Abstract:This study aimed to compare the opinions of dentists and endocrinologists regarding diabetes mellitus (DM) and periodontitis, and to investigate the possible effects on their practice.Cross-sectional data were collected from 297 endocrinologists and 134 dentists practicing in southern China using two separated questionnaires. Questions were close-ended or Likert-scaled. Statistical analyses were done by descriptive statistics, bivariate and binary logistic regression analysis.Compared with endocrinologists, dentists presented more favorable attitudes for the relationship of DM and periodontitis (P&lt;0.001). 61.2% of dentists reported they would frequently refer patients with severe periodontitis for DM evaluation, while only 26.6% of endocrinologists reported they would frequently advise patients with DM to visit a dentist. Nearly all of the respondents (94.4%) agreed that the interdisciplinary collaboration should be strengthened. The logistic regression analysis exhibited that respondents with more favorable attitudes were more likely to advise a dental visit (P=0.003) or to screen for DM (P=0.006).Endocrinologists and dentists are not equally equipped with the knowledge about the relationship between DM and periodontitis, and there is a wide gap between their practice and the current evidence, especially for endocrinologists. It's urgent to take measures to develop the interdisciplinary education and collaboration among the health care providers.

Abstract:Introduction: Periodontal disease is a chronic inflammatory infectious disease that increases the host’s overall inflammatory burden by inducing a persistent systemic inflammation. This burden has been linked to insulin resistance, the development of diabetes, and its complications. Periodontal therapy plays a vital role in controlling diabetes and its potentially fatal complications. Medical and dental professionals should be aware of this interrelationship between the two diseases to facilitate proper diagnosis and predict better treatment outcomes. Aim: To assess the knowledge of medical practitioners regarding periodontal disease and its inter-relationship with diabetes mellitus. Materials and Methods: This was a questionnaire-based crosssectional study conducted by using google forms between May 2022 and October 2022 among 150 medical practitioners. The questionnaire consisted of 15 questions divided into two parts. The first part gathered personal data, while the second part focused on their knowledge about disease awareness. The collected data were analysed using percentages. Results: Out of 149 participants, who sent completed questionnaires, only 8 practitioners (5.4%) referred all while 99 (66.4%) referred only a few of their patients for dental check-ups. Among them, only 39 participants (26.2%) referred patients for regular general dental check-ups without any complaints or findings. Although 112 (75.2%) had knowledge about the association, only 59 participants (39.6%) knew that it is bidirectional. A total of 80 (53.7%) responses understood that periodontal bacterial load and associated inflammation caused alterations in sugar levels. However, 68 (45.6%) were unaware that periodontal treatment can help improve glycaemic status. Conclusion: It can be inferred that medical practitioners were not aware of the connection between periodontal disease and diabetes mellitus.

Abstract:INTRODUCTION: Increased risk of periodontal disease has been associated with diabetes mellitus (DM) and also periodontitis is considered as the sixth complication of DM. Medical physicians may have an advantageous position compared to the dentists to provide early patient counseling about oral health because patients attend medical offices more often than to dental offices and are more likely to be affiliated to medical than to dental insurance. Hence, the medical practitioners should have sufficient knowledge in this regard. The study aimed to assess the knowledge, attitudes, and practice behaviors of endocrinologists, general medicine practitioners, and diabetologists on the relationship between periodontal disease and DM.MATERIALS AND METHODS: Data were collected from sixty participants practicing in Nellore, using a structured questionnaire survey. The obtained data were analyzed through percentages.RESULTS: All the participants (100%) were aware that there existed a relation between oral health and general health. However, only 17.8% (10) of the participants refer their patients to dentists without patients asking for referral.CONCLUSION: The endocrinologists were aware of the didactic relationship, whereas general medicine practitioners and diabetologists are not much well-equipped with the knowledge on relationship between DM and periodontitis. There exists a wide gap between their practice behaviors and current scientific evidence. Therefore, more emphasis must be made by medical practitioners for the diabetic patient toward periodontal health which is most important yet neglected aspect of comprehensive health care.
Assessment of Knowledge and Awareness Regarding Oro-systemic Link among General Population: A Cross-sectional Survey

