Abstract:Cochlear implant (CI) listeners have difficulty understanding speech in complex listening environments. This deficit is thought to be largely due to peripheral encoding problems arising from current spread, which results in wide peripheral filters. In normal hearing (NH) listeners, central processing contributes to segregation of speech from competing sounds. We tested the hypothesis that basic central processing abilities are retained in post-lingually deaf CI listeners, but processing is hampered by degraded input from the periphery. In eight CI listeners, we measured auditory nerve compound action potentials to characterize peripheral filters. Then, we measured psychophysical detection thresholds in the presence of multi-electrode maskers placed either inside (peripheral masking) or outside (central masking) the peripheral filter. This was intended to distinguish peripheral from central contributions to signal detection. Introduction of temporal asynchrony between the signal and masker improved signal detection in both peripheral and central masking conditions for all CI listeners. Randomly varying components of the masker created spectral-variance cues, which seemed to benefit only two out of eight CI listeners. Contrastingly, the spectral-variance cues improved signal detection in all five NH listeners who listened to our CI simulation. Together these results indicate that widened peripheral filters significantly hamper central processing of spectral-variance cues but not of temporal cues in post-lingually deaf CI listeners. As indicated by two CI listeners in our study, however, post-lingually deaf CI listeners may retain some central processing abilities similar to NH listeners.
Systematic review of compound action potentials as predictors for cochlear implant performance

Abstract:Objectives/hypothesis: The variability in speech perception between cochlear implant users is thought to result from the degeneration of the auditory nerve. Degeneration of the auditory nerve, histologically assessed, correlates with electrophysiologically acquired measures, such as electrically evoked compound action potentials (eCAPs) in experimental animals. To predict degeneration of the auditory nerve in humans, where histology is impossible, this paper reviews the correlation between speech perception and eCAP recordings in cochlear implant patients. Data sources: PubMed and Embase. Review methods: We performed a systematic search for articles containing the following major themes: cochlear implants, evoked potentials, and speech perception. Two investigators independently conducted title-abstract screening, full-text screening, and critical appraisal. Data were extracted from the remaining articles. Results: Twenty-five of 1,429 identified articles described a correlation between speech perception and eCAP attributes. Due to study heterogeneity, a meta-analysis was not feasible, and studies were descriptively analyzed. Several studies investigating presence of the eCAP, recovery time constant, slope of the amplitude growth function, and spatial selectivity showed significant correlations with speech perception. In contrast, neural adaptation, eCAP threshold, and change with varying interphase gap did not significantly correlate with speech perception in any of the identified studies. Conclusions: Significant correlations between speech perception and parameters obtained through eCAP recordings have been documented in literature; however, reporting was ambiguous. There is insufficient evidence for eCAPs as a predictive factor for speech perception. More research is needed to further investigate this relation. Laryngoscope, 2016 127:476-487, 2017.
Relationships between the auditory nerve's ability to recover from neural adaptation, cortical encoding of and perceptual sensitivity to within-channel temporal gaps in postlingually deafened adult cochlear implant users

Abstract:Objective: This study assessed the relationships between the auditory nerve's ability to recovery from neural adaptation, cortical encoding of and perceptual sensitivity to within-channel temporal gaps in postlingually deafened adult cochlear implant (CI) users. Design: Study participants included 11 postlingually deafened adults with Cochlear Nucleus devices, including three bilaterally implanted participants. In each participant, recovery from neural adaptation of the auditory nerve (AN) was measured using electrophysiological measures of the electrically evoked compound action potential (eCAP) at up to four electrode locations. The electrode pair with the largest difference in the speed of adaptation recovery was selected for assessing within-channel temporal gap detection threshold (GDT). GDTs were measured using both psychophysical and electrophysiological procedures. Psychophysical GDTs were evaluated using a three-alternative, forced-choice procedure, targeting 79.4% correct on the psychometric function. Electrophysiological GDTs were measured using the electrically evoked auditory change complex (eACC) evoked by electrical pulse trains that contained temporal gaps. Objective GDT was defined as the shortest temporal gap that could evoke an eACC. Related-samples Wilcoxon Signed Rank testwas used to compare psychophysical GDTs and objective GDTs measured at all electrode locations. It was also used to compare psychophysical GDTs and objective GDTs measured at the two electrode locations with different speeds or amounts of adaptation recovery of the AN. A Kendall Rank correlation test was used to assess the correlation between GDTs measured using psychophysical or electrophysiological procedures. Results: Objective GDTs were significantly larger than those measured using psychophysical procedures. There was a significant correlation between objective and psychophysical GDTs. GDTs could not be predicted based on the amount or the speed of adaptation recovery of the AN. Conclusions: Electrophysiological measure of the eACC can potentially be used to assess within-channel GDT in CI users who cannot provide reliable behavioral responses. The difference in adaptation recovery of the AN is not the primary factor accounting for the across-electrode variation in GDT in individual CI users.
Speech comprehension by cochlear implant users assessed with evoked potentials and response times

