Abstract:Vertebral body compression fractures are reliable early signs of osteoporosis. Though these fractures are visible on Computed Tomography (CT) images, they are frequently missed by radiologists in clinical settings. Prior research on automatic methods of vertebral fracture classification proves its reliable quality; however, existing methods provide hard-to-interpret outputs and sometimes fail to process cases with severe abnormalities such as highly pathological vertebrae or scoliosis. We propose a new two-step algorithm to localize the vertebral column in 3D CT images and then to simultaneously detect individual vertebrae and quantify fractures in 2D. We train neural networks for both steps using a simple 6-keypoints based annotation scheme, which corresponds precisely to current medical recommendations. Our algorithm has no exclusion criteria, processes 3D CT in 2 s on a single GPU and provides an intuitive and verifiable output. The method approaches expert-level performance and demonstrates state-of-the-art results in vertebrae 3D localization (the average error is 1 mm), vertebrae 2D detection (precision is 0.99, recall is 1), and fracture identification (ROC AUC at the patient level is 0.93).
Detection of vertebral fractures in CT using 3D Convolutional Neural Networks

Abstract:Osteoporosis induced fractures occur worldwide about every 3 seconds. Vertebral compression fractures are early signs of the disease and considered risk predictors for secondary osteoporotic fractures. We present a detection method to opportunistically screen spine-containing CT images for the presence of these vertebral fractures. Inspired by radiology practice, existing methods are based on 2D and 2.5D features but we present, to the best of our knowledge, the first method for detecting vertebral fractures in CT using automatically learned 3D feature maps. The presented method explicitly localizes these fractures allowing radiologists to interpret its results. We train a voxel-classification 3D Convolutional Neural Network (CNN) with a training database of 90 cases that has been semi-automatically generated using radiologist readings that are readily available in clinical practice. Our 3D method produces an Area Under the Curve (AUC) of 95% for patient-level fracture detection and an AUC of 93% for vertebra-level fracture detection in a five-fold cross-validation experiment.

Abstract:An osteoporosis-related fracture occurs every three seconds worldwide, affecting one in three women and one in five men aged over 50. The early detection of at-risk patients facilitates effective and well-evidenced preventative interventions, reducing the incidence of major osteoporotic fractures. In this study, we present an automatic system for identification of vertebral compression fractures on Computed Tomography images, which are often an undiagnosed precursor to major osteoporosis-related fractures. The system integrates a compact 3D representation of the spine, utilizing a Convolutional Neural Network (CNN) for spinal cord detection and a novel end-to-end sequence to sequence 3D architecture. We evaluate several model variants that exploit different representation and classification approaches and present a framework combining an ensemble of models that achieves state of the art results, validated on a large data set, with a patient-level fracture identification of 0.955 Area Under the Curve (AUC). The system proposed has the potential to support osteoporosis clinical management, improve treatment pathways, and to change the course of one of the most burdensome diseases of our generation.

Abstract:Do black-box neural network models learn clinically relevant features for fracture diagnosis? The answer not only establishes reliability quenches scientific curiosity but also leads to explainable and verbose findings that can assist the radiologists in the final and increase trust. This work identifies the concepts networks use for vertebral fracture diagnosis in CT images. This is achieved by associating concepts to neurons highly correlated with a specific diagnosis in the dataset. The concepts are either associated with neurons by radiologists pre-hoc or are visualized during a specific prediction and left for the user's interpretation. We evaluate which concepts lead to correct diagnosis and which concepts lead to false positives. The proposed frameworks and analysis pave the way for reliable and explainable vertebral fracture diagnosis.