Abstract:Aim: Various surveys aimed at gathering the knowledge and awareness of health practitioners as well as students, both medical and dental, regarding the link between oral health and systemic diseases have been published. However, relatively few studies have attempted to examine the awareness of such knowledge among the general population. Therefore, the current study aimed to explore the public perception regarding the oro-systemic link. Materials and methods: The present study was designed as a cross-sectional survey to be carried out in the Department of Periodontology and Oral Implantology, PGIDS, Rohtak. All survey participants were provided with a written questionnaire which was required to be completed by each participant independently. The study questionnaire was designed to collect demographic data followed by specific questions targeting the awareness and knowledge of the participants regarding the relationship between oral and systemic health. Results: A total of 240 completed questionnaires were available for analysis. Only about a quarter of the total participants (27.5 %) believed that oral health status has any effect on the rest of the body. A positive response (YES) of 25% was observed for effect of diabetes and physician prescribed medicines on systemic health. Regarding the effect of pregnancy, smoking, stress, and obesity on systemic health, less than 20% positive response was recorded. Conclusion: The findings indicate a very poor awareness regarding the impact of systemic conditions on oral status and vice versa, among the studied population. Clinical significance: Pedodontists should lay emphasis on imparting knowledge about oro-systemic connections to their patients as well accompanying guardians. This would go a long way in spreading much needed awareness among the general population as well as ensuring that children adopt healthy lifestyle habits right from childhood. Specialist Periodontal Referral Knowledge, Attitude and Practice by General Dental Practitioner in Nepal

Abstract:Background To enhance the referral process, it is essential for the patient, referring practitioner, and specialist to establish effective communication and ensure they are wellinformed and educated about each other's requirements. Objective To assess the knowledge and attitude of general dental practitioners towards specialist periodontal referral. Method This cross-sectional study was carried out for 6 months from October 2022 to April 2023 using a self-administered questionnaire by general dental practitioners registered in Nepal Medical Council. Knowledge and attitude about periodontics, various diseases and referral to specialist for special treatment were analyzed. Result Total of 238 General Dental Practitioners (GDPs) participated in this web-based questionnaire survey. Majority of general dental practitioners (GDPs) reported that they always performed full-mouth and subgingival scaling (193, 81.09%), providing advice on proper brushing techniques (205, 86.13%), advising patients to stop harmful habits (212, 89.07%), and offering diet counseling (122, 51.26%) for patients displaying signs of gingival or periodontal diseases. Majority of GDPs also indicated that they occasionally consult a periodontist for specific periodontal problems, such as the presence of periodontal pockets (143, 60.084%), mobile teeth (150, 63.025%), gingival enlargement (145, 60.924%), periodontal abscess (146, 61.344%), and gingival recession (149, 62.605%). Conclusion General dental practitioners play a crucial role as the primary point of contact for patients seeking dental treatment. The periodontal health of the general population largely depends on the knowledge, attitude, and practice of GDPs.

Periodontal disease and systemic health: An update for medical practitioners

Abstract:Introduction: Chronic periodontal disease is a highly prevalent dental condition affecting tooth-supporting tissues. Scientific evidence is accumulating on links between periodontal disease and various systemic conditions. This narrative review provides a holistic yet succinct overview that would assist medical practitioners to deliver integrated care for better clinical outcomes. Method: Scientific evidence on associations between periodontal disease and systemic conditions was synthesised and critically appraised. Key findings of latest prospective cohort studies, randomised clinical trials, and meta-analysis were closely assessed and compiled. Results: A bidirectional relationship has been established, indicating that diabetes and periodontal disease are closely linked and amplify one another, if not successfully controlled. Existing evidence also supports the associations of periodontal disease with cardiovascular diseases and adverse pregnancy outcomes. Successful treatment of periodontal disease and dental prophylaxis has been shown to improve clinical outcomes in these systemic conditions. Other systemic conditions associated with periodontal disease include respiratory diseases, Alzheimer's disease, rheumatoid arthritis and chronic kidney disease. Although the underlying mechanisms remain to be fully elucidated, it is generally accepted that the inflammatory burden of chronic periodontal disease has an important systemic impact. Conclusion: Oral-systemic links are multifaceted and complex. While evidence linking periodontal disease with a variety of systemic conditions is still emerging, the nature of the relationship is becoming clearer. The updated understanding of these associations warrants the attention of medical experts and policymakers for a concerted effort to develop a patient-centric, integrated model for the treatment of comorbid dental and medical conditions.
Prevalence of periodontitis among the adolescents aged 15-18 years in Mangalore City: An epidemiological and microbiological study

