Abstract:Background The World Health Organization has projected that by 2030, chronic obstructive pulmonary disease (COPD) will be the third-leading cause of mortality and the seventh-leading cause of morbidity worldwide. Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with an accelerated decline in lung function, diminished quality of life, and higher mortality. Accurate early detection of acute exacerbations will enable early management and reduce mortality. Objective The aim of this study was to develop a prediction system using lifestyle data, environmental factors, and patient symptoms for the early detection of AECOPD in the upcoming 7 days. Methods This prospective study was performed at National Taiwan University Hospital. Patients with COPD that did not have a pacemaker and were not pregnant were invited for enrollment. Data on lifestyle, temperature, humidity, and fine particulate matter were collected using wearable devices (Fitbit Versa), a home air quality–sensing device (EDIMAX Airbox), and a smartphone app. AECOPD episodes were evaluated via standardized questionnaires. With these input features, we evaluated the prediction performance of machine learning models, including random forest, decision trees, k-nearest neighbor, linear discriminant analysis, and adaptive boosting, and a deep neural network model. Results The continuous real-time monitoring of lifestyle and indoor environment factors was implemented by integrating home air quality–sensing devices, a smartphone app, and wearable devices. All data from 67 COPD patients were collected prospectively during a mean 4-month follow-up period, resulting in the detection of 25 AECOPD episodes. For 7-day AECOPD prediction, the proposed AECOPD predictive model achieved an accuracy of 92.1%, sensitivity of 94%, and specificity of 90.4%. Receiver operating characteristic curve analysis showed that the area under the curve of the model in predicting AECOPD was greater than 0.9. The most important variables in the model were daily steps walked, stairs climbed, and daily distance moved. Conclusions Using wearable devices, home air quality–sensing devices, a smartphone app, and supervised prediction algorithms, we achieved excellent power to predict whether a patient would experience AECOPD within the upcoming 7 days. The AECOPD prediction system provided an effective way to collect lifestyle and environmental data, and yielded reliable predictions of future AECOPD events. Compared with previous studies, we have comprehensively improved the performance of the AECOPD prediction model by adding objective lifestyle and environmental data. This model could yield more accurate prediction results for COPD patients than using only questionnaire data.

A Method for the Early Prediction of Chronic Diseases Based on Short Sequential Medical Data.

Abstract:Noncommunicable diseases (NCDs) have become the leading cause of death worldwide. NCDs' chronicity, hiddenness, and irreversibility make patients' disease self-awareness extremely important in disease control but hard to achieve. With an accumulation of electronic health record (EHR) data, it has become possible to predict NCDs early through machine learning approaches. However, EHR data from latent NCD patients are often irregularly sampled temporally, and the data sequences are short and imbalanced, which prevents researchers from fully and effectively using such data. Here, we outline the characteristics of typical short sequential data for NCD early prediction and emphasize the importance of using such data in machine learning schemes. We then propose a novel NCD early prediction method: the short sequential medical data-based early prediction method (SSEPM). The SSEPM network contains two stacked subnetworks for multilabel enhancement. In each subnetwork, long short-term memory (LSTM) and attention layers are implemented to extract both temporal and nontemporal embedded features. During training, with prior clinical knowledge of the NCD characteristics, a random connection (RC) process is proposed for data augmentation. Comparative experiments involving ten-fold cross-validation are performed with real-world medical data to predict 5 NCDs. The result shows that the SSEPM outperforms the state-of-the-art NCD early prediction algorithms and works well in dealing with short sequential data. The results also suggest that the direct use of short sequential data could be more effective than formatting datasets with temporal exclusion limitations.
Improving Cardiovascular Risk Prediction Through Machine Learning Modelling of Irregularly Repeated Electronic Health Records

