Abstract:PurposeTo assess prevalence and associated factors of parapapillary gamma zone enlargement (GZE).MethodsUsing fundus photographs and optical coherence tomographic images of participants of the population-based Beijing Eye Study, we examined gamma zone changes in a 10-year follow-up.ResultsThe study included 89 highly myopic eyes (61 participants; age: 65.0 ± 9.8 years) and 86 randomly selected non-highly myopic eyes. GZE prevalence was significantly higher in highly myopic eyes than non-highly myopic eyes (75/89; 84%; 95% CI: 77, 92 versus 18/86; 21%; 95% CI:12, 30; p < 0.001). None of the eyes showed a reduction in gamma zone size. Higher prevalence of segmental GZE without enlargement of Bruch’s membrane opening (BMO) (mean: 26/175; 14.9%; 95% CI: 9.5, 20.2) was associated with optic disc size reduction (OR: 43.3; 95% CI: 10.9, 172; p < 0.001), disc-fovea distance elongation (OR: 15.4; 95% CI: 3.12, 76.4; p = 0.001) and lower prevalence of high axial myopia (OR: 0.08; 95% CI: 0.01, 0.44; p = 0.001). Higher prevalence of circular GZE (mean: 38/175; 21.7%; 95% CI: 16, 28) was correlated with optic disc enlargement (OR: 4.30; 95% CI: 1.58, 11.7; p = 0.004), and higher prevalence of myopic maculopathy progression (OR: 4.04; 95% CI: 1.60, 10.2; p = 0.003), or alternatively, higher prevalence of high myopia (OR: 4.44; 95% CI: 1.76, 11.2; p = 0.002). Circular GZE or BMO enlargement was associated with lower prevalence of macular BM defect enlargement (p = 0.035). GZE occurred perpendicular to the orientation of myopic lacquer cracks in 12 out of 17 (71%; 95% CI: 46, 95) eyes with lacquer cracks. Segmental GZE occurred in 49 (89%) out of 55 eyes in the same direction as shortening of the disc diameter developed.ConclusionsThe observations support the possibility of a posterior myopic axial elongation-associated BMO shift, leading to a segmental GZE in non-highly myopic eyes, followed by a circular GZE in highly myopic eyes. Large gamma zone might be protective against macular Bruch’s membrane defects.

Prevalence and associations of parapapillary scleral ridges: the Beijing Eye Study

Abstract:Purpose: To explore the prevalence and associated factors of parapapillary scleral ridges (PSRs). Methods: Out of the cohort of the population-based Beijing Eye Study (n=3468 participants), the study included all eyes with an axial length of ≥25 mm and a randomised sample of eyes with an axial length of <25 mm. Using optical coherence tomographic (OCT) images and fundus photographs, we examined the presence and height of PSRs, defined as a ridge-like structure located on the OCT scans in the parapapillary region. Results: The study cohort consisted of 366 eyes (314 individuals; mean age: 63.7±9.7 years). PSR prevalence increased from 0% in the non-myopic group to 3.8% (95% CI 0.3%, 7.3%) in moderately myopic group and 29.2% (95% CI 15.7%, 42.5%) in the highly myopic group. All PSRs were located in the temporal parapapillary gamma zone and corresponded to an ophthalmoscopically visible demarcation line running almost parallel to the optic disc border. Higher PRS prevalence correlated with longer axial length (OR 2.98; 95% CI 1.99, 4.46; p<0.001), female sex (OR 6.48; 95% CI 1.56, 27.0; p=0.01) and older age (OR 1.09; 95% CI 1.01, 1.18; p=0.02). Axial length had the strongest influence (beta: 0.48), followed by sex (beta: 0.20) and age (beta: 0.14). If age was dropped from the multivariable model, myopic maculopathy prevalence (OR 10.0; 95% CI 1.41,70.9; p=0.02) and stage (OR 3.57; 95% CI 1.21, 10.6; p=0.02) became significantly correlated with higher PSR prevalence. Conclusions: With a PSR prevalence of >60% in eyes with an axial length of >28 mm, PSRs are a common morphological feature of high myopia, with age and female sex as additional associated factors. PSRs may be due to a biomechanical interplay between the optic nerve and the posterior ocular segment of markedly axially elongated eyes.
Microstructure of Parapapillary Atrophy: Beta Zone and Gamma Zone.

