Abstract:Acetyl-CoA carboxylase (ACCase; EC 6.4.1.2) catalyzes the synthesis of malonyl-CoA, the first intermediate in fatty acid synthesis. Genes for multi-functional acetyl-CoA carboxylase and two subunits of multi-subunit acetyl-CoA carboxylase, biotin carboxylase and β-Carboxyltransferase, have been cloned from peanut. The ORF of the three proteins are 6783bp, 1623bp, 1479 bp in length, encoding 2260, 540, 492 amino acids, respectively. The predicted amino acid sequences of the multi-functional AhACCase and AhBC from peanut shared high sequence identity, 90.9% and 88%, to the corresponding Glycine max proteins, respectively, while the amino acid sequence of AhβCT gene showed 71.6% identity with that from Nothofagus gunnii. Phylogenetic analysis showed that all of these three genes from peanut were grouped into the big branch of proteins from higher plant. Moreover, observation of the tree revealed that multi-functional ACCase genes may arise by independent gene duplication events in green algae and higher plants branches, and the biotin carboxylase and β-Carboxyltransferase genes from cyanobacteria may be the origin of genes from higher plants, mosses and eukaryotic algae.
Effect of office chair design features on lumbar spine posture, muscle activity and perceived pain during prolonged sitting

Abstract:Chair design features are typically compared using multiple seats, which can lead to confounding effects. Using a single chair, configurable to four designs (control, lumbar support, seat pan tilt and scapular relief), we investigated the effect of chair design on spine posture and movement, muscle activity and perceived pain in a sample of 31 asymptomatic adults. A total of 39% of the population were classified as pain developers, having significantly higher peak pain levels across most body regions. The lumbar support and seat pan tilt condition resulted in more neutral spine and pelvic postures. Greater muscle activity was found in the seat pan condition and non-pain developers displayed lower spine muscle activation levels overall. Despite some improvements in spine posture, sitting-induced pain was present in the study sample at similar proportions to those reported previously. Future studies may consider investigating interventions targeted to sitting-induced pain developers as opposed to the general population. Practitioner summary: Four office chair configurations were tested. The lumbar support and seat pan tilt conditions resulted in the most neutral back posture but did not mitigate the clinically significant levels of sitting-induced pain experienced by a large portion of the tested sample. Future work should target interventions to these individuals.

Surgical Treatment of Cervical Myeloradiculopathy Associated with Movement Disorders: Indications, Technique, and Clinical Outcome

Abstract:Objective: Movement disorders may be associated with advanced cervical myeloradiculopathy, which represents a major management challenge. We report on eight patients with movement disorders causing progressive cervical myeloradiculopathy who were treated successfully by cervical decompression and reconstruction. Results: The mean age of our patients was 44 years with a male/female ratio of 3:1. The average duration of symptoms prior to presentation was 10 months. The most common levels decompressed and reconstructed were C3-C4 and C4-C5. Six cases showed improvement, and two cases showed stabilization of neurologic status at a mean follow-up of 21 months. Our management strategy and results are interpreted in the context of a systematic review of the literature in which 78 cases are reported. Conclusions: Movement disorders cause premature cervical spondylosis most commonly involving the C3-C4 and C4-C5 levels. Ventral pathology with kyphotic angulation requires corpectomy or discectomy with or without posterior decompression and reconstruction. Decompression should always be combined with segmental internal fixation. Perioperative use of botulinum toxin and halo vest immobilization can increase the rate of clinical success but requires vigilance to minimize complications. Laminectomy with lateral mass fixation may be used successfully in the absence of kyphotic deformity.

