Abstract:Background: Substantial evidence suggests that cognitive deficits might persist after remission of a major depressive episode. However, results are inconsistent relative to the importance, pattern, severity, and moderating factors of this impairment. We aimed to determine how cognitive function following a major depressive episode compares with normal function, to specify the pattern and severity of persistent cognitive dysfunctions, and to examine the potential moderator effect of ten prespecified clinical and demographic variables. Methods: We did a systematic review and meta-analysis of the published research. We searched systematically MEDLINE, Embase, PsycARTICLES, PsycINFO, the Cochrane Library, and relevant reviews identified by our database search, for research published from Jan 1, 1972, up to Jan 31, 2018, for studies of patients with past depression. We included all independent studies of patients who were assessed while in remission from a major depressive episode with at least one cognitive test, with inclusion of a healthy control group assessed with either the same test(s) as the major depressive episode group or with a standardised test with published age-stratified normative data. The main outcome was the difference in cognitive performance between major depressive episode remitters and healthy controls. Effect sizes were calculated using random-effects models for cognitive outcomes classified into 18 standard domains. Moderators of between-study variability were assessed using mixed-effects subgroup analyses and meta-regressions. Findings: Of 10 126 citations identified by our search, 75 cognitive variables from 252 eligible studies (11 882 major depressive episode remitters and 8533 healthy controls) were included in our meta-analysis. Significant deficits following major depressive episode remission were observed in 55 (73%) of the 75 cognitive variables. These deficits (in the domains of processing speed, visual selective attention, working memory, verbal learning, and executive functioning), were generally small (30 [40%] of the 75 variables) or medium (22 [29%]) in size, although three long-term memory variables showed large deficits: g=-0·81 [95% CI -1·01 to -0·61] for logical memory immediate recall, g=-0·88 [-1·19 to -0·57] for logical memory delayed recall, and g=-0·84 [-1·18 to -0·50] for Cambridge Neuropsychological Test Automated Battery pattern recognition latency. Auditory attention, general autobiographical memory, inhibition ability unconstrained by speed, and intellectual functioning unconstrained by speed were equivalent between major depressive episode remitters and matched controls. The number of previous depressive episodes explained heterogeneity in the majority of variables (z=-2·06 [p=0·039] to z=-4·26 [p<0·0001]). Interpretation: Deficits in selective attention, working memory, and long-term memory persist in remission from a major depressive episode and worsen with repeated episodes. Depression treatments, including relapse prevention, need to target these cognitive functions to optimise prognosis. Funding: None.

Abstract:Introduction: Cognitive dysfunction or deficits are common in patients with major depressive disorder (MDD). The current study systematically reviews and meta-analyzes multiple domains of cognitive impairment in patients with MDD. Methods: PubMed/MEDLINE, PsycINFO, Cochrane Library, Embase, Web of Science, and Google Scholar were searched from inception through May 17, 2023, with no language limits. Studies with the following inclusion criteria were included: (1) patients with a diagnosis of MDD using standardized diagnostic criteria; (2) healthy controls (i.e., those without MDD); (3) neuropsychological assessments of cognitive impairment using Cambridge Neuropsychological Test Automated Battery (CANTAB); and (4) reports of sufficient data to quantify standardized effect sizes. Hedges’ g standardized mean differences (SMDs) with corresponding 95% confidence intervals (CIs) were used to quantify effect sizes of cognitive impairments in MDD. SMDs were estimated using a fixed- or random-effects models. Results: Overall, 33 studies consisting of 2,596 subjects ( n = 1,337 for patients with MDD and n = 1,259 for healthy controls) were included. Patients with MDD, when compared to healthy controls, had moderate cognitive deficits (SMD, −0.39 [95% CI, −0.47 to −0.31]). In our subgroup analyses, patients with treatment-resistant depression (SMD, −0.56 [95% CI, −0.78 to −0.34]) and older adults with MDD (SMD, −0.51 [95% CI, −0.66 to −0.36]) had greater cognitive deficits than healthy controls. The effect size was small among unmedicated patients with MDD (SMD, −0.19 [95% CI, −0.37 to −0.00]), and we did not find any statistical difference among children. Cognitive deficits were consistently found in all domains, except the reaction time. No publication bias was reported. Conclusion: Because cognitive impairment in MDD can persist in remission or increase the risk of major neurodegenerative disorders, remediation of cognitive impairment in addition to alleviation of depressive symptoms should be an important goal when treating patients with MDD.

