Abstract:Recently, the sub-fertility of patients with adenomyosis, including infertility, abortion, premature delivery, stillbirth, fetal growth restriction, preeclampsia and the increase rate of cesarean section, has attracted much attention. Currently, adenomyomectomy is the priority option to treat adenomyosis patients with infertility following the failure of non-surgical conservative treatment of adenomyosis patients with infertility, such as medical therapy and high-intensity focused ultrasound. However, the clinical pregnancy success rate and complications after conservative surgical surgery for adenomyosis patients with infertility have always been controversial. Here, the difficulties, focal points and surgical indications of fertility-sparing surgery for patients with adenomyosis are put under the microscope, so as to provide reference for the treatment of adenomyosis patients with infertility.

Abstract:To study the clinicopathologic characteristics and the treatment means of pregnant women with uterine adenomyoma.Retrospective, consecutive, controlled study.University hospital for obstetrics and gynecology.Eighteen pregnant women with uterine adenomyoma.Data collection and statistical analysis.Eighteen pregnant women with uterine adenomyoma were diagnosed by excision of adenomyoma tissue during cesarean section and histopathology. The 18 subjects were retrospectively divided into treatment group (achieving this pregnancy by treatment; 10 cases) and control group (having no difficulty conceiving; 8 cases). The clinicopathologic characteristics and treatment means of the patients were analyzed retrospectively.The mean volume of uterine adenomyoma in the treatment group was larger than that of the control group. The methods of treating women with uterine adenomyoma-associated infertility include GnRH agonist (GnRH-a: 6 cases), GnRH-a and IVF and embryo transfer (3 cases), and traditional Chinese medicines (1 case).Severe uterine adenomyoma correlate with infertility in women of childbearing age. GnRH-a is effective in treating women with uterine adenomyoma-associated infertility.

Abstract:In recent years, surgical and non-surgical excision and drug therapy have replaced hysterectomy as the main therapeutic modalities for adenomyosis. It is suggested that the precise clinical diagnosis should be based on the reconstruction of digitized three-dimensional model with original image data of adenomyosis. Patients' age and clinical manifestations should also be considered, and the patients should be stratified according to reproductive requirements, so as to determine the best treatment. In view of the infiltration and diffuse growth of adenomyosis lesions in the myometrium of the uterus, it is suggested that long-term drug management should be adopted after surgical or non-surgical lesion resection.Gonadotropin releasing hormone agonists, levonorgestrel-releasing intrauterine system, dienogest and short-acting oral contraceptives should be recommended to consolidate the curative effect in order to delay the progress of the disease and prevent recurrence.

Abstract:Objective: To review systematically the literature on uterus-sparing surgical treatment options for adenomyosis. Design: Systematic literature review. Setting: Tertiary academic center. Patient(s): Women with histologically proven adenomyosis treated with uterus-sparing surgical techniques. Intervention(s): Conservative uterine-sparing surgery for adenomyosis classified as (1) complete excision of adenomyosis, (2) cytoreductive surgery or incomplete removal of the lesion, or (3) nonexcisional techniques, with studies selected if women with adenomyosis were treated surgically without performing hysterectomy. Main outcome measure(s): The cure rate after interventional strategies, the rate of symptom (dysmenorrhea and menorrhagia) control, and pregnancy rate in each group of intervention. Result(s): A quality assessment tool was used to assess the scientific value of each study. In total, 64 studies dealing with 1,049 patients were identified. After complete excision, the dysmenorrhea reduction, menorrhagia control, and pregnancy rate were 82.0%, 68.8%, and 60.5%, respectively. After partial excision, the dysmenorrhea reduction, menorrhagia control, and pregnancy rate were 81.8%, 50.0%, and 46.9%, respectively. Conclusion(s): Uterine-sparing operative treatment of adenomyosis and its variants appear to be feasible and efficacious. Well-designed, comparative studies are urgently needed to answer the multiple questions arising from this intriguing intervention.

