Abstract:Abstract Background Femoral posterior condylar offset (PCO) and posterior tibial slope (PTS) are important for postoperative range of motion after total knee arthroplasty (TKA). However, normative data of PCO and PTS and the correlation between them among healthy populations remain to be elucidated. The purpose of this study was to determine PCO and PTS in normal knees, and to identify the correlation between them. Methods Eighty healthy volunteers were recruited. CT scans were performed followed by three-dimensional reconstruction. PCO and PTS were measured and analyzed, as well as the correlation between them. Results PTS averaged 6.78° and 6.11°, on the medial and lateral side respectively ( P = 0.002). Medial PCO was greater than lateral (29.2 vs. 23.8 mm, P < 0.001). Both medial and lateral PCO of male were larger than female. On the contrary, male medial PTS was smaller than female, while there was no significant difference of lateral PTS between genders. There was an inverse correlation between medial PCO and PTS, but not lateral. Conclusions Significant differences exhibited between medial and lateral compartments, genders, and among individuals. An inverse correlation exists between PCO and PTS in the medial compartment. These results improve our understanding of the morphology and biomechanics of normal knees, and subsequently for optimising prosthetic design and surgical techniques.

Isozyme frequency patterns in Drosophila pavani associated with geographical and seasonal variables.

Abstract:Fourteen population samples of Drosophila pavani were obtained from a number of localities in Chile. The populations sampled were dispersed over 7 degrees of latitude and 1800 meters of elevation, and were drawn at three different times. Sixteen electrophoretic loci were assayed for each population; eight of the loci were analyzed statistically for geographic variation; the other eight were essentially monomorphic. For all eight variable loci, variation in allelic frequencies among populations was highly significant. In all cases, a significant portion of the variation among populations was associated with variation in gross environmental variables (latitude, elevation, month of collection). The implications of the evidence were discussed, and the authors concluded that there was suggestive evidence for selection.
Comparison of instantaneous knee kinematics during walking and running.

Abstract:BACKGROUND:Accurate measurements of in-vivo knee joint kinematics are essential to elucidate healthy knee motion and the changes that accompany injury and repair. Although numerous experimental measurements have been reported, the accurate non-invasive analysis of in-vivo knee kinematics remains a challenge in biomechanics. RESEARCH QUESTION:The study objective was to investigate in-vivo knee kinematics before, at, and after contact during walking and running using a combined high-speed dual fluoroscopic imaging system (DFIS) and magnetic resonance (MR) imaging technique. METHODS:Three-dimensional (3D) knee models of ten participants were created using MR images. Knee kinematics during walking and running were determined using high-speed DFIS. The 3D knee models were then related to fluoroscopic images to obtain in-vivo six-degrees-of-freedom knee kinematics. RESULTS:Before contact knee flexion, external femoral rotation, and proximal-distal distance were 11.9°, 3.4°, and 1.0 mm greater during running compared to walking, respectively. Similar differences were observed at initial contact (9.9°, 7.9°, and 0.9 mm, respectively) and after contact (6.4°, 2.2°, and 0.8 mm, respectively). Posterior femoral translation at initial contact was also increased during running compared to walking. SIGNIFICANCE:This study demonstrated accurate instantaneous in-vivo knee kinematic characteristics that may further the understanding of the intrinsic biomechanics of the knee during gait.
Increased Posterior Tibial Slope Increases Force on the Posterior Medial Meniscus Root