Abstract:Introduction: Cochlear implant (CI) users differ greatly in their rehabilitation outcomes, including speech understanding in noise. This variability may be related to brain changes associated with intact senses recruiting cortical areas from stimulation-deprived senses. Numerous studies have demonstrated such cross-modal reorganization in individuals with untreated hearing loss. How it is affected by regular use of hearing devices remains unclear, however. To shed light on this, the current study measured cortical responses reflecting comprehension abilities in experienced CI users and normal-hearing controls. Methods: Using multichannel electroencephalography, we tested CI users who had used their devices for at least 12 months and closely matched controls (N = 2 × 13). Cortical responses reflecting comprehension abilities – the N400 and late positive complex (LPC) components – were evoked using congruent and incongruent digit-triplet stimuli. The participants’ task was to assess digit-triplet congruency by means of timed button presses. All measurements were performed in speech-shaped noise 15 dB above individually measured speech recognition thresholds. Three stimulus presentation modes were used: auditory-only, visual-only, and visual-then-auditory. Results: The analyses revealed no group differences in the N400 and LPC responses. In terms of response times, the CI users were slower and differentially affected by the three stimulus presentation modes relative to the controls. Conclusion: Compared to normal-hearing controls, experienced CI users may need more time to comprehend speech in noise. Response times can serve as a proxy for speech comprehension by CI users.

Short- and long-latency components of the eCAP reveal different refractory properties

Abstract:Background: The refractory recovery function (RRF) measures the electrically evoked compound action potential (eCAP) in response to a second pulse (probe) after masking by a first pulse (masker). This RRF is usually used to assess the refractory properties of the electrically stimulated auditory nerve (AN) by recording the eCAP amplitude as a function of the masker probe interval. Instead of assessing eCAP amplitudes only, recorded waveforms can also be described as a combination of a short-latency component (S-eCAP) and a long-latency component (L-eCAP). It has been suggested that these two components originate from two different AN fiber populations with differing refractory properties. The main objective of this study was to explore whether the refractory characteristics revealed by S-eCAP, L-eCAP, and the raw eCAP (R-eCAP) differ from each other. For clinical relevance, we compared these refractory properties between children and adults and examined whether they are related to cochlear implant (CI) outcomes. Design: In this retrospective study, the raw RRF (R-RRF) was obtained from 121 Hi-Focus Mid-Scala or 1 J cochlear implant (Advanced Bionics, Valencia, CA) recipients. Each R-eCAP of the R-RRF was split into an S-eCAP and an L-eCAP using deconvolution to produce two new RRFs: S-RRF and L-RRF. The refractory properties were characterized by fitting an exponential decay function with three parameters: the absolute refractory period (T); the saturation level (A); and the speed of recovery from nerve refractoriness (τ), i.e., a measure of the relative refractory period. We compared the parameters of the R-RRF (RT, RA, Rτ) with those obtained from the S-RRF (ST, SA, Sτ) and L-RRF (LT, LA, Lτ) and investigated whether these parameters differed between children and adults. In addition, we examined the associations between these parameters and speech perception in adults with CI. Linear mixed modeling was used for the analyses. Results: We found that TR was significantly longer than ST and LT, and ST was significantly longer than LT. RA was significantly larger than SA and LA, and SA was significantly larger than LA. Also, Sτ was significantly longer in comparison to Rτ and Lτ, but no significant difference was found between Rτ and Lτ. Children presented a significantly larger SA and LA and a shorter RT in comparison to adults. Shorter Sτ was significantly associated with better speech perception in adult CI recipients, but other parameters were not. Conclusion: We demonstrated that the two components of the eCAP have different refractory properties and that these also differ from those of the R-eCAP. In comparison with the R-eCAP, the refractory properties derived from the S-eCAP and L-eCAP can reveal additional clinical implications in terms of the refractory difference between children and adults as well as speech performance after implantation. Thus, it is worthwhile considering the two components of the eCAP in the future when assessing the clinical value of the auditory refractory properties.