Abstract:Major depressive disorder (MDD) is associated with significant cognitive deficits during the acute and remitted stages. The aim of this systematic review and meta-analysis was to examine the course of cognitive function whilst considering demographic, treatment, or clinical features of MDD that could moderate the extent of cognitive change. Databases were searched to identify studies that reported on cognitive function in MDD with a ≥12-week test–retest interval. Relevant studies were pooled using random effects modelling to generate an inverse-variance, weighted, mean effect size estimate (Hedges' g ) of cognitive change for each cognitive variable and for an overall composite cognitive domain. Of 6898 records, 99 eligible studies were identified from which 69 were meta-analysed, consisting of 4639 MDD patients (age mean = 40.25 years, female % = 64.62%) across 44 cognitive variables. In over 95% of cognitive variables, improvements were either of non-significant, negligible, or of a small magnitude, and when compared to matched healthy controls, the possibility of practice effects could not be precluded. Depressive symptom improvement and the number of previous depressive episodes moderated the extent of cognitive change, demonstrating state- and scar-like features for one-quarter of the cognitive domains. Further longitudinal studies are required to elucidate the MDD cognitive trajectory from initial onset. Findings nonetheless suggest that following pharmacological and non-pharmacological treatment, cognitive change in MDD is typically small, but the capacity for change may be less with episode recurrence. Targeting cognition early in the course of illness may facilitate better prognosis and support a more complete functional recovery.

A High Performance Application Representation for Reconfigurable Systems

Abstract:Modern reconfigurable computing systems feature powerful hybrid architectures with multiple microprocessor cores, large reconfigurable logic arrays and distributed memory hierarchies. Mapping applications to these complex systems requires a representation that allows both hardware and software synthesis. Additionally, this representation must enable optimizations that exploit fine and coarse grained parallelism in order to effectively utilize the performance of the underlying reconfigurable architecture. Our work explores a representation based on the program dependence graph (PDG) incorporated with the static single-assignment (SSA) for synthesis to high performance reconfigurable devices. The PDG effectively describes control dependencies, while SSA yields precise data dependencies. When used together, these two representations provide a powerful, synthesizable form that exploits both fine and coarse grained parallelism. Compared to other commonly used representations for reconfigurable systems, the PDG+SSA form creates faster execution time, while using similar area.
Understanding How Advocates Can Affect Sexual Assault Victim Engagement in the Criminal Justice Process

Abstract:Following a sexual assault, survivors may seek help from multiple community organizations including the criminal justice system (CJS). However, sexual assault survivors often feel apprehensive about participating in the CJS and thus, few report their victimizations to law enforcement. Of those who report, many withdraw their participation from the CJS. Rape crisis center advocates often are one of the first professionals victims encounter while seeking help after a victimization and thus, they may play a key role in reducing barriers to victim participation in the CJS. Qualitative interviews were conducted with forensic nurses and victim advocates at a Midwest Sexual Assault Nurse Examiner (SANE) program to examine how advocacy services may influence victim engagement with the CJS. This study found multiple components of advocacy services that may address common barriers to victim participation. Implications for research and practice are discussed.