Abstract:Background: Periodontitis is a group of inflammatory diseases affecting the supporting tissues of the tooth. Both aggressive periodontitis (AP) and chronic periodontitis (CP) have a multifactorial etiology, with dental plaque as the initiating factor. However, the initiation and progression of periodontitis are influenced by other factors including microbiologic, social and behavioral and systemic and genetic factors. The prevalence of periodontal diseases varies in different regions of the world according to the definition of periodontitis and the study population, and there are indications that they may be more prevalent in developing than in developed countries. Materials and methods: A cross-sectional study was conducted among the adolescents of 15-18 years of age in Mangalore City. One thousand one hundred students aged 15-18 years were selected for the study from the schools and colleges in Mangalore City using a convenient sampling method. The prevalence of AP and CP were assessed in the study using a community periodontal index. Students who were diagnosed clinically and radiographically were subjected to microbiological examination to confirm AP. Results: A high prevalence of gingivitis and periodontitis was found in students belonging to the lower socioeconomic status group compared with the higher socioeconomic groups, which were associated with poor oral hygiene habits. The prevalence of AP was found to be 0.36% and that of CP was found to be 1.5%. Conclusion: Oral diseases have a significant impact on the social and psychological aspects of an individual's life. Exposure to risk factors, such as age, low socio-economic status, poor education, low dental care utilization, poor oral hygiene levels, smoking, psychosocial stress and genetic factors are significantly associated with an increased risk of periodontitis among adolescents. Although genetic factors play a major role in periodontitis, the treatment outcome will still be influenced by environmental and behavioral factors.
Patient awareness of the association between periodontal and systemic diseases in an academic setting

Abstract:Background Periodontal diseases (PD) have been increasingly associated with several systemic conditions such as cardiovascular disease (CVD), diabetes mellitus (DM), rheumatoid arthritis (RA), and Alzheimer's disease (AD). This study aimed to gain insight into patients' awareness of the association between PD and systemic diseases. Methods A survey was developed to analyze patient awareness of the association between PD and systemic diseases. Descriptive and categorical variables were summarized with counts and percentages. Chi‐squared tests were used to evaluate differences between variables. A linear logistical regression model was used to assess the simultaneous, independent association between each variable. Results Data from 161 completed surveys were analyzed. The majority of the participants (61.49%) reported awareness of symptoms of PD, but only 36.36% identified all its major symptoms. Individuals reporting awareness of the association between PD and systemic diseases was 48.4%, 31.7%, 14.9%, and 9.9% for CVD, DM, RA, and AD, respectively. Patients aged ≥51 years and males were more aware of the association between PD and CVD. Increased awareness of an association between PD and DM was observed among patients who had a higher frequency of dental visits and those with a self‐reported history of DM. Conclusions This study provides insight that, even with the vast amount of scientific knowledge on the inter‐relationships that exist between PD and systemic diseases, most patients are still unaware of these associations. This research identified that improvement of health literacy surrounding PD, their symptoms, and their association with systemic diseases may be warranted.