Abstract:Aims:Existing electronic health records (EHRs) often consist of abundant but irregular longitudinal measurements of risk factors. In this study, we aim to leverage such data to improve the risk prediction of atherosclerotic cardiovascular disease (ASCVD) by applying machine learning (ML) algorithms, which can allow automatic screening of the population. Methods and results:A total of 215 744 Chinese adults aged between 40 and 79 without a history of cardiovascular disease were included (6081 cases) from an EHR-based longitudinal cohort study. To allow interpretability of the model, the predictors of demographic characteristics, medication treatment, and repeatedly measured records of lipids, glycaemia, obesity, blood pressure, and renal function were used. The primary outcome was ASCVD, defined as non-fatal acute myocardial infarction, coronary heart disease death, or fatal and non-fatal stroke. The eXtreme Gradient boosting (XGBoost) algorithm and Least Absolute Shrinkage and Selection Operator (LASSO) regression models were derived to predict the 5-year ASCVD risk. In the validation set, compared with the refitted Chinese guideline-recommended Cox model (i.e. the China-PAR), the XGBoost model had a significantly higher C-statistic of 0.792, (the differences in the C-statistics: 0.011, 0.006-0.017, P < 0.001), with similar results reported for LASSO regression (the differences in the C-statistics: 0.008, 0.005-0.011, P < 0.001). The XGBoost model demonstrated the best calibration performance (men: Dx = 0.598, P = 0.75; women: Dx = 1.867, P = 0.08). Moreover, the risk distribution of the ML algorithms differed from that of the conventional model. The net reclassification improvement rates of XGBoost and LASSO over the Cox model were 3.9% (1.4-6.4%) and 2.8% (0.7-4.9%), respectively. Conclusion:Machine learning algorithms with irregular, repeated real-world data could improve cardiovascular risk prediction. They demonstrated significantly better performance for reclassification to identify the high-risk population correctly.
A Dynamic Risk-Based Early Warning Monitoring System for Population-Based Management of Cardiovascular Disease

Abstract:Risk prediction tools are crucial for population-based management of cardiovascular disease (CVD). However, most prediction models are currently used to assess long-term risk instead of the risk of short-term CVD onset. We developed a Dynamic Risk-based Early wArning Monitoring (DREAM) system using large-scale, real-time electronic health record data from 2010 to 2020 from the CHinese Electronic health Records Research in Yinzhou study. The dynamic risk scores were derived from a 1:5 matched nested case–control set comprising 70,470 individuals (11,745 CVD events) and then validated in a cohort of 81,205 individuals (5950 CVD events). The individuals were Chinese adults aged 40–79 years without a history of CVD at baseline. Eleven predictors related to vital signs, laboratory tests, and health service utilization were selected to establish the dynamic scores. The proposed scores were significantly associated with the subsequent CVD onset (adjusted odds ratio, 1.21; 95% confidence interval, 1.20–1.23). The area under the receiver operating characteristic curves (AUCs) was 0.6010 (0.5929–0.6092) and 0.6021 (0.5937–0.6105) for the long-term 10-year CVD risk <10% and ≥10% groups in the derivation set, respectively. In the long-term 10-year CVD risk ≥10% group in the validation set, the change in AUC in addition to the long-term risk was 0.0235 (0.0155–0.0315). By increasing the risk threshold from 7 to 16 points, the proportion of true subsequent CVD cases among those given alerts increased from 40.61% to 85.31%. In terms of management efficiency, the number needed to manage per CVD case ranged from 2.46 to 1.17 using the risk scores. With the increasing popularity and integration of EHR systems with wearable technology, the DREAM scores can be incorporated into an early-warning system and applied in dynamic, real-time, EHR-based, automated management to support healthcare decision making for individuals, general practitioners, and policymakers.
A computational framework of routine test data for the cost-effective chronic disease prediction

