Abstract:BACKGROUND AND AIMS Bamboos have long-lived, evergreen leaves that continue to accumulate silica throughout their life. Silica accumulation has been suggested to suppress their photosynthetic activity. However, nitrogen content per unit leaf area (N(area)), an important determinant of maximum photosynthetic capacity per unit leaf area (P(max)), decreases as leaves age and senescence. In many species, P(max) decreases in parallel with the leaf nitrogen content. It is hypothesized that if silica accumulation affects photosynthesis, then P(max) would decrease faster than N(area), leading to a decrease in photosynthetic rate per unit leaf nitrogen (photosynthetic nitrogen use efficiency, PNUE) with increasing silica content in leaves. METHODS The hypothesis was tested in leaves of Sasa veitchii, which have a life span of 2 years and accumulate silica up to 41 % of dry mass. Seasonal changes in P(max), stomatal conductance, N(area) and silica content were measured for leaves of different ages. KEY RESULTS Although P(max) and PNUE were negatively related with silica content across leaves of different ages, the relationship between PNUE and silica differed depending on leaf age. In second-year leaves, PNUE was almost constant although there was a large increase in silica content, suggesting that leaf nitrogen was a primary factor determining the variation in P(max) and that silica accumulation did not affect photosynthesis. PNUE was strongly and negatively correlated with silica content in third-year leaves, suggesting that silica accumulation affected photosynthesis of older leaves. CONCLUSIONS Silica accumulation in long-lived leaves of bamboo did not affect photosynthesis when the silica concentration of a leaf was less than 25 % of dry mass. Silica may be actively transported to epidermal cells rather than chlorenchyma cells, avoiding inhibition of CO2 diffusion from the intercellular space to chloroplasts. However, in older leaves with a larger silica content, silica was also deposited in chlorenchyma cells, which may relate to the decrease in PNUE.

Abstract:The primary objective of this research is to create a reliable technique to determine whether a patient has glioma, a specific kind of brain tumour, by examining various diagnostic markers, using a variety of machine learning as well as deep learning approaches, and involving XAI (explainable artificial intelligence) methods. Through the integration of patient data, including medical records, genetic profiles, algorithms using machine learning have the ability to predict how each individual will react to different medical interventions. To guarantee regulatory compliance and inspire confidence in AI-driven healthcare solutions, XAI is incorporated. Machine learning methods employed in this study includes Random Forest, decision trees, logistic regression, KNN, Adaboost, SVM, Catboost, LGBM classifier, and Xgboost whereas the deep learning methods include ANN and CNN. Four alternative XAI strategies, including SHAP, Eli5, LIME, and QLattice algorithm, are employed to comprehend the predictions of the model. The Xgboost, a ML model achieved accuracy, precision, recall, f1 score, and AUC of 88%, 82%, 94%, 88%, and 92%, respectively. The best characteristics according to XAI techniques are IDH1, Age at diagnosis, PIK3CA, ATRX, PTEN, CIC, EGFR and TP53. By applying data analytic techniques, the objective is to provide healthcare professionals with practical tool that enhances their capacity for decision-making, enhances resource management, and ultimately raises the bar for patient care. Medical experts can customise treatments and improve patient outcomes by taking into account patient's particular characteristics. XAI provides justifications to foster faith amongst patients and medical professionals who must rely on AI-assisted diagnosis and treatment recommendations.

