Abstract:Nanofibers of VO2(A) with the diameter and length averagely at 100 nm and 10-20 μm were prepared via a facile one-step hydrothermal method by reducing NH4VO3 with 1,3-propylene glycol in an acidic solution. The obtained VO2(A) was coated by SiO2 to form VO2(A)@SiO2 core-shell nanocomposites, which were then transformed into VO2(M)@SiO2 by annealing under nitrogen atmosphere. The resulted composites maintained the original fibrous morphology, particularly with a large amount of pores emerging inside the fiber due to the volume shrinkage during the phase transition, which may improve its thermal insulation ability in real applications. The VO2(M)@SiO2 nanofibers were arranged into a self-supporting film by filtration, which shows excellent thermochromic properties.
Attention Deficits Detected in Cognitive Tests Differentiate Between Sleep Apnea Patients with or Without a Motor Vehicle Accident

Abstract:Objectives: Obstructive sleep apnea (OSA) is associated with an increased motor vehicle accident (MVA) risk. Conventional measures of OSA severity do not predict individual risk. Cognitive function tests have failed to incorporate outcomes in risk prediction. We aimed to identify markers of cognitive function for MVA risk prediction in OSA.Methods: OSA patients [n = 114, 75% male, median age 51 (43-61) years, body mass index (BMI) 30 (27-33) kg/m(2), apnea-hypopnea index 25 (6-49) n/h, and Epworth Sleepiness (ESS) score 11 (8-16)] were recruited from a sleep laboratory. Two cognitive function tests, the Attention Network Test (ANT) and a modified Oxford Sleep Resistance Test (OSLER) test (GOSLING), were assessed.Results: OSA patients with (n = 11) or without (n = 103) a MVA record in the Swedish traffic accident registry were identified. In patients with a MVA, 64% were commercial drivers. In patients with a MVA history, more lapses [42 (5-121) vs. 5 (1-25), P = 0.02] and fewer responses [238 (158-272) vs. 271 (256-277), P = 0.03] to stimuli in the ANT were found. In the GOSLING, the number of lapses was higher (29 (10-97) vs. 7 (2-19), P = 0.01) and the reaction time was longer [462 (393-551) vs. 407 (361-449) ms, P = 0.05]. OSA severity and ESS score poorly predicted MVAs (P > 0.2).Conclusions: We have demonstrated that deficit in sustained attention, assessed by daytime neurocognitive function tests, was associated with MVA risk in OSA patients. We were unable to detect an association between MVA history and severity of OSA or the ESS score. Predictors of driving risk in patients with obstructive sleep apnea syndrome treated by continuous positive airway pressure: a French multicenter prospective cohort

Abstract:Study objectives: To investigate the predictors of persistent driving risk related to sleepiness in patients with obstructive sleep apnea syndrome (OSAS) treated by continuous positive airway pressure (CPAP). Methods: Longitudinal analysis of a prospective national database including 5308 patients with OSAS and an indication of CPAP. Near misses related to sleepiness, accidents related to sleepiness, and sleepiness at the wheel (SAW) were assessed before initiation and after ≥90 days of treatment. Multivariable associations with the cumulative incidence of near-misses and accidents under treatment were calculated using Cox models adjusted for age, sex, obesity, sleep duration, SAW, accidents/near-misses history, depressive symptoms, residual apnea-hypopnea index, and adherence to treatment. Results: Residual SAW under treatment was associated with an eight-fold higher incidence of near-misses related to sleepiness (hazard ratios [HR] = 8.63 [6.08-12.2]) and five-fold higher incidence of accidents related to sleepiness (HR = 5.24 [2.81-9.78]). Adherence ≤4 h/night was also a significant predictor of persistent driving risk (HR = 1.74 [1.12-2.71] for near-misses and HR = 3.20 [1.37-7.49] for accidents). Conclusions: Residual SAW and treatment adherence ≤4 h/night are easy-to-assess markers to detect persistent driving risk during the follow-up evaluations of patients under treatment. Health professionals, but also policymakers, should be aware of the crucial importance of systematically evaluating these elements during the follow-up evaluations of the patients with OSAS treated by CPAP to better evaluate their driving risk.
Comparing the Driving Skills of Adolescents with Obstructive Sleep Apnea to Healthy Controls: The Results of a Case-Controlled Observational Study

