Abstract:Conventional transcranial direct current stimulation (tDCS) targeting the left dorsolateral prefrontal cortex (DLPFC) could improve arousal in disorders of consciousness (DOC). However, the comparative effectiveness of anodal stimulation of the left DLPFC and the electrophysiological effect of tDCS are yet to be determined. In this randomized sham-controlled design, patients were separated into three groups (left/right anodal tDCS, sham). Data on the clinical assessments and EEG were collected at baseline and after 2 weeks of tDCS. The outcome at 3-month follow-up was evaluated using the Glasgow Outcome Scale-Extended. Results showed that sessions of the left tDCS facilitated the excitability of the prefrontal cortex, whereas only one patient had a positive outcome. Targeting the right DLPFC was less effective, merely leading to activation of the stimulation site, with no effect on the state of arousal. Moreover, sham stimulation had minimal or no effect on any of the outcomes. These results provide evidence for a hemispheric asymmetry of tDCS effects in patients with DOC. Left anodal tDCS might be more effective for modulating cortical excitability compared to tDCS on the right DLPFC. However, future studies with large sample sizes are needed to confirm these findings. This trial is registered with NCT03809936.
HD-tDCS Applied on DLPFC Cortex for Sustained Attention Enhancement: A Preliminary EEG Study

Abstract:This study investigated the effect of the high-definition transcranial direct current stimulation (HD-tDCS) on sustained attention reaction. The HD-tDCS was applied to the right dorsolateral prefrontal cortex (DLPFC) and the EEG signal was recorded simultaneously. We found that HD-tDCS could be used to enhance the reaction time and the N2 amplitude in response to the flanker stimuli changed significantly after the HD-tDCS stimulation. Moreover, the behavior enhancement was in accordance with the neural response. The HD-tDCS on DLPFC provides a way to enhance the subject's attention to both the target task and the target task accompanied by distractors. The investigated approach may find application for digital medication in patients with attention problems, such as attention deficit hyperactivity disorder (ADHD).

Abstract:—We propose the concept of Phase-Only Forward (PF) relays for cooperative communication involving constant envelope modulations. The technique enables the relaying nodes to maintain constant envelope signaling without the need to perform decoding and signal re-modulation. The bit-error-probability (BEP) of this PF technique, in conjunction with constant envelope CPFSK modulation in a time-selective Rayleigh fading environment and discriminator detection, was analyzed. Both cases of with and without antenna selection at the relay were considered. In the former case, a clever phase-adjustment scheme that allows the relay to maintain phase continuity was proposed. Our results indicate that without antenna selection, PF has a lower BEP than decode-and-forward (DF). It also delivers the same performance as amplify-and-forward (AF) when fading is quasi-static. In a time-selective fading environment, PF is not as efficient as AF without antenna selection. However with dual-antenna selection at the relay, PF has practically the same performance as AF even in a time-selective fading environment. We thus believe that PF is a cost-effective alternative to AF and DF, since signal re-modulation and expensive linear amplifiers are no longer needed at the relay to support non-constant envelope transmission. Finally, this phase-forward concept can be generalized to other continuous-phase modulation schemes as well as to PSK.
A High-Definition tDCS and EEG study on attention and vigilance: Brain stimulation mitigates the executive but not the arousal vigilance decrement

