Abstract:Housing with care has become increasingly popular in recent years, and has been viewed by policy-makers and commissioners as offering a more enabling, homely and cost-effective alternative to care homes. However, most residents enter with fewer care needs than those admitted to care homes, and with different expectations. Although a previous study (Bäumker et al., 2011) compared the functional outcomes for matched groups of residents, most comparative studies have used unmatched groups. This paper will compare the social care related quality of life (SCRQoL) obtained using the Adult Social Care Outcomes Toolkit (ASCOT) for residents in housing with care and those receiving home care. The housing with care residents were drawn from the ASSET (Adult Social Services Environments and Settings) project, which was commissioned and funded by the Department of Health’s NIHR School for Social Care Research. The home care recipients were drawn from the OSCA (Outcomes for Social Care for Adults) project. Matched groups of 124 individuals in each were created using propensity score matching. Residents in housing with care had less unmet need (current SCRQoL) than people receiving home care (p<0.0001); similar expected SCRQoL in the absence of social services (p>0.05); and a higher gain (current compared with expected) (p<0.0001). The improvements in outcomes were achieved without increased costs in providing personal care. Housing with care is relatively scarce, but this study adds further evidence for the value of developing more specialised housing and increasing the choice available to older people wishing to move from unsuitable accommodation.
Tenant perspectives on the implementation of the community homes for opportunity: a focused ethnographic study in Southwestern Ontario

Abstract:Abstract Background Recovery-oriented programs provide individuals with opportunities for well-being through community integration processes that enhance the degree to which individuals could live, work, and recreate in their community. The current evaluation assessed how tenants experience their home environment after the modernization of Homes for Special Care (HSC) to Community Homes for Opportunity (CHO) in Southwest Ontario, Canada. Our study identifies existing policies and practices that could interfere with or promote the modernization process. Methods We applied ethnographic qualitative techniques to purposefully recruit 188 participants with severe mental illness from 28 group homes. Focus groups were conducted at three time points, i.e., at pre-implementation/Baseline/Time I – spring 2018; Transition/Time II – fall 2018, and Final/Time III – winter 2019. Results Study findings suggest that the transition of HSC to CHO supports activities that empower tenants towards personal growth and development. Participants were largely satisfied with the support they were getting in relation to the program-related services. Tenants disclosed that their quality of life and well-being had been enhanced through participating in the program, and that their social interaction and support for each other had also improved. Most tenants demonstrated autonomy in terms of personal and financial independence. The enhanced financial support for tenants did not only improve their quality of life, but also helped to raise their purchasing power, decision making, sense of responsibility and accountability towards healthy spending of their resources. Despite tenants’ good impression about the CHO, some still encountered problems and provided suggestions to further improve the program. Conclusion It is expected that a more effective and expanded CHO will lead to tenant empowerment and successful social integration.

Abstract:One approach to flat sensor design is to use a lenslet array to form multiple subimages of a scene and then combine the subimages to recover a fully sampled image by using a superresolution algorithm. Previously, superresolution image assembly has been based on information derived from the observed scene. For lenslet arrays, we propose a new scene-independent approach based only on known imager properties in which relative subimage shifts are accurately estimated with a calibration procedure using point source imaging. Thus, the relative resolution enhancement provided by the scene-independent superresolution algorithm is impervious to changes in subimage content, contrast, sharpness, and noise.
Subjective social isolation or loneliness in older adults residing in social housing in Ontario: a cross-sectional study

