Abstract:Purpose: The chromatin protein HMGB2 plays important roles in development, remodeling and disruption of the articular cartilage. The objective of present study was to investigate whether the expression of HMGB2 in the condylar cartilage was correlated to the continuing forward mandibular positioning (FMP), and explore its potential relationship with β-catenin pathway. Methods: Fourty-four healthy adult Japanese white rabbits were used in this experiment. The right tempromandibular joints (TMJ) of rabbits were subjected to surgical and sham surgical operation of anterior disc displacement (ADD) (N=8). The TMJ condyles of rabbits were collected and fixed at 1-, 2-, 4-, and 8- weeks after the surgery. Histological micrographs of the condyle cartilage were mearsured by HE staining. The distribution and expression of HMGB2, Col-II, MMP1, MMP13 and β-catenin were evaluated through immunohistochemical staining. Results: Histological results by HE staining showed that the condyle cartilage turned thinner as well as disordered cell arrangement in cartilage with ADD. The expression of HMGB2 chiefly distributed in fibrous layer and proliferative layer of normal condylar cartilage. After ADD treatment, expression of HMGB2 and β-catenin enhanced at the 1 week ADD, and subsequently decreased at 2 and 4 weeks. COL-II and MMP13 expression also increased at the 1–2 weeks after ADD, suggesting osteoarthritis may occur in condylar cartilage. Most of the cartilages were repaired and chondrocytes were arranged in order at 8 weeks, suggesting spontaneous remodeling of condylar cartilage after external force loading. Conclusions: These findings suggest that HMGB2 may play important roles in chondrocytes following exposure to mechanical loading, which may involve in the pathogenesis of osteoarthritis.
Signs and Symptoms of Temporomandibular Dysfunction and Radiographic Condylar Morphology in Patients with Idiopathic Condylar Resorption

Abstract:Background: Little is known about the clinical characteristics of idiopathic condylar resorption (ICR). The aim of this study was to examine the signs and symptoms of temporomandibular dysfunction (TMD) and evaluate the morphological characteristics of the condyles in patients with ICR. Methods: Sixty patients with ICR (41 in the bilateral ICR group and 19 in the unilateral ICR group) and forty-one healthy controls were examined. Signs and symptoms of TMD were described, and three-dimensional models of the condyles were measured and analyzed. Results: In total, 81.7% of ICR patients had self-reported symptoms and 78.3% of ICR patients had objective-found signs. The anteroposterior diameter, transverse diameter, height, maximal sectional area, volume of the condyles, axial angle, and the distance from the posterior point of the condyle to the Saggittal standard line were significantly smaller in the ICR condyles compared with the controls (p < 0.05). The condylar neck angle was significantly larger in the ICR condyles compared with the controls (p < 0.05). Conclusions: Most patients with ICR had signs and symptoms of TMD. The prevalence of clicking and opening-closing deviation was significantly different between the bilateral and the unilateral ICR groups. In patients with ICR, the size of the condyles decreased significantly; the condyles also rotated inward, moved forward, and inclined posteriorly.

Abstract:The investigation of lichens on soil, plant debris and terricolous mosses in the glacier forefield of the Rötkees yielded 31 lichen taxa (29 species and 2 varieties) and one lichenicolous fungus. Micarea incrassata Hedl. (Lecanorales) is new to Italy. Three sampling sites were established at increasing distance from the glacier, in order to compare species diversity, abundance and composition.

