Abstract:Background & Aims Diabetes mellitus is known to negatively impact the outcome of liver transplant; however, data are scarce regarding risk of waitlist dropout and tumour recurrence in patients with hepatocellular carcinoma. We examined the impact of diabetes mellitus on the outcomes of candidates with hepatocellular carcinoma on an intention-to-treat basis. Methods Our study included 15 776 candidates with hepatocellular carcinoma diagnosis on the Scientific Registry of Transplant Recipients database from 2008 to 2015 to evaluate the risk of waitlist dropout, hepatocellular carcinoma recurrence and overall survival. Results There were more patients dropped out from the waiting list owing to patient disease deterioration or tumour progression in the diabetes mellitus group (15.1% vs 13.7%, P = 0.024). The mean waiting time was similar in the two groups (233 days vs 230 days, P = 0.631). The recurrence rate was higher in the diabetes mellitus group (9.0% vs 6.2%, P < 0.001); however, the mean time to recurrence in the two groups was similar (23.7 months vs 22.6 months, P = 0.371). Diabetes mellitus, tumours exceeding Milan criteria, and AFP >400 ng/mL were independent predictive factors for recurrence. On an intention-to-treat basis, diabetes mellitus was also an independent poor prognostic factor for overall survival; however, the overall survival was comparable with tumours beyond Milan criteria. Conclusions Diabetes mellitus was associated with poor survival outcomes and an increased risk of waitlist dropout and tumour recurrence rates in hepatocellular carcinoma patients. Those patients should be paid more attention to cardiovascular and oncological examination when determining waitlist and post-transplant surveillance strategies.
The effect of diabetes mellitus on the post-transplant survival rate of liver transplant recipients with different grades of acute-on-chronic liver failure

Abstract:Objective:To investigate the effect of diabetes mellitus on the survival rate of liver transplant recipients with different grades of acute-on-chronic liver failure (ACLF) after liver transplantation.Methods:A retrospective study was carried out to investigate 1, 144 recipients with ACLF who underwent liver transplantation from January 1, 2017 to December 31, 2017 in the Scientific Registry of Transplant Recipients of American. According to whether they had diabetes before transplantation, the recipients were divided into two groups: non-diabetic group (n=927) and diabetic group (n=217). The survival rate was calculated by the Kaplan-Meier method and the survival curve was drawn, and then the log-rank test was used to compare the differences between two groups. The Cox proportional-hazards model was used to analyze the independent prognostic factors. A two-tailed P value <0.05 was considered statistically significant.Results:There were statistically significant differences in recipients′ age, body mass index (BMI), underlying liver disease, model for end-stage liver disease score, ACLF grade and postoperative hospital stay between diabetic and non-diabetic group (U/χ2=-6.290, -3.592, 76.451, -2.959, 8.150 and-2.542, all P <0.05). In terms of the donor characteristics, there was significant statistical difference in ABO blood type between the two groups (χ2=8.463, P<0.05). The proportion of recipients died from multiple organ failure after liver transplantation in the non-diabetic group (1.54%) was significantly lower than that in the diabetic group (5.16%) (χ2=8.863, P<0.05). There were no significant differences in terms of graft failure, cardio-cerebrovascular disease, hemorrhage, infection, malignant tumor and other diseases between the two groups (all P>0.05). The 1-, 2- and 3-year survival rates of ACLF1 recipients in the non-diabetic group were 93.1%, 90.5% and 88.7% by September 1, 2021, while those in the diabetic group were 93.8%, 92.2% and 92.2%, which had no statistically significance (χ2=1.021, P>0.05). The corresponding survival rates of ACLF2 recipients in non-diabetic group were 93.6%, 89.8% and 88.4% respectively, while those in diabetic group were 96.8%, 95.7% and 90.3%, which had no statistically significance (χ2=0.850, P>0.05). The corresponding survival rates of ACLF3 recipients in non-diabetic group were 91.5%, 89.2% and 87.5%, significantly better than those in diabetic group, which were 80.0%, 71.7% and 71.7% respectively (χ2=11.444, P<0.05). Multivariate analysis showed that recipient diabetes, ethnicity (African) and BMI were independent predictors of survival after liver transplantation in ACLF3 recipients (HR=2.31, 2.13 and 1.04, all P<0.05).Conclusions:Diabetes can significantly reduce the survival rates of ACLF3 recipients after liver transplantation, but it has no significant effect on ACLF1 and ACLF2 recipients. It is important to assess the diabetes status in ACLF3 recipients in the recipient selection process, in order to achieve better post-transplant survival.
Effect of Donor Diabetes on Prognosis of Liver Transplantation in Patients with Acute-Chronic Liver Failure