Abstract:Attention comprises a wide set of processes such as phasic alertness, orienting, executive control, and the executive (i.e., detecting infrequent targets) and arousal (i.e., sustaining a fast reaction) vigilance components. Importantly, the effects of transcranial direct current stimulation (tDCS) over attentional functioning have been mostly addressed by measuring these processes separately and by delivering offline tDCS with low precision over the stimulation region. In the current study, we examined the effects of online High-Definition tDCS (HD-tDCS) over the behavioral and electrophysiological functioning of attentional and vigilance components. Participants (N = 92) were randomly assigned to one of three stimulation groups: right dorsolateral prefrontal cortex stimulation, right posterior parietal cortex (PPC) stimulation, and sham. All of them performed – in combination with the HD-tDCS protocol – an attentional networks task (ANTI-Vea) suitable to measure the executive and arousal components of vigilance along with three typical attentional functions: phasic alertness, orienting, and executive control. In addition, EEG was registered at the baseline and at the post-stimulation period. We observed that, regardless the stimulation region, online HD-tDCS: (a) reduced phasic alertness (p = .008), but did not modulated the orienting and executive control functioning; and (b) mitigated the executive vigilance decrement (p = .011), but did not modulated arousal vigilance across time-on-task. Interestingly, only HD-tDCS over PPC reduced considerably the increment of alpha power observed across time-on-task (p = .009). The current study provides further evidence for both an empirical dissociation between vigilance components and the cortical regions underlying attentional processes. We highlight the advantages of using online HD-tDCS to examine the stimulation effects on attentional and vigilance functioning.

Abstract:Current theoretical accounts on the oscillatory nature of sustained attention predict that entrainment via transcranial alternating current stimulation (tACS) at alpha and theta frequencies on specific areas of the prefrontal cortex could prevent the drops in vigilance across time-on-task. Nonetheless, most previous studies have neglected both the fact that vigilance comprises two dissociable components (i.e., arousal and executive vigilance) and the potential role of differences in arousal levels. We examined the effects of theta- and alpha-tACS over the right dorsolateral prefrontal cortex in both components of vigilance and in participants who differed in arousal level according to their chronotype and time of testing. Intermediate-types performed the vigilance tasks when their arousal level was optimal, whereas evening-types performed the vigilance tasks when their arousal levels were non-optimal. Both theta- and alpha-tACS improved arousal vigilance in the psychomotor vigilance task (PVT), whereas alpha-tACS, but not theta-tACS, improved executive vigilance in the sustained attention to response task (SART), and counteracted the typical vigilance decrement usually observed in this task. Importantly, these stimulation effects were only found when arousal was low (i.e., with evening-types performing the tasks at their non-optimal time of day). The results support the multicomponent view of vigilance, the relevance of heeding individual differences in arousal, and the role of alpha oscillations as a long-range cortical scale synchronization mechanism that compensates the decrements in performance as a function of time-on-task by exerting and maintaining cognitive control attributed to activation of the right dorsolateral prefrontal cortex.
Effect of Long-term Transcranial Direct Current Stimulation on Glx and GABA: A Pilot Study

Abstract:Previous studies have demonstrated that transcranial direct current stimulation (tDCS) over the dorsolateral prefrontal cortex (dlPFC) can enhance working memory. However, the mechanism underlying the long-term tDCS is still unclear. This pilot study aims to examine neurotransmitters such as gamma-aminobutyric (GABA) and Glx (a measure of glutamate and glutamine combined) and working memory in response to the long-term anodal tDCS over dlPFC. Six healthy, right-handed young adults enrolled in this study received 2-mA anodal tDCS over dlPFC within 4 weeks. Long-term tDCS means that it was applied 5 times per week for the first two weeks and once for the last two weeks with 30 min each time. The other six participants were enrolled as the control group without stimulation for testing the baseline enhancement of working memory due to learning. The GABA and Glx levels were assessed by Magnetic Resonance Spectroscopy (MRS), while a 3-back task was performed to assess working memory. Data were collected at the beginning of the experiment, after two-week tDCS and at the end of the experiment. We found that the working memory was not significantly enhanced by the first two-week tDCS because the accuracy of response in 3-back was not significantly increased compared to the control group. Meanwhile, there were no significant changes in the levels of GABA. However, the Glx level was found significantly decreased in both 2- and 4-week MRS measurements. The observation that the long-term tDCS causes the decrease of excitatory neurotransmitters implies the different underlying mechanisms between the long-term tDCS and the single one.