Abstract:PURPOSE:To examine the morphologic features of parapapillary atrophy by using enhanced depth imaging optical coherence tomography (EDI-OCT) and color fundus photographs.METHODS:The clinical observational comparative study included 80 normal eyes of 46 subjects and 80 eyes of 46 patients with primary open-angle glaucoma. Both groups did not vary significantly in axial length (P = 0.62) and refractive error (P = 0.30). Color fundus photographs and cross-sectional B-scan images obtained by EDI-OCT were examined. On the EDI-OCT images, we measured a gamma zone defined as the region between the temporal disc margin to the beginning of Bruch's membrane, and a beta zone defined as Bruch's membrane without retinal pigment epithelium.RESULTS:The gamma zone (mean area: 1.13 ± 2.04 mm(2)) was significantly associated with longer axial length (P < 0.001; standardized coefficient beta: 0.48), longer vertical disc diameter (P < 0.001; beta: 0.43), older age (P = 0.008; beta: 0.22), and the absence of glaucoma (P = 0.03; beta: -0.19). The beta zone (mean area: 0.85 ± 0.60 mm(2)) was associated with longer axial length (P < 0.001; beta: 0.39) and the presence of glaucoma (P < 0.001; beta: 0.48).CONCLUSIONS:In addition to associations with older age, increasing myopia, and larger disc size, the EDI-OCT-defined gamma zone of parapapillary atrophy was associated with the absence of glaucoma, whereas the EDI-OCT-defined beta zone was associated with the presence of glaucoma. Differentiation between the beta zone and the gamma zone may be clinically useful.

Abstract:BACKGROUND:To examine histomorphometrically the parapapillary region in human eyes. METHODOLOGY/PRINCIPAL FINDINGS:The histomorphometric study included 65 human globes (axial length:21-37 mm). On anterior-posterior histological sections, we measured the distance Bruch's membrane end (BME)-optic nerve margin ("Gamma zone"), BME-retinal pigment epithelium (RPE) ("Beta zone"), BME-beginning of non-occluded choriocapillaris, and BME-beginning of photoreceptor layer. "Delta zone" was defined as part of gamma zone in which blood vessels of at least 50 µm diameter were not present over a length of >300 µm. Beta zone (mean length:0.35±0.52 mm) was significantly (P = 0.01) larger in the glaucoma group than in the non-glaucomatous group. It was not significantly (P = 0.28) associated with axial length. Beta zone was significantly (P = 0.004) larger than the region with occluded choriocapillaris. Gamma zone (mean length:0.63±1.25 mm) was associated with axial length (P<0.001;r(2) = 0.73) with an increase starting at an axial length of 26.5 mm. It was not significantly (P = 0.24) associated with glaucomatous optic neuropathy. Delta zone (present only in eyes with axial length of ≥27 mm) was associated with axial length (P = 0.001) and scleral flange length (P<0.001) but not with glaucoma (P = 0.73). CONCLUSIONS/SIGNIFICANCE:Parapapillary gamma zone (peripapillary sclera without overlying choroid, Bruch's membrane and deep retinal layers) was related with axial globe elongation and was independent of glaucoma. Delta zone (no blood vessels >50 µm diameter within gamma zone) was present only in highly axially elongated globes and was not related with glaucoma. Beta zone (Bruch's membrane without RPE) was correlated with glaucoma but not with globe elongation. Since the region with occluded choriocapillaris was smaller than beta zone, complete loss of RPE may have occurred before complete choriocapillaris closure.
Comparative analysis of OCT-defined parapapillary beta and gamma zones between primary open angle glaucoma and primary angle closure glaucoma

Abstract:The ophthalmoscopic beta zone of parapapillary atrophy has recently been proposed to divide into a gamma zone and a (new) beta zone based on OCT imaging. The present study was undertaken to compare the microstructural characteristics of parapapillary gamma and beta zones and their influencing factors between primary open angle glaucoma (POAG) and primary angle closure glaucoma (PACG). Seventy-three PACG patients that had no evidence of an acute attack and 78 POAG patients were enrolled. Patients were matched by propensity scores for age and visual field mean defect (MD) value. The area and angular extent of both zones were measured. In multivariate analysis, a larger beta zone was correlated with older age, severe MD value and longer axial length. A larger gamma zone was correlated with longer axial length. Older age and severe MD value were correlated with the concentric shape of beta zone. Comparing the PACG and POAG groups that adjusted for age and MD value, gamma zone was larger and more prevalent in the POAG group, while beta zone showed no significant difference. Taken separately, MD value was associated with the area and shape of beta zone in the PACG group. Axial length was associated with the temporal shape of beta zone in the POAG group. These data indicated that OCT-defined parapapillary beta and gamma zones exhibited different characteristics in two types of glaucoma. Clinically, the size of parapapillary beta zone may serve as a better indicator of glaucoma severity in eyes with PACG than that in POAG.