Abstract:Chronic diseases, because of insidious onset and long latent period, have become the major global disease burden. However, the current chronic disease diagnosis methods based on genetic markers or imaging analysis are challenging to promote completely due to high costs and cannot reach universality and popularization. This study analyzed massive data from routine blood and biochemical test of 32 448 patients and developed a novel framework for cost-effective chronic disease prediction with high accuracy (AUC 87.32%). Based on the best-performing XGBoost algorithm, 20 classification models were further constructed for 17 types of chronic diseases, including 9 types of cancers, 5 types of cardiovascular diseases and 3 types of mental illness. The highest accuracy of the model was 90.13% for cardia cancer, and the lowest was 76.38% for rectal cancer. The model interpretation with the SHAP algorithm showed that CREA, R-CV, GLU and NEUT% might be important indices to identify the most chronic diseases. PDW and R-CV are also discovered to be crucial indices in classifying the three types of chronic diseases (cardiovascular disease, cancer and mental illness). In addition, R-CV has a higher specificity for cancer, ALP for cardiovascular disease and GLU for mental illness. The association between chronic diseases was further revealed. At last, we build a user-friendly explainable machine-learning-based clinical decision support system (DisPioneer: http://bioinfor.imu.edu.cn/dispioneer) to assist in predicting, classifying and treating chronic diseases. This cost-effective work with simple blood tests will benefit more people and motivate clinical implementation and further investigation of chronic diseases prevention and surveillance program.

Abstract:Background Consumer-grade wearable devices enable detailed recordings of heart rate and step counts in free-living conditions. Recent studies have shown that summary statistics from these wearable recordings have potential uses for longitudinal monitoring of health and disease states. However, the relationship between higher resolution physiological dynamics from wearables and known markers of health and disease remains largely uncharacterized. Objective We aimed to derive high-resolution digital phenotypes from observational wearable recordings and to examine their associations with modifiable and inherent markers of cardiometabolic disease risk. Methods We introduced a principled framework to extract interpretable high-resolution phenotypes from wearable data recorded in free-living conditions. The proposed framework standardizes the handling of data irregularities; encodes contextual information regarding the underlying physiological state at any given time; and generates a set of 66 minimally redundant features across active, sedentary, and sleep states. We applied our approach to a multimodal data set, from the SingHEART study (NCT02791152), which comprises heart rate and step count time series from wearables, clinical screening profiles, and whole genome sequences from 692 healthy volunteers. We used machine learning to model nonlinear relationships between the high-resolution phenotypes on the one hand and clinical or genomic risk markers for blood pressure, lipid, weight and sugar abnormalities on the other. For each risk type, we performed model comparisons based on Brier scores to assess the predictive value of high-resolution features over and beyond typical baselines. We also qualitatively characterized the wearable phenotypes for participants who had actualized clinical events. Results We found that the high-resolution features have higher predictive value than typical baselines for clinical markers of cardiometabolic disease risk: the best models based on high-resolution features had 17.9% and 7.36% improvement in Brier score over baselines based on age and gender and resting heart rate, respectively (P<.001 in each case). Furthermore, heart rate dynamics from different activity states contain distinct information (maximum absolute correlation coefficient of 0.15). Heart rate dynamics in sedentary states are most predictive of lipid abnormalities and obesity, whereas patterns in active states are most predictive of blood pressure abnormalities (P<.001). Moreover, in comparison with standard measures, higher resolution patterns in wearable heart rate recordings are better able to represent subtle physiological dynamics related to genomic risk for cardiometabolic disease (improvement of 11.9%-22.0% in Brier scores; P<.001). Finally, illustrative case studies reveal connections between these high-resolution phenotypes and actualized clinical events, even for borderline profiles lacking apparent cardiometabolic risk markers. Conclusions High-resolution digital phenotypes recorded by consumer wearables in free-living states have the potential to enhance the prediction of cardiometabolic disease risk and could enable more proactive and personalized health management.