Abstract:Adenomyosis is an intricate pathological condition that negatively impacts the uterus. It is closely related to the more well‐known endometriosis, with which it shares parallels in terms of diagnosis, therapy, and both microscopic and macroscopic features. The purpose of this narrative review is to give a clear univocal definition and outlook on the different, patient‐adapted, surgical treatments. MEDLINE and PubMed searches on these topics were conducted from 1990 to 2022 using a mix of selected keywords. Papers and articles were identified and included in this narrative review after authors' revision and evaluation. From the literature analysis, authors reported the following surgical techniques: laparoscopic double/triple‐flap method, laparotomic wedge resection of the uterine wall, laparotomic transverse H‐incision of the uterine wall, laparotomic wedge‐shaped excision, and laparotomic complete debulking excision by asymmetric dissection technique. Each of these techniques has strengths and weaknesses, but the literature data on the pregnancy rate are somewhat limited. The only certain information is the risk of uterine rupture up to 6.0% after surgical treatment for uterine adenomyosis. Over the years, the surgical approach continued to reach a positive result by minimally invasive treatment, with less hospitalization, less postoperative pain, and less blood loss. Over the years, the gynecological surgeon has gained the skills, training and increasingly sophisticated surgical techniques to target effective therapy. That's why a hysterectomy is no longer the only surgical resource to treat adenomyosis, but in patients who wish to preserve the fertility, there is a wide variety of surgical alternatives.

Abstract:Objective：To investigate the risk in the patients with adenomyosis after supracervical hysterectomy so as to guide the selection of operation on adenomyosis.Methods：Pathological examination were taken on the cutting edge of uterine specimens in the patients with adenomyosis after supracervical hysterectomy.Results：The incidence of ectopic endometrium appearing in the cuttingedge of uterine specimens was 12%.There was no statistically difference betweenpositive group and negative group about clinical symptoms, complicated endometriosis and the thickness of myometrium (P＞0.05).Yet, there was significantly difference between positive group and negative group about the longitudinal length of uterus (P＜0.05).Conclusions：The longer course of disease,diffuse foci,severe dysmenorrhea,complicated endometriosis may be the high risk factors for the positive incised edge of uterinespecimens, and the longer stump is directly relative to the positive incised edge of uterine specimens.It is advisable to perform total hysterectomy for the aged patients with adenomyosis companied by the high risk factors and to make conventionally pathological examination on the incised edge of uterine specimens in the patients with adenomyosis after supracervical hysterectomy.
Conservative surgical treatment for adenomyosis: New options for looking beyond uterus removal

Abstract:Adenomyosis is a common benign uterine disorders and patients may present dysmenorrhea, dyspareunia, abnormal uterine bleeding (AUB) and infertility. The treatment is very complex, including medical, surgical or radiological approaches. Hormonal drugs represent the first line therapy of adenomyosis, highly effective on symptoms and uterine volume reduction. Radiological procedures (UAE and HIFU), RFA and hysteroscopy may be proposed in those cases in which medical therapy is ineffective. Considering surgical treatment, hysterectomy remains the only existing definitive treatment but in the last decades the desire of uterus preservation is becoming more and more diffuse. On the other hand, surgical conservative treatments of adenomyosis are very effective in ameliorating AUB and pelvic pain and in reducing uterine volume, with some post-operative risks and obstetrics complications. Cytoreductive surgery for adenomyosis may be very complex, therefore it should be performed by experienced surgeons in dedicated centers, above all in case of concomitant endometriosis.

Abstract:Objective: Cystic adenomyoma is a lesion located within the myometrium. It is lined with endometrium and has hemorrhagic content. This rare entity has been described as a focal form of adenomyosis. However, it is poorly understood, and the management of symptomatic patients is not codified. Our objective is to evaluate the effectiveness of treatment with planned surgery for cystic adenomyoma on symptoms and fertility in a retrospective series of patients. Study design: This is a retrospective study between January 2011 and January 2018 in a university hospital of patients that had surgery for cystic adenomyoma with uterine preservation. Results: Pre-operative, per-operative, and post-operative data were recorded. Of the 18 patients included, 3 were lost to follow-up; 15 underwent laparoscopic surgery and 3 operative hysteroscopies. Complications during and after surgery were rare, involving only 3 patients, and of low severity. One patient saw no improvement in pain after surgery and 2 patients saw only partial improvement in their pain; 7 patients achieved at least one pregnancy with favorable outcome after surgery. Conclusion: Based on this series, we argue that conservative surgical treatment for cystic adenomyoma generally improves pain and fertility without causing morbidity.

