cancer prevention strategy endorsed by the US and European GI societies [2–4]. Until recently, random sampling of the mucosa throughout the colon has been the mainstay of this conventional surveillance. This requires multiple biopsies to be taken and processed, a practice which is time consuming, expensive, and has a low diagnostic yield. The growth pattern of dysplastic tissue is often multifocal and diffuse, and thus, its detection may not be optimal with the use of traditional white light colonoscopy. According to recent studies, most dysplastic lesions appear to be visible to careful endoscopic inspection [5–7]. Thus, the approach focusing on targeted biopsies of any mucosal abnormalities instead of only random biopsies has been advocated [8]. In addition, the detection and further delineation of any mucosal abnormalities are thought to be improved by the application of dyes that highlight more subtle abnormalities known as chromoendoscopy (CE) as well as a new generation endoscopic system with high-definition white light endoscopy (HDWL). The application of CE in patients with long-term UC was deemed to be beneficial based upon the results of previous clinical trials comparing CE with standard definition white light endoscopy (SDWL) [9•, 10•]. However, there are limited data available comparing CE with the currently broadly used high-definition colonoscopies (HDWL) [11•, 12•, 13•]. High-definition (HD) endoscopy uses a high-definition monitor and a high-resolution CCD (charge-coupled device) providing images of substantially higher resolution than standard video endoscopy. Thus, HDWL colonoscopy may be an alternative to CE in IBD surveillance without the need for the extra time and required experience with CE. Further longitudinal studies are necessary to determine the ultimate advantage of chromoendoscopy or lack of the advantages of chromoendoscopy over that of high-definition colonoscopy in detection of dysplasia. Once lesions are identified by HDWL and CE, they can be further evaluated with evolving technologies to perform in vivo microscopy. Small-field technologies such as confocal endomicroscopy (CLE) permit in vivo microscopic assessment of the colonic mucosa. This evolving technology can be utilized in combination with HDWL and CE in selected cases to further define the lesions and assess their histology, and thus, facilitate real-time in vivo diagnosis and decisions regarding resection of lesions. Further studies to determine the applicability of these newer enhanced technologies in evaluation of dysplasia in routine clinical practice are needed.
What Is the Risk? Epidemiology and Evidence for Surveillance Regimens

Abstract:Patients with inflammatory bowel disease (IBD) have increased risk of colorectal cancer (CRC). The risk for CRC is positively correlated to the duration of disease, extent of colonic involvement, and severity of inflammation. After 8 to 10 years of IBD diagnosis, the risk for CRC rises substantially and screening colonoscopy is recommended. Surveillance colonoscopy interval ranges from 1 to 5 years depending on patient and disease-specific risk factors. IBD patients with high risk factors such as having concomitant primary sclerosing cholangitis, moderate-to-severe inflammation, first-degree relative with CRC at early age, or history of invisible dysplasia or high-risk visible dysplasia should undergo surveillance colonoscopy in 1 year. Meanwhile, those with minimal colonic involvement or ≥2 consecutive unremarkable examinations while in continuous remission may consider extending the surveillance interval to 5 years. Advance in colonoscopy technique such as chromoendoscopy using dyes and/or image digital processing (virtual chromoendoscopy) may enhance dysplasia detection and is the preferred method for IBD surveillance. In the era of high-definition colonoscope, the practice of obtaining extensive biopsies throughout the colon remains controversial but is generally recommended to improve the detection rate of invisible dysplasia. Endoscopic surveillance in IBD has been shown to result in earlier detection of CRC and improved prognosis.
Root-cause analyses of missed opportunities for the diagnosis of colorectal cancer in patients with inflammatory bowel disease