Abstract:Condylar resorption occurs in some cases after orthognathic surgery, and the risk factors associated with postoperative condylar head resorption have been extensively described. Nevertheless, even in cases with a combination of risk factors, postoperative condylar resorption may not appear. This study analyzed the microstructure and three-dimensional positional change of the condylar bone via imaging in patients who have undergone bimaxillary orthognathic surgery to determine whether the microstructure or condylar position differs between patients with and without postoperative condylar resorption. Among asymptomatic patients who underwent bimaxillary surgery between April 2021 and March 2022 at our department, 17 patients were analyzed, limited to "female," "skeletal Class II," and "high-angle cases," which are known risk factors for mandibular head resorption. Multidetector computed tomography was performed on these patients before and 6 months after surgery, and the bone microstructure of the condylar head and the three-dimensional positional changes of the condylar bone and the proximal bony fragments were compared with the presence of postoperative condyle resorption using the bone morphology software TRI/3D-BON. Patients with condylar bone abnormalities before surgery and those with high trabecular bone density can develop postoperative resorption if the condyle is misaligned by surgery.

Abstract:Objective:To investigate the dynamic expression of Sox9 and MMP12 in rabbits' condylar chondrocyte of tem-poromandibular joint internal derangement ( TMJID). Methods:60 Japanese white rabbits were randomly divided into five groups (control group, 1-week group, 2-week group, 4-week group, and 8-week group;n=12), which were sacrificed respec-tively after modeling TMJID at one week, 2 weeks, 4 weeks and 8 weeks respectively, then take out of condylar and made tissue chips from joints. DNA microarray technology, quantitative rtPCR, immunohistochemical technique and image pro-cessing were used to observe the expression of Sox9 and MMP12 in condylar chondrocyte among different groups. Results:The results of genetic testing and rtPCR were basically consistent, which showed that MMP12 expression reached the high-est level in the first week, then decreased with the modeling time going and reached the lowest level in the eighth week (decreased expression). Sox9 expression was low in the first week, then rose slightly, especially it rose significantly in the second, fourth, and eighth week. Conclusion:In TMJID, MMP12 is mainly involved in the early osteolysis destruction and Sox9 mainly involved in the late cartilage repair and reconstruction.
Mandibular stability and condylar changes following orthognathic surgery in mandibular hypoplasia patients associated with preoperative condylar resorption

Abstract:Objectives: To evaluate postoperative mandibular stability and condylar changes in patients with mandibular hypoplasia and preoperative condylar resorption (CR) undergoing orthognathic surgery. Materials and methods: Fifty-four patients were included in this retrospective study. Computed tomography (CT) scans were acquired preoperatively (T0), 2-7 days immediate postoperatively (T1), and at least 1 year postoperatively (T2). Three-dimensional (3D) cephalometric analysis and measurements of condylar angle, volume, and position (joint spaces) were performed. A 2-mm mandibular relapse was deemed clinically acceptable. We also analyzed the correlations between relapse and postoperative CR and susceptible factors using a multivariate logistic regression model. Results: The results showed one year after the surgery, the average mandibular relapse was 1.0 mm (p < 0.05), and the average reduction of condylar volume was 152.4 mm3 (12.7%). Condyle-fossa relationships were improved immediately after the surgery, with a tendency of returning to their original state in the follow-up (p < 0.05). Anteroposterior advancement at point B (B-CP advancement) at T1 and superior joint space (SJS) at T0 were significantly correlated with mandibular relapse, and postoperative CR was mainly associated with vertical increasement at point B (B-AP increasement) at T1. The optimal cut-off values were as follows: 1.6 mm for SJS, 4.2 mm for B-CP advancement, and 1.8 mm for B-AP increasement. Concomitant advancement Genioplasty showed no significant correlation with relapse and postoperative CR. Conclusions: While patients with mandibular hypoplasia and preoperative CR were vulnerable to further condylar resorption after mandibular advancement, the treatment outcomes were generally clinically acceptable. Postoperative relapse was associated with a larger than 4.2 mm of mandibular advancement measured at B-CP and a larger than 1.6 mm of superior joint space measured at SJS, and postoperative CR was associated with a larger than 1.8 mm of mandibular vertical increasement measured at B-AP. Clinical relevance: The findings of this study suggested that the mandibular advancement might be limited to 5 mm for patients with preoperative CR. A concomitant advancement genioplasty might also be considered to achieve a better facial profile in these patients.