Abstract:One of the biggest challenges that face cochlear implant (CI) users is the highly variable hearing outcomes of implantation across patients. Since speech perception requires the detection of various dynamic changes in acoustic features (e.g., frequency, intensity, timing) in speech sounds, it is critical to examine the ability to detect the within-stimulus acoustic changes in CI users. The primary objective of this study was to examine the auditory event-related potential (ERP) evoked by the within-stimulus frequency changes (F-changes), one type of the acoustic change complex (ACC), in adult CI users, and its correlation to speech outcomes. Twenty-one adult CI users (29 individual CI ears) were tested with psychoacoustic frequency change detection tasks, speech tests including the Consonant-Nucleus-Consonant (CNC) word recognition, Arizona Biomedical Sentence Recognition in quiet and noise (AzBio-Q and AzBio-N), and the Digit-in-Noise (DIN) tests, and electroencephalographic (EEG) recordings. The stimuli for the psychoacoustic tests and EEG recordings were pure tones at three different base frequencies (0.25, 1, and 4 kHz) that contained a F-change at the midpoint of the tone. Results showed that the frequency change detection threshold (FCDT), ACC N1′ latency, and P2′ latency did not differ across frequencies ( p > 0.05). ACC N1′-P2 amplitude was significantly larger for 0.25 kHz than for other base frequencies ( p < 0.05). The mean N1′ latency across three base frequencies was negatively correlated with CNC word recognition ( r = −0.40, p < 0.05) and CNC phoneme ( r = −0.40, p < 0.05), and positively correlated with mean FCDT ( r = 0.46, p < 0.05). The P2′ latency was positively correlated with DIN ( r = 0.47, p < 0.05) and mean FCDT ( r = 0.47, p < 0.05). There was no statistically significant correlation between N1′-P2′ amplitude and speech outcomes (all ps > 0.05). Results of this study indicated that variability in CI speech outcomes assessed with the CNC, AzBio-Q, and DIN tests can be partially explained (approximately 16–21%) by the variability of cortical sensory encoding of F-changes reflected by the ACC.

Abstract:Auditory deprivation affects normal age-related changes in the central auditory maturation. Cochlear implants (CIs) have already become the best treatment strategy for severe to profound hearing impairment. However, it is still hard to evaluate the speech-language outcomes of the pediatric CI recipients because of hearing-impaired children with limited speech-language abilities. The cortical auditory evoked potential (CAEP) provides a window into the development of the auditory cortical pathways. This preliminary study is aimed at assessing electrophysical characteristics of P1-N1 of electrically CAEP in children with CIs and at exploring whether these changes could be accounted for in auditory and speech outcomes of these patients. CAEP responses were recorded in 48 children with CIs in response to electrical stimulus to determine the presence of the P1-N1 response. Speech perception and speech intelligibility of the implanted children were further evaluated with the categories of auditory performance (CAP) test and speech intelligibility rating (SIR) test, respectively, to explore the relationship between the latency of P1-N1 and auditory and speech performance. This study found that P1 and N1 of the intracochlear CAEP were reliably evoked in children fitted with CIs and that the latency of the P1 as opposed to that of N1 was negative in relation to the wearing time of the cochlear implant. Moreover, the latency of the P1 produced significantly negative scores in both CAP and SIR tests, which indicates that P1 latency may be reflective of the auditory performance and speech intelligibility of pediatric CI recipients. These results suggest that the latency of P1 could be used for the objective assessment of auditory and speech function evaluation in cochlear-implanted children, which would be helpful in clinical decision-making regarding intervention for young hearing-impaired children.

