Abstract:The aim of this study was to investigate the consensus of skin care advice given by nurses during radiotherapy. Sixty-seven nurses, identified through nine Belgian radiotherapy departments, responded to a questionnaire survey consisting of 58 items regarding prevention and management of erythema, dry desquamation and moist desquamation. Consensus for a given advice was categorized as small if less than 50% of the nurses gave the same answer, as moderate if between 50% and 75% and as large when more than 75%. Overall, 33% of the items showed small consensus, 29% showed moderate consensus and 38% showed large consensus. The highest consensus was seen for advice in cases of moist and dry desquamation. There was less agreement in the case of erythema and it decreased further for preventive advice. Some skin care techniques that were frequently used by the nurses cannot be supported by the literature. Also, some techniques recommended by the literature are not frequently used. Further, few differences (P < 0.05) between nurses working in a university hospital and the ones working in a non-university hospital were found in terms of advice given to patients. To increase consensus on skin care issues more conclusive research is needed. Of equal importance is the translation of existing research results into daily clinical practice.
Contemporary Issues in Estuarine Physics: Definition and classification of estuaries

Abstract:This chapter discusses definitions and classification of estuaries. It presents both the classical and more flexible definitions of estuaries. Then it discusses separate classifications of estuaries based on water balance, geomorphology, water column stratification, and the stratification–circulation diagram – Hansen–Rattray approach and the Ekman–Kelvin numbers parameter space. The most widely accepted definition of an estuary was proposed by Cameron and Pritchard (1963). According to their definition, an estuary is (a) a semienclosed and coastal body of water, (b) with free communication to the ocean, and (c) within which ocean water is diluted by freshwater derived from land. Freshwater entering a semienclosed basin establishes longitudinal density gradients that result in long-term surface outflow and net inflow underneath. In classical estuaries, freshwater input is the main driver of the long-term (order of months) circulation through the addition of buoyancy. The above definition of an estuary applies to temperate (classical) estuaries but is irrelevant for arid, tropical and subtropical basins. Arid basins and those forced intermittently by freshwater exhibit hydrodynamics that are consistent with those of classical estuaries and yet have little or no freshwater influence. The loss of freshwater through evaporation is the primary forcing agent in some arid systems, and causes the development of longitudinal density gradients, in analogy to temperate estuaries. Most of this book deals with temperate estuaries, but low-inflow estuaries are discussed in detail in Chapter 9.

Abstract:OBJECTIVE: This qualitative study applies a community-based participatory research approach to elicit formative data on pediatric patient experiences of racism in the health care setting and to explore clinic-based opportunities for supporting pediatric patients experiencing racism.METHODS: The study is situated within the outpatient practice of a large tertiary academic medical center in a midsize Midwestern city. Community partners were involved in all aspects of the research, including research protocol design, recruitment, data analysis, community dissemination, and manuscript preparation. Participants were youth between 11 and 18 years, in middle or high school, self-identifying as a person of color, Latinx or Indigenous who answered yes to the question "have you ever experienced race-related prejudice and discrimination?" Parent/guardians of youth meeting inclusion criteria participated in separate focus groups. Data were analyzed using an interpretative phenomenological analysis approach.RESULTS: Major findings were divided into 2 categories: 1) racism-related experiences in the health care setting; and 2) patient and parent/guardian recommendations to support pediatric patients experiencing racism. Among health care setting experiences, primary emerging themes included racism experienced in the health care setting, patient-clinician communication around racism, patient-clinician concordance, and high-quality clinical care. Recommendations were presented within the 4 domains of racism: intrapersonal, interpersonal, structural, and institutional.CONCLUSIONS: Racism experiences worsen child biological, psychological, and behavioral functioning, yet research is lacking on how health care professionals may best support pediatric patients experiencing racism. Study findings suggest opportunities for providing safer and more supportive health care spaces for youth experiencing racism.

