Abstract:Background: Graft-versus-host disease (GVHD) is a life-threatening complication of hematopoietic stem cell transplantation (HSCT). The intestine not only is a primary target of immune attack, but also regulates the severity of systematic GVHD. Although the critical roles of immune cells in GVHD have been documented, the dynamic changes of cell populations, and cell-associated transcriptional signatures during immune attack and epithelial regeneration remain elusive. Methods: To better characterize disease progression, tissues were harvested from murine intestine suffering from GVHD (GVHD_D3, GVHD_D10, GVHD_D21), coupled with irradiated and repaired samples (BM_D3, BM_D10, BM_D21), and healthy control (CON). Each sample mixed with segments of proximal and distal small intestine from three mice. Tissues were dissociated into single-cell suspensions and processed using the 10x Genomics Chromium workflow. After data pre-processing and quality control, we profiled 69242 individual cells from 7 samples. Results: Uniform manifold approximation and projection (UMAP) analysis yielded 17 clusters, including 8 types of epithelial cells, 8 types of immune cells and 1 type of stroma cell. In healthy mice, epithelial cells were highly enriched in murine small intestine, taking up 79.31% of total cells: differentiated enterocytes (EC) (59.31%) represented the largest cohort of epithelial cells, while intestinal stem cells (ISC), trans-amplifying (TA) cells and enterocyte progenitor (EP) only occupied very small portions. In mucosal immune system, the abundance of T cells (9.91%) and B cells (10.43%) was much higher than that of ILC, macrophage and neutrophils. Upon radiation and the development of GVHD, cell populations of murine small intestine dramatically changed. Three days after transplantation, the proportion of ECs dramatically dropped. On the contrast, ɣδ T cells and ILC rapidly accumulated in the damaged intestine. From day 3 to day 10, we observed a steep decrease in ɣδ T cells from samples of BM and GVHD group. At our last observation, BM resorted its epithelial components, while GVHD had a great abundance of T cells (41.73%) and a lower level of differentiated ECs (36.90%). In epithelial components, a total of 44468 epithelial cells were subclustered them into 22 subsets. ISC serves as the production department, which was enriched in mitochondrial genes and genes regulating protein folding. The cluster TA_1 from D21 samples was enriched in pathway related to structure organization and BMP signals, which is required for the initiation of intestinal differentiation and morphogenesis. At day 3, a rapid expansion of subsets of TA_2 and Regenerative progenitors caught our attention. Metabolic pathways such as tryptophan catabolic process, arachidonic acid secretion, methionine metabolic process were highly correlated with wound healing and cell proliferation. EP_1 emerging at day 3 majored in material transportation. EP in samples from control and BM (EP_2) was able to repair DNA damage, while EP_3 that mostly detected in GVHD sample were involved in metabolic disorders, which may explain its deficiency in EC differentiation. T cells and ILC were divided into 16 subsets. In addition to its well-known effects in host defense, several genes that expressed in γδT cells are essential for maintaining intestinal homeostasis. The Tgfb3-Vegfa+ subset regulated microvillus organization and intestinal absorption; the Tgfb3-Vegfa- subset participated in sensory and ciliary movement. At day3, the above clusters undergone a rapid expansion, suggesting an enhanced protective role of γδT cells in the early period of transplantation. We also identified different subtypes of T cells between BM and GVHD samples. Two clusters of cycling CD8+ T cells were more enriched in GVHD samples, which also express high levels of cytotoxic markers. While in BM, we found two clusters of anergic T cells without expression of markers of naïve, memory, effector and exhaustion. The double positive T cells in GVHD may regulate immune infiltration, while the CD4+ Th2-like cell in BM may link to immunoglobin production and Treg differentiation. Conclusions: Single-Cell transcriptional analysis revealed that different cell populations between BM and GVHD. T cells in GVHD were highly-proliferative and cytotoxic, and epithelial cells in GVHD had metabolic disorder and were resistant to enterocyte differentiation.
Down Regulation Of Cxc Chemokine Receptor 3 Impedes The Ability Of Anti-Cxcr3 Antibody To Block Graft-Versus-Host Disease