Abstract:Background: Older adults face greater risk of social isolation, but the extent of social isolation among low-income older adults living in social housing is unknown. This study aims to explore the rate of, and risk factors contributing to, subjective social isolation or loneliness among older adults in social housing. Methods: We conducted a cross-sectional study of data collected from a community program held in the common rooms of 55 social housing buildings in 14 communities across Ontario, Canada, from May 2018 to April 2019. Participants were program attendees aged 55 years and older who resided in the buildings. Program implementers assessed social isolation using the 3-Item Loneliness Scale from the University of California, Los Angeles and risk factors using common primary care screening tools. We extracted data for this study from the program database. We compared the rate of social isolation to Canadian Community Health Survey data using a 1-sample χ2 test, and evaluated associations between risk factors and social isolation using univariate and multivariate logistic regressions. Results: We included 806 residents in 30 buildings for older adults and 25 mixed-tenant buildings. Based on the 3-Item UCLA Loneliness Scale, 161 (20.0%) of the 806 participants were socially isolated. For those aged 65 and older, the rate of social isolation was nearly twice that observed in the same age group of the general population (36.1% v. 19.6%; p < 0.001). Risk factors were age (65-84 yr v. 55-64 yr adjusted odds ratio [OR] 1.99, 95% confidence interval [CI] 1.01-3.93), alcohol consumption (adjusted OR 2.45, 95% CI 1.09-5.54), anxiety or depression (adjusted OR 6.05, 95% CI 3.65-10.03) and income insecurity (adjusted OR 2.10, 95% CI 1.24-3.53). Protective factors were having at least 1 chronic cardiometabolic disease (adjusted OR 0.44, 95% CI 0.24-0.80), being physically active (adjusted OR 0.47, 95% CI 0.30-0.73) and having good to excellent general health (adjusted OR 0.60, 95% CI 0.39-0.90). Interpretation: The high rate of social isolation in low-income older adults living in social housing compared with the general population is concerning. Structural barriers could prevent engagement in social activities or maintenance of social support, especially for older adults with income insecurity and anxiety or depression; interventions are needed to reduce subjective social isolation in this population.

Abstract:The article examines how the housing tenure of older Australians, who are primarily or solely dependent on the government age pension for their income, impacts on their health. Drawing on 125 in-depth interviews with older private renters, social housing tenants and homeowners, it focuses mainly on interviewees’ narratives as to the impacts of their housing tenure on their mental health and outlook. It illustrates that security of tenure and cost of accommodation potentially has a profound impact on the psychological health of older Australians. Most of the older private renters told of being constantly stressed due to concerns about being given notice to vacate or an untenable rent increase. In contrast, the strong security of tenure and limited accommodation costs of older social housing tenants and homeowners created a foundation for a positive outlook and the capacity to lead a decent life.

Abstract:A policy of aging in place should be accompanied by physical and social environments that support healthy aging. This article describes how a property development company in Hong Kong sought to elicit the views of older people and their caregivers towards elderly services through a market research company, using questionnaire surveys followed by focus groups. Over 80% of all participants rated healthy dietary habits and exercise, maintaining mental and spiritual health, and maintaining a generally healthy lifestyle as important. Current health concerns include long waiting times for care at public hospitals, lack of carer should dependency occur, and lack of information about what services are available in the community. Interests in services in their neighbourhood include medical care (82%), healthy lifestyle activities (66%), and home care support (55%). There was considerable interest in the provision of services that improve brain and physical function, as well as general health checks. Carers were willing to pay more for services compared with older adults themselves. The findings inform the development of pilot models of aging in place as a sustainable financial model.

Is the Right to Housing Being Realized in Canada? Learning from the Experiences of Tenants in Affordable Housing Units in a Large Canadian City

Abstract:Background: Housing is a critical determinant of health and a basic human right. Historically, Canada’s housing policies have not been grounded in a human rights-based approach. In the 1990s, a policy shift prioritized efficiency in government spending and deficit reductions over the provision of many social programs including affordable housing. With various levels of government now acknowledging and recognizing the need for more affordable housing, it is important to understand tenant experiences, perspectives, and needs to ensure policies and practices are supporting individuals appropriately. Methods: In total, 161 individuals participated in this study by completing online or in-person questionnaires. Results: Exploratory analysis of results revealed that although there were some positive benefits to affordable housing, many tenants continued to struggle financially, physically, mentally, and emotionally without adequate supports in place. Conclusions: These findings highlight the need for affordable housing to be part of a system of care that provides supports along a continuum. The results further reiterate that placing a person or family in affordable housing does not guarantee that their lives have improved. Without robust affordable housing models that prioritize the empowerment of individuals and families, housing policies may fail to fulfil the right to safe and affordable housing for Canadians, especially when considering historically marginalized populations.