Abstract:Background: Acute vertebral fracture is usually caused by low-energy injury with osteoporosis and high-energy trauma. The AOSpine thoracolumbar spine injury classification system (AO classification) plays an important role in the diagnosis and treatment of the disease. The diagnosis and description of vertebral fractures according to the classification scheme requires a great deal of time and energy for radiologists. Purpose: To design and validate a multistage deep learning system (multistage AO system) for the automatic detection, localization and classification of acute thoracolumbar vertebral body fractures according to AO classification on computed tomography. Materials and Methods: The CT images of 1,217 patients who came to our hospital from January 2015 to December 2019 were collected retrospectively. The fractures were marked and classified by 2 junior radiology residents according to the type A standard in the AO classification. Marked fracture sites included the upper endplate, lower endplate and posterior wall. When there were inconsistent opinions on classification labels, the final result was determined by a director radiologist. We integrated different networks into different stages of the overall framework. U-net and a graph convolutional neural network (U-GCN) are used to realize the location and classification of the thoracolumbar spine. Next, a classification network is used to detect whether the thoracolumbar spine has a fracture. In the third stage, we detect fractures in different parts of the thoracolumbar spine by using a multibranch output network and finally obtain the AO types. Results: The mean age of the patients was 61.87 years with a standard deviation of 17.04 years, consisting of 760 female patients and 457 male patients. On vertebrae level, sensitivity for fracture detection was 95.23% in test dataset, with an accuracy of 97.93% and a specificity of 98.35%. For the classification of vertebral body fractures, the balanced accuracy was 79.56%, with an AUC of 0.904 for type A1, 0.945 for type A2, 0.878 for type A3 and 0.942 for type A4. Conclusion: The multistage AO system can automatically detect and classify acute vertebral body fractures in the thoracolumbar spine on CT images according to AO classification with high accuracy.

Abstract:Vertebral compression fractures resulting from osteoporosis contribute to pain and disability among older people, necessitating early detection and treatment. While MRI provides effective diagnosis, its higher cost poses a challenge. Conversely, CT imaging offers a more cost-effective alternative. However, CT-based detection of vertebral fractures may lack the accuracy of MRI. This paper presents a novel YOLO-based object detection method for localizing old and fresh fractures on spine CT images to expedite diagnosis and identify the optimal treatment window. We propose replacing the CSPDarknet53 backbone in the native YOLOR model with MobileViT and EfficientNet_NS, training three separate YOLOR models with different backbones. Subsequently, an ensemble approach is employed to leverage the enhanced feature extraction capabilities of the three models. Experimental results show individual model accuracies of 89%, 89.8%, and 89.2%, respectively. Furthermore, by replacing the convolution layer with the Involution layer and integrating the models using the ensemble method, we achieve a remarkable accuracy rate of 93.4%. The proposed Ensemble YOLOR model surpasses other state-of-the-art approaches, offering a fast and precise solution. This advancement provides valuable insights and guidance for physicians in diagnosing compression between old and new fractures.

An Innovative Deep Learning Approach to Spinal Fracture Detection in CT Images

Abstract:Aim: Spinal fractures, particularly vertebral compression fractures, pose a significant challenge in medical imaging due to their small-scale nature and blurred boundaries in Computed Tomography (CT) scans. However, advanced deep learning models, such as the integration of the You Only Look Once (YOLO) V7 model with Efficient Layer Aggregation Networks (ELAN) and Max-Pooling Convolution (MPConv) architectures, can substantially reduce the loss of small-scale information during computational processing, thus improving detection accuracy. The purpose of this study is to develop an innovative deep learning approach for detecting spinal fractures, particularly vertebral compression fractures, in CT images. Methods: We proposed a novel method to precisely identify spinal injury using the YOLO V7 model as a classifier. This model was enhanced by integrating ELAN and MPConv architectures, which were influenced by the Receptive Field Learning and Aggregation (RFLA) small object recognition framework. Standard normalization techniques were utilized to preprocess the CT images. The YOLO V7 model, integrated with ELAN and MPConv architectures, was trained using a dataset containing annotated spinal fractures. Additionally, to mitigate boundary ambiguities in compressive fractures, a Theoretical Receptive Field (TRF) based on Gaussian distribution and an Effective Receptive Field (ERF) were used to capture multi-scale features better. Furthermore, the Wasserstein distance was employed to optimize the model's learning process. A total of 240 CT images from patients diagnosed with spinal fractures were included in this study, sourced from Ningbo No.2 Hospital, ensuring a robust dataset for training the deep learning model. Results: Our method demonstrated superior performance over conventional object detection networks like YOLO V7 and YOLO V3. Specifically, with a dataset of 200 pathological images and 40 normal spinal images, our method achieved a 3% increase in accuracy compared to YOLO V7. Conclusions: The proposed method offers an innovative and more effective approach for identifying vertebral compression fractures in CT scans. These promising findings suggest the method's potential for practical clinical applications, highlighting the significance of deep learning in enhancing patient care and treatment in medical imaging. Future research should incorporate cross-validation and independent validation and test sets to assess the model's robustness and generalizability. Additionally, exploring other deep learning models and methods could further enhance detection accuracy and reliability, contributing to the development of more effective diagnostic tools in medical imaging.