Abstract:Objective: To study the expression of ADAMTs-5 and TIMP-3 in temporomandibular joint osteoarthritis (TMJOA) model rats, to explore and confer the possible effects of ADAMT5-5 and TIMP-3 involved in the degradation of the early stage of OA.Design: 32 SD rats were divided into four groups: 2-week control group (NC1), 2-week OA group (OA1), 4-week control group (NC2) and 4-week OA group (OA2). Each group had 8 rats. Injection of collagenase was used to build up the TMJOA model. HE staining was used to analyze the structural change of condyle cartilage. Western blot and RT-PCR were used to measure the expression of ADAMTs-5 and TIMP-3 in protein and mRNA levels respectively.Results: HE analysis revealed that no significant changes were observed in NC1, NC2 and OA1 groups, while mild damages appeared in OA2 group. No significant differences were achieved in the expression of ADAMTs-5 in protein levels between NC1 and OA1, but the expression of ADAMTs-5 in 4-week group increased significantly compared to that in the NC2 group. On mRNA level, the expression of ADAMTs-5 in 2-week and 4-week OA groups increased significantly compared to that in the matched control group. Meanwhile, the expression of TIMP-3 decreased significantly, showing a completely different trend.Conclusions: The expression of ADAMTs-5 and TIMP-3 changed significantly in the early stage of TMJOA, which indicated that ADAMTs-5 and TIMP-3 may be play an important part in the initial stage of condylar cartilage degradation. Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>Condylar head fractures (CHFs) are increasingly treated by open reduction and internal fixation (ORIF). However, there are no reports on the three-dimensional postoperative volumetric transformation of the condyle, especially with regard to fragmented cases. Protruding hardware can lead to severe complications, so the goal of this study was to examine the amount of condylar bony resorption occurring after ORIF.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Included were surgically treated CHFs with eligible CBCT datasets immediately after ORIF (T1) and after implant removal (T2), plus fractures of the condylar neck and base as a reference. 2D vertical and 3D volume changes of the condylar head after ORIF of CHFs were evaluated by cone beam computer tomography (CBCT) datasets transformed into 3D models for 3D volumetric assessment using Slicer freeware.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Among a total of 50 fractures (38 patients), including 41 CHFs (ORIF with titanium positional screws, including 15 minor and 12 major fragmented cases) plus nine extracapsular fractures (eight upper neck and one base fracture), postoperative condylar volume decreased by a mean of 0.27 cm³ (median 0.25 cm³; SD 0.23 cm³) or 16% (median 14%; SD 11%). Major fragmented CHFs showed significantly higher resorption rates (<em>p</em> < 0.001, range 8–42%). Age correlated with a decrease in condylar volume (mean 0.024 per 10 years of age, (p=0,011). No significant correlation could be established between loss of condylar volume and fracture localization. A protrusion of metallic implants was seen in 20% of the cases.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Due to substantial volume changes of the condylar head occurring after ORIF, protrusion of implants (both metallic and resorbable) needs to be taken into consideration. An early removal of metallic ostheosyntesis material (around 4 months after ORIF) is strongly recommended to avoid adverse short- and long-term effects.</p>

Abstract:The aim of this investigation was to study the remodelling of cartilage in the mandibular condyle following disc displacement (DD) of the temporomandibular joint (TMJ). Forty adult Japanese white rabbits were used in this study. The right joints of 28 of the 40 rabbits had their discs surgically displaced. Four of the 28 were killed at 4 days or 1, 2, 4, 6, 8 and12 weeks after surgery. The messenger RNA (mRNA) expression levels of aggrecan and type II collagen in cartilages were measured using in situ hybridization techniques. Results showed that aggrecan mRNA expression reduced in the first week after DD. The expression began to recover after 4 weeks and reached a normal level after 6 weeks. Type II collagen mRNA expression reduced from 4 weeks and the expression recovered after 8 weeks. This suggests that the chondrocyte reacting to the displacement of the TMJ disc, alters its matrix gene expression patterns and it is may be the cause of the shape changes of TMJ after DD.