An Explainable AI Framework for Artificial Intelligence of Medical Things

Abstract:The healthcare industry has been revolutionized by the convergence of Artificial Intelligence of Medical Things (AIoMT), allowing advanced data-driven solutions to improve healthcare systems. With the increasing complexity of Artificial Intelligence (AI) models, the need for Explainable Artificial Intelligence (XAI) techniques become paramount, particularly in the medical domain, where transparent and interpretable decision-making becomes crucial. Therefore, in this work, we leverage a custom XAI framework, incorporating techniques such as Local Interpretable Model-Agnostic Explanations (LIME), SHapley Additive exPlanations (SHAP), and Gradient-weighted Class Activation Mapping (Grad-Cam), explicitly designed for the domain of AIoMT. The proposed framework enhances the effectiveness of strategic healthcare methods and aims to instill trust and promote understanding in AI-driven medical applications. Moreover, we utilize a majority voting technique that aggregates predictions from multiple convolutional neural networks (CNNs) and leverages their collective intelligence to make robust and accurate decisions in the healthcare system. Building upon this decision-making process, we apply the XAI framework to brain tumor detection as a use case demonstrating accurate and transparent diagnosis. Evaluation results underscore the exceptional performance of the XAI framework, achieving high precision, recall, and F1 scores with a training accuracy of 99% and a validation accuracy of 98%. Combining advanced XAI techniques with ensemble-based deep-learning (DL) methodologies allows for precise and reliable brain tumor diagnoses as an application of AIoMT.

Abstract:113 Background: In recent years, machine learning algorithms for survival analysis have been developed as an alternative to traditional Cox regression. Despite the good performances, these are viewed as black box models that lack interpretability, which may limit their clinical applications. There has been a growing interest in explaining and interpreting machine learning models, which has led to the emergence of the field of explainable artificial intelligence (XAI). However, few studies have focused on how these methods could be of benefit to survival analysis of real-world healthcare data. Methods: In this study, we explored the use of current XAI techniques to analyze the effects of platinum doublet chemotherapy on small cell lung cancer patients in developed machine learning models.The analyzed data included real-world patients treated at Karolinska University Hospital (n=570) and three phase III randomized clinical trials shared by the Project Data Sphere initiative (n=987). The real-world data covariates included were age, sex, TNM staging, ECOG performance status, lab values, brain metastasis, and concomitant radiotherapy. The aggregated dataset including the clinical trials contained the following variables: age, sex, performance status, brain metastasis, and protocol violations. Eight machine learning models were trained and compared with Cox regression. The performance of the models was evaluated using C-index, and the time variation of Brier Score and C/D AUC.Temporal feature importance and partial dependence were used to explore the overall covariate impact on overall survival (Global XAI). Ceteris-paribus and SurvShap(t) were used to investigate the covariate impact for the single patient prediction (local XAI). The models were firstly trained only on real-world data before aggregation with the clinical trials. Results: Ensemble machine learning provided the best performances. XAI techniques showed the potential to increase explainability of survival predictions in function of time. Global XAI showed the time range of the model reliability, trend inversions regarding treatment decisions and covariate importance along the time. Local XAI allowed to test the impact of covariates between long survival patients and the comparison between real-world and clinical trials. Conclusions: Our results demonstrate the potential of XAI techniques applied to survival machine learning and real-world data, thus providing insights into the mechanisms driving model predictions and demonstrate the utility of this approach in clinical research.

