Abstract:Phantom Limb Syndrome (PLS) can be defined as the disabling or painful sensation of the presence of a body part that is no longer present after its amputation. Anatomical changes involved in Phantom Limb Syndrome, occurring at peripheral, spinal and brain levels and include the formation of neuromas and scars, dorsal horn sensitization and plasticity, short-term and long-term modifications at molecular and topographical levels. The molecular reorganization processes of Phantom Limb Syndrome include NMDA receptors hyperactivation in the dorsal horn of the spinal column leading to inflammatory mechanisms both at a peripheral and central level. At the brain level, a central role has been recognized for sodium channels, BDNF and adenosine triphosphate receptors. In the paper we discuss current available pharmacological options with a final overview on non-pharmacological options in the pipeline.

Abstract:AbstractA potential contributor to impaired motor imagery in amputees is an alteration of the body schema as a result of the presence of a phantom limb. However, the nature of the relationship between motor imagery and phantom experiences remains unknown. In this study, the influence of phantom limb perception on motor imagery was investigated using a hand mental rotation task by means of behavioral and electrophysiological measures. Compared with healthy controls, significantly prolonged response time for both the intact and missing hand were observed specifically in amputees who perceived a phantom limb during the task but not in amputees without phantom limb perception. Event-related desynchronization of EEG in the beta band (beta-ERD) in central and parietal areas showed an angular disparity specifically in amputees with phantom limb perception, with its source localized in the right inferior parietal lobule. The response time as well as the beta-ERD values were significantly positively correlated with phantom vividness. Our results suggest that phantom limb perception during the task is an important interferential factor for motor imagery after amputation and the interference might be related to a change of the body representation resulting from an unnatural posture of the phantom limb.

Abstract:Phantom pain is a frequent sequel of the amputation of a limb or another body part and must be differentiated from residual limb pain, postoperative pain, and other chronic pain problems such as back pain that may occur simultaneously. In this chapter, we first discuss how maladaptive plasticity of the central nervous system in interaction with peripheral variables may contribute to phantom pain and then examine how emotional and cognitive variables modulate the phantom pain experience. We show that anxiety, depression, stress experiences, body representation, and memory processes as well as psychosocial variables are associated with both the development of phantom limb pain and its maintenance. In examining this issue, pain and disability-related emotional and cognitive factors must be differentiated. An integration of the described physiological changes with the psychological variables is still missing. We propose a model that integrates psychological and physiological variables in phantom limb pain and discuss implications for both pain assessment and treatment.
Phantom limbs and cultural ventriloquism: communicating cultural difference as a novelist

Abstract:This essay considers the ‘phantom presences’ that shadow attempts by novelists in contemporary Australia to communicate within and across cultures. Cross-cultural communication is haunted by ‘phantom limbs’ in all sorts of ways: the phantom limb of the revenant white nation, the phantom limbs of various cultures migrants left behind, and the phantom limb of ‘home’ – of ‘landscapes [which] ache in all places of departures’. The essay explores technical issues of cultural representation – a process which ultimately cannot avoid problematic constructions of self-orientalising ethnicity. I explain the personal context through which my novels Love and Vertigo (2000) and Behind the Moon (2005) were produced and the historical context of the novels’ publication. I then consider the content of multicultural/ethnic Australian fiction within the broader context of Australian history, looking at how this legacy – a legacy of phantom presences – shapes cross-cultural writing as well as responses to this genre of fiction.

Abstract:Targeted muscle reinnervation (TMR) surgery involves the coaptation of amputated nerves to nearby motor nerve branches with the purpose of reclosing the neuromuscular loop in order to reduce phantom limb pain. The purpose of this case study was to create a phantom limb therapy protocol for an amputee after undergoing TMR surgery, where the four main nerves of his right arm were reinnervated into the chest muscles. The goal of this phantom limb therapy was to further strengthen these newly formed neuromuscular closed loops. The case participant (male, 21- years of age, height = 5′8′′ and weight = 134 lbs) presented 1- year after a trans-humeral amputation of the right arm along with TMR surgery and participated in phantom limb therapy for 3 months. Data collections for the subject occurred every 2 weeks for 3 months. During the data collections, the subject performed various movements of the phantom and intact limb specific to each reinnervated nerve and a gross manual dexterity task (Box and Block Test) while measuring brain activity and recording qualitative feedback from the subject. The results demonstrated that phantom limb therapy produced significant changes of cortical activity, reduced fatigue, fluctuation in phantom pain, improved limb synchronization, increased sensory sensation, and decreased correlation strength between intra-hemispheric and inter-hemispheric channels. These results suggest an overall improved cortical efficiency of the sensorimotor network. These results add to the growing knowledge of cortical reorganization after TMR surgery, which is becoming more common to aid in the recovery after amputation. More importantly, the results of this study suggest that the phantom limb therapy may have accelerated the decoupling process, which provides direct clinical benefits to the patient such as reduced fatigue and improved limb synchronization.

