Abstract:Abstract Background Femoral posterior condylar offset (PCO) and posterior tibial slope (PTS) are important for postoperative range of motion after total knee arthroplasty (TKA). However, normative data of PCO and PTS and the correlation between them among healthy populations remain to be elucidated. The purpose of this study was to determine PCO and PTS in normal knees, and to identify the correlation between them. Methods Eighty healthy volunteers were recruited. CT scans were performed followed by three-dimensional reconstruction. PCO and PTS were measured and analyzed, as well as the correlation between them. Results PTS averaged 6.78° and 6.11°, on the medial and lateral side respectively ( P = 0.002). Medial PCO was greater than lateral (29.2 vs. 23.8 mm, P < 0.001). Both medial and lateral PCO of male were larger than female. On the contrary, male medial PTS was smaller than female, while there was no significant difference of lateral PTS between genders. There was an inverse correlation between medial PCO and PTS, but not lateral. Conclusions Significant differences exhibited between medial and lateral compartments, genders, and among individuals. An inverse correlation exists between PCO and PTS in the medial compartment. These results improve our understanding of the morphology and biomechanics of normal knees, and subsequently for optimising prosthetic design and surgical techniques.

Can Asymmetry in Total Knee Arthroplasty Design Lead to More Normal-Like Post-Operative Kinematics? A Multi-Implant Evaluation

Abstract:Background Few studies have evaluated the effects of symmetrical versus asymmetrical implant designs, more specifically the femoral condyles, trochlear groove, joint line, and bearing surfaces. The objective of this study is to investigate multiple posterior cruciate-retaining (PCR) total knee arthroplasty (TKA) designs influencing factors related to TKA asymmetry, and whether asymmetry can improve postoperative knee kinematics. Methods In vivo tibio-femoral kinematics for 99 subjects were evaluated in this retrospective study. Overall, ten subjects had a non-implanted, normal knee, and 89 subjects had one of three PCR TKAs with varying degrees of asymmetry within their femoral and tibial components (PCR #1=30, PCR #2=29, PCR #3=30). All TKAs were implanted by the same surgeon and were analyzed using fluoroscopy during a deep knee bend. Results At full extension, all three PCR TKAs experienced a more posteriorized position of the femoral condyles compared to the normal knee, with the two asymmetrical PCR TKAs experiencing more anteriorization compared to the third, symmetrical PCR TKA. Both the normal knee and the PCR TKA with greatest amount of asymmetry experienced statistically more posterior femoral rollback (PFR) of the lateral condyle than the other two PCR TKAs. The PCR TKA with greater asymmetry also experienced statistically greater range-of-motion (ROM) than the other two PCR TKAs. Conclusion With increasing flexion, the design with the most asymmetry also experienced the most PFR, axial rotation, and greatest ROM. The results in this study seem to suggest that the inclusion of asymmetry in a TKA could be beneficial for achieving more normal-like kinematics and greater weight-bearing knee flexion.

Abstract:Bone resection is highly valued in total knee arthroplasty (TKA), but how to determine the amount of distal femur resection is still controversial. The purpose of this study was to explore how to use lateral condyle as a reference for distal femoral osteotomy in TKA. Magnetic resonance imaging (MRI) and Radiographic images from 118 nonarthritic subjects and 123 osteoarthritis (OA) subjects were used to assess the cartilage wear pattern of the distal femur in varus knees. Measurements were performed on three-dimensional reconstruction after virtual bone cutting. The difference between the resection amount of distal (0°) and posterior (90°) was calculated when the medial condyle was used as a reference in OA patients. The osteotomy amount on lateral was calculated in nonarthritic subjects when the medial condylar osteotomy was consistent with the thickness of the implants. In 43% of OA patients, there was > 1 mm difference between the 0° and 90° in medial condyle cartilage, and no difference was observed in lateral. When using medial condyle as a reference for osteotomy, there was a difference of 1.3 ± 0.56 mm between the resection amount of 0° and 90°, and the difference was 0.24 ± 0.27 mm when using lateral condyle. Statistical analysis showed that there was a linear correlation between the resection amount of lateral condyle and mechanical lateral distal femoral angle (mLDFA) in nonarthritic subjects ( r = 0.845, p < 0.001). Lateral distal femoral condyle has more uniform cartilage wear in varus knee osteoarthritis. Using the lateral condyle as the reference for distal femoral osteotomy is more suitable for the cartilage wear pattern of the varus knee. The position of cutting guide can be adjusted by preoperative measurements of mLDFA.

