Abstract:BACKGROUND: Thread lifting with absorbable sutures has recently gained prominence for rejuvenating ptotic and sagging soft tissues in aging face. A barbed, delayed absorbable polyglyconate monofilament shows significantly high tensile loads with superior flexibility and elasticity.OBJECTIVE: To evaluate the safety and efficacy of novel thread lifting regimen with barbed polyglyconate suture for counteracting the descent and laxity of the aging face.METHODS: A retrospective chart review was conducted for 25 Korean patients with mild to moderate facial skin laxity receiving thread lifting procedures using barbed polyglyconate suture. Dermatologists' objective assessments using a 5-point Global Aesthetic Improvement Scale (GAIS), 3D imaging system, and subjective satisfaction were recorded at baseline and 4 months after treatments.RESULTS: In most patients (84%), GAIS scores were more than score 3 ("improved"), with 40% of score 4 ("much improved") and 24% of score 5 ("excellent") at the follow-up point. Subjective assessments paralleled these patterns. Three-dimensional analysis quantitatively showed volume migration of low face upward, clinically interpreted as facial lifting. No serious adverse effect was observed.CONCLUSION: This novel thread lifting procedure using barbed polyglyconate suture is a safe and effective method for tightening and lifting for aging face with mild to moderate face sagging.

Abstract:Introduction: The facial aging process produces changes that are characteristic of the superficial and deep fat framework and skin layers. Subdermal suspension with threads enables the sagging tissues to be lifted by means of a minimally invasive, closed procedure without surgical dissection. This observational study has been carried out on the basis of standardized tridimensional photographic analysis and measurement, aimed at determining objective, repetitive, and reliable evaluation of the soft tissue suspension technique. Materials and Methods: Eight participants were enrolled in this pilot study presenting with mild to moderate ptosis of the jawline tissues. Patient photographs were taken before (t0), immediately after threads implantation (t1), and at the following visit (t2). Each image captured before thread insertion was registered by the software and surface linear lengths in between the mentioned points were calculated. Results: The result showed an overall average improvement in the "tragus-to-marionette distance" (C-A) and the "tragus-to-jowl distance" after a mean follow-up time of 8.16 months (t0-t2). All analyzed parameters improved significantly (P < 0.05) at t1 and at t2 with respect to t0. Conclusions: This pilot study suggest that facial tissue suspension by means of poli-lactic/poli-caprolactone threads is safe and effective in treating skin flaws that affect mild-to-moderate ptosis of the jawline up to 8 months.