Abstract:Background: Colonoscopic surveillance in patients with inflammatory bowel disease (IBD) leads to earlier detection of colorectal cancer (CRC) and reduces CRC-associated mortality. However, it is limited by poor adherence in practice. Aim: To identify missed opportunities to detect IBD-associated CRC at our hospital METHODS: We undertook root-cause analyses to identify patients with missed opportunities to diagnose IBD-associated CRC. We matched patients with IBD-associated CRC to patients with CRC in the general population to identify differences in staging at diagnosis and clinical outcomes. Results: Compared with the general population, patients with IBD were at increased risk of developing CRC (odds ratio 2.7 [95% CI 1.6-3.9], P < 0.001). The mean incidence of IBD-associated CRC between 1998 and 2019 was 165.4 (IQR 130.4-199.4) per 100 000 patients and has not changed over the last 20 years. Seventy-eight patients had IBD-associated CRC. Forty-two (54%) patients were eligible for CRC surveillance: 12% (5/42) and 10% (4/42) patients were diagnosed with CRC at an appropriately timed or overdue surveillance colonoscopy, respectively. Interval cancers occurred in 14% (6/42) of patients; 64% (27/42) of patients had a missed opportunity for colonoscopic surveillance where root-cause analyses demonstrated that 10/27 (37%) patients known to secondary care had not been offered surveillance. Four (15%) patients had a delayed diagnosis of CRC due to failure to account for previous colonoscopic findings. Seventeen (63%) patients were managed by primary care including seven patients discharged from secondary care without a surveillance plan. Matched case-control analysis did not show significant differences in cancer staging or 10-year survival outcomes. Conclusion: The incidence of IBD-associated CRC has remained static. Two-thirds of patients eligible for colonoscopic surveillance had missed opportunities to diagnose CRC. Surveillance programmes without comprehensive and fully integrated recall systems across primary and secondary care are set to fail.

Abstract:Observational studies have suggested that inflammatory bowel disease (IBD) increases the risk of colonic dysplasia. Surveillance colonoscopies can reduce this risk through detection of dysplasia and intervention; however, further research is needed to clarify optimal guidance on interval screening. We therefore aimed to establish a natural history model of dysplasia in IBD assuming no IBD-related surveillance colonoscopies to gain insight into the natural disease course.

Colorectal cancer in inflammatory bowel disease: review of the evidence

Abstract:Inflammatory bowel disease (IBD)-related colorectal cancer (CRC) is responsible for approximately 2% of the annual mortality from CRC overall, but 10-15% of the annual deaths in IBD patients. IBD-related CRC patients are also affected at a younger age than sporadic CRC patients, and have a 5-year survival rate of 50%. Despite optimal medical treatment, the chronic inflammatory state inherent in IBD increases the risk for high-grade dysplasia and CRC, with additional input from genetic and environmental risk factors and the microbiome. Recognizing risk factors, implementing appropriate surveillance, and identifying high-risk patients are key to managing the CRC risk in IBD patients. Chemoprevention strategies exist, and studies evaluating their efficacy are underway. Once dysplasia or invasive cancer is diagnosed, appropriate surgical resection and postoperative treatment and surveillance are necessary. Here, we discuss the current state of IBD-related CRC, prevalence, risk factors, and evidence for surveillance, prophylaxis, and treatment recommendations.
Can we improve the endoscopic resection of dysplastic lesions in colonic inflammatory bowel diseases?

