Abstract:Background: The objectives of this study were to develop a 3D convolutional deep learning framework (CarotidNet) for fully automatic segmentation of carotid bifurcations in computed tomography angiography (CTA) images and to facilitate the quantification of carotid stenosis and risk assessment of stroke. Methods: Our pipeline was a two-stage cascade network that included a localization phase and a segmentation phase. The network framework was based on the 3D version of U-Net, but was refined in three ways: (I) by adding residual connections and a deep supervision strategy to cope with the vanishing problem in back-propagation; (II) by adopting dilated convolution in order to strengthen the capacity to capture contextual information; and (III) by establishing a hybrid objective function to address the extreme imbalance between foreground and background voxels. Results: We trained our networks on 15 cases and evaluated their performance based on 41 cases from the MICCAI Challenge 2009 dataset. A Dice similarity coefficient of 82.3% was achieved for the test cases. Conclusions: We developed a carotid segmentation method based on U-Net that can segment tiny carotid bifurcation lumens from very large backgrounds with no manual intervention. This was the first attempt to use deep learning to achieve carotid bifurcation segmentation in 3D CTA images. Our results indicate that deep learning is a promising method for automatically extracting carotid bifurcation lumens.

Abstract:Background and purpose: Anatomical labeling of the cerebral vasculature is a crucial topic in determining the morphological nature and characterizing the vital variations of vessels, yet precise labeling of the intracranial arteries is time-consuming and challenging, given anatomical structural variability and surging imaging data. We present a U-Net-based deep learning (DL) model to automatically label detailed anatomical segments in computed tomography angiography (CTA) for the first time. The trained DL algorithm was further tested on a clinically relevant set for the localization of intracranial aneurysms (IAs). Methods: 457 examinations with varying degrees of arterial stenosis were used to train, validate, and test the model, aiming to automatically label 42 segments of the intracranial arteries [e.g., 7 segments of the internal carotid artery (ICA)]. Evaluation metrics included Dice similarity coefficient (DSC), mean surface distance (MSD), and Hausdorff distance (HD). Additionally, 96 examinations containing at least one IA were enrolled to assess the model's potential in enhancing clinicians' precision in IA localization. A total of 5 clinicians with different experience levels participated as readers in the clinical experiment and identified the precise location of IA without and with algorithm assistance, where there was a washout period of 14 days between two interpretations. The diagnostic accuracy, time, and mean interrater agreement (Fleiss' Kappa) were calculated to assess the differences in clinical performance of clinicians. Results: The proposed model exhibited notable labeling performance on 42 segments that included 7 anatomical segments of ICA, with the mean DSC of 0.88, MSD of 0.82 mm and HD of 6.59 mm. Furthermore, the model demonstrated superior labeling performance in healthy subjects compared to patients with stenosis (DSC: 0.91 vs. 0.89, p < 0.05; HD: 4.75 vs. 6.19, p < 0.05). Concurrently, clinicians with model predictions achieved significant improvements when interpreting the precise location of IA. The clinicians' mean accuracy increased by 0.04 ( p = 0.003), mean time to diagnosis reduced by 9.76 s ( p < 0.001), and mean interrater agreement (Fleiss' Kappa) increased by 0.07 ( p = 0.029). Conclusion: Our model stands proficient for labeling intracranial arteries using the largest CTA dataset. Crucially, it demonstrates clinical utility, helping prioritize the patients with high risks and ease clinical workload.

Abstract:Time-of-flight (TOF) Magnetic Resonance Angiography (MRA) is a useful imaging technique which reflects blood flow and vasculature information. However, due to the low signal and contrast of arteries in TOF MRA, it is challenging to extract vascular features such as length, volume and tortuosity, through segmentation and tracing. Hence, in this paper, a simultaneous artery tracing and segmentation method is proposed to a generate quantitative intracranial vasculature map from TOF MRA. Instead of using original images, segmentation from a neural network model is used to initiate tracing, avoiding the low signal or contrast for small arteries. A tracing method is proposed based on cross-sectional best matching, followed by an optimization scheme from the multiplanar reformatted view. Centerline positions, lumen radii and centerline deviations are jointly optimized for robust tracing within artery regions. Finally, the refined artery traces are used for better artery segmentation. The method is validated on eight TOF MRAs of both healthy subjects and patients with cerebrovascular disease, showing good agreements with human supervised tracing and segmentation results for representative features such as artery length (<4% mean absolute difference), volume (>0.80 Dice), and tortuosity (<3% mean absolute difference). Our method out-performs three other popular tracing and segmentation methods by a large margin.