Abstract:ObjectiveAnterior disk displacement (ADD) is a most common subtype of temporomandibular disorders (TMD), which is promoted by chondrocytes apoptosis. However, the signaling pathways that trigger apoptosis are still unknown. The aim of this study was to investigate the expression of endoplasmic reticulum (ER) stress-related proteins in the condylar cartilage of rabbits following ADD. MethodsSixty healthy adult rabbits were randomly assigned to the experimental and sham-operated control groups (n=12). The experimental rabbits were subjected to surgical ADD in the right temporomandibular joints. The production of ER stress-related proteins C/EBP homologous protein (CHOP), glucose-regulated protein 78 (GRP78), cleaved caspase-3, and caspase-12 in cartilage was evaluated by immunohistochemistry, quantitative real-time PCR, and Western blot analysis. ResultsOur results showed that the expression of CHOP, GRP78, cleaved caspase-3, and caspase-12 increased significantly along with degenerative changes in cartilage after ADD. ConclusionThese results indicate that the ER stress pathway is activated in ADD cartilage and might promote the development of TMD.
Unilateral Severe Condylar Resorption Following Internal Fixation in Bilateral Mandibular Condylar Fracture

Abstract:Study Design Case Report Objective: A severe condylar resorption following internal fixation of the mandibular condylar fracture is rarely reported. Methods A 35-year-old female caused bilateral condylar base fracture due to fall. She had no connective tissue autoimmune disease, no temporomandibular joint (TMJ) problems, and no history of the orthodontic treatment. Each fractures of condyles were fixed by 2 none-locking titanium miniplates on ideal lines of the osteosynthesis. Results: 6 months after surgery, although patient showed no malocclusion and no TMJ pain, panoramic X-ray showed left side condylar head resorption. Twelve months after surgery, bilateral plate removal was performed. Twenty-six months after internal fixation, panoramic X-ray showed bone formation at the left side of the condylar head. Magnetic resonance imaging of the TMJ showed left TMJ disc displacement without reduction and severe deformity of the left condylar head. Conclusions The condylar resorption might be caused by increasing functional loading due to new position of the condyle following osteosynthesis. Possible contributors for condylar resorption following osteosynthesis were: (1) internal fixation in bilateral condylar fracture, (2) disc displacement without reduction, and (3) female patient (hormonally-mediated theory, eg estrogen). However, the mechanize of condylar resorption was unclear and the further studies were necessary.

Abstract:The aim of this study was to investigate the characteristics of condylar resorption in craniofacial microsomia (CFM) patients following mandibular distraction osteogenesis (MDO). Patients with unilateral type-IIa and type-IIb CFM, who had completed MDO and mandibular distractor extraction (MDE), were recruited. The height and volume of the condyle were measured on three-dimension models created by the analysis of computed tomography (CT) data. Normality analysis was performed using the Shapiro-Wilk test. Data for the affected and unaffected sides were compared using the paired t -test or Wilcoxon signed-rank test. Data for both type-IIa and type-IIb CFM were compared using the independent-samples t -test or Mann-Whitney U test. The Pearson or Spearman correlation was used to determine the correlations of condylar resorption rate with related measurements. In total, 48 type-IIa and 48 type-IIb CFM patients were included. The condylar resorption rate in type-IIa CFM (0.35 ± 0.32) was significantly associated with the height of the condyle ( r = 0.776, p < 0.001) and distraction distance ( r = 0.447, p = 0.001), while the condylar resorption rate in type-IIb CFM (0.49 ± 0.46) was significantly associated with the height of the condyle ( r = 0.924, p < 0.001). However, there was no significant difference in condylar resorption rate between type-IIa and type-IIb CFM ( p = 0.075). In addition to occlusal changes, no other negative symptoms of the TMJ were observed with condylar resorption. Condylar resorption was evident in CFM patients following mandibular distraction osteogenesis, and the condylar resorption rate showed a relationship with distraction distance and condylar height.

