Abstract:The first electrocardiogram (ECG) from the intact human heart was recorded with a mercury capillary electrometer by Augustus Waller in May 1887 at St. Mary's Hospital, London. The tracings were poor and exhibited only 2 distorted deflections. Willem Einthoven (1860-1927) who was professor of physiology at the University of Leiden, The Netherlands, began his studies of the ECG with the mercury capillary electrometer, and improved its distortion mathematically so that he was finally able to register a good representation of the ECG before the beginning of the twentieth century. He later further improved ECG recordings with the introduction of a string galvanometer of his design. Einthoven published his first article about the string galvanometer in 1901, followed by a more detailed description in 1903 which included a report of ECGs taken with the new instrument. The year 2002 marks the centennial of Willem Einthoven's first recording of the ECG in a clinically applicable fashion with the string galvanometer. The clinical use of Einthoven's immobile equipment required transtelephonic transmission of the ECG from the physiology laboratory to the clinic at the Academic Hospital about a mile away as documented in the 1906 paper on the "télécardiogramme". This report contained a wealth of ECG patterns and arrhythmias. Einthoven developed a system of electrocardiographic standardization that continues to be used all over the world and introduced the triaxial bipolar system with 3 limb leads and thus established uniformity of the recording process. Einthoven also conceived the famous equilateral triangle with leads I, II, and III at its sides and the calculation of the electrical axis (in the frontal plane) depicted as a single vector with an arrow at the center of the triangle. Einthoven recognized the great potential importance of the ECG as a diagnostic and investigative tool and his achievements made him the founder of modern electrocardiography. He was awarded the Nobel Prize in 1924 (2 years after Waller's death) in physiology and medicine, "for the discovery of the mechanism of the electrocardiogram."

Abstract:Walter Eichler (1904-1942) performed the first in situ nerve conduction studies in humans. Eichler's work has been largely overlooked and there have been no biographical accounts written of him. His 1937 paper, Über die Ableitung der Aktionspotentiale vom menschlichen Nerven in situ (On the recording of the action potentials from human nerves in situ) was translated and reviewed. Archival material was obtained on his career that was housed predominantly at the University of Freiburg im Breisgau. He had memberships in Nazi organizations but did not appear to be politically active. During his brief career, he constructed novel equipment and established seminal principles for performing nerve conductions on humans. The authors repeated his experiment in the ulnar nerve, which duplicated Eichler's findings. His recordings were quite remarkable given advances in technology. In summary, the Eichler paper is the first study in the development of in situ clinical electroneurography in humans. Many of his procedural observations are still fundamental in the current practice of electroneurography. As best can be determined, his study in humans did not appear ethically compromised. Abstract:In 1924, at the University Hospital of Jena, Hans Berger first recorded an electrical brain signal in humans. This discovery revolutionized clinical neuroscience and neurotechnology, as it contributed to both electrophysiology and the development of the electroencephalogram (EEG). The manuscript provides a historical overview of Hans Berger's seminal contributions, highlighting the importance of his early recordings, the motivations that drove him, and the scientific problems he had to initiate and solve, in a historical context of profoundly changing circumstances. He also faced low acceptance of his works initially, and only belatedly did they become accepted by the scientific community. Berger was known to be a humble but tenacious person who believed in his convictions to the core, and this strength of will is an example of passion for students and scholars of neuroscience.

Abstract:Objective: To recount the evolution of Electrical Cortical Stimulation (ECS) in localizing brain functions with an emphasis on epilepsy, and a discussion of related instruments and personnel. Design/methods: Literature review through historical archives implementing chain-referral sampling. Results: There were important milestones leading to the incorporation of ECS into practice: 1. Aldini's (1802) first known stimulation of exposed brain to defend Galvani's views on excitability in the frog-leg experiment against Volta's, ironically by employing the Voltaic pile. 2. Animal experiments in the 19th-century to study the brain and to optimize the procedure: Rolando (1809) reported on motor induction, Fritsch and Hitzig (ca. 1870) introduced the concepts of bipolar and threshold stimulation, and Ferrier (1873) generated reproducible homunculi in animals. 3. Parallel to 2, advances were made based on clinical observations by Bravais, Todd, Jackson, and Broca among others. 4. First known stimulation in conscious humans by Bartholow (1874) led to catastrophic outcomes. Horsley (1886) performed first intraoperative stimulation on Jackson's epileptic patient. 5. Advances accelerated in the first-half of the 20th century with Cushing (1909) performing first awake-craniotomy eliciting sensory responses to Penfield's work culminating in standardization of clinical use and generation of detailed maps including the famous sensory-motor homunculi. Parallel advances in instrumentation were made from the Leyden jar (1745) to present customizable current-controlled stimulators. Conclusions: ECS is commonly used in neurosurgery for localization of brain functions and is the benchmark for research studies. Significant leaps have been made since ECS first used in the 19th century. It evolved to remain the gold standard for localization of human brain functions in the 21st century.