Abstract:The accurate segmentation of cerebral vessels from time-of-flight magnetic resonance angiography (TOF-MRA) data is crucial for the diagnosis and treatment of cerebrovascular diseases. However, cerebrovascular segmentation remains challenging due to the tiny and complex structure of the vessels. This paper proposed a GAN-based deep-based method that maps the TOF-MRA data to 3D cerebral vessels. To improve the capability of feature expression, the proposed network integrates squeeze-and-excite blocks into a Vnet structure to extract and accumulate features at different scales. In addition, we introduce adversarial loss for network training, which can help the model to learn the distribution of the cerebral vessels. Besides, we introduce island number and minimum diameter to evaluate the spatial consistency and recognized minimal vessel of the cerebrovascular segmentation results. The proposed method is validated on 102 TOF-MRA cases with manually annotated labels. The experimental results indicate that our method can identify more tiny cerebral vessels meanwhile maintaining the spatial consistency of the cerebral vessel structure. Besides, our GAN-based cerebrovascular segmentation method reaches an average dice similar coefficient of 89.89%, outperforming other state-of-the-art segmentation methods.

Abstract:Circular dichroism (CD) and immunochemical measurements have been used to examine conformational properties of COOH-terminal fragments 121-316, 206-316 and 225(226)-316 of thermolysin, and to compare these properties to those of native thermolysin and thermolysin S, the stable partially active two-fragment complex composed of fragments 5-224(225) and 225(226)-316. In aqueous solution at neutral pH, all the COOH-terminal fragments attain a native-like conformation, as judged both by the content of secondary structure deduced from far-ultraviolet CD spectra and by the recognition of rabbit polyclonal antibodies specific for the COOH-terminal region in native thermolysin. The three fragments showed reversible cooperative unfolding transitions mediated by both heat and guanidine hydrochloride (Gdn X HCl). The phase transition curves were analyzed for Tm (temperature of half-denaturation) and Gibbs free energies (delta GD) of unfolding from native to denatured state. The observed order of thermal stability is 225(226)-316 less than or equal to 206-316 less than 121-316 less than thermolysin S less than thermolysin. The ranking of delta GD values for the three fragments correlates with the size of each fragment. Competitive binding studies by radioimmunoassay using 14C-labeled thermolysin and affinity purified antibodies specific for native antigenic determinants in segment 206-316 of native thermolysin indicate that the COOH-terminal fragments adopt native-like conformations which are in equilibrium with non-native conformations. These equilibria are shifted towards the native state as the fragment size increases from 225(226)-316, to 206-316, to 121-316. Fragment 225(226)-316, when combined with fragment 5-224(225) in the thermolysin S complex, adopts a more stable native-like conformation and becomes much more antigenic. It has been shown that the degree of antigenicity of COOH-terminal fragments towards thermolysin antibodies correlates directly with their conformational stability. The results of this study are discussed in relation to the recently proposed correlation between antigenicity and segmental mobility of globular proteins.