Abstract:The search for less invasive surgical techniques to address the effects of facial aging led to the development of barbed polypropylene sutures for facial suspension. Theoretical advantages of these "threadlifts" included limited scarring, rapid recovery, relative safety, and reduced cost when compared with a standard rhytidectomy. The goal of this study was to evaluate the outcomes of patients undergoing threadlifts to determine the actual complication rates, the durability of results, and the rates of reoperative surgery. A single surgeon's initial 2-year experience with 72 patients undergoing threadlifts was retrospectively reviewed. Preoperative patient demographical and clinical data, operative information, and postoperative outcomes data were compiled and evaluated. A total of 72 thread lifts were performed by 1 surgeon over a 24-month period. Of these patients, 76% underwent threadlift alone, whereas concomitant procedures were performed in 24% of patients. Minor complications were common and usually self-limited. Forty-two percent of patients underwent a secondary procedure after primary threadlift, an average of 8.4 months after the original surgery. Thirty-one percent of patients required revisional surgery for cosmetic reasons an average of 8.7 months after their threadlift. Eleven percent of the patients ultimately required removal of palpable threads. Threadlift is a safe procedure associated with minor complications. Rates of revisional surgery for cosmesis are high after threadlift. Time to revisional surgery for cosmesis is short. Results achieved by threadlift are subtle and short-lived. Threadlift is not a minimally invasive replacement of surgical rhytidectomy. Patients should understand the limitations of this technique and its high rates of revisional surgery.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> INTRODUCTION</h3><p>Volume restoration then remains the principal target to be addressed when approaching midface rejuvenation, and absorbable sutures can create a suspension system that addresses ptotic skin located primarily in the midface.</p><p>This observational study has been carried out to determine the outcome of thread lifting procedures of the midface.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> MATERIALS AND METHODS</h3><p>six participants were enrolled and treated with suspension threads to correct mild to moderate ptosis of the flabby tissues of the midface. Volumetric changes were calculated at t0 (pre‐treatment), t1(120days) and t2 (365 days), with a mean follow‐up time of 349.64 days, (range from 304 to 380.5 days).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> RESULTS</h3><p>showed an overall average improvement of 5.59 ml at 120 days (t0‐t1) after thread implantation, up to a mean value of 4.16 ml at the end of the 350 days follow up (t0‐t2). A comparison was made in between and statistical analysis was performed with level of significance set at P < 0.05.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> DISCUSSION</h3><p>The aim of this pilot study was to investigate the effectiveness in lifting sagging tissue of the midface: the results shown suggest that it is possible to achieve volume restoration which lasts up to 12 months as all parameters improved significantly (P < 0.05) at t1 and at t2 with respect to t0.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> CONCLUSIONS</h3><p>The results suggest that facial tissues suspension by means of threads is safe and effective, as it is possible to achieve tissue repositioning which lasts up to 12 months for the correction of mild to moderate ptosis of the midface.</p><p>This article is protected by copyright. Abstract:Background: Threadlifts are classified as absorbable or nonabsorbable, natural or synthetic, and multifilament or monofilament ones, each of which has its own merits and demerits. We placed a novel absorbable polydioxanone monofilament threadlift (Mint Lift Fine; HansBiomed Co. Ltd., Seoul, Korea) in the subcutaneous fat compartment for facial rejuvenation in patients with nasolabial folds, nasojugal groove, marionette lines, or sagging jowl. Here, we describe its short-term treatment outcomes in a retrospective consecutive series of 21 patients.Methods: A total of 21 patients (n = 21) were evaluated; their treatment outcomes were described based on a comparison between preoperative and postoperative findings and Global Aesthetic Improvement Scale (GAIS) scores were assessed at 6 months. This is accompanied by analysis of the incidence of postoperative complications.Results: The patients had a mean GAIS score of 3.62 ± 0.84 points, which corresponds to "Much improved" or "Very much improved." Moreover, there were no significant differences in the GAIS scores between the target sites (midface and lower face: 3.78 ± 1.13; lower face: 3.43 ± 0.50; and midface: 3.60 ± 0.49; P > 0.05). Furthermore, there were a total of 4 cases (19.0%) of postoperative complications, all of which were spontaneously resolved within a maximum period of 4 months.Conclusions: We describe short-term treatment outcomes of facial rejuvenation using the Mint Lift Fine in patients with nasolabial folds, nasojugal groove, marionette lines, or sagging jowl. But further large-scale, prospective, multicenter studies with long periods of follow-up are, therefore, warranted to establish our results.

Abstract:Lymphoma presenting with isolated diffuse infiltration of the skull is exceedingly rare, with less than 20 previously reported cases. The clinical presentation of a 73-year-old female with primary lymphoma of the skull, manifesting multiple cranial nerve palsies and infiltration of the temporalis muscles is described. MRI of the head, revealed an abnormal signal from the diploic space of the call varium and skull base on both T1 and T2 weighted images, infiltration of the temporalis muscles and clivus, and diffuse meningeal enhancement encroaching on the cavernous sinus bilaterally. Biopsy of temporalis muscle and skull showed a diffuse large cell lymphoma, B-cell type. Staging workup failed to reveal any other sites of disease. Despite multiple cranial nerve palsies, there was no evidence of leptomeningeal disease on CSF examination. MRI was instrumental in demonstrating the abnormalities that lead to the diagnostic biopsy.

Abstract:Original lateral SMASectomy has several advantages over conventional SMAS elevation. However, it was felt that lateral SMASectomy alone caused unsatisfactory results with regard to undercorrection of midface ptosis. Thread lifting was added to counteract the remaining aesthetic problem. The purpose of this study is to describe the combined surgical techniques adopted. The procedure was performed under local anaesthesia with intravenous sedation. When indicated, liposuction for free fat transfer to the malar area, nasolabial folds, is performed with a Coleman fat aspiration cannula. After access skin incision, dissection extends across the zygoma to release the zygomatic ligaments and masseteric cutaneous ligaments. Then the SMAS flap was designed. The superior border of the incision was approximately at the level of the zygomatic arch and the inferior border was ∼4-5 cm below the jaw line. After SMAS resection, closure is performed by elevating the inferomedial corner of the flap in a superolateral direction, fixed lateral portion suture to mobile anterior fascia. Then, thread lifting was performed to correct midface ptosis (medial side). The skin was marked preoperatively to determine the appropriate vector of the thread and its 2-3 end fixation points. The facelift sutures were inserted through the cannula from the midface to the dissected open wound. After that free fat graft was performed for volumetric support, when indicated. The overall surgical time was less than 90 minutes. This combination therapy would be ideal therapy with elderly patients with significant midface ptosis and prominent nasolabial folds.

