Abstract:Different kinds of traumatic experiences like natural catastrophes vs. relational traumatic experiences (e.g., sex/physical abuse, interpersonal partner violence) are involved in the development of the self and PTSD psychopathological manifestations. Looking at a neuroscience approach, it has been proposed a nested hierarchical model of self, which identifies three neural-mental networks: (i) interoceptive; (ii) exteroceptive; (iii) mental. However, it is still unclear how the self and its related brain networks might be affected by non-relational vs relational traumatic experiences. Departing from this background, the current study aims at conducting a meta-analytic review of task-dependent fMRI studies (i.e., emotional processing task) among patients with PTSD due to non-relational (PTSD-NR) and relational (PTSD-R) traumatic experiences using two approaches: (i) a Bayesian network meta-analysis for a region-of-interest–based approach; (ii) a coordinated-based meta-analysis. Our findings suggested that the PTSD-NR mainly recruited areas ascribed to the mental self to process emotional stimuli. Whereas, the PTSD-R mainly activated regions associated with the intero-exteroceptive self. Accordingly, the PTSD-R compared to the PTSD-NR might not reach a higher symbolic capacity to process stimuli with an emotional valence. These results are also clinically relevant in support of the development of differential treatment approaches for non-relational vs. relational PTSD.

Abstract:The I or the self is the most intimate and familiar phenomena, but it is one of the most perplexing phenomenon in our intellectual life. The mind science has deeply studied various aspects of mental agents such as intentionality, perception, behavior, memory, learning, image, emotion, thinking, language, imagination, unconscious and even consciousness, however, unless tied closely with the notion of the self, all the aspects of mind mentioned above can be significatively talked about. At the same time, we have to immediately emphasize that the self is not a pole of entity like homunculus. So we argue that the self can be understood just from a perspective of structure. Human ordinary experience shows that the self primarily begins with a kind of living phenomena. F. Maturana J. Varela described the general structure of the self as a living being in terms of the notion of autopoeisis. The most basic feature of living beings—even the simplest live beings—is that living beings are autonomous individuals which have built a kind of the most primitive relationship of intentionality or meaning with the world. There are different morphologies of self in the evolutionary and developmental different levels of life-mind. The unified sense of self is the most direct experience in human ordinary life, which our knowledge of the self starts from. Unlike the self as a pure living being, how does the self with the sense of self come to being? A. Dmamasio argues that the neurobiology of consciousness faces two problems: the problem of how the movie-in-the-brain is generated, and the problem of how the brain generates the sense that there is an observer and owner for the movie, namely the sense of self. How are feelings embodied?—it is central to the current study of neural self. The perplexity from the self not only consists in the inherent particularity, but also the particular circulation relationship between the self and the world. Only when the circulation relationship is understood well, we can completely grasp the nature of self.
Intrinsic Brain Abnormalities in Young Healthy Adults with Childhood Trauma: A Resting-State Functional Magnetic Resonance Imaging Study of Regional Homogeneity and Functional Connectivity

Abstract:Objective: Childhood trauma confers great risk for the development of multiple psychiatric disorders; however, the neural basis for this association is still unknown. The present resting-state functional magnetic resonance imaging study aimed to detect the effects of childhood trauma on brain function in a group of young healthy adults. Methods: In total, 24 healthy individuals with childhood trauma and 24 age- and sex-matched adults without childhood trauma were recruited. Each participant underwent resting-state functional magnetic resonance imaging scanning. Intra-regional brain activity was evaluated by regional homogeneity method and compared between groups. Areas with altered regional homogeneity were further selected as seeds in subsequent functional connectivity analysis. Statistical analyses were performed by setting current depression and anxiety as covariates. Results: Adults with childhood trauma showed decreased regional homogeneity in bilateral superior temporal gyrus and insula, and the right inferior parietal lobule, as well as increased regional homogeneity in the right cerebellum and left middle temporal gyrus. Regional homogeneity values in the left middle temporal gyrus, right insula and right cerebellum were correlated with childhood trauma severity. In addition, individuals with childhood trauma also exhibited altered default mode network, cerebellum-default mode network and insula-default mode network connectivity when the left middle temporal gyrus, right cerebellum and right insula were selected as seed area, respectively. Conclusion: The present outcomes suggest that childhood trauma is associated with disturbed intrinsic brain function, especially the default mode network, in adults even without psychiatric diagnoses, which may mediate the relationship between childhood trauma and psychiatric disorders in later life.
An innate brainstem self-other system involving orienting, affective responding, and polyvalent relational seeking: Some clinical implications for a "Deep Brain Reorienting" trauma psychotherapy approach