Abstract:ObjectivesCognitive impairment occurs in approximately 50% of remitted patients with bipolar disorder (BD). However, there exists no treatment with replicated and robust efficacy on cognition in BD. This is partially due to limited insight into the neuronal underpinnings of cognitive impairment in these patients. This is the first study to investigate neuronal underpinnings of cognitive impairment in a large functional magnetic resonance imaging (fMRI) dataset comparing neural activity patterns between distinct neurocognitive subgroups of partially or fully remitted patients with BD. MethodsPatients (n=153) and healthy controls (HC) (n=52) underwent neuropsychological assessment and fMRI, during which they performed a verbal N‐back working memory (WM) task. Based on hierarchical cluster analysis of neuropsychological test performance, patients were grouped into one of two neurocognitive subgroups (cognitively impaired, n=91; cognitively normal compared to HC, n=62) that were compared on WM‐related neural activity. ResultsCognitively impaired patients displayed WM‐related hypo‐activity in left dorsolateral prefrontal cortex and frontal and parietal regions within a cognitive control network (CCN) as well as hyper‐activity in the default mode network (DMN) compared to cognitively normal patients. In contrast, cognitively normal patients only exhibited hypo‐activity within a small cluster in the superior frontal gyrus relative to HC. ConclusionsCognitive impairment in BD seems to originate from a failure to recruit key regions in the CCN and to suppress task‐irrelevant DMN activity during cognitive performance. These results highlight modulation of aberrant dorsal prefrontal and DMN activity as a putative target for pro‐cognitive treatment in BD.

Neuropsychological Performance in Bipolar I, Bipolar II and Unipolar Depression Patients: a Longitudinal, Naturalistic Study.

Abstract:Background: It has been suggested that cognitive deficits existed in mood disorders. Nevertheless, whether neuropsychological profiles differ three main subtypes of mood disorder (Bipolar I, Bipolar II and UP) remain understudied because most current studies include either mixed samples of bipolar land bipolar II patients or mixed samples of different states of the illness. The main aim of the present study is to determine whether, or to some extent, specific cognitive domains could differentiate the main subtypes of mood disorders in the depressed and clinically remitted status.Method: Three groups of bipolar I (n = 92), bipolar II (n = 131) and unipolar depression (UP) patients (n = 293) were tested with a battery of neuropsychological tests both at baseline (during a depressive episode) and after 6 weeks of treatment, contrasting with 202 healthy controls on cognitive performance. The cognitive domains include processing speed, attention, memory, verbal fluency and executive function.Results: At the acute depressive state, the three patient groups (bipolar I, bipolar II and UP) showed cognitive dysfunction in processing speed, memory, verbal fluency and executive function but not in attention compared with controls. Post comparisons revealed that bipolar I depressed patients performed significantly worse in verbal fluency and executive function than bipolar II and UP depressed patients. No difference was found between bipolar II and UP depressed patients except for the visual memory. After 6 weeks of treatment, clinically remitted bipolar land bipolar II patients only displayed cognitive impairment in processing speed and visual memory. Remitted UP patients showed cognitive impairment in executive function in addition to processing speed and visual memory. The three remitted patient groups scored similarly in processing speed and visual memory.Limitation: Clinically remitted patients were just recovered from a major depressive episode after 6 weeks of treatment and in relatively unstable state.Conclusion: Bipolar I. bipolar II and UP patients have a similar pattern of cognitive impairment during the state of acute depressive episode, but bipolar I patients experience greater impairment than bipolar II and UP patients. In clinical remission, both bipolar and UP patients show cognitive deficits in processing speed and visual memory, and executive dysfunction might be a status-maker for bipolar disorder, but a trait-marker for UP. Depression related cerebral pathology and its relationship with cognitive functioning: A systematic review

Abstract:Background: Depression's relationship with cerebral abnormalities and cognitive decline is temporally dynamic. Despite clear clinical utility, understanding depression's effect on cerebral structures, cognitive impairment and the interaction between these symptoms has had limited consideration. Methods: This review summarised studies examining a clinical depression diagnosis or validated scales measuring depressive symptoms, data concerning amyloid-beta (Aβ) levels, brain structure and function focusing on hippocampal alterations, or white matter hyperintensities (WMH), and at least one validated neuropsychological test. Online database searches of: PsycINFO, EMBASE, MEDLINE, and Scopus were conducted to identify potential articles. Results: While depression was consistently associated with cross-sectionally cognitive decline across multiple domains, the neuropathological basis of this dysfunction remained unclear. Hippocampal, frontal, and limbic dysfunction as well as cortical thinning, WMH, and Aβ burden all provide inconsistent findings, likely due to depression subtypes. The consistency of these findings additionally decreases when examining this relationship longitudinally, as these results are further confounded by pre-dementia states. The therapeutic interventions examined were more efficacious in the younger compared with the older samples, who were characterised by greater WMH and Aβ burden. Limitations: The limited number of longitudinal and interventional studies in addition to the heterogeneity of the samples restricts their generalisability. Conclusions: Symptomatological differences between early-onset and late-onset depression (EOD and LOD) appear crucial in understanding whether late-life depression is the primary or secondary source of cerebral pathology. Though severe cognitive impairments and clearer neuropathological underpinnings are more characteristic of LOD than EOD, the inconsistency of valid biomarkers remains problematic.

