Abstract:PURPOSE: The triglycerides to high-density lipoprotein cholesterol (TG/HDL-C) ratio could be related to insulin resistance (IR). We previously reported that Korean women with polycystic ovary syndrome (PCOS) had a high prevalence of impaired glucose tolerance (IGT). We aimed to determine the cutoff value of the TG/HDL-C ratio for predicting IR and to examine whether the TG/HDL-C ratio is useful for identifying individuals at risk of IGT in young Korean women with PCOS.MATERIALS AND METHODS: We recruited 450 women with PCOS (24±5 yrs) and performed a 75-g oral glucose tolerance test (OGTT). IR was assessed by a homeostasis model assessment index over that of the 95th percentile of regular-cycling women who served as the controls (n=450, 24±4 yrs).RESULTS: The cutoff value of the TG/HDL-C ratio for predicting IR was 2.5 in women with PCOS. Among the women with PCOS who had normal fasting glucose (NFG), the prevalence of IGT was significantly higher in the women with PCOS who had a high TG/HDL-C ratio compared with those with a low TG/HDL-C ratio (15.6% vs. 5.6%, p<0.05).CONCLUSION: The cutoff value of the TG/HDL-C ratio for predicting IR was 2.5 in young Korean women with PCOS, and women with NFG and a high TG/HDL-C ratio had a higher prevalence of IGT. Therefore, Korean women with PCOS with a TG/HDL-C ratio >2.5 are recommended to be administered an OGTT to detect IGT even if they have NFG.

Abstract:Objectives This study aimed to evaluate the diagnostic utility of the Triglyceride Glucose (TyG), Triglyceride Glucose–Body Mass (TyG-BMI), and Lipid Accumulation Product (LAP) indices for both screening Polycystic Ovary Syndrome (PCOS) and diagnosing insulin resistance (IR) in women diagnosed with PCOS. Methods Retrospective data from medical records of 124 women were analyzed, with 71 in the PCOS group and 53 in the non-PCOS group. The PCOS diagnosis followed the 2003 Rotterdam criteria. Basic clinical and biochemical parameters were compared. The TyG index was computed using the formula ln [Triglyceride (TG) (mmol/L) × fasting plasma glukose (FPG) (mg/dL)/2]. TyG-BMI value was derived as TyG × BMI. LAP was calculated as (waist circumference (WC-58) × TG (mmol/L). IR was identified if Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) was >2.7. Results TyG-BMI (AUC=0.62) and LAP indices (AUC=0.61) did not demonstrate statistically significant diagnostic performance for PCOS. Regarding IR in PCOS patients, the highest AUC was for TyG-BMI (0.84, 95 % CI: 0.73–0.93, p<0.001) with a cutoff at 116.15, showing 80 % sensitivity and 86 % specificity. LAP had an AUC of 0.86 with a cutoff of 30.21 (sensitivity 80 %, specificity 81 %), while TyG showed an AUC of 0.78 (95 % CI: 0.67–0.89, p<0.001) with a cutoff of 4.47, demonstrating a sensitivity of 70 % and specificity of 72 %. Conclusions Numerous biochemical markers have been explored for PCOS detection, however, many are expensive, not universally available, and necessitate specific test kits. TyG, TyG-BMI, and LAP indices might not serve as reliable markers for PCOS screening but could offer utility in identifying IR in Turkish women diagnosed with PCOS.

Abstract:AIMS: There are emerging data indicating an association between PCOS (polycystic ovary syndrome) and metabolic derangements with potential impact on its clinical presentation. This study aims to evaluate the pathophysiological processes beyond PCOS with particular focus on carbohydrate metabolism, ectopic lipids and their possible interaction. Differences between the two established classifications of the disease should be additionally evaluated.METHODS: A metabolic characterization was performed in 53 untreated PCOS patients as well as 20 controls including an extended oral glucose tolerance test (OGTT, to assess insulin sensitivity, secretion and ß-cell function) in addition to a detailed examination of ectopic lipid content in muscle and liver by nuclear magnetic resonance spectroscopy.RESULTS: Women with PCOS classified by the original NIH 1990 definition showed a more adverse metabolic risk profile compared to women characterized by the additional Rotterdam 2003 phenotypes. Subtle metabolic derangements were observed in both subgroups, including altered shapes of OGTT curves, impaired insulin action and hyperinsulinemia due to increased secretion and attenuated hepatic extraction. No differences were observed for ectopic lipids between the groups. However, particularly hepatocellular lipid content was significantly related to clinical parameters of PCOS like whole body insulin sensitivity, dyslipidemia and free androgen index.CONCLUSIONS: Subtle alterations in carbohydrate metabolism are present in both PCOS classifications, but more profound in subjects meeting the NIH 1990 criteria. Females with PCOS and controls did not differ in ectopic lipids, however, liver fat was tightly related to hyperandrogenism and an adverse metabolic risk profile.