Abstract:Temporal modulation sensitivity has been studied extensively for Cochlear Implant (CI) users due to its strong correlation to speech recognition outcomes. Previous studies reported that temporal modulation detection thresholds (MDTs) vary across the tonotopic axis and attributed this variation to patchy neural survival. However, correlates of neural health identified in animal models depend on electrode position in humans. Nonetheless, the relationship between MDT and electrode location has not been explored. We tested 13 ears for the effect of distance on modulation sensitivity, specifically targeting the question of whether electrodes closer to the modiolus are universally beneficial. Participants in this study were postlingually deafened and users of Cochlear Nucleus CIs. The distance of each electrode from the medial wall (MW) of the cochlea and mid-modiolar axis (MMA) was measured from scans obtained using computerized tomography (CT) imaging. The distance measures were correlated with slopes of spatial tuning curves measured on selected electrodes to investigate if electrode position accounts, at least in part, for the width of neural excitation. In accordance with previous findings, electrode position explained 24% of the variance in slopes of the spatial tuning curves. All functioning electrodes were also measured for MDTs. Five ears showed a positive correlation between MDTs and at least one distance measure across the array; 6 ears showed negative correlations and the remaining two ears showed no relationship. The ears showing positive MDT-distance correlations, thus benefiting from electrodes being close to the neural elements, were those who performed better on the two speech recognition measures, i.e., speech reception thresholds (SRTs) and recognition of the AzBio sentences. These results could suggest that ears able to take advantage of the proximal placement of electrodes are likely to have better speech recognition outcomes. Previous histological studies of humans demonstrated that speech recognition is correlated with spiral ganglion cell counts. Alternatively, ears with good speech recognition outcomes may have good overall neural health, which is a precondition for close electrodes to produce spatially confined neural excitation patterns that facilitate modulation sensitivity. These findings suggest that the methods to reduce channel interaction, e.g., perimodiolar electrode array or current focusing, may only be beneficial for a subgroup of CI users. Additionally, it suggests that estimating neural survival preoperatively is important for choosing the most appropriate electrode array type (perimodiolar vs. lateral wall) for optimal implant function.
Peripheral neural status and auditory temporal resolution in cochlear implant users

Abstract:Auditory temporal resolution is important for understanding speech with a cochlear implant (CI). The aim of this study was to determine the relative contributions of peripheral neural synchrony and peripheral neural survival to perceptual temporal resolution in CI users. To date, study participants included 8 post-lingually deafened adult CI users. All participants used CochlearTM Nucleus® devices in their test ears. For each implanted ear tested in each participant, auditory temporal resolution, neural synchrony, and neural survival were evaluated at multiple electrode locations along the array. Auditory temporal resolution was estimated using single-channel, psychophysical gap detection thresholds (GDTs), peripheral neural synchrony was estimated using the phase-locking value (PLV), and peripheral neural survival was estimated using the sensitivity of the electrically-evoked compound action potential to changes in interphase gap (i.e., IPG effect). After adjusting for multiple comparisons, our preliminary results indicated that GDT was significantly correlated with the PLV but not with the IPG effect. The PLV and the IPG effect were not significantly correlated. These preliminary results suggested that the degree of peripheral neural synchrony appeared to play a more important role in determining auditory temporal resolution than peripheral neural survival. Direct Elicitation of Cortical Auditory Evoked Potentials by Electrical Stimulation and Their Use to Verify the Most Comfortable Level of Stimulation in Cochlear Implant Users