Abstract:One hundred years ago, Augustus Desiré Waller recorded the human electrocardiogram for the first time, using a capillary electrometer. Electrocardiography only became clinically relevant in 1901 when Willem Einthoven devised his string galvanometer for this purpose. Sir Thomas Lewis was the key figure in showing the value of the electrocardiography for the diagnosis of disorders of cardiac rhythm and conduction, but many others amplified the technique to encompass the assessment of structural heart disease, especially when due to ischemia. Knowledge of their role and work gives us a better perspective when considering the development of surface electrocardiography.

Abstract:In the history of electrocardiography the names of two physiologists stand out: Augustus Waller (1865-1922) and Willem Einthoven (1860-1927). Waller was the first to show that the beating heart produces a weak electric potential, which can be registered by a measuring device connected to electrodes attached to the skin. Einthoven developed a 'string' galvanometer, which was much faster and more sensitive than the system used by Waller. Einthoven's electrocardiograph was ready for use in 1903. To facilitate investigations of patients Einthoven connected his instrument to the Academic Hospital in Leyden, by a telephone line, as suggested by his engineering colleague Johannes Bosscha in Delft. The first successful tele-electrocardiogram was transmitted on Sunday 22 March 1905. The heart tones were registered by wiring a specially developed microphone placed on the subject's chest to another string galvanometer. The event was therefore a first both for tele-electrocardiography and for telephonocardiography. We are still awaiting the full-scale implementation of these achievements, 100 years later.

Abstract:Investigations of structural and mechanical properties of cartilage tissue exposed to laser radiation have shown the effect of laser relaxation of the tension existing in the cartilage and changes in the latter form to be due to phasic transition of water from bound into free state. Laser radiation does not induce macroscopic transformations of the matrix structure. New form of the cartilage proved stable this offering new prospects for wide application of this technique in otorhinolaryngology and plastic surgery.

Abstract:The 1887 introduction of electrocardiography as a noninvasive investigative technique on man by Augustus Waller, and Wilhelm Einthoven's electrocardiograph from 1901 remain the two major landmarks in the 100-year history of electrocardiography. The progress in our knowledge of the mechanism and interpretation of the ECG has evolved throughout these 100 years in a succession of relatively small steps contributed by many unsung heroes, although often even relatively minor contributions by a few dominant "stars", notably Thomas Lewis and Frank Wilson, are perceived as giant leaps. The gradual evolution of our concepts of the ECG are reviewed against the historical background of the events involved from the time of Waller's discovery to the end of the second world war.

Abstract:Within electromyography (EMG), a particular specialty has been developed wherein the aim is to use EMG for the study of muscular function and co-ordination. This area of research is usually called kinesiological EMG. The general aims of kinesiological EMG are to analyse the function and co-ordination of muscles in different movements and postures, in healthy subjects as well as in the disabled, in skilled actions as well as during training, in humans as well as in animals, under laboratory conditions as well as during daily or vocational activities. This is often done by a combination of electromyographical and kinesiological or biomechanical measurement techniques. Because there are over 400 skeletal muscles in the human body and both irregular and complex involvement of the muscles may occur in neuromuscular diseases and in voluntary occupational or sports movements, it is impossible to sample all of the muscles of the entire body during the performance of complex motor skills. In addition, the measurement of kinesiological EMG in sport and specific field circumstances, such as the track and/or soccer field, the alpine ski slope, the swimming pool and the ice rink, demands a specific technological and methodological approach, adaptable to both the field and the sport circumstances. Sport movement techniques and skills, training approaches and methods, ergonomic verification of the human-machine interaction have, amongst others, a highly specialized muscular activity in common. The knowledge of such muscular action in all its aspects, its evaluation and its feedback should allow for the optimization of movement, of sports materials, of training possibilities and, in the end, of sports performance. Drawing conclusions from a review of the EMG research of 32 sports, covering over 100 different complex skills, including methodological approaches, is an impossible task. We have attempted to set standards concerning the EMG methodology at the beginning of this review. Electromyography and sports is a vast area and a complete review is impossible, as information will be found scattered in many different journals, including those on the sports sciences, ergonomics, biomechanics, applied physiology, in different congress proceedings, and so on. Consequently, many important aspects and possibly important publications may have been omitted from this review.