Abstract:e23134 Background: Precision oncology promises tailored cancer treatments, yet current methods often overlook individual patient variability. The AI-powered Case Matching Assistant (ACMA) system addresses this by utilizing real-world data (RWD) from over 100,000 clinical cases to personalize treatment strategies, considering genetic profiles, cancer types, treatment stages, and drug interactions. Methods: ACMA enhances the Large Language Model (LLM) with oncology-specific pretraining that encompasses both molecular and clinical data, ensuring a holistic understanding of patient profiles. It employs cosine similarity for multi-faceted case comparisons, integrating genetic, biomarker, and vital clinical information. Context-aware learning prompts fine-tune the LLM, generating recommendations that reflect the full spectrum of patient-specific data, from genetic variations to clinical histories and treatment responses, facilitating truly personalized treatment pathways. Results: Preliminary results show ACMA's tailored recommendations improve patient outcomes, with increased median survival rates and cost-effective treatment options. The system's 90% relevance match for clinical trial enrollment also expands access to novel therapies. In the absence of a standard treatment protocol for BRCA germline mutations in advanced lung cancer, ACMA provides crucial insights. For instance, a 36-year-old female with BRCA1 c.527del mutation achieved complete remission with a PFS of 38 months after a combination therapy trial. A 56-year-old male with a BRCA2 S1722Yfs*mutation saw partial remission and over 6 months PFS with olaparib. Another male patient, a smoker with concurrent BRCA2 G1712X and TP53 mutations, experienced partial remission following apatinib therapy. ACMA analyzed treatment data from similar genetic profiles and identified a targeted therapy that had shown efficacy in comparable cases. The oncologist used this information as a reference to adjust the treatment plan, resulting in a positive clinical outcome for the patient. This example underscores the potential of ACMA to provide actionable insights for personalized cancer care. Conclusions: Surpassing traditional single-feature matching, ACMA rapidly aligns multi-dimensional characteristics such as cancer type, disease stage, molecular data, and treatment phases. It customizes the interplay of these features with medical logic, delivering tailored outputs through the LLM. This approach offers an economical, convenient, and timely solution, setting a new benchmark in personalized oncology and establishing ACMA as a leader in the field. This distinctive advantage not only enhances patient outcomes but also establishes a formidable barrier to entry for competitors, solidifying ACMA's position at the forefront of oncology practice.

Abstract:In recent years, artificial intelligence (AI) has shown great promise in medicine. However, explainability issues make AI applications in clinical usages difficult. Some research has been conducted into explainable artificial intelligence (XAI) to overcome the limitation of the black-box nature of AI methods. Compared with AI techniques such as deep learning, XAI can provide both decision-making and explanations of the model. In this review, we conducted a survey of the recent trends in medical diagnosis and surgical applications using XAI. We have searched articles published between 2019 and 2021 from PubMed, IEEE Xplore, Association for Computing Machinery, and Google Scholar. We included articles which met the selection criteria in the review and then extracted and analyzed relevant information from the studies. Additionally, we provide an experimental showcase on breast cancer diagnosis, and illustrate how XAI can be applied in medical XAI applications. Finally, we summarize the XAI methods utilized in the medical XAI applications, the challenges that the researchers have met, and discuss the future research directions. The survey result indicates that medical XAI is a promising research direction, and this study aims to serve as a reference to medical experts and AI scientists when designing medical XAI applications.

Abstract:The use of artificial intelligence (AI) systems in biomedical and clinical settings can disrupt the traditional doctor–patient relationship, which is based on trust and transparency in medical advice and therapeutic decisions. When the diagnosis or selection of a therapy is no longer made solely by the physician, but to a significant extent by a machine using algorithms, decisions become nontransparent. Skill learning is the most common application of machine learning algorithms in clinical decision making. These are a class of very general algorithms (artificial neural networks, classifiers, etc.), which are tuned based on examples to optimize the classification of new, unseen cases. It is pointless to ask for an explanation for a decision. A detailed understanding of the mathematical details of an AI algorithm may be possible for experts in statistics or computer science. However, when it comes to the fate of human beings, this “developer’s explanation” is not sufficient. The concept of explainable AI (XAI) as a solution to this problem is attracting increasing scientific and regulatory interest. This review focuses on the requirement that XAIs must be able to explain in detail the decisions made by the AI to the experts in the field.
Insights from explainable AI in oesophageal cancer team decisions