Abstract:Introduction: This study was designed to investigate the use of electrically evoked cortical auditory evoked potentials (eCAEPs) as a tool for cochlear implant (CI) verification, the relationships between the site and intensity of stimulation and the detection rates and morphologies of eCAEPs as well as investigate whether correlations exist between the morphologies of eCAEPs and speech perception in quiet and in noise, duration of hearing loss, age at implantation, whether the hearing loss bilateral or single-sided and the electrode current level required to elicit MCL stimulation. Methods: 32 adult unilateral CI users with postlingual hearing loss were enrolled. The stimuli were 1 kHz biphasic alternating pulses and were presented at either the behaviorally measured MCL or 50% of this value (MCL 0.5 ) via the CI fitting software. Pulses were directed to apical, medial, or basal electrodes. CAEPs were recorded from a scalp electrode placed at the vertex, low forehead, and contralateral mastoid and were evaluated by two electrophysiologists. Results: Overall, eCAEPs could be detected in 31/32 users when stimulating at MCL, and in 29/32 users when stimulating at MCL 0.5 . The detection rates were 31, 31, and 28/32 for apical, medial, and basal stimulation at MCL, and 29, 29, and 26/32 at MCL 0.5 . Significant differences in eCAEP amplitudes and latencies were observed across electrodes and stimulation levels. No significant correlations were found between eCAEP latencies and amplitudes and user age, duration of deafness prior to CI surgery, or with bilateral versus single-sided hearing loss, nor with the charge level required to elicit MCL, or with speech perception scores in quiet. Peak latencies correlated with speech perception scores in some configurations of speech-in-noise. Conclusion: eCAEPs can readily be elicited in the majority of adult CI users and show normal waveform characteristics at stimulation levels corresponding to MCL, as well as at basal, medial, and apical electrode stimulation sites. Neither the latencies nor amplitudes of eCAEPs are confounded by variables of age, duration of deafness prior to CI surgery, or the laterality of hearing loss. eCAEPs are a useful, objective method evaluate sound perception in CI users. Abstract:PURPOSE:The primary purpose of the present study was to investigate the relationship between postimplant electrically evoked auditory brainstem response (EABR) measures (wave V threshold, wave V latency, input-output functions) and auditory performance and speech intelligibility outcomes measured using parental rating scales, such as Categories of Auditory Performance (CAP; Archbold, Lutman, & Marshall, 1995) and the Speech Intelligibility Rating scale (SIR; Allen, Nikolopoulos, Dyar, & O'Donoghue, 2001), respectively. The secondary purpose was to evaluate the relationship between age at implantation and ratings on the CAP and SIR.METHOD:Forty children with congenital sensorineural hearing loss participated. Preimplant parental ratings on the CAP and SIR were obtained, and all the children underwent cochlear implantation. Intracochlear EABRs were recorded postimplantation. Postimplant parental ratings on the CAP and SIR were obtained. The relationships between EABR parameters and auditory performance and speech intelligibility outcomes were studied. The effect of age at implantation on auditory performance and speech intelligibility outcomes was also investigated.RESULTS:A significant negative correlation was found between EABR wave V thresholds and SIR growth (r = -.415, p = .016). Children with better CAP growths tended to have lower wave V thresholds than those with poorer CAP growths. Age at implantation had an effect on the auditory performance as measured using the CAP.CONCLUSIONS:The present study provides evidence for the relationship between wave V thresholds of the intracochlear EABR and auditory performance and speech intelligibility outcomes measured using parental rating scales in pediatric cochlear implant recipients. Data also indicate that early intervention has a positive impact on auditory performance outcomes.
Relationship Between the Ability to Detect Frequency Changes or Temporal Gaps and Speech Perception Performance in Post-lingual Cochlear Implant Users

Abstract:Previous studies, using modulation stimuli, on the relative effects of frequency resolution and time resolution on CI users' speech perception failed to reach a consistent conclusion. In this study, frequency change detection and temporal gap detection were used to investigate the frequency resolution and time resolution of CI users, respectively. Psychophysical and neurophysiological methods were used to simultaneously investigate the effects of frequency and time resolution on speech perception in post-lingual cochlear implant (CI) users. We investigated the effects of psychophysical results [frequency change detection threshold (FCDT), gap detection threshold (GDT)], and acoustic change complex (ACC) responses (evoked threshold, latency, or amplitude of ACC induced by frequency change or temporal gap) on speech perception [recognition rate of monosyllabic words, disyllabic words, sentences in quiet, and sentence recognition threshold (SRT) in noise]. Thirty-one adult post-lingual CI users of Mandarin Chinese were enrolled in the study. The stimuli used to induce ACCs to frequency changes were 800-ms pure tones (fundamental frequency was 1,000 Hz); the frequency change occurred at the midpoint of the tones, with six percentages of frequency changes (0, 2, 5, 10, 20, and 50%). Temporal silences with different durations (0, 5, 10, 20, 50, and 100 ms) were inserted in the middle of the 800-ms white noise to induce ACCs evoked by temporal gaps. The FCDT and GDT were obtained by two 2-alternative forced-choice procedures. The results showed no significant correlation between the CI hearing threshold and speech perception in the study participants. In the multiple regression analysis of the influence of simultaneous psychophysical measures and ACC responses on speech perception, GDT significantly predicted every speech perception index, and the ACC amplitude evoked by the temporal gap significantly predicted the recognition of disyllabic words in quiet and SRT in noise. We conclude that when the ability to detect frequency changes and the temporal gap is considered simultaneously, the ability to detect frequency changes may have no significant effect on speech perception, but the ability to detect temporal gaps could significantly predict speech perception.

