Role of Insulin Resistance in Patients of Acne Vulgaris and Hirsutism in the Western Part of India- A Cross-Sectional Study

Abstract:Background: Despite being common and easy to treat conditions, acne and hirsutism may have a striking consequence on one's perception of self. Though understanding the pathogenesis of acne and hirsutism has undergone a major paradigm shift, one of them being insulin resistance (IR), extensive research is still underway. The purpose of the study is to establish causal relationship linking IR with acne vulgaris and hirsutism, as well as to assess severity of the two with increasing IR. Materials and methods: A cross-sectional observational study was conducted in the dermatology department. Patient's detailed clinical history and necessary parameters like height, weight, body mass index (BMI), etc., were recorded in predefined proforma, noting signs and symptoms of underlying IR, along with acne severity calculated by global acne grading system (GAGS) and hirsutism severity by modified Ferriman Gallwey score (mFG), then analyzing using Pearson's correlation test to establish correlation between IR and acne with/without hirsutism and hirsutism alone. Results: A total of 349 patients included in our study were divided into three groups, (i) Acne-only (N = 308) group showed statistically significant correlation of IR with acne severity (P < 0.0001) and recurrence (P < 0.0001). (ii) In hirsutism-only (N = 24) group, homeostatic model assessment for insulin resistance (HOMA-IR) was strongly associated with modified Ferriman-Gallwey score and recurrence suggesting raised insulin resistance with increased severity of hirsutism (P < 0.0001) and recurrence (P < 0.007). (iii) In acne + hirsutism (N = 17) group, raised BMI was found to be positively associated with IR but not with severity of both conditions. Conclusion: IR has emerged as an important contributory, if not, causative event in pathogenesis of severe, resistant acne, and hirsutism. These conditions have great deal of impact on the psychological health of those affected necessitating different approach to manage such cases.

Abstract:Objective: The study's goals are to identify a causal connection between insulin resistance (IR) and hirsutism and acne vulgaris, as well as to evaluate the severity of the two conditions in relation to rising IR. Study design: An analytical cross-sectional study Place and Duration: This study was conducted at Primary Health Care Corporation, Qatar from August 2021 to August 2022. Methodology: In a predetermined proforma, a patient's entire clinical history as well as required variables, such as height, weight, body mass index (BMI), etc., were recorded in order to establish a link between IR and acne with/without hirsutism and only hirsutism.A HOMA-IR value of more than 2.5 was regarded as important and indicative of IR. Global Acne Grading System (GAGS) was used to determine the degree of acne, and the Modified Ferriman Gallwey Score was used to determine the severity of hirsutism (mFG). Results: The 354 participants in our study were divided into three groups: There was a statistically significant connection between IR and the severity and recurrence of acne in the Acne-only (N = 310) group. The homeostatic model assessment for insulin resistance (HOMA-IR), the modified Ferriman-Gallwey score, and recurrence were substantially correlated in the hirsutism-only (N = 26) group, demonstrating that insulin resistance increased with hirsutism severity and recurrence. In the sample of 18 people with acne and hirsutism (N = 18), higher BMI was positively linked with IR but not with the severity of both disorders. Conclusion: Causing serious, resistant acne and hirsutism, IR has come to be recognized as a significant contributory, if not causal, event. The mental stress affected by these conditions is greatly impacted, requiring a different approach to case management. Abstract:Previous studies have revealed a close connection between acne and insulin resistance (IR). However, studies on the prevalence and characteristics of female acne patients with IR were limited. In the present study, A total of 524 female acne patients and 50 healthy controls were consecutively recruited. Detailed examinations were conducted on all the subjects. The clinical severity of acne was determined by the Global Acne Grading System. Homeostasis model assessment of insulin resistance (HOMA-IR) values was calculated as: fasting glucose (mmol/L)×fasting insulin (mIU/L)/22.5. A HOMA-IR value greater than 2.5 was predefined as IR.