Abstract:Adenomyosis is a benign condition commonly encountered in patients with infertility. While the definitive surgical management is hysterectomy, conservative surgical management is gaining attention in patients desiring future fertility. This review explores whether the surgical treatment of adenomyosis affects fertility outcomes for patients trying to conceive. The PubMed and Medline databases were searched using the keywords: "adenomyosis", "surgery", "radiofrequency", "infertility", "pregnancy", "sterility", "conception", "miscarriage", and "endometrial receptivity". s were screened, and relevant articles were selected for review. This review reveals that surgery appears to improve fertility outcomes with or without medical therapy; however, the risk of uterine rupture remains high and the best technique to reduce this risk is still not known. More studies are needed to formulate the best surgical approach for preserving fertility in treating adenomyosis and to establish standardized guidelines.

Abstract:Purpose of review: Adenomyosis is a benign gynaecological condition associated with menstrual symptoms and pelvic pain in premenopausal women, it has also been linked to subfertility. Improvement in ultrasound and MRI imaging has allowed accurate diagnosis of this condition prior to histological confirmation. Patients opting for surgical management need to be counselled regarding the surgical options available. This review summarizes the surgical management of adenomyosis and reviews the recent surgical developments. Recent findings: For patients who wish to preserve their fertility, cytoreductive surgery is an option in centres with surgical expertise. Recently, modified surgical procedures to remove extensive adenomyosis known as an adenomyomectomy have been described. These operations may enhance subsequent fertility. Summary: Access to accurate imaging modalities such as MRI and transvaginal ultrasound allows for adequate patient counselling preoperatively. Depending on the fertility requirements and the extent of adenomyosis, patients can be offered various surgical treatment options including laparoscopic hysterectomy or adenomyomectomy.

Abstract:Study Objective Enhancing precision in the management of women with adenomyosis interested fertility by optimizing diagnostic methods, surgical treatment, and intraoperative navigation via newly created 3-dimentional thin-slice uterine model combined with the data of functional MRI and MR-elastography. Design Level II, Canadian Task Force. Setting National Referral Center, Department of Operative Gynecology. Patients or Participants 150 patients of reproductive age with adenomiosis were admitted for surgical treatment and enrolled in cohort study. Groups were divided according to the form of adenomyosis (diffuse, nodular, or cystic). All participants had previous medical treatment, which was ineffective. Interventions Preoperative and intraoperative navigation in laparoscopic uterine reconstruction with utilization of new 3D- model of the adenomyotic uterus combined with the data of functional MRI and MR-elastography in patients with various forms of adenomyosis, interested in uterine preservation and following otherwise standard perioperative protocol. Measurements and Main Results Newly created thin-slice 3-D uterine model allowed for studying of the structural features and anatomical patterns in patients with adenomyosis, including the determination of the exact boundaries of the pathological process, the thickness of the uterine wall, the thickness of the Junctional Zone, density of diffuse and nodular adenomyosis, and circulation; and resulted in the optimization of the surgical tactic, reduction of intraoperative blood loss, reduced intraoperative decision-making time, and duration of surgery. All patients had improved quality of life (reduction of pain syndrome, menstrual blood loss, reproductive outcomes). 40% of patients had combined endometriosis and adenomyosis and 7% had congenital uterine malformations. Conclusion Newly created 3-D uterine model combined with the data obtained with functional MRI and MR-elastography allowed for higher level of precision in preoperative and intraoperative navigation of direction and extent of surgical resection/excision, enhanced by reliable assessment of precise nuances of myometrial circulation and adenomyotic involvement, leading to reliable and safe reconstruction of myometrium in patients with advanced adenomyosis, requiring surgical resection/excision.