Abstract:Graft versus host disease (GVHD) is caused by infused donor T cells that primarily target specific organs, such as skin, liver and intestine. While it has been demonstrated that chemokines are critical to directing in vivo trafficking of host-reactive donor T cells into GVHD target organs, blocking GVHD using antagonist antibody (Ab) against chemokine receptors remains an elusive goal. We found that in vivo administration of anti-CXCR3 Ab for 1 week (named short-term treatment) only partially prevented acute GVHD in B6/SJL mice receiving donor C3H.SW naïve CD8+ T cells, with 45% of them dying from the disease. Flow cytometry analysis showed that CXCR3+ CD8+ T cells occurred in the spleens and livers of these GVHD B6/SJL mice by day 3 and peaked by day 7 after transplantation, and persisted throughout the disease course. When donor CXCR3+ CD8+ T cells were recovered from these GVHD B6/SJL recipients at day 14 after transplantation (named day-14 CXCR3+ CD8+ T cells) and adoptively transferred into secondary B6/SJL mice, they caused severe GVHD in these secondary recipients with 90% mortality. Interestingly, short-term treatment using anti-CXCR3 Ab protected 78% of B6/SJL mice receiving these donor day-14 CXCR3+ CD8+ T cells. Thus, besides CXCR3+ T cells, some other pathogenic T cells have developed during GVH reactions. We found that a substantial population of CXCR3− CD8+ T cells occurred by day 14 (termed day-14 CXCR3− CD8+ T cells) after transplantation in the spleens (71.1%) and livers (81.6%) of B6/SJL recipients with ongoing GVHD. As compared to donor day-14 CXCR3+ CD8+ T cells, donor day-14 CXCR3− CD8+ T cells expressed lower levels of CCR2, CCR3, CCR5, CD127, granzyme B and FasL, and produced less IFN-g and TNF-a. Furthermore, adoptive transfer of donor day-14 CXCR3− CD8+ T cells caused delayed onset of GVHD as compared to those recipients of day-14 CXCR3+ CD8+ T cells, with 55% of them dying from the diseases. Prolonged treatment of B6 mice receiving day-14 CXCR3− CD8+ T cells with anti-CXCR3 Ab up to 3 weeks (named long-term treatment) significantly inhibited the development of GVHD. More than 70% of these recipients survived without clinical signs of GVHD. In vivo experiments showed that 23% of day-14 CXCR3− CD8+ T cells were converted into CXCR3+ by 12 hours after adoptive transfer into B6/SJL recipients, and up to 60% by 24 hours. Interestingly, about 25% of CXCR3+ CD8+ T cells were also converted into CXCR3− by 24 hours after their transfer. We found in ex vivo cultures that both dendritic cells and interleukin-2 accounted for this conversion between CXCR3+ and CXCR3− subsets, which was inhibited by addition of Ab specific to MHC-I or CTLA-4 Ig. Taken together, these results suggest that alloantigen-induced repression of chemokine receptor(s) may present a major barrier to the success of GVHD prevention and treatment when using antagonists against chemokines or chemokine receptors.
γδ T Cells May Aggravate Acute Graft-Versus-Host Disease Through CXCR4 Signaling After Allogeneic Hematopoietic Transplantation

Abstract:Graft-versus-host disease (GVHD) is a pathology in which chemokines and their receptors play essential roles in directing the migration of alloreactive donor T cells into GVHD organs, thereby leading to further target tissue damage. Currently, acute GVHD (aGVHD) remains a major cause of high morbidity and mortality in patients who underwent allogeneic hematopoietic cell transplantation (alloHCT). The identification of immune cells that correlate with aGVHD is important and intriguing. To date, the involvement of innate-like γδ T cells in the pathogenesis of aGVHD is unclear. Herein, we found that primary human γδ T cells did not directly trigger allogeneic reactions. Instead, we revealed that γδ T cells facilitated the migration of CD4 T cells via the SDF-1-CXCR4 axis. These results indicate indirect regulation of γδ T cells in the development of aGVHD rather than a direct mechanism. Furthermore, we showed that the expression of CXCR4 was significantly elevated in γδ T cells and CD4 and CD8 T cells in recipients who experienced grades II-IV aGVHD after alloHCT. Consistently, CXCR4-expressing γδ T cells and CD4 T cells were induced in the target organs of mice suffering aGVHD. The depletion of γδ T cells in transplant grafts and treatment with AMD3100, an inhibitor of CXCR4 signaling, delayed the onset of aGVHD and prolonged survival in mice. Taken together, these findings suggest a role for γδ T cells in recruiting alloreactive CD4 T cells to target tissues through the expression of CXCR4. Our findings may help in understanding the mechanism of aGVHD and provide novel therapeutic targets.

Abstract:Recipient antigen-presenting cells (APCs) initiate GVHD by directly presenting host minor histocompatibility antigens (miHAs) to donor CD8 cells. However, later after transplantation, host APCs are replaced by donor APCs, and if pathogenic CD8 cells continue to require APC stimulation, then donor APCs must cross-present host miHAs. Consistent with this, CD8-mediated GVHD is reduced when donor APCs are MHC class I(-). To study cross-presentation, we used hosts that express defined MHC class I K(b)-restricted miHAs, crossed to K(b)-deficient backgrounds, such that these antigens cannot be directly presented. Cross-priming was surprisingly efficient, whether antigen was restricted to the hematopoietic or nonhematopoietic compartments. Cross-primed CD8 cells were cytolytic and produced IFN-gamma. CD8 cells were exclusively primed by donor CD11c(+) cells, and optimal cross-priming required that they are stimulated by both type I IFNs and CD40L. In studying which donor APCs acquire host miHAs, we made the surprising discovery that there was a large-scale trans-fer of transmembrane proteins from irradiated hosts, including MHC class I-peptide complexes, to donor cells, including dendritic cells. Donor dendritic cells that acquired host MHC class I-peptide complexes were potent stimulators of peptide-specific T cells. Cytotoxic T Lymphocyte Antigen-4 Down-Regulates T Helper 1 Cells by Increasing Expression of Signal Transducer and Activator of Transcription 3 in Acute Graft-versus-Host Disease.