Abstract:Background: Social housing tenants have poorer health outcomes than homeowners or those renting privately. Health literacy is associated with access to care and health outcomes. This study aimed to examine the health literacy of Australian adults residing in social housing compared with that of people living in other housing types. Methods: A secondary analysis of the Australian National Health Survey 2017-2018 dataset was undertaken. A total of 5275 respondents were included in the sample and completed the Health Literacy Questionnaire (HLQ). Respondents were categorised according to their housing tenure: 163 (3.1%) respondents were living in social housing, 873 (17%) were living in private rentals, 2085 (40%) were homeowners, and 2154 (41%) were homeowners/mortgages. Mean scores were calculated for each of the nine health literacy domains in the HLQ and compared across housing tenure using linear regression models. Results: Social housing tenants had lower mean domain scores than either homeowners, owner mortgagees, or private renters on six of the nine health literacy domains. This included 'having sufficient information to manage my health', 'social support for health', 'ability to engage with healthcare providers', 'navigating the healthcare system' 'ability to find good health information', and being able to 'understand health information enough to know what to do'. However, the differences in mean scores were small. Conclusions: Increasing health literacy may be an important part of multicomponent interventions seeking to improve the health and wellbeing of social housing tenants.
Local Social and Service Environments of an Urban Housing Market for Seniors

Abstract:Specialized seniors’housing submarkets and their spatial forms in urban areas constitute distinctive and dynamic “landscapes of old age”. In the context of the government-subsidized senior citizen apartment (SCA) market of the city of Winnipeg, Canada, this paper offers an exploratory investigation of key components of the local neighborhood housing project environments in terms of social composition and service provision. The following three specific research questions are addressed. First, to what extent is the distribution of SCA units in the city geographically associated with the distribution of low-income elderly persons in the city? Second, to what extent does proximity to key off-site local service outlets differ between SCA projects located in the inner city and in the suburbs? Third, how do residents of SCA projects located in the inner city and the suburbs evaluate and utilize their local service environments? The data are elicited from published sources, field observations, and focus group interviews, and are analyzed using both descriptive spatial statistics and qualitative methodology. In relation to the first research question, the findings disclose a moderate tendency for SCA units to be over-represented in the inner city relative to the spatial distribution of low-income elderly people. With regard to the second research question, the results reveal that SCA projects located in the inner city typically enjoy greater proximity to key off-site services compared to suburban SCA projects. The findings related to the third research question disclose that although both inner city and suburban SCA residents are aware of the advantages and drawbacks of their respective local service environments, they are at least partly able to mitigate accessibility problems by adopting various supportive strategies (e.g., travel as passengers in cars or use of public transit). It is proposed that these supportive strategies are part of a wider process of residential adjustment in specialized seniors’ housing. To conclude, the implications of the findings for the development of more accommodative environmental settings for senior housing are also discussed.
Population-based estimates of major forms of housing insecurity among community-living older Americans