Abstract:Background In the era of information explosion, the use of the internet to assist with clinical practice and diagnosis has become a cutting-edge area of research. The application of medical informatics allows patients to be aware of their clinical conditions, which may contribute toward the prevention of several chronic diseases and disorders. Objective In this study, we applied machine learning techniques to construct a medical database system from electronic medical records (EMRs) of subjects who have undergone health examination. This system aims to provide online self-health evaluation to clinicians and patients worldwide, enabling personalized health and preventive health. Methods We built a medical database system based on the literature, and data preprocessing and cleaning were performed for the database. We utilized both supervised and unsupervised machine learning technology to analyze the EMR data to establish prediction models. The models with EMR databases were then applied to the internet platform. Results The validation data were used to validate the online diagnosis prediction system. The accuracy of the prediction model for metabolic syndrome reached 91%, and the area under the receiver operating characteristic (ROC) curve was 0.904 in this system. For chronic kidney disease, the prediction accuracy of the model reached 94.7%, and the area under the ROC curve (AUC) was 0.982. In addition, the system also provided disease diagnosis visualization via clustering, allowing users to check their outcome compared with those in the medical database, enabling increased awareness for a healthier lifestyle. Conclusions Our web-based health care machine learning system allowed users to access online diagnosis predictions and provided a health examination report. Users could understand and review their health status accordingly. In the future, we aim to connect hospitals worldwide with our platform, so that health care practitioners can make diagnoses or provide patient education to remote patients. This platform can increase the value of preventive medicine and telemedicine.

Abstract:Background: Hospitalizations account for almost one-third of the US $4.1 trillion health care cost in the United States. A substantial portion of these hospitalizations are attributed to readmissions, which led to the establishment of the Hospital Readmissions Reduction Program (HRRP) in 2012. The HRRP reduces payments to hospitals with excess readmissions. In 2018, >US $700 million was withheld; this is expected to exceed US $1 billion by 2022. More importantly, there is nothing more physically and emotionally taxing for readmitted patients and demoralizing for hospital physicians, nurses, and administrators. Given this high uncertainty of proper home recovery, intelligent monitoring is needed to predict the outcome of discharged patients to reduce readmissions. Physical activity (PA) is one of the major determinants for overall clinical outcomes in diabetes, hypertension, hyperlipidemia, heart failure, cancer, and mental health issues. These are the exact comorbidities that increase readmission rates, underlining the importance of PA in assessing the recovery of patients by quantitative measurement beyond the questionnaire and survey methods. Objective: This study aims to develop a remote, low-cost, and cloud-based machine learning (ML) platform to enable the precision health monitoring of PA, which may fundamentally alter the delivery of home health care. To validate this technology, we conducted a clinical trial to test the ability of our platform to predict clinical outcomes in discharged patients. Methods: Our platform consists of a wearable device, which includes an accelerometer and a Bluetooth sensor, and an iPhone connected to our cloud-based ML interface to analyze PA remotely and predict clinical outcomes. This system was deployed at a skilled nursing facility where we collected >17,000 person-day data points over 2 years, generating a solid training database. We used these data to train our extreme gradient boosting (XGBoost)-based ML environment to conduct a clinical trial, Activity Assessment of Patients Discharged from Hospital-I, to test the hypothesis that a comprehensive profile of PA would predict clinical outcome. We developed an advanced data-driven analytic platform that predicts the clinical outcome based on accurate measurements of PA. Artificial intelligence or an ML algorithm was used to analyze the data to predict short-term health outcome. Results: We enrolled 52 patients discharged from Stanford Hospital. Our data demonstrated a robust predictive system to forecast health outcome in the enrolled patients based on their PA data. We achieved precise prediction of the patients' clinical outcomes with a sensitivity of 87%, a specificity of 79%, and an accuracy of 85%. Conclusions: To date, there are no reliable clinical data, using a wearable device, regarding monitoring discharged patients to predict their recovery. We conducted a clinical trial to assess outcome data rigorously to be used reliably for remote home care by patients, health care professionals, and caretakers.
Predicting Clinical Deterioration of Outpatients Using Multimodal Data Collected by Wearables