Abstract:Background Identification of vertebral fractures (VFs) is critical for effective secondary fracture prevention owing to their association with the increasing risks of future fractures. Plain abdominal frontal radiographs (PARs) are a common investigation method performed for a variety of clinical indications and provide an ideal platform for the opportunistic identification of VF. This study uses a deep convolutional neural network (DCNN) to identify the feasibility for the screening, detection, and localization of VFs using PARs. Methods A DCNN was pretrained using ImageNet and retrained with 1306 images from the PARs database obtained between August 2015 and December 2018. The accuracy, sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) were evaluated. The visualization algorithm gradient-weighted class activation mapping (Grad-CAM) was used for model interpretation. Results Only 46.6% (204/438) of the VFs were diagnosed in the original PARs reports. The algorithm achieved 73.59% accuracy, 73.81% sensitivity, 73.02% specificity, and an AUC of 0.72 in the VF identification. Conclusion Computer driven solutions integrated with the DCNN have the potential to identify VFs with good accuracy when used opportunistically on PARs taken for a variety of clinical purposes. The proposed model can help clinicians become more efficient and economical in the current clinical pathway of fragile fracture treatment.

Opportunistic Screening for Acute Vertebral Fractures on a Routine Abdominal or Chest Computed Tomography Scans Using an Automated Deep Learning Model

Abstract:Objectives: To develop an opportunistic screening model based on a deep learning algorithm to detect recent vertebral fractures in abdominal or chest CTs. Materials and Methods: A total of 1309 coronal reformatted images (504 with a recent fracture from 119 patients, and 805 without fracture from 115 patients), from torso CTs, performed from September 2018 to April 2022, on patients who also had a spine MRI within two months, were included. Two readers participated in image selection and manually labeled the fractured segment on each selected image with Neuro-T (version 2.3.3; Neurocle Inc.) software. We split the images randomly into the training and internal test set (labeled: unlabeled = 480:700) and the secondary interval validation set (24:105). For the observer study, three radiologists reviewed the CT images in the external test set with and without deep learning assistance and scored the likelihood of an acute fracture in each image independently. Results: For the training and internal test sets, the AI achieved a 99.86% test accuracy, 91.22% precision, and 89.18% F1 score for detection of recent fracture. Then, in the secondary internal validation set, it achieved 99.90%, 74.93%, and 78.30%, respectively. In the observer study, with the assistance of the deep learning algorithm, a significant improvement was observed in the radiology resident's accuracy, from 92.79% to 98.2% (p = 0.04). Conclusion: The model showed a high level of accuracy in the test set and also the internal validation set. If this algorithm is applied opportunistically to daily torso CT evaluation, it will be helpful for the early detection of fractures that require treatment.

Abstract:Abstract Vertebral fractures (VFs) cause serious problems, such as substantial functional loss and a high mortality rate, and a delayed diagnosis may further worsen the prognosis. Plain thoracolumbar radiography (PTLR) is an essential method for the evaluation of VFs. Therefore, minimizing the diagnostic errors of VFs on PTLR is crucial. Image identification based on a deep convolutional neural network (DCNN) has been recognized to be potentially effective as a diagnostic strategy; however, the accuracy for detecting VFs has not been fully investigated. A DCNN was trained with PTLR images of 300 patients (150 patients with and 150 without VFs). The accuracy, sensitivity, and specificity of diagnosis of the model were calculated and compared with those of orthopedic residents, orthopedic surgeons, and spine surgeons. The DCNN achieved accuracy, sensitivity, and specificity rates of 86.0% [95% confidence interval (CI) 82.0–90.0%], 84.7% (95% CI 78.8–90.5%), and 87.3% (95% CI 81.9–92.7%), respectively. Both the accuracy and sensitivity of the model were suggested to be noninferior to those of orthopedic surgeons. The DCNN can assist clinicians in the early identification of VFs and in managing patients, to prevent further invasive interventions and a decreased quality of life.