Abstract:Auto crashes are a leading cause of death and injury among adolescents. Untreated obstructive sleep apnea (OSA) can cause sleepiness and inattention, which could negatively impact novice drivers, but OSA-related studies have focused on older drivers. This study used a driving simulator to examine whether licensed 16-19-year-old adolescents with OSA have diminished driving skills. Twenty-one adolescents with OSA and twenty-eight without OSA (both confirmed using polysomnography) completed two randomly ordered driving trials in a simulator (with induced distractions versus without). A mixed ANOVA examined the between-subjects effect of the OSA group, the within-subjects effect of the distraction condition, and the group-by-condition interaction effect on the ability to maintain lane position and the frequency of extended eye glances away from the roadway. T-tests were also used to examine group differences in reported sleepiness and inattention during daily life. The distraction task increased extended off-road glances and difficulties maintaining lane position (p < 0.001). However, adolescents with OSA did not display worse eye glance or lane position than controls and there were no significant group-by-condition interactions. Although the groups differed on polysomonographic features, there were also no significant differences in reported sleepiness or inattention. The distraction task negatively impacted both groups of adolescent drivers, but those with OSA did not fare differentially worse. Most adolescents in our study had mild OSA (median obstructive apnea-hypopnea index = 4.4), the most common form in the community. It remains possible that youth with more severe OSA would show increased driving impairment.
A Study of Moderate-severe Snoring and Sleepiness in Professional Drivers with Potential Effects on the Crash Risk

Abstract:To evaluate the prevalence of moderate-severe snoring and sleepiness in professional drivers with potential effects on the crash risk,449participants were involved in complete questionnaires and calculate the prevalence of moderate-severe snoring and sleepiness.Among 449drivers,20.9% were referred for moderate-severe snoring,12.2% were confirmed to have sleepiness.Regression analysie method identified the ratio of moderate-severe snoring[OR=7.714,95%confidence interval(95%CI)4.173～14.259],sleepiness[OR=2.138,95% CI1.044～4.379](P<0.05).The prevalence of moderate-severe snoring and sleepiness is high in professional drivers,which is an essential element in crash risk and attracting more attention with guidance about correlate administration department.

Abstract:Sleep-disordered breathing (SDB) is prevalent among professional drivers. Although SDB is a known risk factor for truck collisions attributed to microsleep-related behaviors at the wheel (TC-MRBs), the usefulness of overnight pulse oximetry for predicting TC-MRBs is debatable. This retrospective study assessed the association between overnight pulse oximetry parameters, the Epworth Sleepiness Scale (ESS), and TC-MRBs, confirmed by dashcam footage. This study included 108 matched professional truck drivers (TC-MRBs: N = 54; non-TC-MRBs: N = 54), with a mean age and body mass index of 41.9 ± 11.3 years and 23.0 ± 3.7 kg/m 2 , respectively. Night-time drivers, 4% oxygen desaturation index (ODI), and nadir oxygen saturation (SpO 2 ) were associated with TC-MRBs (odds ratio [95% confidence interval]: 25.63 [5.88–111.77], p < 0.0001; 2.74 [1.02–7.33], p = 0.045; and 3.87 [1.04–14.39], p = 0.04, respectively). The area under the curve of 4% ODI and nadir SpO 2 for TC-MRBs were 0.50 and 0.57, respectively. In conclusion, night-time driving, 4% ODI, and nadir SpO 2 were significantly associated with TC-MRBs in professional truck drivers. However, the sensitivity of overnight pulse oximetry parameters to predict TC-MRBs in a real-world application was poor. Therefore, combining subjective and objective assessments such as dashcam video footage may be needed to achieve high accuracy for predicting TC-MRBs among professional truck drivers.

