Abstract:In this paper, we report on research findings from a cross-sectional survey with 143 primarily Mexican migrant agricultural worker respondents in British Columbia (BC), Canada. Participants reported high rates of experiences of threats and violence by employers, limited faith in the follow-through of both Canadian and country-of-origin authorities when reporting concerns, and a unanimous lack of knowledge in how to file a claim of a legal matter (e.g., housing, human rights violation). Most participants also reported that they believed they would receive poorer health care in relation to their Canadian counterparts and that their privacy would not be protected. While certain indicators, such as knowledge of resources for transportation, translation, and legal advocacy were higher than previous research would suggest, most participants did not feel confident that more serious issues would be addressed if they sought help. Our results suggest migrant workers in BC report similar, or even higher, rates of experiences and expectations of poor social support, legal protection, and health care in comparison to prior research in this region and elsewhere. While further research would be required to confirm this hypothesis, the impact of COVID-19 on this population is undeniable. Our findings highlight the need for greater regional and provincial commitments to fund targeted services for migrant agricultural workers that address the unique barriers they face. Additionally, greater attention and funding must be dedicated to supporting this population to navigate and access services that already exist. Together, dedicated initiatives could make a major difference for this workforce. Federal investments in support services of this nature would ensure the sustainability of such efforts. In addition, reforms to temporary migrant agricultural programs, such as open work permits and immediate access to permanent residence, would better afford workers opportunities to access the rights and protections that are currently out of reach for many.
Farmworker Mobility and COVID-19 Vaccination Strategies: Yuma County, Arizona, 2021

Abstract:Farmworkers, a group of essential workers, experience a disproportionately high burden of COVID-19 due to their living and working conditions. This project characterized farmworker mobility in and around Yuma County, Arizona, to identify opportunities to improve farmworker access to COVID-19 vaccination. We collected qualitative and geospatial data through a series of in-person and virtual focus group discussions, key informant interviews, and intercept interviews with participatory mapping. Participants included farmworkers, employers, and representatives of local institutions who serve or interact with farmworkers. We identified participants through purposive and referential sampling and grouped people by sociodemographic characteristics for interviews. We used qualitative and geospatial analyses to identify common themes and mobility patterns. The team interviewed 136 people from February 26 to April 2, 2021. Common themes emerged about how farmworkers have little or no access to COVID-19 vaccination unless offered at their workplaces or at locations where they congregate at convenient times. Further, farmworkers described how their demanding work schedules, long commute times, and caretaker commitments make it challenging to access vaccination services. Geospatial analyses identified three geographic areas in Yuma County where farmworkers reported living and working that did not have a COVID-19 vaccine clinic within walking distance. Coordination between local public health authorities and key partners, including employers and trusted representatives from local community-based organizations or the Mexican consulate, to offer vaccination at worksites or other locations where farmworkers congregate can help improve access to COVID-19 vaccines and booster doses for this population.

Abstract:Background Essential workers across multiple industries faced a disproportionate burden of morbidity and mortality during the COVID‐19 pandemic. The effects were especially severe for agricultural and food processing workers, many of whom are members of the structurally vulnerable Latine community. Under current U.S. federal laws, children under 12 years old can legally work in agriculture, one of the most hazardous U.S. industries. Many of these working youth are Latine and experience health and educational inequities. Methods Using a community‐based participatory research approach and a qualitative design, we conducted semi‐structured in‐depth interviews in North Carolina in 2022, with service providers in health, education, and advocacy fields (n = 10) and Latine youth farmworkers aged 10–17 (n = 24). We used participatory qualitative analysis methods and a reflexive thematic analysis to understand and describe the work experiences of these youth during the first 2 years of the pandemic. Results Amidst precarious economic and school situations exacerbated by the COVID‐19 pandemic, many youths described an increased need to work to support their families. While aware and sometimes fearful of the added occupational health risks of COVID‐19, youth and their families felt they had few other options. Service providers and youth described minimal employer‐provided safety protocols or equipment, yet some workers organized their own attempts at safety protocols. Youth narratives imply limited knowledge of basic workplace safety requirements. Conclusions Study findings emphasize the urgent need to address structural vulnerabilities shaping workplace policies and norms to protect Latine youth farmworkers to support their healthy development.