Abstract:Objective To investigate the influence of donor diabetes on the overall prognosis of patients with acute-chronic liver failure（ACLF） after liver transplantation. Methods A retrospective study was conducted in 839 ACLF patients who received liver transplantation from January 1,2013 to December 31,2013 in Scientific Registry of Transplant Recipients（SRTR） database. Then, the recipients were divided into two groups according to whether they used diabetic donors（93 cases were in DM group, 746 cases were in non-DM group）. Donor and recipient baseline characteristics were compared between two groups, and overall survival rates were compared according to different recipient ACLF grades. Univariate and multivariable analysis were further used. Results The overall survival rates were comparable in patients with ACLF1 and ACLF2. However, in patients with ACLF3, the 1, 3, 5-year overall survival rates were 84.3%, 78.3%, 72.4% in the non-DM group,it was significantly better than that in the DM group, which were 70.4%, 55.6%, 51.9%, respectively（P = 0.016）. It showed statistically significant differences in ACLF3. In multivariate analysis, the age of the recipient（P = 0.001） and the diabetes of the donor（P = 0.007） were independent predictors for post-transplant overall survival for patients with ACLF3. Conclusion In patients with ACLF3, donor diabetes is an independent predictor for inferior overall survival after liver transplantation.
The interaction of T2DM and BMI with NASH in recipients of liver transplants: an SRTR database analysis

Abstract:BackgroundNASH-related liver transplants are increasing because of the obesity epidemic, but the influence of T2DM on various levels of BMI among NASH recipients is unclear.Research design and methodsWe analyzed data retrieved from SRTR on 4,515 patients. We divided patients by BMI into five groups: normal weight; overweight; class 1 obesity; class 2 obesity; and class 3 obesity. Statistical analysis was done.ResultsPatients in the NASH group with T2DM had a lower patient and graft survival than patients without T2DM (5-year patient and graft survival: 77.5% vs. 79.8%; P = 0.001 and 76.4% vs. 78.2%; P = 0.002, respectively). Multivariate Cox proportional regression showed an independent association between T2DM and decreased patient and graft survival (HR, 1.170; P = 0.015 and HR, 1.133; P = 0.048, respectively). In the lean and the class 3 obesity NASH groups, patients with T2DM had lower patient and graft survival than the patients without T2DM. In the class 3 obesity NASH group, T2DM was independently associated with decreased patient survival (HR, 1.581; P = 0.027).ConclusionOur research reveals that the focus of the post-transplantation treatment should be different for different BMI patients.

Whereas there is huge data on NAFLD, little is known about NAFLD in LT. In this review, we aim to explore extrahepatic organs and their potential mechanisms in the development of NAFLD in LT patients and discuss current limitations in preclinical and clinical scenarios with suggestions for future study. Methods: The following keywords, such as NAFLD, NASH, liver transplant, therapy, pathogenesis and biomarkers, were set for literature retrieval. The articles which were published articles in English till 25th June 2020 in PubMed database were included, and there is no limit for the study design type. Key Content and Findings: Following LT, there are significant shifts in the microbiota and farnesoid X receptor may be a potential therapeutic target for NAFLD in LT settings. The roles of probiotics and diet on NALFD remain inconclusive in LT background. Nevertheless, the adipokines and cytokines disorder and local insulin resistance of adipose tissue may contribute to NAFLD process. Bariatric surgeries are promising in controlling de novo and recurrent NAFLD with significant reduction in abdominal adipose tissue, despite the optimal timing is inconclusive in LT cases. Furthermore, circumstantial evidence indicates that miRNA33a may function as a mediator bridging sarcopenia and NAFLD of post-LT. beta-Hydroxy-beta-Methyl-Butyrate treatment could improve muscle status in graft recipients and shows protective potential for NAFLD in LT settings. Conclusions: Gut, adipose tissue and muscle are intricately intertwined in promoting NAFLD in LT cases. Further animal studies are needed to deepen our understanding of mechanisms in multi-organ crosstalk. High quality clinical trials are warrant for making guidelines and developing management strategies on NAFLD after LT.