Abstract:Background Spinal degeneration and vertebral compression fractures are common among the elderly that adversely affect their mobility, quality of life, lung function, and mortality. Assessment of vertebral fractures in chronic obstructive pulmonary disease (COPD) is important due to the high prevalence of osteoporosis and associated vertebral fractures in COPD. Purpose We present new automated methods for (1) segmentation and labelling of individual vertebrae in chest computed tomography (CT) images using deep learning (DL), multi‐parametric freeze‐and‐grow (FG) algorithm, and separation of apparently fused vertebrae using intensity autocorrelation and (2) vertebral deformity fracture detection using computed vertebral height features and parametric computational modelling of an established protocol outlined for trained human experts. Methods A chest CT‐based automated method was developed for quantitative deformity fracture assessment following the protocol by Genant et al. The computational method was accomplished in the following steps: (1) computation of a voxel‐level vertebral body likelihood map from chest CT using a trained DL network; (2) delineation and labelling of individual vertebrae on the likelihood map using an iterative multi‐parametric FG algorithm; (3) separation of apparently fused vertebrae in CT using intensity autocorrelation; (4) computation of vertebral heights using contour analysis on the central anterior‐posterior (AP) plane of a vertebral body; (5) assessment of vertebral fracture status using ratio functions of vertebral heights and optimized thresholds. The method was applied to inspiratory or total lung capacity (TLC) chest scans from the multi‐site Genetic Epidemiology of COPD (COPDGene) (ClinicalTrials.gov: NCT00608764) study, and the performance was examined (n = 3231). One hundred and twenty scans randomly selected from this dataset were partitioned into training (n = 80) and validation (n = 40) datasets for the DL‐based vertebral body classifier. Also, generalizability of the method to low dose CT imaging (n = 236) was evaluated. Results The vertebral segmentation module achieved a Dice score of .984 as compared to manual outlining results as reference (n = 100); the segmentation performance was consistent across images with the minimum and maximum of Dice scores among images being .980 and .989, respectively. The vertebral labelling module achieved 100% accuracy (n = 100). For low dose CT, the segmentation module produced image‐level minimum and maximum Dice scores of .995 and .999, respectively, as compared to standard dose CT as the reference; vertebral labelling at low dose CT was fully consistent with standard dose CT (n = 236). The fracture assessment method achieved overall accuracy, sensitivity, and specificity of 98.3%, 94.8%, and 98.5%, respectively, for 40,050 vertebrae from 3231 COPDGene participants. For generalizability experiments, fracture assessment from low dose CT was consistent with the reference standard dose CT results across all participants. Conclusions Our CT‐based automated method for vertebral fracture assessment is accurate, and it offers a feasible alternative to manual expert reading, especially for large population‐based studies, where automation is important for high efficiency. Generalizability of the method to low dose CT imaging further extends the scope of application of the method, particularly since the usage of low dose CT imaging in large population‐based studies has increased to reduce cumulative radiation exposure.

Abstract:Background: Pathological low-energy (LE) vertebral compression fractures (VFs) are common complications of osteoporosis and predictors of subsequent LE fractures. In 84% of cases, VFs are not reported on chest CT (CCT), which calls for the development of an artificial intelligence-based (AI) assistant that would help radiology specialists to improve the diagnosis of osteoporosis complications and prevent new LE fractures. Aims: To develop an AI model for automated diagnosis of compression fractures of the thoracic spine based on chest CT images. Materials and methods: Between September 2019 and May 2020 the authors performed a retrospective sampling study of ССТ images. The 160 of results were selected and anonymized. The data was labeled by seven readers. Using the morphometric analysis, the investigators received the following metric data: ventral, medial and dorsal dimensions. This was followed by a semiquantitative assessment of VFs degree. The data was used to develop the Comprise-G AI mode based on CNN, which subsequently measured the size of the vertebral bodies and then calculates the compression degree. The model was evaluated with the ROC curve analysis and by calculating sensitivity and specificity values. Results: Formed data consist of 160 patients (a training group - 100 patients; a test group - 60 patients). The total of 2,066 vertebrae was annotated. When detecting Grade 2 and 3 maximum VFs in patients the Comprise-G model demonstrated sensitivity - 90,7%, specificity - 90,7%, AUC ROC - 0.974 on the 5-FOLD cross-validation data of the training dataset; on the test data - sensitivity - 83,2%, specificity - 90,0%, AUC ROC - 0.956; in vertebrae demonstrated sensitivity - 91,5%, specificity - 95,2%, AUC ROC - 0.981 on the cross-validation data; for the test data sensitivity - 79,3%, specificity - 98,7%, AUC ROC - 0.978. Conclusions: The Comprise-G model demonstrated high diagnostic capabilities in detecting the VFs on CCT images and can be recommended for further validation.

