Abstract:Contemporary biomechanical modeling of traumatic brain injury (TBI) focuses on either the global brain as an organ or a representative tiny section of a single axon. In addition, while it is common for a global brain model to employ real-world impacts as input, axonal injury models have largely been limited to inputs of either tension or compression with assumed peak strain and strain rate. These major gaps between global and microscale modeling preclude a systematic and mechanistic investigation of how tissue strain from impact leads to downstream axonal damage throughout the white matter. In this study, a unique subject-specific multimodality dataset from a male ice-hockey player sustaining a diagnosed concussion is used to establish an efficient and scalable computational pipeline. It is then employed to derive voxelized brain deformation, maximum principal strains and white matter fiber strains, and finally, to produce diverse fiber strain profiles of various shapes in temporal history necessary for the development and application of a deep learning axonal injury model in the future. The pipeline employs a structured, voxelized representation of brain deformation with adjustable spatial resolution independent of model mesh resolution. The method can be easily extended to other head impacts or individuals. The framework established in this work is critical for enabling large-scale (i.e., across the entire white matter region, head impacts, and individuals) and multiscale (i.e., from organ to cell length scales) modeling for the investigation of traumatic axonal injury (TAI) triggering mechanisms. Ultimately, these efforts could enhance the assessment of concussion risks and design of protective headgear. Therefore, this work contributes to improved strategies for concussion detection, mitigation, and prevention.

Computational modelling of traumatic brain injury predicts the location of chronic traumatic encephalopathy pathology

Abstract:Traumatic brain injury can lead to the neurodegenerative disease chronic traumatic encephalopathy. This condition has a clear neuropathological definition but the relationship between the initial head impact and the pattern of progressive brain pathology is poorly understood. We test the hypothesis that mechanical strain and strain rate are greatest in sulci, where neuropathology is prominently seen in chronic traumatic encephalopathy, and whether human neuroimaging observations converge with computational predictions. Three distinct types of injury were simulated. Chronic traumatic encephalopathy can occur after sporting injuries, so we studied a helmet-to-helmet impact in an American football game. In addition, we investigated an occipital head impact due to a fall from ground level and a helmeted head impact in a road traffic accident involving a motorcycle and a car. A high fidelity 3D computational model of brain injury biomechanics was developed and the contours of strain and strain rate at the grey matter-white matter boundary were mapped. Diffusion tensor imaging abnormalities in a cohort of 97 traumatic brain injury patients were also mapped at the grey matter-white matter boundary. Fifty-one healthy subjects served as controls. The computational models predicted large strain most prominent at the depths of sulci. The volume fraction of sulcal regions exceeding brain injury thresholds were significantly larger than that of gyral regions. Strain and strain rates were highest for the road traffic accident and sporting injury. Strain was greater in the sulci for all injury types, but strain rate was greater only in the road traffic and sporting injuries. Diffusion tensor imaging showed converging imaging abnormalities within sulcal regions with a significant decrease in fractional anisotropy in the patient group compared to controls within the sulci. Our results show that brain tissue deformation induced by head impact loading is greatest in sulcal locations, where pathology in cases of chronic traumatic encephalopathy is observed. In addition, the nature of initial head loading can have a significant influence on the magnitude and pattern of injury. Clarifying this relationship is key to understanding the long-term effects of head impacts and improving protective strategies, such as helmet design.

Abstract:This paper reviews the traditional areas of impact biomechanics as they relate to brain injury caused by blunt impact. These areas are injury mechanisms, human response to impact, human tolerance to impact and the use of human surrogates. With the advent of high-speed computers, it is now possible to add computer models to the list of human surrogates that used to be limited to animals and human cadavers. The advantages and shortcomings of current computer models are discussed. One of the computer models was used to predict the pressures and shear stresses developed in the brain and the extent of stretch of the bridging veins in the brains of American football players who sustained severe helmet-to-helmet head impact during the game. It was found that increases in intracranial pressure were more dependent on translational acceleration while the primary determinant for the development of shear stresses in the brain is rotational acceleration. Although the current head injury criterion is based almost entirely on translational acceleration, it is recommended that any new criterion should reflect the contribution of both translational and rotational acceleration.