Significance of Posterior Condyle Offset in Total Knee Arthroplasty

Abstract:Posterior condyle offset (PCO), firstly proposed by Bellmans in 2002, was thought to be associated with knee flexion after total knee arthroplasty (TKA) in the beginning. At present, four mainly methods have been widely used for measuring the PCO. X-ray is a simple and easy way, except for ignoring the asymmetry between medial and lateral femoral condyles, which leads to defects in principle. CT and MRI are not restricted by the projection angle, but the interferences of prosthetic metal artifacts cannot be ignored. Full-thickness cartilage-based posterior femoral condylar offset is the most accurate method, but the measurement is complicated and subjective. PCO has important clinical significance in TKA surgery. As for posterior cruciate ligament-retained TKA, the PCO affects the maximum knee flexion function by changing the radius of the rotation between femur and tibia. However, it doesn't happen in the posterior cruciate ligament replacement TKA, which may be related to the blocking of the soft tissue behind the knee and the restriction of the knee extension device. In addition, changes in PCO affect the flexion gap, which influences the stability of the knee joint, especially for flexion and mid-flexion. It also affects the anatomical structure and biomechanical structure of the knee joint, which may lead to osteolysis and early loosening. Excessive PCO increases the tension of the posterior joint capsule, resulting in a reduction of the extension gap, which brings the risk of knee flexion contracture. Therefore, it is necessary for physicians to restore the PCO to the preoperative state, in case of the occurrence of adverse consequences. This article summarized the clinical significance in TKA. Effect of joint line orientation parameters on initial bone resection in mechanically aligned total knee arthroplasty: a retrospective clinicoradiological correlation study

Abstract:Background: Discrepancies in bone resection between the medial and lateral compartments are very common in total knee arthroplasty (TKA) when mechanical alignment (MA) is used. The purpose of this study was to explore whether and how joint line orientation affects the initial bone resection in mechanically aligned TKA. Methods: A total of 194 patients (225 knees) diagnosed with osteoarthritis (OA) were included. Virtual bone resection was conducted in the coronal view using full-length weight-bearing radiographs according to the technical requirements of MA, and the reliability of the virtual resection was verified via intraoperative caliper measurements. Correlation and regression analyses were conducted between the initial bone resection within the extension gap (EG) and various parameters, including the hip-knee-ankle (HKA) angle, mechanical lateral distal femoral angle (mLDFA), joint line congruence angle (JLCA), and medial proximal tibial angle (MPTA). Moreover, the correlation between intraoperative bone resection adjustments and joint line orientation parameters was also investigated. Results: All knees in the current case series were artificially divided into 4 subgroups: subgroup 1, containing 148 varus knees (65.8%) with valgus femurs; subgroup 2, containing 48 varus knees (21.3%) with varus femurs; subgroup 3, containing 17 valgus knees (7.6%) with varus tibias; and subgroup 4, containing 12 valgus knees (5.3%) with valgus tibias. In subgroup 1, the mLDFA and MPTA were positively correlated with the initial bone resection with regression coefficients of 0.670 and 0.089, respectively. Moreover, in all varus knees, intraoperative bone resection adjustments were negatively correlated with mLDFA and MPTA, with categorical regression coefficients of -0.426 and - 0.230, respectively. Conclusion: When MA-TKAs are performed in varus knees with valgus femurs, the initial bone resection within the EG is mainly positively correlated with mLDFA, while the intraoperative bone resection adjustment is significantly correlated with mLDFA and MPTA in all varus knees.
Femoral Anterior Condyle Height Decreases As the Distal Anteroposterior Size Increases in Total Knee Arthroplasty: A Comparative Study.