Abstract:Background: Clinician-led quality control into oncological decision-making is crucial for optimising patient care. Explainable artificial intelligence (XAI) techniques provide data-driven approaches to unravel how clinical variables influence this decision-making. We applied global XAI techniques to examine the impact of key clinical decision-drivers when mapped by a machine learning (ML) model, on the likelihood of receiving different oesophageal cancer (OC) treatment modalities by the multidisciplinary team (MDT). Methods: Retrospective analysis of 893 OC patients managed between 2010 and 2022 at our tertiary unit, used a random forests (RF) classifier to predict four possible treatment pathways as determined by the MDT: neoadjuvant chemotherapy followed by surgery (NACT + S), neoadjuvant chemoradiotherapy followed by surgery (NACRT + S), surgery-alone, and palliative management. Variable importance and partial dependence (PD) analyses then examined the influence of targeted high-ranking clinical variables within the ML model on treatment decisions as a surrogate model of the MDT decision-making dynamic. Results: Amongst guideline-variables known to determine treatments, such as Tumour-Node-Metastasis (TNM) staging, age also proved highly important to the RF model (16.1 % of total importance) on variable importance analysis. PD subsequently revealed that predicted probabilities for all treatment modalities change significantly after 75 years (p < 0.001). Likelihood of surgery-alone and palliative therapies increased for patients aged 75-85yrs but lowered for NACT/NACRT. Performance status divided patients into two clusters which influenced all predicted outcomes in conjunction with age. Conclusion: XAI techniques delineate the relationship between clinical factors and OC treatment decisions. These techniques identify advanced age as heavily influencing decisions based on our model with a greater role in patients with specific tumour characteristics. This study methodology provides the means for exploring conscious/subconscious bias and interrogating inconsistencies in team-based decision-making within the era of AI-driven decision support.
Explainable Artificial Intelligence Reveals Novel Insight into Tumor Microenvironment Conditions Linked with Better Prognosis in Patients with Breast Cancer

Abstract:We investigated the data-driven relationship between immune cell composition in the tumor microenvironment (TME) and the ≥5-year survival rates of breast cancer patients using explainable artificial intelligence (XAI) models. We acquired TCGA breast invasive carcinoma data from the cbioPortal and retrieved immune cell composition estimates from bulk RNA sequencing data from TIMER2.0 based on EPIC, CIBERSORT, TIMER, and xCell computational methods. Novel insights derived from our XAI model showed that B cells, CD8+ T cells, M0 macrophages, and NK T cells are the most critical TME features for enhanced prognosis of breast cancer patients. Our XAI model also revealed the inflection points of these critical TME features, above or below which ≥5-year survival rates improve. Subsequently, we ascertained the conditional probabilities of ≥5-year survival under specific conditions inferred from the inflection points. In particular, the XAI models revealed that the B cell fraction (relative to all cells in a sample) exceeding 0.025, M0 macrophage fraction (relative to the total immune cell content) below 0.05, and NK T cell and CD8+ T cell fractions (based on cancer type-specific arbitrary units) above 0.075 and 0.25, respectively, in the TME could enhance the ≥5-year survival in breast cancer patients. The findings could lead to accurate clinical predictions and enhanced immunotherapies, and to the design of innovative strategies to reprogram the breast TME.

Abstract:The continuous development of artificial intelligence (AI) theory has propelled this field to unprecedented heights, owing to the relentless efforts of scholars and researchers. In the medical realm, AI takes a pivotal role, leveraging robust machine learning (ML) algorithms. AI technology in medical imaging aids physicians in X-ray, computed tomography (CT) scans, and magnetic resonance imaging (MRI) diagnoses, conducts pattern recognition and disease prediction based on acoustic data, delivers prognoses on disease types and developmental trends for patients, and employs intelligent health management wearable devices with human-computer interaction technology to name but a few. While these well-established applications have significantly assisted in medical field diagnoses, clinical decision-making, and management, collaboration between the medical and AI sectors faces an urgent challenge: How to substantiate the reliability of decision-making? The underlying issue stems from the conflict between the demand for accountability and result transparency in medical scenarios and the black-box model traits of AI. This article reviews recent research grounded in explainable artificial intelligence (XAI), with an emphasis on medical practices within the visual, audio, and multimodal perspectives. We endeavour to categorise and synthesise these practices, aiming to provide support and guidance for future researchers and healthcare professionals.