Abstract:This article presents the integration of brain injury biomechanics and graph theoretical analysis of neuronal connections, or connectomics, to form a neurocomputational model that captures spatiotemporal characteristics of trauma. We relate localized mechanical brain damage predicted from biofidelic finite element simulations of the human head subjected to impact with degradation in the structural connectome for a single individual. The finite element model incorporates various length scales into the full head simulations by including anisotropic constitutive laws informed by diffusion tensor imaging. Coupling between the finite element analysis and network-based tools is established through experimentally-based cellular injury thresholds for white matter regions. Once edges are degraded, graph theoretical measures are computed on the "damaged" network. For a frontal impact, the simulations predict that the temporal and occipital regions undergo the most axonal strain and strain rate at short times (less than 24 hrs), which leads to cellular death initiation, which results in damage that shows dependence on angle of impact and underlying microstructure of brain tissue. The monotonic cellular death relationships predict a spatiotemporal change of structural damage. Interestingly, at 96 hrs post-impact, computations predict no network nodes were completely disconnected from the network, despite significant damage to network edges. At early times (t < 24 hrs) network measures of global and local efficiency were degraded little; however, as time increased to 96 hrs the network properties were significantly reduced. In the future, this computational framework could help inform functional networks from physics-based structural brain biomechanics to obtain not only a biomechanics-based understanding of injury, but also neurophysiological insight.

Abstract:Blast traumatic brain injury is ubiquitous in modern military conflict with significant morbidity and mortality. Yet the mechanism by which blast overpressure waves cause specific intracranial injury in humans remains unclear. Reviewing of both the clinical experience of neurointensivists and neurosurgeons who treated service members exposed to blast have revealed a pattern of injury to cerebral blood vessels, manifested as subarachnoid hemorrhage, pseudoaneurysm, and early diffuse cerebral edema. Additionally, a seminal neuropathologic case series of victims of blast traumatic brain injury (TBI) showed unique astroglial scarring patterns at the following tissue interfaces: subpial glial plate, perivascular, periventricular, and cerebral gray-white interface. The uniting feature of both the clinical and neuropathologic findings in blast TBI is the co-location of injury to material interfaces, be it solid-fluid or solid-solid interface. This motivates the hypothesis that blast TBI is an injury at the intracranial mechanical interfaces. In order to investigate the intracranial interface dynamics, we performed a novel set of computational simulations using a model human head simplified but containing models of gyri, sulci, cerebrospinal fluid (CSF), ventricles, and vasculature with high spatial resolution of the mechanical interfaces. Simulations were performed within a hybrid Eulerian-Lagrangian simulation suite (CTH coupled via Zapotec to Sierra Mechanics). Because of the large computational meshes, simulations required high performance computing resources. Twenty simulations were performed across multiple exposure scenarios-overpressures of 150, 250, and 500 kPa with 1 ms overpressure durations-for multiple blast exposures (front blast, side blast, and wall blast) across large variations in material model parameters (brain shear properties, skull elastic moduli). All simulations predict fluid cavitation within CSF (where intracerebral vasculature reside) with cavitation occurring deep and diffusely into cerebral sulci. These cavitation events are adjacent to high interface strain rates at the subpial glial plate. Larger overpressure simulations (250 and 500kPa) demonstrated intraventricular cavitation-also associated with adjacent high periventricular strain rates. Additionally, models of embedded intraparenchymal vascular structures-with diameters as small as 0.6 mm-predicted intravascular cavitation with adjacent high perivascular strain rates. The co-location of local maxima of strain rates near several of the regions that appear to be preferentially damaged in blast TBI (vascular structures, subpial glial plate, perivascular regions, and periventricular regions) suggest that intracranial interface dynamics may be important in understanding how blast overpressures leads to intracranial injury.
Strain concentration drives the anatomical distribution of injury in acute and chronic traumatic brain injury