Abstract:Underlying any complex relational intersubjectivity there is an inherent urge to connect, to have proximity, to engage in an experience of interpersonal contact. The hypothesis set out here is that this most basic urge to connect is dependent on circuits based in three main components: the midbrain superior colliculi (SC), the midbrain periaqueductal gray (PAG), and the mesolimbic and mesocortical dopamine systems originating in the midbrain ventral tegmental area. Firstly, there is orienting towards or away from interpersonal contact, dependent on approach and/or defensive/withdrawal areas of the SC. Secondly, there is an affective response to the contact, mediated by the PAG. Thirdly, there is an associated, affectively-loaded, seeking drive based in the mesolimbic and mesocortical dopamine systems. The neurochemical milieu of these dopaminergic systems is responsive to environmental factors, creating the possibility of multiple states of functioning with different affective valences, a polyvalent range of subjectively positive and negative experiences. The recognition of subtle tension changes in skeletal muscles when orienting to an affectively significant experience or event has clinical implications for processing of traumatic memories, including those of a relational/interpersonal nature. Sequences established at the brainstem level can underlie patterns of attachment responding that repeat over many years in different contexts. The interaction of the innate system for connection with that for alarm, through circuits based in the locus coeruleus, and that for defence, based in circuits through the PAG, can lay down deep patterns of emotional and energetic responses to relational stimuli. There may be simultaneous sequences for attachment approach and defensive aggression underlying relational styles that are so deep as to be seen as personality characteristics, for example, of borderline type. A clinical approach derived from these hypotheses, Deep Brain Reorienting, is briefly outlined as it provides a way to address the somatic residues of adverse interpersonal interactions underlying relational patterns and also the residual shock and horror of traumatic experiences. We suggest that the innate alarm system involving the SC and the locus coeruleus can generate a pre-affective shock while an affective shock can arise from excessive stimulation of the PAG. Clinically significant residues can be accessed through careful, mindful, attention to orienting-tension-affect-seeking sequences when the therapist and the client collaborate on eliciting and describing them.
The Intrinsic Hierarchy of Self – Converging Topography and Dynamics

Abstract:The brain can be characterized by an intrinsic hierarchy in its topography which, as recently shown for the uni-transmodal distinction of core and periphery, converges with its dynamics. Does such intrinsic hierarchical organization in both topography and dynamic also apply to the brain’s inner core itself and its higher-order cognitive functions like self? Applying multiple fMRI data sets, we show how the recently established three-layer topography of self (internal, external, mental) is already present during the resting state and carried over to task states including both task-specific and -unspecific effects. Moreover, the topographic hierarchy converges with corresponding dynamic changes (measured by power-law exponent, autocorrelation window, median frequency, sample entropy, complexity) during both rest and task states. Finally, analogous to the topographic hierarchy, we also demonstrate hierarchy among the different dynamic measures themselves according to background and foreground. Finally, we show task-specific- and un-specific effects in the hierarchies of both dynamics and topography. Together, we demonstrate the existence of an intrinsic topographic hierarchy of self and its convergence with dynamics.

Disturbance at the self-other boundary in schizophrenia: Linking phenomenology to clinical neuroscience

