Abstract:Background: Pulmonary complications (PCs) may influence long-term survivor. We sought to determine the long-term impacts of major pulmonary complications (MPCs) and identify the independent risk factors in non-small cell lung cancer (NSCLC) patients. Methods: A retrospective study was conducted to analyze the PCs of 828 NSCLC patients who underwent video-assisted thoracic surgery (VATS) lobectomy in Peking University Hospital, the complications were graded according to TMM classification. The effects of PCs on the long-term prognosis were analyzed by using the Kaplan-Meier method. Multivariate logistic regression analysis was used to determine the risk factors of MPCs. Results: Of 828 patients, 139 had PCs, including 66 (8%) MPCs, those patients who developed a PC had longer drainage time, hospital stay and higher perioperative mortality rate. Excluding perioperative deaths, those who develop a MPC had a reduced 3-year disease-free survival (DFS) and 5-year DFS (68.2% vs. 78.7%, 44.7% vs. 70.3%; P=0.001), as well as the reduced 3-year overall survival (OS) and 5-year OS (81.8% vs. 88.6%, 66.6% vs. 80.9%; P=0.023). MPCs were independent prognostic factors of patients with lung cancer. Multivariate logistic regression analysis showed that the independent risk factors for MPCs were age [P=0.007; hazard ratio (HR): 1.05, 95% confidence interval (CI): 1.01-1.08], male (P=0.001; HR: 3.33, 95% CI: 1.87-5.94) and American Society of Anesthesiologist (ASA) grade. Conclusions: MPC after VATS lobectomy is associated with a poorer long-time outcome. The independent risk factors for MPCs were age, male and ASA grade.

Abstract:ObjectivesIn lung cancer resection, chronic obstructive pulmonary disease is a risk factor for post-operative complications. Few studies on post-operative complications of lung cancer resection have considered radiographic emphysematous change as an index. Here, we have examined the relationship between the regional ratio of the emphysematous area in pre-operative computed tomography images and cardiopulmonary complications in patients with chronic obstructive pulmonary disease who underwent lung cancer resection.MethodsWe retrospectively evaluated 159 patients with chronic obstructive pulmonary disease who underwent lobectomy for lung cancer at Shizuoka Cancer Center Hospital, Shizuoka, Japan, between 2002 and 2011. Pre-operative factors, including the proportion of the emphysematous area measured by computed tomography as a percentage of the low attenuation area (LAA%), as well as intraoperative factors were analyzed. Cardiopulmonary complications, including pyothorax, pneumonia and atelectasis, acute pulmonary injury, indwelling chest tube, long duration of oxygen supply, and arrhythmia, were evaluated.ResultsCardiopulmonary complications were observed among 61 patients (38%). Univariate analysis revealed that patient age, percentage of forced expiratory volume in 1 s, LAA%, and volume of blood loss were significantly associated with cardiopulmonary complications. Multivariate analysis indicated patient age and LAA% as being significant independent predictors of cardiopulmonary complications.ConclusionsThe regional ratio of the emphysematous area is useful for predicting cardiopulmonary complications in patients with chronic obstructive pulmonary disease who undergo lobectomy for lung cancer. In such patients who are also ≥ 70 years of age and exhibit LAA% ≥ 1.0%, careful intra- and post-operative management is warranted.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>When pulmonary complications occur, postlobectomy patients have a higher mortality rate, increased length of stay, and higher readmission rates. Because of a lack of high-quality consolidated clinical data, it is challenging to assess and recognize at-risk thoracic patients to avoid respiratory failure and standardize outcome measures.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The National (Nationwide) Inpatient Sample for 2015 was used to establish our model. We identified 417 respiratory failure from a total of 4,062 patients who underwent pulmonary lobectomy. Risk factors for respiratory failure were identified, analyzed, and used in novel machine learning models to predict respiratory failure.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Factors that contributed to increased odds of respiratory failure, such as preexisting chronic diseases, and intraoperative and postoperative events during hospitalization were identified. Two machine learning-based prediction models were generated and optimized by the knowledge accrued from the clinical course of postlobectomy patients. The first model, with high accuracy and specificity, is suited for performance evaluation, and the second model, with high sensitivity, is suited for clinical decision making.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>We identified risk factors for respiratory failure after lobectomy and introduced 2 machine learning-based techniques to predict respiratory failure for quality review and clinical decision-making settings. Such techniques can be used to not only provide targeted support but also standardize quality peer review measures.</p>