Abstract:Purpose: Occupational musculoskeletal disorders are prevalent in ophthalmic surgeons and can impact surgeons' well-being and productivity. Heads-up displays may reduce ergonomic stress compared to conventional microscopes. This cross-sectional, non-interventional study compared ergonomic experience between heads-up display and conventional ocular microscopes. Methods: The study protocol was approved by the independent ethics committee and nonprofit organization MINS Institutional Review Board. An online questionnaire was distributed to a sample of ophthalmic surgeons in Japan with experience operating with heads-up display. The questionnaire captured surgeon-specific variables, the standardized Nordic Musculoskeletal Questionnaire, and custom questions to compare heads-up display and conventional microscope and understand long-term impacts of musculoskeletal disorders. Results: Analysis was conducted on responses from 67 surgeons with a mean 25 years of practice and 2.7 years using heads-up display. Many surgeons agreed or strongly agreed that heads-up display reduced the severity (40%) and frequency (40%) of pain and discomfort, improved posture (61%), and improved overall comfort (61%). Of respondents who experienced asthenopia (n = 59) or pain/discomfort during operation (n = 61), 54% reported improvement in asthenopia and 72% reported feeling less pain/discomfort since using heads-up display. Overall, 69% reported preference for heads-up display. Conclusion: This study provides novel data on musculoskeletal disorders and the long-term impacts of ergonomic strain reported by ophthalmologists building on existing literature demonstrating ergonomic and other advantages of heads-up display. Future studies with objective ergonomic assessment are warranted to validate these findings.

Abstract:Laparoscopic surgeons can experience musculoskeletal disorders due to standing for long periods during surgical procedures. Surgery chair can help laparoscopic surgeons in overcoming these problems so that they carry out surgical procedures in a sitting position. However, the surgery chairs currently on the market use a hand-controlled pedal for the lifting mechanism, which is an obstacle for laparoscopic surgeons because it interferes with the concentration of the hands in using surgical tools. This study contributes to the development of the latest design of a surgery chair designed for Indonesian laparoscopic surgeons with a foot-controlled pedal so that it does not interfere with the concentration of the laparoscopic surgeon in using surgical tools to carry out surgical procedures. The lifting mechanism of the proposed surgery chair uses a worm gear with the rationalization of the rotation lock, space efficiency, and minimizes sound when lifting mechanism. Computational simulations via the finite element method were carried out on the mechanical components of the current surgery chair to analyze the von Mises stress under the down, middle, and top lifting mechanism conditions. The von Mises stress results show that the proposed surgery chair design is safe. This achievement is expected to be an initiation for the development of a better surgery chair in the future.

Effects of backrest design on biomechanics and comfort during seated work

Abstract:The purpose of this study was to examine the effects of backrest configuration on seatpan and backrest pressure, spinal posture, and comfort. Thirty volunteers (15 male, 15 female) typed a standardized text passage while seated at a computer workstation in five backrest configurations: chair only, chair with a supplementary backrest, and with each of three lumbar pad thicknesses. Pressure, lumbar and cervical angles were collected during 15-min trials. Subjective data were collected during each trial and at the end of the entire protocol. The addition of a supplementary backrest to a standard chair reduced peak and average pressure on the back by 35% and 20%, respectively (P<0.02). Lumbar lordosis was observed only when lumbar pads were used, being greatest with the large pad. Participants preferred backrest configurations that had lower pressure on the back and less lordotic lumbar posture (backrest only or 3 cm lumbar pad), regardless of anthropometrics. Comfort was rated highest in conditions that would not necessarily be considered biomechanically ideal. Further delineation between specific comfort and objective seating variables is required to effectively reduce and prevent low back pain.