Abstract:Numerous previous studies have suggested that cytotoxic T lymphocyte antigen-4 (CTLA-4) plays an important role in acute graft-versus-host disease (GVHD). How CTLA-4 acts in regulating acute GVHD remains unknown, however. In the present study, we found that, compared with healthy controls, CTLA-4 plasma and relative mRNA levels in patients with acute GVHD were initially decreased and then markedly elevated after 28 days of treatment. CTLA-4 levels were higher in patients with grade I-II acute GVHD compared with those with grade III-IV acute GVHD both before and after treatment. Up-regulation of CTLA-4 significantly increased the luciferase activity and degree of phosphorylation of signal transducer and activator of transcription 3 (STAT3). Meanwhile, T cell activation was significantly inhibited, and levels of IFN-γ, IL-17, and IL-22 decreased. These findings suggest that CTLA-4 might be involved in the pathogenesis of acute GVHD, and may down-regulate T helper 1 cells by increasing STAT3 expression in acute GVHD.
MiR-17-92 is Required for the Pathogenicity of T and B Cells in Chronic Gvhd

Abstract:Chronic graft-versus-host disease (cGVHD) remains to be a major cause of mortality and morbidity after allogeneic hematopoietic cell transplantation (allo-HCT). cGVHD is characterized as autoimmune-like fibrosis and antibody production, mediated by pathogenic T and B cells. Through producing pro-inflammatory cytokines, CD4 T cells are the driving force of cGVHD. Donor B cells augment the pathogenesis of cGVHD not only by acting as antigen-presenting cells (APCs) and promoting CD4 T-cell expansion and survival, but also by producing autoantibodies. microRNA (miR)-17-92 has been shown to regulate T-cell immunity including allogeneic, anti-viral, and anti-tumor responses. Recently, miR-17-92 was found to act together with Bcl-6 to promote the differentiation of Follicular help T (Tfh) cells. Furthermore, B-cell deficiency of miR-17-92 impairs IgG2c production. Since Tfh differentiation and antibody production are required for the development of cGVHD, we hypothesize that miR-17-92 contributes to the pathogenesis of cGVHD by promoting pathogenic T- and B-cell responses.
Donor Th17 Cells and Il-21 May Contribute to the Development of Chronic Graft-Versus-Host Disease after Allogeneic Transplantation

Abstract:The roles of Th17 cells and IL-21 in the pathogenesis of chronic graft-versus-host disease (cGVHD) in patients who undergo allogeneic transplantation are still unknown. Here, we examined this question by monitoring eight patients with new-onset cGVHD for the presence of Th17 cells, Th1 cells, and IL-21. Allografts from an additional 41 patients were also analyzed for Th17 and Th1 cells. Out of these 41 patients, the last 32 enrolled patients were further analyzed for Th17 cells, Th1 cells, and plasma IL-21 levels at day 30 post-transplantation regarding cGVHD. Th17 cells and IL-21 plasma levels were significantly increased at cGVHD onset and drastically decreased after complete remission. Patients who received a higher number of Th17 cells in their allograft had a higher incidence of cGVHD compared with patients who received a lower number of Th17 cells. Meanwhile, positive plasma IL-21 levels at day 30 post-transplantation predicted cGVHD occurrence. These results suggest that Th17 cells and IL-21 may contribute to the development of cGVHD.