Abstract:Abstract Transcranial Direct Current Stimulation (tDCS) has shown that stimulation of Dorsolateral Prefrontal Cortex (DLPFC) facilitates task performance in working-memory tasks. However, little is known about its potential effects on effort. This study examined whether tDCS affects effort during a working-memory task. Participants received anodal, cathodal and sham stimulation over DLPFC across three sessions before carrying out a 2-back task. During the task, effort-related cardiovascular measures were recorded—especially the Initial Systolic Time Interval (ISTI). Results showed that anodal stimulation produced a shorter ISTI, indicating a greater effort compared to cathodal and sham conditions, where effort was lower. These findings demonstrate that anodal stimulation helps participants to maintain engagement in a highly demanding task (by increasing task mastery), without which they would otherwise disengage. This study is the first to show that tDCS impacts the extent of effort engaged by individuals during a difficult task.

Abstract:Background The vigilance fluctuation and decrement of sustained attention have large detrimental consequences to most tasks in daily life, especially among the elderly. Non-invasive brain stimulations (e.g., transcranial direct current stimulation, tDCS) have been widely applied to improve sustained attention, however, with mixed results. Objective An infraslow frequency oscillatory tDCS approach was designed to improve sustained attention. Methods The infraslow frequency oscillatory tDCS (O-tDCS) over the left dorsolateral prefrontal cortex at 0.05 Hz was designed and compared with conventional tDCS (C-tDCS) to test whether this new protocol improves sustained attention more effectively. The sustained attention was evaluated by reaction time and accuracy. Results Compared with the C-tDCS and sham, the O-tDCS significantly enhanced sustained attention by increasing response accuracy, reducing response time, and its variability. These effects were predicted by the evoked oscillation of response time at the stimulation frequency. Conclusion Similar to previous studies, the modulation effect of C-tDCS on sustained attention is weak and unstable. In contrast, the O-tDCS effectively and systematically enhances sustained attention by optimizing vigilance fluctuation. The modulation effect of O-tDCS is probably driven by neural oscillations at the infraslow frequency range.

Abstract:Background: Decrements in performance and the propensity for increased mind-wandering (i.e., task-unrelated thoughts) across time-on-task are two pervasive phenomena observed when people perform vigilance tasks. In the present study, we asked whether processes that lead to vigilance decrement and processes that foster the propensity for mind-wandering (MW) can be dissociated or whether they share a common mechanism. In one experiment, we introduced two critical manipulations: increasing task demands and applying anodal high-definition transcranial direct current stimulation (HD-tDCS) to the left dorsolateral prefrontal cortex. Method: Seventy-eight participants were randomly assigned to one of four groups resulting from the factorial combination of task demand (low, high) and stimulation (anodal, sham). Participants completed the sustained attention to response task (SART), which included thought probes on intentional and unintentional MW. In addition, we investigated the crucial role of alpha oscillations in a novel approach. By assessing pre-post resting EEG, we explored whether participants' variability in baseline alpha power predicted performance in MW and vigilance decrement related to tDCS or task demands, respectively, and whether such variability was a stable characteristic of participants. Results: Our results showed a double dissociation, such that task demands exclusively affected vigilance decrement, while anodal tDCS exclusively affected the rate of MW. Furthermore, the slope of the vigilance decrement function and MW rate (overall, intentional and unintentional) did not correlate. Critically, resting state alpha-band activity predicted tDCS-related gains in unintentional MW alone, but not in vigilance decrement, and remained stable after participants completed the task. Conclusion: These results show that when a sustained attention task involving executive vigilance, such as the SART, is designed to elicit both vigilance decrement effects and MW, the processes leading to vigilance decrement should be differentiated from those responsible for MW, a claim that is supported by the double dissociation observed here and the lack of correlation between the measures chosen to assess both phenomena. Furthermore, the results provide the first evidence of how individual differences in alpha power at baseline may be of crucial importance in predicting the effects of tDCS on MW propensity.