Abstract:It will probably surprise many readers of this Journal to know that occupational exposures to chemicals are rarely assessed with quantitative data, either for epidemiology or for control of hazardous conditions. Only about 13% of epidemiology studies published in the 1980s, on the aetiology of chronic diseases, used any exposure measurements,1 and even those tended to rely on trivial sample sizes (median n = 4).2 Thus, uncertainty in exposure levels is the largest source of error in defining exposure–response relationships. Likewise, industrial surveys of air contaminants typically obtain only a single air measurement per occupational group.3 Surprisingly, this was not always the case. In the period between 1920 and 1960, engineers and health professionals routinely collected many air samples from a given working population, despite the primitive state of measurement technology.4–7 The likely motivation for such effort was the tremendous variability of air concentrations observed over time and among locations; thus, many measurements were needed to accurately assess exposures. The resulting databases allowed early investigators to delve into the sources and magnitudes of exposure, relationships between exposures and health outcomes, and strategies for comparing air levels with occupational exposure limits (OELs). Such applications are beautifully illustrated in a classic set of contributions to the British Journal of Industrial Medicine ( BJIM ), published more than 50 years ago. I vividly recall discovering these papers in the Berkeley library shortly after joining the faculty of the University of California in 1976. Abstract:Hepatic tumors unassociated with cirrhosis were encountered in seven patients aged 10 to 19 years. Four patients had received androgens for aplastic anemia. Two patients had transfusional hemosiderosis. One patient had had a renal transplant 2 1/2 years ago. Two patients are alive at 2 3/4 and 2 1/2 years after surgical resections. Nodules were found at autopsy in the others. The tumors were well differentiated and, in the androgen-related cases, differed from the others in the following features: canalicular bile retention, mild nuclear atypia, and acinar formation. No mitoses, vascular invasion, or metastatic tumor were evident. The clinical setting was variable; different factors, including iron overload and androgen therapy, played a role in the development of tumor. Although the androgen-related cases showed mild cellular atypia, biologic evidence of malignancy was lacking as in most previous reports.

Abstract:Driver drowsiness contributes to 10-20% of motor vehicle crashes. To reduce crash risk, ideally drivers would be aware of the drowsy state and cease driving. The extent to which drivers can accurately identify sleepiness remains under much debate. We systematically examined whether individuals are aware of sleepiness while driving, and whether this accurately reflects driving impairment, using meta-analyses and narrative review. Within this scope, there is high variability in measures of subjective sleepiness, driving performance and physiologically-derived drowsiness, and statistical analyses. Thirty-four simulated/naturalistic driving studies were reviewed. To summarise, drivers were aware of sleepiness, and this was associated to physiological drowsiness and driving impairment, such that high levels of sleepiness significantly predicted crash events and lane deviations. Subjective sleepiness was more strongly correlated (i) with physiological drowsiness compared to driving outcomes; (ii) under simulated driving conditions compared to naturalistic drives; and (iii) when examined using the Karolinska sleepiness scale (KSS) compared to other measures. Gaps remain in relation to how age, sex, and varying degrees of sleep loss may influence this association. This review provides evidence that drivers are aware of drowsiness while driving, and stopping driving when feeling 'sleepy' may significantly reduce crash risk.

Abstract:Background: The expression of acetylcholinesterase (AChE) could be induced during apoptosis in various cell types. And reduced AChE expression either by siRNA could prevent apoptosis. However, the detailed mechanisms underlying the AChE regulation are largely unknown in human breast cancer cell. Material and methods: MCF-7 cells were cultured and treated by cisplatin in the absence or presence of p53 siRNA. Results: In this study, the regulation of AChE expression during apoptosis induced by cisplatin, a current used anticancer drug, was investigated in human breast cancer cell line MCF-7. Exposure of MCF-7 cells to cisplatin resulted in apoptosis in a time- and concentration-dependent manner. Meanwhile, the upregulated AChE and p53 were also observed during apoptosis. Silencing interfering RNA directed against p53 blocked the expression of AChE. Conclusion: Taken together, these results suggested that AChE expression could be upregulated by the activation of p53 during apoptosis induced by cisplatin in MCF-7 cells.
Association between the risk of aberrant driving behavior and sleep disorder indices: A pilot study involving urban taxi drivers