Abstract:Abstract Abstract 2310 Acute GVHD is a proinflammatory process mediated in part by mature donor T cells present in the stem cell or marrow inoculums that are polarized toward a Th1 phenotype and recognize minor or major histocompatibility disparities between the donor and host. A large amount of data has clearly shown that a newly identified subset of interleukin (IL)-17-producing CD4 T lymphocytes, named TH-17 cells, play a crucial role in triggering inflammation and tissue injury in various autoimmune diseases. The role of TH17 cells in acute GVHD had been controversial in recent mice and human transplantation. The aim of this study was to investigate the effects of IL17-producing T cells, including Th17 and Tc17 cells, on GVHD in patients receiving granulocyte colony-stimulating factor (G-CSF)-mobilized peripheral blood stem cell (PBPCs) and G-CSF-primed bone marrow (GBM) transplantation. Forty-one patients were analyzed according to the Th17 and Tc17 cell content in allograft in relation to aGVHD. Furthermore, ten patients with acute GVHD onset were monitored for the presence of Th17 cells by flow cytometry in the peripheral blood. Patients who subsequently developed aGVHD have greater proportions and doses of Th17 and Tc17 cells in GBM and PBPCs allograft infused into the patients (p=0.049 and 0.029 for Th17 in GBM; p=0.078 and p=0.033 for Tc17 in GBM; p=0.103 and 0.008 for Th17 in PBPCs; p=0.007 and 0.001 for Tc17 cells in PBPCs). At the same time, there were also significantly higher proportions and doses of Th1 and Tc1 cells in GBM and PBPCs allograft in patients with aGVHD compared with those levels in patients without aGVHD. Acute GVHD occurred 20 patients occurred, among which were 2 patients with gastrointestinal GVHD, 4 patients with skin plus gastrointestinal GVHD and the 14 others with simple skin GVHD. When we further compared patients according to the aGVHD target organ, we also found the significant association between dose of IL17 producing T cells (Th17 and Tc17) and the occurrence of simple skin GVHD. Cox regression models demonstrated that dose of Th17 in GBM (RR 1.095, CI 1.032– 1.162, P=0.003), dose of Tc17 in PBPCs (RR 1.063, CI 1.017– 1.112, P=0.008) and the number of HLA locus mismatch (RR 1.84, CI 1.18– 2.87, P=0.008) emerged as the independent factors influencing the occurrence of aGVHD. Patients receiving a higher dose of Th17 cells in GBM allograft (>8.5×104/kg, p=0.005), and Tc17 cells in PBPCs (>16.8×104/kg, p=0.001) exhibited a higher incidence of aGVHD. An increased Th17 population (up to 4.99% of CD4 T lymphocytes) was observed in patients with acute GVHD onset. In contrast, the percentage of Th17 cells drastically decreased in GVHD patients when they were treated to achieve partial and complete remission (p=0.013 and p=0.008, respectively). All percentages of Th17 and Tc17 were significantly reduced after G-CSF in vivo application. Our results suggest IL-17 producing T cells contribute to mediate aGVHD. Furthermore, G-CSF in vivo application helps to reduce the occurrence of aGVHD through reducing the secretion of IL17 in T cells. Disclosures: No relevant conflicts of interest to declare.
T Helper Cell Lineage-Defining Transcription Factors: Potent Targets for Specific GVHD Therapy?

Abstract:Allogenic hematopoietic stem cell transplantation (allo-HSCT) represents a potent and potentially curative treatment for many hematopoietic malignancies and hematologic disorders in adults and children. The donor-derived immunity, elicited by the stem cell transplant, can prevent disease relapse but is also responsible for the induction of graft-versus-host disease (GVHD). The pathophysiology of acute GVHD is not completely understood yet. In general, acute GVHD is driven by the inflammatory and cytotoxic effect of alloreactive donor T cells. Since several experimental approaches indicate that CD4 T cells play an important role in initiation and progression of acute GVHD, the contribution of the different CD4 T helper (Th) cell subtypes in the pathomechanism and regulation of the disease is a central point of current research. Th lineages derive from naïve CD4 T cell progenitors and lineage commitment is initiated by the surrounding cytokine milieu and subsequent changes in the transcription factor (TF) profile. Each T cell subtype has its own effector characteristics, immunologic function, and lineage specific cytokine profile, leading to the association with different immune responses and diseases. Acute GVHD is thought to be mainly driven by the Th1/Th17 axis, whereas Treg cells are attributed to attenuate GVHD effects. As the differentiation of each Th subset highly depends on the specific composition of activating and repressing TFs, these present a potent target to alter the Th cell landscape towards a GVHD-ameliorating direction, e.g. by inhibiting Th1 and Th17 differentiation. The finding, that targeting of Th1 and Th17 differentiation appears more effective for GVHD-prevention than a strategy to inhibit Th1 and Th17 cytokines supports this concept. In this review, we shed light on the current advances of potent TF inhibitors to alter Th cell differentiation and consecutively attenuate GVHD. We will focus especially on preclinical studies and outcomes of TF inhibition in murine GVHD models. Finally, we will point out the possible impact of a Th cell subset-specific immune modulation in context of GVHD.

Consistent condom use with different types of partners: evidence from two Nigerian surveys.

Abstract:HIV prevalence in Nigeria is increasing rapidly. Increased condom use is the most viable solution to slow down or reverse this trend. This article uses data from two waves of a nation-wide survey, each with over 5,000 respondents, to examine factors that influence consistency of condom use with various types of partners. The results show that while the overall level of consistent condom use has remained low, reported consistent condom use with occasional partners and commercial sex workers exceeds 60%. There is also some evidence of an increasing trend in consistent condom use, even after controlling for differences in sample composition and other factors. The most important factors affecting consistency of condom use are awareness that condoms are effective at preventing HIV and that they are effective at preventing unwanted pregnancy, concern about unwanted pregnancy, and concern about HIV. Although concern about unwanted pregnancy has a strong effect on consistency of condom use with stable partners, concern about HIV infection has a strong effect on consistency of condom use with nonstable partners. The results suggest that HIV prevention programs need to do more than provide education about the modes of transmission and the ways to prevent infection. Improving the effectiveness of HIV prevention programs is likely to require focusing more explicitly on people's personal risk perception and condom efficacy. Focusing on these topics may further accelerate the observed positive trend in condom use.
Possible involvement of cytokines, chemokines and chemokine receptors in the initiation and progression of chronic GVHD