Abstract:Purpose Insulin resistance (IR) is common in patients with polycystic ovary syndrome (PCOS). Metabolic syndrome (MS) includes, inter alia, IR, hypertension, dyslipidemia, and disturbances in glucose metabolism. The triglyceride–glucose (TyG) index and non-traditional lipid parameters are strong predictors of IR and cardiovascular disease and can be considered as screening indicators for MS. This study aimed to evaluate the predictive potential of non-traditional lipid parameters and the TyG index to identify MS in PCOS. Methods This cross-sectional study included 134 women diagnosed with PCOS (50 patients with comorbid MS and 84 patients without MS). Biochemical indices were collected, and triglycerides (TG)/high-density lipoprotein cholesterol (HDL-C), total cholesterol (TC)/HDL-C, low-density lipoprotein cholesterol (LDL-C)/HDL-C, non-HDL-C, TyG, and TyG–BMI indices were calculated. Logistic regression analysis was used to compare and determine the association of the six parameters with MS, and the receiver operating characteristic (ROC) curve was used to evaluate the performance of each parameter in identifying MS in the PCOS population. Results After adjusting for age and body mass index (BMI), TG/HDL-C, TC/HDL-C, LDL-C/HDL-C, non-HDL-C, TyG, and TyG–BMI were associated with MS (all P <0.05). The odds ratios were 4.075 (0.891, 1.107), 3.121 (1.844, 5.282), 3.106 (1.734, 5.561), 2.238 (1.302, 3.848), 13.422 (4.364, 41.282), and 1.102 (1.056, 1.150), respectively. TG/HDL-C, TC/HDL-C, LDL-C/HDL-C, non-HDL-C, TyG, and TyG–BMI are effective predictors of MS in PCOS, and their cut-off values can be used for the early detection of MS. TyG–BMI had the strongest performance in predicting MS (area under the curve 0.905, 95% CI 0.855–0.956), and its optimal critical value for predicting MS was 202.542. Conclusions TG/HDL-C, TC/HDL-C, LDL-C/HDL-C, non-HDL-C, TyG, and TyG–BMI are novel, clinically convenient and practical markers for the early identification of MS risk in PCOS patients.

Abstract:Background: We sought to explore the potential relationship between serum levels of thyroid hormones with those of androgen and metabolic parameters in women with polycystic ovary syndrome (PCOS). Methods: Data from 1059 Chinese women with PCOS and 1015 healthy women was retrospectively collected. This data including fasting glucose and insulin, thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4), total triiodothyronine (TT3), total thyroxine (TT4), anti-thyroperoxidase antibody (ANTI-TPO), anti-thyroglobulin (ATG), dehydroepiandrosterone sulfate (DHEAS), total testosterone (TTE), follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), prolactin (PRL), progesterone (PGN), triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL) and low-density lipoprotein cholesterol (LDL). Thyroid-related indicators were compared between PCOS and non-PCOS patients enrolled in this study. Independent variables of PCOS were compared among subgroups in accordance with the classification of TSH, homeostatic model assessment of insulin resistance (HOMA-IR), and TTE levels. To further explore the association between thyroid hormones levels and correlated metabolic parameters in PCOS, multiple regression analyses were conducted. Results: Our study found that PCOS patients had significantly higher serum TSH, FT3, TT3 and TT4 levels than non-PCOS patients. PCOS patients with TSH >= 2.5 mIU/L had significantly higher TG, fasting insulin, HOMA-IR and homeostatic model assessment of beta-cell function (HOMA-B), however, these patients also displayed significantly lower DHEAS, HDL, and quantitative insulin sensitivity check index (QUICKI) when compared to patients with TSH levels <2.5 mIU/L. PCOS patients with HOMA-IR levels >= 2.5 mIU/L demonstrated significantly higher FT3 and TSH, but lower TT3 when compared to women with HOMA-IR levels <2.5 mIU/L. Four groups divided by TTE displayed significant differences in FT3 in PCOS patients. Multiple linear regression analysis showed that TSH was significantly negatively associated with DHEAS and QUICKI. Conclusions: TSH levels are closely correlated to the metabolic and endocrine characteristics of PCOS, especially dyslipidemia and insulin resistance.
Feasibility analysis of triglyceride as a surrogate marker for insulin resistance in Chinese women with polycystic ovary syndrome