Abstract:A nanoscale field-effect transistor (FET) was fabricated based on pseudo Y-junction single-walled carbon nanotubes (Y-SWNTs). The Y-junction transistor was modulated by a metallic branch of the Y-SWNTs, and exhibited ambipolar FET characteristics at room temperature. A subthreshold swing of 700 mV/decade and an on/off ratio of 105 with a low off-state current of ~10-13 A were obtained. An Approach to Selection of Face-Lift Techniques for Different Types of Faces

Abstract:Background A face-lift or rhytidectomy is the procedure most directly associated with rejuvenation. There are several surgical techniques for face-lifts, but criteria for the selection of techniques, based on the patient's face shape, are lacking. In this study, we report on our experience with specific indication criteria for each technique and the consequent achievement of good outcomes. Methods From 2015 to 2023, 1 surgeon performed face-lifts on 1000 patients. Three different superficial musculoaponeurotic system (SMAS) techniques (SMAS dissection, SMASectomy, and SMAS plication) were applied depending on the degree of sagging of the patient's lower face, lateral facial profile, and SMAS mobility and condition. Superficial musculoaponeurotic system dissection was considered for the improvement of a square face, sagging jowls, and marionette lines. Superficial musculoaponeurotic system plication was applied with patients with less sagging jowls and prominent zygoma with concave lateral facial profile. SMASectomy was applied with patients with convex lateral facial profiles or when the SMAS was too thin or damaged. Patient postoperative satisfaction was surveyed using the FACE-Q score 1 year post surgery. Results Most of the patients attained natural-looking and long-lasting aesthetic outcomes and exhibited high satisfaction. The patients indicated that they looked about 11.2 ± 5.2 years younger than their actual age after the surgery. The mean satisfaction scores for each facial feature were as follows: cheeks (91.1 ± 7.8), marionette lines (88.5 ± 13.6), lower face and jawline (92.5 ± 14.2), under chin (87.8 ± 15.1), and neck (86.2 ± 18.5). Complications such as facial nerve injury, infection, hematoma, and flap necrosis were very rare. Conclusions Establishing criteria for the selection of face-lift surgical techniques based on the degree of lower face sagging, lateral facial profile, and SMAS mobility and condition led to good outcomes. These criteria can be used by physicians to determine the most effective face-lift surgery technique based on a patient's individual features, which may improve surgical outcomes.

Abstract:Background Minimally invasive facial rejuvenation is a popular and well‐established treatment for patients seeking facial lifting. Objective The authors combined facial microliposuction, radiofrequency and thread lifting technology as a comprehensive treatment (LRT) for facial rejuvenation in a case series of 57 patients. Methods A total of 76 patients underwent surgery, and 75% completed 6 months of follow‐up (57 patients); the patients were aged 26 to 51 (33.6 ± 4.5) years. All patients were followed up for hematoma within 1 week after the operation and for skin numbness and muscle paralysis at 1 week and 3 months postoperatively. Patient satisfaction with the postoperative aesthetic effects was analyzed by Face‐QTM at 6 months after the operation. Results The score for hematoma within 1 week was 1.21±0.33, scores for postoperative skin numbness within 1 week and 3 months were 2.06±0.51 and 1.17±0.32, respectively, and scores for postoperative muscle paralysis within 1 week and 3 months were 1.31±0.55 and 1.00 ± 0, respectively. Fifty‐seven patients completed Face‐QTM (score: 22.1±1.8, standard score: 82.2±14.8). Conclusion LRT is a minimally invasive method that can achieve a synergistic and satisfactory result for patients with mild to moderate facial skin sagging.