Abstract:In this selective review, we describe the current neuroscientific literature on disturbances of the self-other boundary in schizophrenia as they relate to structural and experiential aspects of the self. Within these two broad categories, the structural self includes body ownership and agency, and the experiential self includes self-reflection, source monitoring, and self-referential and autobiographical memory. Further, we consider how disturbances in these domains link to the phenomenology of schizophrenia. We identify faulty internal predictive coding as a potential mechanism of disturbance in body ownership and agency, which results in susceptibility to bias (over- or under-attributing outcomes to one's own actions or intentions). This is reflected in reduced activity in the temporoparietal junction (TPJ), a heteromodal association area implicated in several aspects of self-other processing, as well as reduced fronto-parietal functional connectivity. Deficits of the experiential self in schizophrenia may stem from a lack of salience of self-related information, whereby the mental representation of self is not as rich as in healthy controls and therefore does not result in the same level of privileged processing. As a result, memory for self-referential material and autobiographical memory processes is impaired, which hinders creation of a cohesive life narrative. Impairments of the experiential self implicate abnormal activation patterns along the cortical midline, including medial prefrontal cortex and posterior cingulate/precuneus, as well as TPJ. In fact, TPJ appears to be involved in all the reviewed aspects of the self-other disturbance. We conclude with suggestions for future work, including implications for interventions with critical timing considerations.

Abstract:The neuroscience of trauma in increasingly oriented to the importance of understanding right brain to right brain dynamics and helping the therapist to respond to implicit communication, which is discussed, as well as Porges’ polyvagal theory. The traumatized patient presents a variety of challenges to clinicians, including a dysregulated autonomic nervous system (ANS), compromised ability to self-soothe and diminished capacity for relatedness with others. Therefore, one effective approach to treating trauma (especially dissociation) includes conceptualizing and responding to our patients through three separate but interconnected lenses: (1) patient and therapist as human mammals—orientation to the ANS, (2) patient and therapist as personalities—orientation to the internal world and, (3) patient and therapist as inhabiting an intersubjective field together—orientation to shared, co-created body/psychological states. The capacity to be guided by countertransference is an especially valuable skill for therapists, as well as the ability to know when they are being experienced as helper or adversary by their patients. Different dynamics driving dissociation and different strategies for responding to the dissociating patient are discussed. Clinical examples are included.

Abstract:Intriguing explorations at the intersection of the fields of neuroscience and psychology are driven by the quest to understand the neural underpinnings of “the self” and their psychotherapeutic implications. These translational efforts pertain to the unique Creative Arts Therapies (CATs) and the attributes and value of the self-related processes they offer. The self is considered as a multi-layered complex construct, comprising bodily and mental constituents, subjective–objective perspectives, spatial and temporal dimensions. Neuroscience research, mostly functional brain imaging, has proposed cogent models of the constitution, development and experience of the self, elucidating how the multiple dimensions of the self are supported by integrated hierarchical brain processes. The psychotherapeutic use of the art-forms, generating aesthetic experiences and creative processes, touch upon and connect the various layers of self-experience, nurturing the sense of self. The present conceptual analysis will describe and interweave the neural mechanisms and neural network configuration suggested to lie at the core of the ongoing self-experience, its deviations in psychopathology, and implications regarding the psychotherapeutic use of the arts. The well-established, parsimonious and neurobiologically plausible predictive processing account of brain-function will be discussed with regard to selfhood and consciousness. The epistemic affordance of the experiential CATs will further be portrayed, enabling and facilitating the creation of updated self-models of the body in the world. The neuropsychological impact of the relational therapeutic encounter will be delineated, acknowledging the intersubjective brain synchronization through communicative verbal and non-verbal means and aesthetic experiences. The recognition and assimilation of neuroscientific, phenomenological and clinical perspectives concerning the nested dimensionality of the self, ground the relational therapeutic process and the neuroplastic modulations that CATs have to offer on the premise of fostering, shaping and integrating selfhood.