Abstract:Chronic GVHD (cGVHD) after allogeneic hematopoietic SCT (HSCT) is characterized by an infiltration of T cells into target organs including the oral mucosa and salivary glands. This study was designed to clarify the molecular mechanism of the local accumulation of pathogenic T cells in cGVHD. The expression of cytokines, chemokines and chemokine receptors in the buccal mucosa (BM), labial salivary glands (LSG) and PBMC from 16 patients with cGVHD after allogeneic HSCT was examined. The mRNA expression of T helper 1 (Th1) and Th2 cytokines, and several chemokines and chemokine receptors was significantly increased in the BM and LSG from cGVHD patients, in comparison with both those in the BM and LSG from controls, respectively, and also with those in the PBMC from cGVHD patients. Furthermore, the mRNA expression of Th2 cytokines, macrophage-derived chemokine and CC chemokine receptor 4 was closely associated with a strong T-cell infiltration in the BM and LSG from cGVHD patients. These results suggest that cGVHD might be initiated and/or maintained by Th1/Th0 cells and thereafter progresses in association with Th2 cell accumulation via the interaction of particular chemokine and chemokine receptors.

Abstract:Chronic graft-versus-host disease (cGVHD) is a major complication in long-term survivors of allogeneic hematopoietic stem cell transplantation (allo-HSCT). Graft-derived T cells (TG) have been implicated in the induction of cGVHD; however, the extent of their contribution to the pathogenesis of cGVHD remains unclear. Using a mouse model of cGVHD, we demonstrate that TG predominate over hematopoietic stem cell-derived T cells generated de novo (THSC) in cGVHD-affected organs such as the liver and lung even at day 63 after allo-HSCT. Persisting TG, in particular CD8+ TG, not only displayed an exhausted or senescent phenotype but also contained a substantial proportion of cells that had the potential to proliferate and produce inflammatory cytokines. Host antigens indirectly presented by donor HSC-derived hematopoietic cells were involved in the maintenance of TG in the reconstituted host. Selective depletion of TG in the chronic phase of disease resulted in the expansion of THSC and thus neither the survival nor histopathology of cGVHD was ameliorated. On the other hand, THSC depletion caused activation of TG and resulted in a lethal TG-mediated exacerbation of GVHD. The findings presented here clarify the pathological role of long-lasting TG in cGVHD.

Abstract:Abstract Salivary glands are frequently affected in chronic graft-versus-host disease (cGVHD), an autoimmune-like disease that occurs after allogeneic hematopoietic stem cell transplant resulting in tissue sclerosis and gland dysfunction. Alloreactive T cells have been identified as disease drivers, yet the function/pathogenicity of tissue-resident memory cells (Trm) in target organs remain unclear. In this study, we examined glandular Trm populations and tissue-specific signals that regulate Trm differentiation and functionality. scRNAseq analysis of human labial minor salivary glands (MSG) revealed enrichment of immune cell clusters in MSG of oral cGVHD patients. Focusing on T CD8 cells, we identified 4 clusters, one of them unique to affected patients which had low expression of CD69, but expressed other markers of Trm, including CD103, BLIMP1 and PD1. Additionally, this cluster was enriched for genes associated with cytotoxicity (IFNG, PRF1 and GZMB) and exhaustion (TIM3, LAG3 and TIGIT). Interestingly, this population showed high expression of CXCR6 and ligand-receptor analysis predicted CXCL16 an important signaling ligand that targets T CD8 cells. Further analysis implicates macrophages as the main source of CXCL16, suggesting their role in local accumulation of CXCR6+ Trm in the gland. Flow cytometry confirmed increased number of CD69+CD103±CD8+ Trm cells in cGVHD patients. Lymphocytic infiltration in MSG using IHC, spatially related the elevated frequency of CD8+IFNg+ T cells within interacinar stroma and damaged acini. The importance of Trm in site-specific cGVHD still needs further investigation. Abstract:Allogeneic hematopoietic stem cell transplantation (allo-HCT) is an effective means by which to treat a wide variety of diseases resulting from dysfunctional hematopoiesis. However, the development of graft-versus-host disease (GVHD) remains the major cause of morbidity and mortality after allo-HCT. GVHD has been shown to occur in 2 phases: acute GVHD (aGVHD) and chronic GVHD (cGVHD). The IL-12 family of cytokines is comprised of IL-12, IL-23, IL-27 and IL-35. This family of cytokines is biologically distinct in that they are composed of functional heterodimers consisting of α and β-chain subunits. Of these, IL-12 and IL-23 have well documented proinflammatory functions responsible for Th1 differentiation and Th17 stabilization, respectively, and play critical roles in GVHD development. IL-12 (p35/p40) and IL-23 (p19/p40) share a common receptor chain, IL-12 receptor β1 (IL-12Rβ1), among the IL-12R and IL-23R signaling complexes, which consist of IL-12Rβ1/IL-12Rβ2 and IL-12Rβ1/IL-23Rα, respectively. While IL-23R expression is widely implicated in autoimmunity, the role of IL-12Rβ1 in this context remains much less defined. In agreement with previous publications, we found a pathogenic role for IL-23R on donor T cells in aGVHD; however, a similar effect was not seen for IL-12Rβ1. In contrast, using a transition mouse model for cGVHD (B6 to BALB/c), donor splenocytes deficient for either IL-12Rβ1 or IL-23R had an impaired ability to induce cGVHD compared to WT controls, indicating distinct roles for these receptors in the development of GVHD. We further investigated how these receptors on T cells function during cGVHD development by evaluating donor T and B cell reconstitution and differentiation in recipients at day 60 post-BMT. In the T cell compartment, cohorts that received IL-12Rβ1 and IL-23R- deficient splenocytes showed significantly decreased frequencies of donor Tfh cells (CD4+CXCR5+PD1+) in addition to decreased IFNg, IL-17 and IL-4/5. Conversely, donor B cell frequencies and double positive thymocytes were significantly increased in recipients of both IL-12Rβ1-deficient and IL-23R-deficient donor splenocytes compared to WT controls. Interestingly, germinal center B cells (B220+GL7+FAS+) were significantly decreased with a consistent decrease CD86 MFI in cohorts receiving IL-23RKO, but not that received IL-12Rβ1KO splenocytes, implying a pathogenic role for IL-23R on B cells in cGVHD. Taken together, our studies indicate that IL-23R plays an essential role, whereas IL-12Rβ1 is dispensable, for donor T cells to induce aGVHD. However, both IL-12Rβ1 and IL-23R play a similar, albeit not identical, pathogenic role on donor T cells in the induction of cGVHD. We interpret that donor T cells have a differential requirement for IL-12Rβ1 or IL-23R which is dependent on the level of inflammation (e.g. acute vs. chronic phase of GVHD) and, under certain circumstances, that IL-23R signaling can be transmitted without IL-12Rβ1.
The CCR4/CCL17 axis drives intestinal acute Graft versus Host disease after allogeneic bone marrow transplantation