Abstract:Liver transplantation (LT) is the most effective way to save patients with acute-on-chronic liver failure (ACLF). However, the impact of donor diabetes mellitus (DM) on LT outcomes in patients with ACLF has not been fully investigated. We retrospectively analyzed data from the Scientific Registry of Transplant Recipients (SRTR) between January 1st, 2008 to December 31st, 2017 in this study. All the patients were divided into donors with DM and without DM group (DM: 1,394; non-DM: 11138). We compared the overall survival (OS) and graft survival (GS) across different estimated ACLF (estACLF) grades between two groups. There were 25.10% estACLF-3 patients in the entire cohort. And in estACLF-3 patients, 318 patients had DM donors. The estACLF-3 associated 5-year OS rate in the non-DM group was 74.6%, significantly better than that in the DM group, with corresponding survival rate at 64.9% (P < 0.001). Donor DM was an independent predictor for OS in the entire cohort as well as in estACLF-3 patients. Donor DM was associated with inferior outcomes of LT in patients with estACLF-3. However, the differences were not obvious in recipients with other estACLF grades.

Abstract:Background The correlation between preoperative lipid profiles and new-onset diabetes after transplantation (NODAT) remains relatively unexplored in liver transplant recipients (LTRs). Thus, we aimed to investigate the preoperative lipid profiles in Chinese LTRs and evaluate the different influences of preoperative total cholesterol, total triglycerides (TG), high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol on the development of NODAT in both sexes. Methods A total of 767 Chinese LTRs from Zhongshan Hospital were retrospectively evaluated. NODAT was defined according to the American Diabetes Association guidelines; the relationship between each preoperative lipid index and NODAT development was analyzed separately in men and women. Results Pretransplant hypotriglyceridemia was observed in 35.72% of the total LTRs. In men, only the preoperative TG level was significantly associated with incident NODAT after adjusting for potential confounders (hazard ratio 1.37, 95% confidence interval 1.13-1.66, P = .001). There was a nonlinear relationship between the preoperative TG level and NODAT risk. The risk of NODAT significantly increased with preoperative a TG level above 0.54 mmol/L (log-likelihood ratio test, P = .043). In women, no significant association was observed. Conclusion Among male LTRs, a higher preoperative TG level, even at a low level within the normal range, was significantly and nonlinearly associated with an increased risk of NODAT.
Liver Grafts for Transplantation from Donors with Diabetes: an Analysis of the Scientific Registry of Transplant Recipients Database

Abstract:Patients with a history of diabetes mellitus (DM) have worse survival than those without DM after liver transplantation. However, the effect of liver grafts from DM donors on the post-transplantation survival of recipients is unclear. Using the Scientific Registry of Transplant Recipients database (2004-2008), 25,413 patients were assessed. Among them, 2,469 recipients received grafts from donors with DM. The demographics and outcome of patients were assessed. Patient survival was assessed using Kaplan-Meier methodology and Cox regression analyses. Recipients from DM donors experienced worse graft survival than recipients from non-DM donors (one-year survival: 81% versus 85%, and five-year survival: 67% versus 74%, P<0.001, respectively). Graft survival was significantly lower for recipients from DM donors with DM duration >5 years (P<0.001) compared with those with DM duration <5 years. Cox regression analyses showed that DM donors were independently associated with worse graft survival (hazard ratio, 1.11; 95% confidence interval, 1.02-1.19). The effect of DM donors was more pronounced on certain underlying liver diseases of recipients. Increases in the risk of graft loss were noted among recipients from DM donors with hepatitis-C virus (HCV) infection, whereas those without HCV experienced similar outcomes compared with recipients from non-DM donors. These data suggest that recipients from DM donors experience significantly worse patient survival after liver transplantation. However, in patients without HCV infection, using DM donors was not independently associated with worse post-transplantation graft survival. Matching these DM donors to recipients without HCV may be safe.
New-Onset Diabetes after Liver Transplantation and Its Impact on Complications and Patient Survival