Abstract:Brain tissue injury caused by mild traumatic brain injury (mTBI) disproportionately concentrates in the midbrain, cerebellum, mesial temporal lobe, and the interface between cortex and white matter at sulcal depths. The bio-mechanical principles that explain why physical impacts to different parts of the skull translate to common foci of injury concentrated in specific brain structures are unknown. A general and longstanding idea, which has not to date been directly tested in humans, is that different brain regions are differentially susceptible to strain loading. We use Magnetic Resonance Elastography (MRE) in healthy participants to develop whole-brain bio-mechanical vulnerability maps that independently define which regions of the brain exhibit disproportionate strain concentration. We then validate those vulnerability maps in a prospective cohort of mTBI patients, using diffusion MRI data collected at three cross-sectional timepoints after injury: acute, sub-acute, chronic. We show that regions that exhibit high strain, measured with MRE, are also the sites of greatest injury, as measured with diffusion MR in mTBI patients. This was the case in acute, subacute, and chronic subgroups of the mTBI cohort. Follow-on analyses decomposed the biomechanical cause of increased strain by showing it is caused jointly by disproportionately higher levels of energy arriving to high-strain structures, as well as the inability of high-strain structures to effectively disperse that energy. These findings establish a causal mechanism that explains the anatomy of injury in mTBI based on in vivo rheological properties of the human brain.

Abstract:Blast-induced traumatic brain injury (bTBI) is a prevalent consequence of modern warfare and explosion hazards. In recent years, mild primary brain injury caused by blast waves has become the predominant form of injury, garnering significant attention from researchers. Due to ethical and technical limitations, human testing is challenging to conduct; therefore, numerical simulation has emerged as one of the most critical methods for studying bTBI. By combining reasonable physical modeling with reliable modes, we can quantitatively predict the biomechanical response of the human head and brain to blast waves. This approach reveals the mechanical mechanisms underlying brain injury, which is essential for understanding bTBI's biomechanical characteristics and designing protective equipment for individuals. The aim of this review is to furnish a comprehensive overview of the current research on numerical simulation of primary bTBI, encompassing advancements in computational modeling, mechanical mechanisms and protective measures. Focusing on the multi-scale nature of the human brain and biomechanical modeling of bTBI, this article introduces linear elastic, hyper-elastic, and viscoelastic constitutive models for brain tissue; development and evolution of finite element models for the human head in terms of brain structure and mesh size; as well as macroscopic, mesoscopic, and multi-scale modeling methods along with numerical simulation techniques for bTBI. Aiming at the direct effects of wave propagation, cerebral vasculature influence, and the continuous process of bodily response, the mechanical mechanism obtained through numerical simulation is analyzed and discussed. The advancements in numerical simulation of protective strategies for bTBI, including the significance of enhancing head closure and the implementation of novel structures and materials, are expounded upon. Ultimately, a summary is provided regarding current research and application of numerical simulation for bTBI, along with an assessment of future development and improvement.

Abstract:In this study, using computational biomechanics models, we investigated influence of the skull-brain interface modeling approach and the material property of cerebrum on the kinetic, kinematic and injury outputs. Live animal head impact tests of different severities were reconstructed in finite element simulations and DAI and ASDH injury results were compared. We used the head/brain models of Total HUman Model for Safety (THUMS) and Global Human Body Models Consortium (GHBMC), which had been validated under several loading conditions. Four modeling approaches of the skull-brain interface in the head/brain models were evaluated. They were the original models from THUMS and GHBMC, the THUMS model with skull-brain interface changed to sliding contact, and the THUMS model with increased shear modulus of cerebrum, respectively. The results have shown that the definition of skull-brain interface would significantly influence the magnitude and distribution of the load transmitted to the brain. With sliding brain-skull interface, the brain had lower maximum principal stress compared to that with strong connected interface, while the maximum principal strain slightly increased. In addition, greater shear modulus resulted in slightly higher the maximum principal stress and significantly lower the maximum principal strain. This study has revealed that using models with different modeling approaches, the same value of injury metric may correspond to different injury severity.
The dynamic triple peak impact factor in traumatic brain injury influences native protein structures in gray and white matter as measured with computational simulation