Abstract:Aims: Health disparities exist and refer to the chasms in health status between the advantaged and disadvantaged. Intense multiculturalism will require different approaches and moral obligations to work with these groups and urgency exists to develop nursing caring strategies when dealing with these populations. Development of nursing curricula which identify prejudicial thinking and intolerance for marginalized groups will help to decrease fears and increase nurses' willingness to provide culturally competent health care for underserved and disenfranchised populations. Background: Caring for members of disenfranchised groups instills fear at some level in nurses who are working with these individuals. This fear may be due, in part, to the potential harm nurses perceive the patient may cause them, or perhaps it is because they feel they could possibly be in the individual's situation at some point in their lives. Prejudice and discrimination continue to exist in society and have adversely affected the health care system and the nursing profession. Discrimination may be based on differences due to age, ability, gender, race, ethnicity, religion, sexual orientation, or any characteristics by which people differ. Relevance to clinical practice: Registered Nurses are accountable for nursing decisions and actions regardless of personal preferences. Due to the rapidly changing healthcare system the nurse faces increasing ethical dilemmas and human rights issues. Nurses are individually accountable for caring for each patient and the right to refuse an assignment should be carefully interpreted to avoid patient abandonment. Nurses' objections can be based on moral, ethical, or religious beliefs not on personal preferences and in an emergency the nurse must provide treatment regardless of any personal objections.

Abstract:By employing the 2,2'-thiobis(2,4-di-tert-butylphenolate) ligand ((S)L(2-)) a novel oxovanadium(V) complex, (PPh(4))(2)[(S)LV(O)(μ(2)-O)(2)V(O)(S)L] (1), was synthesised that exhibits haloperoxidase activity: on addition of H(2)O(2) a sequence of successive peroxide formation and intramolecular thioether oxidation events (sulfoxide and sulfone) led to a mixture of five products, which were all identified unambiguously, partly through an independent synthesis and characterisation. It was shown that internal thioether oxidation proceeds through peroxide formation, but the sulfoxidation of external thioether functions requires further activation of the peroxide function by protons or alkyl cations. Consistently, the employment of tBuOOH instead of H(2)O(2) led to a very active system for the catalytic sulfoxidation of thioethers.

Abstract:Importance Uncertainty remains among clinicians regarding processes to address and resolve conflict around anti-Black racism. Objective To elicit clinicians' perceptions of their role in addressing concerns about anti-Black racism among Black patients with serious illness as well as their families. Design, Setting, and Participants In this qualitative study, one-on-one semistructured interviews were conducted with 21 physicians at an academic county hospital between August 1 and October 31, 2022. Participants were provided clinical scenarios where anti-Black racism was a concern of a patient with serious illness. Participants were asked open-ended questions about initial impressions, prior similar experiences, potential strategies to address patients' concerns, and additional resources to support these conversations. A framework based on restorative justice was used to guide qualitative analyses. Main Outcomes and Measures Perspectives on addressing anti-Black racism as described by physicians. Results A total of 21 medical subspecialists (mean [SD] age, 44.2 [7.8] years) participated in the study. Most physicians were women (14 [66.7%]), 4 were Asian (19.0%), 3 were Black (14.3%), and 14 were White (66.7%). Participants identified practices that are normalized in clinical settings that may perpetuate and exacerbate perceptions of anti-Black racism. Using provided scenarios and personal experiences, participants were able to describe how Black patients are harmed as a result of these practices. Last, participants identified strategies and resources for addressing Black patients' concerns and facilitating conflict resolution, but they stopped short of promoting personal or team accountability for anti-Black racism. Conclusions and Relevance In this qualitative study, physicians identified resources, skills, and processes that partially aligned with a restorative justice framework to address anti-Black racism and facilitate conflict resolution, but did not provide steps for actualizing accountability. Restorative justice and similar processes may provide space within a mediated setting for clinicians to repair harm, provide accountability, and facilitate racial healing.