Abstract:BackgroundThe aim of the present study was to investigate the incidence and risk factors of new-onset diabetes after transplantation (NODAT) in liver transplant recipients and the influence of NODAT on complications and long-term patient survival.MethodsWe examined 438 patients who underwent liver transplantation between April 2001 and December 2008 and were not diabetic before transplantation.ResultsThe mean (SD) follow-up duration was 2.461.62 years. The incidence of NODAT 3, 6, 9, 12, 36, and 60 months after transplantation was 44.24%, 25.59%, 23.08%, 25.17%, 17.86%, and 18.18%, respectively. Multifactor analysis indicated that preoperative fasting plasma glucose (FPG) levels and donor liver steatosis were independent risk factors for NODAT, whereas administration of an interleukin-2 receptor (IL-2R) antagonist reduced the risk of NODAT. Compared with the no NODAT group (N-NODAT), the NODAT group had a higher rate of sepsis and chronic renal insufficiency. Mean survival was significantly longer in the N-NODAT than NODAT group. Cox regression analysis showed that pre- and/or postoperative FPG levels, tumor recurrence or metastasis, and renal insufficiency after liver transplantation were independent risk factors of mortality. Pulmonary infection or multisystem failure were specific causes of death in the NODAT group, whereas patients in both groups died primarily from tumor relapse or metastasis.ConclusionsPreoperative FPG levels and donor liver steatosis were independent risk factors for NODAT, whereas administration of an IL-2R antagonist reduced the risk of NODAT. Patients with NODAT had reduced survival and an increased incidence of sepsis and chronic renal insufficiency. Significant causes of death in the NODAT group were pulmonary infection and multisystem failure.

Abstract:Background: This study investigated the association between the preoperative lipid profiles and new-onset diabetes after transplantation (NODAT) in Chinese kidney transplant recipients (KTRs).Methods: In this study, of 1140 KTRs registered between January 1993 and March 2018 in Zhongshan Hospital, Fudan University, 449 were enrolled. Clinical data, obtained through a chart review of the patient records in the medical record system, were evaluated, and NODAT was diagnosed based on the American Diabetes Association guidelines. Multivariate Cox regression analysis was conducted to determine whether the preoperative lipid profiles in KTRs were independently associated with NODAT incidence. The preoperative lipid profiles were analyzed as continuous variables and grouped into tertiles. Smooth curve fitting was used to confirm the linear associations.Results: During a median follow-up of 28.03 (interquartile range 12.00–84.23) months, 104 of the 449 (23.16%) participants developed NODAT. In the multivariate model analysis, after adjusting for all potential covariates, increased preoperative total cholesterol (TC) [hazard ratio (HR)=1.25; 95% confidence interval (CI):1.09–1.58, p=0.0495], low-density lipoprotein cholesterol (LDL-C, HR=1.33, 95% CI: 1.02–1.75, p=0.0352), non-high-density lipoprotein cholesterol (non-HDL-C, HR=1.41, 95% CI: 1.09–1.82, p=0.0084), TC/HDL-C(HR=1.28, 95% CI: 1.06–1.54, p=0.0109), and non-HDL-C/HDL-C(HR=1.26 , 95% CI: 1.05–1.52, p=0.0138) were associated with NODAT. The association between the preoperative triglyceride (TG), HDL-C, or TG/HDL-C and NODAT were not significant.Conclusions: Preoperative TC, LDL-C, non-HDL-C, TC/HDL-C, and non-HDL-C/HDL-C were independent risk factors for NODAT.
Impact of living donor liver transplantation on long-term cardiometabolic and graft outcomes in cirrhosis due to nonalcoholic steatohepatitis