Abstract:Transcranial direct current stimulation (tDCS) of the left dorsolateral prefrontal cortex (DLPFC) has been shown to promote working memory (WM), however, its efficacy against time-on-task-related performance decline and associated cognitive fatigue remains uncertain. This study examined the impact of anodal tDCS of the left DLPFC on performance during a fatiguing visuospatial WM test. We adopted a repeated measures design, where 32 healthy adults (16 female), underwent anodal, control and sham tDCS on separate days. They completed an hour long two-back test, with stimulation intensity, onset, and duration set at 1 mA, at the 20th minute for 10 minutes respectively. Task performance, subjective responses, and heart rate variability (HRV) were captured during the experiment. Anodal tDCS substantially improved WM relative to sham tDCS and control in both sexes. These benefits lasted beyond the stimulation interval, and were unique across performance measures. However, no perceptual changes in subjective effort or fatigue levels were noted between conditions, although participants reported greater discomfort during stimulation. While mood and sleepiness changed with time-on-task, reflecting fatigue, these were largely similar across conditions. HRV increased under anodal tDCS and control, and plateaued under sham tDCS. We found that short duration anodal tDCS at 1 mA was an effective countermeasure to time-on-task deficits during a visuospatial two-back task, with enhancement and preservation of WM capacity. However, these improvements were not available at a perceptual level. Therefore, wider investigations are necessary to determine "how" such solutions will be operationalized in the field, especially within human-centered systems.
Network-based transcranial direct current stimulation enhances attention function in healthy young adults: a preliminary study

Abstract:Purpose: Attention, a complex cognitive process, is linked to the functional activities of the brain's dorsal attention network (DAN) and default network (DN). This study aimed to investigate the feasibility, safety, and blinding efficacy of a transcranial direct current stimulation (tDCS) paradigm designed to increase the excitability of the DAN while inhibiting the DN (DAN+/DN-tDCS) on attention function in healthy young adults. Methods: In this randomized controlled experiment, participants were assigned to either the DAN+/DN-tDCS group or the sham group. A single intervention session was conducted at a total intensity of 4 mA for 20 min. Participants completed the Attention Network Test (ANT) immediately before and after stimulation. Blinding efficacy and adverse effects were assessed post-stimulation. Results: Forty participants completed the study, with 20 in each group. Paired-sample t-test showed a significant post-stimulation improvement in executive effect performance (t = 2.245; p = 0.037) in the DAN+/DN-tDCS group. The sham group did not exhibit any significant differences in ANT performance. Participants identified the stimulation type with 52.50% accuracy, indicating no difference in blinding efficacy between groups (p = 0.241). Mild-to-moderate adverse effects, such as stinging, itching, and skin reddening, were reported in the DAN+/DN-tDCS group (p < 0.05). Conclusion: DAN+/DN-tDCS enhanced attention function in healthy young individuals, particularly in improving executive effect performance. This study presents novel strategies for enhancing attentional performance and encourages further investigation into the mechanisms and outcomes of these interventions across diverse populations.
Transcranial Direct Current Stimulation Does Not Counteract Cognitive Fatigue, but Induces Sleepiness and an Inter-Hemispheric Shift in Brain Oxygenation