Abstract:BACKGROUND: Traumatic brain injuries (TBIs) cause a substantial burden to the patient, relatives, and the society as a whole. Much experience and knowledge during the last two decades have improved the neurosurgical treatment as well as the outcome. However, there is still much debate on what actually happens when external kinetic energy is transferred to the head immediately after a TBI. Better knowledge about the cascades of mechanical events at the time of accident is a prerequisite to further reduce the burden in all categories and improve the neurosurgical care of TBI patients.METHODS: In the present study, we use the finite element modeling of the human brain to numerically simulate impact velocities of 10, 6, and 2 m/s to clarify some of the immediate consequences of the external kinetic energy transfer focusing on the gray (GM) and white matters (WM).RESULTS: The numerical simulation was focused on the external kinetic energy transfer with a level of 227·3 J reaching the head, intracranial pressure (ICP), strain energy density, 1st principal strain level, and their respective impacts on the brain tissue. The results show that, for a 10 m/s impact, a total internal potential energy of 208·6 J was absorbed, of which 14·3% (29·81 J) was absorbed by the scalp, 22·05% (46·0 J) by the outer compact bone, 17·12% (35·72 J) by the porous bone, 27·44% (57·23 J) by the inner compact bone, and 7·31% (15·24 J) by the facial bone. The rest of the internal potential energy was defined to reach the GM (3·6%, 7·51 J) and the WM 1·59% (3·31 J). Also, the ICP, strain energy density, and 1st principal strain levels, defined as the dynamic triple peak impact factor, influenced the GM and WM with their own impact peaks during the first 10 ms after the accident and were the highest for the 10 and 6 m/s impacts, while the 2 m/s impact had only a slight influence on the GM and WM structures.CONCLUSIONS: The present study shows for the first time that following an impact of 10 m/s, 88·31% of the calculated external kinetic energy was absorbed by the external parts of the head before the remaining energy of 5·19% reached the GM and WM. GM absorbed about twice as much of the energy compared to the WM. It is suggested that the dynamic triple peak impact factor may have a profound effect on native protein structures in the cerebral metabolism after a TBI.

Influence of Skull Fracture on Traumatic Brain Injury Risk Induced by Blunt Impact

Abstract:This study is aimed at investigating the influence of skull fractures on traumatic brain injury induced by blunt impact via numerous studies of head-ground impacts. First, finite element (FE) damage modeling was implemented in the skull of the Total HUman Model for Safety (THUMS), and the skull fracture prediction performance was validated against a head-ground impact experiment. Then, the original head model of the THUMS was assigned as the control model without skull element damage modeling. Eighteen (18) head-ground impact models were established using these two FE head models, with three head impact locations (frontal, parietal, and occipital regions) and three impact velocities (25, 35, and 45 km/h). The predicted maximum principal strain and cumulative strain damage measure of the brain tissue were employed to evaluate the effect of skull fracture on the cerebral contusion and diffuse brain injury risks, respectively. Simulation results showed that the skull fracture could reduce the risk of diffuse brain injury risk under medium and high velocities significantly, while it could increase the risk of brain contusion under high-impact velocity.

Abstract:In this study, we investigate the effects of modelling choices for the brain–skull interface (layers of tissues between the brain and skull that determine boundary conditions for the brain) and the constitutive model of brain parenchyma on the brain responses under violent impact as predicted using computational biomechanics model. We used the head/brain model from Total HUman Model for Safety (THUMS)—extensively validated finite element model of the human body that has been applied in numerous injury biomechanics studies. The computations were conducted using a well-established nonlinear explicit dynamics finite element code LS-DYNA. We employed four approaches for modelling the brain–skull interface and four constitutive models for the brain tissue in the numerical simulations of the experiments on post-mortem human subjects exposed to violent impacts reported in the literature. The brain–skull interface models included direct representation of the brain meninges and cerebrospinal fluid, outer brain surface rigidly attached to the skull, frictionless sliding contact between the brain and skull, and a layer of spring-type cohesive elements between the brain and skull. We considered Ogden hyperviscoelastic, Mooney–Rivlin hyperviscoelastic, neo–Hookean hyperviscoelastic and linear viscoelastic constitutive models of the brain tissue. Our study indicates that the predicted deformations within the brain and related brain injury criteria are strongly affected by both the approach of modelling the brain–skull interface and the constitutive model of the brain parenchyma tissues. The results suggest that accurate prediction of deformations within the brain and risk of brain injury due to violent impact using computational biomechanics models may require representation of the meninges and subarachnoidal space with cerebrospinal fluid in the model and application of hyperviscoelastic (preferably Ogden-type) constitutive model for the brain tissue.