Abstract:Cochlear implant (CI) recipients face great challenges in speech-in-noise recognition, partially due to the fact that only temporal envelopes from a limited number of bands are preserved in most CI signal processing strategies. In “n-of-m” strategies (e.g., the Advanced Combinational Encoder, ACE), the number of maxima (nmax) and electrical dynamic range (EDR) are two essential parameters that may affect the envelope representation and further influence speech perception. Speech recognition can be improved by optimizing parameter settings in CI programming. To investigate the effects of nmax and EDR on speech-in-noise perception, Mandarin speech reception thresholds (SRTs) in babble noise were measured in CI recipients using ACE. The nmax was set to 2, 4, 6, 8, and 16. The EDR was set to the base EDR (i.e., participants’ clinical EDR) and 50 % EDR (i.e., 50 %-compressed base EDR). Results showed that: 1) there was no significant interaction effect between nmax and EDR, 2) SRTs with nmax = 2, 4, and 16 were significantly higher (or worse) than those with moderate nmax (6–8), 3) narrower EDRs significantly lead to higher SRTs. Simulation experiments using a Gaussian-Enveloped Tones Vocoder in normal-hearing listeners were also conducted and provided both supportive and additional observations to the CI results. This study suggests that, in CI programming, nmax and EDR are two independent influencing factors. Large nmax (e.g., 16) is not recommended as it may harm speech intelligibility in noisy environments, and inaccurate EDR measurements should be avoided.

Abstract:Objectives: Adult cochlear implant (CI) recipients obtain varying levels of speech perception from their device. Adult CI users adapt quickly to their CI if they have no peripheral "bottom-up" or neurocognitive "top-down" limiting factors. Our objective here was to understand the influence of limiting factors on the progression of sentence understanding in quiet and in noise, initially and over time. We hypothesized that the presence of limiting factors, detected using a short test battery, would predictably influence sentence recognition with practical consequences. We aimed to validate the test battery by comparing the presence of limiting factors and the success criteria of >90% sentence understanding in quiet 1 month after activation. Design: The study was a single-clinic, cross-sectional, retrospective design incorporating 32 adult unilateral Nucleus CI users aged 27 to 90 years (mean = 70, SD = 13.5). Postoperative outcome was assessed through sentence recognition scores in quiet and in varying signal to noise ratios at 1 day, 1 to 2 months, and up to 2 years. Our clinic's standard test battery comprises physiological and neurocognitive measures. Physiological measures included electrically evoked compound action potentials for recovery function, spread of excitation, and polarity effect. To evaluate general cognitive function, inhibition, and phonological awareness, the Montreal Cognitive Assessment screening test, the Stroop Color-Word Test, and tests 3 and 4 of the French Assessment of Reading Skills in Adults over 16 years of age, respectively were performed. Physiological scores were considered abnormal, and therefore limiting, when total neural recovery periods and polarity effects, for both apical and basal electrode positions, were >1.65 SDs from the population mean. A spread of excitation of >6 electrode units was also considered limiting. For the neurocognitive tests, scores poorer than 1.65 SDs from published normal population means were considered limiting. Results: At 1 month, 13 out of 32 CI users scored ≥90% sentence recognition in quiet with no significant dependence on age. Subjects with no limiting peripheral or neurocognitive factors were 8.5 times more likely to achieve ≥90% score in quiet at 1 month after CI switch-on (p = 0.010). In our sample, we detected 4 out of 32 cases with peripheral limiting factors that related to neural health or poor electrode-neural interface at both apical and basal positions. In contrast, neurocognitive limiting factors were identified in 14 out of 32 subjects. Early sentence recognition scores were predictive of long-term sentence recognition thresholds in noise such that limiting factors appeared to be of continuous influence. Conclusions: Both peripheral and neurocognitive processing factors affect early sentence recognition after CI activation. Peripheral limiting factors may have been detected less often than neurocognitive limiting factors because they were defined using sample-based criteria versus normal population-based criteria. Early performance was generally predictive of long-term performance. Understanding the measurable covariables that limit CI performance may inform follow-up and improve counseling. A score of ≥90% for sentence recognition in quiet at 1 month may be used to define successful progress; whereas, lower scores indicate the need for diagnostic testing and ongoing rehabilitation. Our findings suggest that sentence test scores as early as 1 day after activation can provide vital information for the new CI user and indicate the need for rehabilitation follow-up.
A cortical biomarker of audibility and processing efficacy in children with single-sided deafness using a cochlear implant