Abstract:Aberrant driving behaviors (ADBs) may degrade road safety and pose significant dangers to both drivers and the public. Taxi drivers, who regularly work in complex urban environments for extended periods, are at a higher risk of ADBs. While many studies have focused on the development of ADBs, limited research has examined impacts of sleep-related factors. In this study, we investigated associations between the occurrence of ADBs and sleep-disordered indices. Thirty-one taxi drivers were recruited and monitored over four consecutive days and nights, with daytime driving behaviors tracked after nighttime sleep was assessed. The severity of obstructive sleep apnea (OSA) was measured using the apnea-hypopnea index (AHI) through hospital-based polysomnography, while sleep-disordered indices were recorded at home using wearable devices. Driving behavioral data were collected via navigation software. Statistical regression models were used to explore relationships between ADBs and sleep-disordered indices. Key findings include that drivers with severe OSA exhibited significantly higher ADB event frequencies per kilometer compared to those without severe OSA. Home-based sleep-disordered indices, such as higher cyclic variation of the heart rate index (CVHRI) and chest effort index (CEI), were significantly associated with an increased frequency of ADB events the following day, with respective beta coefficients of 0.062 and 0.111. These associations remained significant after adjusting for potential confounders. Our findings suggest a strong link between sleep-disordered indices and the occurrence of ADBs among taxi drivers, emphasizing the importance of addressing sleep health to reduce ADB risks.

Awareness of sleepiness and ability to predict sleep onset: can drivers avoid falling asleep at the wheel?

Abstract:Objectives: Regarding the causes of sleep-related accidents, this study assesses whether individuals can anticipate sleep onset accurately and how individuals acknowledge and use physiological and cognitive cues to make judgments related to sleep onset. Methods: A group of 41 partially sleep-deprived subjects predicted the likelihood of sleep in 30 consecutive two-minute intervals and noted physiological and cognitive signs of sleepiness, including involuntary eye closure, head-nodding, wandering thoughts, yawns, and instances of sleep, collectively referred to as "sleep complaints". Continuous polysomnographic recording compared these predictions to actual instances of sleep. Results: Subjects varied in their ability to predict sleep onset. For all subjects, the mean prediction of the likelihood of sleep prior to sleep was significantly higher than the mean prediction of the likelihood of sleep prior to intervals in which no sleep occurred (78% vs. 42%; p<.05). However, subjects tended to predict much lower likelihoods of sleep onset before their first sleep event (55%) than before later sleep events. On average, the rate at which subjects reported miscellaneous sleep complaints (such as head-nodding, eye closure, and wandering thoughts) was higher prior to sleep than prior to intervals in which sleep did not occur. Conclusion: Subjects who acknowledged a limited number of physiological and cognitive indicators of sleepiness tended to be poor predictors. Subjects whose physiological and cognitive signs of sleepiness failed to provide a strong indication of whether or not sleep onset would occur also tended to be poor predictors. Inability to judge sleep onset and, hence, susceptibility to sleep-related accidents, may be attributable to both a scarcity of meaningful warning signs and a failure to acknowledge the importance of physiological and cognitive indicators.

Abstract:Abstract Introduction Sleep dysfunction increases the risk of mild cognitive impairment (MCI) and Alzheimer’s Disease (AD). Emerging sensor technologies can index real world (RW) sleep and instrumental activities of daily living affected by neurodegeneration, as outlined below. Methods 85 legally licensed active drivers with cognitive decline (76 MCI, 9 mild AD) (mean age = 75.6 years; 37 females) and 54 age and education-matched controls (mean age= 74.7 years; 36 females) participated. Cognitive status was assessed using standardized neuropsychological tests according to NIA-AA guidelines. Sensor systems installed in participants’ own vehicles recorded driving incidence (driving or not each day). Sleep was recorded using wrist-worn actigraphy, and verified with self-reported sleep diaries over three continuous months. A mixed-effect logistic regression analyzed daily driving incidence with predictors including z-scores of total sleep time (TST) and sleep efficiency (SE) in previous night, and cognitive status (MCI or AD v. controls), adjusted for age, gender, employment status, and driving season. Results From 9,214 days of data on nightly sleep and next day driving, increase in TST significantly reduced driving incidence (21% per standard deviation [SD]; Odds Ratio [OR] = 0.79, 95% Confidence Interval [CI] = 0.74 – 0.84, p < 0.001). Increase in SE was associated with increased driving incidence among those with cognitive decline, compared to controls (15% per SD; OR = 1.14, 95% CI = 1.03 – 1.27, p = 0.01). Conclusion Findings underscore the value of RW digital biomarkers to track the effects of neurodegeneration on sleep and instrumental activities of daily living. Deciding to drive increased with greater sleep efficiency, particularly for those with cognitive decline. Quantitative profiles of sleep and driving behavior offer novel opportunities for early detection of the effects of AD. Effects of scheduled manual driving on drowsiness and response to take over request: A simulator study towards understanding drivers in automated driving