Abstract:Introduction: The purpose of this study was to determine the level of oral health knowledge, attitude, and practise among engineering, paramedical, and nursing staffs, as well as the factors that influence them. Materials and methods: A cross-sectional questionnaire-based study was conducted using voluntarily filled questionnaires which was circulated among 342 study participants.The recorded data were analyzed using the SPSS version 26. The descriptive statistics included computation of percentages, means and standard deviation. The statistical tests applied for the analysis were, Chi-square test and One Way ANOVA. The Confidence Level and the Level of Significance were set at 95% and 5%, respectively. Results: The response rate was 95.4%. More than half (69.1%) of the professionals and other staff members participating in the study were females. While comparing the scores of the three groups among education levels attitude section and practice section was found to be statistically significant (p=0.05). The difference in scores in the practice domain when compared with gender was found to be statistically significant (p=0.004). Conclusion: It is concluded that paramedical personnel, engineering, and nursing professionals had significantly less understanding of oral health and endodontic operations. However, all of the groups practised appropriate dental hygiene.

Abstract:AIMS: Periodontitis is a multifactorial, chronic inflammatory disease that can cause irreversible damage to the supporting tissues surrounding the teeth and consequently tooth loss if left untreated, and has been listed as the sixth major complication of diabetic patients. The aim of our study was to evaluate the knowledge levels and attitudes of internists about the relationship between periodontal disease and diabetes. METHODS: In our study, our questionnaire with 22 questions was applied to internists via whatsapp®, e-mail and directly. Participants were asked about specific periodontal complications that they believed patients diagnosed with diabetes were more susceptible to, and their awareness of the bidirectional relationship between diabetes and periodontal diseases was evaluated. 112 internists answered (45% male, 55% female) our questionnaire. RESULTS: 70% of the internists participating in our survey are between the ages of 25-35. 92.7% of internists know that the department that diagnoses and treats gingival diseases is the periodontology clinic. Despite this, only 72.7% refer their patients with gingival bleeding to the periodontology clinic. 58.4% of internists reported that they did not learn about the relationship between periodontal disease and systemic health during medical school education, and 89% stated that the effect of periodontal disease on systemic health should be explained in medical school education. CONCLUSIONS: Periodontal disease and diabetes are thought to share a common pathogenesis that includes increased inflammatory response at local and systemic levels, and it is known that there is a bidirectional relationship. Therefore, successful treatment of both will affect each other positively. It has been observed that internists have knowledge about diabetes and periodontal disease in Turkey, but it is thought that it would be beneficial to consider the relationship between diabetes and periodontal disease in more detail within the education program of the medical faculty.

Abstract:Driven by health care reform and the advent of the private sector in the late 1980s, and by commitments made to the Association of Southeast Asian Nations (ASEAN), Vietnam is faced with a need to increase the regulation and training of its health care professionals. Previously, a diploma from an accredited health professional school was sufficient to practice for a lifetime. Legislation has recently been passed that will institute a licensing system, will require continuing medical education (CME) to maintain the license, and will probably place a large burden on the health professional schools and training institutes to provide CME. Supported by international nongovernmental organizations and foreign universities, the medical universities in Vietnam are responding and are preparing for their new and expanded role.
Assessment of the Knowledge of the Association between Periodontal Status and Pregnancy Outcome among Obstetricians and Gynecologists

Abstract:Background: It has been more than two decades since a link was found between maternal oral health and adverse pregnancy outcome. Obstetricians and gynecologists play a core role in the screening, prevention and referral of the pregnant women for periodontal diseases. Thus, a study was conducted to assess the knowledge of the association between periodontal status and pregnancy outcomes. Methods: An online survey was done by distributing self-administered questionnaire to the obstetricians and gynecologists. The survey consisted of three parts: demographic variables, consent and questionnaire. The qualitative data was recorded and analyzed. Results: Out of 237 participants, 95.8% of the participants agreed that there is a relation between periodontal disease and pregnancy; 73.8% with preterm birth, 60.3% with low birth weight infants and 38.4% with pre-eclampsia. Conclusions: The knowledge of the association between periodontal status and pregnancy outcomes was found to be insufficient. The gynecologists must be provided with periodic orientation to enhance and update their knowledge on maternal periodontal health.
Management of Cardiovascular Disease Patients with Periodontitis Requiring Dental Treatment - A KAP Study among Dental Students

