Abstract:Objective A small animal model would be an effective tool for research on the pathophysiology of cardiopulmonary bypass (CPB). However, numerous CPB models do not involve myocardial arrest and resuscitation. The aim of this research is to establish an easily achievable myocardial arrest and resuscitation CPB model through hyperkalemia and landiolol, simulating clinical cardiac surgery. Materials and methods Ten Sprague-Dawley rats were chosen for CPB. Rats underwent sevoflurane inhalation induction anesthesia and were sustained in an anesthesia state by intubation and intraperitoneal injection’s of esketamine and propofol. The entire CPB circuit include a reservoir, a membrane oxygenator and a roller pump, which were connected into a complete loop via silicon tubes and infusion tube. After CPB was established through the tail artery and internal jugular vein, cardioplegic arrest was induced and maintained for 5 min at a rectum temperature of 28.5 ± 0.5°C with hyperkalemia and landiolol. Calcium chloride, epinephrine and insulin were then used for resuscitation. Result All rats successfully finished cardioplegic arrest, resuscitation procedure and survived 2 h postoperatively. Mean hematocrit during CPB was significantly lower than physiologic values of the baseline. The mean time of arrest-resuscitation and CPB was 5.4 ± 0.8 min and 98.5 ± 5.0 min. The blood gas at each detection point were in range with the normal standard requirement of CPB. Conclusion The establishment of cardioplegic arrest and resuscitation procedure via hyperkalemia and landiolol during CPB of WD rat could be achieved successfully. This animal model could be an alternative organ injury research on organ injury of patients undergoing cardiac surgery.
Establishment of a novel rat model without blood priming during normothermic cardiopulmonary bypass

Abstract:Objective: An effective animal model was needed for research on the pathophysiology of cardiopulmonary bypass (CPB). Rat models were considered suitable for research into CPB, recently. The aim of the article is to establish a simple and safe CPB model without blood priming in rats, containing the advantages of controlling temperature precisely, being similar to the clinical process and laying the foundation for the further study of a deep hypothermic circulatory arrest (DHCA) model. Materials and Methods: Ten Sprague-Dawley rats, divided into a CPB group (n=7) and a sham group (n=3), received sevoflurane inhalation anesthesia and were maintained in an anesthesia state by intubation. The entire CPB circuit consisted of a reservoir, a membrane oxygenator, a roller pump, a heat exchanger and a heat cooler, all of which were connected via silicon tubes. The volume of the priming solution, composed of 6% HES130/0.4 and 125 IU heparin, was less than 12 ml. In the CPB group, a 22G catheter was placed in the left femoral artery for monitoring arterial blood pressure, a 20G catheter was placed in a tail artery for arterial inflow and a homemade, multiorificed catheter was inserted into a right jugular vein for venous drainage. After 90 minutes, the CPB process was terminated when vital signs were stable. In the sham group, the same surgical process was conducted except for the venous drainage. Post-oxygenator blood gas and hemodynamic parameters were measured at each time point before CPB, during CPB and after CPB. Results: All CPB processes were successfully achieved. Blood gas analysis and hemodynamic parameters of each time point were in accordance with normal ranges. The vital signs of all rats were stable. Conclusion: The establishment of CPB without blood priming in rats can be achieved successfully. The rat model could be used to study short-term or long-term organ injury mechanisms caused by CPB. Furthermore, on the basis of the precise control of temperature and the depth of anesthesia, the DHCA model in rats could be developed further to study pathophysiological changes of neurological and other organ functions in the future.
A Long-Term Survival Model of 40-Min Deep Hypothermic Circulatory Arrest in Rats

Abstract:Objective To establish a long-term survival model of deep hypothermic circulatory arrest (DHCA) in rats,which could contribute to the research of organ damage mechanism and clinical treatments related to DHCA.Methods Twenty Sprague-Dawley rats were randomly (random number) divided into the sham group (n=10) and DHCA group (n=10).After anesthesia,a 20 G catheter was cannulated in the tail artery for arterial inflow,a multiorificed catheter in the right jugular vein for venous drainage,and a 24G catheter in the branch of left femoral artery for artery blood pressure monitoring.Rats in the DHCA group underwent DHCA procedure for 40 min after brain temperature cooled to 18℃,then rewarmed for 40 min,till the brain temperature were above 34℃.Rats in the sham group were cannulated but did not undergo cardiopulmonary bypass (CPB).Hemodynamic parameters and blood gas analysis were measured for 5 times (pre-CPB,15 min after CPB,10 min after rewarming,40 min after rewarming,and 30 min after CPB).Results One rat in the DHCA group died,and the rest rats survived.The lactate level in the DHCA group after rewarming during operation was significantly higher than that in the sham group (7.84 mmol/L vs 1.93 mmol/L,P＜0.05).Conclusions In this study,40-min DHCA model in rats is characterized by safe and long-term survival.