The Effect of Uterine Adenomyosis on IVF Outcomes: a Systematic Review and Meta-analysis

Abstract:To investigate the impact of uterine adenomyosis on in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) clinical outcomes and also to evaluate the impact of gonadotropin-releasing hormone agonist (GnRHa) and surgical treatments. Infertile women with adenomyosis undergoing conventional IVF or ICSI were compared with women without adenomyosis. Fertility outcomes were evaluated in two patients with adenomyosis untreated and treated surgically or medically therapy. The review protocol was registered in PROSPERO (CRD42020214586). We consider the live birth rate (LBR) as the primary outcome. The analysis showed that women with adenomyosis had lower LBR (OR 0.59, 95% CI 0.37-0.92, p = 0.02), clinical pregnancy rate (OR 0.66, 95% CI 0.48-0.90), and ongoing pregnancy rate (OR 0.43, 95% CI 0.21-0.88) compared to those without adenomyosis, and miscarriage rate was higher in women with adenomyosis (OR 2.11, 95% CI 1.33-3.33). Surgical treatment increases natural conception in women with adenomyosis although the paucity of data (only two studies), and conversely, treatment with GnRHa does not increase the IVF outcomes. Women with adenomyosis have decreased IVF clinical outcomes. Pretreatment with the use of long-term GnRHa could not be beneficial, even though only three studies were included in the meta-analysis. The major part of the studies has not divided focal and diffuse adenomyosis, and this represents a relevant source of bias: studies conducted with standardized diagnostic criteria for adenomyosis are still needed to determine if the different clinical presentations of such condition could compromise IVF outcomes.

Abstract:Adenomyosis is a diffuse or localized organic disease caused by benign invasion of endometrial glands and stroma into the myometrium. It is a common disease that seriously affects reproductive health of women in childbearing age. Due to the unknown etiology and pathophysiological mechanism, and the lack of unified diagnostic criteria and effective treatment methods, total or subtotal hysterectomy has become a radical treatment for adenomyosis, which will lead to the complete loss of fertility. With the continuous exploration of the treatment to adenomyotic patients who have infertility or fertility intentions, new drugs, surgical methods and treating concepts appears. Adopt individualized conservative therapeutic strategies for patients with different conditions, preserve the uterus as much as possible and protect the patient's fertility, which will play an important role on the follow-up assisted reproductive treatment and long-term management of adenomyosis.

Abstract:The Brachyury (T) gene is required for formation of posterior mesoderm and for axial development in both mouse and zebrafish embryos. In this paper, we first show that the Xenopus homologue of Brachyury, Xbra, and the zebrafish homologue, no tail (ntl), both function as transcription activators. The activation domains of both proteins map to their carboxy terminal regions, and we note that the activation domain is absent in two zebrafish Brachyury mutations, suggesting that it is required for gene function. A dominant-interfering Xbra construct was generated by replacing the activation domain of Xbra with the repressor domain of the Drosophila engrailed protein. Microinjection of RNA encoding this fusion protein allowed us to generate Xenopus and zebrafish embryos which show striking similarities to genetic mutants in mouse and fish. These results indicate that the function of Brachyury during vertebrate gastrulation is to activate transcription of mesoderm-specific genes. Additional experiments show that Xbra transcription activation is required for regulation of Xbra itself in dorsal, but not ventral, mesoderm. The approach described in this paper, in which the DNA-binding domain of a transcription activator is fused to the engrailed repressor domain, should assist in the analysis of other Xenopus and zebrafish transcription factors.
Efficacy of Laparoscopic Adenomyomectomy Using Double-Flap Method for Diffuse Uterine Adenomyosis