Abstract:INTRODUCTION: Computed tomography angiography (CTA) is the most widely used imaging modality for intracranial aneurysm (IA) management, yet it remains inferior to digital subtraction angiography (DSA) for IA detection, particularly of small IAs in the cavernous carotid region. METHODS: Using 50 paired CTA-DSA images, we trained (n = 27), validated (n = 3) and tested (n = 20) a deep-learning model (3D-DeepMedic) for cerebrovasculature segmentation from CTA. A landmark-based co-registration algorithm was used for registration and up-sampling of CTAs to paired DSAs. Segmented vessels from DSAs were used as the ground-truth. Accuracy of the model was evaluated using conventional metrics (dice similarity coefficient-DSC) and vessel-specific metrics, like connectivity-area-length (CAL). On the test cases (20 IAs), 3 experts detected and segmented IAs. For each rater, we recorded the rate of IA detection. For detected IAs, raters segmented IAs and calculated morphology parameters to quantify the differences in IA segmentation by raters to DeepMedic. The rater agreements were assessed using Krippendorf’s alpha. RESULTS: In testing, the DeepMedic model yielded a DSC and CAL of 0.971 ± 0.007 and 0.868 ± 0.008, respectively. The model delineated all IAs with errors <10% for all IA morphometrics. Conversely, average IA detection accuracy from raters was 0.653 (undetected IAs were present to a significantly greater degree on the ICA cavernous region and were significantly smaller). Error rates for IA morphometrics in rater-segmentations were significantly higher than in DeepMedic-segmentations, particularly for neck (p = 0.003), and surface-area (p = 0.003). For IA morphology, agreement between the raters was acceptable for metrics except UI (α=0.36) and NSI (α=0.69). The agreement between DeepMedic and ground truth was consistently higher. CONCLUSIONS: Our CTA segmentation algorithm provides a high-fidelity solution for CTA vessel segmentation, particularly for vessels and IAs in the carotid cavernous region.

Abstract:Time-of-flight magnetic resonance angiography (TOF-MRA) is a non-contrast technique used to visualize neurovascular. However, manual reconstruction of the volume render (VR) by radiologists is time-consuming and labor-intensive. Deep learning-based (DL-based) vessel segmentation technology may provide intelligent automation workflow. To evaluate the image quality of DL vessel segmentation for automatically acquiring intracranial arteries in TOF-MRA. A total of 394 TOF-MRA scans were selected, which included cerebral vascular health, aneurysms, or stenoses. Both our proposed method and two state-of-the-art DL methods are evaluated on external datasets for generalization ability. For qualitative assessment, two experienced clinical radiologists evaluated the image quality of cerebrovascular diagnostic and visualization (scoring 0-5 as unacceptable to excellent) obtained by manual VR reconstruction or automatic convolutional neural network (CNN) segmentation. The proposed CNN outperforms the other two DL-based methods in clinical scoring on external datasets, and its visualization was evaluated by readers as having the appearance of the radiologists' manual reconstructions. Scoring of proposed CNN and VR of intracranial arteries demonstrated good to excellent agreement with no significant differences (median, 5.0 and 5.0, P ≥ 12) at healthy-type scans. All proposed CNN image quality were considered to have adequate diagnostic quality (median scores > 2). Quantitative analysis demonstrated a superior dice similarity coefficient of cerebrovascular overlap (training sets and validation sets; 0.947 and 0.927). Automatic cerebrovascular segmentation using DL is feasible and the image quality in terms of vessel integrity, collateral circulation and lesion morphology is comparable to expert manual VR without significant differences.

Abstract:Intracranial arteries are critical blood vessels that supply the brain with oxygenated blood. Intracranial artery labels provide valuable guidance and navigation to numerous clinical applications and disease diagnoses. Various machine learning algorithms have been carried out for automation in the anatomical labeling of cerebral arteries. However, the task remains challenging because of the high complexity and variations of intracranial arteries. This study investigates a novel graph convolutional neural network with deep feature fusion for cerebral artery labeling. We introduce stacked graph convolutions in an encoder-core-decoder architecture, extracting high-level representations from graph nodes and their neighbors. Furthermore, we efficiently aggregate intermediate features from different hierarchies to enhance the proposed model's representation capability and labeling performance. We perform extensive experiments on public datasets, in which the results prove the superiority of our approach over baselines by a clear margin.

For each rater, the authors recorded the rate of IA detection, and for detected IAs, raters segmented and calculated important IA morphology parameters to quantify the differences in IA segmentation by raters to segmentations by DeepMedic. The agreement between raters, DeepMedic, and ground truth was assessed using Krippendorf's alpha. RESULTS In testing, the DeepMedic model yielded a CAL of 0.971 ± 0.007 and a DSC of 0.868 ± 0.008. The model prediction delineated all IAs and resulted in average error rates of < 10% for all IA morphometrics. Agreement between DeepMedic and ground truth was consistently higher compared with that between expert raters and ground truth. CONCLUSIONS This CTA segmentation network (DeepMedic trained on DSA-segmented vessels) provides a high-fidelity solution for CTA vessel segmentation, particularly for vessels and IAs in the carotid cavernous region.