Abstract:Objectives: The number of older adults struggling to maintain adequate housing is growing. Prior studies have used various criteria to measure housing insecurity; however, no standardized definition exists to date. Using a multidimensional approach, our study sought to calculate population-based estimates of various forms of housing insecurity among community-living older Americans and determine how these estimates differ across key characteristics. Methods: This study utilized data from the 2011 round of the National Health and Aging Trends Study (NHATS), a prospective longitudinal study of Medicare beneficiaries aged 65 years or older. Three key forms of housing insecurity were operationalized: poor housing affordability (PHA), poor housing quality (PHQ), and poor neighborhood quality (PNQ). Population-based estimates of these forms of housing insecurity were calculated using analytic sampling weights and stratified by age, gender, race and ethnicity, frailty status, and dementia status. Results: Totally 6466 participants were included in the analysis, representing 29,848,119 community-living older Americans. The mean (standard deviation) age was 77.3 (7.7) years; by weighted percentages, 56.0% identified as female, 81.3% as White, 8.2% Black, and 7.1% Hispanic. At least one form of housing insecurity was identified in 38.5% of older Americans. Individually, the prevalence of PHA was 14.8%, PHQ 24%, and PNQ 12.5%. The prevalence of at least one form of housing insecurity was higher among persons of color (62.9% Black and 66% Hispanic vs White; p < 0.001), those with frailty (40.9% pre-frail and 49.4% frail vs robust; p < 0.001), and those with cognitive impairment (48.1% possible and 51% probable dementia vs no dementia; p < 0.001). Discussion: Nearly one in three community-living older Americans experience at least one form of housing insecurity. This was most common among vulnerable subgroups. Our multidimensional approach to defining various forms of housing insecurity can be used for future studies focused on improving social determinants of health among high-risk older adults.

Abstract:Abstract: In the U.S., more than one million older adults with low incomes live in apartment buildings subsidized by the Low-Income Housing Tax Credit. Although this population experiences disproportionate rates of nursing home admission, little is known about residents' perspectives on factors that influence their ability to live independently in these settings. Fifty-eight residents aged 62 and older and eight study partners participated in qualitative interviews about their perspectives on living independently in subsidized housing, including barriers and facilitators. We analyzed transcripts using a hybrid inductive and deductive approach to qualitative thematic analysis. Barriers and facilitators for living independently in subsidized housing related to the influence of the social and physical environment on individuals' experiences of living independently, including factors unique to subsidized housing. Findings suggest how interventions to optimize functional status and promote independence among older adults living in subsidized housing can build on existing strengths of the subsidized housing environment to improve outcomes.

Abstract:Population ageing presents significant challenges for many countries, one of which is the provision of adequate housing. Developing understanding of the needs and preferences of ageing societies will be crucial in order to assist in the provision of suitable housing and communities that are sustainable in the long term. While a preference to ‘age in place’ is clear in the literature, comparatively less academic research is available on older people’s preferences for more specific housing and environment attributes. The aim of this study is to identify the main housing and environment characteristics that are linked to the health and wellbeing of the elderly and determine the preferences for such characteristics via a survey with UK residents aged 55+. The results indicate a strong preference for independent living and an increasing desire for bungalows in later life. Housing conditions, energy efficiency, thermal comfort, and home adaptions to facilitate ageing in place are particularly important housing characteristics to older people. The location and environment are also key drivers of housing preferences; a safe neighbourhood, accessibility to amenities, public transport, and a clean and walkable environment are particularly important. Preferences varied with age, but gender has a less significant impact on the preferences expressed. The findings of this study will be valuable for stakeholders engaged in housing policy and provision for older people.

The effect of housing on the mental health of older people: the impact of lifetime housing history in Whitehall II

Abstract:Background: This study describes differences in trajectories of self-reported mental health in an ageing cohort, according to their housing, while controlling for confounders. Methods: The General Health Questionnaire was measured on six occasions as part of Whitehall II cohort study of office-based British civil servants (1985-2009); 10,308 men and women aged 35-55 at baseline. Results: Home-ownership was the predominant tenure at baseline and increased over the life-course, but the social gradient remained. In the bivariate analysis, by phase nine, renters had higher (poorer mental health) GHQ scores (55.48) than owner occupiers (51.98). Those who reported difficulty paying bills or problems with housing had higher GHQ scores at baseline (financial difficulties 57.70 vs 54.34; house problems 58.06 vs 53.99) and this relative difference increased by phase nine (financial difficulties 59.64 vs 51.67; house problems 56.68 vs 51.22). In multivariate models, the relative differences in GHQ scores by tenure increased with age, but were no longer significant after adjusting for confounders. Whereas GHQ scores for those with housing problems and financial difficulties were still significantly higher as participants grew older. Conclusion: The social gradient in the effect of home ownership on mental health, which is evident at baseline, diminishes as people get older, whereas housing quality and financial problems become relatively more important in explaining older people's health. Inequalities in housing quality and ability to deal with household financial problems will become increasingly important mental health issues as the population ages.