Abstract:Insulin Resistance Associated Acne alludes to a subset of acne vulgaris that is driven by exaggerated insulin/IGF-1 signalling. It has a distinctive clinical profile defined by attendant companion morphologies, notably, acanthosis nigricans and benign cutaneous hyperplasias. It has comorbidities derived from hormonal disturbances (PCOS), immune dysfunction, hypovitaminoses, and features of evolving metabolic syndrome. It begins earlier than acne vulgaris, commonly as preadolescent acne, and can be predicted to persist as adult acne, even beyond menopause. It is a spectral entity with complex pathogenesis with mTORC1 overactivity as the most important aberration. It demands additional therapeutic interventions such as insulin-sensitizing agents and mTORC1 inhibitors.

Abstract:Acne vulgaris is a chronic inflammatory disease with high incidence, diverse clinical manifestations, poor clinical efficacy, and easy recurrence. Recent studies have found that the occurrence of acne is related to metabolic factors such as insulin resistance; however, the specific mechanism of action remains unclear. This study aimed to identify significantly different metabolites and related metabolic pathways in the serum of acne vulgaris patients with or without insulin resistance. LC–MS/MS was used to analyze serum samples from patients about acne with insulin resistance ( n = 51) and acne without insulin resistance ( n = 69) to identify significant metabolites and metabolic pathways. In this study, 18 significant differential metabolites were screened for the first time. In the positive-ion mode, the upregulated substances were creatine, sarcosine, D -proline, uracil, Phe–Phe, L -pipecolic acid, and DL -phenylalanine; the downregulated substances were tridecanoic acid (tridecylic acid), L -lysine, cyclohexylamine, sphingomyelin (d18:1/18:0), gamma- L -Glu-epsilon- L -Lys, and 1,2-dioleoyl-sn-glycero-3-phosphatidylcholine. In the negative-ion mode, the upregulated substance was cholesterol sulfate, and the downregulated substances were D (-)-beta-hydroxybutyric acid, myristic acid, D -galacturonic acid, and dihydrothymine. Cholesterol sulfate showed the most significant expression among all differential metabolites (VIP = 7.3411). Based on the KEGG database, necroptosis and ABC transporters were the most significantly enriched metabolic pathways in this experiment. The differential metabolites and pathways identified in this study may provide new possibilities for the clinical diagnosis and development of targeted drugs for acne patients with insulin resistance.

Abstract:BACKGROUND: Acne vulgaris is a multifactorial disorder of the pilosebaceous units. Several studies have reported that insulin-like growth factor (IGF)-1, forkhead box transcription factor (Fox)O1 and mammalian target of rapamycin (mTOR) interactions may be the key to understanding the links between genetic and environmental factors in acne vulgaris.OBJECTIVES: To evaluate the immunohistochemical detection of mTOR and FoxO1 in the skin, and the serum level of IGF-1 in patients with acne vulgaris.METHODS: This study was carried out on 60 participants, including 40 patients with acne and 20 controls. A diet questionnaire was administered to the patients and controls. Serum levels of IGF-1 were measured using enzyme-linked immunosorbent assay, and skin biopsies were taken from lesions on the backs of the patients and controls. FoxO1 and mTOR expression was detected using immunohistochemistry.RESULTS: A significantly higher serum IGF-1 level was found in the patients with acne than in the controls. The cytoplasmic expression of FoxO1 was found to be significantly greater in the acne group, whereas in the control subjects this expression was likely to be nuclear. Both the cytoplasmic expression and the nuclear expression of mTOR were significantly more intense in the patients with acne than in the controls. Excess consumption of a high-glycaemic-load diet was significantly associated with higher serum levels of IGF-1 and cytoplasmic expression of FoxO1 and mTOR.CONCLUSIONS: These results suggest that FoxO1, mTOR, serum IGF-1 and a high-glycaemic-load diet may play a role in acne pathogenesis.

