Abstract:Introduction: Pulsed electric field (PEF) cardiac ablation has been recently proposed as a technique to treat drug resistant atrial fibrillation by inducing cell death through irreversible electroporation (IRE). Improper PEF dosing can result in thermal damage or reversible electroporation. The lack of comprehensive and systematic studies to select PEF parameters for safe and effective IRE cardiac treatments hinders device development and regulatory decision-making. Human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) have been proposed as an alternative to animal models in the evaluation of cardiac electrophysiology safety. Methods: We developed a novel high-throughput in vitro assay to quantify the electric field threshold (EFT) for electroporation (acute effect) and cell death (long-term effect) in hiPSC-CMs. Monolayers of hiPSC-CMs were cultured in high-throughput format and exposed to clinically relevant biphasic PEF treatments. Electroporation and cell death areas were identified using fluorescent probes and confocal microscopy; electroporation and cell death EFTs were quantified by comparison of fluorescent images with electric field numerical simulations. Results: Study results confirmed that PEF induces electroporation and cell death in hiPSC-CMs, dependent on the number of pulses and the amplitude, duration, and repetition frequency. In addition, PEF-induced temperature increase, absorbed dose, and total treatment time for each PEF parameter combination are reported. Discussion: Upon verification of the translatability of the in vitro results presented here to in vivo models, this novel hiPSC-CM-based assay could be used as an alternative to animal or human studies and can assist in early nonclinical device development, as well as inform regulatory decision-making for cardiac ablation medical devices.

Abstract:Background: Pulsed-field ablation (PFA) of atrial fibrillation (AF) is a new method in clinical practice. Despite a favorable safety profile of PFA in AF ablation, rare cases of renal failure, probably due to hemolysis, have been recently reported. Objective: The aim of this study was to determine the rate of hemolysis and cardiac cell death during in vitro PFA with different electric field intensities. Methods: Blood samples from healthy volunteers and mouse HL-1 cardiomyocyte cell lines were subjected to in vitro irreversible electroporation (IRE) using 216 bipolar pulses, each lasting 2 μs with 5 μs intervals, repeated 20 times at a frequency of 1 Hz. These pulses varied in from 500 to 1500 V. Cell-free hemoglobin levels were assessed spectrophotometrically, and red blood cell microparticles (RBCμ) were evaluated using flow cytometry. Cardiomyocyte death was quantified using propidium iodide. Results: PF energy (1000 V/cm, 1250 V/cm, and 1500 V/cm) was associated with a significant increase in cell-free hemoglobin (0.31 ± 0.16 g/l, 2.33 ± 0.90 g/l, and 5.7 ± 0.20 g/l, p< 0.05), and similar increase in the concentration of RBCμ. Significant rates of cardiomyocyte death were observed at electric field strengths of 750 V/cm, 1000 V/cm, 1250 V/cm and 1500 V/cm (26.5 ± 5.9%, 44.3 ± 6.2%, 55.5 ± 6.9% and 74.5 ± 17.8% of cardiomyocytes, p < 0.05). Conclusion: The most effective induction of cell death in vitro was observed at 1500 V/cm. This intensity was also associated with a significant degree of hemolysis.

Abstract:Cardiac ablation with irreversible electroporation (IRE) is quickly being established as a modality of choice for atrial fibrillation treatment. While it has not yet been optimised, IRE has the potential to significantly limit collateral damage and improve cell-specific targeting associated with other energy sources. However, more tissue and cell-specific evidence is required to demonstrate the selective threshold parameters for human cells. The aim here is to determine the optimal ablation threshold parameters related to lesion size for human cardiomyocytes in 2D culture. Conventional biphasic pulses of different field strengths and on-times were delivered in a monolayer culture system of human AC16 cardiomyocytes. The dynamics of cell death and lesion dimensions were examined at different time points. Human cardiomyocytes are susceptible to significant electroporation and cell death at a field strength of 750 V/cm or higher with 100 μs pulses. Increasing the IRE on-time from 3 ms to 60 ms reduces the effective field threshold to 250 V/cm. Using very short pulses of 2 μs and 5 μs also causes significant cell death, but only at fields higher than 1000 V/cm. A longer on-time results in more cell death and induced greater lesion area in 2D models. In addition, different forms of cell death are predicted based on the evolution of cell death over time. This study presents important findings on the ability of different IRE parameters to induce human cardiomyocyte cell death. Lesion size can be tuned by appropriate choice of IRE parameters and cardiomyocytes display an upregulation of delayed cell death 24 h after electroporation, which is an important consideration for clinical practice.