Abstract:Research on programs offering senior-specific housing supports and enabling "aging in the right place" (AIRP) for "older persons with experiences of homelessness" (OPEH) is limited. This paper presents an environmental assessment of a "transitional housing program" (THP) in Metro Vancouver, Canada, for OPEH to AIRP. Data were collected using Aging in the Right Place Environmental (AIRP-ENV) and Secondary Observation (AIRP-ENV-SO) audit tools designed to evaluate multi-unit housing for OPEH. The 241-item AIRP-ENV tool was used to assess the built environmental features of four multi-unit buildings of the THP. The AIRP-ENV-SO tool was used to collect contextual data on the function, safety, and land use of the surrounding neighborhood. Findings identified built environment and urban design features that support THP residents' safety, security, accessibility, functionality, social activity, autonomy, and identity. The THP buildings were rated 'Good' for accessibility, functionality, autonomy and identity, while 'Satisfactory' or 'Poor' for safety, security, and social activity. Findings point to the built environmental features (e.g., size and layout of spaces) required in the THP to create opportunities for increased social engagement among residents and enhanced safety and security. The AIRP-ENV and AIRP-ENV-SO audit tools can help inform programs across the housing continuum to develop supportive built environments that promote AIRP for OPEH.

Abstract:As supporting active and healthy aging calls for different community-based housing alternatives, integrated knowledge of their impacts on older adults' social participation is required. This study aimed to explore how community-based housing fostered social participation in older adults. A scoping review was used to systematically identify relevant studies according to 32 keywords in 8 electronic databases. Published during 2000–2021, the 46 studies were carried out in North America, Europe, Oceania, and Asia, documenting older adults' social participation in six community-based housing models. Targeting different clienteles, these models mainly promote older adults' social participation by providing: 1) accessible common space in a design favorable to social interactions; 2) proximity to resources; 3) flexible rules and policies that facilitate residents' interactions with not only co-residents but community neighbors; and 4) benevolent communities. These results highlight the interactions between the physical and social environments; they suggest the importance of building benevolent communities as well as of sharing resources among residences and local communities to create a supportive living and neighborhood environment for active and healthy aging. Future studies should further explore the role of different stakeholders in developing benevolent communities by considering the dynamics between the person and the environment.

Occupant satisfaction on facility services: case studies of six multi-generational condominiums