Abstract:Hospital readmission rate is high for heart failure patients. Early detection of deterioration will help doctors prevent readmissions, thus reducing health care cost and providing patients with just-in-time intervention. Wearable devices (e.g., wristbands and smart watches) provide a convenient technology for continuous outpatient monitoring. In the paper, we explore the feasibility of monitoring outpatients using Fitbit Charge HR wristbands and the potential of machine learning models to predicting clinical deterioration (readmissions and death) among outpatients discharged from the hospital. We developed and piloted a data collection system in a clinical study which involved 25 heart failure patients recently discharged from a hospital. The results from the clinical study demonstrated the feasibility of continuously monitoring outpatients using wristbands. We observed high levels of patient compliance in wearing the wristbands regularly and satisfactory yield, latency and reliability of data collection from the wristbands to a cloud-based database. Finally, we explored a set of machine learning models to predict deterioration based on the Fitbit data. Through 5-fold cross validation, K nearest neighbor achieved the highest accuracy of 0.8800 for identifying patients at risk of deterioration using the health data from the beginning of the monitoring. Machine learning models based on multimodal data (step, sleep and heart rate) significantly outperformed the traditional clinical approach based on LACE index. Moreover, our proposed weighted samples one class SVM model can reach high accuracy (0.9635) for predicting the deterioration happening in the future using data collected by a sliding window, which indicates the potential for allowing timely intervention.

EHR-Based Mobile and Web Platform for Chronic Disease Risk Prediction Using Large Language Multimodal Models

Abstract:Traditional diagnosis of chronic diseases involves in-person consultations with physicians to identify the disease. However, there is a lack of research focused on predicting and developing application systems using clinical notes and blood test values. We collected five years of Electronic Health Records (EHRs) from Taiwan's hospital database between 2017 and 2021 as an AI database. Furthermore, we developed an EHR-based chronic disease prediction platform utilizing Large Language Multimodal Models (LLMMs), successfully integrating with frontend web and mobile applications for prediction. This prediction platform can also connect to the hospital's backend database, providing physicians with real-time risk assessment diagnostics. Accurate Prediction of Coronary Heart Disease for Patients With Hypertension From Electronic Health Records With Big Data and Machine-Learning Methods: Model Development and Performance Evaluation

Abstract:Background Predictions of cardiovascular disease risks based on health records have long attracted broad research interests. Despite extensive efforts, the prediction accuracy has remained unsatisfactory. This raises the question as to whether the data insufficiency, statistical and machine-learning methods, or intrinsic noise have hindered the performance of previous approaches, and how these issues can be alleviated. Objective Based on a large population of patients with hypertension in Shenzhen, China, we aimed to establish a high-precision coronary heart disease (CHD) prediction model through big data and machine-learning Methods Data from a large cohort of 42,676 patients with hypertension, including 20,156 patients with CHD onset, were investigated from electronic health records (EHRs) 1-3 years prior to CHD onset (for CHD-positive cases) or during a disease-free follow-up period of more than 3 years (for CHD-negative cases). The population was divided evenly into independent training and test datasets. Various machine-learning methods were adopted on the training set to achieve high-accuracy prediction models and the results were compared with traditional statistical methods and well-known risk scales. Comparison analyses were performed to investigate the effects of training sample size, factor sets, and modeling approaches on the prediction performance. Results An ensemble method, XGBoost, achieved high accuracy in predicting 3-year CHD onset for the independent test dataset with an area under the receiver operating characteristic curve (AUC) value of 0.943. Comparison analysis showed that nonlinear models (K-nearest neighbor AUC 0.908, random forest AUC 0.938) outperform linear models (logistic regression AUC 0.865) on the same datasets, and machine-learning methods significantly surpassed traditional risk scales or fixed models (eg, Framingham cardiovascular disease risk models). Further analyses revealed that using time-dependent features obtained from multiple records, including both statistical variables and changing-trend variables, helped to improve the performance compared to using only static features. Subpopulation analysis showed that the impact of feature design had a more significant effect on model accuracy than the population size. Marginal effect analysis showed that both traditional and EHR factors exhibited highly nonlinear characteristics with respect to the risk scores. Conclusions We demonstrated that accurate risk prediction of CHD from EHRs is possible given a sufficiently large population of training data. Sophisticated machine-learning methods played an important role in tackling the heterogeneity and nonlinear nature of disease prediction. Moreover, accumulated EHR data over multiple time points provided additional features that were valuable for risk prediction. Our study highlights the importance of accumulating big data from EHRs for accurate disease predictions.