Abstract:Elucidating the underlying mechanisms and developing protective strategies for the pathophysiological consequences of cardiopulmonary bypass (CPB) have been hampered due to the absence of a satisfactory long-term recovery animal model. The objective of this study was to establish a survival experimental model of CPB in rats to meet the requirement of these studies.Male SD rats (450-550 g) were randomly divided into CPB (n = 10) group and Sham group (n = 10). All rats were anaesthetized and mechanically ventilated. The femoral artery and vein were cannulated for continuous blood pressure recordings and fluid replacement, respectively. The CPB circuit comprised a venous reservoir, a membrane oxygenator, and a roller pump. Blood was drained from the right atrium via a jugular vein catheter and returned to the right carotid artery. Priming consisted of 8 ml of homologous blood and 8 ml of colloid. CPB was conducted for 60 min at a flow rate of 100-150 ml/kg/min in the CPB group. Haemodynamic investigations, blood gas analysis, and survival studies were performed subsequently.Our data show that the rat model principally simulated the clinical setting of CPB in terms of its construction, configuration, performance, material surface area, and priming volume to blood volume ratio. All CPB rats survived and the 2-week follow-up period remained uneventful.The rat model of CPB was easy to establish and was associated with excellent survival. This model should facilitate the investigation of the pathophysiological processes concerning CPB-related multiple organ dysfunction and possible protective interventions.
Effects of deep hypothermia circulation arrest on morphologic changes of rat brain

Abstract:Objective To develop a new rat model of deep hypothermia circulation arrest (DHCA) that functionally mimic the clinical setting and to study the effects of DHCA on the morphologic changes of the rat brain. Methods Male Sprague-Dawley rats were randomly divided into two groups, experimental group and the sham-operation group. An-esthesia was induced using a mixture of fentanyl, ketamine and droperidol, then the rat was intubated through the mouth under direct vision. The rats of the experimental group underwent extracorporeal circulation circuit priming without blood which consisted of 10 ml of Ringer lactate solution, 20% mannite 1 ml, heparin 1 ml (100 U) and 6% HES 8 ml. The body temperature of the rats was reduced to 18℃ and then paused the extracorporeal circulation circuit for 90 min. Blood gas analysis, blood pressure and heart rate monitorings were performed during the experiment. After 60 min of rewarming and resuming the extracorporeal circulation circuit, the rats were sacrifical perfused and the brains were dissected for examination by light microscopy and electronmicroscopy. Results Three rats of the experimental group died because of failure of jugular vein cannulation, the rest passed the experiment thoroughly and uneventfully. The hematocrit was about 20% after the extracorporeal circulation circuit. Degeneration and necrosis of the neurons could be found at the hippocampus, cortex and the thalamus after DHCA. With electronmicroscopy, swelling of mitochondria was found and organelles in the cytoplasm reduced or even disappeared. Conclusions The entire circuit is similar to clinical setting in terms of its construction, configuration, performance and priming volume to blood volume ratio. The new rat model of DHCA is easy to create and the morphologic changes have been proved to be a successful rat model for future study of neurological injury under DHCA.
The Establishment of Cardioplegic Arrest Model on the Basis of General Cardiopulmonary Bypass in Rats