Abstract:Recent scholarly attention to the affinity of neuroscience and Pierre Janet’s psychology has led to his reassessment as a precursor of neuroscientific trauma studies. This essay examines Janet’s trauma theory in the context of this reappraisal of his psychology in the age of neuroscience. Janet’s resurgence is primarily due to the fact that he constructed his psychological system on his keen observations of the inseparable connection and dynamic interaction between the mind and the body. Janet’s observation of hysterical patients’ unconscious automatic activities enables an understanding of human activity as a psychological phenomenon, leading him to the discovery of the subconscious which is dissociated from the normal consciousness and holds fixed ideas that cause symptoms of involuntary movement and paralysis. Dissociation results from the narrowing of consciousness and its failure to integrate perceptions. Traumatic events, together with heredity factors, induce the retraction of consciousness, precipitating the mind’s lower function such as psychological automatism, and impairing its higher function of integration. Janet’s theory of psychological healing aims to reintegrate dissociated consciousness by finding and liquidating fixed ideas through suggestion, but it also substitutes those ideas for positive re-conceptualization of traumatic events and even dismantling and excising traumatic memories. Unable to assimilate traumatic events into the narrative of his life history, the patient repeatedly and unwittingly re-enacts them. Accordingly, healing involves “presentification” which empowers the patient to narrate traumatic events as past accidents from the vantage point of the present. Adaptation is accomplished by economically assessing the level of patient’s mental energy, which has been depleted by trauma, and promoting diverse methods to prevent further exhaustion and reinvest psychological “force” into higher “tendencies” of the mind. Janet also proposes the therapist’s “moral guidance” that balances the therapist’s active intervention and his support of the patient’s psychological independence, which reminds neuroscientific traumatologists of the importance of the subject’s agency and human interaction in the healing process.

Abstract:A deep understanding of the subjective experience of time in traumatized individuals may require a comprehensive framework that takes into account both psychoanalytic and phenomenological contributions. Referring to the retroactive interpretation of past experiences, the concept of Nachträglichkeit is critical to analyze how trauma can be signified in the form of the après-coup , in which the original traces of traumatic experiences are signified only at a later time. Trauma alters the temporal sequence of past, present, and future, thus leaving the psyche in a time-shifted dimension, where the shadow of the past extends over the present, and the unbearable present hinders growth and development. A clinical vignette is presented to illustrate how trauma can disrupt the temporal nature of subjective experience by reshaping the meaning of psychic events. Ultimately, trauma treatment aims at inscribing the person's experience into a unified and coherent self-narrative.
Embodied intersubjectivity: Forms of psyche-soma structuring in the encounter between self and other-than-self

Abstract:This paper explores the mechanisms that lead to a destructive tendency in the formation and functioning of the psychic apparatus, to the characteristic states of subjects who are drawn to non-life. The dynamics of the primary mother-child relationship involve a structural interaction between mind and body, subject and object. The dialectic between the life drive and the death drive is conceptualized as the structuring of homeostatic dynamic equilibria, in which both drives belong to the living, provided they are kept in a non-isolated system. This conception has analogies with other disciplines that have changed their paradigms, such as neurobiology, which, for living beings in open systems, hypothesises a continuous interconnected Becoming of undivided separation and of discontinuity. In unitary psyche-soma functioning, a dynamic homoeostatic balance marks the state of health of the relating subject; or if, instead, the system is isolated, a pathological dysregulation depending on the emotional-affective vicissitudes it undergoes. Two clinical cases illustrate these dynamics. For this tendency on the level of the somatopsychic unit, the name alloiosis has been put forward, in analogy with cellular apoptosis.
Self and the Brain. The Immune Metaphor

Abstract:One of the fundamental questions in neuroscience is how brain activity relates to conscious experience. Even though self-consciousness is considered an emergent property of the brain network, a quantum physics-based theory assigns a momentum of consciousness to the single neuron level. In this work, we present a brain self theory from an evolutionary biological perspective by analogy with the immune self. In this scheme, perinatal reactivity to self inputs would guide the selection of neocortical neurons within the subplate, similarly to T lymphocytes in the thymus. Such self-driven neuronal selection would enable effective discrimination of external inputs and avoid harmful "autoreactive" responses. Multiple experimental and clinical evidences for this model are provided. Based on this self tenet, we outline the postulates of the so-called autophrenic diseases, to then make the case for schizophrenia, an archetypic disease with rupture of the self. Implications of this model are discussed, along with potential experimental verification.
Homeorhesis: envisaging the logic of life trajectories in molecular research on trauma and its effects