Abstract:Background: This study was prompted by the prevalence of back injury and pain in the working population, particularly amongst workers who are increasingly exposed to sedentary work in industrialised countries, and the corresponding limited evidence regarding the effectiveness of seating designs currently used in the workplace. Objective: The purpose of this study was to investigate the differences in comfort, productivity, and posture between the BambachTM saddle seat and a standard office chair (two chair designs used within the workplace today). Method: A single system, multiple-baseline research design across a sample of four subjects was used. A withdrawal A B A B design was utilised, with the ordering of the sequence varying with each subject. "A" represented the standard office chair, and "B" the BambachTM saddle seat. Results: Discomfort ratings tended to increase over time regardless of the seat being used. However, while the saddle seat provided reduced levels of lower back discomfort, it demonstrated higher discomfort in the lower limbs, particularly the hips and buttocks. There were no significant differences identified in productivity between the two chairs. The saddle seat consistently promoted a greater trunk-to-thigh angle for all subjects, a position associated with optimum sitting posture. Conclusion: This study has implications for the treatment of low back injury and pain at work, as well as other daily activities that involve prolonged static sitting, such as those incorporated in self maintenance, leisure and rest activities. This study provides health professionals with a systematic investigation of the immediate effects of using both the BambachTM saddle seat and standard office chair in sitting. The findings of this study should be considered in future research.

Abstract:INTRODUCTION: Workplace posture has long been associated with back and neck pain, and surgeons are not exempt. Though intraoperative surgeon posture has progressively improved with the visualization of the operative microscope, surgeons nonetheless continue to sustain deviated postures during operations which may result in increased rates of neck and back pain. The surgical exoscope is designed to improve surgical ergonomics. This study aims to quantify the difference in posture during standard anterior cervical spine procedures when performed with the aid of an operative microscope versus an exoscope. METHODS: This is a prospective cohort study utilizing the MetaWare MetaMotion S device placed at the surgeon’s cervicothoracic junction to measure Euler angles of upper thoracic spine during each case. Primary outcome was the percentage of time spent with greater than 10deg deviation from a surgeon’s baseline posture while using either the microscope or the exoscope. Additional metrics were analyzed for average deviation from baseline for each axis, as well was percentage of operative time spent with the microscope or the exoscope. RESULTS: Overall, 37 anterior cervical procedures were recorded, 18 performed with an exoscope and 19 with a microscope. Surgeons spent significantly more time in a deviated posture with the operative microscope than with the exoscope (32% vs 8% with x-axis >10 deg, p 10 deg, p < 0.05). This is also reflected with significant differences in the average deviation in the x- and y- axis while under scope. CONCLUSIONS: Utilizing the exoscope for anterior cervical spine procedures allowed for less time spent in deviated posture.

Abstract:The three-dimensional heads-up display system (3D HUDS) is increasingly utilized by ophthalmologists and suggested to offer ergonomic benefits compared to conventional operating microscopes. We aimed to quantitatively assess the surgeon's neck angle and musculoskeletal discomfort during cataract surgery using commercially available 3D HUDS and conventional microscope. In this single-center comparative observational study, the surgeon conducted routine phacoemulsification surgeries using Artevo® 800 and Opmi Lumera® 700 (both from Carl Zeiss Meditec, Jena, Germany). The surgeon's intraoperative neck angle was measured using the Cervical Range of Motion device. Postoperative musculoskeletal discomfort was assessed using the Visual Analog Scale (VAS) score after each surgery. A total of 80 cataract surgeries were analyzed, with 40 using Artevo® 800 and 40 using Opmi Lumera® 700. The neck angle was extended when using Artevo® 800 and flexed when using Opmi Lumera® 700 during continuous curvilinear capsulorhexis (CCC), phacoemulsification, and intraocular lens (IOL) placement (− 8.18 ± 2.85° vs. 8.27 ± 2.93° in CCC, − 7.83 ± 3.30° vs. 8.87 ± 2.83° in phacoemulsification, − 7.43 ± 3.80° vs. 7.67 ± 3.73° in IOL placement, respectively; all p < 0.001). The VAS score was significantly lower in surgeries performed with Artevo® 800 (1.27 ± 0.55 vs. 1.73 ± 0.64, p < 0.001). The findings suggest that 3D HUDS help reduce neck flexion and lower work-related musculoskeletal discomfort through ergonomic improvements.