Abstract:Sustained cognitive demands may result in cognitive fatigue (CF), eventually leading to decreased behavioral performance and compromised brain resources. In the present study, we tested the hypothesis that transcranial direct current stimulation (tDCS) would counteract the behavioral and neurophysiological effects of CF. Twenty young healthy participants were tested in a within-subject counterbalanced order across two different days. Anodal tDCS (real vs. sham) was applied over the left prefrontal cortex. In the real tDCS condition, a current of 1.5 mA was delivered for 25 min. Cortical oxygenation changes were measured using functional Near Infrared Spectroscopy (fNIRS) on the frontal cortices. CF was triggered using the TloadDback task, a sustained working memory paradigm that allows tailoring task demands according to each individual's maximal cognitive capacity. Sustained cognitive load-related effects were assessed using pre- versus post-task subjective fatigue and sleepiness scales, evolution of performance accuracy within the task, indirect markers of dopaminergic activity (eye blinks), and cortical oxygenation changes (fNIRS) both during the task and pre- and post-task resting state periods. Results consistently disclosed significant CF-related effects on performance. Transcranial DCS was not effective to counteract the behavioral effects of CF. In the control (sham tDCS) condition, cerebral oxygen exchange (COE) levels significantly increased in the right hemisphere during the resting state immediately after the induction of CF, suggesting a depletion of brain resources. In contrast, tDCS combined with CF induction significantly shifted interhemispheric oxygenation balance during the post-training resting state. Additionally, increased self-reported sleepiness was associated with brain activity in the stimulated hemisphere after recovery from CF during the tDCS condition only, which might reflect a negative middle-term effect of tDCS application.

Abstract:Introduction: Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation method, popular due to its low cost, ease-of-application, and portability. As such, it has gained traction in examining its potential for cognitive enhancement in a diverse range of populations, including active-duty military. However, current literature presents mixed results regarding its efficacy and limited evaluations of possible undesirable side-effects (such as degradation to cognitive processes). Methods: To further examine its potential for enhancing cognition, a double-blind, randomized, sham-controlled, within-subjects design, was used to evaluate both online active-anodal and -cathodal on several cognitive tasks administered. Potential undesirable side effects related to mood, sleepiness, and cognitive performance, were also assessed. Active tDCS was applied for 30 min, using 2 mA, to the left dorsolateral prefrontal cortex with an extracephalic reference placed on the contralateral arm of 27 (14 males) active-duty Soldiers. Results: We report mixed results. Specifically, we found improvements in sustained attention (active-anodal) for males in reaction time ( p = 0.024, η p 2 = 0.16) and for sensitivity index in females ( p = 0.013, η p 2 = 0.18). In addition, we found faster reaction time ( p = 0.034, η p 2 = 0.15) and increased accuracy ( p = 0.029, η p 2 = 0.16) associated with executive function (active-anodal and -cathodal), and worsened working memory performance (active-cathodal; p = 0.008, η p 2 = 0.18). Additionally, we found increased risk-taking with active-anodal ( p = 0.001, η p 2 = 0.33). Discussion: tDCS may hold promise as a method for cognitive enhancement, as evidenced by our findings related to sustained attention and executive function. However, we caution that further study is required to better understand additional parameters and limitations that may explain results, as our study only focused on anode vs. cathode stimulation. Risk-taking was examined secondary to our main interests which warrants further experimental investigation isolating potential tradeoffs that may be associated with tDCS simulation.

Abstract:Executive functions are of vital importance in the process of active cognition, which is thought to be associated with the dorsolateral prefrontal cortex (DLPFC). As a valid brain stimulation technology, high-definition transcranial direct current stimulation (HD-tDCS) has been used to optimize cognitive function in healthy adults. Substantial evidence indicates that short-term or single anodal tDCS sessions over the left DLPFC will enhance the performance of executive functions. However, the changes in performance and cortical activation of executive functions after modulation by repeated anodal HD-tDCS is as yet unexplored. This study aims to examine changes in three core components of executive functions (inhibitory control, working memory, and cognitive flexibility) produced by nine HD-tDCS sessions (1.5 mA, over left DLPFC, 20 min per session), and to use functional near-infrared spectroscopy (fNIRS) to bilaterally record DLPFC neural activity. A total of 43 participants were divided randomly into two study groups (anodal group vs. sham group) to complete nine interventions. Our results demonstrate that the enhancement of cognitive flexibility in the anodal group was significantly better than that in the sham group. Additionally, a Stroop effect-related decrease in oxygenated hemoglobin (HbO) concentration in the DLPFC was observed in the anodal group but not the sham group. In conclusion, our study found that repeated anodal HD-tDCS sessions can significantly promote cognitive flexibility, one of the core components of executive function, and that alterations in DLPFC activation can enhance our understanding of the neuroplastic modifications modulated by HD-tDCS.