Abstract:Radiological imaging to examine intracranial blood vessels is critical for preoperative planning and postoperative follow-up. Automated segmentation of cerebrovascular anatomy from Time-Of-Flight Magnetic Resonance Angiography (TOF-MRA) can provide radiologists with a more detailed and precise view of these vessels. This paper introduces a domain generalized artificial intelligence (AI) solution for volumetric monitoring of cerebrovascular structures from multi-center MRAs. Our approach utilizes a multi-task deep convolutional neural network (CNN) with a topology-aware loss function to learn voxel-wise segmentation of the cerebrovascular tree. We use Decorrelation Loss to achieve domain regularization for the encoder network and auxiliary tasks to provide additional regularization and enable the encoder to learn higher-level intermediate representations for improved performance. We compare our method to six state-of-the-art 3D vessel segmentation methods using retrospective TOF-MRA datasets from multiple private and public data sources scanned at six hospitals, with and without vascular pathologies. The proposed model achieved the best scores in all the qualitative performance measures. Furthermore, we have developed an AI-assisted Graphical User Interface (GUI) based on our research to assist radiologists in their daily work and establish a more efficient work process that saves time.

An algorithm for the labeling and interactive visualization of the cerebrovascular system of ischemic strokes

Abstract:During the diagnosis of ischemic strokes, the Circle of Willis and its surrounding vessels are the arteries of interest. Their visualization in case of an acute stroke is often enabled by Computed Tomography Angiography (CTA). Still, the identification and analysis of the cerebral arteries remain time consuming in such scans due to a large number of peripheral vessels which may disturb the visual impression. We propose VirtualDSA++, an algorithm designed to segment and label the cerebrovascular tree on CTA scans. Especially with stroke patients, labeling is a delicate procedure, as in the worst case whole hemispheres may not be present due to impeded perfusion. Hence, we extended the labeling mechanism for the cerebral arteries to identify occluded vessels. In the work at hand, we place the algorithm in a clinical context by evaluating the labeling and occlusion detection on stroke patients, where we have achieved labeling sensitivities comparable to other works between 92% and 95%. To the best of our knowledge, ours is the first work to address labeling and occlusion detection at once, whereby a sensitivity of 67% and a specificity of 81% were obtained for the latter. VirtualDSA++ also automatically segments and models the intracranial system leading to further processing possibilities. We present the generic concept of iterative systematic search for pathways on all nodes of said model, which enables new interactive features. Exemplary, we derive in detail, firstly, the interactive planning of vascular interventions like the mechanical thrombectomy and secondly, the interactive suppression of vessel structures that are not of interest in diagnosing strokes (like veins). We discuss both features as well as further possibilities emerging from the proposed concept.

[The impact of precursors on reading, spelling, and arithmetic at school 2nd grade].

Abstract:AIM OF THE STUDY The aim of this study was to determine the role of precursors in the prediction of school achievement. 372 children were followed from health examination for school entry to the second grade. METHODS Preschool skills assessed by a screening of developmental stage (SOPESS) were related to second-grade reading (ELFE 1-6), spelling (DERET 1-2+), and arithmetic (DEMAT 1+) by correlation and regression analyses. RESULTS Correlations between numeracy and math abilities (r=0,340) met with our expectations as also did those between verbal abilities and spelling (r=0,276). The subtests of attention and counting (SOPESS) contribute significantly to an explanation of the variance in school achievement. Numeracy predicts math achievement, and verbal memory contributes to school achievement in math and spelling. CONCLUSIONS Our findings support the contribution of visual selective attention, phonological memory, and verbal abilities to the development of reading, spelling, and math at primary school.
Towards Automated Brain Aneurysm Detection in TOF-MRA: Open Data, Weak Labels, and Anatomical Knowledge