Tunable high-efficiency light absorption of monolayer graphene via Tamm plasmon polaritons.

Abstract:Highly efficient absorption of light at telecommunication wavelengths in monolayer graphene is numerically and theoretically investigated in a multi-layer photonic configuration, which consists of an ultrathin metal film coated on a dielectric Bragg mirror and a graphene sheet in a silica spacer. Our results show that the absolute absorption of light in the single-atomic-layer graphene can significantly reach 80% (34.8-fold enhancement compared to the intrinsic value of 2.3%), due to the strong field confinement of Tamm plasmon polaritons in the silica spacer. Moreover, we find that the absorption efficiency of graphene is critically dependent on the position of the graphene in the spacer. The operating wavelength is flexibly tunable by adjusting the incident angle of light and the thicknesses of metal and Bragg mirror layers. The meaningful improvement and tunability of graphene absorption may find favorable applications for the realization of high-performance graphene optoelectronic devices, such as detectors and modulators.

Abstract:Introduction: Pulsed Field Ablation (PFA) is a novel non-thermal method for cardiac ablation, relying on irreversible electroporation induced by high-energy pulsed electric fields (PEFs) to create localized lesions in the heart atria. A significant challenge in optimizing PFA treatments is determining the lethal electric field threshold (EFT), which governs ablation volume and varies with PEF waveform parameters. However, the proprietary nature of device developer's waveform characteristics and the lack of standardized nonclinical testing methods have left optimal EFTs for cardiac ablation uncertain. Methods: To address this gap, we introduced a laboratory protocol employing human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) in monolayer format to evaluate the impact of a range of clinically relevant biphasic pulse parameters on lethal EFT and adiabatic heating (AH). Cell death areas were assessed using fluorescent dyes and confocal microscopy, while lethal EFTs were quantified through comparison with electric field numerical simulations. Results and conclusion: Our study confirmed a strong correlation between cell death in hiPSC-CMs and the number and duration of pulses in each train, with pulse repetition frequency exerting a comparatively weaker influence. Fitting of these results through machine learning algorithms were used to develop an open-source online calculator. By estimating lethal EFT and associated temperature increases for diverse pulse parameter combinations, this tool, once validated, has the potential to significantly reduce reliance on animal models during early-stage device de-risking and performance assessment. This tool also offers a promising avenue for advancing PFA technology for cardiac ablation medical devices to enhance patient outcomes.

Abstract:Membrane permeabilization and thermal injury are the major cause of cell death during irreversible electroporation (IRE) performed using high electric field strength (EFS) and small number of pulses. In this study, we explored cell death under conditions of reduced EFS and prolonged pulse application, identifying the contributions of electrolysis, reactive oxygen species (ROS) and ATP loss. We performed ablations with conventional high-voltage low pulse (HV-LP) and low-voltage high pulse (LV-HP) conditions in a 3D tumor mimic, finding equivalent ablation volumes when using 2000 V/cm 90 pulses or 1000 V/cm 900 pulses respectively. These results were confirmed by performing ablations in swine liver. In LV-HP treatment, ablation volume was found to increase proportionally with pulse numbers, without the substantial temperature increase seen with HV-LP parameters. Peri-electrode pH changes, ATP loss and ROS production were seen in both conditions, but LV-HP treatments were more sensitive to blocking of these forms of cell injury. Increases in current drawn during HV-LP was not observed during LV-HP condition where the total ablation volume correlated to the charge delivered into the tissue which was greater than HV-LP treatment. LV-HP treatment provides a new paradigm in using pulsed electric fields for tissue ablation with clinically relevant volumes.