Abstract:Introduction: Chronic stress is a long-term condition that negatively affects cognitive ability and mental health. Individuals who experience chronic stress show poor attentional control. Transcranial direct current stimulation (tDCS) to the dorsolateral prefrontal cortex (DLPFC) modulates executive function domains. Therefore, it is beneficial to investigate whether tDCS of the DLPFC could improve attentional control and relieve stress in chronically stressed individuals. Methods: We assess the event-related potentials (ERPs) associated with attentional control in individuals with chronic stress after the tDCS intervention. Forty individuals were randomly assigned to either the anodal tDCS group, which received 5 sessions of the 20 min tDCS over the DLPFC (2 mA; n = 20), or the sham tDCS ( n = 20). Participants' stress levels, anxiety, depressive symptoms, and state affects were assessed and compared before and after the intervention. The ERP was collected through electroencephalography (EEG) technology during an attentional network test. Results: After the anodal tDCS, we found a significant decrease in the perceived stress scale (PSS) scores (from an average score of 35.05 to 27.75), p = 0.01 as well as the State-Trait Anxiety Inventory (STAI) scores, p = 0.002. Better performance in the attentional network test, a significant reduction in the N2 amplitudes, and an enhancement in the P3 amplitudes (both cues and targets) were also found in the anodal tDCS group. Discussion: Our study findings suggest that tDCS to the left DLPFC could effectively relieve chronic stress, potentially reflected by increased attentional control.

Abstract:Objective: This study examined effects of transcranial direct current stimulation (tDCS) over the dorsolateral prefrontal cortex (DLPFC) and orbitofrontal cortex (OFC) on major executive functions (EFs), including response inhibition, executive control, working memory (WM), and cognitive flexibility/task switching in ADHD. Method: ADHD children received (a) left anodal/right cathodal DLPFC tDCS and (b) sham stimulation in Experiment 1 and (a) left anodal DLPFC/right cathodal OFC tDCS, (b) left cathodal DLPFC/right anodal OFC tDCS, and (c) sham stimulation in Experiment 2. The current intensity was 1 mA for 15 min with a 72-hr interval between sessions. Participants underwent Go/No-Go task, N-back test, Wisconsin Card Sorting Test (WCST), and Stroop task after each tDCS condition. Results: Anodal left DLPFC tDCS most clearly affected executive control functions (e.g., WM, interference inhibition), while cathodal left DLPFC tDCS improved inhibitory control. Cognitive flexibility/task switching benefited from combined DLPFC-OFC, but not DLPFC stimulation alone. Conclusion: Task-specific stimulation protocols can improve EFs in ADHD.

Abstract:Background: To assess the efficacy of using transcranial direct current stimulation (tDCS) to remediate the deleterious effects of fatigue induced by sleep deprivation and compare these results to caffeine, a commonly used fatigue countermeasure. Objective/hypothesis: Based on previous research, tDCS of the dorsolateral prefrontal cortex (DLPFC) can modulate attention and arousal. The authors hypothesize that tDCS can be an effective fatigue countermeasure. Methods: Five groups of ten participants each received either active tDCS and placebo gum at 1800, caffeine gum with sham tDCS at 1800, active tDCS and placebo gum at 0400, caffeine gum with sham tDCS at 0400, or sham tDCS with placebo gum at 1800 and 0400 during 36-h of sustained wakefulness. Participants completed a vigilance task, working memory task, psychomotor vigilance task (PVT), and a procedural game beginning at 1800 h and continued every two hours throughout the night until 1900 the next day. Results: tDCS dosed at 1800 provided 6 h of improved attentional accuracy and reaction times compared to the control group. Caffeine did not produce an effect. Both tDCS groups also had an improved effect on mood. Participants receiving tDCS reported feeling more vigor, less fatigue, and less bored throughout the night compared to the control and caffeine groups. Conclusions: We believe tDCS could be a powerful fatigue countermeasure. The effects appear to be comparable or possibly more beneficial than caffeine because they are longer lasting and mood remains more positive.