Abstract:Background: The relationship between risk factors of common postoperative complications after pulmonary resection, such as air leakage, atelectasis, and arrhythmia, and patient characteristics, including nutritional status or perioperative factors, has not been sufficiently elucidated. Methods: One thousand one hundred thirty-nine non-small cell lung cancer patients who underwent pulmonary resection were retrospectively analyzed for risk factors of common postoperative complications. Results: In a multivariate analysis, male sex (P = 0.01), age ≥ 65 years (P < 0.01), coexistence of chronic obstructive pulmonary disease (COPD) (P < 0.01), upper lobe (P < 0.01), surgery time ≥ 155 min (P < 0.01), and presence of lymphatic invasion (P = 0.01) were significant factors for postoperative complication. Male sex (P < 0.01), age ≥ 65 years (P = 0.02), body mass index (BMI) < 21.68 (P < 0.01), coexistence of COPD (P = 0.02), and surgery time ≥ 155 min (P = 0.01) were significant factors for severe postoperative complication. Male sex (P = 0.01), BMI < 21.68 (P < 0.01), thoracoscopic surgery (P < 0.01), and surgery time ≥ 155 min (P < 0.01) were significant risk factors for postoperative air leakage. Coexistence of COPD (P = 0.01) and coexistence of asthma (P < 0.01) were significant risk factors for postoperative atelectasis. Prognostic nutrition index (PNI) < 45.52 (P < 0.01), lobectomy or extended resection more than lobectomy (P = 0.01), and surgery time ≥ 155 min (P < 0.01) were significant risk factors for postoperative arrhythmia. Conclusion: Low BMI, thoracoscopic surgery, and longer surgery time were significant risk factors for postoperative air leakage. Coexistence of COPD and coexistence of asthma were significant risk factors for postoperative atelectasis. PNI, surgery time, and surgical procedure were revealed as risk factors of postoperative arrhythmia. Patients with these factors should be monitored for postoperative complication. Trial registration: The Institutional Review Board of Kanazawa Medical University approved the protocol of this retrospective study (approval number: I392), and written informed consent was obtained from all patients.

Abstract:Background. The amount and type of intraoperative fluid in patients with pulmonary resection currently are controversial. This study evaluated the dose-response relationship between intraoperative fluid administration and postoperative outcomes in minimally invasive lobectomy patients. Methods. A retrospective analysis of adult patients undergoing minimally invasive lobectomy between May 2016 and April 2017 was performed. The primary exposure variables were intraoperative total fluid infusion rate and intraoperative colloid infusion rate. The observation outcomes were postoperative pulmonary complications (PPCs), acute kidney injury, in-hospital mortality, postoperative length of stay, and costs. Univariate analyses and multivariate analyses were performed. Results. In 446 patients all resections were minimally invasive lobectomies. Two hundred one PPCs were observed in 172 patients. Binary logistics regression analysis demonstrated that compared with the moderate group of intraoperative total fluid infusion rate, the risk for PPCs was significantly increased at restrictive (odds ratio [OR], 2.202; 95% confidence interval [CI], 1.1894.078; p = 0.012), moderately liberal (OR, 2.743; 95% CI, 1.451-5.184; p = 0.002), and liberal (OR, 2.609; 95% CI, 1.278-5.328; p = 0.008) groups. Compared with the moderate group of intraoperative colloid infusion rate, the risk for PPCs significantly increased at no colloid (OR, 2.095; 95% CI, 1.193-3.680; p = 0.010) and restrictive (OR, 2.911; 95% CI, 1.443-5.873; p = 0.003) groups. Conclusions. In patients undergoing minimally invasive lobectomy the infusion rates of intraoperative total fluid and intraoperative colloid were all significantly associated with PPCs. Abstract:Objective To evaluate the risk and influence of lobectomy on pulmonary function of COPD patients comparied with the patients with peripheral lung cancer. Methods 32 cases of peripheral cancer complicated with moderate COPD who underwent lobectomy from January 1998 to January 2005 were analyzed retrospectively. 29 patients were male and the average age was (65.0±5.4) years. Squamous cell carcinoma in 21 patients and adenocarcinoma in 11 patients were diagnosed histologically. Pathologic staging was stage I in 2 patients, stage II in 5 patients, staging IIIa in 24 patients and Stage IIIb in 1 patient. All cases were undergone lobectomy. Results The average preoperative FEV1 and arterial oxygen tension were (64±9)% and (85±12)mmHg, The postoperative arterial oxygen tension was (87±11)mmHg and FEV1 was (62±10)%, there was no significant deference (P<0.05) although FEV1 was slightly decreased. Post-operative mortality was 0. Conclusions The postoperative pulmonary function in the patients with lung cancer comparing with COPD who had performed lobectomies is well preserved than predicted. In this study, we clarified that in moderate COPD patients, pulmonary function is unthreatened after thoracic operation.
Impact of Postoperative Prolonged Air Leakage on Long-Term Pulmonary Function after Lobectomy for Lung Cancer