Abstract:Circulation: Arrhythmia and Electrophysiology, Ahead of Print. BACKGROUND:Electroporation is a promising nonthermal ablation method for cardiac arrhythmia treatment. Although initial clinical studies found electroporation pulsed-field ablation (PFA) both safe and efficacious, there are significant knowledge gaps concerning the mechanistic nature and electrophysiological consequences of cardiomyocyte electroporation, contributed by the paucity of suitable human in vitro models. Here, we aimed to establish and characterize a functional in vitro model based on human-induced pluripotent stem cells (hiPSCs)-derived cardiac tissue, and to study the fundamentals of cardiac PFA.METHODS:hiPSC-derived cardiomyocytes were seeded as circular cell sheets and subjected to different PFA protocols. Detailed optical mapping, cellular, and molecular characterization was performed to study PFA mechanisms and electrophysiological outcomes.Results:PFA generated electrically silenced lesions within the hiPSC-derived cardiac circular cell sheets, resulting in areas of conduction block. Both reversible and irreversible electroporation components were identified. Significant electroporation reversibility was documented within 5 to 15-minute post-PFA. Irreversibly electroporated regions persisted at 24-hour post-PFA. Per single pulse, high-frequency PFA was less efficacious than standard (monophasic) PFA, whereas increasing pulse-number augmented lesion size and diminished reversible electroporation. PFA augmentation could also be achieved by increasing extracellular Ca2+levels. Flow-cytometry experiments revealed that regulated cell death played an important role following PFA. Assessing for PFA antiarrhythmic properties, sustainable lines of electric block could be generated using PFA, which could either terminate or isolate arrhythmic activity in the hiPSC-derived cardiac circular cell sheets.CONCLUSIONS:Cardiac electroporation may be studied utilizing hiPSC-derived cardiac tissue, whereas providing novel insights into PFA temporal and electrophysiological characteristics, facilitating electroporation protocol optimization, screening for potential PFA-sensitizers, and investigating the mechanistic nature of PFA antiarrhythmic properties.

Reversible and Irreversible Effects of Electroporation on Contractility and Calcium Homeostasis in Isolated Cardiac Ventricular Myocytes

Abstract:Irreversible electroporation is an energy form utilizing high-voltage pulsed electric field, leading to cellular homeostasis disruption and cell death. Recently, irreversible electroporation has shown promising results for the treatment of cardiac arrhythmias. However, reversible and irreversible effects of pulsed electric field on cardiac myocytes remain poorly understood. Here, we evaluated the influence of a monophasic single electric pulse (EP) on the contractility, Ca 2+ homeostasis and recovery of cardiac myocytes. Isolated rat left ventricular myocytes were electroporated using single monophasic EP of different durations and voltages. Sarcomere length and intracellular Ca 2+ were simultaneously monitored for up to 20 minutes after EP application in Fura-2 loaded left ventricular myocytes. Lethal voltage thresholds were determined using 100 μs and 10 ms pulses and by discriminating cell orientation with respect to the electric field. Electroporation led to an immediate increase in intracellular Ca 2+ which was dependent upon the voltage delivered to the cell. Intermediate-voltage EP (140 V, 100 μs) increased sarcomere shortening, Ca 2+ transient amplitude, and diastolic Ca 2+ level measured 1 minute post-EP. Although sarcomere shortening returned to pre-EP level within 5 minutes, Ca 2+ transient amplitude decreased further below pre-EP level and diastolic Ca 2+ level remained elevated within 20 minutes post-EP. Spontaneous contractions were observed after sublethal EP application but their frequency decreased progressively within 20 minutes. Lethal EP voltage threshold was lower in myocytes oriented perpendicular than parallel to the electric field using 100 μs pulses while an opposite effect was found using 10 ms pulses. Sublethal EP affected rat left ventricular myocytes contractility and disrupted Ca 2+ homeostasis as a function of the EP voltage. Moreover, EP-induced lethality was preceded by a large increase in intracellular Ca 2+ and was dependent upon the EP duration, amplitude and left ventricular myocytes orientation with respect to the electric field. Microsecond Pulsed Electric Fields Induce Myocardial Ablation by Damaging Mitochondria