Abstract:Background and Objectives: Acne is a common chronic inflammatory skin condition; Acne could present as inflammatory or non-inflammatory lesion. Acne can be varied in severity from patient to another, generally, it can present in mild, moderate or severe degree. Excessive production of sebum by sebaceous glands in the skin contributes in acne formation. One of the factors associated with the production of sebum is Insulin-Like Growth Factor in serum. Current study tries to find any association between acne severity and serum Insulin-Like Growth Factor. Method: On a sample of 40 female patients with acne, aged 14-26 years at the outpatient clinic as a cross sectional study. Serum IGF-1 level was compared to their acne severity across three groups, mild, moderate and severe acne. Global acne grading system was used to classify the acne severity. Results: Median levels of IGF-1 were 208.09 ng/ml. Mean serum level of IGF-1 is 254.53ng/ml, 200.03ng/ml and 172.39ng/ml in severe, moderate and mild acne respectively. Acne severity strongly associated with serum level of IGF-1 which is statistically significant. No significantly association between serum level of Insulin-Like Growth Factor and the age of patients was found. Likewise, the severity degree of acne has no association with body mass index Conclusion: There is a direct relationship between mean level of IGF-1 in patients with AV with degree of their acne severity

Insulin Resistance in Healthy U.S. Adults: Findings from the National Health and Nutrition Examination Survey (NHANES)

Abstract:Background: Insulin is fundamental in two conditions that are epidemic in the United States and globally: obesity and type II diabetes. Given insulin's established mechanistic involvement in energy balance and glucose tolerance, we examined its relationship to common health-related endpoints in a large population-based sample. Methods: The National Health and Nutrition Examination Survey is a cross-sectional study that uses a complex multistage probability design to obtain a representative sample of the United States population. Adult participants were included from 8 successive 2-year data waves (1999-2014), including 9,224 normal individuals, 7,699 prediabetic, and 3,413 diabetic subjects. The homeostatic model for insulin resistance (HOMA-IR) was available for 20,336 participants and its relationship with demographic, anthropometric, and clinical data was analyzed. We examined the relationship of HOMA-IR to 8 groups of outcome variables: general health, anthropometric/metabolic [waist size, body mass index (BMI)], cardiovascular (blood pressure), lipid [triglycerides, high-density lipoprotein (HDL)], hepatic [alanine aminotransferase (ALT), gamma-glutamyl transferase (GGT)], hematologic [white blood cells (WBC), hemoglobin (Hgb), platelets], inflammatory (C-reactive protein), and nutritional (vitamins D and C, serum folate, and pyridoxine) variables. Results: HOMA-IR was generally strongly, monotonically, and highly significantly associated with adjusted outcomes in normal subjects, although clinical laboratory values were generally within normal bounds across insulin quartiles. In the normal subset, the odds ratio and 95% confidence interval for a quartile change in HOMA-IR for obesity (BMI > 30) was 3.62 (3.30-3.97), and for the highest quintile for the triglyceride/HDL the ratio was 2.00 (1.77-2.26), for GGT it was 1.40 (1.24-1.58), and for WBC it was 1.28 (1.16-1.40). The relationship of HOMA-IR to the various outcomes was broadly similar to that observed in prediabetics and diabetics with a few exceptions. Conclusions: HOMA-IR levels in a large sample of normal individuals are associated with poorer general health and adverse changes across a wide range of markers. A similar pattern of alterations is observed in prediabetic and diabetic samples. Impact: Clinically, checking insulin levels may be helpful to identify patients that merit further observation and are candidates for early interventions.
Risk Assessment and Association with Acne Severity