Abstract:Background: This study aimed to evaluate the long-term impact of postoperative prolonged air leak (PAL) on pulmonary function. Methods: We enrolled 1,316 patients with pathologic stage I-III lung cancer who underwent lobectomy. The cohort was divided into 2 groups: those who experienced PAL (n=55) and those who did not (n=1,261). Propensity score matching was conducted at a 1:4 ratio, resulting in 49 patients in the PAL group and 189 in the non-PAL group. Changes in pulmonary function were compared among preoperative, 6-month postoperative, and 12-month postoperative measurements between the 2 groups. Results: The variables used for propensity score matching included age, sex, smoking history, body mass index, baseline pulmonary function, pathologic stage, and surgical approach. All standardized mean differences were less than 0.1. Six months postoperatively, the PAL group showed a greater reduction in both forced expiratory volume in 1 second (FEV1) (-13.0% vs. -10.0%, p=0.041) and forced vital capacity (FVC) (-15.0% vs. -9.0%, p<0.001) than the non-PAL group. In cases of upper lobectomy, there were no significant differences in FEV1 changes between the PAL and non-PAL groups at both 6 and 12 months. However, in lower lobectomy, the PAL group demonstrated a more pronounced decrease in FEV1 (-14.0% vs. -11.0%, p=0.057) and FVC (-20.0% vs. -13.0%, p=0.006) than the non-PAL group at 6 months postoperatively. Conclusion: Postoperative PAL delayed the recovery of pulmonary function after lobectomy. These effects were markedly more pronounced after lower lobectomy than after upper lobectomy.

Abstract:OBJECTIVE In New South Wales (NSW), influenza surveillance is informed by a number of discrete data sources, including laboratories, emergency departments, death registrations and community surveillance programs. The purpose of this study was to evaluate the NSW influenza surveillance system using the US Centers for Disease Control and Prevention guidelines for evaluating public health surveillance systems. Importance of study: Having a strong influenza surveillance system is important for both seasonal and pandemic influenza preparedness. The findings will inform recommendations for strengthening surveillance in NSW. METHODS The scope was limited to all sources included in the NSW Health Influenza Report in 2012-13. To assess the performance of the system, in-depth interviews (N = 21) were conducted with key stakeholders and thematically analysed. Respiratory testing data gathered through the sentinel laboratories in 2012 were used to estimate sensitivity, and laboratory notifications were analysed to assess timeliness and representativeness. Key documents - including reports, guidelines, correspondence and meeting minutes - were also reviewed, providing a method of triangulation. RESULTS The NSW influenza surveillance system integrates multiple sources of surveillance of influenza and influenza-like illness to provide a comprehensive picture of influenza in the community. Despite its structural complexity, the system delivers quality, timely and relevant data to inform a range of public health activities, and the NSW Health Influenza Report is well regarded by stakeholders. Challenges include managing system complexity, key person risk and cross-jurisdictional issues. Stakeholders commented that system flexibility would depend on additional resourcing. Although the sensitivity of sentinel laboratory surveillance was estimated as 1-25%, depending on the time of year, understanding sensitivity remains a challenge in influenza surveillance where the true incidence of infection is unknown. CONCLUSION Influenza surveillance is critical for monitoring virological changes, understanding disease epidemiology and informing public health responses. The system was found to deliver timely and good-quality surveillance information. Additional value could be achieved by increasing flexibility and stability, automating systems (where possible) and formalising processes of data acquisition. The system continues to negotiate a number of constraints, including complexity and cross-jurisdictional issues, which are ongoing obstacles to realising some potential system improvements.
Postoperative Atrial Fibrillation in Lung Cancer Lobectomy—Analysis of Risk Factors and Prognosis