Abstract:Background Microsecond pulsed electric field (µsPEF) is a newer treatment modality to replace catheter ablation treatment of Atrial fibrillation (AF) due to its fewer side effects. This study aims to find out the optimal experimental parameters and the precise mechanisms for microsecond pulsed electric fields (µsPEFs) ablation of cardiomyocytes. Methods CCK8 and flow apoptosis analysis were employed to examine the effects of different µsPEFs on cardiomyocytes in vitro. The mechanisms by which the µsPEFs ablation works were explored through a combination of transcriptome study, transmission electron microscope (TEM) observation of mitochondria, pathway enrichment analysis, and interaction network analysis. In vivo experiments on mice involving HE, Masson, TUNEL and Immunofluorescence staining examinations were conducted to confirm the in vitro experimental results. Results When more than 30 pulses were applied, a continuous decline in post-ablation relative cell activity was observed, decreasing from 0.36 at 3 hours to 0.13 (p < 0.01) at 48 hours. Notably, at a voltage of 1500 V/cm and a pulse count of 50, the apoptosis rate exceeded 95%, coupled with a more stable and consistent cell ablation. The optimal parameters for µsPEFs-mediated cardiomyocytes ablation in vitro were around 1500 V/cm, 50 pulses and 100 µs in duration. Following ablation, a notable upregulation in mitochondria-related transcription levels was observed, accompanied by mitochondrial membrane disruption and an increase in Cytochrome C levels. Conclusions Within a certain range, an increase in voltage and number of electric pulses corresponded to a greater quantity of cell mortality in the ablation zone. The µsPEFs induced cell injury by impairing mitochondrial function and potentially triggering the mitochondrial apoptosis pathway.

Abstract:Pulsed electrical field (PEF) energy is a promising technique for catheter ablation of cardiac arrhythmias. In this article, the key aspects that need to be considered for safe and effective PEF delivery are reviewed, and their impact on clinical feasibility is discussed. The most important benefit of PEF appears to be the ability to kill cells through mechanisms that do not alter stromal proteins, sparing sensitive structures to improve safety, without sacrificing cardiomyocyte ablation efficacy. Many parameters affect PEF treatment outcomes, including pulse intensity, waveform shape, and number of pulses, as well as electrode configuration and geometry. These physical and electrical characteristics must be titrated carefully to balance target tissue effects with collateral implications (muscle contraction, temperature rise, risk of electrical arcing events). It is important to note that any combination of parameters affecting PEF needs to be tested for clinical efficacy and safety. Applying PEF clinically requires knowledge of the fundamentals of this technology to exploit its opportunities and generate viable, durable health improvements for patients.

Determination of lethal electric field threshold for pulsed field ablation in ex vivo perfused porcine and human hearts

Abstract:Introduction: Pulsed field ablation is an emerging modality for catheter-based cardiac ablation. The main mechanism of action is irreversible electroporation (IRE), a threshold-based phenomenon in which cells die after exposure to intense pulsed electric fields. Lethal electric field threshold for IRE is a tissue property that determines treatment feasibility and enables the development of new devices and therapeutic applications, but it is greatly dependent on the number of pulses and their duration. Methods: In the study, lesions were generated by applying IRE in porcine and human left ventricles using a pair of parallel needle electrodes at different voltages (500-1500 V) and two different pulse waveforms: a proprietary biphasic waveform (Medtronic) and monophasic 48 × 100 μs pulses. The lethal electric field threshold, anisotropy ratio, and conductivity increase by electroporation were determined by numerical modeling, comparing the model outputs with segmented lesion images. Results: The median threshold was 535 V/cm in porcine ((N = 51 lesions in n = 6 hearts) and 416 V/cm in the human donor hearts ((N = 21 lesions in n = 3 hearts) for the biphasic waveform. The median threshold value was 368 V/cm in porcine hearts ((N = 35 lesions in n = 9 hearts) cm for 48 × 100 μs pulses. Discussion: The values obtained are compared with an extensive literature review of published lethal electric field thresholds in other tissues and were found to be lower than most other tissues, except for skeletal muscle. These findings, albeit preliminary, from a limited number of hearts suggest that treatments in humans with parameters optimized in pigs should result in equal or greater lesions.