Abstract:PURPOSE:The anterior flange height of the current femoral component increases with an increasing distal femoral anteroposterior dimension. During total knee arthroplasty (TKA), we have observed that a large femur may have a thinner anterior condyle, whereas a small femur may have a thicker anterior condyle. The first purpose of this study was to examine whether the femoral anterior condyle height decreases as the distal femoral anteroposterior size increases and whether gender differences exist in anterior condyle height.METHODS:A total of 1218 knees undergoing TKA intraoperative and computed tomography scans from 303 healthy knees were used to measure the anterior lateral condylar height (ALCH), anterior medial condylar height (AMCH), and the lateral anteroposterior (LAP) and medial anteroposterior (MAP) dimensions of distal femurs. The LAP and MAP measurements were used for adjustments to determine whether gender differences exist in anterior condyle heights. Linear regression analysis was performed to determine correlations between ALCH and LAP or between AMCH and MAP.RESULTS:There were significant differences between males and females in ALCH in both the CT and TKA groups and AMCH in the CT group (all P<0.01). After adjusting for LAP and MAP, there were significant gender differences in the lateral and medial condylar heights in both groups (P<0.01). There were significant negative correlations between ALCH and LAP values and between AMCH and MAP values in both CT and TKA measurements, with the LAP and MAP values increasing as ALCH and AMCH decreased.CONCLUSIONS:The results demonstrate that femoral anterior condylar height decreased with increasing anteroposterior dimension in both the medial and lateral condyle. In addition, this study also showed that anterior condylar heights are highly variable, with gender differences. The data may provide an important reference for designing femoral anterior flange thickness to precisely match the natural anterior condylar anatomy.

Abstract:Purpose The present study was designed to compare the dimensions of the anterior femoral condyle with those of the anterior component flange using intraoperative morphological data. Methods Overall, 1227 knees in 962 patients were included in this study. The height of the anterior lateral/medial condylar height (ALCH/AMCH) was measured and compared between men and women. These morphological data were compared with the dimensions of the chosen component for each patient. The lateral/medial anterior femoral offset was calculated, and the over-/understuffing rates were compared between men and women. Results The median ALCH in men was 8.5 mm (6.5, 10.0 mm) versus 7.0 mm (6.0, 9.0 mm) in women. The median AMCH in men was 4.0 mm (2.5, 6.0 mm) versus 3.5 mm (2.5, 6.0 mm) in women. There was a significant difference between the genders with respect to ALCH ( p < 0.05). When the dimensions of the component were compared with those of the native knee, the anterior lateral flange height was smaller than the native knees (1.3 mm in male, 0.7 mm in female), but the anterior medial flange height was larger than the corresponding condyle (1.8 mm in male, 1.8 mm in female). A significant difference was observed between the genders with respect to the ideal fit rate of ALCH ( p < 0.01). Conclusion The results demonstrate that gender differences exist in the anterior lateral condyles of knees. The anterior flange of component is not designed to precisely reproduce normal trochlear anatomy. Level of evidence II.

Abstract:This study analyzed 31 patients with symptomatic osteoarthritic knees scheduled to undergo knee arthroplasty or high tibial osteotomy and demonstrated shape variations in their proximal tibia using an average three-dimensional (3D) bone model. Preoperative computed tomography of the affected knees was reconstructed as 3D bone models using a triangle mesh of surface layers. The initial case was defined as the template, and the other models were reconstructed into homologous models with the same number of mesh vertices as that in the template. The corresponding mesh vertices of the other models were averaged to evaluate the spatial position on the particular mesh vertex of the template. This was applied to all the mesh vertices of the template to generate the average 3D model. To quantify the variation in surface geometry, average minimum distance from the average bone model to 31 models was recorded. The medial proximal tibial cortex (1.63 mm) revealed lesser variation compared to the tibial tuberosity (2.50 mm) and lateral cortex (2.38 mm), ( p = 0.004 and p = 0.020, respectively). The medial tibial plateau (1.46 mm) revealed larger variation compared to the lateral tibial plateau (1.16 mm) ( p = 0.044). Understanding 3D geometry could help in development of implants for arthroplasty and knee osteotomy.

An MRI-Based Correlation Study Investigating the Effect of Varying Posterior Femoral Condyle Angles on the Patellofemoral Joint