Abstract:Transcranial direct current stimulation (tDCS) has been shown to have mixed effects on working memory (WM) capacity in healthy individuals. Different stimulation paradigms may account for these discrepancies, with certain features being favored. To determine the effect in the context of anodal tDCS, we investigated whether anodal tDCS induced cortical oscillatory changes during a WM task. Specifically, we tested whether anodal offline tDCS over the left prefrontal cortex (PFC) enhances WM capacity by modulating the oscillatory activity in the left dorsolateral PFC (DLPFC) using magnetoencephalography (MEG). This study employed a double-blind, randomized, crossover design, in which 24 healthy right-handed participants conducted MEG recordings during a 3-back task after administration of 2 mA tDCS or sham stimulation as a placebo. Our results showed that the effect of tDCS did not appear in the behavioral indices—WM accuracy (d′) or reaction time (RT). From the results of the time-frequency analysis, significant event-related synchronization (ERS) in the high-gamma band (82–84 Hz) of the left DLPFC was found under the tDCS condition; however, ERS was not correlated with WM capacity. Furthermore, we calculated the modulation index (MI), which indicates the strength of phase-amplitude coupling (PAC). tDCS significantly decreased MI of the left DLPFC, representing the theta-gamma PAC during the n-back task using color names as verbal stimuli. Our results suggest that although tDCS increased the gamma-band oscillation indicating greater neural activity in the left DLPFC, it did not lead to an improvement of WM capacity; this may be due to the inability of gamma-band oscillation to couple with the task-induced theta wave. WM capacity might not increase unless theta-gamma PAC is not enhanced by tDCS.

Abstract:Background: Transcranial direct current stimulation (tDCS) has been widely studied for treatment of patients with prolonged disorders of consciousness (PDOC). The dorsolateral prefrontal cortex (DLPFC) is a hot target for intervention, but some controversies remain. Purpose: This review aimed to systematically investigate the therapeutic effects of DLPFC-anodal-tDCS for patients with PDOC through a meta-analysis approach. Data sources: Searches for relevant articles available in English were conducted using EMBASE, Medline, Web of Science, EBSCO, and Cochrane Central Register of Controlled Trials from inception until March 26, 2022. Study selection: All randomized parallel or cross-over controlled trials comparing the effect of intervention with active-tDCS and Sham-tDCS on Coma Recovery Scale Revised (CRS-R) score in individuals with PDOC were included. Data extraction: Two authors independently extracted data, assessed the methodological quality, and rated each study. Data synthesis: Ten randomized parallel or cross-over controlled trials were eligible for systematic review, and eight of the studies involving 165 individuals were identified as eligible for meta-analysis. Compared with Sham-tDCS, the use of anode-tDCS over DLPFC improved the CRS-R score (SMD = 0.71; 95% CI: 0.47–0.95, I 2 = 10%). Patients with PDOC classified as MCS and clinically diagnosed as CVA or TBI may benefit from anode-tDCS. Limitations: Failure to evaluate the long-term effects and lack of quantitative analysis of neurological examination are the main limitations for the application of anode-tDCS. Conclusions: Anodal-tDCS over the left DLPFC may be advantageous to the recovery of patients with MCS and clinically diagnosed with CVA or TBI. There is a lack of evidence to support the duration of the disease course will limit the performance of the treatment. Further studies are needed to explore the diversity of stimulation targets and help to improve the mesocircuit model.