Social Vulnerabilities Among Hired Migrant and Seasonal Farmworkers During COVID-19: The Need for Employment-Based Indicators

Abstract:Objectives: This study aimed to explore farmworkers' experiences of social vulnerability during the peak period of the COVID-19 pandemic. Methods: Telephone surveys of 63 migrant and seasonal farmworkers across three major agricultural areas in Florida were conducted. The survey, designed and disseminated with critical support from a statewide farmworker membership and advocacy organization, included items related to social and occupational precarity and a suite of demographic conditions, including specific employment-based indicators and categories. Data were analyzed with SPSS using a series of statistical significance tests including Pearson's Chi-Square, Fisher's Exact, T-tests, and Mann-Whitney U. An open-ended question regarding employment precarity was also analyzed for frequencies of responses pertaining to a set of descriptive categories. Results: Survey findings demonstrated a high degree of social vulnerability among the farmworker sample, with notable variation in the type and severity of vulnerability and risk exposure across employment-based indicators and occupational categories. For example, a cross-industry comparison between vegetable field workers and greenhouse nursery workers revealed a disparity in COVID-exposure risk through commuting characteristics, as 43% of vegetable field workers used shared, employer-provided transport, while 68% of nursery workers used personal vehicles. Conclusion: While previous research has broadly established the extreme precarity of migrant and seasonal farmworkers during the peak COVID-19 period, the variability of experience, exposure to risk, and social vulnerability between farmworkers representing distinct employment-based indicators and occupational categories demonstrated in this study contributes to widening awareness of the importance of assessing farmworker experiences at a more granular level. In addition to delineating social vulnerability across key demographic categories, cross-industry comparisons between farmworkers revealed significant discrepancies in risk and vulnerability to COVID-19. Future research that further explores this variability may reveal opportunities to improve disaster-relief planning and mitigate social vulnerability in future disaster scenarios. The importance of surveying the vulnerability of worker populations, aside from geographic communities, is highlighted.
The Intersection of Workplace and Environmental Exposure on Health in Latinx Farm Working Communities in Rural Inland Southern California

Abstract:Workplace and environmental exposures pose health risks for racial/ethnic minorities in rural agricultural communities, placing them at a disadvantage in accessing needed health care. Over three fourths (76%) of the 2.4 million farmworkers in the United States are immigrants, mostly from Mexico. However, little is known of the community health concerns and barriers to care of Latinx farmworkers in inland southern California. This qualitative study used a community-based participatory research approach, conducting nine in-home meetings to obtain meaningful community input on health concerns and barriers to access healthcare services among rural residents of the Eastern Coachella Valley, who are also located near the desert-bound Salton Sea of inland southern California. All interviews were audio-recorded and analyzed via listening to the audio recordings and summarizing data in templates and matrices. Participants discussed health concerns related to agricultural labor, including heat-related illness, musculoskeletal ailments and injuries, skin disorders, respiratory illness, and trauma. Participants raised concerns about environmental exposures related to agriculture and the nearby Salton Sea, a highly saline lakebed, and proposed solutions to improve the health of their communities. The findings from this study suggest farmworkers are aware of the health risks posed by living and working in rural farmlands but lack resources and information to act upon and advocate for improved public health.

Abstract:Background: Although COVID-19 has disproportionately impacted immigrant agricultural workers, vaccination efforts have been challenging. Barriers to immunization include language, visa status, and access to medical care. Additionally, vaccine hesitancy, mistrust, and misinformation contributed to low uptake. Methods: A community needs assessment was conducted with immigrant agricultural workers (n = 9) and community stakeholders (n = 15) in rural areas of Maryland and Delaware. In-depth interviews were recorded, transcribed verbatim, and analyzed using template analysis. Results: The participants named trusted and untrusted sources, sources of misinformation, and barriers to accessing information and physical locations to receive the vaccine as reasons for not getting the vaccine. Trusted sources included known community leaders and television programs. Some barriers mentioned were fear of deportation, lack of paid time off, language barriers, and difficulty navigating the U.S. healthcare system. Discussion: There is a need for culturally-tailored programs that address misinformation and barriers and use community-based programming.