Abstract:Acute-Graft- -Host disease (aGvHD) is a life-threatening complication after allogeneic stem-cell-transplantation. It is mediated by alloreactive T cells whose trafficking to aGvHD target organs is orchestrated by chemokines. We here asked whether CCL17 and its corresponding receptor CCR4 are involved in aGvHD development and severity. We applied an experimental mouse model of aGvHD in CCR4/CCL17 knockout mice and analyzed gut biopsies of GvHD patients. We show that the absence of CCR4 in transplanted T cells induced significantly less severe aGvHD. This was accompanied by reduced expression of Gata3. Mechanistically, only CD4 , but not CD8 CCR4 T cells protected from aGvHD. We next identified dendritic cells in the small intestine to produce CCL17, which selectively recruited CD4 T cells. IL-4 production by intestinal CD4 T cells promoted proliferation of CD8 T cells. In line, we detected an upregulation of CCL17 and in human aGvHD samples. Our results indicate that local CCL17 production in aGvHD target organs recruits T cells, reinforcing local tissue damage and immune cell recruitment. We identified the JAK1/2-inhibitor ruxolitinib to dampen CCL17-expression, thereby reducing GvHD severity. We here dissect a to date unknown role of the CCL17-CCR4 axis in aGvHD, which might help to develop novel therapeutic strategies.

T-Bet is Critical for the Development of Acute Graft-Versus-Host Disease by Regulating Hematopoietic Antigen Presenting Cells