Abstract:Objective: To analyze the feasibility of triacylglycerol (TAG) as a surrogate marker for insulin resistance in Chinese women with polycystic ovary syndrome (PCOS). Methods: Height, weight, TAG, total cholesterol (TC), high density lipoprotein (HDL), OGTT, insulin release test were measured in 171 women with PCOS according to Rotterdam consensus, who went to Department of Obstetrics and Gynecology of Ruijin Hospital from May, 2012 to June, 2015. Body mass index (BMI) and HOMA-IR were calculated. PCOS patients were divided into obese and non-obese group by BMI to analyze the feasibility of TAG as a surrogate marker for insulin resistance in two groups. Results: TAG, HDL, TC, plasma glucose and insulin levels in obese group were significantly higher than those in non-obese group (P<0.05). The proportion of patients with insulin resistance was also higher in obese group (P=0.000). The ROC curve analysis demonstrated that TAG could not predict insulin resistance in obese group. But in non-obese group the areas under the ROC curve was 0.791. The optimal cut-off point was 1.585 mmol/L with sensitivity of 0.484 and specificity of 0.979. Conclusion: TAG can be used as a surrogate marker for insulin resistance in non-obese Chinese women with PCOS.
Relationship between hyperandrogen and impaired glucose regulation in women with polycystic ovary syndrome

Abstract:Objective To explore the relationship between hyperandrogen and abnormal glucose tolerance (AGT) in women with polycystic ovary syndrome (PCOS), and to analyze the potential association between free androgen index (FAI) and insulin sensitivity/islet beta cell function. Methods Clinical data of 378 PCOS women including sex hormone and other parameters obtained by 75 g glucose tolerance test (OGTT) were retrospectively analyzed from Endocrinology Department of Drum Tower Hospital between April 2013 and March 2017. Free androgen index (FAI), 1/homeostasis model assessment of insulin resistance (1/HOMA-IR) index, Matsuda insulin sensitivity index (ISIM), HOMA-β, InsAUC-30/GluAUC-30, InsAUC-120/GluAUC-120 and disposition index (DI) were calculated. Variance analysis orχ2 test was used for comparison between groups. Bonferroni method was used for post hoc comparison. Multivariate logistic regression was used to analyze the relationship between FAI and AGT. Spearman correlation (corrected by age and body mass index) was used to analyze the relationship between FAI and insulin sensitivity index/βcell function. Results Of 378 PCOS women, the mean age was (27.8 ± 4.4) years, 59.8％(226 of 378) had normal glucose tolerance (NGT), 31.5％(119 of 378) had pre-diabetes, and 8.7％ (33 of 378) had type 2 diabetes mellitus (T2DM). FAI in AGT [pre-diabetes 6.2(3.6, 10.3) and T2DM 7.9(4.4, 15.5) ] were higher than those in NGT [5.0(2.8, 7.6), F=9.155, P<0.001]. FAI(OR: 1.166, 95％CI: 1.069-1.272) was an independent risk factor for AGT in women with PCOS. With the increase of FAI, insulin sensitivity [1/HOMA-IR, ISIM and β cell function (DI-30, DI-120) decreased (F=7.638, 6.863, 3.366, all P<0.05), while insulin secretion (HOMA-β, InsAUC-30/GluAUC-30 and InsAUC-120/GluAUC-120, DI-120) had a trend of increase, but the differences were not statistically significant (F=1.828, 1.315, both P>0.05). Conclusion High FAI levels are associated with high incidence of AGT, and FAI can be considered as an indicator of insulin resistance orβcell dysfunction in PCOS women.
Combined obesity- and lipid-related indices are associated with hypogonadism in Chinese male patients with type 2 diabetes: a cross-sectional study