Posterior Eye Shape in Myopia

Abstract:Purpose: To explore prevalence and associated factors of abnormalities of the posterior eye shape in dependence of axial length. Design: Population-based study. Participants: Of the participants (n = 3468) of the Beijing Eye Study, we included all eyes with an axial length of ≥25 mm, and a randomized sample of eyes with an axial length of <25 mm. Methods: Using 30°-wide, serial horizontal, and fovea-centered radial, OCT images, we examined location and depth of the most posterior point of the retinal pigment epithelium/Bruch's membrane line (PP-RPE/BML). Main outcome measures: Prevalence and depth of an extrafoveal PP-RPE/BML. Results: The study included 366 eyes (314 individuals). On the radial OCT scans, the PP-RPE/BML was located in the foveola in 190 (51.9%) eyes, in 121 (33.1%) eyes in the 6 o'clock part of the vertical meridian (distance to foveola: 1.73 ± 0.70 mm), and in 54 (14.8%) eyes in the 12 o'clock part of the vertical meridian (fovea distance: 2.01 ± 0.66 mm). On the horizontal OCT scans, the PP-RPE/BML was located in the foveola in 304 (83.1%) eyes, between foveola and optic disc in 36 (9.8%) eyes (fovea distance: 1.59 ± 0.76 mm), and temporal to the foveola in 26 (7.1%) eyes (fovea distance: 1.20 ± 0.60 mm). Higher prevalence of an extrafoveal PP-RPE/BML correlated with longer axial length (odds ratio [OR]: 1.55; 95% confidence interval [CI]: 1.28, 1.89), higher corneal astigmatism (OR: 1.78; 95% CI: 1.14, 2.79), and female sex (OR: 2.74; 95% CI: 1.30, 5.77). The curvature of the RPE/BML at the posterior pole was similar to the RPE/BML curvature outside of the posterior pole in 309 (84.4%) eyes, and it was steeper (i.e., smaller curvature radius) in 57 (15.6%) eyes. In these eyes, axial length was longer (24.41 ± 1.78 mm versus 27.74 ± 1.88 mm; P < 0.001). Conclusions: With longer axial length, the foveola is more often located outside of the geometrical posterior pole. It may be of importance for biometric axial length measurements. An extrafoveal location of the PP-RPE/BML may be due to an axial elongation-associated, meridionally asymmetric enlargement of Bruch's membrane in the fundus midperiphery. Retinal nerve fibre layer thickness in association with gamma zone width and disc-fovea distance

Abstract:Purpose: Previous studies have shown that the retinal nerve fibre layer (RNFL) thickness as surrogate of the optic nerve decreases with longer axial length. We explored which explanatory parameters might explain that association. Methods: Participants of the population-based Beijing Eye Study 2011 without any retinal or optic nerve disease were selected based on a refractive error-based stratified randomization. Results: The study included of 632 participants (age: 59.1 ± 7.3 years; axial length: 23.5 ± 1.2 mm; range: 20.88-28.68 mm). Thicker RNFL (mean: 101.0 ± 10.0 μm) was associated (multivariable analysis) with smaller parapapillary gamma zone (defined as Bruch's membrane-free parapapillary region) (standardized regression coefficient beta: -0.13; non-stadardized regression coefficient B: -0.008; 95% confidence interval (CI): -0.014, -0.003; p = 0.005) and shorter disc-fovea distance (beta: -0.15; B: -3.91; 95% CI: -6.60; -1.22; p = 0.004), after adjusting for age (beta: -0.22; B: -030; 95% CI: -0.41, -0.19; p < 0.001), gender (beta: 0.12; B: 2.37; 95% CI 0.77, 3.97; p = 0.004) and larger optic disc area (beta: 0.12; B: 0.12; 95% CI: 2.14; 95% CI: 0.54, 3.73; p = 0.009). RNFL thickness decreased by 8 μm (95% CI: 3, 14) for each mm increase in gamma zone width, and by 3.91 μm (95% CI: 1.22, 6.60) for each mm elongation of the disc-fovea distance. If disc-fovea distance and gamma zone width were replaced by axial length, the latter was correlated with RNFL thickness (beta: -0.37; B: -3.12; 95% CI: -3.88, -2.35; p < 0.001). Longer disc-fovea distance was directly associated with larger gamma zone (beta: 0.50; B: p < 0.001). Conclusions: The RNFL thickness decrease with longer axial length is associated with a longer optic disc-fovea distance and larger parapapillary gamma zone. Longer disc-fovea distance and larger gamma zone lead to an elongation and stretching of the retinal nerve fibres, potentially causing a thinning or loss of the nerve fibres. It may explain the occurrence of visual field defects in some non-glaucomatous highly myopic eyes without macular correlates of the perimetric defects.