Abstract:Abstract Graft-versus-host disease (GVHD) remains to be a major obstacle for the efficacy and continuing success of allogeneic hematopoietic stem cell transplantation in the treatment of various malignant and non-malignant diseases. Activation of antigen presenting cells (APCs), both host and donor origin, plays a crucial role in priming alloreactive donor T cells to induce and intensify acute GVHD (aGVHD). Beyond its critical effects on T cells, the T-box transcription factor T-bet also regulates activity of APCs, including dendritic cells (DCs) and B cells. However, the effect and mechanism of T-bet in regulating APCs in the development of aGVHD has not been investigated. To evaluate the role of T-bet in modulating APC function and aGVHD development, we compared the severity of aGVHD in WT versus T-bet-/- recipients using several well-defined, clinically relevant murine models of allogeneic bone marrow transplantation (allo-BMT). We observed that T-bet-/- recipients developed much milder aGVHD than their WT counterparts, reflected by significantly higher rate of survival, lower clinical scores, and better donor BM-derived B- and T-cell reconstitution. In T-bet-/- recipients, donor T cells significantly reduced IFN-γ production, proliferation and migration, and caused less damage in aGVHD target organs, such as liver and gut. By using various BM chimeras as the recipients, we further observed that T-bet expressed on recipient hematopoietic APCs was primarily responsible for donor T-cell response and pathogenicity in causing aGVHD. Additionally, we evaluated the role of T-bet in donor APCs by transplanting WT or T-bet-/- BM together with WT T cells into lethally irradiated allogeneic recipients. We observed that recipients of T-bet-/- BM developed attenuated aGVHD compared with those of WT BM, suggesting that T-bet also contributes to the function of donor APCs in the induction of GVHD. Given DCs are the most potent hematopoietic APCs, we subsequently focused on recipient DCs. DCs in T-bet-/- recipient produced less IFN-γ, expressed higher levels of Trail, but not FasL or TNF, to induce significantly higher levels of apoptosis on donor T cells prior to their massive proliferation. To test whether Trail/DR5 interaction is responsible for the induction of donor T cell apoptosis and subsequent reduction of aGVHD in T-bet-/- recipients, we compared the ability of WT or DR5-/- T cells in inducing aGVHD in WT versus T-bet-/- recipients after allo-BMT. While WT T cells induced severe aGVHD in WT recipients, they failed to do in T-bet-/- recipients. In contrast, DR5-/- donor T cells were capable to induce severe aGVHD in the recipients regardless of T-bet expression. These data suggests that Trail/DR5 interaction is a major signaling pathway responsible for donor T-cell apoptosis induced by T-bet-/- APCs, through which alleviates the development of aGVHD. In conclusion, we demonstrate that T-bet up-regulates IFN-γ production and down-regulates Trail expression on recipient DCs, which promotes donor T-cell activation and mitigates T-cell apoptosis, respectively. Thus, T-bet plays a critical role in the development of aGVHD by regulating the activity of hematopoietic APCs, particularly DCs. Taken together with our previous findings, we propose that T-bet is a potential therapeutic target for the control of aGVHD through regulating T-cell activation and differentiation as well as APC functions. Chemokine receptor CCR5 mediates alloimmune responses in graft-versus-host disease

Abstract:Allogeneic bone marrow transplantation (BMT) is an effective therapy for hematologic malignancies. However graft-versus-host disease (GVHD) is a major limiting factor for a successful patient outcome. GVHD is a result of alloimmune responses of donor T lymphocytes attacking the recipient's cells and tissues. Chemokine receptor CCR5 plays a role in solid organ allograft rejection and mediates murine GVHD pathogenesis. Herein, we report that infiltrating lymphocytes in the skin of human acute GVHD (aGVHD) samples are predominantly CCR5(+) T cells. In addition, we characterized the features of the CCR5 expression on alloreactive T lymphocytes. We found that the CCR5(+) population exhibits the characteristics of the activated effector T cell phenotype. CCR5 expression is upregulated upon allogenic stimulation, and CCR5(+) cells are proliferating with coexpression of T cell activation markers. Furthermore, the activated T cells producing inflammatory cytokine tumor necrosis factor (TNF)alpha, interleukin (IL)-2, or interferon (IFN)-gamma, are positive for CCR5. Thus, CCR5 is a marker for GVHD effector cells and CCR5(+) T cells are active participants in the pathogenesis of human aGVHD.
Anticipitated and Surprising Effect of IL-17-producing CD4+ T Cells on Chronic Graft-Versus-Host Disease after Unmanipulated Allogeneic Blood and Marrow Transplantation (126.17)

Abstract:The aim of this study was to investigate the effects of Th17 cells on chronic graft-versus-host disease (cGVHD) in patients who had undergone allogeneic transplantation. Allograft from 41 patients were analyzed for Th17 with respect to cGVHD. Seven patients with cGVHD onset and after those achieved complete remission (CR) were monitored for the Th17 in the peripheral blood (PB). RNA was isolated from PB mononuclear cells to detect the transcript levels of STAT5, T-bet, Foxp3 and RORγ. There were significantly differences in Th17 dose in allograft between patients with and without cGVHD (p=0.012), especially with local or pure cGVHD (p=0.001). Patients received a higher Th17 dose in allograft (>1.67×106/kg, p=0.018,n=20) exhibited a higher incidence of cGVHD compared to those received lower Th17 dose (n=21). Th17 cells were highly increased when cGVHD onset and decreased drastically following CR (p=0.018). Patients received a higher Th17 dose exhibited a trend to have the lower incidence of transplantation related mortality (p=0.084) and relapse (p=0.181), and achieved higher incidence of overall survival (p=0.082) and leukemia free survival (p=0.026). No differences in the transcript levels of STAT, T-bet, Foxp3 and RORγwere found between patients with cGVHD onset and after CR. These results suggested that Th17 cells contributed to cGVHD, especially skin cGVHD, and might account for the anti-leukemia effect, therefore increasing the leukemia free survival.
TCF-1is Required By CD8 + T Cells for the Maintenance of Alloimmune Responses in Graft-Vs-Host Disease