Abstract:Background: It is frequently observed that dental students require so many consents to manage medically compromised patients, especially cardiovascular patients with periodontitis requiring dental treatment. Aims and objectives: The present study aimed to assess the dental students' knowledge, attitudes, and practices regarding the management of cardiovascular disease (CVD) patients with periodontitis requiring dental treatment. Material and methods: The cross-sectional study was conducted among 611 dental undergraduate and postgraduate students (branches of Periodontology, Oral and Maxillofacial Surgery, Oral Medicine and Radiology, Endodontics, Prosthodontics, and Public Health and Dentistry) of central Gujrat. Self-administered structured questionnaires with 23 questions were distributed in pen-and-paper physical format. Statistical analysis for descriptive statistics like frequency, percentage, mean, standard deviation, confidence interval, one-way analysis of variance, post hoc test for multiple comparisons, Fisher's exact test, and Pearson's correlation has been done by MS Excel and STATA/IC-13. Results: As per the post hoc test, both groups of students have significantly different knowledge (-4.20210*, P = 0.028) and their practices (0.68632, P = 0.968) but no significant difference between their attitudes (-6.22828, P = 0.089). According to the Pearson correlation test, a weak positive/no relationship between knowledge, attitudes, and practice is reported in both study groups. There is a weak positive relation between knowledge and attitudes (0.270) and between knowledge and practices (0.121), and a similar relationship between their attitudes and practices (0.172) was observed in both groups. Conclusion: Correct knowledge, attitudes, and practices regarding dental management of CVD patients with periodontitis can reduce medical consent to improve the treatment quality of dental students.

Abstract:Introduction Periodontal diseases, encompassing gingivitis and periodontitis, are prevalent and complex conditions with significant implications for overall health, particularly in India. This study aimed to assess the knowledge, attitudes, and practices (KAP) regarding periodontal diseases among first-year undergraduate students across various faculties at Krishna Vishwa Vidyapeeth Deemed to be University, Karad. Methods A cross-sectional questionnaire-based survey was conducted among 200 students from the medical, dental, physiotherapy, pharmacy, and nursing faculties. Descriptive statistics and Pearson's chi-squared test were utilized for data analysis. Results They revealed suboptimal oral hygiene practices among participants, with only 30% reporting twice-daily tooth cleaning. Additionally, less than half of the participants used soft toothbrushes and fluoridated toothpaste. Notably, 40% had never visited a dentist. While 86% recognized the correlation between gum and systemic health, significant proportions had experienced symptoms of gum disease. Dental first-year undergraduate students have a better understanding, attitude, and practices toward periodontal diseases, while nursing first-year undergraduate students displayed the lowest KAP scores compared to other faculties. Conclusion The first-year dental students demonstrated a better understanding, attitude, and behavior towards periodontal diseases compared to students in other faculties. Our study emphasizes the necessity of incorporating oral and periodontal health education into both healthcare professional curricula and those of higher secondary school students. Such educational programs will empower individuals to enhance their oral and systemic health. Moreover, equipping medical and paramedical practitioners with proper knowledge of periodontal health will ultimately lead to improved oral health outcomes for the general population.

Abstract:Dentists are one among the medical professionals who often encounter complex ethical issues. Playing the role of a health professional, business enterpriser and academician they often run into specific and conflicting ethical demands in practice and research. This cross-sectional study was conducted among dental professionals attached to educational institutions and private practitioners. A total of 95 participants, including private practitioners, faculty members, and post-graduate students, were recruited. A pre-validated, questionnaire containing questions related to awareness and perception in dental ethics was administered. Descriptive statistics was done to report the level of awareness and perception of dental professionals. About 65% of post-graduate professionals had awareness in clinical ethics when compared to undergraduate professionals who had 58% awareness. However, 75% of undergraduate professionals had awareness in research ethics compared to post-graduate professionals with 70% awareness. There was no difference in the perception level among graduate and post-graduate professionals. The awareness of dental ethics varied among dental professionals. There is a need to introduce specific continuing dental educational (CDE) programs to promote knowledge on dental ethics and dental jurisprudence to save dentist and their patients from encountering uneventful situations.