Abstract:PURPOSE: To measure the optic disc rotation around the vertical and horizontal disc axis and to evaluate associations with general and ocular parameters.DESIGN: Population-based study.METHODS: In the Beijing Eye Study, 3468 participants (mean age:64.6±9.8 years; range:50-93 years) underwent an ophthalmological examination which included spectral-domain optical coherence tomography (OCT) with enhanced depth imaging. Using the OCT images, we determined the amount of the rotation of the optic disc (defined as Bruch´s membrane opening (BMO)) around the vertical axis and horizontal axis.RESULTS: Optic disc rotation measurements were available for 3037 (87.6%) individuals. In multivariate analysis, larger optic disc rotation around the vertical axis (range:-4.90° to 41.0°) was associated (regression coefficient r:0.27) with high axial myopia (axial length ≥26.5 mm) (P<0.001;standardized regression coefficient beta beta:0.09), longer disc-fovea distance (P = 0.001;beta:0.09) and wider parapapillary beta/gamma zone (P<0.001;beta0.12). Larger optic disc rotation around the horizontal axis (range:-7.10° to 26.4°) was associated (r:0.32) with high axial myopia (P = 0.001;beta:0.08), larger optic disc-fovea angle (P<0.001;beta:0.13), thinner superior nasal retinal nerve fiber layer (RNFL) thickness (P<0.001;beta:-0.19) and thicker inferior nasal RNFL thickness (P<0.001;beta:0.17).CONCLUSIONS: Vertical optic disc rotation was associated with highly myopic axial elongation, increased disc-fovea distance and development or enlargement of parapapillary, Bruch´s membrane free, gamma zone, while macular Bruch´s membrane length is not affected. Horizontal optic disc rotation was associated with inferior dislocation of the fovea, in addition to a thinner superior nasal RNFL and thicker inferior nasal RNFL. The latter association may be taken into account in the interpretation of RNFL thickness profiles.

Clinical features of microvasculature in subzones of parapapillary atrophy in myopic eyes: an OCT-angiography study

Abstract:Purpose To investigate the microvasculature in subzones of parapapillary atrophy (PPA) and their associated factors in healthy myopic eyes using optical coherence tomography (OCT) angiography. Methods The cross-sectional study included 55 healthy myopic eyes with PPA. The superficial radial peripapillary capillary (RPC) and choroidal microvascular densities were delineated and measured in alpha, beta, and gamma zone PPA using OCT angiography, respectively. The structural parameters including the width and area in each subzone and corresponding retinal nerve fiber layer thickness were further measured. Statistical analysis was performed to explore the relationship between the vascular and structural parameters. Results The mean age and mean axial length of the participants were 27.55 ± 5.72 years and 25.19 ± 1.08 mm. Among alpha, beta, and gamma zone PPA, the beta zone showed the lowest RPC density, which was negatively correlated with the area and width of beta zone ( P < 0.05). The gamma zone showed the highest RPC density, which was positively correlated with the retinal nerve fiber layer thickness of gamma zone ( P < 0.01). Compared with alpha zone, both gamma and beta zones showed marked decrease of choriocapillaris. The beta zone showed a lower choroidal microvascular density than that of gamma zone ( P < 0.01). Choroidal microvascular density in beta and gamma zones were negatively correlated with the width of beta zone and gamma zone, respectively ( P < 0.01). Conclusions Topographic differences on superficial RPC and choroidal microvasculature were found among the subzones of PPA. A microcirculatory deficiency in beta zone PPA may exist in myopic eyes.
Association of macular thickness with parapapillary atrophy in myopic eyes