Abstract:The psychological world of The English Patient is explored to deepen the understanding of schizoid states. The protagonist, Almásy, is a remote desert explorer whose triangular sadomasochistic affair with the married Katharine destroys them all. His damaged skin is understood as a symbolic representation of his psychological condition. For the schizoid, love consumes and leads to obliteration of the self, represented by the loss of identifying features, and to traumatic permeability (i.e., the loss of boundaries between self and other, and between the ego and repressed desires). Other schizoid themes are the animation of the inanimate, as in the depiction of the desert as a woman; hidden or buried identities; the digital and destructive experience of emotion represented by the conundrum of the bomb defuser; the sense that everything good is imaginary and might suddenly explode; and the moral unevenness of the characters. Almásy collaborates with the Nazis so he can retrieve Katharine's three-year-old corpse, with which he has necrophilic contact in a cave. Fantasies of the lost object buried within the self, of being buried alive, and of being skinned alive are related to the schizoid condition. Hyperpermeability is proposed as a core schizoid state, underlying schizoid withdrawal.

Beating phantasies: Mourned and unmourned

Abstract:This paper intends to explore the organizing function and fate of sado-masochistic phantasies in their fixed form, in the psychic life of patients who have suffered early traumas, due to environmental vicissitudes and maternal psychopathology. The theoretical starting point is provided by the Novicks' research into transitory and permanent beating phantasies in adult and child patients, their onset and their psychic function, based on an examination of Freud's paper 'A child is being beaten'. In this text Freud achieves an unprecedented syncretism, locating the phantasy at the intersection between the oedipal complex, which is a vertical structure organizing sexual and generational differences, and the fraternal dimension, which is horizontal and organizes the lateral relationship with that similar but different other who is the sibling. Reporting in some detail material from the analyses of two young women, whose clinical presentation and early traumas show some similarities, the author puts in the context of the analytic work the emerging of the sado-masochistic phantasies, emphasizing their use and function in the transference-countertransference interplay. The beating phantasies shore up a precarious sense of self, threatened with psychic depletion and death, in identification with the absent mother of early infancy. An overly close relationship with a sibling, experienced as a narcissistic double, compounds the clinical picture, to establish the triangle formed by the parent(s) and two children which features in the beating phantasy. Finally, the author explores the contribution of ameliorating factors, such as alternative identificatory figures, constitutional endowment and capacity for sublimation, to account for the different fate of the fixed beating phantasies.

However, new neuroscientific research indicates that body and mind are much more connected than previously thought. Magnetic Resonance Materials in Physics Biology and Medicine

Abstract:In this paper, we review the value of phantoms for body MRI in the context of their uses for quantitative MRI methods research, clinical trials, and clinical imaging. Certain uses of phantoms are common throughout the body MRI community, including measuring bias, assessing reproducibility, and training. In addition to these uses, phantoms in body MRI methods research are used for novel methods development and the design of motion compensation and mitigation techniques. For clinical trials, phantoms are an essential part of quality management strategies, facilitating the conduct of ethically sound, reliable, and regulatorily compliant clinical research of both novel MRI methods and therapeutic agents. In the clinic, phantoms are used for development of protocols, mitigation of cost, quality control, and radiotherapy. We briefly review phantoms developed for quantitative body MRI, and finally, we review open questions regarding the most effective use of a phantom for body MRI.