Abstract:Apoptosis is a potential contributor to anterior disk displacement (ADD) in temporomandibular joint disorders (TMD). The aim of this study was to investigate the expression of key protein regulators involved in apoptosis in the chondrocytes of mandibular condyles with induced ADD in experimental animals. ADD was surgically induced in the left temporomandibular joint (TMJ) of 15 rabbits without opening their bursas. A sham operation was performed on the right TMJ without displacing the disk forward. At the end of one, two, and four weeks after surgery, mandibular condyle samples were collected for protein extraction. The production of apoptosis-associated proteins Fas, capase-8, Bcl-2, and Bax was determined using Western blotting. The results indicate that the production of Fas and caspase-8 increased continuously after ADD. Meanwhile, the level of Bcl-2 decreased, and the production of Bax gradually increased following ADD. These results indicate that alterations in protein production of Fas, caspase-8, and the ratio of Bcl-2 to Bax are consistent with an increase of apoptotic activity in the chondrocytes, which may eventually lead to TMD.
Condylar Remodeling Accompanying Splint Therapy: A Cone-Beam Computerized Tomography Study Of Patients With Temporomandibular Joint Disk Displacement

Abstract:Objective. The aim of this study was to evaluate osseous changes accompanying anterior repositioning splint (ARS) therapy in patients with temporomandibular joint disk displacement.Study Design. Cone-beam computerized tomography (CBCT) data of 36 patients with intermittent or permanent closed-lock were used; 23 patients with permanent closed-lock had their displaced disks physically reduced by mandibular manipulation before ARS therapy. CBCT was performed before and similar to 6 months after ARS therapy. The presence and location of "double contour" images suggesting condylar bone remodeling were statistically analyzed.Results. The "double contour" images after ARS therapy were observed in similar to 80% of patients, more frequently in joints with signs of displaced disks. The "double contour" appeared more often on the posterior bevel as well as the medial and middle part of condyles (P < .01).Conclusions. ARS therapy can facilitate regenerative remodeling of condyles. CBCT is a useful tool for monitoring osseous changes in condyles. (Oral Surg Oral Med Oral Pathol Oral Radiol 2012;114:259-265)
Changes of Temporomandibular Joint and Semaphorin 4D/Plexin-B1 Expression in a Mouse Model of Incisor Malocclusion

Abstract:AIMS: To investigate the changes in condylar cartilage and subchondral bone of the temporomandibular joint (TMJ) in a mouse model of incisor malocclusion.METHODS: By bonding a single (single group) or a pair (pair group) of metal tube(s) to the left incisor(s), a crossbite-like relationship was created between left-side incisors in mice. The morphological changes in the TMJ condyles were examined by hematoxylin and eosin and toluidine blue staining. Indices of osteoclastic activity, including tartrate-resistant acid phosphatase (TRAP) staining and macrophage colony stimulating factor (M-CSF) were investigated by histochemistry or real-time polymerase chain reaction (PCR). The osteoblastic activity was indexed by osteocalcin expression. Expressions of semaphorin 4D and its receptor, Plexin-B1, were detected by real-time PCR. Two-way analysis of variance was used to assess the differences between groups.RESULTS: One week and 3 weeks after bonding the metal tube(s), cartilage degradation and subchondral bone loss were evident histologically. Both indices of osteoclastic activity (TRAP and M-CSF) were significantly increased in cartilage and subchondral bone after bonding the metal tube(s). Osteocalcin expression in cartilage was significantly increased at week 3, while its expression in subchondral bone was significantly increased at week 1 but decreased at week 3. The semaphorin 4D expression in cartilage and subchondral bone was significantly decreased at week 1 but significantly increased at week 3. For Plexin-B1 expression, a significant increase was detected in subchondral bone at week 3.CONCLUSION: Bonding a single or a pair of metal tube(s) to left incisor(s) is capable of inducing remodeling in the TMJ, which involved cartilage degradation and alteration of osteoclastic and osteoblastic activity.