Abstract:Background: To investigate whether macular structure could be affected by axial elongation and to determine the association between macular intraretinal thickness and the microstructure of β-zone parapapillary atrophy (PPA) in myopic eyes. Methods: The study recruited 113 healthy myopic subjects (113 eyes). Images of the macula, subfoveal choroid, and optic nerve head were acquired using spectral-domain optical coherence tomography (SD-OCT). An automatic segmentation algorithm was used to segment the macular images into 7 intraretinal layers. PPA widths with and without Bruch's membrane (PPA+BM and PPA-BM, respectively) were evaluated. Linear regression analysis was performed to evaluate the association between macular intraretinal thickness and axial length and the microstructure of PPA. Results: An increase in axial length was associated with a decrease in whole macular thickness of the peripheral region and an increase in whole macular thickness of the central region. Thickness alterations of the macular intraretinal layers were most apparent in the peripheral region. A significant correlation was found between PPA-BM width and macular intraretinal layer thickness, whereas no significant correlation was found between PPA+BM width and macular intraretinal layer thickness. Moreover, both PPA+BM and PPA-BM widths significantly correlated with subfoveal choroidal thickness. Conclusions: Macular intraretinal layer thickness may be affected by PPA-BM width. These findings indicate that the microstructure of PPA should be considered when evaluating the macula in patient with myopia and glaucoma.
Choriocapillaris, photoreceptor and inner retinal layers in spatial relationship to parapapillary alpha, beta, gamma and delta zone

Abstract:Aims/Purpose: To examine the spatial relationships between the choriocapillaris, retinal layers and retinal pigment epithelium (RPE) in the parapapillary region. Methods: Human eyes enucleated due to uveal melanomas or secondary angle‐closure glaucoma were histomorphometrically examined. Results: The investigation consisted of 100 globes. In non‐highly myopic non‐glaucomatous eyes, the photoreceptor layer, choriocapillaris and retinal inner nuclear layer (INL) ended approximately at the end of the RPE layer, with no significant (all p ≥ 0.10) difference between the layers. In the non‐highly myopic, glaucomatous eyes and the highly myopic eyes, the photoreceptor layer, choriocapillaris and inner nuclear layer ended in the majority of eyes beyond the RPE‐end in direction to the optic disc border, with the INL extending beyond the RPE‐end significantly more than the photoreceptor layer (non‐highly myopic non‐glaucomatous group: 133 ± 183 μm versus 66.5 ± 96.8 μm; p = 0.001; highly myopic glaucomatous eyes: (266 ± 395 μm vs. 211 ± 286 μm; p = 0.03). A larger extension of the photoreceptor layer, choriocapillaris and INL beyond the RPE‐end correlated with longer axial length (beta: 0.50; B: 32.2; 95% CI: 21.6, 42.8) and wider beta zone (beta: 0.28; B: 0.10; 95% CI: 0.04, 0.16), with longer axial length (beta: 0.24; B: 23.0; 95% CI: 1.6, 44.5) and longer beta zone (beta: 0.59; B: 0.32; 95% CI: 0.22, 0.41; p < 0.001), and with longer axial length (beta: 0.34; B: 43.7; 95% CI: 11.6, 75.7), longer gamma zone (beta: 0.52; B: 0.28; 5% CI: 0.15, 0.41) and diagnosis of non‐highly myopic glaucoma (beta: 0.28; B: 267; 95% CI: 80.8, 454), respectively. Conclusions: Non‐glaucomatous non‐highly myopic eyes differ from highly myopic eyes and glaucomatous eyes in the spatial relationship of the parapapillary tissue layers. The findings support the notion of an axial elongation‐associated shifting of the parapapillary RPE and Bruch's membrane for development of myopic beta zone, and an RPE‐cell loss for development of glaucomatous beta zone.

Gene mutations in chronic kidney disease patients with secondary hyperparathyroidism and Sagliker syndrome.