Abstract:Electroporation-based therapies are powerful biotechnological tools for enhancing the delivery of exogeneous agents or killing tissue with pulsed electric fields (PEFs). Electrochemotherapy (ECT) and gene therapy based on gene electrotransfer (EGT) both use reversible electroporation to deliver chemotherapeutics or plasmid DNA into cells, respectively. In both ECT and EGT, the goal is to permeabilize the cell membrane while maintaining high cell viability in order to facilitate drug or gene transport into the cell cytoplasm and induce a therapeutic response. Irreversible electroporation (IRE) results in cell kill due to exposure to PEFs without drugs and is under clinical evaluation for treating otherwise unresectable tumors. These PEF therapies rely mainly on the electric field distributions and do not require changes in tissue temperature for their effectiveness. However, in immediate vicinity of the electrodes the treatment may results in cell kill due to thermal damage because of the inhomogeneous electric field distribution and high current density during the electroporation-based therapies. Therefore, the main objective of this numerical study is to evaluate the influence of pulse number and electrical conductivity in the predicted cell kill zone due to irreversible electroporation and thermal damage. Specifically, we simulated a typical IRE protocol that employs ninety 100-µs PEFs. Our results confirm that it is possible to achieve predominant cell kill due to electroporation if the PEF parameters are chosen carefully. However, if either the pulse number and/or the tissue conductivity are too high, there is also potential to achieve cell kill due to thermal damage in the immediate vicinity of the electrodes. Therefore, it is critical for physicians to be mindful of placement of electrodes with respect to critical tissue structures and treatment parameters in order to maintain the non-thermal benefits of electroporation and prevent unnecessary damage to surrounding healthy tissue, critical vascular structures, and/or adjacent organs.

Abstract:Short electric field pulses represent a novel potential approach for achieving uniform electroporation within tissue containing elongated cells oriented in various directions, such as electroporation-based cardiac ablation procedures. In this study, we investigated how electroporation with nanosecond pulses with respect to different pulse shapes (unipolar, bipolar, and asymmetric) influences cardiomyocyte permeabilization and gene transfer. For this purpose, rat cardiomyocytes (H9c2) were used. The efficacy of the pulsed electric field protocols was assessed by flow cytometry and electrogene transfer by fluorescent and holotomographic microscopy. The response of the cells was assessed by the metabolic activity (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide [MTT] assay), F-actin distribution in cells by confocal microscopy, and muscle atrophy F-box (MAFbx) marker. We show nano- and microsecond pulse protocols, which are not cytotoxic for cardiac muscle cells and can be efficiently used for gene electrotransfection. Asymmetric nanosecond pulsed electric fields were similarly efficient in plasmid delivery as microsecond and millisecond protocols. However, the millisecond protocol induced a higher MAFbx expression in H9c2 cells.

Abstract:Objective The effect of nanosecond pulsed electric field（nsPEF） on cardiomyocytes apoptosis and cardiac activity was investigated in vitro and in vivo, and the mechanism was further explored by transcriptome study. Methods H9c2 cardiomyocytes were ablated by 50 times of electric shock at 0-20 kV/cm field intensity to determine the parameters that could achieve effective cell apoptosis. Then, mouse cardiomyocytes were ablated under these parameters and transcriptomic studies were conducted. Results The apoptosis rates of cardiomyocytes at 0 h, 24 h and 48 h were 62.3%, 78.8% and 80.6%, respectively, at 15 kV/cm field intensity plus 50 shocks. The mitochondrial membrane structure in the ablation area was obviously damaged, and the cardiomyocytes were induced to enter apoptosis regulation, leading to the release of cytochrome c. The total ablation rate of mouse myocardium was 76.2%. Conclusion 15 kV/cm field intensity +50 shocks can induce myocardial apoptosis by destroying mitochondria and eventually achieve necrotic apoptosis of myocardial cells in ablation area without affecting other tissues around the heart, which can be used as an effective means for ablation of atrial fibrillation.
Quantification of thermal side effects during pulsed field ablation