Abstract:Objective:To establish cardioplegic arrest(CA) model on the basis of general cardiopulmonary bypass(CPB) in rats;to compare the myocardial ischemia-reperfusion injury related to CA and CPB and to observe the survival and the recovery status of CA in rats.Method:A double-lumen balloon catheter was cannulated through the right carotid artery into the ascending aortic artery.The cardioplegic arrest was achieved by intraaortic clamping through balloon inflation and by cardioplegia infusion.To compare the myocardial ischemia-reperfusion injury related to CA and CPB and respectively observe the survival and the recovery of CA rats at 1 day,1 week and 2 weeks after cardioplegic arrest.Result:At sixty minutes after cardiac arrest,HFABP and cTnI were significantly increased compared with baseline and the rats in CPB group,reached peak value at 3 hours after reperfusion,reduced 1 day postoperatively and returned to baseline levels at 1 and 2 week.Left ventricular ejection fraction didn't show obvious change between CA and CPB group;E/A ratio was reduced after CPB and further reduced after CA.All rats survived with significant body weight loss 1 day and 1 week postoperatively but fully recovered 2 weeks after surgery.Conclusion:The cardioplegic arrest model is successfully established on the basis of general CPB model in rats.CA rats showed significant myocardial injury but presented a satisfactory survival rate.
Establishment and identification of a hypoxia-ischemia brain damage model in neonatal rats.

Abstract:The present study was designed to set up a reliable model of severe hypoxia-ischemia brain damage (HIBD) in neonatal rats and several methods were used to identify whether the model was successful. A total of 40 healthy 7-day-old Sprague-Dawley rats were randomly divided into 2 groups: The sham-surgery group (n=18) and the HIBD model group (n=22). The HIBD model was produced according to the traditional Rice method. The rats were anesthetized with ethyl ether. The left common carotid artery (CCA) was exposed, ligated and cut. Following this, the rats were exposed to hypoxia in a normobaric chamber filled with 8% oxygen and 92% nitrogen for 2 h. In the sham-surgery group, the left CCA was exposed but was not ligated, cut or exposed to hypoxia. The neurobehavioral changes of the rats were observed in the 24 h after HIBD. The brains were collected after 72 h to observe the pathological morphological changes of the brain tissue. The behavioral ability and neurobehavioral changes were studied in each group. The water maze test was used for evaluating the learning-memory ability when the rats were 28 days old. Compared with the sham-surgery group, all the HIBD model rats had a lag of motor development. The rats had evident changes in anatomy and Nissl staining, and cognitive impairment was shown through the result of the water maze. Therefore, the model of HIBD in neonatal rats is feasible and provides a reliable model for subsequent studies.
Histidine-trytophan-ketoglutarate cardioplegia reduces inflammatory response and serum levels of myocardial enzymes in newly developed right-thoracotomy rat model

Abstract:The objective of this research was to establish a rat model for cardiopulmonary bypass (CPB) with cardiac arrest and resuscitation that is both practical and economical and simulates clinical cardiac surgery. Concurrently, the study aimed to evaluate the myocardial protective effects conferred by histidine–tryptophan–ketoglutarate (HTK) cardioplegia. Thirty rats were randomly assigned to three groups: the histidine–tryptophan–ketoglutarate (HTK), 4:1 blood cardioplegia (BC) and del Nido cardioplegia (DN) groups. The cardiopulmonary bypass (CPB) procedure was implemented and sustained for a duration of one hour. Subsequent to the cessation of CPB, the rats were subjected to monitoring and observation for an additional two hours. Following this observation period, the heart and blood samples were procured for subsequent analysis. During CPB, the average hematocrit level was significantly below the typical physiological range ( P < 0.001). Histopathological scores were notably lower in the HTK group in contrast to the BC group ( P < 0.001) or the DN group ( P < 0.001). At 2 h after weaning off CPB, the levels of CK and CKMB in the DN and BC groups were notably elevated compared to the HTK group ( P < 0.001). The levels of IL-6 and TNF-α proteins were notably increased in all three groups ( P < 0.001), with the BC and DN groups showing higher increases compared to the HTK group ( P < 0.001). This compact animal model of cardiopulmonary bypass (CPB) with cardiac arrest and resuscitation might allow for both the study of myocardial ischemia-reperfusion injury as well as cardioprotective strategies. HTK cardioplegia could reduce inflammatory response and serum levels of myocardial enzymes in this newly developed right thoracotomy rat model.