Abstract:Given the worldwide adverse impact of traumatic brain injury (TBI) on the human population, its diagnosis and prediction are of utmost importance. Historically, many studies have focused on associating head kinematics to brain injury risk. Recently, there has been a push towards using computationally expensive finite element (FE) models of the brain to create tissue deformation metrics of brain injury. Here, we develop a new brain injury metric, the Brain Angle Metric (BAM), based on the dynamics of a 3 degree-of-freedom lumped parameter brain model. The brain model is built based on the measured natural frequencies of an FE brain model simulated with live human impact data. We show it can be used to rapidly estimate peak brain strains experienced during head rotational accelerations that cause mild TBI. On our dataset, the simplified model correlates with peak principal FE strain (R2=0.82). Further, coronal and axial brain model displacement correlated with fiber-oriented peak strain in the corpus callosum (R2=0.77). Our proposed injury metric BAM uses the maximum angle predicted by our brain model and is compared against a number of existing rotational and translational kinematic injury metrics on a dataset of head kinematics from 27 clinically diagnosed injuries and 887 non-injuries. We found that BAM performed comparably to peak angular acceleration, translational acceleration, and angular velocity in classifying injury and non-injury events. Metrics which separated time traces into their directional components had improved model deviance to those which combined components into a single time trace magnitude. Our brain model can be used in future work to rapidly approximate the peak strain resulting from mild to moderate head impacts and to quickly assess brain injury risk.

Finite element simulations of the head–brain responses to the top impacts of a construction helmet: Effects of the neck and body mass

Abstract:Traumatic brain injuries are among the most common severely disabling injuries in the United States. Construction helmets are considered essential personal protective equipment for reducing traumatic brain injury risks at work sites. In this study, we proposed a practical finite element modeling approach that would be suitable for engineers to optimize construction helmet design. The finite element model includes all essential anatomical structures of a human head (i.e. skin, scalp, skull, cerebrospinal fluid, brain, medulla, spinal cord, cervical vertebrae, and discs) and all major engineering components of a construction helmet (i.e. shell and suspension system). The head finite element model has been calibrated using the experimental data in the literature. It is technically difficult to precisely account for the effects of the neck and body mass on the dynamic responses, because the finite element model does not include the entire human body. An approximation approach has been developed to account for the effects of the neck and body mass on the dynamic responses of the head–brain. Using the proposed model, we have calculated the responses of the head–brain during a top impact when wearing a construction helmet. The proposed modeling approach would provide a tool to improve the helmet design on a biomechanical basis.
Peaks and Distributions of White Matter Tract-related Strains in Bicycle Helmeted Impacts: Implication for Helmet Ranking and Optimization

Abstract:Traumatic brain injury (TBI) in cyclists is a growing public health problem, with helmets being the major protection gear. Finite element head models have been increasingly used to engineer safer helmets often by mitigating brain strain peaks. However, how different helmets alter the spatial distribution of brain strain remains largely unknown. Besides, existing research primarily used maximum principal strain (MPS) as the injury parameter, while white matter fiber tract-related strains, increasingly recognized as effective predictors for TBI, have rarely been used for helmet evaluation. To address these research gaps, we used an anatomically detailed head model with embedded fiber tracts to simulate fifty-one helmeted impacts, encompassing seventeen bicycle helmets under three impact locations. We assessed the helmet performance based on four tract-related strains characterizing the normal and shear strain oriented along and perpendicular to the fiber tract, as well as the prevalently used MPS. Our results showed that both the helmet model and impact location affected the strain peaks. Interestingly, we noted that helmets did not alter strain distribution, except for one helmet under one specific impact location. Moreover, our analyses revealed that helmet ranking outcome based on strain peaks was affected by the choice of injury metrics (Kendall tau coefficient: 0.58 ~ 0.93). Significant correlations were noted between tract-related strains and angular motion-based injury metrics. This study provided new insights into computational brain biomechanics and highlighted the helmet ranking outcome was dependent on the choice of injury metrics. Our results also hinted that the performance of helmets could be augmented by mitigating the strain peak and optimizing the strain distribution with accounting the selective vulnerability of brain subregions, although more research was needed to develop region-specific injury criteria.