Abstract:Purpose Feedback from condominium occupants on multi-generational condominium facility services makes it possible to identify areas in need of improvement and development concerning facilities management in this residential project type. This paper aims to examine behaviours, needs, preferences and patterns of space use in condominium occupants from three different age groups (under 30, 30 to 50 and over 50 years old) relative to their satisfaction with facility services. The paper examines the impact of the physical environment and facility service arrangement (i.e. cleaning and security) on occupant satisfaction in the common areas of six multi-generational condominiums and investigates whether occupant satisfaction varies between the three age groups. Design/methodology/approach Six case studies classified as multi-generational condominiums were conducted. Research methods included document analysis, field surveys, interviews with juristic person managers and surveys from condominium occupants regarding their satisfaction with cleaning and security services in the common areas. The findings were then applied one-way ANOVA to test occupant satisfaction mean differences between age groups. Findings The findings indicate that physical environment characteristics including the location, size, number and design attributes of common areas have an impact on the provision of facility services and may influence occupant satisfaction. In terms of the facility service arrangement, both the service method and the ratio of staff to areas of responsibility can be explained in connection to functional and technical quality, respectively. Occupants’ demands and behaviours, such as the strong requirement for privacy, the preference towards service staff accessibility and the frequency of space usage, contribute to different levels of occupant satisfaction. Research limitations/implications More case studies in Thailand and also in other countries, based on surveys with large samples of respondents, are required to increase the validity and to establish whether it is possible to generalise the study’s findings. Practical implications The findings and reflections upon them help understand the impact of physical environment characteristics and the provision of facility services on occupant satisfaction in various age groups. Recommendations are provided concerning building design and the provision of facility services in connection to perceived service quality and occupant satisfaction in multi-generational condominiums in Thailand that can apply to other types of real estate projects where similar facility services are offered. Originality/value This paper contributes to research on occupant satisfaction with facility services and provides evidence on occupants’ feedback in different types of perceived service quality (i.e. technical and functional quality). Abstract:Background In this article, we apply a gender-based analysis plus framework to research the housing experiences of older, low-income adults living and aging in Hamilton. Low-income older adults with intersectional identities are at risk of not aging in place due to marginalization and housing instability. Objective Policy currently homogenizes the experience of aging by sidelining intersectional factors that have a bearing on aging well in place. The research aims to develop policy recommendations to address this gap. Methods Several methods captured the housing experiences of low-income older adults, including interviews, participant observation, and arts-based techniques. Findings Findings illustrate how gender and intersectional factors shape both housing trajectories and agentive practices low-income adults utilize to try to age well and in place. These strategies encompass practicing cultural citizenship, which is a claim for inclusion when excluded from mainstream ideals of aging in place. Discussion We provide policy recommendations informed by participants' lived experiences aimed at promoting equitable aging in place as fundamental to full citizenship.

Abstract:INTRODUCTION Elderly persons who live alone are more likely to be socially isolated and at increased risk of adverse health outcomes, unnecessary hospital re-admissions and premature mortality. We aimed to understand the health-seeking behaviour of elderly persons living alone in public rental housing in Singapore. METHODS In-depth interviews were conducted using a semi-structured question guide. Participants were selected using a purposive sampling approach. Interviews were conducted until theme saturation was reached. Qualitative data collected was analysed using manual thematic coding methods. RESULTS Data analysis revealed five major themes: accessibility of healthcare services and financial assistance schemes; perceived high cost of care; self-management; self-reliance; and mismatch of perceived needs and services. CONCLUSION Elderly persons living in one-room rental flats are a resilient and resourceful group that values self-reliance and independence. Most of the elderly who live alone develop self-coping mechanisms to meet their healthcare needs rather than seek formal medical consultation. The insightful findings from this study should be taken into consideration when models of healthcare delivery are being reviewed and designed so as to support the disadvantaged elderly living alone.

Living Arrangements of The Elderly: Social Participation and Life Satisfaction

Abstract:This paper examines the relationship between two types of living arrangements for the elderly, the resulting activity level and life satisfaction. The sample consists of 117 elderly individuals. Fifty-two of the respondents reside in five shared-living houses, while 65 of the respondents live in two senior-citizen apartment complexes. Responses to questions concerning participation in leisure activities, voluntary associations, life satisfaction and selected socio-demographic characteristics are analyzed using Pearson product-moment correlation coefficients and least squares analysis of variance. The results indicate that participati on in leisure activities and voluntary associations is affected by age, living site and educational level. Participati on in these activities is relatively low. Respondents living in the citizen apartments rank higher in participation levels than those living in shared-living houses. The respondents report moderate levels of life satisfaction, slightly below the established means for the index. No significant difference in life satisfaction between sites is evident. Life satisfaction is positively correlated with health and participation in group activities.