Abstract:Metabolic preconditioning, characterized by conditions like obesity and insulin resistance syndrome, disrupts hormonal balance. Elevated androgen levels stimulate excessive sebum production and follicular cell proliferation, leading to acne lesions. Similarly, thyroid hormone imbalances affect sebaceous gland activity, epidermal lipid composition, and skin cell turnover, impacting acne occurrence and severity. This study aimed to assess the potential contribution of metabolic and endocrine preconditions to acne development. A total of 389 patients diagnosed with acne were included and divided into three groups: the metabolic precondition group (MPG, N = 163, 41.9%), the endocrine precondition group (EPG, N = 162, 41.65%), and the control group (CG, N = 89, 22.88%). Data related to the degree of acne severity and comorbidities of interest were collected from the patients' medical records. In the groups with concomitant diseases, moderate and severe acne were significantly more prevalent (56.44% and 41.10% in MPG, and 35.80% and 61.11% in EPG) compared to the control group (5.61% and 4.89%). The most prevalent preconditions observed were insulin resistance syndrome in MPG (63.8%) and autoimmune thyroiditis in EPG (95.06%). Significant age-related differences in acne severity were found across all study groups (p < 0.05). In MPG, the age variable was significantly higher in the presence of mild acne, while in EPG, the age variable was significantly lower for the mild acne group. A positive association was observed between the severity of acne and insulin resistance syndrome, obesity, autoimmune thyroiditis, and hypothyroidism (p 1, 95% CI RR > 1, p < 0.001) of developing moderate and severe acne in the presence of these preconditions. The presence of both metabolic and endocrine preconditions significantly increased the likelihood of developing severe acne, leading to the hypothesis that both conditions may be contributing factors to the development of acne.

Relationship between Plasma Phospholipid Fatty Acid Profile and Insulin Resistance in Type 2 Diabetics

Abstract:Objective To investigate the relationship between plasma phospholipid fatty acid profile and insulin resistance(IR) in patients with type 2 diabetes(T2DM).Methods Totally 74 patients with T2DM from coastal areas of Zhejiang province were enrolled.Plasma phospholipid fatty acids were analyzed with capillary gas chromatography.Homeostasis model assessment(HOMA-IR) was applied to assess the status of insulin resistance(IR).Plasma lipids were measured by enzymatic assay.Results(1) Bivariate correlation analysis showed fasting plasma glucose(FPG) was negatively correlated with C20:2n-6,HbA1c was negatively correlated with C20:2n-6 and C20:3n-6;C-peptide was positively correlated with C14:0,C16:0,saturated fatty acid(SFA),waist hip ratio(WHR),triglyceride(TG),and negatively correlated with C23:0,polyunsaturated fatty acid(PUFA),high-density lipoprotein cholesterol(HDL-C).Fasting insulin(FINS) was positively correlated with C16:0,n-6/n-3,Body mass index(BMI),WHR and TG,and negatively correlated with C22:6n-3,n-3PUFA and HDL-C.HOMA-IR was positively correlated with C16:0,BMI,total cholesterol(TC),TG and Low-density lipoprotein cholesterol(LDL-C),and negatively correlated with HDL-C.(2) In the multiple regression analysis(Stepwise regression),the HOMA-IR was predicated significantly only by C16:0,BMI and TC;and the general correlation coefficient was 0.716.Conclusion In patients with T2DM the insulin sensitivity is modulated by C16:0,BMI and TC;Increasing plasma phospholipid n-3 PUFA proportion and reducing C16:0 proportion may improve the insulin sensitivity.

Abstract:Backgrounds/objectives: Acne is an inflammatory disease of the pilosebaceous unit (PSU). The over-expression of survivin and insulin-like growth factor (IGF)-I in some fibrotic disorders suggests a possible implication in the pathogenesis of acne and or post-acne scar. We aimed to evaluate their potential role in pathogenesis in acne and post-acne scar. Methods: Serum survivin and IGF-I levels were estimated in 30 patients with acne and post-acne scar compared to 30 controls. Results: There was a statistically significant difference in survivin and IGF-I levels between controls and patients (P < 0.05). However, there was no linear correlation between survivin and IGF-I. Conclusions: Survivin and IGF-I could have a possible role in the pathogenesis of active acne and in post-inflammatory acne scar.