Abstract:Background: Thread lifting rejuvenation procedures are re-evolved again, after developing absorbable threads, with very popular spread among plastic surgeons and dermatologists. This study evaluates two years’ outcome of absorbable barbed thread lifting for face and suppose methods for assessment. Material and Methods: Prospective comparative study both objectively and subjectively is designed for 24 months follow up. Results: Thread lifting for face has significant long-lasting considerable skin lifting from3-10mm and high degree of patients’ satisfaction with rate of complications of about 4.8%. Augmented results are obtained when threads were combined with other lifting and rejuvenation modalities Conclusion: Significant facial rejuvenations are got by thread lifting and highly augmented results when they are combined with Botox, fillers and/or platelet rich plasma rejuvenations.

Abstract:BACKGROUND: Face-lifting procedures are often performed to hide the effects of aging. Thread-lifting, a minimally invasive technique for the correction of facial aging, has become increasingly popular, and various materials for the procedure have been developed.OBJECTIVE: This study compared tissue responses to two types of threading sutures placed under rat skin: polypropylene (PP) monofilament mesh suspension thread (a novel face-lifting material) and polydioxanone (PDO) barbed thread.METHODS: Eight rats each were assigned to the PP monofilament mesh suspension, PDO barbed thread, and control groups. Tissue reactions were evaluated 28 days after subcutaneous loading of the materials.RESULTS: Significant increases in tensile strength and the mean area occupied by collagen fibers were evident in skin loaded with PDO barbed thread and PP monofilament mesh suspension thread compared to control skin (<i>p</i><0.05). Compared to sites loaded with PDO barbed thread, those loaded with PP monofilament mesh suspension thread showed a significant increase in the number of collagen fibers and a lower grade of inflammation (<i>p</i><0.05).CONCLUSION: PP monofilament mesh suspension thread has skin-rejuvenating effects comparable to those of PDO barbed thread, but induces a less severe inflammatory response. This indicates that it is a safe and effective material for use in thread-lifting procedures on aging skin.

Abstract:Background Thread lifting is a minimally invasive procedure that enhances facial aesthetics by repositioning sagging tissues with absorbable threads. It requires a comprehensive understanding of facial anatomy for safe and effective results. Aims This study aims to highlight the critical anatomical considerations in thread lifting, including the navigation of facial vascular structures, the protection of facial nerves, manipulation of fat compartments, and engagement of retaining ligaments. These factors are essential for minimizing complications and achieving optimal outcomes. Patients/Methods A review was conducted focusing on the anatomical elements critical to thread lifting. The study analyzed clinical outcomes related to vascular structures, nerve pathways, fat compartments, and ligaments in patients undergoing the procedure. Results The review revealed that careful navigation of facial blood vessels is crucial to avoid complications such as bleeding and bruising. Knowledge of facial nerve pathways is essential to prevent nerve damage, which could result in facial weakness or paralysis. Proper manipulation of facial fat compartments helps address aging‐related changes, and engaging retaining ligaments is vital for a sustainable lift without tissue distortion. Conclusions Thread lifting demands not only technical skill but also a deep understanding of facial anatomy to ensure patient safety and desired aesthetic results. Expertise in these anatomical considerations is essential for minimizing complications and preserving the natural function of facial structures.

Abstract:INTRODUCTION: The increased interest in minimally-invasive treatments, such as the thread lifting, with lower risk of complications, minimum length of time away from work and effectiveness in correcting ptosis and aging characteristics has led many specialists to adopt this technique, but many doubts about its safety and effectiveness still limit its overall use.OBJECTIVE: To analyze data published in the literature on the durability of results, their effectiveness, safety, and risk of serious adverse events associated with procedures using several types of threading sutures.METHODS: Literature review using the key words "thread lift", "barbed suture", "suture suspension" and "APTOS". Due to the scarcity of literature, recent reports of facial lifting using threads were also selected, complemented with bibliographical references.RESULT: The first outcomes of facial lifting with barbed sutures remain inconclusive. Adverse events may occur, although they are mostly minor, self-limiting, and short-lived. The data on the maximum effect of the correction, the durability of results, and the consequences of the long-term suture stay are yet to be clarified.CONCLUSION: Interest in thread lifting is currently high, but this review suggests that it should not yet be adopted as an alternative to rhytidectomy.