Abstract:What sets someone on a life trajectory? This question is at the heart of studies of 21st-century neurosciences that build on scientific models developed over the last 150 years that attempt to link psychopathology risk and human development. Historically, this research has documented persistent effects of singular, negative life experiences on people's subsequent development. More recently, studies have documented neuromolecular effects of early life adversity on life trajectories, resulting in models that frame lives as disproportionately affected by early negative experiences. This view is dominant, despite little evidence of the stability of the presumably early-developed molecular traits and their potential effects on phenotypes. We argue that in the context of gaps in knowledge and the need for scientists to reason across molecular and phenotypic scales, as well as time spans that can extend beyond an individual's life, specific interpretative frameworks shape the ways in which individual scientific findings are assessed. In the process, scientific reasoning oscillates between understandings of cellular homeostasis and organisms' homeorhesis, or life trajectory. Biologist and historian François Jacob described this framework as the "attitude" that researchers bring to bear on their "objects" of study. Through an analysis of, first, historical and contemporary scientific literature and then ethnographic research with neuroscientists, we consider how early life trauma came to be associated with specific psychological and neurobiological effects grounded in understandings of life trajectories. We conclude with a consideration of the conceptual, ontological, and ethical implications of interpreting life trajectories as the result of the persistence of long-embodied biological traits, persistent life environments, or both.

Abstract:The interplay between the brain and interoceptive signals is key in maintaining internal balance and orchestrating neural dynamics, encompassing influences on perceptual and self-awareness. Central to this interplay is the differentiation between the external world, others and the self, a cornerstone in the construction of bodily self-awareness. This review synthesizes physiological and behavioral evidence illustrating how interoceptive signals can mediate or influence bodily self-awareness, by encompassing interactions with various sensory modalities. To deepen our understanding of the basis of bodily self-awareness, we propose a network physiology perspective. This approach explores complex neural computations across multiple nodes, shifting the focus from localized areas to large-scale neural networks. It examines how these networks operate in parallel with and adapt to changes in visceral activities. Within this framework, we propose to investigate physiological factors that disrupt bodily self-awareness, emphasizing the impact of interoceptive pathway disruptions, offering insights across several clinical contexts. This integrative perspective not only can enhance the accuracy of mental health assessments but also paves the way for targeted interventions.

Abstract:The paper by Northoff et al. [1] is an outstanding example of the way to overcome the translational crisis in contemporary psychiatry. We completely agree with their proposal that the focus on first-person experiences of time and space is in an ideal position to address the anomalies of subjectivity that we find in the clinic of psychopathological disorders and, if extended to the brain's spatiotemporal topographic-dynamic structure, is the key to addressing the anomalies of neurobiological mechanism. We hope that this approach to the mind-brain gap will find an increasingly favourable echo in the field of translational research and yet we wish to focus on a common limitation in Northoff et al.'s model and the other translational hypotheses of which we are aware. Northoff et al. are on the right track in that they put subjective experience—and not other non-specific parameters (e.g. behaviour)—at the centre of translational studies. However, this is but a step however great in the right direction. A science of the human brain cannot but take into account the individual peculiarities that characterise the human condition. How this is achievable is not for us to formulate. We can, however, say what the clinical, epistemological and ethical necessities are for insisting in this direction.

Abstract:This paper provides a discussion of the life cycle using a framework constructed from ideas in chaos theory. Building upon psychodynamic ideas from Freud, Jung, and Erikson, the integration of the Self is discussed from the viewpoint of Analytical Psychology. It is proposed that throughout our lives we encounter crisis points that fractionate the ego and necessitate use of more complex and adaptive psychic structures to contain or handle the seemingly disparate qualities that arise. We may say that eventually one aspires to realize Analytical Psychology's goal of integrating the superior and the inferior functions of the psyche into a cohesive Self. Jung seemed to state that the Self could never be attained, but it is argued in this paper that a form of transitory self may be realized periodically during the course of the life cycle.

Abstract:Some people diagnosed with "schizophrenia" showed a fundamental alteration of the sense of self. From a psychodynamic perspective, it has been hypothesized that patients diagnosed as schizophrenic have disorders of the embodied self and its boundaries. Phenomenologically, it has been observed a self-disorder, at an implicit and pre-reflective level of bodily awareness. Neuroscientific evidence seems to confirm the aforementioned aspects. The mean levels of Interoceptive Accuracy (IA), which is an objective empirical measure of interoception, namely a measure of basic awareness distinct from subjective measures, are significantly lower in patients with a diagnosis of schizophrenia than among healthy controls. Affiliative touch is crucial for constructing a basic sense of self and its boundaries, and it affects interoception. In order to reduce the disturbance of the sense of self, Angelo participated for three years in an experimental intervention plan including Amniotic Therapy (AT) and individual psychodynamic psychotherapy. AT is a group-therapy based on sensory-motor interactions, especially affiliative touch, similar to those involved in "holding". Results showed an increase in the Angelo's IA and global functioning, as well as a significant decrease in positive symptoms. In Angelo, AT seems to strengthen a basic sense of self, reducing self-disorder.