Abstract:Sagliker syndrome (SS) develops particularly before puberty while chronic kidney disease (CKD) reaches stage 3 with overt secondary hyperparathyroidism. We conducted screening for mutations in all the 13 exons of GNAS1 gene, all 3 exons of FGF23, and all 18 exons in FGFR3 genes in 23 patients. In 73.9% (17 of 23) patients, 17 genetic abnormalities in GNAS1 were detected. Seven (58.3%) of 12 nucleotide alterations comprised novel missense mutations and 3 nonsense. Mismutations were in different manner. There were also 6 heterozygous transversion polymorphisms in exons. Six were introngenic mutations (introns 65626, 70387, 70817). We found 10 mutations with different manner in FGF23 gene. Two were defined previously but remaining 8 were novel mutations. Three were in intronic region near exon 2. We sequenced all exons and intronic regions near exon-exon junction regions of FGFR3 gene. We found 22 mutations with different manner. Six were defined previously and remaining 16 were novel mutations. Eight of them were in intronic region near exon 11 and the last 2 were in noncoding exonic region of exons. One was in the exon-exon junction region between exon 11 and 12, therefore this mutation might be preventing splicing of this intron. Because the incidence of CKD late stage 3 is around 8% but the incidence of SS is around 0.5% in CKD, these gene mismutations might be responsible for bone deformities such as McCune-Albright syndrome and achondroplasias. Although our patients were not resembling any of them, they could be in between, and SS might be a combination-compulsion of bone dysplasias-hereditary osteodystrophies and CKD.
Impact of peripapillary staphylomas on the vascular and structural characteristics in myopic eyes: a propensity score matching analysis

Abstract:Purpose: To apply propensity score matching to evaluate the impact of peripapillary staphylomas (PPS) on vascular and structural characteristics in the myopic eyes. Methods: This was a prospective, cross-sectional study. Forty-one control eyes and 41 eyes with PPS were analyzed. The eyes were selected using propensity score matching analysis based on the age and axial length. All subjects underwent ophthalmologic examinations for assessing vessel and structure parameters using swept-source optical coherence tomography (SS-OCT), OCT angiography, color fundus photography, and ocular biometry. Results: As compared with control eyes, the eyes with PPS had shallower anterior chamber depth (3.61 ± 0.24 mm vs 3.77 ± 0.24 mm, P = 0.004), higher intraocular pressure (IOP) (16.59 ± 2.88 mmHg vs 14.53 ± 2.45 mmHg, P = 0.002), and higher myopic spherical equivalent (- 11.52 ± 3.22D vs - 9.88 ± 2.20D, P = 0.009). while corneal curvature and lens thickness between the two groups were not statistically different. Compared with control eyes, increased macular deep vessel density, reduced macular choriocapillaris and radial peripapillary capillary, and thinning retinal layer, ganglion cell complex, choroidal layer as well as the superior and inferior peripapillary retinal nerve fiber layer were observed in eyes with PPS, apart from larger disc area, parapapillary atrophy area, and degree of disc rotation. Logistic regression analysis revealed that the IOP (P = 0.046), disc rotation (P = 0.003), and average peripapillary choroidal thickness (P = 0.009) were associated with the presence of PPS. Conclusion: Close association of PPS with exacerbation of myopia and anatomical alterations was observed which not only affected the eye posterior segment but also the anterior segments. We further identified significant reductions in the radial peripapillary capillary and macular choroidal perfusion with the increase in macular deep retinal flow blood of myopic eyes with PPS. Higher IOP, thinner peripapillary choroidal thickness, and rotated optic disc were risk factors for the presence of PPS.
The protective role of gamma zone peripapillary atrophy in diabetic retinopathy: insights from deep learning and SS-OCT angiography