Abstract:Background: Posterior medial meniscus root (PMMR) tears have been associated with increased posterior tibial slope, but this has not been fully evaluated biomechanically. In addition, the effects of knee flexion and rotation on the PMMR are not well understood biomechanically because of technological testing limitations. A novel multiaxial force sensor has made it possible to elucidate answers to these questions. Purpose: (1) To determine if increased posterior tibial slope results in increased posterior shear force and compression on the PMMR, (2) to evaluate how knee flexion angle affects PMMR forces, and (3) to assess how internal and external rotation affects force at the PMMR. Study design: Controlled laboratory study. Methods: Ten fresh-frozen cadaveric knees were tested in all combinations of 3 posterior tibial slopes and 4 flexion angles. A multiaxial force sensor was connected to the PMMR and installed below the posterior tibial plateau maintaining anatomic position. The specimen underwent a 500-N compression load followed by a 5-N·m internal torque and a 5-N·m external torque. The magnitude and direction of the forces acting on the PMMR were measured. Results: Under joint compression, an increased tibial slope significantly reduced the tension on the PMMR between 5° and 10° (from 13.5 N to 6.4 N), after which it transitioned to a significant increase in PMMR compression, reaching 7.6 N at 15°. Under internal torque, increased tibial slope resulted in 4.7 N of posterior shear at 5° significantly changed to 2.0 N of anterior shear at 10° and then 8.2 N of anterior shear at 15°. Under external torque, increased tibial slope significantly decreased PMMR compression (5°: 8.9 N; 10°: 4.3 N; 15°: 1.1 N). Under joint compression, increased flexion angle significantly increased medial shear forces of the PMMR (0°, 3.8 N; 30°, 6.2 N; 60°, 7.3 N; 90°, 8.4 N). Under internal torque, 90° of flexion significantly increased PMMR tension from 2.3 N to 7.5 N. Under external torque, 30° of flexion significantly increased PMMR compression from 4.7 N to 12.2 N. Conclusion: An increased posterior tibial slope affects compression and anterior shear forces at the PMMR. An increased flexion angle affects compression, tension, and medial shear forces at the PMMR. Clinical relevance: The increase in compression and posterior shear force when the knee is loaded in compression may place the PMMR under increased stress and risk potential failure after repair. This study provides clinicians with information to create safer protocols and improve repair techniques to minimize the forces experienced at the PMMR.
Increased Posterior Tibial Slope Is Associated With Increased Risk of Meniscal Root Tears: A Systematic Review

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:While increased posterior tibial slope (PTS) is an established risk factor for anterior cruciate ligament tears, the association between tibial slope and meniscal posterior root tears is not well-defined.Purpose:To summarize the available literature evaluating the association between PTS and meniscus root injuries compared with patients without root tears.Study Design:Systematic review; Level of evidence, 4.Methods:A literature search was performed using the Scopus, PubMed, and Embase databases. Human clinical studies evaluating the associations between the medial tibial slope (MTS), lateral tibial slope (LTS), lateral-to-medial (L-to-M) slope asymmetry, and the risk of meniscus root tears were included. Patients with medial meniscus posterior root tears (MMPRTs) and lateral meniscus posterior root tears (LMPRTs) were compared with a control group without root injury. Study quality was assessed using the methodological index for non-randomized studies criteria.Results:Ten studies with 1313 patients were included (884 patients with root tears; 429 controls). The LMPRT subgroup (n = 284) had a significantly greater LTS (mean ± SD, 7.3°± 1.5° vs 5.7°± 3.91°; P < .001), MTS (5.26°± 1.2° vs 4.8°± 1.25°; P < .001), and increased L-to-M asymmetry (2.3°± 1.3° vs 0.65°± 0.5°; P < .001) compared with controls. The MMPRT group (n = 600) had significantly increased MTS relative to controls (8.1°± 2.5° vs 4.3°± 0.7°; P < .001). Furthermore, there was a higher incidence of noncontact injuries (79.3%) and concomitant ramp lesions (56%) reported in patients with LMPRT.Conclusion:Increased MTS, LTS, and L-to-M slope asymmetry are associated with an increased risk of LMPRTs, while increased MTS is associated with MMPRTs. Surgeons should consider how proximal tibial anatomy increases the risk of meniscus root injury.

Anterior tibial subluxation measured under a modified protocol is positively correlated with posterior tibial slope: a comparative study of MRI measurement methods