Abstract:Background: Since the beginning of total knee arthroplasty, implant alignment has been a central point of discussion. As diverse as the discussed alignment theories are, as uniform is the implant design, which is supposed to be based on the "average knee." Steady upgrades in prosthesis design and modern alignment theories have achieved improvements. However, knee arthroplasty continues to be burdened by a significant percentage of dissatisfied patients. In current knee arthroplasty, femoral implant alignment is referenced by the dorsal and distal condylar axes. The patellofemoral joint variance is not sufficiently considered. Predominantly dorsal and distal referencing at the femoral condyle determines the postoperative shape of the anterior knee joint. The present study investigated a possible relationship between dorsal and distal joint parameters and the patellofemoral joint. Methods: In this explorative retrospective monocentric study, MRI cross-sectional images of 100 native knee joints were evaluated. By determining parametric correlations according to Pearson, the study investigates whether the independent variables "posterior femoral condyle angle" and "lateral distal femoral angle" are related to "lateral trochlear inclination", "patella tilt", and "bisect offset". Results: The posterior condylar angle significantly correlates with lateral trochlear inclination, patella tilt, and bisect offset. There is a positive correlation with patella tilt and bisect offset but a negative correlation with lateral trochlear inclination. The lateral distal femoral angle did not correlate with the studied parameters. Conclusion: The lateral trochlear inclination decreases with an increased posterior femoral condylar angle. The posterior referencing of the femoral component in total knee arthroplasty simultaneously establishes the shape of the anterior knee joint. Our results indicate that increasing posterior condyle angles significantly correlate with flattened lateral trochlear inclinations in native knees and suggest a systematic biomechanical conflict in total knee arthroplasty.

Three-Dimensional Bowing Measurement of Distal Femur at Actual Size and Clinical Implications of Total Knee Arthroplasty

Abstract:Background and Objectives: To assess femoral shaft bowing (FSB) in coronal and sagittal planes and introduce the clinical implications of total knee arthroplasty (TKA) by analyzing a three-dimensional (3D) model with virtual implantation of the femoral component. Materials and Methods: Sixty-eight patients (average age: 69.1 years) underwent 3D model reconstruction of medullary canals using computed tomography (CT) data imported into Mimics® software (version 21.0). A mechanical axis (MA) line was drawn from the midportion of the femoral head to the center of the intercondylar notch. Proximal/distal straight centerlines (length, 60 mm; diameter, 1 mm) were placed in the medullary canal's center. Acute angles between these centerlines were measured to assess lateral and anterior bowing. The acute angle between the distal centerline and MA line was measured for distal coronal and sagittal alignment in both anteroposterior (AP) and lateral views. The diameter of curve (DOC) along the posterior border of the medulla was measured. Results: The mean lateral bowing in the AP view was 3.71°, and the mean anterior bowing in the lateral view was 11.82°. The average DOC of the medullary canal was 1501.68 mm. The average distal coronal alignment of all femurs was 6.40°, while the distal sagittal alignment was 2.66°. Overall, 22 femurs had coronal bowing, 42 had sagittal bowing, and 15 had both. Conclusions: In Asian populations, FSB can occur in coronal, sagittal, or both planes. Increased anterolateral FSB may lead to cortical abutment in the sagittal plane, despite limited space in the coronal plane. During TKA, distal coronal alignment guides the distal femoral valgus cut angle, whereas distal sagittal alignment aids in predicting femoral component positioning to avoid anterior notching. However, osteotomies along the anterior cortical bone intended to prevent notching may result in outliers due to differences between the distal sagittal alignment and the distal anterior cortical axis.
Gender differences in morphometry of resected proximal tibia surface in total knee arthroplasty for osteoarthritic knees

Abstract:[Objective] To investigate the morphologic features of resected proximal tibia surface in total knee arthroplasty (TKA) for osteoarthritic knees and find out the differences between the men and women.[Method] A total of 118 osteoarthritic knees (58 from men,58 from women) received computed tomography measurement for the proximal tibial mediolateral (ML),middle anteroposterior (AP),medial anteroposterior (MAP),lateral anteroposterior (LAP) dimensions and ML/AP ratio.Then the ML/AP was divided into 3 groups based on the different AP dimensions (＜48 mm,48-52mm,＞52mm) to find out the differences of resected proximal tibia surface between men and women.[Result] Significant differences were found between the men and women in terms of ML,AP,MAP and LAP dimensions (P＜0.01).As increasing of AP dimension,the ML/AP ratio decreased,with a negative correlation between them (P＜0.005).In the same AP dimension condition,a higher ML/AP ratio was noted in men and a lower ML/AP ratio in women.The results demonstrated that under a given AP dimension prosthesis,the tibial ML dimension could be undersized in men,conversely overhang in women.[Conclusion] Proximal tibia showed significant differences in the size and shape between men and women.The results of this study can provide important reference in designing proper gender-specific tibia prosthesis with different ML/AP aspect ratio in clinics.