Abstract:Background: The aim of the present study was to compare various risk indicators of chronic periodontitis (CP) and aggressive periodontitis (AP) among patients of Davangere population. Materials and methods: Totally, 89 CP and 90 AP patients were selected from outpatient Department of Periodontics, College of Dental Sciences, Davangere. Various clinical parameters proven to be risk indicators were determined for each patient such as age, gender, occupation, oral hygiene habits, personal habits, income, level of education, place of residence, frequency of dental visits, various oral hygiene indices, gingival status, wasting diseases, malocclusion, laboratory investigations, and the results were subjected to statistical analysis. Results: This study demonstrated that AP is manifested early in life in susceptible individuals. Proven risk indicators for AP and CP in the present study population included young age, place of residence, income and education levels, frequency of dental visits. Patients with AP had better oral hygiene habits and oral hygiene index results than patients with CP. Paan chewing and smoking could be considered as risk factors, both in CP and AP cases. The similar association of plaque scores but higher bleeding tendency in AP patients supported the fact of higher susceptibility of AP patients to periodontal breakdown. Malocclusion being present in the majority of cases could also be put forth as a risk factor for AP and CP. Conclusion: This study identifies the different risk indicators for CP and AP and demonstrates the need for constructing nationwide oral health promotion programs to improve the level of oral health awareness and standards in Indian population.
Patients’ Understanding of the Relationship Between Their Diabetes and Periodontal Disease

Abstract:INTRODUCTION: There is a paucity of research on awareness, education, and interventions that address increased risk of developing periodontal disease, the sixth complication of diabetes. Examining the knowledge of patients with diabetes and understanding of the bidirectional relationship between periodontal disease and diabetes could inform future diabetes self-management care. We assessed the knowledge and understanding of the bidirectional relationship between diabetes and periodontal disease; examined gender, education, and income differences in this knowledge and understanding, as well as other differences in dental hygiene practices.METHODS: A Web-based survey was conducted using a convenience sample of patients with diabetes.RESULTS: A total of 927 patients with diabetes participated in the study. Gender and education were significantly associated with knowledge and understanding of the bidirectional relationship between diabetes and periodontal disease in patients with diabetes. After controlling for diabetes duration, dental insurance status, and other covariates, males had less knowledge and understanding of the bidirectional relationship between diabetes and periodontal disease compared with females (P < .0001). Those with higher education (P < .002) and those who received health care provider recommendations to obtain regular dental visits (P < .00001) had greater understanding of the association between diabetes and periodontal disease.CONCLUSIONS: This study highlights the need for 1) educating male and low-education patients with diabetes in particular about their increased risk of periodontal disease and the need for regular dental visits, and 2) health care providers and dental health providers to work together to address the needs of their diabetic patients.

Abstract:Periodontal disease is associated with other non-communicable diseases including diabetes mellitus, coronary heart disease and atherosclerosis, hypertension, and respiratory tract infections. This association merits careful study of the general population's awareness level in order to leverage the current state of science to improve general health and quality of life. This study included 502 residents of Saudi Arabia who received computer-assisted interviews to fill up the survey. Results indicated a low level of awareness among the study population regarding the association of periodontal disease to diabetes mellitus, coronary heart disease and atherosclerosis, hypertension, and respiratory tract infections. A higher level of awareness was noticed with individuals with periodontal disease, themselves or a member of their family having a systemic disease, and who have a specialized person or scientific article as their source of information. This observed low level of awareness deserves the attention of public health authorities to prioritize programs that increase the awareness, improve health, and reduce burden of systemic diseases of high prevalence, morbidity, and mortality.