Abstract:The use of artificial Intelligence in wearable health sensor has brought a great change in the way chronic diseases are being managed since it monitors the health and gives prompt prediction of diseases coupled with recommendations. This paper seeks to investigate the part played by AI wearables in augmenting early detection & proactive plans relating to chronic illnesses, including diabetics, cardiovascular diseases, and hypertension. Implicitly, the purpose of this study is to update the current understanding of the subject by examining the effect that predictive analytics has on patient outcomes and cost. Technologically, the concepts of the research rely on the data collected from different kinds of wearable devices and analyze by machine learning and real time analytic to examine the abnormal of vital signs before symptoms occur. Analysis is carried out on the outcomes which reveals a 25 percent decrease in hospitalization incidences per early diagnosis of chronic diseases and a 30 percent enhancement in the adherence to treatment provided formed by precautions embraced from various sensors data. This research also increases the stock of knowledge on the role of artificial intelligence in healthcare by presenting empirical evidence regarding the use of wearable devices in chronic diseases. The uniqueness of this concept is in the synchrony of monitoring processes with the application of the predictive model in providing proactive health care solutions. Some potential future research and development considerations in AI-assisted technologies in healthcare are also described.
Panic attack prediction using wearable devices and machine learning: Development and cohort study (Preprint)

Abstract:BACKGROUND: A panic attack (PA) is an intense form of anxiety accompanied by multiple somatic presentations, leading to frequent emergency department visits and impairing the quality of life. A prediction model for PAs could help clinicians and patients monitor, control, and carry out early intervention for recurrent PAs, enabling more personalized treatment for panic disorder (PD).OBJECTIVE: This study aims to provide a 7-day PA prediction model and determine the relationship between a future PA and various features, including physiological factors, anxiety and depressive factors, and the air quality index (AQI).METHODS: We enrolled 59 participants with PD (Diagnostic and Statistical Manual of Mental Disorders, 5th edition, and the Mini International Neuropsychiatric Interview). Participants used smartwatches (Garmin Vívosmart 4) and mobile apps to collect their sleep, heart rate (HR), activity level, anxiety, and depression scores (Beck Depression Inventory [BDI], Beck Anxiety Inventory [BAI], State-Trait Anxiety Inventory state anxiety [STAI-S], State-Trait Anxiety Inventory trait anxiety [STAI-T], and Panic Disorder Severity Scale Self-Report) in their real life for a duration of 1 year. We also included AQIs from open data. To analyze these data, our team used 6 machine learning methods: random forests, decision trees, linear discriminant analysis, adaptive boosting, extreme gradient boosting, and regularized greedy forests.RESULTS: For 7-day PA predictions, the random forest produced the best prediction rate. Overall, the accuracy of the test set was 67.4%-81.3% for different machine learning algorithms. The most critical variables in the model were questionnaire and physiological features, such as the BAI, BDI, STAI, MINI, average HR, resting HR, and deep sleep duration.CONCLUSIONS: It is possible to predict PAs using a combination of data from questionnaires and physiological and environmental data.

Abstract:The most direct challenge to the healthcare system in Taiwan associated with population aging is the chronic and growing shortfall in medical and nursing practitioners. The integration of artificial intelligence, Internet of Things, and advanced 5G communication technologies is making healthcare more streamlined and intuitive to improve healthcare service efficiency, encourage cross-disciplinary cooperation to improve patient health management, and introduce and mainstream innovative service models such as telecare. To this end, the wearable smart cardiac care system 'Vipasyana', jointly developed by the interdisciplinary research team of the Departments of Electrical Engineering and Computer Science at National Yang Ming Chiao Tung University, not only has an innovative fully functional wireless wearable electrocardiograph but also uses machine learning-based detection/prediction methods to detect and monitor for emergencies such as myocardial infarction, ventricular tachycardia/ventricular fibrillation, and heart failure. To handle the real-time streaming sensing data generated during clinical trials, a big data real-time streaming framework based on Apache Storm and microservices has also been integrated. In addition, a mobile phone app and virtual reality 3D ward round system providing a user-friendly management and monitoring tool for medical staffs were developed and integrated into the system. The 'Vipasyana' system is capable of providing precision medical services to individual patients.
Care for the Mind Amid Chronic Diseases: An Interpretable AI Approach Using IoT