Abstract:The goals of the current study were to evaluate audibility and cortical speech processing, and to provide insight into binaural processing in children with single-sided deafness (CHwSSD) using a cochlear implant (CI). The P1 potential to acoustically-presented speech stimuli (/m/, /g/, /t/) was recorded during monaural [Normal hearing (NH), CI], and bilateral (BIL, NH + CI) listening conditions within a clinical setting in 22 CHwSSD (mean age at CI/testing 4.7, 5.7 years). Robust P1 potentials were elicited in all children in the NH and BIL conditions. In the CI condition: (1) P1 prevalence was reduced yet was elicited in all but one child to at least one stimulus; (2) P1 latency was prolonged and amplitude was reduced, consequently leading to absence of binaural processing manifestations; (3) Correlation between P1 latency and age at CI/testing was weak and not significant; (4) P1 prevalence for /m/ was reduced and associated with CI manufacturer and duration of CI use. Results indicate that recording CAEPs to speech stimuli in clinical settings is feasible and valuable for the management of CHwSSD. While CAEPs provided evidence for effective audibility, a substantial mismatch in timing and synchrony of early-stage cortical processing between the CI and NH ear remains a barrier for the development of binaural interaction components.
Analysis of Long-Term Cochlear Implantation Outcomes and Correlation With Imaging Characteristics in Patients With Common Cavity Deformity

Abstract:Object To investigate the long-term development of auditory and speech in patients with common cavity deformity (CCD) after cochlear implantation (CI) and its relationship to imaging characteristics. Methods Twenty-three CCD patients and 59 age- and sex-matched CI children with normal inner ear structure were recruited. The auditory and speech development of these two groups were evaluated at 0, 1, 3, 6, 12, and 18 months after CI activation using four parent reports questionnaires [Categories of Auditory Performance (CAP), Speech Intelligibility Rating (SIR), Meaningful Auditory Integration Scale/Infant-Toddler Meaningful Auditory Integration Scale (MAIS/ITMAIS), and Meaningful Use of Speech Scale (MUSS)]. Computed tomography-based 3-dimensional reconstruction of the surgical side of 18 CCD children was performed, the volume and surface area were calculated. Correlation analysis was performed on the imaging performance and post-operative outcomes. Results The percentages of MAIS/IT-MAIS scores and CAP scores at different evaluation time points are significantly different ( p < 0.05). When comparing SIR results across time points, significant growth was observed in most of the comparisons. In addition, significant differences ( p < 0.05) are observed among the percentages of MUSS scores at different time points except the comparison between 0 and 1 month after CI activation. Patients in the CCD group had poorer auditory and speech performances at different stages after CI compared with those in the control group. According to the reconstruction of CCD patients, the volume ranged from 12.21 to 291.96 mm 3 ; the surface area ranged from 27.81 to 284.7 mm 2 . When the lumen surface area was <190.45 mm 2 or the volume was <157.91 mm 3 , the survival time for CCD children to achieve a CAP score of 4 after CI was significantly shorter. Conclusion Cochlear implantation are less effective in CCD patients than in patients with normal inner ear structures, but they can still achieve significant improvement post-operatively. The morphology and size of the inner ear vary in CCD patients, which reflects the degree of inner ear development influences the outcome after CI surgery.

Abstract:A multichannel cochlear implant (CI) is designed to take advantage of the tonotopic arrangement of auditory nerve fibers within a cochlea. The electrically evoked compound action potential (ECAP) is a safe and reliable objective measure for qualifying neural survival and local spatial resolution in the cochlea. The present study was aimed to explore the effect of recording position on the ECAP Amplitude Growth Function (AGF). The stimulating electrode (EL) was fixed at EL8, in the middle of the electrode array in 6 Clarion II or HiRes 90K subjects. The ECAP AGFs were obtained respectively from two recording positions (EL6 and EL10) using monopolar stimulation mode. The result showed that, in all subjects for both EL6 and EL10 recording positions, the ECAP is a linear function of stimuli level (Mean R2= 0.96, standard deviation =0.06). Moreover, there was a significant correlation of the ECAP AGFs recorded from EL6 and EL10. These results demonstrate the fundamentals of the electrical hearing and will be utilized in the clinical application for diagnosing neural degeneration of CI users.