Abstract:In 1935, Kazumi Yamagiwa wrote a paper on “the origin of the Berger rhythm" at the Adrian's Lab. in Cambridge Univ. Although he was the first Japanese researcher contributing to early human EEG study, he could not continue EEG study in Japan because of the outbreak of the World War Ⅱ in 1941. During the World War Ⅱ, Koichi Motokawa and Seizo Katsumura had emphasized the importance of EEG research, and the Japan Society for Promotion of Science organized the first official EEG committee in 1942. After the World War Ⅱ, the Japanese Ministry of Education formed the national EEG committee in 1946. The members were consisted from eight National University of Medicine. In 1951, a Japanese company marketed a 2-channel ink-written EEG machine named “Mokusei-go". In 1952, the Japan EEG Society was established and domestic EEG machines were widely distributed during 1960s. The subcommittee of the Society “the brain death and EEG" made the first definition of brain death in Japan, i.e., total brain death including flat-EEG, in 1969. During 1970s, computer technology had developed and fundamental analyzing methods of EEG, EPs and ERPs became widely used. During 1980s, digitalization of and storage of EEG data to electric devices gained in popularity. During the years since then, a large number of EEG researchers was grown and the neurophysiology markedly flourished in Japan.
[The historical bases of a super-specialty: electrocardiography]

Abstract:In the XVIII century the first structured experiments in the field of bioelectricity were performed, and the Italian scientist Luigi Galvani documented the muscular contraction of a frog undergoing an electric shock. In 1791 he showed that the electric stimulation of the heart of a frog determined the contraction of the heart itself. In the first thirty years of the XIX century galvanometers were developed, and in 1842-42 Carlo Matteucci documented that electric activity was present even in the cardiac muscle at rest. At the end of the XIX century Augustus Waller was among the first scientists to publish an electrocardiographic recording obtained from the human body surface; most of his contemporaneous, however, did not retain that electrocardiography might have been an effective clinical application. Willem Einthoven, instead, was convinced of the widespread feasibility of clinical electrocardiography, and promoted a number of improvements and refinements in electrocardiographic technique. The most important and diagnostic-technical development of electrocardiography occurred in the second half of the XX century, and still today, even if many different sophisticated instrumental examinations are available for cardiologic evaluation, electrocardiography represents an essential first-line diagnostic tool in clinical cardiology.

Abstract:I was approached by the federal public defender’s office to perform a comprehensive psychiatric evaluation on a death row inmate. Before hiring me, the team of attorneys and paralegals interviewed me about my opinion of the death penalty. This seemed to be at least as important as my credentials. My first response to them on the telephone, even before the interview, was that I felt it was unethical, and in my view immoral, for a psychiatrist to evaluate death row inmates’ competency to be executed. I agreed to be interviewed in person when the attorney told me it was not a competency evaluation. The defending attorney informed me that his client might be mentally ill and it could prevent his execution if it were true. During the team interview, they told me that if my decision were unfavorable to their client, they would want it verbally so they would not have to share it with the state. They would then try another route of appeals. They made it abundantly clear that they were all opposed to the death penalty on moral grounds. I told them I would give my honest opinion about their client and would have no problem giving it verbally so it would not be discoverable. Although I said this, I actually felt excited about possibly saving a man from execution. I was not completely aware of it at the time, but in retrospect, my objectivity was surely affected. To what degree, I still don’t know. Abstract:This paper describes a computer system which accurately defines the EMG patterns of the lower extremities during gait. Footswitches are used to identify the temporal relationships and determine the phases of the gait cycle. Fine wire electrodes, inserted in the desired muscles of the patient being tested, provide EMG signals for comparison with a normal database. The system is also usable with surface electrodes when an appropriate normal database for surface electrodes is incorporated. Descriptive qualifiers (such as 'premature onset', 'delayed cessation', 'no clinically significant EMG', 'continuous activity' etc.) are used to produce a clinically relevant printed (textual) report. The intensity filtered average (IFA) of the EMG is shown graphically with the representative profile of each stride. The IFAs for all muscles tested can be plotted together (up to six on a page) and the graphic representation of the 'raw' EMG can be produced. The methods of generating the normal database by creating time-adjusted mean profiles (TAMP) are enumerated. The clinical use of the system is discussed. A detailed analysis of 31 of the most recent patient tests for which the system was used provides an indication of its accuracy. For 86% of the 428 muscle tests examined, the EMG analyser was considered to have given the correct result as compared with a visual analysis of the raw EMG record by a trained expert. Recommendations for the use and future improvements of the EMG analyser are made.