Abstract:Background Adenomyomectomy has recently been considered the priority option for the treatment of adenomyosis, however, the surgical efficacy and modes are still debated. We aimed to evaluate the efficacy of laparoscopic adenomyomectomy using a double-flap method for the treatment of uterine diffuse adenomyosis when compared with conventional laparoscopic adenomyomectomy. Methods Laparoscopic adenomyomectomy using the conventional method (group A, n = 48) and the double-flap method (group B, n = 46) to treat diffuse uterine adenomyosis, respectively. Visual analog scale (VAS), menstrual amount, serum CA125 levels, and uterine volume were comparatively analyzed in both groups. Results The VAS scores, menstrual amount, serum CA125 levels, and uterine volume at 12 or 24 months after surgery significantly reduced in group B than in group A (P < 0.05); these parameters were statistically decreased in both groups after surgery compared with those obtained before surgery (P < 0.001). Moreover, serum CA125 levels and uterine volume at six months of follow up were significantly lower in group B than in group A (P < 0.01). In addition, blood loss during surgery was similar in groups A and B (P > 0.05), although the operative time was significantly longer in group B than that in group A (P < 0.05). Conclusions Laparoscopic adenomyomectomy using the double-flap method may be an effective technique to treat uterine diffuse adenomyosis.

However, it is now become a more common diagnosis with long-standing implications especially for fertility. In this literature review, the pathophysiology and diagnosis along with management options for uterine preservation and fertility along with more definitive options are reviewed. While there is a better understanding of adenomyosis, there is still more research that is needed to fully elucidate the best ways of management for patients especially in those seeking fertility.

Efficiency of routing and scheduling system for small and medium size enterprises utilizing vehicle location data

Abstract:ABSTRACT The routing and scheduling for trucks and vans in an urban road network depends critically on the state of the road network. Trucks and vans impose significant costs on other road users and the environment, so improved routing and scheduling benefits more than just the logistics industry. However, small and medium size enterprises (SMEs) in the logistics business cannot justify investment in planning systems. In this paper, an autonomous routing and scheduling system which is available to SMEs is proposed and the efficiency of the system is investigated. The proposed system accumulates vehicle location data in a central server and uses it to generate traffic information. Test simulations using a grid network demonstrate the effects of utilizing and sharing vehicle location data on delivery efficiency. The simulation results show that the improvement of delivery efficiency is mainly due to the reduction of penalty cost for early and late arrival at the customer location. It is also shown that the system leads to the buffer effect from variations in traffic conditions on delivery cost and this effect is enhanced by taking travel time uncertainty into consideration. It is further shown that the presence of measurement periods with insufficient data results in unreliable routing and scheduling. For a reliable system, data collection over a wider area is required rather than dense data in a subset of links.
A South African overview of the virus, vectors, surveillance and unique features of bluetongue.

Abstract:The origin of bluetongue (BT) is probably African and the disease was first recognised in South Africa in Merino sheep in the late 18th Century. Diagnostic and research findings for a number of years have been summarised to obtain data relevant to the distribution of BT and its serotypes in the country. The role of ruminant game and cattle as maintenance hosts for BT virus (BTV) is mentioned although cattle appear to have largely replaced antelope in this role. Only about 30% of over 1,000 game animals tested for export were found to be BT-antibody positive. An outbreak of a bluetongue-like disease in cattle is mentioned as are the BT and epizootic haemorrhagic disease of deer (EHD) isolates in the outbreak. A summary by serotype and province of sheep isolates is given and it is pointed out that the sheep population in a province does not reflect the number of isolates made and the province with the largest sheep population has almost the smallest number of BTV isolates and vice-versa. South Africa currently has 21 of the 24 BTV serotypes with 17, 20 and 21 being exotic to the country. The recent retrospective typing of serotype 17 in South Africa is being investigated, as type 17 crosses strongly with type 20, which is absent and also with type 4 which is present. 1, 3, 4 and 2 were the most common serotypes while 18, 19, 22 and 23 were not found among the isolates. Mention is made of BTV isolates made from Culicoides bolitinos catches during two devastating outbreaks of African horse sickness in an unvaccinated population. A six-year Culicoides monitoring project is mentioned and the many BTV isolates made of a variety of serotypes. BTV is endemic in Africa and in South Africa unvaccinated indigenous breeds appear to have achieved a balance with the virus. Indeed, it is possible to find virus, antibody and lesions in asymptomatic animals in different situations.