Abstract:Background: Major depressive disorder (MDD) is associated with a wide range of cognitive deficits. However, it remains unclear whether there will be a major cognitive deficit independently caused by depression at acute episodes of MDD. Method: A comprehensive neurocognitive test battery was used to assess the executive function, processing speed, attention, and memory in 162 MDD patients and 142 healthy controls (HCs). A multivariate analysis of variance, hierarchical regression analyses and general linear regression analyses were used to explore the possible major cognitive deficits and their predictor variables. Results: MDD patients showed extensive impairment in all four cognitive domains. Impairment of executive function and processing speed were found to persist even with other cognitive domains and clinical variables being accounted for. Executive function and processing speed were further predicted by total disease duration and depression severity, respectively. Conclusions: Executive function and processing speed may be two distinct major deficits at acute episodes of MDD. Furthermore, the executive function is likely originated from the cumulative effect of disease duration and processing speed is possibly derived from the temporary effect of current depressive episode.
Pharmacological and non-pharmacological interventions to improve cognitive dysfunction and functional ability in clinical depression – A systematic review

Abstract:Cognitive dysfunction is of clinical significance and exerts longstanding implication on patients׳ function. Pharmacological and non-pharmacological treatments of cognitive dysfunction are emerging. This review evaluates pharmacological and non-pharmacological treatments of cognitive impairment primarily in the domains of memory, attention, processing speed and executive function in clinical depression. A total of 35 studies were retrieved from Pubmed, PsycInfo and Scopus after applying inclusion and exclusion criteria. Results show that various classes of antidepressants exert improving effects on cognitive function across several cognitive domains. Specifically, studies suggest that SSRIs, the SSRE tianeptine, the SNRI duloxetine, vortioxetine and other antidepressants such as bupropion and moclobemide may exert certain improving effects on cognitive function in depression, such as in learning and memory and executive function. Class-specific cognitive domains or specific dose-response relationships were not identified yet. The few non-pharmacological studies conducted employing cognitive orientated treatments and cognitive remediation therapy show promising results for the improvement of cognitive impairment in depression. However, several methodological constraints of studies limit generalizability of the results and caution the interpretation. Future direction should consider the development of a neuropsychological consensus cognitive battery to support the discovery, clinical assessment, comparison of studies and registration of new agents in clinical depression.

Abstract:Objective To investigate the impairment and change of cognitive function in patients with depression in remission.Methods The cognitive function was evaluated with group neurocognitive test tool from multi -dimensional evaluation of attention,memory and execution function among 14 depressive patients during diseased episode and in remission after one year.The control group was set and the results of the two groups were compared.Results The result of the attention test showed that the error counts of the second and third situation of Continuous Performance Test (CPT)in depression group in diseased episode and in remission were more than those in the control group (P <0.05),and the time used in Stroop -C and the time used in A of Trail making test(TMT)were longer than those in the control group (P <0.05),and the right numbers in Stroop -CW were less than those in the control group (P <0.05 ).The numbers completed in Stroop -CW in diseased episode of depression group were less than those in control group(P <0.05),and the numbers completed in Stroop -CW in remission of depression group were more than those in diseased episode(P <0.01).The result of the memory test showed that the score of a in Logical memory test (LMT)in diseased episode of depression group was less than that in control group(P <0.05),and the score of b and aggregate score in LMT and the score of b in Visal reproduction test (VRT)in diseased episode and in remission of depression group were less than that in control group (P <0.05).In VRT,the score of a and aggregate score in diseased episode were less than that in remission of depression group and in control group(P <0.05),and the score of c in diseased episode and the aggregate score in remission were less than that in control group(P <0.05).The result of the execution function test showed that the numbers of sustained error of mWCST in diseased episode of depression group were more and the numbers of categories were less than those in control group (P <0.05),and the correct numbers in Verbal fluency test (VFT)of the depressive group in diseased episode and in remission were less than those in control group(P <0.01 ).Conclusion When the depressive disorder is remitted,the degree of cognitive impairment in different dimension is different,and the cognitive impairment is still exist for a long time.
Revisiting Cognitive Deficits in Outpatients with Psychotic Disorders: A Transdiagnostic Comparison of Cognitive Performance While Accounting for Putative Confounding Factors