Abstract:Background and aim: Non-alcoholic steatohepatitis (NASH) is a leading indication for liver transplantation (LT). This study aimed to determine whether living donor LT (LDLT) recipients experienced less recurrent NASH, cirrhosis, and cardiometabolic complications compared to deceased donor LT (DDLT). Method: Patients with LDLT and DDLT for NASH between February 2002 and May 2018 at University Health Network (UHN) were compared. Cox Proportional Hazard model was used to analyze overall survival (OS), Fine and Gray's Competing Risk models were conducted to analyze cumulative incidence of post LT outcomes. Results: One hundred and ninety-nine DDLTs and 66 LDLTs were performed for NASH cirrhosis. Time and rate of recurrence of NAFLD and NASH were comparable in both groups. Graft cirrhosis was more common in DDLT recipients (n = 14) versus LDLT (n = 0) (p < .0001). Significant fibrosis (Fibrosis ≥ F2) developed in 50 recipients (12 LDLT and 38 DDLT) post LT (DDLT vs. LDLT: HR = 1.00, 95% CI = (.52-1.93), p = .91) and there was no difference in time to significant fibrosis (p = .57). There was no difference in development of post-transplant diabetes, dyslipidemia, metabolic syndrome, cardiovascular disease, and cancers. LDLT group had better renal function at 10 years (MDRD eGFR of 57.0 mL/min vs. 48.5 mL/min, p = .047). Both groups had a comparable OS (HR = 1.83 (95% CI = .92-3.62), p = .08). Conclusion: Overall, LDLT recipients had significantly better renal function by virtue of having early transplantation in their disease course. LDLT was also associated with significantly less graft cirrhosis, although OS and cardiometabolic outcomes were comparable between LDLT and DDLT.
Responding to Technology-Enabled Organizational Transformation

Abstract:As a result of the emergence of the Internet and net-enabled business processes, many industries have experienced a period of IT-enabled transformation in which organizations and business operations changed very rapidly. A natural question that arises is how can firms survive and even thrive during such transformations? In addressing this question, we show how a firm’s strategic change orientation—a meta-construct consisting of technological opportunism, market orientation, and entrepreneurial orientation—can influence the assimilation of IT and the resulting performance of business processes. We identify and examine two separate change enablers through which this influence occurs: (1) the development of IT capabilities; (2) the creation of a positive climate for IT use. These two change enablers influence the assimilation of technology within the organization and the resulting business process performance. Results explain 34% of the variance in process performance, and 34% of the variance in financial performance.

Abstract:Liver transplant (LT) recipients have experienced a significant improvement in short-term survival during the last three decades due to advancements in surgical techniques, perioperative management and effective immunosuppressive regimens. However, long-term survival is affected by a high incidence of metabolic disorders and their consequences including cardiovascular disease and malignancies. Pre-transplant metabolic impairments especially in those with NASH cirrhosis are aggravated by addition of post-transplant weight gain, physical inactivity, and reversal from catabolic to anabolic state. Moreover, although immunosuppressants are vital to avoid graft rejection, long term exposure to these medications is implicated in metabolic impairments post-LT. In this review, we aim to summarize molecular pathogenesis of different metabolic disorders post-LT including diabetes mellitus, dyslipidemia, and non-alcoholic fatty liver disease. Furthermore, cardiovascular disease, malignancies, and graft rejections were provided as significant complications of post-LT metabolic conditions threatening both patient and graft survival. Ultimately, emerging preventive and treatment strategies for post-transplant diabetes mellitus are summarized. This review highlighted the significant need for more clinical trials of antihyperglycemic agents in LT recipients. Also, translational studies will help us to better understand the molecular and genetic factors underlying these metabolic complications and could lead to a more personalized management in this high-risk population.