Abstract:BACKGROUND GM2 gangliosidosis variant 0 (human Sandhoff disease) is a lysosomal storage disorder caused by deficiencies of acid β-hexosaminidase (Hex) A and Hex B because of an abnormality of the β-subunit, a common component in these enzyme molecules, which is coded by the HEXB gene. OBJECTIVE To describe the clinical, pathological, biochemical, and magnetic resonance imaging (MRI) findings of Sandhoff-like disease identified in a family of Toy Poodles. ANIMALS Three red-haired Toy Poodles demonstrated clinical signs including motor disorders and tremor starting between 9 and 12 months of age. The animals finally died of neurological deterioration between 18 and 23 months of age. There were some lymphocytes with abnormal cytoplasmic vacuoles detected. METHODS Observational case study. RESULTS The common MRI finding was diffuse T2-hyperintensity of the subcortical white matter in the cerebrum. Bilateral T2-hyperintensity and T1-hypointensity in the nucleus caudatus, and atrophic findings of the cerebrum and cerebellum, were observed in a dog in the late stage. Histopathologically, swollen neurons with pale to eosinophilic granular materials in the cytoplasm were observed throughout the central nervous system. Biochemically, GM2 ganglioside had accumulated in the brain, and Hex A and Hex B were deficient in the brain and liver. Pedigree analysis demonstrated that the 3 affected dogs were from the same family line. CONCLUSIONS AND CLINICAL IMPORTANCE The Sandhoff-like disease observed in this family of Toy Poodles is the 2nd occurrence of the canine form of this disease and the 1st report of its identification in a family of dogs.
Animal Autobiography; Or, Narration beyond the Human

Abstract:In engaging with acts of self-narration that cross species lines, creators of animal autobiographies also broach questions about genre, truth status, and the structure as well as the politics of narrative representation. To address these questions, the present article draws not just on scholarship on (animal) autobiography but also on ideas from the fields of linguistic semantics, politeness theory, and discourse analysis, including the “framing and footing” approach that focuses on talk emerging in contexts of face-to-face interaction and that derives most directly from the work of Erving Goffman. On the basis of this research, and using case studies that range from animal riddles to Ceridwen Dovey’s Only the Animals (2014), a collection of life stories posthumously narrated by a variety of nonhuman tellers, I profile autobiographical acts that reach beyond the human as ways of speaking for or in behalf of animal others. Some animal autobiographies correlate with acts of telling for which humans themselves remain the principals as well as authors; their animal animators remain relegated to the role of commenting on human institutions, values, practices, and artifacts. Other examples, however, can be read as co-authored acts of narrating in behalf of equally hybrid (or “humanimal”) principals. These experiments with narration beyond the human afford solidarity-building projections of other creatures’ ways of being-in-the-world—projections that enable a reassessment, in turn, of forms of human being.

Abstract:The development of assistive lower-limb exoskeletons can be time-consuming. Testing prototype medical devices on vulnerable populations such as children also has safety concerns. Mechanical phantoms replicating the lower-limb kinematics provide an alternative for the fast validation and iteration of exoskeletons. However, most phantoms treat the limbs as rigid bodies and fail to capture soft tissue deformation at the human/exoskeleton interface. Human soft tissue can absorb and dissipate energy when compressed, leading to a mismatch between simulated and human exoskeleton testing outcomes. We have developed a methodology for quickly testing and validating the performance of knee exoskeletons using a mechanical phantom capable of emulating knee kinematics soft-tissue deformation of the lower-limb. Our phantom consisted of 3D-printed bones surrounded by ballistic gel. A motorized hexapod moved the knee to follow a walking trajectory. A custom inverse dynamics model estimated the knee assistance moment from marker and load cell data. We applied this methodology to quantify the effects of soft-tissue deformation on exoskeleton assistance by loading the phantom knee with a torsional spring exoskeleton interfacing and bypassing the ballistic gel. We found that including soft-tissue deformation led to a lower knee assistance moment and stiffness. Some but not all of this difference could be explained by the deflection of the exoskeleton relative to the knee angle, suggesting energy absorption within soft tissue. The direct measurements of exoskeleton assistance provide insight into increasing the assistive moment transmission efficacy. The phantom provided a relatively accurate framework for knee exoskeleton testing, aiding future exoskeleton design.