Abstract:Addressing mental health is an important part of the COVID-19 response among historically underserved communities, which have been disproportionately affected. Community Health Workers (CHWs) are well placed to offer insights about barriers to mental health service use in their communities, and they are well positioned to address mental health gaps by providing education, resources, and assistance to bridging the gap for the use of more traditional mental health services. Using the perspectives of CHWs, this project identified barriers faced by CHWs in assisting community members with their mental health needs, along with relevant training needs to more effectively deliver mental health resources, referrals, and recommendations to community members. Survey data along with data from focus groups were collected among 43 CHWs in communities that have been historically underserved near the U.S.-Mexico border region. Quantitative data were analyzed using descriptive statistics whereas qualitative data were analyzed through systematic methods. Identified barriers to assisting community members with their mental health needs exist at the personal, community, environmental and organizational levels, and ranged from fear and mistrust to limited services, resources, funding and training opportunities. To help address the aforementioned barriers and facilitate access to mental health service use in their communities, CHWs identified and described opportunities for training in core areas including communication, mental illness symptom identification, trauma, self-care and stress reduction, and cultural awareness and sensitivity. Needs-based training programs that incorporate the insights of CHWs are a crucial part of promoting community-based mental health to address existing mental health disparities in access to and use of mental health services.

Abstract:At the onset of the COVID-19 pandemic in early 2020, there were various impacts on global food supply chains, including on food systems workers. Media reports documented COVID-19 outbreaks among food systems workers in the United States, however due to the ongoing nature of the pandemic, a thorough review of literature on the workplace impacts of COVID-19 on workers in our food system was scarce. This report reviews (i) the impacts of COVID-19 on global food supply chain workers; (ii) the existing and new vulnerabilities workers in this sector faced pertaining to workplace health and safety; (iii) the effectiveness of various initiatives or mechanisms to mitigate those risks; and (iv) how best practices could be incorporated into university institutional procurement practices. In order to narrow the scope of the research, this report focuses specifically on the human rights impact of COVID-19 on agricultural and meatpacking workers within the United States. Extensive interviews, as well as analysis of existing data and literature, found that the primary reason the frontline workers had a high risk of contracting COVID-19 was due to multiple pre-existing structural failures of historically eroded worker protections in the United States. Based on evidence gathered from desk research and interviews, the report coalesces around two sets of recommendations. The first is a broad set of recommendations focusing on policy and legal changes that can create greater protections for workers in these sectors. The second set of recommendations are for institutional food procurement actors, specifically universities, on actions to be taken to support values-driven procurement practices that can enhance greater protections for workers in these sectors through procurement decisions.
Immediate impact of COVID-19 pandemic on farming systems in Central America and Mexico

Abstract:Context: The COVID-19 pandemic has affected all sectors and human activities around the World. Objective: In this article we present a first attempt to understand the immediate impact of COVID-19 and the sanitary measures taken by governments on farming systems in Central America and Mexico (CAM). Methods: Through a review of information generated in these initial months of the COVID-19 pandemic (webinars, blogs, electronic publications, media) and 44 interviews with key informants across the region, we have identified the main impacts felt by different types of farming systems in the region. Results and conclusions: From corporate agricultural production systems, to small and medium scale entrepreneurs and smallholder subsistence farm households, all types of farming systems were impacted, more or less severely, by the different measures implemented by governments such as reduced mobility, closure of public and private venues and restrictions in borders. Larger corporate farming systems with vertical market integration and high level of control or coordination within the supply chain, and smallholder or subsistence farming systems with important focus on production for self-consumption and little external input use, were both relatively less impacted and showed greater adaptive capacity than the medium and small entrepreneurial farming systems dependent on agriculture as their primary income and with less control over the upstream and downstream parts of their supply chain. All types of farming systems implemented a series of mechanisms to cope with the COVID-19 pandemic including the development of alternative value chains, food and agricultural products delivery systems and the exponential use of digital means to communicate and maintain the viability of the different agricultural systems. Collective action and organization of farmers also proved to be an important coping mechanism that allowed some farmers to acquire inputs and deliver outputs in the context of restricted mobility, price volatility, and general uncertainty. Some features of the CAM region played an important role in mediating the impact of COVID-19 and associated sanitary measures. We identify as particularly relevant the nature of agricultural exports, the current structure of the agricultural sector, the diversified livelihood strategies of rural households, and the importance of mobility for rural livelihoods. Significance: The results presented focus only in the immediate effect of COVID-19 pandemic and the mechanisms implemented by farmers in the first months. Whether these impacts and response mechanisms will result in a transformation of the farming systems towards greater resilience and sustainability is still an open question.