Abstract:Acne vulgaris is a common disease, which occurs in adolescents as well as adults and has a significant influence on the patient's quality of life (QoL) in every aspect. Due to resistance to standard therapies, it has become necessary to prospect for new treatment strategies. It is important to highlight that the diagnosis and treatment of the underlying cause of acne such as metabolic and hormonal disorders may significantly improve the effectiveness of acne treatment. The correlation between Insulin Resistance (IR) and acne has been proven. Both disorders share many common occurrence factors and activation pathways. Metformin, an antihyperglycemic agent, seems to be a possible therapy option, not only because of its insulin sensitizing ability but also via plenty of additional effects of this medicine. While the efficiency of metformin therapy in patients with acne and Polycystic Ovary Syndrome (PCOS) is well explored, it is still necessary to evaluate it in patients without any endocrinopathies. This meta-analysis aimed to estimate the effectiveness of oral metformin as a monotherapy in acne patients without PCOS or other endocrinopathies. Study selection was performed with included criteria such as no PCOS and other endocrinopathies diagnosed, oral administration of metformin, and metformin in monotherapy. Selected studies contained comparisons in the Global Acne Grading System (GAGS) before and after metformin therapy. Statistical analysis detected significant improvement in skin condition after treatment with metformin.

Abstract:Objective: To investigate the relationship of inflammation and endothelial activation with insulin resistance and adiposity in type 2 diabetes. Methods and results: Hundred and thirty-four (45 female) type 2 diabetic subjects aged 50-75 in the Fenofibrate Intervention and Event Lowering in Diabetes Study in Helsinki were examined before fenofibrate intervention. Fasting levels of circulating intercellular cell adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1) (vascular cell adhesion molecule), ultra-sensitive C-reactive protein (CRP), human serum amyloid A (hSAA), interleukin-6 (IL-6), macrophage colony-stimulating factor (M-CSF), secretory phospholipase A(2) IIA (PLA(2)), total, HDL and LDL cholesterol, triglycerides, P-glucose, HbA1c, and serum free insulin were determined. Insulin resistance was assessed by the homeostasis model. HOMA IR correlated significantly with all measures of adiposity and markers of inflammation and endothelial dysfunction. BMI was significantly associated with inflammation and endothelial activation, but with neither lipoproteins nor glycaemic control. After controlling for age, gender and BMI, HbA1c correlated significantly with CRP, hSAA, ICAM-1, E-selectin, and HOMA IR. HDL cholesterol correlated inversely with IL-6, M-CSF, E-selectin, and HOMA IR. HbA1c, phospholipase A(2), VCAM-1, and HDL cholesterol remained independent determinants of HOMA IR in the linear regression analysis controlled for age, gender, and BMI. Conclusion: Endothelial activation and acute-phase reaction correlate with insulin resistance and obesity in type 2 diabetic patients.
The effects of acitretin on insulin resistance, glucose metabolism, and lipid levels in patients with psoriasis

Abstract:Context: Acitretin increases serum lipids. Data on its effects on insulin resistance and glucose metabolism are sparse and contradicting. Aims: The aim of this study is to investigate the effects of acitretin on insulin resistance, glucose metabolism, and lipids. Methods: Dermatology clinic in a public tertiary hospital. A cross sectional study involving chronic plaques psoriasis patients on acitretin plus topical therapy or topical therapy alone was performed. Fasting blood glucose (FBG), serum lipids, serum insulin, and glucose tolerance test (GTT) were performed. Homeostatic model of insulin resistance (HOMA-IR) was calculated. Psoriasis severity was evaluated using Psoriasis Area and Severity Index. Chi square and t-tests determined differences between cases and controls. Pearson's correlation coefficient test determined the relationship between continuous variables. Results: A total of 60 patients participated, 30 were on acitretin while 30 were on topical therapy. Psoriasis duration, disease severity, BMI, presence of metabolic syndrome, and other comorbidities between the two groups were similar. There were no significant differences in GTT, FBG, HOMA-IR, and serum lipids. Patients on acitretin >25 mg daily had lower FBG [4.4 (0.8) versus 4.9 (0.9), P = 0.04] and triglyceride [1.05 (0.33) versus 1.57 (1.03), P = 0.02] compared with doses ≤25 mg. Higher acitretin dose correlated with lower FBG (r = −0.36, P = 0.05) and triglycerides (r = −0.37, P = 0.05) while longer therapy duration correlated with lower total cholesterol (r = −0.37, P = 0.05). HOMA-IR showed inverse correlation with acitretin dose and duration (r = −0.10, P = 0.61 and r = −0.12, P = 0.53, respectively). Conclusion: Acitretin therapy resulted in increased triglyceride. The effect of acitretin on glucose metabolism and insulin resistance maybe dependent on the dose and duration of therapy.