Abstract:Methodological use of explainable artificial intelligence for the comprehension of the predictions generated by a machine learning models in drug discovery via visualization strategies. Artificial intelligence (AI) is having a growing impact in many areas related to drug discovery. However, it is still critical for their adoption by the medicinal chemistry community to achieve models that, in addition to achieving high performance in their predictions, can be trusty explained to the end users in terms of their knowledge and background. Therefore, the investigation and development of explainable artificial intelligence (XAI) methods have become a key topic to address this challenge. For this reason, a comprehensive literature review about explanation methodologies for AI based models, focused in the field of drug discovery, is provided. In particular, an intuitive overview about each family of XAI approaches, such as those based on feature attribution, graph topologies, or counterfactual reasoning, oriented to a wide audience without a strong background in the AI discipline is introduced. As the main contribution, we propose a new taxonomy of the current XAI methods, which take into account specific issues related with the typical representations and computational problems study in the design of molecules. Additionally, we also present the main visualization strategies designed for supporting XAI approaches in the chemical domain. We conclude with key ideas about each method category, thoroughly providing insightful analysis about the guidelines and potential benefits of their adoption in medical chemistry. Abstract:Explainable Artificial Intelligence (XAI) is an emerging research topic of machine learning aimed at unboxing how AI systems' black-box choices are made. This research field inspects the measures and models involved in decision-making and seeks solutions to explain them explicitly. Many of the machine learning algorithms can not manifest how and why a decision has been cast. This is particularly true of the most popular deep neural network approaches currently in use. Consequently, our confidence in AI systems can be hindered by the lack of explainability in these black-box models. The XAI becomes more and more crucial for deep learning powered applications, especially for medical and healthcare studies, although in general these deep neural networks can return an arresting dividend in performance. The insufficient explainability and transparency in most existing AI systems can be one of the major reasons that successful implementation and integration of AI tools into routine clinical practice are uncommon. In this study, we first surveyed the current progress of XAI and in particular its advances in healthcare applications. We then introduced our solutions for XAI leveraging multi-modal and multi-centre data fusion, and subsequently validated in two showcases following real clinical scenarios. Comprehensive quantitative and qualitative analyses can prove the efficacy of our proposed XAI solutions, from which we can envisage successful applications in a broader range of clinical questions.

As Artificial Intelligence (AI) technologies continue to permeate the healthcare sector, particularly in oncology, the need for transparent and interpretable models becomes imperative to enhance clinical decision-making and patient care. This review discusses the integration of various XAI approaches, such as SHAP, LIME, Grad-CAM, and others, with machine learning and deep learning models utilized in breast cancer detection and classification. By investigating the modalities of breast cancer datasets, including mammograms, ultrasounds and their processing with AI, the paper highlights how XAI can lead to more accurate diagnoses and personalized treatment plans. It also examines the challenges in implementing these techniques and the importance of developing standardized metrics for evaluating XAI's effectiveness in clinical settings. Through detailed analysis and discussion, this article aims to highlight the potential of XAI in bridging the gap between complex AI models and practical healthcare applications, thereby fostering trust and understanding among medical professionals and improving patient outcomes.

Abstract:Background and objective The value of implementing artificial intelligence (AI) on ultrasound screening for thyroid cancer has been acknowledged, with numerous early studies confirming AI might help physicians acquire more accurate diagnoses. However, the black box nature of AI's decision-making process makes it difficult for users to grasp the foundation of AI's predictions. Furthermore, explainability is not only related to AI performance, but also responsibility and risk in medical diagnosis. In this paper, we offer Explainer, an intrinsically explainable framework that can categorize images and create heatmaps highlighting the regions on which its prediction is based. Methods A dataset of 19341 thyroid ultrasound images with pathological results and physician-annotated TI-RADS features is used to train and test the robustness of the proposed framework. Then we conducted a benign-malignant classification study to determine whether physicians perform better with the assistance of an explainer than they do alone or with Gradient-weighted Class Activation Mapping (Grad-CAM). Results Reader studies show that the Explainer can achieve a more accurate diagnosis while explaining heatmaps, and that physicians' performances are improved when assisted by the Explainer. Case study results confirm that the Explainer is capable of locating more reasonable and feature-related regions than the Grad-CAM. Conclusions The Explainer offers physicians a tool to understand the basis of AI predictions and evaluate their reliability, which has the potential to unbox the "black box" of medical imaging AI.

