Wearable-Based Integrated System for In-Home Monitoring and Analysis of Nocturnal Enuresis

Abstract:Nocturnal enuresis (NE) is involuntary bedwetting during sleep, typically appearing in young children. Despite the potential benefits of the long-term home monitoring of NE patients for research and treatment enhancement, this area remains underexplored. To address this, we propose NEcare, an in-home monitoring system that utilizes wearable devices and machine learning techniques. NEcare collects sensor data from an electrocardiogram, body impedance (BI), a three-axis accelerometer, and a three-axis gyroscope to examine bladder volume (BV), heart rate (HR), and periodic limb movements in sleep (PLMS). Additionally, it analyzes the collected NE patient data and supports NE moment estimation using heuristic rules and deep learning techniques. To demonstrate the feasibility of in-home monitoring for NE patients using our wearable system, we used our datasets from 30 in-hospital patients and 4 in-home patients. The results show that NEcare captures expected trends associated with NE occurrences, including BV increase, HR increase, and PLMS appearance. In addition, we studied the machine learning-based NE moment estimation, which could help relieve the burdens of NE patients and their families. Finally, we address the limitations and outline future research directions for the development of wearable systems for NE patients

What problem does this paper attempt to address?
The problem that this paper attempts to solve is the insufficient long - term home monitoring of patients with nocturnal enuresis (NE). Although nocturnal enuresis is relatively common in children and home monitoring has potential benefits for the improvement of research and treatment, this area has not been fully explored yet. To solve this problem, the author proposes a home - monitoring system NEcare based on wearable devices. This system utilizes machine - learning techniques to collect and analyze patients' data, aiming to predict the occurrence time of nocturnal enuresis by monitoring indicators such as bladder volume (BV), heart rate (HR) and periodic limb movements in sleep (PLMS), thereby reducing the burden on patients and their families and improving the effectiveness of traditional treatment methods. Specifically, the NEcare system addresses two main challenges in the following ways: 1. **Robustness of data collection**: In view of the data collection obstacles encountered in the preliminary system, such as inaccurate or missing data caused by patients' vigorous exercise or the wearable device being blocked by the body, NEcare has developed a data collection supervisor and an NE - moment detection algorithm to improve the reliability and accuracy of data collection. 2. **NE - moment prediction**: In order to predict the NE - moment, NEcare adopts heuristic - rule - based and deep - learning - based methods and designs NE - moment estimation algorithms. These algorithms can analyze the data of NE patients according to the characteristics of event - driven and sequential distribution, so as to predict the possibility of NE occurrence more accurately. In addition, NEcare also introduces new bioimpedance (BI) features for effectively estimating changes in bladder volume, which is a significant improvement over using the original BI signal. Through these innovations, NEcare not only improves the quality of data collection but also provides new tools and technical support for the management of NE patients.