Abstract:A large and growing literature argues that the agricultural labor market is stratified by ethnicity and legal status. However, other markers of distinction, such as indigeneity, may overlap with legal status to reveal additional health inequalities. Our study contributes to this scholarly dialogue by assessing the relation between indigeneity, legality, and health among immigrant-origin farmworkers from Latin America. To this end, we use data from the National Agricultural Worker Survey ( N = 21,092) to examine health outcomes among indigenous and non-indigenous immigrants Results indicate that indigenous farmworkers experience higher rates of pain—but not chronic conditions—than their non-indigenous counterparts. While undocumented farmworkers who are not indigenous exhibit especially favorable health, indigenous workers who are documented are significantly more likely to suffer from pain. Finally, there is limited evidence that differences in insurance coverage can fully explain the patterning between legal status, indigeneity, and health. This project sheds light on the well-being of indigenous migrant laborers in the U.S. food system, a population that experiences pervasive barriers to socioeconomic advancement at home and abroad.

Abstract:Background Community health workers represent a critical part of the health outreach and services for migrant and seasonal farmworkers ('farmworkers') in rural areas of the United States. Purpose We sought to identify adaptations to farmworker patient engagement and health outreach made by community health workers during the first 18 months of the COVID‐19 pandemic. Methods In this qualitative study, we used semi‐structured interviews with community health workers from August 2020 to February 2022 (n = 21). Two coders used thematic analysis to identify three themes related to the experiences of community health workers in conducting health education and outreach to farmworkers prior to and following the onset of the pandemic. Findings We found themes related to pre‐pandemic outreach efforts to provide health education resource sharing with farmworkers and pandemic‐related outreach efforts that included adoption of porch drops and distanced delivery of health education, adaptation of modes of health education and communication through technology and the internet, and taking on new roles related to COVID‐19. Finally, we identified changes that reverted after the pandemic or will continue as adaptations. Conclusions Community health workers created practice‐based innovations in outreach in response to the COVID‐19 pandemic. These innovations included new COVID‐19 related roles and new modes of health education and outreach, including the use of digital resources. The changes developed for emergency use in COVID‐19, particularly related to internet and technology, have likely altered how community health workers conduct outreach in North Carolina going forward. Funders, community health worker training programs, and researchers should take note of these innovations. Patient or Public Contribution Community health workers who typically come from patient populations and provide critical navigation and connection with the health care system advised on the design and creation of this research project, including serving on an advisory board. Two authors have experience working as community health workers.

Immigrant Legal Status among Essential Frontline Workers in the United States during the COVID-19 Pandemic Era

Abstract:Emerging evidence suggests that the COVID-19 pandemic has extracted a substantial toll on immigrant communities in the United States, due in part to increased potential risk of exposure for immigrants to COVID-19 in the workplace. In this article, we use federal guidance on which industries in the United States were designated essential during the COVID-19 pandemic, information about the ability to work remotely, and data from the 2019 American Community Survey to estimate the distribution of essential frontline workers by nativity and immigrant legal status. Central to our analysis is a proxy measure of working in the primary or secondary sector of the segmented labor market. Our results indicate that a larger proportion of foreign-born workers are essential frontline workers compared to native-born workers and that 70 percent of unauthorized immigrant workers are essential frontline workers. Disparities in essential frontline worker status are most pronounced for unauthorized immigrant workers and native-born workers in the secondary sector of the labor market. These results suggest that larger proportions of foreign-born workers, and especially unauthorized immigrant workers, face greater risk of potential exposure to COVID-19 in the workplace than native-born workers. Social determinants of health such as lack of access to health insurance and living in overcrowded housing indicate that unauthorized immigrant essential frontline workers may be more vulnerable to poor health outcomes related to COVID-19 than other groups of essential frontline workers. These findings help to provide a plausible explanation for why COVID-19 mortality rates for immigrants are higher than mortality rates for native-born residents.

Abstract:COVID-19 elucidated the urgency for health justice advocacy for Latinx farmworkers in Florida. While deemed essential, farmworkers’ value was not reflected in policy responses to ensure their protection. The deficiency of culturally and linguistically relevant guidance from government agencies and state restrictions requiring identification for COVID-19 services were impediments to farmworkers accessing care. Agricultural Worker Perspectives on Climate Hazards and Risk Reduction Strategies