Abstract:Brain aneurysm detection in Time-Of-Flight Magnetic Resonance Angiography (TOF-MRA) has undergone drastic improvements with the advent of Deep Learning (DL). However, performances of supervised DL models heavily rely on the quantity of labeled samples, which are extremely costly to obtain. Here, we present a DL model for aneurysm detection that overcomes the issue with "weak" labels: oversized annotations which are considerably faster to create. Our weak labels resulted to be four times faster to generate than their voxel-wise counterparts. In addition, our model leverages prior anatomical knowledge by focusing only on plausible locations for aneurysm occurrence. We first train and evaluate our model through cross-validation on an in-house TOF-MRA dataset comprising 284 subjects (170 females / 127 healthy controls / 157 patients with 198 aneurysms). On this dataset, our best model achieved a sensitivity of 83%, with False Positive (FP) rate of 0.8 per patient. To assess model generalizability, we then participated in a challenge for aneurysm detection with TOF-MRA data (93 patients, 20 controls, 125 aneurysms). On the public challenge, sensitivity was 68% (FP rate = 2.5), ranking 4th/18 on the open leaderboard. We found no significant difference in sensitivity between aneurysm risk-of-rupture groups ( p = 0.75), locations ( p = 0.72), or sizes ( p = 0.15). Data, code and model weights are released under permissive licenses. We demonstrate that weak labels and anatomical knowledge can alleviate the necessity for prohibitively expensive voxel-wise annotations.

This study aims to explore the potential of the emerging four-dimensional arterial spin labeling magnetic resonance angiography (4D ASL-MRA) technique for fast and accurate cerebrovascular segmentation with a simple machine-learning approach. Nine temporal features were extracted from the intensity-time signal of each voxel, and eight spatial features from the neighboring voxels. Then, the unsupervised outlier detection algorithm, i.e. Isolation Forest, is used for segmentation of the vascular voxels based on the extracted features. The total length of the centerlines of the intracranial arterial vasculature, the dice similarity coefficient (DSC), and the average Hausdorff Distance (AVGHD) on the cross-sections of small- to large-sized vessels were calculated to evaluate the performance of the segmentation approach on 4D ASL-MRA of 18 subjects. Experiments show that the temporal information on 4D ASL-MRA can largely improve the segmentation performance. In addition, the proposed segmentation approach outperforms the traditional methods that were performed on the 3D image (i.e. the temporal average intensity projection of 4D ASL-MRA) and the previously proposed frame-wise approach. In conclusion, this study demonstrates that accurate and robust segmentation of cerebral vasculature is achievable on 4D ASL-MRA by using a simple machine-learning approach with appropriate features.
A vascular synthetic model for improved aneurysm segmentation and detection via Deep Neural Networks

Abstract:We hereby present a full synthetic model, able to mimic the various constituents of the cerebral vascular tree: the cerebral arteries, the bifurcations and the intracranial aneurysms. By building this model, our goal was to provide a substantial dataset of brain arteries which could be used by a 3D Convolutional Neural Network (CNN) to either segment or detect/recognize various vascular diseases (such as artery dissection/thrombosis) or even some portions of the cerebral vasculature, such as the bifurcations or aneurysms. In this study, we will particularly focus on Intra-Cranial Aneurysm (ICA) detection and segmentation. The cerebral aneurysms most often occur on a particular structure of the vascular tree named the Circle of Willis. Various studies have been conducted to detect and monitor the ICAs and those based on Deep Learning (DL) achieve the best performances. Specifically, in this work, we propose a full synthetic 3D model able to mimic the brain vasculature as acquired by Magnetic Resonance Angiography (MRA), and more particularly the Time Of Flight (TOF) principle. Among the various MRI modalities, the MRA-TOF allows to have a relatively good rendering of the blood vessels and is non-invasive (no contrast liquid injection). Our model has been designed to simultaneously mimic the arteries geometry, the ICA shape and the background noise. The geometry of the vascular tree is modeled thanks to an interpolation with 3D Spline functions, and the statistical properties of the background MRI noise is collected from MRA acquisitions and reproduced within the model. In this work, we thoroughly describe the synthetic vasculature model, we build up a neural network designed for ICA segmentation and detection, and finally, we carry out an in-depth evaluation of the performance gap gained thanks to the synthetic model data augmentation.