Abstract:Background and Aims: Pulsed field ablation (PFA) has been described as non-thermal, but abundant data exist in oncology applications, and growing data are emerging in cardiology, highlighting that thermal effects are in fact present with PFA. Our objective was to develop a reliable model of the thermal effects arising from PFA of myocardial and esophageal tissue over a range of typical peak voltage operating conditions. Methods: We developed a three-dimensional computer model of the left atrium that can quantify the thermal effects from PFA applications. Energy was applied using a bipolar configuration, and far-field and symmetry boundaries were set as electrically insulating. The model utilizes a monophasic waveform with a pulse width of 100 μs and gap between pulses of 1 s was applied for a total of 50 pulses in a single train, with variations to this easily implemented to accommodate various conditions. Results: Over a range of peak voltage operating conditions (1 kV, 1.5 kV, and 2 kV), minimal temperature rise in the esophagus was seen with 1 kV pulses (corresponding to 215 J input), but with 1.5 kV and 2 kV peak voltages (corresponding to 570 J and 1.23 kJ), temperature elevations reaching 46.3 C and > 62 C were seen, respectively. These elevations occurred after only a single pulse train of 50 pulses, implying that further elevations in temperature would be seen with subsequent applications. These findings concur with data published in other fields of medicine where pulsed field treatments are utilized. Conclusions: Thermal effects from PFA occur and can be quantified with in silico modeling. The model described here offers an efficient means of determining temperature effects from PFA over various operating conditions. Energy levels used clinically appear to have the potential to induce collateral thermal injury with repeated applications of pulsed field energy.

Electroporation and cell killing by milli- to nanosecond pulses and avoiding neuromuscular stimulation in cancer ablation

Abstract:Abstract Ablation therapies aim at eradication of tumors with minimal impact on surrounding healthy tissues. Conventional pulsed electric field (PEF) treatments cause pain and muscle contractions far beyond the ablation area. The ongoing quest is to identify PEF parameters efficient at ablation but not at stimulation. We measured electroporation and cell killing thresholds for 150 ns–1 ms PEF, uni- and bipolar, delivered in 10- to 300-pulse trains at up to 1 MHz rates. Monolayers of murine colon carcinoma cells exposed to PEF were stained with YO-PRO-1 dye to detect electroporation. In 2–4 h, dead cells were labeled with propidium. Electroporation and cell death thresholds determined by matching the stained areas to the electric field intensity were compared to nerve excitation thresholds (Kim et al. in Int J Mol Sci 22(13):7051, 2021). The minimum fourfold ratio of cell killing and stimulation thresholds was achieved with bipolar nanosecond PEF (nsPEF), a sheer benefit over a 500-fold ratio for conventional 100-μs PEF. Increasing the bipolar nsPEF frequency up to 100 kHz within 10-pulse bursts increased ablation thresholds by < 20%. Restricting such bursts to the refractory period after nerve excitation will minimize the number of neuromuscular reactions while maintaining the ablation efficiency and avoiding heating.

Abstract:Background: Pulsed field ablation, as a non-thermal ablation modality, has received increasing attention. The aim of this study is to explore whether a reversible pulsed electric field (RPEF) can temporarily inhibit electrical conduction and provide a novel method for precise ablation of arrhythmia. Methods: RPEF energy was delivered from an ablation catheter to the atrium of six dogs, followed by a series of electrogram and histology assessments. Results: RPEF ablation of ordinary myocardium resulted in an average reduction of 68.3% (range, 53.7%-83.8%) in electrogram amplitude, while 5 min later, the amplitude in eight electrograms returned to 77.9% (range, 72.4%-87.3%) of baseline. Similarly, the amplitude of the sinoatrial node electrograms reduced by an average of 73.0% (range, 60.2%-84.4%) after RPEF ablation, but recovered to 84.9% (range, 80.3%-88.5%) of baseline by 5 min. No necrotic change was detected in histopathology. Transient third-degree atrioventricular block occurred following the ablation of the maximum His potential sites with RPEF, the duration of which was voltage dependent. The histopathological results showed necrosis of the myocardium at the ablation sites but no injury to His bundle cells. Conclusions: RPEF can be applied to transiently block electrical conduction in myocardial tissues contributing to precise ablation.