Abstract:Racism and events of racial violence have dominated the US news in 2020 almost as much as the novel coronavirus pandemic. The resultant civil unrest and demands for racial justice have spawned a global call for change. As a subset of a society that struggles with racism and other explicit biases, it is inescapable that some physicians and healthcare employees will have the same explicit biases as the general population. Patients receiving care at academic medical centers interact with multiple individuals, some of whom may have explicit and implicit biases that influence patient care. In fact, multiple reports have documented that some physicians, healthcare workers, and health professional students have negative biases based on race, ethnicity, obesity, religion, and sexual identity, among others. These biases can influence decision-making and aggravate healthcare disparities and patient-physician mistrust. We review four actual cases from academic medical centers that illustrate how well-intended physicians and healthcare workers can be influenced by bias, and how this can put patients at risk. Strategies to mitigate bias are discussed and recommended. We introduce what we believe can be a powerful teaching tool: periodic "bias and racism rounds" in teaching hospitals, in which real patient interactions are critically reviewed to identify opportunities to reduce bias and racism and attenuate the impact of bias and racism on patient outcomes.

Abstract:To the Editor: There were three communications in the February issue of CALIFORNIA MEDICINE* about the prevention of Rh immunization through the use of anti-Rh immune globulin, which cover the subject completely. A few additional points must be made, however, in order to avoid misinterpretation of the letter from Dr. Hattersley. In his opening paragraph, he states that 1 ml (300 micrograms) of the anti-Rh gamma globulin concentrate "given intramuscularly to an Rhwoman within 72 hours after she delivers an Rh+ child almost invariably prevents primary immunization to the Rh factor." This statement is supported by a rapidly enlarging bibliography, including the article by Jennings et al in the same February issue of CALIFORNIA MEDICINE. Abstract:PURPOSE Despite increased clinician awareness of systemic racism, lack of substantial action toward antiracism exists within health care. Clinical staff perspectives, particularly those of racial-ethnic minorities/persons of color (POC) who disproportionately occupy support staff roles with less power on the team, can yield insights into barriers to progress and can inform future efforts to advance diversity, equity, and inclusion (DEI, also referred to as EDI) within health care settings. This qualitative study explored the perspectives of staff members on race and role power dynamics within community health clinic teams. METHODS We conducted semistructured 45-minute interviews with staff members working in community health clinics in a large urban health care system from May to July 2021. We implemented purposeful recruitment to oversample POC and support staff and to achieve equal representation from the 13 community health clinics in the system. Interviews were audio recorded, transcribed, and analyzed over 6 months using a critical-ideological paradigm. Themes reflecting experiences related to race and role power dynamics were identified. RESULTS Our cohort had 60 participants: 42 (70%) were support staff (medical assistants, front desk clerks, care navigators, nurses) and 18 (30%) were clinicians and clinic leaders. The large majority of participants were aged 26 to 40 years (60%), were female (83%), and were POC (68%). Five themes emerged: (1) POC face hidden challenges, (2) racial discrimination persists, (3) power dynamics perpetuate inaction, (4) interpersonal actions foster safety and equity, and (5) system-level change is needed for cultural shift. CONCLUSIONS Understanding the race and role power dynamics within care teams, including experiences of staff members with less power, is critical to advancing DEI in health care.

Abstract:What was surprising about Karvonen et al.'s publication? 1 Very little was surprising for many of us who work in children's hospitals. What is new and refreshing is seeing data that our personal experiences of bigotry and racism are not just anecdotes but clearly can provide disincentives to stay in such a working environment as a resident, fellow, or faculty. In this well-constructed single-center study, Karvonen et al. used a mixed-method approach to identify racism in a NICU located in San Francisco. The nature of the study design, mixed method, and the observation that this study was done in a single center provides some evidence of racism. Still, the results of this study cannot be extrapolated to experiences outside the single NICU. The study addresses a systematic approach to understanding the staff and families' perception of the patient's treatment. Too often, the child cannot voice their complaints or concerns. The observation that staff witnessed parent's experiencing racism than parents reporting racism reinforces the challenges in the workplace for non-white staff, medical students, residents, fellows, and faculty. Where do you draw the line and report the covert, overt racism or unprofessional behavior? In my narrow experience, despite the rhetoric that all children and their parents should be treated the same in a children's hospital, rarely have I seen published data or parent-pediatric satisfaction information based on race to determine the fidelity to this aspirational goal. When I asked for such data after five site visits for hematology-oncology programs over the last decade, I have yet to be provided patient-driven data demonstrating that there is any difference in parent-patient experience based on race.