Abstract:Background There is insufficient attention to hypogonadism in Chinese males with type 2 diabetes mellitus (T2DM). We evaluated the relationship between Combined obesity- and lipid-related indices [Visceral Adiposity Index (VAI), Chinese Visceral Adiposity Index (CVAI), Triglyceride Glucose Index (TyG) and Lipid Accumulation Product (LAP)] with total testosterone (TT) and analyzed the predictive capability of the respective cut-off values. Methods We recruited 958 hospitalized male patients with T2DM at the Affiliated Hospital of Qingdao University, collected baseline data and four calculated indices, and obtained their dominance ratio (OR) and corresponding 95% confidence intervals (CI) with TT by multivariate logistic regression. Receiver operating characteristic (ROC) curves were then used to determine cutoff values in predicting hypogonadism (TT< 12 nmol/L), and we also analyzed the combinations between the different indices. Results VAI, CVAI, TyG, and LAP all have satisfactory predictive capabilities. The test capability (sensitivity and specificity) of all four indices was better or not worse than that of body mass index (BMI), homeostasis model assessment of insulin resistance (HOMA-IR) and waist circumference (WC). All four indices were effective predictors of hypogonadism at their respective cutoff values (VAI ≥ 2.284, CVAI ≥ 145.779, TyG ≥ 4.308, and LAP ≥ 59.850). Of these, LAP had the largest area under the curve (AUC, AUC = 0.852, Std. Error = 0.014, 95% CI = 0.818-0.873). However, the predictive capability of the combined indices was not significantly improved over the individual indices. Conclusions VAI, CVAI, TyG, and LAP are sensitive indices for predicting hypogonadism in Chinese male patients with T2DM. Considering the need for concise and accurate indices in clinical practice, we suggest LAP as a commonly used index.

Abstract:Objective: To investigate the prevalence, pattern and risk predictors for dyslipidemia among Chinese women with polycystic ovary syndrome (PCOS). Study Design and Methods: A total of 1,000 women diagnosed as PCOS by modified Rotterdam criteria were enrolled in 27 hospitals across China in a randomized controlled trial. Anthropometric, metabolic parameters, sex hormone, and lipid levels were measured at the baseline visit. Dyslipidemia was defined according to total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and triglycerides (TG) level. Independent t -test and logistic regression were used to identify predictors for dyslipidemia. Area under the receiver operating characteristic curve (AUC) was calculated. Results: A total of 41.3% of the women had dyslipidemia, and the prevalence of abnormal TC, LDL-C, HDL-C, and TG were 8.6, 9.1, 26.9, and 17.5%, respectively. Logistic regression found that age, waist circumference, insulin, follicle-stimulating hormone, and sex hormone-binding globulin were independent predictors for dyslipidemia. When combining these predictors, the AUC was 0.744. The cut-off points were age >28.5 years, waist circumference >86.5 cm, insulin >96.0 pmol/L, follicle-stimulating hormone <5.6 mIU/mL, and sex hormone-binding hormone <31.0 nmol/L, respectively. Conclusion: Dyslipidemia was common in Chinese women with PCOS, and low HDL-C level was the predominant lipid abnormality. Age, waist circumference, follicle-stimulating hormone, insulin and sex hormone-binding globulin were predictive for dyslipidemia among Chinese women with PCOS.

Effect of HbA1C Detection on the Diagnostic Screening for Glucose Metabolic Disorders in Polycystic Ovary Syndrome.

Abstract:PURPOSE This study aimed to assess the effect of hemoglobin A1C (HbA1C) detection on the diagnostic screening for glucose metabolic disorders in women with polycystic ovary syndrome (PCOS). MATERIALS AND METHODS A total of 161 patients with PCOS (mean age = 23.68 +/- 4.23 years) were subjected to an oral glucose tolerance test (OGTT). The receiver operating characteristic (ROC) curve was plotted to evaluate the fasting plasma glucose (FPG), and HbA1C was used to probe the sensitivity and specificity of abnormal glucose tolerance. RESULTS Based on the traditional standards of blood sugar, the prevalence of type 2 diabetes was 5.6%, and the pre-diabetes prevalence was 7.5%. Based on the HbA1C standards, 4.3% of patients were diagnosed with type 2 diabetes, and 10.6% of the diabetic patients can be considered as high-risk populations. Based on the combined standards of OGTT and HbA1C, the prevalence of type 2 diabetes was 6.2%, and the pre-diabetes prevalence was 12.4%. OGTT is considered the gold standard for identifying abnormal glucose tolerance, and HbA1C detection is considered to be stronger than FPG. The areas under the ROC curves of HbA1C and FPG were 0.968 and 0.672, respectively (p < 0.01). The American Diabetes Association (ADA) recommends the cut-off value of HbA1c > or = 5.7% and FPG > or = 5.6 mmol/l for identifying abnormal glucose tolerance. The sensitivity and specificity were 76.7% and 89.5% for HBA1C, as well as 40.5% and 94.3% for FPG, respectively. The positive and negative likelihood ratios were 7.3 and 0.26 for HbA1C, as well as 7.1 and 0.63 for FPG, respectively. CONCLUSION HbA1C detection can be used as a method for diagnosis and screening.
Association of Androgen Excess with Glucose Intolerance in Women with Polycystic Ovary Syndrome