Abstract:BackgroundThe incidence of postoperative atrial fibrillation (POAF) after pulmonary lobectomy ranges from 6.4 to 12.6%. This study aimed to analyze the postoperative risk factors and prognosis for POAF in lobectomy for lung cancer.MethodsData were collected from patients undergoing pulmonary lobectomy from April 2010 to March 2019. We analyzed risk factors for POAF among perioperative factors and compared postoperative complications or overall survival between POAF and non-POAF groups. We classified POAF as either the temporary or non-temporary type and compared perioperative factors, postoperative complications, and overall survival.ResultsPOAF was identified in 49 (5.2%) of the 947 lobectomies. The POAF group included more males, patients with poor performance status (PS), history of paroxysmal atrial fibrillation (AF), chronic obstructive pulmonary disease (COPD), and intraoperative blood transfusions. Poor PS, COPD, previous paroxysmal AF, and intraoperative blood transfusion were independent risk factors for POAF in multivariate analysis. The POAF group had a poorer prognosis than the non-POAF group (p = 0.0045). POAF was divided into 29 temporary and 20 non-temporary types. The onset date of non-temporary-type POAF was significantly later than that of the transient type (P < 0.01), and diabetes mellitus was significantly higher in non-temporary-type POAF. Non-temporary-type POAF had a significantly poorer prognosis in terms of overall survival (p = 0.005).ConclusionsPoor PS, COPD, history of PAF, and intraoperative blood transfusion were independent risk factors for POAF. Non-temporary-type POAF occurred significantly later than transient type and caused poorer prognosis after lobectomy for lung cancer.

Abstract:Background: There are incresing lung cancer patients detected and diagnosed at the intermediate stage when the pre-malignant or early lesions are amenable to resection and cure, owing to the progress of medical technology, the renewal of detection methods, the popularity of medical screening and the improvement of social health consciousness. The aim of this study is to investigate the risk factors of the occurrence of postoperative cardio-pulmonary complications in stage I non-small cell lung cancer (NSCLC) patients, based on routine laboratory tests, basic characteristics, and intraoperative variables in hospital. Methods: The 421 patients after lobectomy in patients with stage I NSCLC at the West China Hospital of Sichuan University from January 2012 to December 2013 were included into the study and stratified into complication group and non-complication group, according to whether to occur postoperative cardio-pulmonary complications after lobectomy in 30 days. Results: Of them, 64 (15.2%) patients were finally identified and selected into the complication group, compared with 357 (84.8%) in non-complication group: pneumonia (8.8%, 37/421) was the primary complication, and other main complications included atelectasis (5.9%, 25/421), pleural effusion (≥middle) (5.0%, 21/421), persistent air leak (3.6%, 15/421); The operation time (P=0.007), amount of blood loss (P=0.034), preoperative chronic obstructive pulmonary disease (COPD) (P=0.027), white blood cell (WBC) count (P<0.001), neutrophil-lymphocyte ratio (NLR) (P<0.001) were significantly different between the two groups. According to the binary logistics regression analysis, preoperative COPD (OR=0.031, 95%CI: 0.012-0.078, P<0.001) and WBC count (OR=1.451, 95%CI: 1.212-1.736, P<0.001) were independent risk factors for postoperative cardio-pulmonary complications. Conclusions: Among an array of clinical variables in hospital, operation time, preoperative white blood cell count, preoperative COPD, may be the independent risk factors of the occurrence of postoperative cardio-pulmonary complications.
Risk Factor Analysis and Nomogram Prediction Model Construction of Postoperative Complications of Thoracoscopic Non-Small Cell Lung Cancer