Abstract:BACKGROUND: Most posture problems encountered in persons who use wheelchairs in a seated posture for extended periods are related to sacral sitting due to posterior pelvic tilt. Posterior pelvic tilt places pressure and shearing force on the sacrococcygeal area that can lead to pressure ulcers, but the relationship between pelvic tilt and force applied to the sacrococcygeal and ischial tuberosity areas has not yet been investigated.OBJECTIVE: To investigate the relationships of posterior pelvic tilt in a seated posture with vertical force and horizontal force on the sacrococcygeal and ischial tuberosity areas.STUDY DESIGN: Repeated measures design.METHODS: Thirty male and female subjects aged ≥60 years sat in a measurement chair at varying pelvic tilt angles, and force on the sacrococcygeal and ischial tuberosity areas was measured.RESULTS: The pressure on the sacrococcygeal area increased with pelvic tilt in all subjects, with vertical force averaging 19% of the body weight at a pelvic tilt angle of 30°. The horizontal force on the sacrococcygeal area increased in 93% of the subjects, with an average increase equal to 3% of the body weight.CONCLUSIONS: We confirmed changes in vertical and horizontal forces on the sacrococcygeal and ischial tuberosity areas with a change in seated posture (pelvic tilt).CLINICAL RELEVANCE: We propose guidelines for rehabilitation practitioners working with wheelchair users to suggest improved ways of sitting in wheelchairs that avoid pelvic tilt angles that might promote pressure ulcers on the buttocks.

Abstract:Objective: Many plastic surgeons experience musculoskeletal strain and injury from poor ergonomics during surgery. This is associated with reduced performance, shortened careers, and decreased quality of life. This study compared the ergonomics of the conventional operating table headrest and the Mayfield headrest in craniofacial surgery. Methods: A prospective cohort study of patients undergoing craniofacial operations between November 20, 2022 and April 26, 2023, within a single craniofacial surgeon’s practice. The authors obtained data on the total duration of the operation and Rapid Entire Body Assessment (REBA) scores for the primary surgeon and assistant. Results: Four operations (mean: 147 ± 60.9 min) were included in the regular headrest group, and 8 in the Mayfield headrest group (mean: 61±53.4 min). Four hundred fifty-five regular headrest time points and 851 Mayfield time points were recorded. Eight hundred thirty-five regular headrest time points and 538 Mayfield time points were recorded. The mean REBA score for the regular headrest was 5.79 ± 1.9, which was higher than the Mayfield (5.01 ± 2.0; P < 0.0001). Subgroup analysis showed the mean REBA score for the primary surgeon (5.89 ± 2.0) was higher than the assist (5.48 ± 1.6) in the regular headrest group ( P < 0.0001), whereas the converse was true for the Mayfield headrest (primary surgeon: 4.67 ± 1.8, assist: 5.65 ± 2.15, P < 0.0001). Conclusions: Ergonomic scores were better using the Mayfield headrest than the regular headrest. The primary surgeon scored better with the Mayfield headrest, whereas the assists had better scores with the regular headrest.

Abstract:Delayed response performance was assessed in 120 7-, 9-, and 11-month-old infants with correct response defined as either retrieval of a hidden object or gaze toward the location where the object was hidden. Performance improved with age, was above chance for each age group in each condition, and was more often correct with the gaze response. When direction of gaze and reach differed, direction of gaze was more likely to be correct. Infants in the reach condition were more likely to fail to reverse a previously correct response (i.e., to make the A-not-B error). Perseverative responding occurred frequently and was more likely in the reach than the gaze condition. This effect emerged primarily in the context of an incorrect response, which suggests modality-specific sensitivity to the effect of priming rather than reinforcement. Many infants showed strong side biases, and there was a tendency for more reaches to the left but gazes to the right. In a second experiment, 12 5-month-olds gazed toward the correct location more frequently than would be expected by chance but failed to reverse a previously correct response more often than older infants. These findings indicate that response modality has a significant effect on delayed-response performance.