Use of Brain Biomechanical Models for Monitoring Impact Exposure in Contact Sports

Abstract:Head acceleration measurement sensors are now widely deployed in the field to monitor head kinematic exposure in contact sports. The wealth of impact kinematics data provides valuable, yet challenging, opportunities to study the biomechanical basis of mild traumatic brain injury (mTBI) and subconcussive kinematic exposure. Head impact kinematics are translated into brain mechanical responses through physics-based computational simulations using validated brain models to study the mechanisms of injury. First, this article reviews representative legacy and contemporary brain biomechanical models primarily used for blunt impact simulation. Then, it summarizes perspectives regarding the development and validation of these models, and discusses how simulation results can be interpreted to facilitate injury risk assessment and head acceleration exposure monitoring in the context of contact sports. Recommendations and consensus statements are presented on the use of validated brain models in conjunction with kinematic sensor data to understand the biomechanics of mTBI and subconcussion. Mainly, there is general consensus that validated brain models have strong potential to improve injury prediction and interpretation of subconcussive kinematic exposure over global head kinematics alone. Nevertheless, a major roadblock to this capability is the lack of sufficient data encompassing different sports, sex, age and other factors. The authors recommend further integration of sensor data and simulations with modern data science techniques to generate large datasets of exposures and predicted brain responses along with associated clinical findings. These efforts are anticipated to help better understand the biomechanical basis of mTBI and improve the effectiveness in monitoring kinematic exposure in contact sports for risk and injury mitigation purposes.

Abstract:The effect of repetitive sub-concussive head impact exposure in contact sports like American football on brain health is poorly understood, especially in the understudied populations of youth and high school players. These players, aged 9-18 years old may be particularly susceptible to impact exposure as their brains are undergoing rapid maturation. This study helps fill the void by quantifying the association between head impact exposure and functional connectivity, an important aspect of brain health measurable via resting-state fMRI (rs-fMRI). The contributions of this paper are three fold. First, the data from two separate studies (youth and high school) are combined to form a high-powered analysis with 60 players. These players experience head acceleration within overlapping impact exposure making their combination particularly appropriate. Second, multiple features are extracted from rs-fMRI and tested for their association with impact exposure. One type of feature is the power spectral density decomposition of intrinsic, spatially distributed networks extracted via independent components analysis (ICA). Another feature type is the functional connectivity between brain regions known often associated with mild traumatic brain injury (mTBI). Third, multiple supervised machine learning algorithms are evaluated for their stability and predictive accuracy in a low bias, nested cross-validation modeling framework. Each classifier predicts whether a player sustained low or high levels of head impact exposure. The nested cross validation reveals similarly high classification performance across the feature types, and the Support Vector, Extremely randomized trees, and Gradboost classifiers achieve F1-score up to 75%.

Abstract:The common belief that animals with larger heads are more tolerated to brain injury faces challenges under the extreme conditions of blast loading. Recent studies indicate that humans, who have notably larger heads than other species of similar body weight, exhibit a unique vulnerability. Integrating animal experimental data, advanced head modeling, and pressure propagation theories, this research elucidates the injury mechanisms across species as the blast wave transitions from the extremely hard skull to the extremely soft brain. We propose a new interspecies scaling law based on consistent peaks of intracranial pressure, rather than head size, to redefine the translation from animal exposure thresholds to human risk assessment. This shift in perspective underscores the imperative to comprehensively consider both head geometry and size in predicting tolerance to blast brain injury, moving beyond simplistic size-based comparisons. Our study's insights contribute significantly to redefining injury risk models and fostering innovative prevention strategies against blast-induced traumatic brain injury (bTBI).
[How can psychological help be provided for the patient/caregiver tandem home?].