Abstract:PurposeTo explore the relationship between peripapillary atrophy (PPA) and diabetic retinopathy (DR), and to uncover potential mechanisms using swept-source optical coherence tomography (SS-OCT) angiography.MethodsThis cross-sectional study included 845 patients with type 2 diabetes (T2DM), who underwent detailed systemic and ophthalmic evaluations. A state-of-the-art deep learning method was employed to precisely identify the parapapillary beta and gamma zones. Based on PPA characteristics, eyes were categorized into four groups: without beta or gamma zone (Group A), isolated beta zone (Group B), isolated gamma zone (Group C), and with both beta and gamma zone (Group D). Digital fundus photography was utilized to diagnose and stage DR severity, while SS-OCT angiography quantified retinal and choroidal vasculature.ResultsParticipants had a mean age of 66 ± 8.8 years, with 437 (51.7%) male. Beta and gamma PPA zones were observed in 574 (67.9%) and 256 (30.3%) eyes, respectively. Beta zone PPA was associated with older age, whereas gamma zone PPA was correlated with longer axial length (AL), lower vessel density, and reduced choroidal thickness. Adjusted analyses revealed that eyes with isolated (Group C) or concurrent (Group D) gamma zone PPA conferred significantly lower DR grade, independent of known risk factors including systemic diabetes risk factors and AL.ConclusionThis study finds that gamma zone PPA is linked to a reduced risk of developing DR. These results imply that the gamma zone may reflect progressive myopia-associated thinning and microvascular depletion in posterior ocular tissues, potentially contributing to structural resilience against DR. This novel insight offers a promising avenue for understanding the interplay between PPA and DR.

Abstract:OBJECTIVE:To evaluate the variety of peripapillary atrophy (PPA) and its relationship with visual field among primary open angle glaucoma (POAG), normal tension glaucoma (NTG), and normal subjects with glaucoma-like disk (GLD) that had no evidence of visual field defect and retinal nerve fiber layer defect. METHODS:The study included 42 eyes of 42 patients with POAG, 40 eyes of 40 patients with NTG, and 45 eyes of normal subjects with GLD. Stereo color optic disc photographs were morphometrically evaluated by a video camera and computer system. The refractive range ( spherical equivalent) of all these subjects was between + 3.00D and -3.00D. RESULTS:The alpha-zone and beta-zone of PPA occurred significantly (P <0. 05) more frequent in both patients with POAG( 100. 0% ,48. 9% )and with NTG(95.0% ,37. 5% )than that of normal subjects with GLD(85.4% ,19. 1% ) , but no difference was found between the POAG and NTG groups (P > 0. 05). The beta-zone area of PPA was significantly ( P < 0. 05) enlarged in POAG compared with GLD normal subjects. While there is no significant difference in alpha-and beta-zone area between all other groups. In glaucoma patients, The alpha-zone or the beta-zone area of PPA weakly correlated with the glaucomatous visual field defect While cup/disk area ratio showed a relatively strong correlation. Cup/disc area ratio and age were the only two risk factors for the occurrence of glaucomatous visual field defect among all the factors including PPA by logistic regression analysis. CONCLUSIONS:Our results demonstrate that cup/disc area ratio is well correlated with glaucomatous visual field defect. The occurrence of alpha-zone, beta-zone, and beta-zone areas of PPA may not be a clinically useful parameter, for diagnosing or differentiating POAG and NTG .

Abstract:BackgroundMyopia subjects usually have long axial lengths and are predisposed to deep anterior chamber depths and open angles. Jiangning eye study is a randomized, population based study in Shanghai to determine whether myopia has an effect on the prevalence of occludable angles.MethodsA total of 2478 residents aged 50 years and over were randomly selected, and 2044 (82.5%) individuals of them participated in ophthalmic examinations between November 2012 and February 2013. Eye examinations included autorefraction, noncontact tonometry, measurement of axial length, anterior chamber depth, lens thickness and spectral domain optical coherence tomography.ResultsThe crude prevalences of iris trabecular meshwork contact in Jiangning Chinese individuals with myopia, emmetropia, and hyperopia are 1.25%, 6.44%, and 7.43%, respectively. There is also iris trabecular meshwork contact and shallow anterior chamber depth in myopia subjects even with long axial lengths.ConclusionsThe prevalence of iris trabecular meshwork contact in myopia subjects is lower than their emmetropia or hyperopia counterparts. Axial length is an important factor associate with occludable angle.
Influence of choroidal microvasculature dropout on progressive retinal nerve fibre layer thinning in primary open-angle glaucoma: comparison of parapapillary β-zones and γ-zones