Abstract:Objectives Surgeons experience a risk for physical strain and injury secondary to physical demands in the workplace. To minimize injury and maximize career longevity, physicians should be aware of ergonomics pitfalls and postural correction methods. This study investigates ergonomic trends in Facial Plastic and Reconstructive Surgery (FPRS) clinic by quantifying surgeons' and trainees' cervicothoracic spine posture. Methods Participants completed a 22‐item questionnaire to evaluate current ergonomic practices. A lightweight device was calibrated and attached to the mid‐scapular region of participants, providing real‐time posture feedback. The percentage of time in upright posture was recorded during clinical and operative workdays. Upright posture was defined as neutral spine positioning with acceptable mild to moderate deviations. Results Two FPRS attending surgeons, 1 FPRS fellow, and 11 otolaryngology residents participated over 12 months. Discomfort was most commonly reported in the neck, shoulders, and upper back during clinic. Symptoms were self‐treated by changing body position, wearing specialized footwear, adjusting height of the chair or examination table, or ignoring discomfort. Intraoperative Electromyogram Analysis of Postural Musculature With Three-dimensional Screen Display Versus Operating Microscope in Ophthalmic Surgery: A Clinical Methods Study

Abstract:Background and objective: To objectively analyze the effect of three-dimensional screen-based surgery (3D SBS) versus traditional operating microscope (TOM) on operating surgeon posterior chain postural musculature during ophthalmic surgery. We hypothesized an increase in median amplitude of electromyography (EMG) signals when using a TOM compared to 3D SBS. The goal was to assess surgical ergonomics that may contribute to cervical and lumbar spine pathology. EMG analysis was conducted at the University of Cincinnati Medical Center Epilepsy Monitoring Unit. Data were collected in the private practice setting at Cincinnati Eye Institute. Materials and methods: This was an institutional review board exempt, nonrandomized, prospective, single center, n = 1 clinical methods study. EMG surface electrodes were affixed to the bilateral splenius capitis, sternocleidomastoid, upper trapezius, anterior deltoid, and quadratus lumborum musculature. EMG data were collected across four sessions consisting of one day each using 3D SBS or a TOM, and two cross-over days. A survey regarding surgeon comfort was administered before, during, and after each surgery, and at the end of each day. Results: EMG cross-over data demonstrated an increase in muscle activation in all measured muscles except the anterior deltoids and right quadratus lumborum with use of a TOM. Survey data showed increased fatigue when using a TOM relative to 3D SBS. Conclusion: EMG demonstrated increased postural muscle activation when utilizing TOM relative to 3D SBS. Use of Soft Cervical Collar Improves Surgeon Ergonomics During Simulated Otologic Surgery