Abstract:Recent research suggests that cognitive deficits in individuals with psychotic disorders could be overestimated because poor cognitive test performance is partly attributable to non-cognitive factors. To further test this, we included non-hospitalized individuals with psychotic disorders (PSY, n = 38), individuals with attenuated psychotic symptoms (n = 40), individuals with obsessive-compulsive disorders (n = 39), and healthy controls (n = 38). Relevant cognitive domains were assessed using the MATRICS Consensus Cognitive Battery. Putative confounding non-cognitive factors—heart rate, self-reported stress, negative affect, performance-related beliefs, and actigraphy-derived sleep—were assessed before cognitive testing. A multivariate analysis of covariance was calculated to examine group differences in cognitive performance while controlling for non-cognitive factors. PSY showed decreased test performance in graphomotor speed, attention, and verbal tasks compared to the other groups, whereas non-verbal/visual-spatial tasks were unimpaired. After accounting for non-cognitive factors, group differences diminished in verbal learning, whereas differences in the other domains remained significant. Against our hypotheses, the present findings indicate that some cognitive deficits in PSY cannot be attributed to momentary confounding factors.

Abstract:Introduction Bipolar disorder and schizophrenia exhibit different patterns of cognitive impairment, with schizophrenia demonstrating more profound deficiencies in verbal memory and bipolar disorder in social cognition. Understanding these patterns may guide the development of interventions to enhance cognition in these disorders. Aim This study aims to assess and compare the cognitive abilities of persons diagnosed with bipolar illness and schizophrenia. Methodology A facility-based cross-sectional study was done from December 2016 to June 2017 among 30 schizophrenia and 30 bipolar disorder patients aged 18-45 years, in remission selected after screening through Hamilton Depression Rating Scale (HDRS), Young Mania Rating Scale (YMRS), or Positive and Negative Syndrome Scale (PANSS). Exclusions included schizoaffective disorder, systemic illness, brain/neurological conditions, and substance abuse. After collecting the baseline demographic and clinical profile of the selected patients, the cognitive domains were assessed such as attention (digit span), verbal memory (Rey's Auditory Verbal Learning Test (RAVLT)), visual memory (Rey Complex Figure), verbal fluency (Animal Naming), and executive functions (Stroop and Trail Making). The data was analyzed using the IBM SPSS Statistics for Windows, Version 16 (Released 2007; IBM Corp., Armonk, New York, United States) using standard descriptive and inferential statistics. Results Sociodemographic and clinical characteristics were largely similar between groups. Schizophrenia patients showed poorer attention, working memory, and visual attention/task-switching compared to bipolar patients. Bipolar patients demonstrated relatively preserved abilities in these domains but exhibited more impairments in visual and verbal memory. Distinct patterns highlight unique neurobiological underpinnings, showing association of more generalized cognitive deficits in schizophrenia and more localized impairments in memory functions in bipolar disorder. Conclusion The study findings explain these disorders' unique neurobiological mechanisms and may help develop targeted cognitive remediation and pharmacological interventions to improve functional outcomes and quality of life.
Impaired cognition and decision-making in bipolar depression but no 'affective bias' evident