Abstract:Women with polycystic ovary syndrome (PCOS) show high prevalence of glucose intolerance. This study aimed to investigate the association of androgen excess with glucose intolerance in PCOS. A total of 378 women with PCOS participated in the study. Free androgen index (FAI) was selected as indicator of hyperandrogenism. Insulin sensitivity was assessed by 1/homeostasis model assessment of insulin resistance (1/HOMA-IR) and Matsuda insulin sensitivity index (ISIM); β-cell function was assessed by disposition index (DI). We found that women with glucose intolerance had higher FAI levels compared to women with normal glucose tolerance (NGT) (prediabetes 6.2, T2DM 7.9 versus NGT 5.0, resp.; p < 0.001). Furthermore, there was a direct association between FAI levels and frequency of glucose intolerance (OR = 2.480, 95% CI 1.387-4.434), even after adjusting for age, BMI, waist circumference, hypertension, fasting insulin, testosterone, SHBG, and family history of diabetes. In addition, with FAI increase, glycosylated hemoglobin (HbA1c), plasma glucose concentrations, and serum insulin levels increased, while insulin sensitivity and β-cell function decreased. Our results suggested that androgen excess indicated by high FAI levels might serve as indicator of glucose intolerance, as it might promote insulin resistance and β-cell dysfunction in women with PCOS.

Abstract:BACKGROUND: Cardiovascular disease (CVD) is one of the debilitating consequences of polycystic ovary syndrome (PCOS). Early diagnosis of metabolic syndrome (MetS) with a simple but accurate method can reduce the risk of progression to CVD in PCOS.AIMS: This study aimed to determine the accuracy of various anthropometric indices and lipid accumulation product (LAP), in assessing the risk of MetS in PCOS.SETTINGS AND DESIGN: This is a cross-sectional study including 150 PCOS women and 100 control subjects.MATERIALS AND METHODS: Anthropometric parameters were measured and calculated. Lipid profile, fasting plasma glucose (FPG), and insulin were estimated. MetS was detected according to the International Diabetes Federation criteria.STATISTICAL ANALYSIS: Logistic regression and receiver operating characteristic curve analysis were applied to determine the potential association of anthropometric indices such as body mass index, waist circumference (WC), waist-to-hip ratio, waist-to-height ratio, conicity index (CI), visceral adiposity index (VAI), abdominal volume index (AVI), body adiposity index (BAI), and a body shape index (ABSI) and LAP with MetS.RESULTS: In our study of PCOS women of the south Indian population, the prevalence of MetS was 59.3%, which was higher than other populations and the cutoff values of VAI and LAP were 6.05 and 53, respectively. VAI showed the strongest association with MetS, followed by diastolic blood pressure BP, FPG, and LAP.CONCLUSIONS: We recommend VAI and LAP as new indices for MetS diagnosis. As these indices exhibit population specificity, it is imperative that independent cutoffs are determined for every demographic population.