Abstract:A systematic review of the literature on horseback riding therapy as an intervention for children with cerebral palsy (CP) was carried out. The terms horse, riding, hippotherapy, horseback riding therapy, equine movement therapy, and cerebral palsy were searched in electronic databases and hand searched. Retrieved articles were rated for methodological quality using PEDro scoring to assess the internal validity of randomized trials and the Newcastle Ottawa Quality Assessment Scale to assess cohort studies. PICO questioning (Population, Intervention, Comparison, and Outcomes) was used to identify questions of interest to clinicians for outcomes within the context of the International Classification of Functioning, Disability and Health. Levels of evidence were then accorded each PICO question. There is Level 2a evidence that hippotherapy is effective for treating muscle symmetry in the trunk and hip and that therapeutic horseback riding is effective for improved gross motor function when compared with regular therapy or time on a waiting list. No studies addressed participation outcomes.
Morphological Measurement and Clinical Significance of Abnormal Development of Distal Femur with Hemophilia Knee Arthritis: A Consideration on the Renewal of Total Knee Prosthesis

Abstract:Objective: The knee joint of hemophiliacs may face the result of local morphological changes due to long-term irritation of synovitis. This study aims to elucidate the morphological characteristics of distal femur in hemophilic arthritis (HA) and compare the compatibility of three types of prostheses with the anteroposterior (AP) and mediolateral (ML) dimensions of the femoral osteotomy surface. Methods: This study retrospectively and randomly selected 50 patients with HA registered for treatment at our hospital from June 2016 to August 2022 as the study subjects, with an equal number of male osteoarthritis (OA) patients and healthy male individuals set as the control group. This study used medical digitalization software to simulate osteotomies on the distal femur during total knee arthroplasties (TKA) for 50 patients with HA, OA patients, and the healthy population, respectively, and measure the morphological parameters to compare with three commonly used femoral components of TKA in clinical practice. The differences between the femur resection of anteroposterior and mediolateral (FRAP, FRML) osteotomy surface and the prosthesis's BOX-AP/ML were compared in three prostheses. One-way ANOVA and multiple Kruskal-Wallis H test were used for the normal or non-normal distribution data, and pairwise comparisons between groups were conducted using the Bonferroni method, and the linear correlation analysis was utilized to assess the relationship between section femoral morphological data and prosthesis parameters. Result: In HA patients, the morphological characteristics of the distal femur were shown as shorter than femur AP (FAP), medial and lateral condyle anterior-posterior dimension (FMCAP, FLCAP), notch width (NW), posterolateral condyle height (PLCH), posteromedial condyle width (PMCW), and posterior condylar axis length (PCAL) dimension. They had comparatively smaller femur section aspect ratios (p < 0.005). They showed longer posterolateral condyle width (PLCW), anterior condyle mediolateral dimension (FRACML), anterolateral condyle height (ALCH), and femur resection anterior condylar mediolateral (FRACML) dimension (p < 0.005). They showed larger distal femur aspect ratio and resection aspect ratio (FAR, FRAR, p < 0.005). All selected prostheses showed ML undercoverage under similar AP dimensions, and ML undersizing of Attune systems was more obvious in three femoral prostheses. Conclusion: The distal femur morphological change of HA patient is shown as smaller AP dimension, narrow posterior condyle spacing, lower and shallower trochlear, thinner anterior condyle, wider and lower intercondylar notch and higher posterior-lateral condyle. The selected prostheses showed ML undercoverage under similar AP dimensions. This typical morphological tendency of the distal femur seems to warrant consideration in the process of knee joint prosthesis upgrading.
Relationship between anatomy of knee collateral ligament and geometry of posterior femoral condylar articular surface]

Abstract:To analyze the relationship between the collateral ligament attachment and the epicondylar axis with rotational alignment of the femoral component in the total knee arthroplasty(TKA).Twenty normal cadaver knee joints were anatomized and 2 holes were drilled on the distal femur from the deep and superficial insertions of the medial collateral ligaments to the lateral condylar part, respectively. Then, all the knees were scanned by MRI on the sagittal plane, making the drilled hole located relatively to the posterior condylar joint surface on the axial plane, and the posterior condylar angle (PCA) and the condylar twist angle (CTA) were measured.The collateral ligament had the deep and superficial parts, and the deep part was strained during the knee flexing. PCA and CTA were 4.50 +/- 1.26 degrees and 7.10 +/- 0.30 degrees respectively, and there was a significant difference between them (P < 0.05), which were significantly greater than those reported abroad. On the sagittal plane, there was no significant difference between the radius of the posterior medial and lateral condylar circles (P > 0.05). The distance from the center of the posterior condylar circle to the deep insertion of the medial collateral ligament (MCL) (dl) was 4.22 +/- 0.20 mm, and the distance to the superficial insertion of MCL (d2) was 7.36 +/- 0.13 mm. The difference between dl and d2 was significant (P < 0.05).The center of the posterior condylar circle passes from the deep insertion of the collateral ligament, which can be regarded as a fixed flexion-extension axis of the knee. By releasing the different parts of the collateral ligaments, the balance of the flexion and extension gap can be obtained, and then varus, valgus or flexed contracture deformity of the knee can be realigned. Besides, the rotational orientation of the femoral prosthesis can be made by a reference to the epicondylar insertion of the collateral ligament.
Quantitative Analysis of Rotational Alignment after Total Knee Arthroplasty in Osteoarthritis