Abstract:Because current automated vehicles have operational limitations, it is important to ensure that the fallback-ready driver is able to perform appropriately when required to take over control of the vehicle. However, time-related increase in driver drowsiness is well-known, and drowsy driving can affect response to take-over request (TOR). It was previously reported that a scheduled period of manual driving during automated driving was beneficial in maintaining driver arousal level. The present driving simulator study investigates the effects of scheduled manual driving on driver drowsiness and performance, as well as age differences therein. A total of 115 participants, whose gender was balanced and age was distributed uniformly from 20 to 70 years, drove an automated vehicle for 31 min, and a TOR was prompted before a collision event. A between-subjects design comprised two conditions: with versus without a scheduled 10-min interval of manual driving that ended 10 min before TOR. The Karolinska Sleepiness Scale and eyeblink durations estimated from electrooculograms (EOG) were used to subjectively and objectively measure participant's drowsiness. Reaction time, standard deviation of steering wheel angle, and minimum Time-to-Collison (TTC) were extracted to measure driver performance in response to TOR. The alleviating effect on drowsiness of 10-min scheduled manual driving became non-significant after another 10-min period of automated driving. Although the scheduled manual driving had no significant effect for younger drivers, older drivers reacted significantly more slowly in both steering and braking at the critical event. These findings provide essential insights for human-vehicle interactions: Scheduled manual driving cannot maintain drivers' arousal level for 10 min afterwards, and for older drivers, it would be better to avoid unnecessary task-switching between manual and automated driving.
The impact of sleep disorders on driving safety—findings from the Second Strategic Highway Research Program naturalistic driving study

Abstract:Study Objectives: This study investigated the association between driving safety and seven sleep disorders amongst 3541 participants of the Second Strategic Highway Research Program (SHRP 2) naturalistic driving study.Methods: SHRP 2 collected naturalistic driving data from participants between 16 and 98 years old by instrumenting participants' vehicles. The analyses used logistic regression to determine the likelihood of crash or near-crash involvement, Poisson log-linear regression to assess crash or near-crash rate, and ordinal logistic regression to assess driver maneuver appropriateness and crash or near-crash severity. These analyses did not account for any medical treatments for the sleep disorders.Results: Females with restless legs syndrome/Willis-Ekbom disease (RLS/WED), drivers with insomnia or narcolepsy, are associated with significantly higher risk of crash or near-crash. Drivers with shift work sleep disorder (SWSD) are associated with significantly increased crash or near-crash rate. Females with RLS/WED or sleep apnea and drivers with SWSD are associated with less safe driver maneuver and drivers with periodic limb movement disorder are associated with more severe events. The four analyses provide no evidence of safety decrements associated with migraine.Conclusions: This study is the first examination on the association between seven sleep disorders and different measures of driving risk using large-scale naturalistic driving study data. The results corroborate much of the existing simulator and epidemiological research related to sleep-disorder patients and their driving safety, but add ecological validity to those findings. These results contribute to the empirical basis for medical professionals, policy makers, and employers in making decisions to aid individuals with sleep disorders in balancing safety and personal mobility.

Abstract:Drowsy drivers cause the most car accidents thus, adopting an efficient drowsiness detection system can alert the driver promptly and precisely which will reduce the numbers of accidents and also save a lot of money. This paper discusses many tactics and methods for drowsy driving warning. The non-intrusive nature of most of the strategies mentioned and contrasted means both vehicular and behavioural techniques are examined here. Thus, the latest strategies are studied and discussed for both groups, together with their benefits and drawbacks. The goal of this review was to identify a practical and low-cost approach for analysing elder drivers' behaviour.
A New Screening Tool (BAMSA) for Sleep Apnea in Male Professional Truck Drivers