Abstract:We present a novel method for explainable vertebral fracture assessment (XVFA) in low-dose radiographs using deep neural networks, incorporating vertebra detection and keypoint localization with uncertainty estimates. We incorporate Genant's semi-quantitative criteria as a differentiable rule-based means of classifying both vertebra fracture grade and morphology. Unlike previous work, XVFA provides explainable classifications relatable to current clinical methodology, as well as uncertainty estimations, while at the same time surpassing state-of-the art methods with a vertebra-level sensitivity of 93% and end-to-end AUC of 97% in a challenging setting. Moreover, we compare intra-reader agreement with model uncertainty estimates, with model reliability on par with human annotators.

Abstract:Background: The reaserch of artificial intelligence (AI) model for predicting spinal refracture is limited to bone mineral density, X-ray and some conventional laboratory indicators, which has its own limitations. Besides, it lacks specific indicators related to osteoporosis and imaging factors that can better reflect bone quality, such as computed tomography (CT). Objective: To construct a novel predicting model based on bone turn-over markers and CT to identify patients who were more inclined to suffer spine refracture. Methods: CT images and clinical information of 383 patients (training set = 240 cases of osteoporotic vertebral compression fractures (OVCF), validation set = 63, test set = 80) were retrospectively collected from January 2015 to October 2022 at three medical centers. The U-net model was adopted to automatically segment ROI. Three-dimensional (3D) cropping of all spine regions was used to achieve the final ROI regions including 3D_Full and 3D_RoiOnly. We used the Densenet 121-3D model to model the cropped region and simultaneously build a T-NIPT prediction model. Diagnostics of deep learning models were assessed by constructing ROC curves. We generated calibration curves to assess the calibration performance. Additionally, decision curve analysis (DCA) was used to assess the clinical utility of the predictive models. Results: The performance of the test model is comparable to its performance on the training set (dice coefficients of 0.798, an mIOU of 0.755, an SA of 0.767, and an OS of 0.017). Univariable and multivariable analysis indicate that T_P1NT was an independent risk factor for refracture. The performance of predicting refractures in different ROI regions showed that 3D_Full model exhibits the highest calibration performance, with a Hosmer-Lemeshow goodness-of-fit (HL) test statistic exceeding 0.05. The analysis of the training and test sets showed that the 3D_Full model, which integrates clinical and deep learning results, demonstrated superior performance with significant improvement (p-value < 0.05) compared to using clinical features independently or using only 3D_RoiOnly. Conclusion: T_P1NT was an independent risk factor of refracture. Our 3D-FULL model showed better performance in predicting high-risk population of spine refracture than other models and junior doctors do. This model can be applicable to real-world translation due to its automatic segmentation and detection.

Abstract:Long-term vertebral fractures severely affect the life quality of patients, causing kyphotic, lumbar deformity and even paralysis. Computed tomography (CT) is a common clinical examination to screen for this disease at early stages. However, the faint radiological appearances and unspecific symptoms lead to a high risk of missed diagnosis, especially for the mild vertebral fractures. In this paper, we argue that reinforcing the faint fracture features to encourage the inter-class separability is the key to improving the accuracy. Motivated by this, we propose a supervised contrastive learning based model to estimate Genent's Grade of vertebral fracture with CT scans. The supervised contrastive learning, as an auxiliary task, narrows the distance of features within the same class while pushing others away, enhancing the model's capability of capturing subtle features of vertebral fractures. Our method has a specificity of 99% and a sensitivity of 85% in binary classification, and a macro-F1 of 77% in multi-class classification, indicating that contrastive learning significantly improves the accuracy of vertebrae fracture screening. Considering the lack of datasets in this field, we construct a database including 208 samples annotated by experienced radiologists. Our desensitized data and codes will be made publicly available for the community.