Abstract:Purpose Anatomic factors, such as posterior tibial slope (PTS) and anterior tibial subluxation (ATS) obtained by quantitative measurement, have been proposed as predictors for clinical outcomes of anterior cruciate ligament (ACL) reconstruction. However, the correlation between PTS and ATS is controversial, and the method for quantitative ATS measurement remains unsettled. This study aimed to identify the correlation between PTS and ATS in patients with injured and intact ACLs and compare the two ATS measuring protocols. Methods This study included 128 ACL-injured and 176 ACL-intact patients with no concomitant ligament injuries. PTS and ATS were measured on sagittal MRI. ATS was measured using two measuring protocols, including the modified protocol using the longitudinal tibial axis (axis protocol) and the established protocol using a line perpendicular to the tibial plateau (plateau protocol). Correlation analyses between PTS and ATS and between PTS and the difference in the ATS value measured under the two protocols (ATS diff ) were performed. The difference between the two ATS measuring protocols was further analyzed by trigonometric analysis. Intra- and interobserver reliability tests were performed for the axis protocol. Results Under the axis protocol, ATS was positively correlated with PTS in both the ACL-injured and ACL-intact groups ( p < 0.001). Under the plateau protocol, no correlation was observed in the ACL-injured group. In the ACL-intact group, no correlation was observed for lateral ATS, and a negative correlation was observed for medial ATS ( p = 0.001). ATS diff was positively correlated with PTS ( p < 0.001), indicating that the two protocols varied greatly in those with a steep PTS. Trigonometric analysis showed that a steep PTS influenced the measurement of ATS under the plateau protocol but not the axis protocol. Intra- and interobserver reliability tests showed good-to-excellent strength of reliability for the ATS measurement under the axis protocol. Conclusion ATS measured under the axis protocol was positively correlated with PTS, indicating that a steep PTS was associated with a worse anatomic tibiofemoral relationship. The axis protocol for ATS measurement is a promising method for clinical use since it is not influenced by PTS and reflects the global position of the tibia. Level of evidence III.
Posterior Tibial Slope Measured on Plain Radiograph Versus MRI and Its Association With Revision Anterior Cruciate Ligament Reconstruction: A Matched Case-Control Study

Background:Posterior tibial slope (PTS) has been identified as a possible modifiable risk factor for anterior cruciate ligament (ACL) injury and ACL reconstruction (ACLR) failure. However, the findings in the literature are inconsistent. This may be explained by several different reasons, including different measurement methods, differing definitions of ACLR failure, and possible inclusion of suboptimal films.Purpose:To compare PTS values obtained using plain radiographs (XR-PTS) in a young (≤21 years of age), skeletally mature patient population with those obtained using magnetic resonance imaging (MRI), as well as to quantify the number of suboptimal lateral knee radiographs obtained across an integrated health care system and determine the potential effect of including these radiographs on summary statistics of XR-PTS.Study Design:Case-control study; Level of evidence, 3.Methods:Skeletally mature adolescent and young adult patients ≤21 years of age were identified from the ACLR registry of Kaiser Permanente. The cases of concern were patients requiring revision ACLR. The controls were patients who had an ACLR and did not require a revision procedure. The XR-PTS measurements were made on plain radiographs by a single blinded reviewer. These results were compared with measurements obtained using MRI. The quality of each plain radiograph was evaluated by measuring posterior/distal femoral condylar overlap and length of tibial diaphysis captured on the radiograph. Summary statistics with and without inclusion of measurements made on suboptimal radiographs were calculated.Results:Of the initial 634 patients with ACLR (317 case-control pairs), 561 (88.5%) had radiographs available and were included for the analysis comparing radiograph to MRI slope measurements. For the evaluation of slope between case and control pairs with radiograph information available, there were 257 case-control pairs; there were 124 pairs when those with suboptimal radiographs were excluded. There was no difference in MRI-measured lateral tibial posterior slope or medial tibial posterior slope for the 257 case-control pairs with XR-PTS information and for the 124 pairs with optimal radiographs. XR-PTS in the revision cohort was significantly steeper than in the control group when suboptimal radiographs were included in the analysis. There was no difference when patients with suboptimal radiographs were excluded. PTS measurements made on plain radiographs were larger than those made on MRI. There was a poor correlation between measurements made using these 2 modalities (r = 0.22 for radiograph and medial PTS).Conclusion:This study did not find a significantly steeper XR-PTS in patients who had to undergo revision ACLR when suboptimal radiographs were not included in the analysis. The present study's results confirmed the findings from a previous study of the same patient population that used MRI. However, there was poor correlation between PTS measurements made using plain radiograph and MRI.