Abstract:Objective To determine whether surgeon use of a soft cervical collar during endoscopic and microscopic otologic surgery is feasible and impacts surgeon ergonomics as measured by inertial sensors. Study Design Prospective crossover trial. Setting US-based otolaryngology training program. Patients Otolaryngology residents and fellows. Interventions Therapeutic—use of a soft cervical collar during simulated otologic surgery. Main Outcome Measures Time spent in high-risk angles of neck and back flexion and extension; average angle of neck flexion, extension, rotation, and lateral bending; validated assessment of neck pain; average daily phone use. Results Fifteen subjects met criteria for inclusion. Ten of 15 (67%) were male. Seven of 15 (47%) were postgraduate year 1–2. Seven of 15 (47%) reported a history of neck pain. None reported prior spinal steroid injections or surgery. Across all subjects, use of the soft cervical collar significantly reduced time spent in high-risk angles of neck flexion/extension during both endoscopic (56% vs. 35%, p < 0.05) and microscopic (60% vs. 32%, p < 0.05) otologic surgery. There was no effect on back flexion or extension. There was no difference in time spent in high-risk neck or back angles between endoscopic and microscopic surgery. Average angles of neck or back flexion, extension, lateral bending, and rotation were not significantly different for subgroups with more operative experience, increased phone use, perception of good posture, or history of neck pain. Conclusions Use of a soft cervical collar during simulated otologic surgery significantly reduced time spent in high-risk neck positions. These data support feasibility of soft collar use during otologic surgery and hold promise for reduction in the high rates of neck pain reported by neurotologists. Abstract:Wnt proteins initiate the canonical (β-catenin-regulated) signaling cascade by binding to seven-transmembrane spanning receptors of the Frizzled (Fz) family together with the coreceptors LRP5 and -6, members of the low density lipoprotein receptor-related protein family (LRP). Several reports have shown physical and functional associations between various Wnt, LRP, and Frizzled molecules; however, the underlying mechanisms for selectivity remain poorly understood. We present data on a novel set of Wnt-Fz fusion constructs that are useful for elucidating mechanisms of Wnt signal transduction specificity in both Xenopus embryos and 293T cells. In 293T cells, coexpression of several Wnt-Fz fusion proteins with LRP6, but not LRP5, significantly activated a Wnt-responsive promoter, Optimized TOPFlash. Interestingly, Wnt proteins from both the Wnt1 and Wnt5A classes, when fused to the same Frizzled, can synergize with LRP6 to activate signaling and induce secondary axes in Xenopus embryos. However, when several Wnt-Fz constructs containing different Frizzled molecules were tested, it was found that all Frizzled molecules are not equivalent in their ability to activate the canonical Wnt pathway in this context. The data suggest that the distinction between the two Wnt classes lies not in intrinsic differences in the molecules but via the Frizzled molecules with which they interact.
Postural shifts and body perceived discomfort during 1-hour sitting when leaning and sitting on an air-filled seat cushion among healthy office workers

Abstract:This study compared the number of postural shifts and perceived discomfort while leaning and sitting on an air-filled seat cushion for 1 hour. Sixty office workers typed a standard text while leaning on a cushion placed behind the low back, sitting on a cushion placed under the buttocks, and sitting without a cushion (a control condition). The number of postural shifts was collected using a seat pressure mat device. Low back discomfort was assessed using the Borg CR-10 scale. Leaning on a seat cushion (22 shifts/h) led to a significantly higher number of postural shifts than sitting on a seat cushion (18 shifts/h) and the control condition (20 shifts/h). Leaning or sitting on a seat cushion significantly decreased low back discomfort compared to the control condition (p < 0.05). Leaning on a seat cushion placed behind the low back may be an effective means of preventing low back pain among office workers. During prolonged sitting, using a seat cushion, whether leaning or sitting on it, may be more efficient in preventing low back pain compared to not using one. Particularly, leaning on a seat cushion led to more postural shifts during sitting compared to sitting on one or not using any.

Ergonomics in Spine Surgery: A Systematic Review

Abstract:Study design: This study is a systematic review. Objective: This systematic review aims to synthesize existing studies and highlight the significance of ergonomic considerations for spine surgeons' well-being and the impact on patient outcomes. Summary of background data: Spine surgery is a physically demanding field that poses several risks to surgeons, particularly with musculoskeletal disorders. Despite the well-documented consequences of musculoskeletal injuries endured by surgeons, surgical ergonomics in spine surgery has received limited attention. Methods: Following PRISMA guidelines, a comprehensive literature search was conducted in PubMed and Embase. Studies focusing on surgeon ergonomics in spine surgery were selected. Data extracted included study details, surgeon demographics, ergonomic factors, and outcomes. Qualitative analysis was performed due to the heterogeneous nature of study designs and criteria. Results: Eleven studies met the inclusion criteria. Six studies utilized surveys to explore physical challenges, prevalence rates of pain, work practices, and ergonomic tools. Two studies employed optoelectronic motion analysis to assess the spinal angles of the surgeon during surgery. Two studies assesed ergonomics in different visualization methods using rapid entire body assessment (REBA). One study applied video analysis to scrutinize surgeons' neck postures during the case. The results demonstrated a varying prevalence and diverse presentations of musculoskeletal disorders, varying impact on surgical performance, and nuanced relationships between experience, workload, and ergonomic concerns. Conclusion: This systematic review summarizes the heterogeneous evaluations of ergonomics in spine surgery. Overall, upwards of three-quarters of spine surgeons have reported musculoskeletal discomfort, most commonly presented as back pain, neck pain, and hand/wrist discomfort. These symptoms are often exacerbated by the use of loupes, operating bed height, and extended periods of time in various positions. Studies demonstrate that physical discomfort is associated with the surgeons' mental and emotional well-being, leading to stress, burnout, and reduced job satisfaction, all of which impact patient care.
Effect of operation table height on ease of mask ventilation, laryngeal view, and endotracheal intubation success