Abstract:Health sensing for chronic disease management creates immense benefits for social welfare. Existing health sensing studies primarily focus on the prediction of physical chronic diseases. Depression, a widespread complication of chronic diseases, is however understudied. We draw on the medical literature to support depression prediction using motion sensor data. To connect human expertise in the decision-making, safeguard trust for this high-stake prediction, and ensure algorithm transparency, we develop an interpretable deep learning model: Temporal Prototype Network (TempPNet). TempPNet is built upon the emergent prototype learning models. To accommodate the temporal characteristic of sensor data and the progressive property of depression, TempPNet differs from existing prototype learning models in its capability of capturing the temporal progression of depression. Extensive empirical analyses using real-world motion sensor data show that TempPNet outperforms state-of-the-art benchmarks in depression prediction. Moreover, TempPNet interprets its predictions by visualizing the temporal progression of depression and its corresponding symptoms detected from sensor data. We further conduct a user study to demonstrate its superiority over the benchmarks in interpretability. This study offers an algorithmic solution for impactful social good - collaborative care of chronic diseases and depression in health sensing. Methodologically, it contributes to extant literature with a novel interpretable deep learning model for depression prediction from sensor data. Patients, doctors, and caregivers can deploy our model on mobile devices to monitor patients' depression risks in real-time. Our model's interpretability also allows human experts to participate in the decision-making by reviewing the interpretation of prediction outcomes and making informed interventions.

Abstract:Background Self-reporting digital apps provide a way of remotely monitoring and managing patients with chronic conditions in the community. Leveraging the data collected by these apps in prognostic models could provide increased personalization of care and reduce the burden of care for people who live with chronic conditions. This study evaluated the predictive ability of prognostic models for the prediction of acute exacerbation events in people with chronic obstructive pulmonary disease by using data self-reported to a digital health app. Objective The aim of this study was to evaluate if data self-reported to a digital health app can be used to predict acute exacerbation events in the near future. Methods This is a retrospective study evaluating the use of symptom and chronic obstructive pulmonary disease assessment test data self-reported to a digital health app (myCOPD) in predicting acute exacerbation events. We include data from 2374 patients who made 68,139 self-reports. We evaluated the degree to which the different variables self-reported to the app are predictive of exacerbation events and developed both heuristic and machine learning models to predict whether the patient will report an exacerbation event within 3 days of self-reporting to the app. The model’s predictive ability was evaluated based on self-reports from an independent set of patients. Results Users self-reported symptoms, and standard chronic obstructive pulmonary disease assessment tests displayed correlation with future exacerbation events. Both a baseline model (area under the receiver operating characteristic curve [AUROC] 0.655, 95% CI 0.689-0.676) and a machine learning model (AUROC 0.727, 95% CI 0.720-0.735) showed moderate ability in predicting exacerbation events, occurring within 3 days of a given self-report. Although the baseline model obtained a fixed sensitivity and specificity of 0.551 (95% CI 0.508-0.596) and 0.759 (95% CI 0.752-0.767) respectively, the sensitivity and specificity of the machine learning model can be tuned by dichotomizing the continuous predictions it provides with different thresholds. Conclusions Data self-reported to health care apps designed to remotely monitor patients with chronic obstructive pulmonary disease can be used to predict acute exacerbation events with moderate performance. This could increase personalization of care by allowing preemptive action to be taken to mitigate the risk of future exacerbation events.