Identification of Kinetic Abnormalities in Male Patients after Anterior Cruciate Ligament Deficiency Combined with Meniscal Injury: A Musculoskeletal Model Study of Lower Limbs During Jogging

Abstract:There is little known about kinetic changes in anterior cruciate ligament deficiency (ACLD) combined with meniscal tears during jogging. Therefore, 29 male patients with injured ACLs and 15 healthy male volunteers were recruited for this study to investigate kinetic abnormalities in male patients after ACL deficiency combined with a meniscal injury during jogging. Based on experimental data measured by an optical tracking system, a subject-specific musculoskeletal model was employed to estimate the tibiofemoral joint kinetics during jogging. Between-limb and interpatient differences were compared by the analysis of variance. The results showed that decreased knee joint forces and moments of both legs in ACLD patients were detected during the stance phase compared to the control group. Meanwhile, compared with ACLD knees, significantly fewer contact forces and flexion moments in ACLD combined with lateral and medial meniscal injury groups were found at the mid-stance, and ACLD with medial meniscal injury group showed a lower axial moment in the loading response (p < 0.05). In conclusion, ACLD knees exhibit reduced tibiofemoral joint forces and moments during jogging when compared with control knees. A combination of meniscus injuries in the ACLD-affected side exhibited abnormal kinetic alterations at the loading response and mid-stance phase.
Correlation Between Posterior Tibial Slope and the Gray Scale, Volume and Cross-sectional Area of Anterior Cruciate Ligament

Abstract:Objective To investigate the relationship between the posterior tibial slope（PTS） and the gray scale, volume and cross-sectional area of anterior cruciate ligament（ACL） in healthy people. Methods A total of 212 healthy subjects（including 106 males and 106 females）, aged from 30 to 60 years old（mean, 39.3±7.3 years old）, with BMI from 17.7 to 36.3（mean, 24.3±3.3）, recruited from January 2020 to August 2020 were selected to measure the lateral and medial tibial slope and the gray scale, volume and cross-sectional area of ACL through MRI images and 3D models. The measured results were compared between the male and female subjects, and the relationships between the PTS and the gray scale, volume and cross-sectional area of ACL were analyzed. Results The lateral tibial slope of females was greater than that of males [（8.0±3.8）° vs.（6.3±4.1）°, t=-3.068, P=0.002]. The ACL volume and cross-sectional area of females were smaller than those of males [（1339.4±458.4） mm~3 vs.（1699.6±468.6） mm~3, t=5.658, P=0.000;（36.4±13.0） mm~2 vs.（45.2±14.8） mm~2, t=4.614, P=0.000]. There was no statistical difference in medial tibial slope and ACL gray scale between males and females（P＞0.05）.In male and female subjects, there was no correlation between PTS and ACL gray scale, volume and cross-sectional area（P＞0.05）. Conclusion In healthy people, PTS has no correlation with the gray scale, volume or cross-sectional area of ACL. LTS in healthy women is greater than that in healthy men.
Asynchrony between subtalar and knee joint function during running

Abstract:PURPOSE: It has been suggested that during running proper coordination between subtalar joint pronation/supination and knee joint flexion/extension via tibial rotation is important to attenuate ground reaction impact forces (GRIF). Lack of coordination may produce over time a wide range of injuries. The goal of this study was to investigate the relationship between subtalar pronation/supination and knee flexion/extension with GRIF increases during distance running.METHODS: Eight subjects ran under different speeds (a self-selected pace, 10% faster, 10% slower, and 20% faster) and over different obstacle heights (5%, 10%, and 15% of their standing height) on their self-selected pace. Sagittal, rear-view kinematic, and GRIF data were collected. The biomechanical results were also compared with data from a clinical evaluation of the subjects.RESULTS: Speed changes and obstacle heights produced increases in GRIF and differences between rearfoot and knee angular velocities. The higher the obstacle and the faster the speed, the greater the GRIF and the greater the velocity differences. A change of the rearfoot angle curve from a unimodal (one minimum) to a bimodal (two minimums) parabolic configuration was also observed. The appearance of the second minimum was attributed to a lateral deviation of the tibia as a rebound effect due to the increased impact with the ground. The velocity differences between the actions of the subtalar and the knee joint, which in essence capture the antagonistic nature of their relationship, produced the highest correlation with the clinical evaluation.CONCLUSIONS: It was suggested that a possible mechanism responsible for various running injuries could be lack of coordination between subtalar and knee joint actions. This mechanism may have potential for predicting runners with susceptibility to injury.