Abstract:Leading scholars have argued that when a healthcare professional and patient's race align, a provider can speak better to certain beliefs, religious practices, and cultural norms of their patients. There are also subtler, yet equally important, benefits of having your provider look like you, including patient-compliance as well as the potential for less polarising power dynamics in the provider–patient relationship. But not all reasons for race-based requests should be accepted, especially where they are based on discriminatory attitudes. In what follows, I hope to show that requests for race-based concordance is a complicated area of medicine, and it is one that is not easily dealt with through formalised policies. Instead, well-reasoned judgements by the care team through a deliberative process, that begin with ethical frameworks, might provide a more fruitful way forward. There is evidence that requests for race-based concordance, where the race of the healthcare professional and patient align, can lead to better trust between the physician and patient, especially where the patient is of a background that has been historically disenfranchised by institutional racism. Research suggests minority individuals may be more likely to trust a provider that looks like them, and this can contribute to patient compliance and better outcomes. In a similar vein, Dayna Bowen Matthew (2015) argues that if a white, middle-class physician tells an intercity, minority person to take their medication, that patient may be less likely to comply. However, where the provider–patient relationship is good, or the patient feels more comfortable with the provider, it is possible to improve health outcomes. Abstract:Health disparities continue to exist in the United States, with the most significant differences in care occurring between racial groups. Racial health disparities are largely a result of the strong association between race and structural inequities, (differentials in the distribution of power, resources, opportunities). The use of case-based learning is common practice in pharmacy education, and the race of the patient who is the subject of the case is often included out of convention. In some cases, race is included to inform treatment based on guidelines developed from epidemiological and clinical studies that link race to disease by conferring biological significance to race categories. This continuing use of race and ethnicity to guide treatment contributes to racial health disparities and may further perpetuate existing provider implicit bias. This paper discusses the pedagogical approach of using patient cases and the convention, propriety, and implications of including race in patient cases, and guides pharmacy educators in how to use information on race.
Medical Mistrust Among Black Patients with Serious Illness: A Mixed Methods Study

Abstract:Background: Medical mistrust among Black patients has been used to explain the existence of well-documented racial inequities at the end of life that negatively impact this group. However, there are few studies that describe patient perspectives around the impact of racism and discriminatory experiences on mistrust within the context of serious illness. Objective: To better characterize experiences of racism and discrimination among patients with serious illness and its association with medical mistrust. Participants: Seventy-two Black participants with serious illness hospitalized at an academic county hospital. Approach: This is a convergent mixed methods study using data from participant-completed surveys and existing semi-structured interviews eliciting participants' perspectives around their experiences with medical racism, communication, and decision-making. Main measures: The experience of medical racism and its association with Group-Based Medical Mistrust (GBMM) scale scores, a validated measure of medical mistrust. Key results: Of the 72 Black participants, 35% participated in interviews. Participants were mostly men who had significant socioeconomic disadvantage, including low levels of wealth, income, and educational attainment. There were reported high levels of race-based mistrust in the overall GBMM scale score (mean [SD], 36.6 [9.9]), as well as high scores within the suspicion (14.2 [5.0]), group disparities in healthcare (9.9 [2.8]), and lack of support (9.1 [2.7]) subscales. Three qualitative themes aligned with the GBMM subscales. Participants expressed skepticism of healthcare workers (HCWs) and modern medicine, recounted personal experiences of discrimination in the medical setting, and were frustrated with poor communication from HCWs. Conclusions: This study found high levels of mistrust among Black patients with serious illness. Suspicion of HCWs, disparities in healthcare by race, and a lack of support from HCWs were overarching themes that influenced medical mistrust. Critical, race-conscious approaches are needed to create strategies and frameworks to improve the trustworthiness of healthcare institutions and workers.