Abstract:OBJECTIVETo analyze the characteristics of abnormal glucose tolerance in patients with polycystic ovary syndrome (PCOS).METHODSA 75 g oral glucose tolerance test (OGTT) with plasma glucose and serum insulin (INS) levels was performed in 257 PCOS patients. The states of glucose tolerance were classified by fasting plasma glucose (FPG) and plasma glucose at 120 minutes according to the Word Health Organization (WHO) criterion.RESULTSAccording to the WHO criteria, 69.3% were classified as normal glucose tolerance (NGT) and 30.7% as with abnormal glucose metabolism, with 26.1% of them presenting impaired glucose tolerance (IGT) and 4.7% diabetes mellitus (DM). Age, BMI, fasting glucose or 2 h glucose concentration, fasting insulin or 2 h glucose concentration and homeostasis model assessment-insulin resistance (HOMA-IR) increased gradually among NGT, IGT and 2DM. QUICKI and the ratio of fasting glucose/ fasting insulin decreased gradually. The prevalence of abnormal glucose tolerance was significantly higher after 30 years old. There was significant correlation between abnormal glucose tolerance and age or BMI. A positive correlation was found between BMI and fasting glucose or 2-h glucose concentration.CONCLUSIONAbnormal glucose tolerance was correlated with age and BMI in PCOS patients. Insulin resistance is the mechanistic basis of abnormal glucose tolerance. Thus PCOS patients should be screened by glucose tolerance at an early time and a regular interval.
Optimal obesity- and lipid-related indices for predicting type 2 diabetes in middle-aged and elderly Chinese

Abstract:To investigate the screening and predicting functions of obesity- and lipid-related indices for type 2 diabetes (T2D) in middle-aged and elderly Chinese, as well as the ideal predicted cut-off value. This study's data comes from the 2011 China Health and Retirement Longitudinal Study (CHARLS). A cross-sectional study design was used to investigate the relationship of T2D and 13 obesity- and lipid-related indices, including body mass index (BMI), waist circumference (WC), waist–height ratio (WHtR), visceral adiposity index (VAI), a body shape index (ABSI), body roundness index (BRI), lipid accumulation product (LAP), conicity index (CI), Chinese visceral adiposity index (CVAI), triglyceride- glucose index (TyG index) and its correlation index (TyG-BMI, TyG-WC, TyG-WHtR). The unadjusted and adjusted correlations between 13 indices and T2D were assessed using binary logistic regression analysis. The receiver operating characteristic curve (ROC) was used to determine the usefulness of anthropometric indices for screening for T2D and determining their cut‐off value, sensitivity, specificity, and area under the curve (AUC). The study comprised 9488 people aged 45 years or above in total, of whom 4354 (45.89%) were males and 5134 (54.11%) were females. Among them were 716 male cases of T2D (16.44%) and 870 female cases of T2D (16.95%). A total of 13 obesity- and lipid-related indices were independently associated with T2D risk after adjusted for confounding factors ( P < 0.05). According to ROC analysis, the TyG index was the best predictor of T2D among males (AUC = 0.780, 95% CI 0.761, 0.799) and females (AUC = 0.782, 95% CI 0.764, 0.799). The AUC values of the 13 indicators were higher than 0.5, indicating that they have predictive values for T2D in middle-aged and elderly Chinese. The 13 obesity- and lipid-related indices can predict the risk of T2D in middle‐aged and elderly Chinese. Among 13 indicators, the TyG index is the best predictor of T2D in both males and females. TyG-WC, TyG-BMI, TyG-WHtR, LAP, and CVAI all outperformed BMI, WC, and WHtR in predicting T2D.

Abstract:Background: There are various parameters to analyze obesity, however, no standard reference to predict, screen or diagnose PCOS with various obesity parameters has been established, and the accuracy of these parameters still needs to be studied. This study was to use the receiver operating characteristic (ROC) curve to explore the different values of three obesity parameters, body mass index (BMI), waist circumference (WC) and percentage of body fat (PBF) in the diagnosis of polycystic ovary syndrome (PCOS) in Chinese childbearing women.Methods: Three hundred patients who were diagnosed with PCOS at Center of Reproductive Medicine and Genetics of Peking University First Hospital were enrolled in this study, and 110 healthy age-matched women were enrolled as controls. The characteristics of BMI, WC and PBF in PCOS patients were analyzed.Results: Compared with the control group, all the three obesity parameters were significantly increased in PCOS group. In terms of ROC area under the curve, WC > PBF > BMI, and they were all significantly different from those of the control. At a cut-off point of 80.5 cm, WC has a sensitivity of 73.6 % and a specificity of 85 % in diagnosis of PCOS; At a cut-off point of 29 %, PBF has a sensitivity of 88.2 % and a specificity of 57.7 % in diagnosis of PCOS; and at a cut-off point of 26.6 kg/m(2), BMI has a sensitivity of 54.5 % and a specificity of 98 % in diagnosis of PCOS.Conclusion: WC, BMI and PBF are valuable in screening and diagnosis of PCOS in Chinese childbearing women. PBF can be used to screen PCOS as it has a better sensitivity, while BMI can be used in the diagnosis of PCOS as it has a better specificity.