Abstract:Objective To establish an animal model of closed chest cardiopulmonary bypass (CPB) in Sprague-Dawley (SD) rats. Methods The SD rats were assigned to undergo CPB via right carotid and jugular cannulation,and self-designed CPB circuit.The cardiac arrest was induced by hypothermal ventricular fibrillation.Twenty-four rats were equally divided into three groups which underwent CPB for 120 minutes,deep hypothermic low flow (DHLF) bypass 60 minutes,deep hypothermic circulatory arrest (DHCA) 60 minutes individually.During bypass,invasive arterial blood pressure,electrocardiograph and arterial and mixed venous blood gas analysis were monitored. Results The CPB was performed successfully in all the rats.The blood gas analysis results were all maintained in normal range and the mean arterial pressure was over 50 mmHg when off-pump.The cardiac and respiratory functions recovered after the rat weaned from the CPB. Conclusions The animal model of closed chest CPB can be established successfully in rats with excellent survival.This model is a stable platform for the study of the pathophysiology of CPB and post-CPB multiple organ dysfunctions.
Neurologic Recovery after Deep Hypothermic Circulatory Arrest in Rats: A Description of a Long-Term Survival Model Without Blood Priming

Abstract:Brain injury associated with deep hypothermic circulatory arrest (DHCA) has been recognized in patients with congenital heart diseases and those undergoing aortic arch surgeries. However, the preclinical investigation of long-term cerebral injury and recovery mechanisms related to DHCA has been restricted to a satisfactory recovery animal model with a determined recovery time. This study aimed to evaluate the feasibility of a long-term surviving DHCA model without blood priming in rats, in order to investigate the pathophysiology of long-term complications in further studies. Twelve Sprague-Dawley rats were divided into 2 groups: sham group (n = 3) and DHCA group (n = 9). The DHCA group was further assigned to the surviving time of postoperative day 2, 14, and 30 (n = 3, respectively). The entire cardiopulmonary bypass (CPB) circuit consisted of a modified reservoir, a custom-designed small-volume membrane oxygenator, a roller pump and a heater/cooler. A 24-G catheter was cannulated in the branch of the left femoral artery for arterial blood pressure monitoring. Cardiopulmonary bypass was established via the right external jugular vein-right atrium and tail artery. Rats were cooled to a rectal temperature of 18 degrees C, followed by 30 minutes of DHCA with global ischemia. After re-warming for approximately 40 minutes, the animals were weaned from the CPB at 35.5 degrees C. Blood gas and hemodynamic parameters were recorded preoperatively and intraoperatively, and at 2, 14, and 30 days postoperatively. Thereafter, the brains were perfusion fixed and histologically analyzed. All DHCA processes were successfully achieved, and none of the rats died. Blood gas analysis and hemodynamic parameters at each time point were normal, and vital signs of all rats were stable. Histopathologic deficits in the hippocampus (pathological score, surviving hippocampal neurons, and Ki67-positive neurons) manifested after 30 minutes of DHCA, which persisted for at least 14 days and recovered after 30 days. A novel and simple long-term recovery model of DHCA in rats was established in the present study, and histopathologic deficits were observed after clinically relevant 30-minute DHCA durations, in order to determine the 30-day recovery time frame.
[Effect of Postconditioning on Rat Hearts Suffered from Long-Term Hypothermic Preservation].

Abstract:OBJECTIVE:To assess the effect of postconditioning on cardiac protection of rat hearts suffered from long-term hypothermic preservation.METHODS:The Langendorff model of isolated rat heart was used. After 30 min of stabilization, the hearts were stored in 4 degrees C Celsior solution for 3 or 5 h followed by 60 min of reperfusion. Postconditioning was initiated by 3 cycles of 30 s ischemia followed by 30 s reperfusion at the beginning of subsequent persistent reperfusion. The recovery of cardiac contractile function and arrhythmia score were observed.RESULTS:(1) Compared with control group, postconditioning increased the recovery of heart rate (HR), left ventricular systolic pressure (LVDP), maximal rise/fall rate of ventricular pressure (dP/dt(max)) and coronary flow (CF) and rate-pressure product (RPP) during reperfusion after 3 h of hypothermic preservation. However, left ventricular end-diastolic pressure (LVEDP) and the cardiac arrhythmia score during the first 10 min of reperfusion was significantly lower in 3 h postconditioning group than that in 3 h control group. (2) The rat hearts treated by postconditioning with 5-HD(100 micromol/L) abolished the amelioration of contract function induced by postconditioning. And it could also increase the cardiac arrhythmia score. (3) Compared with 5 h control group, the HR, LVDP,dP/dt(max), CF, LVEDP, RPP and the cardiac arrhythmia score were not significantly different in postconditioning treated hearts during reperfusion after 5 h of hypothermic preservation.CONCLUSION:Postconditioning could provide the cardiac protection on 3 h hypothermic preserved rat hearts,but not on 5 h hypothermic preserved rat hearts. The cardiac protection effect might be partly associated with activation of selective mitochondrial ATP-sensitive potassium channel.