However, data on body composition in patients with acne vulgaris are scarce. Aim: We aimed to assess body composition in patients with acne vulgaris, compare the results with those of healthy individuals, and evaluate the relationship between body composition and acne severity. Methods: Between March 2023 and May 2023, body composition of patients with acne vulgaris and healthy individuals was prospectively evaluated using Tanita TBF-300 body composition analyzer. Results: This study included a total of 320 subjects, 160 patients with acne vulgaris and 160 healthy individuals. Body mass index (BMI), body fat percentage and fat mass were significantly higher in patients with acne vulgaris compared to healthy individuals (p < 0.001, each). BMI, body fat percentage and fat mass were significantly higher in female patients compared to healthy females (p < 0.001, each), higher in male patients compared to healthy males (p = 0.001, p < 0.001, p < 0.001, respectively). BMI (p = 0.006), fat free body mass (p < 0.001) and total body water (p < 0.001) were higher in male patients, while body fat percentage (p < 0.001) was higher in female patients. Patients with moderate to severe acne had higher BMI (p < 0.001), body fat percentage (p = 0.001) and fat mass (p < 0.001) than in those with mild acne. Conclusion: Since high body fat may indicate increased risk and severity of acne, body composition analysis may be useful in treating patients with acne and taking preventive measures against metabolic syndrome in this population.

Abstract:Background Psoriasis patients are at increased risk of developing metabolic diseases. Proinflammatory cytokines such as IFN-γ, IL-17, IL-23, and TNF-α,IGF that are increased in psoriasis play an important role in the development of diabetes mellitus(DM), hypertension, dyslipidemia, obesity, insulin resistance and their complications. Aims To study prevalence of insulin resistance (IR) using HOMA-IR index in psoriasis patients and its association with severity of psoriasis. To study prevalence of metabolic diseases, macro and microvascular complications of these diseases in psoriasis. Materials and methods A hospital based case control study was conducted involving 102 psoriasis patients and 102 age and sex matched controls. All patients were analysed for the presence of metabolic diseases and their complications. Results Mean age of cases was 38.41±16.37 years. Majority of cases(58.8%) as well as controls (55.9%) were males. Prevalence of hypertension, prediabetes, DM, raised TG, low HDL, MetS and IR was 46.1%, 28.4%, 27.5%, 42.2%, 31.4%, 31.4% and 48% respectively in cases as compared to 26.5%, 13.7%, 11.8%, 24.3%, 16.7%, 10.8% and 26.5% respectively in controls. However there was no significant difference in obesity (29.4% vs 21.6%, p=0.2024) and LDL among the cases and controls(14.7 %vs 10.8%,p= 0.4). Prevalence of all the metabolic disorders except LDL was significantly higher in cases as compared to that in controls. Prominent complications noted were retinopathy, neuropathy and CVD. With increased psoriasis severity insulin resistance increased. Conclusion There is a positive correlation of psoriasis with IR and it is associated with increased risk of metabolic diseases and their complications.