Moreover, within the psychological level, the landscape of available theories is quite fragmented. Schizophrenia is a complex condition in which both genetic and environmental factors are known to play a role, but few models have been proposed that account for both. These are limits of current approaches that the present work tries to address. Objective In this work, we present a holistic model of normal and traumatic mental functioning, which seeks to merge into a single theoretical framework various theories used to explain different aspects of trauma: psychodynamic theory, attachment theory, theory of dissociation as response to trauma. The insight gained from the model is subsequently used to provide an interpretation for schizophrenia. Method We do not contribute new data to the field of study: the method used is purely theoretical. Our toolset includes the concept of artificial neural networks and, more in general, borrows ideas from the fields of artificial intelligence and artificial life. Results and conclusion We argue that dissociation is a standard tool used by the mind to protect itself from painful emotions. In case of repeated traumas, the mind learns to adopt selective forms of dissociation to avoid pain without losing touch with external reality. We conjecture that this process is defective in schizophrenia, where dissociation is either too weak, giving rise to positive symptoms, or too strong, causing negative symptoms.

Abstract:Seminal research has identified differential brain activation in individuals with PTSD and depersonalization/derealization symptoms in task-based and seed-based resting state functional connectivity analyses. However, trauma-related dissociation encompasses a wider array of symptoms. Furthermore, interrogation of large-scale intrinsic brain network connectivity related to the full spectrum of dissociative experiences could reveal a novel network of brain abnormalities.
Introducing the Neuroplastic Narrative: a non-pathologizing biological foundation for trauma-informed and adverse childhood experience aware approaches

Abstract:Most people accessing mental health services have adverse childhood experiences (ACEs) and/or histories of complex trauma. In recognition of this, there are calls to move away from medical model approaches and move toward trauma-informed approaches which privilege the impact of life experience over underlying pathology in the etiology of emotional and psychological suffering. Trauma-informed approaches lack a biological narrative linking trauma and adversity to later suffering. In its absence, this suffering is diagnosed and treated as a mental illness. This study articulates the Neuroplastic Narrative, a neuroecological theory that fills this gap, conceptualizing emotional and psychological suffering as the cost of surviving and adapting to the impinging environments of trauma and adversity. The Neuroplastic Narrative privileges lived experience and recognizes that our experiences become embedded in our biology through evolved mechanisms that ultimately act to preserve survival in the service of reproduction. Neuroplasticity refers to the capacity of neural systems to adapt and change. Our many evolved neuroplastic mechanisms including epigenetics, neurogenesis, synaptic plasticity, and white matter plasticity allow us to learn from, and adapt to, past experiences. This learning and adaption in turn allows us to better anticipate and physiologically prepare for future experiences that (nature assumes) are likely to occur, based on past experiences. However, neuroplastic mechanisms cannot discriminate between experiences; they function to embed experience regardless of the quality of that experience, generating vicious or virtuous cycles of psychobiological anticipation, to help us survive or thrive in futures that resemble our privileged or traumatic pasts. The etiology of suffering that arises from this process is not a pathology (a healthy brain is a brain that can adapt to experience) but is the evolutionary cost of surviving traumatizing environments. Misidentifying this suffering as a pathology and responding with diagnosis and medication is not trauma-informed and may cause iatrogenic harm, in part through perpetuating stigma and exacerbating the shame which attends complex trauma and ACEs. As an alternative, this study introduces the Neuroplastic Narrative, which is situated within an evolutionary framework. The Neuroplastic Narrative complements both Life History and Attachment Theory and provides a non-pathologizing, biological foundation for trauma-informed and Adverse Childhood Experience aware approaches.