Abstract:Magnetoencephalography (MEG) plays a pivotal role in the diagnosis of brain disorders. In this review, we have investigated potential MEG applications for analysing brain disorders. The signal-to-noise ratio (SNRMEG =2.2 db, SNREEG <1 db) and spatial resolution (SRMEG =2–3 mm, SREEG =7–10 mm) is higher for MEG than EEG, thus MEG potentially facilitates accurate monitoring of cortical activity. We found that the direct electrophysiological MEG signals reflected the physiological status of neurological disorders and play a vital role in disease diagnosis. Single-channel connectivity, as well as brain network analysis, using MEG data acquired during resting state and a given task has been used for the diagnosis of neurological disorders such as epilepsy, Alzheimer's, Parkinsonism, autism, and schizophrenia. The workflow of MEG and its potential applications in the diagnosis of disease and therapeutic planning are also discussed. We forecast that computer-aided algorithms will play a prominent role in the diagnosis and prediction of neurological diseases in the future. The outcome of this narrative review will aid researchers to utilise MEG in diagnostics.

Abstract:In late 19th century Paris, people with epilepsy were treated alongside those with hysteria in the now famous Salpêtrière Hospital, where both conditions were deemed to have a neurological basis. When Jean Martin Charcot became chief physician at the Salpêtrière Hospital in 1862, he described himself 'in possession of a kind of museum of living pathology whose holdings were virtually inexhaustible'. He opened the doors of his 'living museum' and exhibited his prize specimens to all of Paris. By putting his patients on display, Charcot introduced a vogue for pathology that permeated well beyond the world of medical enquiry and into the public psyche and vernacular. Not only did Charcot's demonstrations provide the inspiration for high culture in the form of operas, plays and novels, they also provided the inspiration for the 'gommeuses epileptiques' (epileptic singers), who entertained the masses at the café concerts. This paper explores the foundations of our current medical approaches to mental illness and epilepsy, with a particular focus on the boundaries that emerged between hysteria and epilepsy in 19th century Paris. These clinical boundaries were both shaped by and reflected in the popular entertainments in the city.
Charcot and the psychology of hysteria, with special reference to a never published final case history

Abstract:Jean-Martin Charcot is perhaps best remembered for his contributions to organic neurology. However, his pursuit of hysteria, the most prevalent diagnosis in his hospital clinic, yielded no anatomical lesion to account for hysteria's plethora of somatic disorders assumed due to a purely functional or dynamic lesion in the cerebral cortex. This led Charcot to turn his attention to the psychology of hysteria. Taking advantage of institutional reforms at the Salpêtrière-notably, the establishment of his professorship in nervous diseases-Charcot from the early 1880s focused his teaching increasingly on case histories of hysteria in male as well as female patients. Already renown for his earlier dramatic public lessons on female hysteria, his lessons of the 1880s, of which two volumes were published at the end of the decade, elaborated the issue of psychology in terms of altered states of patient's suggestibility. By the decade's end, Charcot's worldwide reputation rested on the prospects of this work as acknowledged by numerous students, notably medical psychologists Pierre Janet and Sigmund Freud. Yet Charcot's views remained sketchy. They were discussed at length in his unpublished notes for a lesson intended for May 1893, just a few months before his sudden death. His unpublished notes reveal a detailed case for dreams as illustrating a psychological mechanism underlying hysteria in a 17-year-old Paris artisan. I conclude by considering why this significant climactic case of Charcot's might have been overlooked by his entourage.