Abstract:Obstructive sleep apnea (OSA) is common in professional truck drivers. It is important that OSA is recognized since undiagnosed and/or untreated sleep apnea is a risk factor for sleepiness-related traffic accidents. In this study, we developed a new simple tool to screen for obstructive sleep apnea (OSA) in this population. Altogether, 2066 professional truck drivers received a structured questionnaire. A total of 175 drivers had a clinical examination and were invited to participate in sleep laboratory studies, including cardiorespiratory polygraphy. We studied associations of different risk factors with the presence of sleep apnea. We established a new simple screening tool for obstructive sleep apnea (OSA) that was compared to other existing screening tools. A total of 1095 drivers completed the questionnaire. Successful cardiorespiratory polygraphy was obtained for 172 drivers. Full data were available for 160 male drivers included in the analyses. The following five risk factors for sleep apnea formed the BAMSA score (0 to 5): BMI > 30 kgm−2, age > 50 years, male gender, snoring at least one night per week, and the presence of apnea at least sometimes. BAMSA showed a sensitivity of 85.7% and a specificity of 78.8% in detecting AHI ≥ 15 when using a cut-off point of 4, and the ROC area was 0.823. BAMSA is a sensitive and easy-to-use tool in predicting obstructive sleep apnea in male professional drivers.

Abstract:Study objectives: Obstructive sleep apnea (OSA) is considered a risk factor for sleepiness at the wheel (SW) and near-miss accidents (NMA). To date, there are subjective and objective methods such as the Maintenance of Wakefulness Test (MWT) to investigate sleepiness. However, these methods have limitations. Therefore, a new analysis of the MWT was introduced based on the identification of microsleeps (MS). So, we tested MS analysis to improve the discriminative ability of MWT in recognizing individuals at risk for SW and NMA in a population with OSA. Methods: The study was conducted on 100 naïve patients with suspected OSA referred to our Sleep Medicine Unit. All patients performed a full-standard polysomnography and MWT. The MWT was analyzed according to standard criteria, and the presence of MS episodes, the mean MS latency and the MS density (the mean absolute or relative number of MS) were assessed. Results: Microsleeps were observed in 100% of alert or sleepy patients and 47% of the fully alert (P <0.0001). Almost 90% of patients reporting NMA showed episodes of MS during MWT. The occurrence of NMA was related to EDS, MS latency and MS density (P <0.001). The discriminative power for the NMA of MS density measures was higher than that derived from latency analysis, particularly in patients without EDS and with a simultaneous mean sleep latency >33 minutes. Conclusions: MS analysis provides objective evidence of sleepiness and, therefore, could improve the discriminative ability of the MWT in recognizing individuals at high risk for accidents.

Abstract:It is challenging to determine which patients with obstructive sleep apnea (OSA) have impaired driving ability. Vulnerability to this neurobehavioral impairment may be explained by lower brain metabolites levels involved in mitochondrial metabolism. This study compared markers of brain energy metabolism in OSA patients identified as vulnerable vs resistant to driving impairment following extended wakefulness. 44 patients with moderate-severe OSA underwent 28hr extended wakefulness with three 90min driving simulation assessments. Using a two-step cluster analysis, objective driving data (steering deviation and crashes) from the 2nd driving assessment (22.5 h awake) was used to categorise patients into vulnerable (poor driving, n = 21) or resistant groups (good driving, n = 23). 1 H magnetic resonance spectra were acquired at baseline using two scan sequences (short echo PRESS and longer echo-time asymmetric PRESS), focusing on key metabolites, creatine, glutamate, N-acetylaspartate (NAA) in the hippocampus, anterior cingulate cortex and left orbito-frontal cortex. Based on cluster analysis, the vulnerable group had impaired driving performance compared with the resistant group and had lower levels of creatine (PRESS p = ns, APRESS p = 0.039), glutamate, (PRESS p < 0.01, APRESS p < 0.01), NAA (PRESS p = 0.038, APRESS p = 0.035) exclusively in the left orbito-frontal cortex. Adjusted analysis, higher glutamate was associated with a 21% (PRESS) and 36% (APRESS) reduced risk of vulnerable classification. Brain mitochondrial bioenergetics in the frontal brain regions are impaired in OSA patients who are vulnerable to driving impairment following sleep loss. These findings provide a potential way to identify at risk OSA phenotype when assessing fitness to drive, but this requires confirmation in larger future studies.