Abstract:Automated cerebrovascular segmentation of time-of-flight magnetic resonance angiography (TOF-MRA) images is an important technique, which can be used to diagnose abnormalities in the cerebrovascular system, such as vascular stenosis and malformation. Automated cerebrovascular segmentation can direct show the shape, direction and distribution of blood vessels. Although deep neural network (DNN)-based cerebrovascular segmentation methods have shown to yield outstanding performance, they are limited by their dependence on huge training dataset. In this paper, we propose an unsupervised cerebrovascular segmentation method of TOF-MRA images based on DNN and hidden Markov random field (HMRF) model. Our DNN-based cerebrovascular segmentation model is trained by the labeling of HMRF rather than manual annotations. The proposed method was trained and tested using 100 TOF-MRA images. The results were evaluated using the dice similarity coefficient (DSC), which reached a value of 0.79. The trained model achieved better performance than that of the traditional HMRF-based cerebrovascular segmentation method in binary pixel-classification. This paper combines the advantages of both DNN and HMRF to train the model with a not so large amount of the annotations in deep learning, which leads to a more effective cerebrovascular segmentation method.

Abstract:Segmentation of brain arterio-venous malformations (bAVMs) in 3D rotational angiographies (3DRA) is still an open problem in the literature, with high relevance for clinical practice. While deep learning models have been applied for segmenting the brain vasculature in these images, they have never been used in cases with bAVMs. This is likely caused by the difficulty to obtain sufficiently annotated data to train these approaches. In this paper we introduce a first deep learning model for blood vessel segmentation in 3DRA images of patients with bAVMs. To this end, we densely annotated 5 3DRA volumes of bAVM cases and used these to train two alternative 3DUNet-based architectures with different segmentation objectives. Our results show that the networks reach a comprehensive coverage of relevant structures for bAVM analysis, much better than what is obtained using standard methods. This is promising for achieving a better topological and morphological characterisation of the bAVM structures of interest. Furthermore, the models have the ability to segment venous structures even when missing in the ground truth labelling, which is relevant for planning interventional treatments. Ultimately, these results could be used as more reliable first initial guesses, alleviating the cumbersome task of creating manual labels.

Abstract:Background and objectivesIntracranial atherosclerotic stenosis of a major intracranial artery is the common cause of ischemic stroke. We evaluate the feasibility of using deep learning to automatically detect intracranial arterial steno-occlusive lesions from time-of-flight magnetic resonance angiography.MethodsIn a retrospective study, magnetic resonance images with radiological reports of intracranial arterial stenosis and occlusion were extracted. The images were randomly divided into a training set and a test set. The manual annotation of lesions with a bounding box labeled “moderate stenosis,” “severe stenosis,” “occlusion,” and “absence of signal” was considered as ground truth. A deep learning algorithm based on you only look once version 5 (YOLOv5) detection model was developed with the training set, and its sensitivity and positive predictive values to detect lesions were evaluated in the test set.ResultsA dataset of 200 examinations consisted of a total of 411 lesions—242 moderate stenoses, 84 severe stenoses, 70 occlusions, and 15 absence of signal. The magnetic resonance images contained 291 lesions in the training set and 120 lesions in the test set. The sensitivity and positive predictive values were 64.2 and 83.7%, respectively. The detection sensitivity in relation to the location was greatest in the internal carotid artery (86.2%).ConclusionsApplying deep learning algorithms in the automated detection of intracranial arterial steno-occlusive lesions from time-of-flight magnetic resonance angiography is feasible and has great potential.

Abstract:Abstract Three-dimensional vessel model reconstruction from patient-specific magnetic resonance angiography (MRA) images often requires some manual maneuvers. This study aimed to establish the deep learning (DL)-based method for vessel model reconstruction. Time of flight MRA of 40 patients with internal carotid artery aneurysms was prepared, and three-dimensional vessel models were constructed using the threshold and region-growing method. Using those datasets, supervised deep learning using 2D U-net was performed to reconstruct 3D vessel models. The accuracy of the DL-based vessel segmentations was assessed using 20 MRA images outside the training dataset. The dice coefficient was used as the indicator of the model accuracy, and the blood flow simulation was performed using the DL-based vessel model. The created DL model could successfully reconstruct a three-dimensional model in all 60 cases. The dice coefficient in the test dataset was 0.859. Of note, the DL-generated model proved its efficacy even for large aneurysms (> 10 mm in their diameter). The reconstructed model was feasible in performing blood flow simulation to assist clinical decision-making. Our DL-based method could successfully reconstruct a three-dimensional vessel model with moderate accuracy. Future studies are warranted to exhibit that DL-based technology can promote medical image processing. Graphical Abstract