Abstract:Objective Rotational malalignment in total knee arthroplasty(TKA) may be the main cause of patellofemoral complications and the determining factor of postoperative function and survival rate of prosthesis.We measured rotational alignment of femoral and tibial component relative to different landmarks on CT scan images during follow-up.Methods 21 osteoarthritic patients with 35 varus knees were followed up at 7 years after TKA with posterior stabilized prothesis.The posterior femoral condyles had been used as a reference for femoral component positioning,and the tibial component was positioned to maximize tibial coverage and the posterior condyles of the cut proximal tibial surface.The patients were evaluated with physical examination,KSS score,radiological assessment and CT scan.Rotational alignment of femoral and tibial component was measured on CT images with reference to SEA(surgical epicondyle axis)and medial 1/3 of tibial tubercle.Merchant view was used to measure patella tilt angle and shift.Results Rotational angles of femoral and tibial component were in normal distribution.Femoral component rotation was 3.02±3.88° relative to SEA;tibial component rotation was-17° relative to the medial 1/3 of tibial tubercle and 0o relative to SEA.The combined rotation was 2.99±7.90(relative to SEA and-2.64±8.39(relative to the medial 1/3 of tibial tubercle;the rotational mismatch was within 10o.Conclusion Compared with the medial 1/3 of tibial tubercle,the surgical epicondylar axis of femur was a more reliable landmark for the quantitative analysis of rotational alignment after total knee arthroplasty on CT image.The maximal tibial coverage and the posterior condyles of the cut proximal tibial surface could be used to determine tibial component rotation,although still with great variability.

Abstract:OBJECTIVETo investigate the rotational mismatch of total knee replacement with medial 1/3 of tibial tuberosity as bony landmark in osteoarthritic patients with varus or valgus deformity.METHODSAxial images on computed tomography of 62 knees (including 55 varus deformities and 7 valgus deformities) in 32 Chinese osteoarthritic patients who had total knee arthroplasty were analyzed, compared with that of 10 healthy knees. On images of the distal femur, the angle between the lines of surgical epicondylar axis (SEA) and posterior condylar axis was measured as posterior condylar angle (PCA), and on images of the proximal tibia, a baseline for the anteriorposterior axis of each component was drawn based on the SEA for the femur and the medial 1/3 of the tibial tuberosity for the tibia. The angle between these lines (Angle a) was defined as the rotational mismatch between the components when they were aligned to the anatomic landmarks of each bone.RESULTSThe sulcus of medial epicondyle of femur could be identified on CT images of over 80% osteoarthritic knees; the median value of PCA was +2.36 degrees, with an individual variation of 0 degree to +7.5 degrees. Angle a was +6.45 +/- 3.68 degrees (range, 0 degree to +11.8 degrees) in 10 healthy knees, which increased significantly to +10.85 +/- 10.47 degrees (range, 0 degree to +28.1 degrees) in 55 varus knees (P < 0.05), which also increased significantly to +11.6 +/- 7.3 degrees (range, -6.5 degrees to +26.8 degrees) in 7 valgus knees (P < 0.05).CONCLUSIONWith the medial 1/3 of the tibial tuberosity as the rotational landmark for the tibial component, there was a tendency to align the tibial component in external rotational position relative to the femoral component in knees with normal alignment, the rotational mismatch increased in Chinese osteoarthritic knees with varus and valgus deformity.