Surging Liver Transplantation for Nonalcoholic Steatohepatitis from 2000-2022: A National Database Study

Abstract:Objectives: Our aim was to provide an up-to-date, large-scale overview of the trends and clinicodemographics for NASH LTs performed in the United States compared with all other LT indications between 2000 and 2022. We also examined the demographic factors that will predict future demand for NASH LT. Methods: Our analysis of NASH LT from the Organ Procurement & Transplantation Network database spanning 2000-2022 consisted primarily of descriptive statistics and hypothesis testing with corrections for multiple testing when necessary. Trend lines and linear correlations were also explored. Results: NASH LTs have experienced a remarkable surge, escalating from 0.12% of all LTs in 2000 to a substantial 14.7% in 2022, marking a 100-fold increase. Examining demographic trends, a significant proportion of NASH LTs recipients fall within the 50- to 64-year-old age group. Moreover, 52% of these recipients concurrently exhibit type 2 diabetes mellitus, a notably higher percentage than the 19% observed in all LT recipients. Type 2 diabetes mellitus emerges as a prominent risk factor for NASH progressing to end-stage liver disease. The phenomenon of repeat transplantation is noteworthy; although 6% of all LTs necessitate repeat procedures, this figure dramatically drops to 0.6% for NASH LTs. Ethnic disparities are apparent, with African Americans representing a mere 2% of NASH LT recipients, significantly lower than their representation in the overall population. Regionally, the East Coast has a higher proportion of NASH LT recipients compared with waitlist additions. This trend holds true across demographics. Conclusions: Our findings underscore the need for increased resources, particularly for minority, uninsured, or noncitizen individuals requiring LT for NASH. This analysis provides valuable insights into the dynamic landscape of LTs in the context of NASH, shaping the trajectory of medical interventions in the 21st century.

Abstract:Nonalcoholic fatty liver disease (NAFLD) affects 25% of the global adult population with a range of 13.5% in Africa and 31.8% in the Middle East. Nonalcoholic fatty liver disease is closely associated with a constellation of metabolic comorbidities which include: obesity, type 2 diabetes mellitus, hypertension, and hypercholesteremia. In fact, the increasing number of metabolic comorbidities not only increases the prevalence of NAFLD but also places patients at higher risk for progressive liver disease. As such, NAFLD is presently among the top etiologies for hepatocellular carcinoma and an indication for liver transplantation (LT) in the United States. Therefore, the following recommendations are made based on our current knowledge of NAFLD and its consequences: (1) the evaluation of the risk of liver disease progression can be affected by patient's ethnic origin and sex; (2) fibrosis in NAFLD is the most important predictor of mortality; (3) we recommend that individuals who present with features of metabolic syndrome in the presence of elevated liver enzymes should be screened for NAFLD and, more importantly, nonalcoholic steatohepatitis (NASH); (4) we recommend that NAFLD patients, especially those with multiple risk factors, should be screened for cardiovascular diseases and managed accordingly; (5) comorbidities in NAFLD/NASH patients who are considered for LT need to be assessed in the pretransplant and posttransplant settings because these factors can affect waitlist mortality, resource utilization, as well as posttransplant complications, morbidity, and perhaps, mortality; (6) any attempt to decrease the incidence of NAFLD should ideally address the development of obesity in childhood and early adulthood, favoring the adoption of healthy lifestyles through comprehensive health policy programs.

Abstract:Background and Aims: Liver transplantation has become an effective therapy for patients with end-stage liver disease. The risk of new-onset diabetes after transplantation (NODAT) and posttransplant metabolic syndrome (PTMS) is high among patients after liver transplantation. These are thought to be associated with increased risks of graft rejection, infection, cardiovascular disease, and death. Our study aimed to document the incidence of NODAT and PTMS and analyze pre and posttransplant predictive factors for their development in patients undergoing a liver transplant. Methods: This was a prospective comparative study on 51 patients who underwent live donor liver transplantation. They were evaluated at baseline, 3 and 6 months after transplantation with fasting glucose, lipids, serum insulin levels, C-peptide, and HbA1C. They were followed up at 5 years to document any cardiovascular events or rejection. Results: The incidence of preoperative diabetes mellitus (DM) in the study group was 25/51 (49%). The incidence of NODAT was 38.5% (10/26 patients) and PTMS 29% (10/35), respectively. Age (47.7 ± 5.4 vs 41.5 ± 12.7 years), HOMA2 -IR (2.3 ± 1.8 vs 2.1 ± 1.6), serum insulin (16.1 ± 12.0 vs 17.9 ± 14.5), and C-peptide (4.6 ± 0.5 vs 4.8 ± 0.7) were similar at baseline in the NODAT group compared to those who did not develop it. Mean tacrolimus levels were higher in PTMS group (6.8 ± 2.9 vs 5.0. ± 2.0 P value = 0.042). By the end of 5 years, 7 patients expired; 6 due to rejection and one due to cardiovascular disease. Moreover, 2 of these patients had preexisting DM and 2 had NODAT. Conclusions: None of the baseline metabolic factors in patients undergoing liver transplant were predictive of the development of NODAT or PTMS. Mean tacrolimus levels were significantly higher in the PTMS group. A 5-year follow-up showed no excess risk of cardiovascular events or rejection in those with preexisting DM or in those who developed NODAT.