ObjectivesThe posterior tibial slope (PTS) has been widely studied because of its relationship with tibial translation, knee stability and possible injuries related to the anterior cruciate ligament. But there is no consensus on the normal and pathological values for PTS and the best referral axes.MethodsIn this study, 96 people with non-orthopedic disorder were evaluated trough 3-D computed tomography (CT) scan. The TS of the medial and lateral tibial plateaus were measured using techniques adapted from previous protocols with three referral axes: the proximal tibial long axis, the anterior and posterior cortices. The CT scan were evaluated with two independent observers.ResultsThe averages and standard deviations of medial PTS (MPTS) in the three methods were 10.4 ± 2,8, 8,3 ± 2,8 and 5,9 ± 2,8, separately. The results of lateral PTS (LPTS) were 9,9 ± 3,3, 7,8 ± 3,1 and 5,7 ± 3,1. There was no significant difference between the male and the female. All the referral axes had high interobserver reliability, with greater value (0.829) for posterior cortice axe for medial TS and anterior cortice for lateral TS (0.861).ConclusionThe mean value for posterior tibial slope in a small Brazilian sample is similar to other ethnicity described in the literature. Tibial slope measurements have a high degree of variability between different measurement methods, but all referral axes had a high interobserver reliability.

Is Genome Complexity a Consequence of Inefficient Selection? Evidence from Intron Creation in Nonrecombining Regions.

Abstract:Genomes show remarkable variation in architecture and complexity across organisms, with large differences in genome size and in numbers of genes, gene duplicates, introns and transposable elements. These differences have important implications for transcriptome and regulatory complexity and ultimately for organismal complexity. Numbers of spliceosomal introns show particularly striking differences, ranging across organisms from zero to hundreds of thousands of introns per genome. The causes of these differences remain poorly understood. According to one influential perspective, differences across species reflect the differential ability of selection in different populations to eliminate allegedly deleterious intron-containing alleles. Direct tests of this theory have been elusive. Here, I study evolution of intron-exon structures in genomic regions of recombination suppression (RRSs), which experience drastically reduced selective efficiency due to hitchhiking and background selection. I studied intron creation in eight independently evolved RRSs, spanning substantial diversity phylogenetically (plants, animals, fungi and brown algae) and biologically (sex chromosomes, mating type chromosomes, genomic regions flanking self-incompatibility loci, and the Drosophila "dot" chromosome). To identify newly created introns in RRSs, I compared intron positions in RRS genes with those in homologous genes. I found very few intron gains: no intron gains were observed in 7/8 studied data sets, and only three intron gains were observed overall (on the Drosophila dot chromosome). These results suggest that efficiency of selection may not be a major cause of differences in intron-exon structures across organisms. Instead, rates of spontaneous intron-creating and intron-deleting mutations may play the central role in shaping intron-exon structures.
Ankle and hindfoot motion of healthy adults during running revealed by dynamic biplane radiography: Side-to-side symmetry, sex-specific differences, and comparison with walking

Abstract:This study aimed to characterize ankle and hindfoot kinematics of healthy men and women during overground running using biplane radiography, and to compare these data to those previously obtained in the same cohort during overground walking. Participants ran across an elevated platform at a self-selected pace while synchronized biplane radiographs of their ankle and hindfoot were acquired. Motion of the tibia, talus, and calcaneus was tracked using a validated volumetric model-based tracking process. Tibiotalar and subtalar 6DOF kinematics were obtained. Absolute side-to-side differences in ROM and kinematics waveforms were calculated. Side-to-side and sex-specific differences were evaluated at 10 % increments of stance phase with mixed model analysis. Pearson correlation coefficients were used to assess the relationship between stance-phase running and walking kinematics. 20 participants comprised the study cohort (10 men, mean age 30.8 ± 6.3 years, mean BMI 24.1 ± 3.1). Average absolute side-to-side differences in running kinematics waveforms were 5.6°/2.0 mm or less at the tibiotalar joint and 5.2°/3.2 mm or less at the subtalar joint. No differences in running kinematics waveforms between sides or between men and women were detected. Correlations were stronger at the tibiotalar joint (42/66 [64 %] of correlations were p < 0.05), than at the tibiotalar joint (38/66 [58 %] of correlations were p < 0.05). These results provide a normative reference for evaluating native ankle and hindfoot kinematics which may be informative in surgical or rehabilitation contexts. Sex-specific differences in ankle kinematics during overground running are likely not clinically or etiologically significant. Associations seen between walking and running kinematics suggest one could be used to predict the other.