Abstract:Background: A series of complications will inevitably occur after thoracoscopic pulmonary resection. How to avoid or reduce postoperative complications is an important research area in the perioperative treatment of thoracic surgery. This study analyzed the risk factors for thoracoscopic postoperative complications of non-small cell lung cancer (NSCLC) and established a nomogram prediction model in order to provide help for clinical decision-making. Methods: Patients with NSCLC who underwent thoracoscopic surgery from January 2017 to December 2021 were selected as study subjects. The relationship between patient characteristics, surgical factors, and postoperative complications was collected and analyzed. Based on the results of the statistical regression analysis, a nomogram model was constructed, and the predictive performance of the nomogram model was evaluated. Results: A total of 872 patients who met the study criteria were included in the study. A total of 171 patients had complications after thoracoscopic surgery, accounting for 19.6% of the study population. Logistic regression analysis showed that thoracic adhesion, history of respiratory disease, and lymphocyte-monocyte ratio (LMR) were independent risk factors for complications after thoracoscopic surgery (P<0.05). Variables with P<0.1 in logistic regression analysis were included in the nomogram model. The verification results showed that the area under curve (AUC) of the model was 0.734 [95% confidence interval (CI): 0.693-0.775], and the calibration curve showed that the model had good differentiation. The decision curve analysis (DCA) curve showed that this model has good clinical application value. In subgroup analysis of complications, gender, history of respiratory disease, body mass index (BMI), type of surgical procedure, thoracic adhesion, and Time of operation were identified as significant risk factors for prolonged air leak (PAL) after surgery. Tumor location and forced expiratory volume in the first second (FEV1) 1 ) were identified as important risk factors for postoperative pulmonary infection. N stage and thoracic adhesion were identified as significant risk factors for postoperative pleural effusion. The AUC for PAL was 0.823 (95% CI: 0.768-0.879). The AUC of postoperative pulmonary infection was 0.714 (95% CI: 0.627-0.801). The AUC of postoperative pleural effusion was 0.757 (95% CI: 0.650-0.864). The calibration curve and DCA curve indicated that the model had good predictive performance and clinical application value. Conclusions: This study analyzed the risk factors affecting the postoperative complications of NSCLC through thoracoscopic surgery, and the nomogram model built based on the influencing factors has certain significance for the identification and reduction of postoperative complications.
Development and validation of a nomogram for predicting pulmonary complications after video-assisted thoracoscopic surgery in elderly patients with lung cancer

Abstract:Background: Postoperative pulmonary complications (PPCs) significantly increase the morbidity and mortality in elderly patients with lung cancer. Considering the adverse effects of PPCs, we aimed to derive and validate a nomogram to predict pulmonary complications after video-assisted thoracoscopic surgery in elderly patients with lung cancer and to assist surgeons in optimizing patient-centered treatment plans. Methods: The study enrolled 854 eligible elderly patients with lung cancer who underwent sub-lobectomy or lobectomy. A clinical prediction model for the probability of PPCs was developed using univariate and multivariate analyses. Furthermore, data from one center were used to derive the model, and data from another were used for external validation. The model's discriminatory capability, predictive accuracy, and clinical usefulness were assessed using the receiver operating characteristic (ROC) curve, calibration curve, and decision curve analysis, respectively. Results: Among the eligible elderly patients with lung cancer, 214 (25.06%) developed pulmonary complications after video-assisted thoracoscopic surgery. Age, chronic obstructive pulmonary disease, surgical procedure, operative time, forced expiratory volume in one second, and the carbon monoxide diffusing capacity of the lung were independent predictors of PPCs and were included in the final model. The areas under the ROC curves (AUC) of the training and validation sets were 0.844 and 0.796, respectively. Ten-fold cross-validation was used to evaluate the generalizability of the predictive model, with an average AUC value of 0.839. The calibration curve showed good consistency between the observed and predicted probabilities. The proposed nomogram showed good net benefit with a relatively wide range of threshold probabilities. Conclusion: A nomogram for elderly patients with lung cancer can be derived using preoperative and intraoperative variables. Our model can also be accessed using the online web server https://pulmonary-disease-predictor.shinyapps.io/dynnomapp/. Combining both may help surgeons as a clinically easy-to-use tool for minimizing the prevalence of pulmonary complications after lung resection in elderly patients.