Abstract:BACKGROUND:New-onset diabetes mellitus (NODM) after liver transplantation (LT) occurs with increased frequency in recipients with hepatitis C virus (HCV). We compared the incidence and risk factors for NODM in HCV vs non-HCV recipients.METHODS:Among 24,956 liver recipients, 18,741 without pretransplantation diabetes were identified. NODM-free survival was analyzed using Kaplan-Meier and log-rank tests, and risk factors for NODM were examined using multivariate Cox regression analysis.RESULTS:The overall incidence of NODM was 13.0% at 1 year after LT. At 1, 2, 3, and 5 years after LT, incidence of NODM in HCV recipients was 14.4%, 4.3%, 3.1%, and 3.5%, respectively, compared with 11.9%, 3.5%, 3.2%, and 6.4%, respectively, in non-HCV recipients. HCV recipients had a higher risk of NODM than non-HCV recipients (hazard ratio 1.17 [1.09-1.27], P < .001). Predictors of NODM in HCV recipients were age, body mass index (BMI), tacrolimus and steroid usage at discharge, acute rejection episode, and donor with diabetes mellitus. Risk factors in non-HCV recipients were male recipient, BMI, and recipients with nonalcoholic steatohepatitis diagnosis.CONCLUSIONS:HCV recipients have a higher incidence and more risk factors for NODM than non-HCV recipients. Early identification of modifiable risk factors will assist clinical interventions to prevent NODM complications after LT.
End-Stage Liver Disease Comorbidities in Patients Awaiting Transplantation: Identification and Impact on Liver Transplant Survival

Abstract:End-Stage Liver Disease (ESLD), a complex condition, has high rates of co-occurring comorbidities affecting multiple organ systems. There is no clear evidence-based practice (EBP) guidelines addressing the progression of comorbidities in ESLD patients awaiting liver transplantation (LT) and their impact on survival, both pre- and post-transplant. This study aimed to identify and quantify the trajectory of the most common and deteriorating comorbidities in ESLD patients awaiting LT and to analyze their effect on patient outcomes. An initial exploratory phase to identify frequent comorbidities in ESLD patients. Relevant EBP-driven data for diagnosing and measuring the progression of these conditions were collected and organized into five research matrices. In the quantitative phase, a retrospective analysis was conducted using longitudinal de-identified data from electronic health records (EHR) for patients who underwent LT between 2011-2021. Data included demographics, labs, procedures, and medications. Descriptive statistics and survival analysis assessed the association of comorbidities with post-transplant survival. The five most frequent comorbidities identified were Diabetes Mellitus (DM), Chronic Kidney Disease (CKD), Malnutrition, Portal Hypertension (PH), and Ascites. Of the 722 patients analyzed, 68.2% were male, mean age of 54.81 (SD = 11.24), and 19.8% died post-LT. Survival analysis showed a gradual decline over time, with the most significant drop at five years post-LT. Significant predictors of post-LT survival were age at transplant (p=0.01), waitlist time (p=0.004), DM at listing (p=0.02), low albumin (p=0.03), and CKD stage 5 development after listing (p=0.04). This study highlights the variability in diagnosing and measuring comorbidities in ESLD patients and provides insights into their progression and impact on post-LT survival.