Abstract:Neurologic injury after cardiopulmonary resuscitation is the main cause of the low survival rate and poor quality of life among patients who have experienced cardiac arrest. In the United States, as the American Heart Association reported, emergency medical services respond to more than 347,000 adults and more than 7,000 children with out-of-hospital cardiac arrest each year. In-hospital cardiac arrest is estimated to occur in 9.7 per 1,000 adult cardiac arrests and 2.7 pediatric events per 1,000 hospitalizations. Yet the pathophysiological mechanisms of this injury remain unclear. Experimental animal models are valuable for exploring the etiologies and mechanisms of diseases and their interventions. In this review, we summarize how to establish a standardized rat model of asphyxia-induced cardiac arrest. There are four key focal areas: (1) selection of animal species; (2) factors to consider during modeling; (3) intervention management after return of spontaneous circulation; and (4) evaluation of neurologic function. The aim was to simplify a complex animal model, toward clarifying cardiac arrest pathophysiological processes. It also aimed to help standardize model establishment, toward facilitating experiment homogenization, convenient interexperimental comparisons, and translation of experimental results to clinical application.
Early Initiation of Continuous Renal Replacement Therapy Induces Fast Hypothermia and Improves Post-Cardiac Arrest Syndrome in a Porcine Model.

Abstract:Rapid induction of hypothermia early after resuscitation can be an effective strategy against post-cardiac arrest syndrome (PCAS). Preliminary data suggested that continuous renal replacement therapy (CRRT) might be an efficient method to rapidly induce hypothermia. In this study, we investigated the efficacy of cooling induced by CRRT and its effects on the outcomes of PCAS in a porcine model.Thirty-two male domestic pigs weighing 36 ± 2 kg were randomized into 4 groups: sham control (n = 5), normothermia (n = 9), surface cooling (SC, n = 9), and CRRT (n = 9). Sham animals underwent the surgical preparation only. The animal model was established by 8 min of untreated ventricular fibrillation and then 5 min of cardiopulmonary resuscitation. At 5 min after resuscitation, the animals were cooled by either the combination of an earlier 8-h CRRT and later 16-h SC or the whole 24-h SC in the two hypothermic groups. For the other two groups, a normal temperature of 38.0 ± 0.5°C was maintained throughout the experiment.Blood temperature was decreased to 33°C within 28 min in animals treated with CRRT, which was significantly faster than that in the SC group requiring 185 min to achieve target temperature. Post-resuscitation myocardial dysfunction, brain injury and systemic inflammation were significantly improved in the two hypothermic groups compared to the normothermia group. However, the improvement was significantly greater in the CRRT group than in the SC group.In conclusion, fast hypothermia was successfully induced by CRRT and significantly alleviated the severity of PCAS in a porcine model.
Remote Ischemic Postconditioning Alone and Combined with Therapeutic Hypothermia Improve Neurological Outcomes after Cardiopulmonary Resuscitation in A Porcine Model

Abstract:Introduction: The degree of post-resuscitation cerebral injury is an important determinant on the outcomes for CA victims.We previously demonstrated remote ischemic pre- and post-conditioning equally attenuated post-CPR cerebral dysfunction in a rat model. Here we investigated the effects of remote ischemic post-conditioning (RIpostC) alone and combined with therapeutic hypothermia (TH) on the neurological outcomes after CPR. Hypothesis: RIpostC would improve post-CPR neurological function and cerebral injury, which would be enhanced by its combination with TH. Methods: Twenty-one male domestic pigs weighing 37 ± 2 kg were subjected to 10 mins of untreated VF followed by 5 mins of CPR. The animals were randomized to RIpostC, RIpostC+TH or control. Coincident with the start of CPR, RIpostC was induced by four cycles of 5 mins of limb ischemia followed by 5 mins of reperfusion. At 5 mins after resuscitation, TH was implemented by surface cooling to reach a temperature of 32-34 °C until 4 hrs post-resuscitation, followed by a rewarming rate of 1°C/h for 4 hrs. The resuscitated animals were observed for 72 hrs. Results: Six of the seven animals in each group were successfully resuscitated. Significantly better neurological function was observed in the RIpostC group than in the control group. Surprisingly the combined RIpostC and TH further improved post-CPR neurological function, which was significantly better than that in the RIpostC and control groups. There was less cerebral injury in the animals that received RIpostC alone and combined with TH; which was significantly better at 48 and 72 hrs post-resuscitation when compared to the control group. Furthermore, significantly less cerebral injury was measured at 72 hrs in the combination group than in the RIpostC group (Table). Conclusion: In a porcine model of CA, RIpostC significantly improved post-resuscitation neurological function and cerebral injury. Its combination with TH further enhanced its protective effects
Limb Ischemic Postconditioning Alleviates Postcardiac Arrest Syndrome Through the Inhibition of Mitochondrial Permeability Transition Pore Opening in a Porcine Model