Abstract:Context Though discrimination in healthcare settings is increasingly recognized, the discriminatory experiences of patients with serious illness has not been well studied. Objectives Describe racial differences in patient-reported experiences with discrimination in the healthcare setting and examine its association with mistrust. Methods We used surveys containing patient-reported frequency of discrimination using the Discrimination in Medical Setting (DMS) and Microaggressions in Health Care Settings (MHCS) scales, mistrust using the Group Based Medical Mistrust (GBMM) scale, and patient characteristics including patient-reported race, income, wealth, insurance status, and educational attainment. Univariable and multivariable linear regression models as well as risk ratios were used to examine associations between patient characteristics including self-reported race, and DMS, MHCS, and GBMM scores. Results In 174 participants with serious illness, racially minoritized patients were more likely to report experiencing discrimination and microaggressions. In adjusted analyses, DMS scores were associated with elements of class and not with race. Black, Native American/Alaskan Native (NA/AN), and multiracial participants had higher MHCS scores compared to White participants with similar levels of income and education. Higher income was associated with lower GBMM scores in participants with similar DMS or MHCS scores, but Black and NA/AN participants still reported higher levels of mistrust. Conclusion In this cross-sectional study of patients with serious illness, discriminatory experiences were associated with worse mistrust in the medical system, particularly for Black and NA/AN participants. These findings suggest that race-conscious approaches are needed to address discrimination and mistrust in marginalized patients with serious illness and their families. KEY MESSAGE Racially minoritized patients and patients with socioeconomic disadvantage experience discrimination and mistrust in the medical system. While wider systemic and structural changes are necessary, race-conscious and intersectional approaches are needed to address and validate these negative and racist interpersonal experiences, especially during serious illness.

What are the levers for the healthcare professionals in order to have the care accepted? The problem that emerges from these first reflections is: “How can the healthcare professionals deal with a patient’s refusal of care?” The patient has the right to withdraw his consent, such as refusing an act, treatment or care. The refusal of care questions professionals even about the very meaning of their mission. It challenges them on the Health professionals-patient relationship, on its foundation and raises many ethical questions: when should we or not accept not to provide care to a patient who refuses it? How to obtain the consent of a patient who refuses care? Am I mistreating when I give a patient treatment that is essential to him? For this work, certain aspects and or dimensions of the refusal of care in the elderly from the healthcare professional’s point of view and the role of Health professionals in dealing with this refusal but also on the legislation, ethics, deontology, law, the patient- healthcare professionals relationship, public health, as well as the contribution of multi-professional care in this context . Health care professionals will have to take into account these dimensions based on the patient's autonomy, dignity and vulnerability. while respecting their consent or non-consent to care despite their illness, with the aim of adherence to the care of their patient, maintaining a relationship of trust, engaging in dialogue will make it possible to find a compromise, and ultimately respect the patient's autonomy.

Abstract:We reviewed 1500 acute transfusion reactions that were reported to the Auckland Regional Blood centre over a 7 year period, from approximately 440,000 transfusions. The majority of reactions were to red cells, and these had the highest reaction incidence per unit (0.73%) of all blood products. The reaction incidence per unit transfused for plasma was 0.1%, for stable plasma protein solution 0.01%, and for platelets 0.04%. The majority of symptoms reported were mild and transient. The commonest were fever (72%), rigors (33%), and rash or urticaria (30%). Although more serious reactions were reported such as angioedema, hypotension and pulmonary oedema, none of these were severe, as judged from the data reported to the centre. There were two transfusion related deaths during the study period, one due to an ABO incompatible transfusion, the other due to bacterial contamination of a unit of blood. Leucocyte agglutinins or antibodies were detected in 29% of those with a febrile reaction, but were also detected in 22% of those who remained afebrile. Serological abnormalities that may have accounted for the reaction were only detected in 12 patients six of whom had autoantibodies. As laboratory investigation reveals little that accurately defines the aetiology of a reaction, a rationalisation of the investigation into acute transfusion reactions is suggested.

Abstract:Objectives A growing body of evidence points to persistent health inequities within racialized minority communities, and the effects of racial discrimination on health outcomes and health care experiences. While much work has considered how anti-Black racism operates at the interpersonal and institutional levels, limited attention has focused on internalized racism and its consequences for health care. This study explores patients' attitudes towards anti-Black racism in a Canadian health care system, with a particular focus on internalized racism in primary health care.