Abstract:Explainable artificial intelligence (XAI) has experienced a vast increase in recognition over the last few years. While the technical developments are manifold, less focus has been placed on the clinical applicability and usability of systems. Moreover, not much attention has been given to XAI systems that can handle multimodal and longitudinal data, which we postulate are important features in many clinical workflows. In this study, we review, from a clinical perspective, the current state of XAI for multimodal and longitudinal datasets and highlight the challenges thereof. Additionally, we propose the XAI orchestrator, an instance that aims to help clinicians with the synopsis of multimodal and longitudinal data, the resulting AI predictions, and the corresponding explainability output. We propose several desirable properties of the XAI orchestrator, such as being adaptive, hierarchical, interactive, and uncertainty-aware.

Abstract:With the power of parallel processing, large datasets,and fast computational resources, deep neural networks (DNNs) have outperformed highly trained and experienced human experts in medical applications. However, the large global community of healthcare professionals, many of whom routinely face potentially life-or-death outcomes with complex medicolegal consequences, have yet to embrace this powerful technology. The major problem is that most current AI solutions function as a metaphorical black-box positioned between input data and output decisions without a rigorous explanation for their internal processes. With the goal of enhancing trust and improving acceptance of AI-based technology in clinical medicine, there is a large and growing effort to address this challenge using eXplainable AI (XAI), a set of techniques, strategies, and algorithms with an explicit focus on explaining the “hows and whys” of DNNs. Here, we provide a comprehensive review of the state-of-the-art XAI techniques concerning healthcare applications and discuss current challenges and future directions. We emphasize the strengths and limitations of each category, including image, tabular, and textual explanations, and explore a range of evaluation metrics for assessing the effectiveness of XAI solutions. Finally, we highlight promising opportunities for XAI research to enhance the acceptance of DNNs by the healthcare community.

Leveraging State-of-the-Art AI Algorithms in Personalized Oncology: From Transcriptomics to Treatment

Abstract:Background: Continuous breakthroughs in computational algorithms have positioned AI-based models as some of the most sophisticated technologies in the healthcare system. AI shows dynamic contributions in advancing various medical fields involving data interpretation and monitoring, imaging screening and diagnosis, and treatment response and survival prediction. Despite advances in clinical oncology, more effort must be employed to tailor therapeutic plans based on each patient's unique transcriptomic profile within the precision/personalized oncology frame. Furthermore, the standard analysis method is not compatible with the comprehensive deciphering of significant data streams, thus precluding the prediction of accurate treatment options. Methodology: We proposed a novel approach that includes obtaining different tumour tissues and preparing RNA samples for comprehensive transcriptomic interpretation using specifically trained, programmed, and optimized AI-based models for extracting large data volumes, refining, and analyzing them. Next, the transcriptomic results will be scanned against an expansive drug library to predict the response of each target to the tested drugs. The obtained target-drug combination/s will be then validated using in vitro and in vivo experimental models. Finally, the best treatment combination option/s will be introduced to the patient. We also provided a comprehensive review discussing AI models' recent innovations and implementations to aid in molecular diagnosis and treatment planning. Results: The expected transcriptomic analysis generated by the AI-based algorithms will provide an inclusive genomic profile for each patient, containing statistical and bioinformatics analyses, identification of the dysregulated pathways, detection of the targeted genes, and recognition of molecular biomarkers. Subjecting these results to the prediction and pairing AI-based processes will result in statistical graphs presenting each target's likely response rate to various treatment options. Different in vitro and in vivo investigations will further validate the selection of the target drug/s pairs. Conclusions: Leveraging AI models will provide more rigorous manipulation of large-scale datasets on specific cancer care paths. Such a strategy would shape treatment according to each patient's demand, thus fortifying the avenue of personalized/precision medicine. Undoubtedly, this will assist in improving the oncology domain and alleviate the burden of clinicians in the coming decade.