Abstract:Background: Insulin resistance (IR) is a pivotal pathogenic component of metabolic diseases. It is crucial to identify convenient and reliable indicators of insulin resistance for its early detection. This study aimed at assessing the predictive ability of seven novel obesity and lipid-related indices. Methods: A total of 5,847 female and 3,532 male healthy subjects were included in the study. The triglyceride glucose (TyG) index, TyG-body mass index (TyG-BMI), TyG-waist circumference (TyG-WC), lipid accumulation products (LAP), body roundness index (BRI), body adiposity index (BAI), and visceral adiposity index (VAI) were measured and calculated using the established formulae. IR was diagnosed using the homeostatic model assessment of insulin resistance (HOMA-IR) index over the third quantile. Results: The levels of all seven lipid-related indices were significantly higher in subjects with higher HOMA-IR values than in those with lower HOMA-IR values. These indices displayed moderate to high effectiveness [receiver operating characteristic (ROC) curve-area under the curve (AUC) > 0.6] in predicting IR. Among them, TyG-BMI (AUC: 0.729), LAP (AUC: 0.708), and TyG-WC (AUC: 0.698) showed the strongest association with HOMA-IR. In the female population, the AUC for TyG-BMI, LAP, and TyG-WC in predicting IR was 0.732, 0.705, and 0.718, respectively. Logistic regression analysis showed the optimal cut-off values of those indicators in predicting IR as follows: TyG-BMI: male subjects - 115.16 [odds ratio (OR) = 6.05, 95% CI: 5.09-7.19], female subjects - 101.58 (OR = 4.55, 95% CI: 4.00-5.16); LAP: male subjects - 25.99 (OR = 4.53, 95% CI: 3.82-5.38), female subjects - 16.11 (OR = 3.65, 95% CI: 3.22-4.14); and TyG-WC: male subjects - 409.43 (OR = 5.23, 95% CI: 4.48-6.24), female subjects - 342.48 (OR = 4.07, 95% CI: 3.59-4.61). Conclusion: TyG-index-related parameters and LAP appear to be effective predictors of IR in the Chinese population. Specifically, TyG-BMI may be the most appropriate predictor of IR.

Abstract:Objective To explore the levels of cardiometabolic index (CMI) and its relationship with early phase β-cell function in overweight/obese polycystic ovary syndrome (PCOS) women. Method Three hundred and eighty-two overweight and obese women from Nanjing Drum Tower Hospital between April, 2016 and September, 2017 were recruited and divided into body mass index matched simple overweight/obese group (n=75) and overweight/obese PCOS group (n=307). Data of weight, height, blood pressure, heart rate and related laboratory examinations at their first visit were collected, indexes of early phaseβ-cell function (AUCIns30/AUCGlu30, DI30, Ins30/FINS) and CMI were calculated. Independent sample t test, analysis of variance and Pearson correlation coefficient were used for statistical analysis. Result Compared with simple overweight/obese women, waist-hip ratio, serum insulin levels during oral glucose tolerance test (OGTT), CMI (1.86 ± 0.69 vs 1.64 ± 0.91) and AUCIns30/AUCGlu30 (1.08 ± 0.24 vs 0.98 ± 0.32) were significantly higher respectively (t=-5.631--2.522, P<0.05), while HDL-C, 1/HOMA-IR (0.20±0.13 vs 0.28±0.16), DI30 (1.69±0.69 vs 1.88±0.72) and Ins30/FINS (5.76±3.29 vs 6.79±3.90) were significantly lower respectively (t=1.938-4.974, P<0.05) in overweight/obese PCOS women. Indexes relating early phase insulin secretion were significantly lower in overweight/obese PCOS women with higher CMI levels than those in overweight/obese PCOS women with lower CMI levels (P<0.05). In addition, CMI was negatively correlated with lgDI30 and Ins30/FINS after adjusting age, BMI, blood pressure and heart rate (r=-0.167,-0.162, P=0.004, 0.006). Conclusion Compared with simple overweight/obese women, overweight/obese PCOS women have higher cardiovascular risk and impaired early phase insulin secretion function. Early phase β-cell function indexes were negatively correlated with cardiovascular metabolic risk in overweight/obese PCOS women.