Abstract:The incidence of colonic inflammatory bowel disease (IBD) is increasing globally, with prevalence rates of approximately 1 in 200 adults in the highest-risk populations [1]. One of the consequences of longstanding colonic inflammation is the potential development of premalignant lesions and colorectal cancer (CRC). Although the risk of CRC in IBD is declining, the cumulative risk is estimated to be 1%, 2%, and 5% after 10, 20, and >20 years of disease, respectively [2]. "We should be optimistic that with improved assessment and resection techniques, high rates of en bloc, safe, and complete resection will be achieved in IBD, resulting in the very low recurrence rates seen in the non-IBD population." Therefore, patients with IBD are usually invited to undergo screening colonoscopies 8 years after symptom onset, followed by surveillance procedures depending on patient-specific risk factors. The aim of any screening and surveillance program is to reduce morbidity and mortality due to the disease. With improvements in colonoscopy, most IBD dysplasia can be detected and removed, meaning that the prevention of CRC is possible. To improve prevention, complete endoscopic resection of the dysplastic lesions is important. The challenges associated with IBD dysplasia are that these lesions are often flat, without clear and discrete borders, and can be within inflamed areas of the colon. Inflammation can lead to fibrosis and tethering, limiting endoscopic resection. In this issue of Endoscopy , Angajala et al. [3] explore the challenges associated with endoscopic resection of laterally spreading IBD-associated dysplastic lesions. The authors compared outcomes between IBD- and non-IBD-associated lesions at a tertiary referral center between 2015 and 2021. The majority of the procedures were performed by "expert endoscopists" with dedicated training in endoscopic colonic resection. A total of 210 lesions were included (187 non-IBD and 23 IBD). The average size of the lesions was "large" at 25 mm in the IBD group and 20 mm in the non-IBD group. Most lesions were tubular adenomas with low grade dysplasia or sessile serrated lesions (91% in both cohorts). The morphology of the lesions was similar, and they were also distributed similarly throughout the colon. Despite comparable baseline characteristics, the primary outcome of the study reported significantly higher recurrence rates in the IBD cohort (30%) than in the controls (21%). The recurrence rates in this tertiary center were high in both groups, which is a notable finding. While most recurrences can usually be dealt with endoscopically, this can be more challenging with laterally spreading lesions, and current or prior inflammation can hamper the complete removal of recurrent tissue. With techniques such as excision of a 2–3-mm margin of normal tissue and thermal ablation of defect margins following endoscopic mucosal resection, much lower rates of recurrence are possible. In recent non-IBD cohorts, rates as low as 2% have been reported [4]. Whether these results can be achieved in IBD remains uncertain and it would be interesting to observe whether a significant difference in recurrence persists in contemporary cohorts. The rates of en bloc resection were significantly lower in the IBD cohort (9% vs. 57%), but the difference in recurrence was maintained after adjustment, bearing in mind the small sample size. There are several reasons why en bloc endoscopic mucosal resection may be more challenging in patients with IBD. These lesions can be more difficult to inspect in areas of the colon with active or prior inflammation and scarring, and submucosal injection can be complicated by inflammation and fibrosis. Endoscopic submucosal dissection (ESD) is an obvious candidate technique for improving the rate of en bloc resection. Although ESD is technically challenging and time consuming, the efficacy of the technique is supported in the literature, with an en bloc resection rate of 100% and R0 rate of 96% achieved in a recent IBD cohort [5]. The increased risk with ESD is important to consider, with perforation and bleeding rates being 6% and 8%, respectively. Complications from ESD could be higher in a delicate IBD colon, but most can be managed endoscopically [6]. The additional benefit of achieving R0 resection is that there is a reduced need for "site-check" procedures and surveillance, which is important as the IBD cohort can be subjected to numerous endoscopies, particularly those with dysplastic lesions who undergo more intense surveillance. There will be a rise in the overall number of IBD dysplastic lesions due to the increasing incidence of IBD, longer survival, and increased use of colonoscopy. However, it is important to remember that the overall scale of this problem is relatively small. Advanced Endoscopic Imaging Techniques for the Study of Colonic Mucosa in Patients with Inflammatory Bowel Disease

Abstract:Background: Crohn's disease and ulcerative colitis are inflammatory bowel diseases (IBD) associated with colorectal cancer risk in long-standing diseases. In order to assess the colonic mucosa and to discover dysplastic or neoplastic lesions, advanced endoscopic techniques are needed. Such techniques are detailed in this review: chromoendoscopy, autofluorescence imaging (AFI), narrow band imaging (NBI), i-SCAN, Fujinon Intelligent Color Enhancement (FICE) and confocal laser endomicroscopy (CLE). Aim: The aim of the review is to describe and establish the clinical impact of advanced endoscopic techniques, that could be used in IBD patients'examination in order to assess mucosal healing, microscopic inflammation, dysplasia or neoplasia. Materials and methods: A literature research about new endoscopic approaches of patients with IBD was made. Results: A lot of studies have been performed to reveal which imaging technique might be used for IBD surveillance. Regarding dysplasia or neoplasia detection and mucosal healing or inflammation assessment, CE proved to be superior to white light endoscopy (WLE), while NBI and AFI did not show an encouraging result. I-SCAN did not improve the colonoscopy quality while FICE has been used in a few studies. CLE could be used to characterize a lesion, providing the same results as conventional histology. Conclusion: At the moment, CE is the only technique which has been included in guidelines for IBD surveillance. CLE can be used to assess any lesion detected with WLE during surveillance, while the other imaging techniques require.more studies to determine their efficacy or inefficacy.