Abstract:As one of fundamental ways to interpret spine images, detection of vertebral landmarks is an informative prerequisite for further diagnosis and management of spine disorders such as scoliosis and fractures. Most existing machine learning-based methods for automatic vertebral landmark detection suffer from overlapping landmarks or abnormally long distances between nearby landmarks against anatomical priors, and thus lack sufficient reliability and interpretability. To tackle the problem, this paper systematically utilizes anatomical prior knowledge in vertebral landmark detection. We explicitly formulate anatomical priors of the spine, related to distances among vertebrae and spatial order within the spine, and integrate these geometrical constraints within training loss, inference procedure, and evaluation metrics. First, we introduce an anatomy-constraint loss to regularize the training process with the aforementioned contextual priors explicitly. Second, we propose a simple-yet-effective anatomy-aided inference procedure by employing sequential prediction rather than a parallel counterpart. Third, we provide novel anatomy-related metrics to quantitatively evaluate to which extent landmark predictions follow the anatomical priors, as is not reflected within the widely-used landmark localization error metric. We employ the localization framework on 1410 anterior-posterior radiographic images. Compared with competitive baseline models, we achieve superior landmark localization accuracy and comparable Cobb angle estimation for scoliosis assessment. Ablation studies demonstrate the effectiveness of designed components on the decrease of localization error and improvement of anatomical plausibility. Additionally, we exhibit effective generalization performance by transferring our detection method onto sagittal 2-D slices of CT scans and boost the performance of downstream compression fracture classification at vertebra-level.
AI-based automated detection and stability analysis of traumatic vertebral body fractures on computed tomography

Abstract:PURPOSE: We developed and tested a neural network for automated detection and stability analysis of vertebral body fractures on computed tomography (CT).MATERIALS AND METHODS: 257 patients who underwent CT were included in this Institutional Review Board (IRB) approved study. 463 fractured and 1883 non-fractured vertebral bodies were included, with 190 fractures unstable. Two readers identified vertebral body fractures and assessed their stability. A combination of a Hierarchical Convolutional Neural Network (hNet) and a fracture Classification Network (fNet) was used to build a neural network for the automated detection and stability analysis of vertebral body fractures on CT. Two final test settings were chosen: one with vertebral body levels C1/2 included and one where they were excluded.RESULTS: The mean age of the patients was 68 ± 14 years. 140 patients were female. The network showed a slightly higher diagnostic performance when excluding C1/2. Accordingly, the network was able to distinguish fractured and non-fractured vertebral bodies with a sensitivity of 75.8 % and a specificity of 80.3 %. Additionally, the network determined the stability of the vertebral bodies with a sensitivity of 88.4 % and a specificity of 80.3 %. The AUC was 87 % and 91 % for fracture detection and stability analysis, respectively. The sensitivity of our network in indicating the presence of at least one fracture / one unstable fracture within the whole spine achieved values of 78.7 % and 97.2 %, respectively, when excluding C1/2.CONCLUSION: The developed neural network can automatically detect vertebral body fractures and evaluate their stability concurrently with a high diagnostic performance.

Abstract:Accurately detecting spine vertebrae plays a crucial role in successful orthopedic surgery. However, identifying and classifying lumbar vertebrae from arbitrary spine X-ray images remains challenging due to their similar appearance and varying sizes among individuals. In this paper, we propose a novel approach to enhance vertebrae detection accuracy by leveraging both global and local spatial relationships between neighboring vertebrae. Our method incorporates a two-stage detector architecture that captures global contextual information using an intermediate heatmap from the first stage. Additionally, we introduce a detection head in the second stage to capture local spatial information, enabling each vertebra to learn neighboring spatial details, visibility, and relative offset. During inference, we employ a fusion strategy that combines spatial offsets of neighboring vertebrae and heatmap from a conventional detection head. This enables the model to better understand relationships and dependencies between neighboring vertebrae. Furthermore, we introduce a new representation of object centers that emphasizes critical regions and strengthens the spatial priors of human spine vertebrae, resulting in an improved detection accuracy. We evaluate our method using two lumbar spine image datasets and achieve promising detection performance. Compared to the baseline, our algorithm achieves a significant improvement of 13.6% AP in the CM dataset and surpasses 6.5% and 4.8% AP in the anterior and lateral views of the BUU dataset, respectively.

Abstract:Fractures of the cervical spine are a medical emergency and may lead to permanent paralysis and even death. Accurate diagnosis in patients with suspected fractures by computed tomography (CT) is critical to patient management. In this paper, we propose a deep convolutional neural network (DCNN) with a bidirectional long-short term memory (BLSTM) layer for the automated detection of cervical spine fractures in CT axial images. We used an annotated dataset of 3,666 CT scans (729 positive and 2,937 negative cases) to train and validate the model. The validation results show a classification accuracy of 70.92% and 79. 18% on the balanced (104 positive and 104 negative cases) and imbalanced (104 positive and 419 negative cases) test datasets, respectively.