Abstract:Background: Coronal alignment is considered key to the function and longevity of a TKA. However, most studies do not consider femoral and tibial anatomical features such as lateral femoral bowing and the effects of these features and subsequent alignment on function after TKA are unclear. Questions/purposes: We therefore determined (1) the prevalence of lateral femoral bowing, varus femoral condylar orientation, and severe tibia plateau inclination in female Koreans undergoing TKA; (2) whether postoperative alignments are affected by these anatomical features and improved by the use of navigation; and (3) whether postoperative coronal alignments are associated with function. Methods: We measured alignment in 367 knees that underwent TKA and 60 sex- and age-matched normal knees (control group). We determined patterns and degrees of femoral bowing angle, femoral condylar orientation, and tibial plateau inclination on preoperative full-limb radiographs. Postoperatively, coronal alignment of limbs and of femoral and tibial components was measured. We compared American Knee Society scores, WOMAC scores, and SF-36 scores in aligned knees and outliers (beyond ± 3° or ± 2°) at 1 year. Results: The prevalence of lateral femoral bowing was 88% in the TKA group and 77% in the control group. Mean femoral condylar orientation angle was varus 2.6° in the TKA group and valgus 1.1° in the control group, and mean tibial plateau inclination was varus 8.3° in the TKA group and varus 5.4° in the control group. Femoral lateral bowing and varus femoral condylar orientation were associated with postoperative alignments. Several clinical outcome scales were inferior in the outliers in mechanical tibiofemoral angle, anatomical tibiofemoral angle, and tibial coronal alignment but not in femoral coronal alignment outliers. Conclusions: Lateral femoral bowing, varus condylar orientation, and severe varus inclination of the tibia plateau should be considered when performing TKA in Korean patients or patients with otherwise similar anatomical features.
Comprehensive Assessment of Patellar Morphology Using Computed Tomography-Based Three-Dimensional Computer Models.

Abstract:BACKGROUND:The objectives of this study were to assess patellar morphology using computed tomography-based three-dimensional computer models and to determine the utility of the subchondral method for patellar resection in total knee arthroplasty (TKA).METHODS:Overall, 120 subjects (60 males, 60 females) from 20 to 50 years old were included. Computer tomography images were used to construct 3D computer models of 240 knees. An anthropometric analysis of variance was used with the models to detect differences between the sides, age groups, and genders.RESULTS:Based on the available numbers, there were significant gender differences in the patellar length, width, and thickness even with strict control for the height and weight of the patients. The average patellar width/thickness ratio was 1.95, regardless of gender, and there was a good correlation between the patellar width and thickness (male: r=0.68, p<0.01; female: r=0.75, p<0.01). After using the subchondral method to virtually resect the patellae, the mean thickness for males and female was 14.0mm and 12.3mm, respectively. The proportion of the residual bony thickness that was less than 12 mm was 5.83% (seven cases) in the male group and up to 42.5% (51 cases) in the female group. There was excellent intra-observer reliability and inter-observer reliability regarding the dimensional measurements in this study.CONCLUSIONS:A robust method of measuring and virtually resecting the patella was established. The anthropometric dimensions of this study could provide basic data for guiding surgical management of the patella in TKA and are useful in designing patellar implants.CLINICAL RELEVANCE:The anthropometric patellar information is essential for surgeons to perform patellar resurfacing in TKA.
Influence of the angle between tibial and prosthesis mechanical axes on tibial bone remodeling in total knee arthroplasty

Abstract:Proceedings of the Institution of Mechanical Engineers, Part H: Journal of Engineering in Medicine, Ahead of Print. Osteoarthritis of the knee is one of the most common diseases that affect the quality of life in the elderly population, and Total Knee Arthroplasty is considered the only real treatment for it, and as with any other surgery, a suboptimal technique may lead to an undesirable outcome. This paper aims to investigate the effects of the angle between mechanical axes of the tibia and the implant on the bone remodeling process. A 3D model was reconstructed using CT images, which was then used in an ABAQUS model with a USDFLD subroutine to simulate bone remodeling post TKA. The USDFLD subroutine compares the strain energy density from each increment to that of the previous increment to determine how the bone density will change. Simulation results suggest that when the prosthesis is inclined to one side, stress and density distribution increase, whereas stress and bone density decrease substantially on the opposite side. This decrease in bone density can be as much as 35% in the coronal plane. Sagittal malalignment results suggest that the effect would be relatively localized to the vicinity of the cutting plane. Results suggest uniform load distribution may be achieved when the two mechanical axes are kept parallel, which in turn can lead to decreased prosthesis loosening and bone fractures.