Abstract:Objective: The purpose of this study was to understand the experiences of agricultural workers during periods of heat and wildfire smoke exposure and to support the development and implementation of protective workplace interventions. Methods: Using community-engaged research and the Center for Disease Control (CDC) framework for policy evaluation, a qualitative descriptive study was conducted with current and former agricultural workers in Central Washington (WA). Twelve participants answered semi-structured questions via interviews or by attending a focus group. Interviews and focus groups were conducted in Spanish, recorded, transcribed, and translated into English; one interview was conducted in English. Results: Using Braun and Clarke's Reflexive Thematic Analysis, five themes were identified among workers from various worksites: 1) Extreme weather and working conditions are becoming increasingly hazardous to worker health, 2) Employers and supervisors lack training and education on current labor laws, and health and safety rules, 3) Employers and supervisors use intimidation and retaliation to ensure productivity and to evoke feelings of replaceability among workers, 4) Workers do not trust regulatory agencies to enforce rules or hold employers accountable, 5) Solutions to climate-driven problems in the agricultural industry need to value worker health and safety, not just productivity. Participants reported experiencing adverse health symptoms related to heat and smoke exposure at work. Workers proposed solutions including improving education, training, and communication, and increased enforcement of existing and forthcoming occupational health and safety rules. Conclusion: The agricultural workforce is essential for ensuring a robust food supply and is facing extreme weather events due to climate change. Western states impacted by wildfires and heat are working to develop and implement occupational health and safety rules. Developing effective policies and interventions inclusive of worker perspectives is critical to adapt to a changing climate, retain a stable workforce and promote optimal health.

Abstract:In the midst of the COVID-19 pandemic, farmers and farm workers have been deemed essential workers across the world. Yet, despite working in one of the most dangerous occupations, and despite being especially vulnerable to the virus (due to existing health risk factors and risk of infection stemming from difficulties adopting control measures), many farmers and farm workers in the United States have long lacked essential resources to ensure they can meet their health needs: affordable and accessible health insurance and health care. In this commentary, we draw on our own research focused on farm families and collective experiences to discuss three main challenges farm families have faced meeting their health needs: reliance on off-farm work for health insurance coverage, the need to forecast income when purchasing a plan on the health insurance marketplace, and barriers to health care in rural areas. As we discuss these challenges, we highlight the ways in which the COVID-19 pandemic is likely exacerbating these pressures. Recognizing that major crises in the past have led to major shifts in economic, social, and political systems, the disruptions brought on by COVID-19 could be leveraged to work toward increasing access to affordable and adequate health insurance and health care. As such, we conclude our commentary by outlining policy reforms and research efforts that are needed to ensure that those working in the farm sector have access to essential resources to preserve their health and safety.

Abstract:During the pandemic, Mount Pleasant, Texas became a hotspot for Covid-19 cases in the Hispanic community employed by a local meat packing plant and many other industries. An important consideration for rural communities is the language barrier and lack of easily accessible Spanish information explaining Covid-19. In addition, rapidly changing discoveries about the virus and subsequent vaccines creates a sense of confusion within this population already burdened with difficulty understanding health information leading to even more confusion about prevention, treatment and vaccine acceptance.In response to a request for assistance from Titus Regional Medical Center, focus groups were held to investigate patient voices in person. Hiring of a bilingual Community Health Worker (CHW) empowered the hospital to connect with the community and hold the focus groups where patient voices could be heard.Barriers identified included inadequate billing explanation, rushed and impersonal service, language barriers and lacking cultural competency. TRMC aims to build infrastructure and relationships employing health literacy practices to (1) communicate in a manner to bridge Spanish/English; (2) take proactive steps to build a bilingual workforce; and (3) leverage CHWs, health literacy and cultural competency to improve care outcomes.This brief report demonstrates that healthcare system openness to patient and community-centered care offers lessons to providers, public health personnel and policy makers about the importance of effective and sustained patient and community-centered care. Such care should include health communication that is culturally and linguistically congruent to abate this pandemic and future threats to public health.

Abstract:In the first year of the COVID-19 pandemic, U.S. undocumented immigrants were at heightened risk of contracting COVID-19, with many of them being "frontline essential workers" and residing in crowded living spaces. Given undocumented immigrants' elevated susceptibility to contracting COVID-19 at the peak of uncertainty surrounding the virus, we used the structural influence model of communication to explore: (1) how undocumented immigrants acquired information about COVID-19; (2) how they assessed information trustworthiness and accuracy; (3) their perceptions of COVID-19; and (4) how they prevented or managed COVID-19. Drawing from semi-structured interviews with 46 Latina/o/x/e undocumented immigrants residing in California, we found four key themes: (1) (dis)trust in traditional media as participants relied heavily on social media for COVID-19 information; (2) weak and strong ties played a crucial role in co-constructing health outcomes with different levels of organization; (3) learning about COVID-19 through (in)direct experiences; and (4) coping through health literacy and cultural beliefs. The study's findings can inform future efforts to reach highly-vulnerable immigrant communities during a crisis (or different outbreaks in COVID-19 variants), and hopefully, help reduce health inequities.