Abstract:OBJECTIVE:To retrospectively explore the correlation between anterior cruciate ligament (ACL)-ruptured knees, stability of ACL-rupture knee and posterior tibial slope (PTS).METHODS:From January 2008 to October 2012, 150 knees with ACL rupture underwent arthroscopic surgery for ACL reconstruction. A control group was established for subjects undergoing arthroscopic surgery without ACL rupture during the same period. PTS was measured on a digitalized lateral radiograph. Lachman and mechanized pivot shift tests were performed for assessing the stability of knee.RESULTS:There was significant difference (P = 0.007) in PTS angle between the patients with ACL rupture (9.5 ± 2.2 degrees) and the control group (6.6 ± 1.8 degrees). Only among females, increased slope of tibial plateau had effect on the Lachman test. There was a higher positive rate of pivot shift test in patients of increased posterior slope in the ACL rupture group.CONCLUSION:Increased posterior tibial slope (>6.6) appears to contribute to non-contact ACL injuries in females. And the changes of tibial slope have no effect upon the Lachman test. However, large changes in tibial slope affect pivot shift.
Ankle and subtalar joint axes of rotation and center of rotation during walking and running in healthy individuals measured using dynamic biplane radiography

Abstract:The goal of this study was to determine how foot type and activity level affect ankle and hindfoot motion. Dynamic biplane radiography and a validated volumetric registration process was used to measure ankle and hindfoot motion of 20 healthy adults during walking and running. The helical axes of motion (HAM) during stance were calculated at the tibiotalar and subtalar joints. The intersection of each HAM and the rotation plane of interest defined the tibiotalar and subtalar centers of rotation (COR). Correlations between foot type and hindfoot kinematics were calculated using Pearson's correlations. The effect of activity, phase of gait, and dominant vs. non-dominant limb on HAM and COR were evaluated using linear mixed effects models. Activity and phase of gait influenced the superior location of the tibiotalar (p < 0.041) and subtalar (p < 0.044) CORs. Activity and gait phase affected tibiotalar (p < 0.049) and subtalar (p < 0.044) HAM direction during gait. Both HAM orientation and COR location changed with activity and phase of gait. These ankle and hindfoot kinematics have implications for total ankle replacement design and musculoskeletal models that estimate force and moment generating capabilities of muscles.

Abstract:Purpose Multifaceted factors contribute to inferior outcomes following anterior cruciate ligament (ACL) reconstruction surgery. A particular focus is placed on the posterior tibial slope (PTS). This study introduces the integration of machine learning and artificial intelligence (AI) for efficient measurements of tibial slopes on magnetic resonance imaging images as a promising solution. This advancement aims to enhance risk stratification, diagnostic insights, intervention prognosis and surgical planning for ACL injuries. Methods Images and demographic information from 120 patients who underwent ACL reconstruction surgery were used for this study. An AI‐driven model was developed to measure the posterior lateral tibial slope using the YOLOv8 algorithm. The accuracy of the lateral tibial slope, medial tibial slope and tibial longitudinal axis measurements was assessed, and the results reached high levels of reliability. This study employed machine learning and AI techniques to provide objective, consistent and efficient measurements of tibial slopes on MR images. Results Three distinct models were developed to derive AI‐based measurements. The study results revealed a substantial correlation between the measurements obtained from the AI models and those obtained by the orthopaedic surgeon across three parameters: lateral tibial slope, medial tibial slope and tibial longitudinal axis. Specifically, the Pearson correlation coefficients were 0.673, 0.850 and 0.839, respectively. The Spearman rank correlation coefficients were 0.736, 0.861 and 0.738, respectively. Additionally, the interclass correlation coefficients were 0.63, 0.84 and 0.84, respectively. Conclusion This study establishes that the deep learning‐based method for measuring posterior tibial slopes strongly correlates with the evaluations of expert orthopaedic surgeons. The time efficiency and consistency of this technique suggest its utility in clinical practice, promising to enhance workflow, risk assessment and the customization of patient treatment plans. Measurement of posterior tibial slope and its significance in total knee arthroplasty