Abstract:BACKGROUND: The aim of this study is to evaluate the serial changes in pulmonary function and the recovery time for the observed postoperative values to reach the predicted postoperative values after video-assisted thoracic surgery (VATS) lobectomy for lung cancer.PATIENTS AND METHODS: Patients undergoing VATS lobectomy for lung cancer were prospectively evaluated using complete preoperative and repeated postoperative pulmonary function tests (PFTs). The parameters of PFT at each time were compared according to the resected lobe as well as the presence of chronic obstructive pulmonary disease (COPD). The differences between the observed and predicted postoperative values of PFT and the recovery time for the observed values to reach the predicted values were calculated.RESULTS: Seventy-two patients (33 men, 39 women; mean age: 63.9 years) received complete pre- and postoperative regular PFT after undergoing VATS lobectomy. Of these patients, 24 (33.3%) patients satisfied the criteria for COPD. During the immediate postoperative period, forced vital capacity (FVC) percentage of the patients who received right lower lobectomy patients was decreased most significantly compared with the preoperative values. Compared with the upper lobectomy (UL) group, the lower lobectomy (LL) group showed a significant decrease of FVC% up to 6 months. However, there was no significant difference at 12 months after surgery. Patients with COPD showed little reduction of FEV1% that persisted significantly until 1 month after the surgery in both UL and LL groups. The recovery time was shortest in the left lower lobectomy patients, and it was shorter in the LL group than in the UL group.CONCLUSIONS: Postoperative pulmonary function and recovery time were different depending on the lobe resected and presence of COPD in VATS lobectomy patients. The information obtained from postoperative serial PFT would help accurately predict postoperative pulmonary function changes and recovery time after VATS lobectomy for lung cancer.

Abstract:<span><strong>Background:</strong> To determine the clinical prognosis after sublobectomy versus lobectomy in elderly patients ≥75 years old with stage I invasive lung adenocarcinoma ≤3 cm in size. <strong>Methods:</strong> In patients ≥75 years old, 255 patients were diagnosed with stage I invasive lung adenocarcinoma ≤3 cm in size between 2010 and 2014 in Shanghai Chest Hospital, they were all treated with sublobectomy or lobectomy. Potential confounding factors that consisted in the baseline characteristics of these two groups was balanced by the method of propensity score matching (PSM). The stratified analysis was conducted to compare the relapse-free survival (RFS) and lung cancer special survival (LCSS) rates in the sublobectomy and lobectomy groups. <strong>Results:</strong> As for the 255 patients, 112 cases conducted sublobectomy and 143 with lobectomy. Significant difference existed in RFS before (P=0.002) and after (P=0.010) PSM. Similarly, we still recognized significant difference in LCSS between the two groups before (log-rank P<0.001) or after (log-rank P=0.002) PSM. We still identified different RFS or LCSS rates between the stratified tumor size group and the stratified lymph node dissection group after adjustment of PSM. <strong>Conclusions:</strong> Lobectomy showed a survival advantage for sublobectomy for patient ≥75 years old with stage I lung adenocarcinoma ≤3 cm in size. Considering that lobectomy could get a better prognosis, it should be preferable for the treatment of patient ≥75 years old with stage I lung adenocarcinoma ≤3 cm in size.</span>

Abstract:Objectives Postoperative atrial fibrillation (POAF) is a common complication of thoracic surgery. Although POAF is known to cause increased cardiovascular events and pulmonary pneumonia, its etiology and risk factors are poorly understood. Methods This single-center, retrospective, case-control study was conducted at Ehime University Hospital. The study included patients who underwent pulmonary resections at our institution between July 2010 and October 2021. The patients with atrial fibrillation confirmed by electrocardiography during postoperative hospitalization were assigned to the POAF group and compared with those in the non-POAF group. Logistic regression analysis was used for calculating crude and adjusted odds ratios and 95% confidence intervals for POAF. Results The POAF group comprised 62 (6.5%) of 955 patients, with a higher proportion of male patients in this group than in the non-POAF group (p=0.003). Postoperative poor cardiac function with ejection fraction (EF) <55% was observed in six (9.7%) patients in the POAF group and in 26 (2.9%) patients in the non-POAF group (p=0.013). There was no significant difference in POAF between thoracotomy and video-assisted thoracoscopic surgery (p=0.614), whereas lobectomy or more extensive resection was associated with a significantly higher POAF rate than did sublobar resection (p<0.0001). Multivariate logistic regression analysis revealed significant associations between POAF and male sex, poor EF (<55%), and lobectomy or more extensive resection. Conclusions This study showed that the risk factors for POAF include male sex, poor EF, and an association with lobectomy or more extensive resection. Patients with these factors should be considered high-risk for POAF.