Abstract:OBJECTIVE:To study the chondrocyte apoptosis in rabbit temporomandibular joint after anterior disc displacement.METHODS:Experimental anterior disc displacement was induced surgically in 20 Japanese rabbits without opening their temporomandibular joint bursas. Histopathologic and apoptotic (TUNEL) analysis was used to evaluate the changes in articular cartilage, disc and synovium.RESULTS:Condyle chondrocyte showed apoptosis most obviously at 1 or 2 weeks after surgery, and apoptotic cells concentrated in proliferative zone and hypertrophic zone. 4 or 6 weeks after surgery, the joint went into a remodelling period.CONCLUSIONS:Chondrocyte apoptosis in temporomandibular joint will be activated after anterior disc displacement, which initiates the remodelling in temporomandibular joint.

Abstract:PURPOSE:To investigate the change of osteoprotegerin (OPG) and OPGL expression and its significance in condylar cartilage after anterior disc displacement (ADD).METHODS:Twenty healthy adult Japanese rabbits (from medical animal center of Zhejiang University) weighing from 2 to 3 kg were used in this study. Sixteen rabbits were subjected to surgical anterior disc displacement in the right temporomandibular joints (TMJ), four rabbits were sacrificed 1, 2, 4, 8 weeks post-operation,respectively; two rabbits with sham-operation and two without operation were as control. The structure of temporomandibular joint was observed under microscope. The expression of OPG and OPGL in condylar cartilage were examined by S-P immunohistochemical staining and measured with Image Computer Analysis Software. Date was statistically analyzed by one-way ANOVA.RESULTS:Experimental and sham-operative animals tolerated the surgical procedures well. The discs displaced anteriorly were observed in the surgical rabbits by general and microscope observation. The expression of OPG and OPGL was detected in the proliferating zone and hypertrophy zone cells of condyle in all animals. However, the expression of OPG and OPGL was stronger in the surgical group. There was significant difference in average gray degree (P<0.05) between the experimental and control groups. The ratio of OPG/OPGL was stable.CONCLUSIONS:The expression of OPG and OPGL in condylar cartilage was increased after ADD. They might play a role in the development and prognosis of the temporomandibular condylar cartilage alteration after ADD.

Abstract:Purpose: This study evaluated the impact of mandibular advancement on Sdf1 and Foxc1 gene expression in the mandibular condylar cartilage of young Wistar rats. By examining the changes that occur during a unique one-month recovery period, it highlights the critical role of gene expression and condylar adaptation during the recovery phase. The analysis focused on whether, during the recovery period, reversal changes occur when functional appliances are removed and whether genetic expression important for condyle growth and adaptation downregulates. Material and methods: The study involved 30 male Wistar rats divided into 2 control groups Appliance Control and Recovery Control groups, and 2 experimental groups, the Appliance group with mandibular advancement bite-jumping appliance for 30 days, and the Recovery group with appliance for 30 days followed by a 30-day recovery. Molecular analysis of condylar cartilage using real-time RT-PCR and histological assessments was conducted. Results: Significant genetic expression alterations were noted in both the experimental groups for Sdf1 (p < 0.05) and Foxc1 (p < 0.05). According to histological investigations, significant alterations with an increase in the proliferative and hypertrophic layer in condylar cartilage were seen. Conclusion: Mandibular advancement bite-jumping appliances induce proliferative and hypertrophic layer changes in mandibular condylar cartilage, shown by elevated Foxc1 levels and decreased Sdf1 levels. Post-appliance removal, persistent gene expression reveals a true joint stimulation.