Abstract:Graft-vs-host disease (GVHD) is a common complication of allogeneic stem cell transplant (alloSCT) wherein donor T cells target alloantigens on recipient tissues. It is unclear how alloimmune responses are maintained in GVHD despite abundant antigen, which causes T cell anergy, deletion and exhaustion. Previously, we identified alloreactive TCF-1 high T cells arising post-transplant that resemble exhausted progenitors (T EXP) capable of propagating immune responses in other chronic antigen models. Here, we sought to further characterize these cells in the B6→129 MHC-matched GVHD mouse model, in which 129 recipients express the immunodominant H-2K b-restricted minor histocompatibility antigen (miHA) H60. At day +7 post-transplant, alloreactive CD8 + cells specific to H60 (as determined by MHC-I-tetramer staining; Tet H60+) were nearly uniformly PD-1 hiTox hi whereas Tet H60- cells displayed a bimodal distribution into discrete PD-1 hiTox hi and PD-1 loTox lo populations, indicative of more diverse antigen experiences. Among these both Tet H60+ and Tet H60- cells were TCF-1 hi cells. TCF-1 hi Tet H60+ cells were uniformly CD39 loTox hiPD-1 hi, which is a canonical T EXP phenotype. In contrast, among activated Tet H60- cells there were TCF-1 hi cells that were CD39 loTox hiPD-1 hi and Tox loPD-1 lo. At later times in spleen and lymph node, and in GVHD target tissues, these populations of TCF-1 + Tet H60+ and Tet H60- were found. To test if these CD39 loTCF-1 hi T EXP had proliferative advantages in GVHD, we sorted congenic TCF-1 hiCD39 lo and TCF-1 loCD39 hi CD8 + cells from recipient spleens 14-days post-transplant and adoptively transferred them in competition in a 1:1 ratio (of Tet H60+ cells) into newly transplanted recipients. Among Tet H60+ cells in all tissues at day 14 post-transfer, TCF-1 hiCD39 lo-sorted progeny greatly outperformed TCF-1 loCD39 hi-sorted progeny. In line with their role as a source of GVHD effectors, progeny of TCF-1 hiCD39 lo cells were mostly TCF-1 loCD39 hi; however, a fraction remained TCF-1 hi consistent with their being able to undergo self-renewal. Conversely, we observed few if any TCF-1 + progeny of CD39 hi cells. We next tested whether TCF-1 was an important mediator of T cell fitness or whether it was only a marker for functionality. To do so we competed congenic wild-type (WT) and Tcf7 p45-/- (p45 -/-) donor CD8 cells, which lack the N-terminal β-catenin binding domain of TCF-1, in allogeneic (129) and syngeneic (B6) recipients. Strikingly, p45 -/- CD8 cells were greatly outcompeted by WT CD8 cells in 129 recipients in all tissues and at all times post-transplant, among both Tet H60+ and Tet H60- cells. In contrast, in B6 recipients, WT and p45 -/- cells remained evenly matched, suggesting that full-length TCF-1 isoforms are dispensable for lymphopenia-induced T cell expansion. Further, p45 -/- cells were also not disadvantaged when adoptively transferred into B6 mice and acutely challenged with H60 antigen by vaccination. Together these data suggest a model wherein TCF-1 hi progenitor like T cells are seeded in GVHD target organs where they may serve as a key local source for GVHD effectors, and moreover, full-length TCF-1 is itself critical for alloreactive T cell fitness in GVH responses.

Abstract:Abstract Introduction Dysregulation of interleukin 6 (IL-6) is pivotal for the development of graft-versus-host-disease (GVHD) after allogeneic stem cell transplantation. IL-6 drives many protective as well as inflammatory effects, the latter involving differentiation of T cells into pathogenic Th22, Th17, and Tc17 subsets. IL-6 signals through three principal pathways; classical, trans, and cluster, but their contribution to T cell differentiation and GVHD remains unclear. Importantly, these pathways are differentially targeted by clinical IL-6 inhibitors. Aims To dissect the contribution of the IL-6 signaling pathways to T cell differentiation and GVHD in order to guide therapy. Methods Donor bone marrow and T cells were transplanted from lineage-specific Cre × IL-6fl/flor IL-6Rfl/fl mice into irradiated, MHC disparate, wild-type or sgp130:Fc transgenic recipient mice with or without IL-6R mAb. Donor T cell differentiation and GVHD was assessed after transplant. Results Surprisingly, IL-6 trans-signaling was not required for T cell differentiation and GVHD, but instead regulated Th22 cell development. Indeed, inhibition of IL-6 trans-signaling in sgp130:Fc recipients promoted the expansion of Th22 cells and generated severe cutaneous GVHD. In contrast, IL-6 classical-signaling was critical for the development of Th22 and Th17 cells and its inhibition significantly attenuated GVHD broadly. Interestingly, unlike Th17 differentiation, Tc17 differentiation was highly IL-6-dependent but independent of classical and trans-signaling pathways, consistent with induction by cluster-signaling. Conclusion These data highlight cluster, in addition to classical IL-6-signalling as key therapeutic targets.