Abstract:PURPOSE To develop novel immunoprotective alginate microcapsule formulations containing perfluorocarbons (PFCs) that may increase cell function, provide immunoprotection for xenografted cells, and simultaneously enable multimodality imaging. MATERIALS AND METHODS All animal experiments were approved by an Institutional Animal Care and Use Committee. Cadaveric human islet cells were encapsulated with alginate, poly-l-lysine, and perfluorooctyl bromide (PFOB) or perfluoropolyether (PFPE). In vitro viability and the glucose-stimulation index for insulin were determined over the course of 2 weeks and analyzed by using a cross-sectional time series regression model. The sensitivity of multimodality (computed tomography [CT], ultrasonography [US], and fluorine 19 [(19)F] magnetic resonance [MR] imaging) detection was determined for fluorocapsules embedded in gel phantoms. C57BL/6 mice intraperitoneally receiving 6000 PFOB-labeled (n = 6) or 6000 PFPE-labeled (n = 6) islet-containing fluorocapsules and control mice intraperitoneally receiving 6000 PFOB-labeled (n = 6) or 6000 PFPE-labeled (n = 6) fluorocapsules without islets were monitored for human C-peptide (insulin) secretion during a period of 55 days. Mice underwent (19)F MR imaging at 9.4 T and micro-CT. Swine (n = 2) receiving 9000 PFOB capsules through renal artery catheterization were imaged with a clinical multidetector CT scanner. Signal intensity was evaluated by using a paired t test. RESULTS Compared with nonfluorinated alginate microcapsules, PFOB fluorocapsules increased insulin secretion of encapsulated human islets, with values up to 18.5% (3.78 vs 3.19) at 8-mmol/L glucose concentration after 7 days in culture (P < .001). After placement of the immunoprotected encapsulated cells into mice, a sustained insulin release was achieved with human C-peptide levels of 19.1 pmol/L ± 0.9 (standard deviation) and 33.0 pmol/L ± 1.0 for PFPE and PFOB capsules, respectively. Fluorocapsules were readily visualized with (19)F MR imaging, US imaging, and CT with research- and clinical-grade imagers for all modalities. CONCLUSION Fluorocapsules enhance glucose responsiveness and insulin secretion in vitro, enable long-term insulin secretion by xenografted islet cells in vivo, and represent a novel contrast agent platform for multimodality imaging.

Abstract:While critics recently have found The Sun Also Rises a fruitful text for reexamining issues of gender and sexuality in Hemingway's work, a significant aspect of Jake Barnes's genital wounding has been overlooked. At least from the time of the American Civil War, a diverse body of medical literature has documented the psychosomatic reality of phantom genitalia in traumatically injured men. Revisiting the novel from this perspective-imagining that Jake Barnes is haunted by a penile ghost-allows us to see this character as something more than a victim of disability. Instead, we might think of Jake's material self as figuratively masculine but accidentally transgendered: a body that others can desire and that still can choose, or not, to reciprocate sexual feeling.

Abstract:Introduction Muscle activity patterns in the residual arm are systematically present during phantom hand movements (PHM) in transhumeral amputees. However, their characteristics have not been directly investigated yet, leaving their neurophysiological origin poorly understood. This study pioneers a neurophysiological perspective in examining PHM-related muscle activity patterns by characterizing and comparing them with those in the arm, forearm, and hand muscles of control participants executing intact hand movements (IHM) of similar types. Methods To enable rigorous comparison, we developed meta-variables independent of electrode placement, quantifying the phasic profile of recorded surface EMG signals and the specificity of their patterns across electrode sites and movement types. Results Similar to the forearm and hand muscles during IHM, each signal recorded from the residual upper arm during PHM displays a phasic profile, synchronized with the onset and offset of each movement repetition. Furthermore, the PHM-related patterns of phasic muscle activity are specific not only to the type of movement but also to the electrode site, even within the same upper arm muscle, while these muscles exhibit homogeneous activities in intact arms. Discussion Our results suggest the existence of peripheral reorganization, eventually leading to the emergence of independently controlled muscular sub-volumes. This reorganization potentially occurs through the sprouting of severed axons and the recapture of muscle fibers in the residual limb. Further research is imperative to comprehend this mechanism and its relationship with PHM, holding significant implications for the rehabilitation process and myoelectric prosthesis control.

SPL can occur in various neural regions, but it is rare for SPL to be caused by left hemisphere cerebral infarction. In this report, we describe a case of a 64-year-old woman with SPL after a stroke. She had a history of handedness change. A neurological examination revealed that she had right hemiplegia, unilateral spatial neglect (USN), anosognosia, and pan-sensory loss on the right side of her body. Brain magnetic resonance (MR) imaging disclosed cerebral infarction in the left corona radiata region. She suffered from SPL in the right upper limb. Although SPL was prolonged, the recovery of USN was noted four months after onset, followed by the improvement of SPL.