Abstract:Background and aims: Optimal patient positioning and operating table height are essential for an ergonomic posture of an anesthesiologist in which there is minimal or no strain on thewrist during mask ventilation. It also avoids flexion of the neck, lower back, and knee bending at the time of laryngoscopy and intubation. Material and methods: One hundred eighty patients were randomly allocated to three groups based on different table heights. The height of the table is kept at the mid-sternum level of an anesthesiologist in group 1, at the xiphoid process in group 2, and at the level of umbilicus in group 3. Laryngoscopic view with or without postural changes (exertion at wrist joint, flexion of the neck, lower back, or knee bending) was graded as per Cormack Lehane's (CL) grading. The degree of discomfort experienced by the anesthesiologist during mask ventilation or tracheal intubation was graded subjectively (1 = no discomfort, 2 = mild discomfort, 3 = moderate discomfort, and 4 = severe discomfort) at different table heights. Postural changes required to obtain the best glottic view and quality of endotracheal (ET) intubation (intubation time and attempts required) were also noted. For analysis, quantitative variables were expressed as mean ± SD and compared using unpaired t or analysis of variance test. Qualitative variables were expressed as frequencies/percentages and compared using the Chi-square test. Results with P value <0.05 were considered significant statistically. Results: Moderate discomfort (strain at wrist joint) during bag-mask ventilation was experienced by the anesthesiologist in a maximum number of patients in group 1 (81.7%). Significant improvement was seen in CL grade after the use of postural modifications in groups 1 and 2 (P value ≤0.05). Greater postural modifications were required during ET intubation at lower table heights (group 3). Conclusions: It is advisable to adopt higher table positioning in relation to anesthesiologist performing the laryngoscopy for smooth and single-attempt ET intubation since the best laryngoscopic view and intubation with minimal postural modifications was seen at higher table heights (at the mid-sternum level of an anesthesiologist).

Abstract:Background and Objective: Three-dimensional heads-up display (HUD) systems have emerged as an alternative to standard operating microscope (SOM) in the operating room. The goal of this study was to quantitatively measure vitreoretinal surgeon posture across visualization methods. Methods: Ergonomic data was collected from 64 cases at two tertiary eye care centers. Surgeons wore an Upright Go 2 TM posture training device while operating either using the NGENUITY 3D heads-up display visualization system or the SOM. Results: Total percentage of time with upright posture as primary surgeon was significantly higher in surgeries performed using HUD (median 100%, interquartile range [IQR], 85.1% to 100.0%) as compared to surgeries performed using the SOM (median 60.0%, IQR 1.8% to 98.8%) ( P = 0.001, Wilcoxon rank-sum test). Percent time with upright posture was significantly higher in surgeries performed using HUD for two of the three surgeons when assessed independently across systems. Results remained significant when accounting for length of surgery ( P < 0.001, multiple linear regression). Conclusions: Ergonomic positioning was improved for surgeons operating using HUD. Given the high prevalence of back and neck pain among vitreoretinal surgeons, increased use of HUD systems may limit musculoskeletal pain and long-term disability from poor ergonomics.