Role of Certain Diet and Body Mass Index on Severity of Acne

Abstract:Introduction: Acne vulgaris (AV) is one of the most prevalent chronic dermatological conditions among adolescents. Recent data indicated that a certain diet may influence the development and appearance of acne. Materials and methods: This was a cross sectional study conducted in the outpatient department of Erbil dermatology teaching center from May 2022 to December 2022. All the newly diagnosed patients with acne were included in the study. Height and weight of the patients was recorded and BMI was calculated. BMI was categorized as underweight, normal weight, overweight and obese. Type of lesions was noted and severity of acne was graded from 1 to 4. Result: In this study, 27 patients (27%) were male and 73 patients (73%) were female. The highest rate of AV was seen in age group 16-20years and the lowest rate was observed in age group ≥21 years. Moderate to severe acne was strongly associated with a family history of acne in first-degree relatives (P= 0.0141). There was a significant association between Acne treatment received (<0.0007) and smoking habit (<0.0001) with severity of acne. The risk increased with increased red meat consumption (P= 0.0486). Majority of patients (62%) had normal BMI. Most of the patients had Grade 1 acne (20%). There was no significant association between the BMI and severity of acne (p= 0.3094) Conclusion: BMI are not risk factors for severity of AV, but a family history of acne is. Abstract:Background: Although acne is usually recognized as an adolescent skin disorder, the prevalence of adults with acne is increasing. The clinical and epidemiological data of adult acne were evaluated with a view to establishing possible contributing etiological factors and observing whether clinical features differ from adolescent acne. Methods: A total of 170 patients over the age of 25 years with acne were taken up for the study and a detailed history and clinical examination was done. Hormonal profile was done in all the patients. The data was statistically evaluated and inferences drawn. Results: Out of 170 patients included in the study 87.6% were women. The mean age of the patients was 33.5 years. Persistent acne was observed in 62.4%, while it was late onset in 37.6%. Most common predominant site of involvement was cheek (78%), followed by chin (62%), and mandibular area (33.2%). Family history of acne was present in 15.3%. Premenstrual flare was seen in 31.2% of female patients, obesity in 25.9%, hirsutism in 41.8% and menstrual abnormalities in 15.1%, but raised laboratory markers of hyperandrogenism were observed in only 4.02%. Conclusions: The study identified key contributing factors and patterns of post adolescent acne, providing valuable insights for tailored management strategies.

Abstract:BACKGROUND: Elevation of C-reactive protein (CRP) has been reported to occur in psoriasis, urticaria, acne, rosacea and many other dermatological and nondermatological conditions. Chronic systemic inflammation has been implicated in the development of neuropsychiatric/degenerative disorders, atherosclerosis, coronary artery disease, diabetes mellitus and even carcinogenesis. The present study is designed to determine whether the level of inflammation created by acne vulgaris could be high enough to raise the serum levels of high-sensitive CRP.MATERIALS AND METHODS: Forty-two patients with moderate and severe acne vulgaris were enrolled, along with 44 age and sex matched healthy blood donors as controls. Hypersensitive-CRP (Hs-CRP) was measured in both groups.RESULTS: Hypersensitive-C-reactive protein levels in the case group varied between 0 and 28.1 μg/ml with an average of 2.24 ± 4.87 μg/ml (mean ± standard deviation) and a median of 0.6 μg/ml (interquartile range [IQR] =0.3, 1.4 μg/ml). Hs-CRP levels of the control group varied between 0 and 14 μg/ml with an average of 3.12 ± 3.67 μg/ml and a median of 1.5 μg/ml (IQR = 0.55, 5.0 μg/ml). No significant difference of Hs-CRP level between the two groups was seen (t = -0.961, 95% confidence interval: Lower = -2.6942, upper = 0.9377; P = 0.339). Additionally, no significant difference in the level of Hs-CRP was noted between the moderate and severe acne groups (95% confidence interval: Lower = -5.2495, upper = 1.6711; P = 0.165).CONCLUSION: Acne vulgaris, even in its severe grades (excluding acne fulminans and acne conglobata), does not induce significant inflammation at the systemic level.

Abstract:Acne vulgaris is a widespread a chronic inflammatory dermatosis that affects millions of people around the world, which has a significant influence on patients' standard of living. The progression of this dermatosis results in the appearance of inflammatory and non-inflammatory changes, and, in severe cases, disfiguring scars and hyperpigmentation. The aetiopathogenesis of acne is complex. It involves a complex interaction of many different factors, both endo- and exogenous in their effect on the hair and sebaceous unit. Genetic predisposition, hormones, the skin and gut microbiome, psychological stress, air pollutants, aggressive facial products, and certain medications are cited as factors influencing acne formation. The link between nutrition and acne is extensively debated for many years and is still relatively controversial. Diet is commonly recognised to have a direct relationship with certain biochemical markers and the transcription of genes related to sebaceous gland function, and the proliferation of bacteria and inflammation that encourage the progression of the disease. In this review, the authors take a closer look at the existing scientific reports on the involvement of nutrition in the development of acne vulgaris.