Abstract:The number of patients with cardiovascular diseases is rapidly increasing in the world. The workload of existing clinicians is consequently increasing. However, the number of cardiovascular clinicians is declining. In this paper, we aim to design a mobile and automatic system to improve the abilities of patients’ cardiovascular health management while also reducing clinicians’ workload. Our system includes both hardware and cloud software devices based on recent advances in Internet of Things (IoT) and Artificial Intelligence (AI) technologies. A small hardware device was designed to collect high-quality Electrocardiogram (ECG) data from the human body. A novel deep-learning-based cloud service was developed and deployed to achieve automatic and accurate cardiovascular disease detection. Twenty types of diagnostic items including sinus rhythm, tachyarrhythmia, and bradyarrhythmia are supported. Experimental results show the effectiveness of our system. Our hardware device can guarantee high-quality ECG data by removing high-/low-frequency distortion and reverse lead detection with 0.9011 Area Under the Receiver Operating Characteristic Curve (ROC–AUC) score. Our deep-learning-based cloud service supports 20 types of diagnostic items, 17 of them have more than 0.98 ROC–AUC score. For a real world application, the system has been used by around 20,000 users in twenty provinces throughout China. As a consequence, using this service, we could achieve both active and passive health management through a lightweight mobile application on the WeChat Mini Program platform. We believe that it can have a broader impact on cardiovascular health management in the world.

Exacerbation predictive modelling using real-world data from the myCOPD app

Abstract:Background: Acute exacerbations of COPD (AECOPD) are episodes of breathlessness, cough and sputum which are associated with the risk of hospitalisation, progressive lung function decline and death. They are often missed or diagnosed late. Accurate timely intervention can improve these poor outcomes. Digital tools can be used to capture symptoms and other clinical data in COPD. This study aims to apply machine learning to the largest available real-world digital dataset to develop AECOPD Prediction tools which could be used to support early intervention and improve clinical outcomes. Objective: To create and validate a machine learning predictive model that forecasts exacerbations of COPD 1-8 days in advance. The model is based on routine patient-entered data from myCOPD self-management app. Method: Adaptations of the AdaBoost algorithm were employed as machine learning approaches. The dataset included 506 patients users between 2017 and 2021. 55,066 app records were available for stable COPD event labels and 1263 records of AECOPD event labels. The data used for training the model included COPD assessment test (CAT) scores, symptom scores, smoking history, and previous exacerbation frequency. All exacerbation records used in the model were confined to the 1-8 days preceding a self-reported exacerbation event. Results: TheEasyEnsemble Classifier resulted in a Sensitivity of 67.0 % and a Specificity of 65 % with a positive predictive value (PPV) of 5.0 % and a negative predictive value (NPV) of 98.9 %. An AdaBoost model with a cost-sensitive decision tree resulted in a a Sensitivity of 35.0 % and a Specificity of 89.0 % with a PPV of 7.08 % and NPV of 98.3 %. Conclusion: This preliminary analysis demonstrates that machine learning approaches to real-world data from a widely deployed digital therapeutic has the potential to predict AECOPD and can be used to confidently exclude the risk of exacerbations of COPD within the next 8 days.

Abstract:International Journal of Modeling, Simulation, and Scientific Computing, Ahead of Print. The state of the environment and human behavior today contributes to a wide range of diseases that affect people. Medical professionals often find it challenging to detect disorders by themselves appropriately; it is crucial to recognize and anticipate them early on. This paper aims to detect and predict people with more widespread chronic illnesses. To prevent such diseases from worsening, this research proposes a new deep learning-based technique to predict chronic diseases. Initially, patient data will be collected using Internet of Things (IoT) devices. Then, the missing values from input data are eliminated, and categorical data encoding, outlier detection, and data transformation are performed in the pre-processing stage. After that, the necessary attributes are selected to optimize the performance by eliminating unnecessary features using Binary Grasshopper Whale Optimization Algorithm (BGWOA), which combines the benefits of the Binary Grasshopper Optimization Algorithm (BGOA) and Binary Whale Optimization Algorithm (BWOA) algorithms. Then, the disease can be classified as chronic or not, utilizing a three-layer stacked bidirectional long short-term memory (TLSBLSTM) technique. The performance is evaluated on two chronic disease datasets that are publicly available. It successfully obtained good results by preparing the dataset on heart disease and comparing the findings using the most recent state-of-the-art approaches. According to the experimental findings, the proposed approach performs better in evaluating performance measures than the existing approaches. The observed accuracy of the proposed method is 99.87% and 99.84% for chronic kidney disease dataset and cardiovascular disease dataset, respectively.