Abstract:Objective. Previously, the opening of mitochondrial permeability transition pore (mPTP) was confirmed to play a key role in the pathophysiology of postcardiac arrest syndrome (PCAS). Recently, we demonstrated that limb ischemic postconditioning (LIpostC) alleviated cardiac and cerebral injuries after cardiac arrest and resuscitation. In this study, we investigated whether LIpostC would alleviate the severity of PCAS through inhibiting mPTP opening. Methods. Twenty-four male domestic pigs weighing 37±2 kg were randomly divided into three groups: control, LIpostC, and LIpostC+atractyloside (Atr, the mPTP opener). Atr (10 mg/kg) was intravenously injected 30 mins prior to the induction of cardiac arrest. The animals were subjected to 10 mins of untreated ventricular fibrillation and 5 mins of cardiopulmonary resuscitation. Coincident with the beginning of cardiopulmonary resuscitation, LIpostC was induced by four cycles of 5 mins of limb ischemia and then 5 mins of reperfusion. The resuscitated animals were monitored for 4 hrs and observed for an additional 68 hrs. Results. After resuscitation, systemic inflammation and multiple organ injuries were observed in all resuscitated animals. However, postresuscitation systemic inflammation was significantly milder in the LIpostC group than in the control group. Myocardial, lung, and brain injuries after resuscitation were significantly improved in the LIpostC group compared to the control group. Nevertheless, pretreatment with Atr abolished all the protective effects induced by LIpostC. Conclusion. LIpostC significantly alleviated the severity of PCAS, in which the protective mechanism was associated with the inhibition of mPTP opening.

Abstract:Objective Cardiac arrest (CA) is caused by a nonshockable rhythm with a low success rate of return of spontaneous circulation (ROSC) and a poor prognosis. This study intended to establish a nonshockable rhythm CA model caused by asphyxia. Materials and methods Healthy adult male Wistar rats were injected with vecuronium bromide to induce CA. After the CA duration reached the target time point, cardiopulmonary resuscitation was performed. The survival status and neurological and cardiac function were evaluated after ROSC. Brain histopathology, including hematoxylin staining, Nissl staining and Terminal dUTP nick-end labeling (TUNEL) staining, was performed to evaluate the surviving cells and apoptotic cells. Apoptosis-related proteins after ROSC for 72 h were analyzed by western blot. Results CA was successfully induced in all animals. The time for the three groups of animals to PEA was 320 ± 22 s in the CA-8 group, 322 ± 28 s in the CA-12 group and 320 ± 18 s in the CA-15 group. The time to asystole was 436 ± 54 s in the CA-8 group, 438 ± 62 s in the CA-12 group and 433 ± 56 s in the CA-15 group. The NDS of rats in the CA group was significantly decreased after ROSC for 24 h. The NDS in the CA-15 group was 5–16 points, while it was 58–67 points and 15–43 points in the CA-8 and CA-12 groups, respectively. The cardiac function of animals in the CA group was impaired after ROSC, and the ejection fraction, fractional shortening, stroke volume and cardiac output, were all significantly decreased. Brain histopathology showed that the number of surviving neurons was decreased, and the number of apoptotic cells was increased in CA group, the longer the CA duration, the more apoptotic cells increased. The expression of the proapoptotic protein Bax and the apoptotic executive protein caspase3 in the hippocampus of CA rats was significantly increased, while the expression of the antiapoptotic protein Bcl-2 was significantly reduced. Conclusions The use of vecuronium can successfully induce CA caused by nonshockable rhythm in rats, which will help to further study the pathophysiological changes after CA by nonshockable rhythm.
Hippocampal bcl-2 and bax expressions and neuronal apoptosis after moderate hypothermic cardiopulmonary extracorporeal circulation in rats