Abstract:Background: Management of facial rejuvenation has evolved over past decades. Facelift with barbed suture is a minimally invasive surgical technique for facial rejuvenation. This study examined the efficacy and associated complications of a new thread-looping procedure called minimal access multiple plane suspension. Methods: A total of 103 thread lifts were performed between 2014 and 2017. Patient satisfaction and adverse effects were evaluated. Results: In the majority of patients (88/103, 85.4%), the results obtained were considered satisfactory 3 months after the procedure. The incidence of complications was low. Only 5.8% of the patients had slight postoperation asymmetry that was easily corrected. Minor complications experienced by patients included palpable suture knots (12.6%), persistent facial swelling (7.88%), and facial dimpling (2.9%). The causes of procedure-related complications were reviewed and discussed. Conclusion: Reinforced by select anchoring points, "minimal access multiple plane suspension" suspends ptotic anatomic tissues, serving as an effective facial rejuvenation procedure with minimal downtime and satisfactory cosmetic results.

Abstract:We recently reported the existence of several new ultrastructural components of the eumelanosome matrix from developing chick pigment epithelium. The material used in the original study was obtained from a random sample of heterogeneous chicken. We have now extended this analysis to four genetically well‐characterized chicken types: jungle fowl, blue, dominant white, and recessive wheaten. In these four genotypes there are specific ultrastructural alterations in the eumelanosome matrix. These alterations are different for each of the genotypes studied and are probably reflective of differences in the functional nature of the genes involved. The data thus provide evidence as to which genes are structural and which regulatory in action. Furthermore, since the material analyzed was all derived from pigmented pigment epithelium, it is clear that these melanosomes are affected by genes previously thought to leave them untouched. Finally, data are presented that these new ultrastructural elements are also found in a different class of animals, the C57BL/10 mouse.

Abstract:BACKGROUND: Thread-lifting has experienced a recent surge in popularity as interest in minimally invasive techniques for facial rejuvenation continues to grow. By placing barbed sutures along specific trajectories to lift the superficial musculoaponeurotic system to address facial ptosis, thread-lifting carries several advantages over the traditional facelift. Over the past few decades, there have been ongoing advances in the implementation of thread-lifting using barbed sutures.OBJECTIVE: To review the current evidence regarding the safety and efficacy of thread-lifting for facial rejuvenation.MATERIALS AND METHODS: An analysis was conducted of PubMed indexed English language literature regarding thread-lifting using barbed sutures.RESULTS: Early studies using nonabsorbable barbed sutures described high revision and complication rates. Newer techniques implementing absorbable sutures have fewer adverse events, although the long-term efficacy of these procedures remains unclear.CONCLUSION: Thread-lifts play a substantial role in minimally invasive techniques for facial rejuvenation. Additional studies are needed to better evaluate the safety and long-term efficacy of this quick, in-office procedure.

Innovative techniques for thread lifting of face and neck

Abstract:To investigate the efficacy and outcomes associated with eyebrow, midface, mandibular, and neck lifting using the innovative techniques.In this prospective review chart study, a total of 151 patients, were included in the study from January 2016 to May 2017. All participants underwent thread lifting using our innovative thread‐lift technique for head and neck. The patients were followed up first week, first, third, and sixth month after lifting surgery. Outcomes were assessed by two surgeon and patients based on the GAIS score. All data were analyzed by SPSS‐17 software with chi‐square test.A total of 151 patients (mean age: 52.49 ± 10.67, range: 27‐78 years), 16 (10.6%) men and 135 (89.4%) women, were included in the study. The most common sites for the face‐lifting surgery were the mandible (49.7%), midface (37.1%), neck (7.9%), and eyebrow (5.3%), respectively. The level of satisfaction increased from 77.5% in the first week after surgery to 89.4% in the sixth month after surgery for patients, remained unchanged, 96% for the surgeon, and increased from 74.8% to 94.7% for the independent surgeon. The highest rate of potential complications after the lifting surgery was related to ecchymosis (23.2%), followed by complications of tumefaction (6.6%), pain (6.6%), and pain (5.2%).The results showed that the level satisfaction of patients and surgeons increased over time. It seems that facial rejuvenation thread lifting with this innovative technique is effectiveness, safe, and cosmetically more acceptance with minor complications than other face‐lifting techniques.