Abstract:Posterior tibial slope (PTS) is an important parameter in total knee arthroplasty (TKA). Clinicians can use X-ray, CT and MRI to measure PTS. When using X-ray, clinicians need to select a suitable reference axis. The available reference axes include: tibial proximal anatomic axis, tibial shaft anatomic axis, fibular shaft axis, anterior tibial cortex, posterior tibial cortex. Different reference axes have their own advantages and disadvantages. The X-ray measurement of PTS is affected by the projection angle, and cannot measure the lateral posterior tibial slope and the medial posterior tibial slopeseparately. CT and MRI can measure PTS by using 3D reconstruction techniques, which is not influenced by the projection angle and can measure the lateral and medial tibial slope separately. MRI can also measure PTS containing cartilage and meniscus. In TKA, PTS can affect the range of motion, stability and ligaments’ tension of the knee through a variety of mechanisms. Increasing of PTS can effectively increase postoperative flexion through the roll-back mechanism and relaxation of the ligament. However, increasing of PTS will also cause relaxation of the ligament and anterior impingement of the tibial prosthesis, which results in instability of the knee and wear of the prosthesis. Furthermore, increasing of PTS can significantly reduce the tension of the posterior cruciate ligament, but increase the risk of posterior cruciate ligament injury due to osteotomy. In addition, increasing of PTS can reduce the tension of the quadriceps tendon in extension by increasing the torque. It is worth noting that clinical affects of PTS is influenced by different osteotomy references and the femoral condyle. At present, the researches on PTS are mostly based on European population. The measurement and clinical significance of PTS in Asian population need further exploration.
Posterior Tibial Slope Measurements of the Medial and Lateral Plateaus Vary Widely Between Magnetic Resonance Imaging And Computed Tomography

Abstract:Purpose: This study aims to compare posterior tibial slope (PTS) measurements of the medial tibial plateau (MTP) and lateral tibial plateau (LTP) on magnetic resonance imaging (MRI) versus computed tomography (CT) to determine the agreement of measurement between imaging modalities. Methods: Patients aged 15-65 years with concurrent MRI and CT imaging were initially included. Knees with significant arthrosis (Kellgren-Lawrence grade >2), proximal tibia fracture, or artifact obscuring visualization were excluded. Two independent raters measured PTS of the MTP and LTP on paired MRI and CT. Interrater and intrarater reliability were assessed using the intraclass correlation coefficient (ICC). Intermethod agreement was assessed using ICC and Bland-Altman analyses. An acceptable Bland-Altman limit of agreement (LOA) was set at ±2°, requiring 95% of measurement differences between imaging modalities to fall between ±2° for an acceptable level of agreement. Results: 46 knees in 45 patients met final inclusion criteria. Interrater reliability was good for MRI (ICC 0.78-0.83) and moderate-to-good for CT (ICC 0.64-0.80) studies. Intrarater reliability was moderate-to-excellent (ICC 0.64-0.94). Intermethod agreement between MRI and CT was poor at the MTP (ICC 0.34-0.42) and moderate at the LTP (ICC 0.59-0.70). Bland-Altman analysis demonstrated high variability of PTS measurements between MRI and CT: 0.16° (95% LOA -6.10-6.41°) for MTP for Rater 1; 0.22° (95% LOA -5.01-5.45°) for LTP for Rater 1; -0.95° (95% LOA -7.22-5.33°) for MTP for Rater 2; -0.99° (95% LOA -6.48-4.85°) for LTP for Rater 2, with only 47.83 to 60.87% of measurement differences falling within the predetermined acceptable LOA of ±2°. Conclusion: Although the interrater and intrarater reliability was moderate-to-excellent, the degree of agreement between PTS measurements on MRI and CT was highly variable at both medial and lateral plateaus. Although some variability may have been due to the study's limitations, PTS measurements at individual plateaus may not be interchangeable between MRI and CT. Background:An elevated posterior tibial slope (PTS) is associated with an increased risk for anterior cruciate ligament and meniscal injury. Recent evidence suggests that the PTS is elevated in patients with Osgood-Schlatter disease.Purpose:To determine whether there is an association between objective measures of anterior tibial tubercle growth and PTS.Study Design:Cross-sectional study; Level of evidence, 3.Methods:A total of 100 radiographs were randomly selected from a sample of patients who had received a lateral knee radiograph that captured at least 15 cm of the tibia distal to the knee joint line at a single institution between December 2020 and March 2022. The PTS was measured, and tibial tubercle growth was quantified with 2 novel measurements. For these measurements, a line was drawn on the radiograph from the most anterosuperior point on the tibia to the point on the anterior cortex of the tibia 10 cm distal from the starting point. The tibial tubercle height (TTH) was measured as the perpendicular distance from this line to the most prominent portion of the anterior tibia. The anterior tibial tubercle angle (TTA) was measured as the angle between the endpoints of the line made previously and the most prominent portion of the tibial tubercle, with a more acute angle indicating a more prominent tibial tubercle. The relationship between TTA, TTH, and PTS was evaluated using a univariate linear regression model.Results:The mean patient age was 33.1 ± 14.1 years. The mean TTA was 158.6°± 4.7°, the mean TTH was 8.8 ± 2.0 mm, and the mean PTS was 9.7°± 2.6°. A significant correlation was found between PTS and TTA (r = −0.46; β = −0.46; P < .001) as well as TTH (r = 0.43; β = 0.43; P < .001).Conclusion:Objective measures of anterior tibial tubercle overgrowth correlated with an elevated PTS. Every 2.2° of anterior TTA deviation from the mean and every 2.3 mm in TTH deviation from the mean correlated with a 1° difference in the PTS. This suggests a link between the development of the tibial tubercle and PTS, and it potentially helps to explain why the PTS is elevated in certain patients.