Abstract:BACKGROUND: Hippocampus injury is wildly believed to involve in neurocognitive dysfunction; the establishment of a rat model of cardiopulmonary bypass(CPB) allows us to investigate the mechanism of CPB-related hippocampus injury.OBJECTIVE: To investigate the effects of moderate hypothermic CPB with a hemodilution on hippocampal bcl-2 and bax gene expression and neuronal apoptosis in rats.DESIGN: A randomized group division study based on the experimental animals.SETTING: Department of anesthesiology in a university hospital.MATERIALS: Thirty Sprague-Dawley (SD) rats were randomly divided into two groups, CPB group and sham-CPB group with 15 rats in each group.METHODS: Total 15 rats of CPB group were subjected to 60-minute moderate hypothermic nonpulsatile CPB using a peristaltic pump and a membrane oxygenator. The CPB circuit was primed with approximately 20 mL 1:1crystaloid-colloid liquid, while another 15 rats of sham-CPB group underwent identical anesthetic and surgical procedures(including cannulation) except CPB itself. At 1 hour post-CPB, six rats in each group were decapitated, and hippocampi were removed, homogenized, and processed for apoptotic gene ( bcl-2 and bax) mRNAs detection. Reverse transcriptase polymerase chain reaction(RT-PCR) is used to detect expression of mRNA by comparing the PCR product of bcl-2 or bax to those of β-actin housekeeping gene. Immunohistochemistry is used to detect bcl-2 and bax protein expressions and terminal deoxynucleiotidyl transferase-mediated dUTP-biotion nick end labeling(TUNEL) staining method was used to detect neuronal apoptosis at 6 hours post-CPB ( n = 6 in each group) . The protein expression was quantitated as percentage of the positively stained area in the total stained. In addition, hippocampal neuronal ultrastructures were studied by electron microscopy at 6 hours post-CPB( n = 3 in each group).ronal apoptosis and ultrastructure changes between the two groups.RESULTS: At 1 hour post-CPB, the expressions of bcl-2 and bax gene, and the ratio of bax to bcl-2 in CPB group were significantly increased compared with those of sham-CPB group( P ＜ 0.05) . At 6 hours post-CPB, the expressions of Bcl-2 and Bax protein in hippocampal CA1 region had significantly increased in CPB group(P ＜ 0.05) . TUNEL staining showed that hippocampal CA1 neuronal apoptosis was significantly increased in CPB group compared with sham-CPB group at 6 hours post-CPB( P ＜ 0. 05).Electron Microscopy demostrated that, at 6 hours post-CPB, neuronal ultrastructures in CPB group had obvious abnormalities, many mitochondria being moderately to severely swollen with vacuolation as well as decreasing number of mitochondrial cristae, some neurons having characteristic morphological changes of earlier periods of apoptosis such as neuronal pycnosis, irregular nucleus, nuclear membrane with notchs, chromatin condensation, and nucleoli movement to the nuclear periphery, etc.CONCLUSION: Moderate hypothermic CPB with a hemodilution can induce hippocampal bax, bcl-2 gene expression and neuronal apoptosis in rats,which maybe partly explain the mechanism of post-CPB neurocognitive dysfunction.
Mild Hypothermia Alleviates Cardiomyocyte Necroptosis after Cardiopulmonary Resuscitation by Regulating TRPV1