Abstract:Purpose: This study investigated the perioperative risk factors of postoperative pulmonary complications (PPCs) after minimally invasive anatomic resection for lung cancer. Patients and methods: We retrospectively reviewed the data from medical records of 729 lung cancer patients undergoing minimally invasive anatomic lung resections between January 2017 and December 2017. Univariate and binary logistic regression analyses were performed to select the independent risk factors for PPCs during the patient's postoperative hospitalization after surgery. Results: The incidence of PPCs was 24.8% (n=181/729). No patient died during the period of hospitalization. Logistic regression analysis revealed that body mass index (BMI) >= 24.0 kg/m(2) (vs <24.0 kg/m(2) : OR 1.514, 95% CI 1.057-2.167, P=0.024), single segmentectomy (vs single lobectomy: OR 2.115, 95% CI 1.150-3.891, P=0.016), bilobectomy or combined lobectomy and segmentectomy (vs single lobectomy: OR 2.731, 95% CI 1.013-7.361, P=0.047), and right lung lobe surgery (vs left lung lobe surgery: OR 1.519, 95% CI 1.046 2.205, P=0.028) were independent risk factors for PPCs in lung cancer patients who received minimally invasive anatomic lung resections. Conclusion: Individual factors such as BMI >= 24.0 kg/m(2) , single segmentectomy, bilobectomy or combined lobectomy and segmentectomy, and right lung lobe surgery were independent risk factors of PPCs, which should be helpful for risk stratification, patient counseling, and perioperative care for lung cancer patients.
A Nomogram for Preoperative Prediction of Prolonged Air Leak after Pulmonary Malignancy Resection

Abstract:Background Prolonged air leak (PAL) is one of the most common postoperative complications after lung surgery. This study aimed to identify risk factors of PAL after lung resection and develop a preoperative predictive model to estimate its risk for individual patients. Methods Patients with pulmonary malignancies or metastasis who underwent pulmonary resection between January 2014 and January 2018 were included. PAL was defined as an air leak more than 5 days after surgery, risk factors were analyzed. Forward stepwise multivariable logistic regression analysis was performed to identify independent risk factors, and a derived nomogram was built. Data from February 2018 to September 2018 were collected for internal validation. Results A total of 1,511 patients who met study criteria were enrolled in this study. The overall incidence of PAL was 9.07% (137/1,511). Age, percent forced expiratory volume in 1 second, surgical type, surgical approach and smoking history were included in the final model. A nomogram was developed according to the multivariable logistic regression results. The C-index of the predictive model was 0.70, and the internal validation value was 0.77. The goodness-of-fit test was non-significant for model development and internal validation. Conclusions The predictive model and derived nomogram achieved satisfied preoperative prediction of PAL. Using this nomogram, the risk for an individual patient can be estimated, and preventive measures can be applied to high-risk patients.

The aim of this study is to evaluate the accuracy of QCT based PPO FEV1.METHODS: Fifteen patients (mean age 67.4 + 15.5 years) with lung cancer who underwent either a lobectomy (n=12) or pneumonectomy (n=3) and had pre and postoperative pulmonary function testing were included. Quantitative analysis of preoperative 1mm slice CT scan was performed using Apollo software (VIDA Diagnostics). We correlated the PPO FEV1 calculated by QCT, SM and QS methods with the actual FEV1 values measured postoperatively.RESULTS: The mean preoperative FEV1 was 1.89 + 0.39L (79.4 + 20% predicted). The mean measured post-operative FEV1 among patients who underwent lobectomy and pneumonectomy was 1.61 + 0.69 L (71.5 + 21.8% predicted) and 1.29 + 0.33 L (46.3 + 9.5% predicted), respectively. Among patients who underwent lobectomy, PPO FEV1 calculated by QCT had a high correlation with measured postoperative FEV1 (r=0.92), and it was comparable to SM (r=0.94). In the pneumonectomy group, the correlation coefficient with QCT, SM, and QS were r=0.89, r=0.47, and r=0.71 respectively.CONCLUSIONS: The PPO FEV1 calculated by QCT has a high correlation with actual postoperative FEV1 in patients who undergo lobectomy or pneumonectomy.