Abstract:Background/aims To compare the influence of choroidal microvasculature dropout (cMvD) on progressive retinal nerve fibre layer (RNFL) thinning in glaucomatous eyes with parapapillary β-zones and γ-zones. Methods 294 eyes with primary open-angle glaucoma (POAG) and parapapillary atrophy (PPA) underwent optical coherence tomography (OCT) to determine the type of PPA and OCT angiography scanning of the optic nerve head to determine the presence of cMvD. Eyes were classified based on the type of PPA (β-zones and γ-zones), and their clinical characteristics were compared. Factors associated with the rate of rapid progressive RNFL thinning were determined in each group, including the presence of cMvD as an independent variable. Results Of the 294 eyes, 186 and 108 were classified as having β-zones and γ-zones, respectively. The rate of RNFL thinning was slower (p<0.001), axial length was longer (p<0.001) and presence of cMvD was less frequent (57.4% vs 73.1%, p=0.006) in eyes with γ-zone than those with β-zone. Multivariate analyses showed that greater lamina cribrosa curvature (p=0.047) and the presence of cMvD (p=0.010) were associated with a faster rate of RNFL thinning in eyes with β-zone, whereas larger intraocular pressure fluctuation (p<0.001), shorter axial length (p=0.042) and greater baseline RNFL thickness (p<0.001) were associated with a faster rate of RNFL thinning in eyes with γ-zone. Conclusions The presence of cMvD was significantly associated with a faster rate of RNFL thinning in POAG eyes with β-zone, but not γ-zone. The pathogenic consequences of cMvD in POAG eyes may depend on accompanying peripapillary structures.

Abstract:The aim of the study was to assess longitudinal changes in the spatial relationship of the choroidal vasculature to retinal vasculature in myopic eyes. In the population-based longitudinal Beijing Eye Study in 2001/2011, we examined all highly myopic eyes with assessable fundus photographs and a randomized group of non-highly myopic. Using fundus photographs, we qualitatively assessed changes in the location of major choroidal vessels in relationship to retinal vessels. The study consisted of 85 highly myopic eyes (58 participants;age:64.8 ± 9.4 years) and 85 randomly selected non-highly myopic eyes. A choroidal shift in relationship to the retinal vessels was detected more often in the highly myopic group than the non-highly myopic group (47/85 (55%) vs 6/85 (7%); P < 0.001). In the highly myopic group, the choroidal vessel shift occurring on the disc-fovea line in 39 (44%) eyes, was similar to, or smaller than, the enlargement in gamma zone width in 26 (67%) eyes and in 11 (28%) eyes respectively. The choroidal vessel shift was larger (P = 0.002) in eyes without choroidal vessels in gamma zone than in eyes with large choroidal vessels in gamma zone. In 14 (17%) eyes, a localized centrifugal choroidal shift was observed in association with an increase in the stage of myopic maculopathy. The results suggest that highly myopic eyes show a change in the position of large choroidal vessels in relationship to retinal vessels, in association with development or enlargement of gamma zone and an increase in the stage of myopic maculopathy.
Correlation between choroidal parameters and primary angle-closure suspect in different age groups

Abstract:Objective: To investigate the correlations between choroidal parameters and primary angle-closure suspect (PACS) in different age subgroups. Methods and analysis: Participants aged 50 years or older in a rural area of Daxing District, Beijing, were recruited. Swept-source optical coherence tomography was used to measure the choroidal parameters. Demographic, ocular biometry parameters and choroidal parameters were compared between the PACS and non-PACS (NPACS) eyes. Logistic analysis was performed to explore the association between the choroidal parameters and PACS. Results: 192 (26.89%) subjects with PACS and 509 (71.29%) with PACS were analysed. Subjects were divided into two groups: group 1 (50-60 years, n=286) and group 2 (>60 years, n=415). In group 1, the mean subfoveal choroidal thickness of PACS eyes was 341.82±88.23 µm and thicker than NPACS eyes (315.07±83.53 µm, p=0.035). The choroidal volume was greater in PACS eyes (10.61±2.78 mm3) compared with NPACS eyes (9.66±2.49 mm3, p=0.013). In group 2, no significant difference in any choroidal parameters between PACS and NPACS was found. Multivariate regression demonstrated that increased choroidal volume was associated with PACS (OR 1.298, 95% CI 1.117 to 1.510, p<0.001) in group 1. Conclusions: In the age group of 50-60 years, PACS eyes had greater choroidal thickness and volume than NPACS eyes, and the increased total choroidal volume was a predisposing factor for PACS.