Abstract:INTRODUCTION Elderly people and their family helpers are often isolated at home and do not have access to the professional psychological help that they require. For an elderly population, the trips to consulting rooms are difficult, tedious and expensive. Besides, maintaining a patient at home is heavy to manage for close relatives because of the organization, financial issues and above all the risk of psychological burnout. The literature shows us that psychological assistance is more common at distance from home, in hospitals, in special institutions or specific organizations. However, there is a clear need of help at home. We propose to develop psychological assistance at home for the patient/helper tandem in cases of dementia. This prospective study reports three different cases. METHOD This study is a qualitative pilot study. Participants characteristics: patients were diagnosed with severe dementia, assistance showed deep fatigue of the helper, both members of the tandem lived at home, age above 69 years, the eed for nursing care at home. THERAPEUTIC ASSESSMENT: The psychological clinician acts after proposing his helpand not on a clear request from the patient. He will meet his patient 7 times during 4 months. The meetings always take place in the same place and last 45 minutes. Confidentiality during the meeting is explained and guaranteed so the patient can speak openly. EVALUATION An independent psychologist assessed the monitoring in the hospital. Our methodology included two assessments: the first, assessing the task of the helper before the evaluation and the second taking into account a new measurent of the task and the opinions of the participants following a semi-directed interview. The evaluation of the task was performed using Zarit's scale. The evaluation was also based on the clinical observation of the psychologist. RESULTS Organization of the monitoring: one meeting per week was sufficient for all participants with a minimal duration of 45 minutes. The fact that the meetingsrook place at home was appreciated, because of their simple and convenient organization. The patient/helper tandems in those suffering from advanced dementia could only have taken place in the home because transport any where else would have beent too difficult for them. The sessions during this research study were free of charge. Two out of three participants were ready to pay so long as the study could continue. The managers were bothered a few times by phone calls and/or unexpected visits, invitations for a cup of tea and requests for a small service (like mailing a letter). Assistance for the caregiver: all the caregivers declared that assistance was a personal improvement in a period of doubt, loss of self-confidence and isolation. Furthermore, clinical observation of the therapeutic assistance appears to show that psychological help at home could lead to the improved psychological function of the helper. This was emphasized when we established the limits of the caregiver/patient relationship. We observed a better balance in the input of investing and de-investing and better acceptance of the identity modifications which were required for the caregiver. Assistance for the patient: we believe that this sort of intervention has positive effects on the patients themselves. This care at home protects the destructurated identity of the patients and their intimity. Moreover, intrapsychic tension can be lowered by being shared with the psychologist. Assessment of the burden: among the three patients who wete studied, the burden was unaltered in one case (43/88; 43/88), significantly improved in one case (41/88; 24/88) and remained light in one case (18/88; 16/88). DISCUSSION We discovered that Zarit's test showed some limits. The time to complete the test is quite long and tedious for caregivers. Some questions are too direct and can put the caregivers in a guilty position. The mini Zarit version with only seven items, appears more satisfactory because it's shorter and provokes less guilt. The other point concerns the therapeutic frame at home. The usually represents all the constants of the therapeutic process including the role of the psychologist and all the items that refer to space, time, management of the timetable, payement and interruptions in care. Could the fact of being at home be harmful for the therapeutic process? Of course, the place of residence is less neutral than a office in an institution and the superposition of the frame of life and of the therapeutic frame raises lots of questions for the psychologist: entering a private house is like entering a private life, which is not without consequences on the follow-up. We are here far from a classical frame of therapeutic interviews, so the frame must be clearly defined. CONCLUSION This prospective study leads us to the conclusion that the superposition of the frame of life and of the therapeutic frame represents a limit to psychotherapy but is not exclusive of psychological support at home. As a supplement to this face to face follow up at home, we could imagine other ways of providing such psychological support, by phone or by telemedicine for instance. Could the new technologies of communication help to compensate the lack of means in favour of the caregivers at home? Although these new technologies are more dedicated to institutions than to providing care at home, could they not be helpful for organizing psychological help at home? However, in order to validate such devices, they need to be fried and assessed at home.

Abstract:Abstract The brain response to the explosion-induced primary blast waves is actively sought. Over the past decade, reasonable progress has been made in the fundamental understanding of bTBI using head surrogates and animal models. Yet, the current understanding of how blast waves interact with human is in nascent stages, primarily due to the lack of data in human. The biomechanical response in human is critically required to faithfully establish the connection to the aforementioned bTBI models. In this work, the biomechanical cascade of the brain under a primary blast has been elucidated using a detailed, full-body human model. The full-body model allowed us to holistically probe short- (<5 ms) and long-term (200 ms) brain responses. The full-body model has been extensively validated against impact loading in the past. We have further validated the head model against blast loading. We have also incorporated the structural anisotropy of the brain white matter. The blast wave transmission, linear and rotational motion of the head were dominant pathways for the loading of the brain, and these loading paradigms generated distinct biomechanical fields within the brain. Blast transmission and linear motion of the head governed the volumetric response, whereas the rotational motion of the head governed the deviatoric response. Blast induced head rotation alone produced diffuse injury pattern in white matter fiber tracts. The biomechanical response under blast was comparable to the impact event. These insights will augment laboratory and clinical investigations of bTBI and help devise better blast mitigation strategies.