Abstract:Background Therapeutic mild hypothermia (MH) is expected to improve the neurological outcome and the survival rate of CPR after cardiac arrest (CA). However, the mechanisms by which MH protects cardiomyocytes remain largely unexplored. Methods In this study, three- to four-month-old male Wistar rats, weighing between 319.1 and 480.0 grams, were randomly divided into normothermia (NT) groups (acceptable range, 36.0-38.0°C) and MH groups (acceptable range, 32-34°C). Ten minutes after CA, CPR was perfumed with a slow injection of 0.30-0.50 mL epinephrine solution (30.00 μg/mL). Moreover, to investigate the role of TRPV1, capsaicin was administered through the right femoral artery in both NT and MH groups. Results We found that MH improved the spontaneous breathing recovery, increased survival rate, and inhibited cardiac necroptosis in Wistar rats after CPR. Additionally, MH alleviated primary cardiomyocytes necroptosis after oxygen-glucose deprivation and reperfusion through upregulating TRPV1, downregulating EGFR, and subsequently suppressing MLKL. Furthermore, the combination of capsaicin, a TRPV1 activator, with MH enhanced blood pressure in Wistar rats after CPR compared to MH alone. Capsaicin improved the spontaneous breathing rate and survival rate of Wistar rats after CPR. Conclusions MH may protect cardiac function by mitigating cardiomyocyte necroptosis through regulating TRPV1.

Abstract:Objective To establish and evaluate a swine model of traumatic cardiac arrest (TCA) induced by haemorrhage and ventricular fibrillation. Methods Thirteen male pigs were divided into a sham group ( n = 5) and TCA group ( n = 8). Animals in the sham-operated group underwent intubation and monitoring but not haemorrhage and resuscitation, while animals in the TCA group underwent 40% blood volume haemorrhage over 20 min followed by 5 min of ventricular fibrillation and 5 min of cardiopulmonary resuscitation with fluid resuscitation. Results Restoration of spontaneous circulation was achieved in seven of eight animals in the TCA group. After resuscitation, the heart rate was significantly increased while the mean arterial pressure and ejection fraction were significantly decreased in the TCA group. The TCA group had significant cardiac and neurological injuries post-resuscitation and had higher serum creatinine and blood lactic acid levels and lower PaO2 than the sham group. Animals in the TCA group also exhibited significantly higher apoptotic indices and caspase-3 protein levels in the heart, brain and kidney than the sham group. Conclusion Animals in this swine model of TCA exhibited high rates of successful resuscitation, significant vital organ injury and prolonged survival. The model is suitable for use in further TCA research.

Abstract:Introduction: BKCa channels are large conductance Ca++-activated K+ channels, which reside in the inner mitochondrial membrane in cardiomyocytes. We have demonstrated that the BKCa channel activator rottlerin (a.k.a mallotoxin) is cardioprotective during cardioplegic arrest in isolated rodent hearts and in vitro models. Here we used a clinically relevant in vivo swine model of cardioplegic arrest and cardiopulmonary bypass (CP/CPB) with and without rottlerin supplemented CP solution. Mitochondrial signaling alterations were identified by TMT-MS analysis of phosphorylation enriched mitochondrial protein fractions. Methods: Yorkshire swine (50 Kg) of both sexes were subjected to hypothermic CP/CPB (1hour) followed by 1 hour reperfusion. Groups included sham (n = 5), CP/CPB alone (n = 8) and CP/CPB with rottlerin supplemented cardioplegia (1uM) (CP/CPB+R) (n = 7). Mitochondria-enriched fractions were isolated from the left ventricular free wall and subjected to Tandem Mass Tag (TMT) labeling coupled mass spectrometry analysis. Results: CP/CPB pigs had reduced myocardial function indicative of myocardial stunning as shown by decreased LVDevP, and ±dP/dt vs sham (P<.05). BKCa activation significantly restored left ventricular function compared to CP/CPB alone (LVDevP: 74.0 ± 2.8 vs . 52.5±5.0 mmHg, P < .01, +dP/dt: 1482±116 vs . 923±126 mmHg/s, P < .01, and - dP/dt: 1120±116 vs . 629±82 mmHg/s, P < .01). TMT-MS analysis identified 3363 proteins. 410 unique as well as numerous novel phosphorylation sites were significantly regulated in CP/CPB vs sham. Rottlerin treatment generally normalized phosphorylation changes with 175 and 63 different vs CP/CPB and sham, respectively. Enrichment analysis demonstrated significant changes in pathways associated with adrenergic and PKA signaling, calcium handling, mitochondrial dynamics, cristae organization, and oxidative phosphorylation. Conclusions: BKCa-activator supplemented cardioplegia significantly improves myocardial functional recovery following CP/CPB in a clinically relevant large animal model. Furthermore, mitochondrial BKCa-channel activation during cardioplegic arrest normalizes numerous signaling alterations associated with myocardial stunning and cardiac surgery.