Background:The posterior tibial slope (PTS) is an important feature in knee joint biomechanics and indicates anterior-posterior knee stability. Increased PTS is a known risk factor for both primary anterior cruciate ligament (ACL) rupture and postreconstruction rerupture.Purpose:To investigate the effect of long-term exposure to ACL deficiency on the PTS and the sagittal anatomy of the proximal tibia.Study Design:Cross-sectional study; Level of evidence, 3.Methods:A total of 44 patients (38 men, 6 women) with a history of knee injury and ACL rupture confirmed by magnetic resonance imaging and physical examination were included in this study. Patients were divided into those with chronic ACL deficiency (group 1: injured ≥5 years prior; n = 22) and acute ACL deficiency (group 2: injured <1 year prior; n = 22). The medial and lateral tibial plateau PTS and anterior tibial translation were measured on monopodal weightbearing knee radiographs at 20° of flexion. The mechanical tibiofemoral angle (MTFA) and the medial proximal tibial angle (MPTA) were measured using an orthoroentgenogram. The side-to-side difference between the affected and unaffected knees was also calculated for all measurements.Results:The mean duration of exposure to ACL deficiency was 7.6 years (range, 5-15 years) in group 1 and 4.4 months (range, 1-11 months) in group 2. Regarding the side-to-side differences in angular measurements, a higher medial PTS (affected vs unaffected: 12.4° vs 10.1°; P = .007), higher lateral PTS (11° vs 8.9°; P = .011), and increased varus alignment on both the MTFA (4.3° vs 2.4°; P = .036) and the MPTA (84.9° vs 86.3°; P = .033) were found in group 1, while no significant differences in angular measurements were found in group 2. Compared with group 2, patients in group 1 had a significantly higher side-to-side difference in the medial PTS (2.3° vs 0.1°; P = .0001), lateral PTS (2.1° vs 0.4°; P = .0001), and MPTA (1.4° vs 0.1°; P = .002).Conclusion:This study showed that the affected knees of patients with chronic ACL deficiency (≥5 years) had higher medial and lateral PTS compared with the unaffected contralateral knees. Therefore, when planning ACL reconstruction for patients with a history of long-term ACL deficiency, it is crucial to measure the preoperative PTS accurately.