Abstract:Background: Depression is usually the predominant affective state in bipolar disorder. There are few studies, with discrepant views, examining the extent of cognitive impairment in patients with bipolar depression. To our knowledge, there are no previous studies examining decision-making ability or whether there is an affective attentional bias in bipolar depression. Method: We ascertained 24 depressed bipolar I patients from acute psychiatric hospital wards and out-patient clinics and 26 age- and IQ-matched healthy controls. Using computerized tests we evaluated their performance on 'neutral' (non-emotional) cognitive tasks (i.e. memory, attention and executive function) and on novel tasks of emotional cognition (i.e. the decision-making task and the affective go/no-go task). Results: Accuracy measures were significantly impaired on tests of visual and spatial recognition and attentional set-shifting in bipolar depression compared with age- and IQ-matched controls. The quality of decision-making was also significantly impaired in the patients. A mood-congruent attentional bias for 'sad' targets was not evident on the affective go/no-go task. Conclusions: We found widespread evidence of significant cognitive impairment and impaired quality of decision-making in symptomatically severe depressed bipolar patients. This cognitive impairment may contribute to difficulties with daily living, decision-making and the ability to engage and comply with psychological and drug treatments.
Cognitive mechanisms underlying depressive disorders in ADHD: A systematic review

Abstract:The risk for major depressive disorder (MDD) is considerably increased in young adults with attention-deficit/hyperactivity disorder (ADHD) but underlying mechanisms are poorly understood. This review explores ADHD-specific neurocognitive impairments as possible underlying mechanisms for ADHD-depression comorbidity. Two systematic literature searches were conducted in EBSCOhost, PubMED, and Cochrane Reviews databases according to PRISMA guidelines. The first search identified 18 meta-analyses of cross-sectional and longitudinal studies on cognitive dysfunctions in MDD across the lifespan. The second search identified six original studies on reaction time variability in MDD. During acute depression, children and adults showed cognitive deficits that overlapped with some of the ADHD-related impairments. Findings from remitted patients, high-risk individuals, and few prospective studies suggest that a subset of these shared impairments, specifically executive dysfunctions (selective attention, verbal fluency, working memory) and long-term memory problems, are candidate pre-existing risk markers of depression. We discuss if and how these specific neurocognitive mechanisms may mediate developmental pathways from ADHD to depression. If replicated by longitudinal studies, these findings may guide future prevention strategies.
Neuropsychological Impairments in Unipolar Depression: the Influence of Perceived Failure on Subsequent Performance

Abstract:The CANTAB battery of neuropsychological tests was used to compare the performance of 28 patients with unipolar depression with that of 22 age and IQ matched controls. The patients were impaired on almost all tests studied with deficits in pattern and spatial recognition memory, matching to sample, spatial span, spatial working memory and planning. Most of the patients showed at least some impairment and deficits were seen across cognitive domains. An important finding was the detrimental effect of failure on subsequent performance; having solved one problem incorrectly, patients were far more likely than controls to fail the subsequent problem. Superimposed on the general deficits, there were also specific deficits in executive tasks characteristic of frontostriatal dysfunction and deficits in mnemonic tasks characteristic of temporal lobe dysfunction. This combination of a specific form of motivational deficit, resulting in oversensitivity to negative feedback, and superimposed specific neuropsychological deficits were correlated with severity of depression. The most significant correlations were seen between mnemonic deficits and clinical rating scores. Comparisons of the deficits seen in the depressed patients in this study with other patient groups assessed with the CANTAB neuropsychological battery, showed that one of the hypotheses of the neuropsychological deficits in depression, that of "frontosubcortical' or "frontostriatal' dysfunction, was not supported. These findings are discussed in relation to the likely neural substrates of depression.

Abstract:Schizophrenia is a serious mental disorder that affects up to 1% of the population worldwide. Most patients with the illness suffer from disturbances of key aspects of cognitive function, e.g., neurocognition and social cognition, which worsen real-world functional outcomes. For example, neurocognitive impairment affects up to 75% of patients across multiple domains including processing speed, working memory, verbal memory, attention, executive functioning, while emotional processing, theory of mind, attributional bias, and social perception represent key domains of social cognition which are typically dysfunctional in schizophrenia. Several features of cognitive impairment are commonly found across schizophrenia and mood disorders, as well as their prodromal stages, indicating its transdiagnostic importance. Specifically, this Topic presents the cutting-edge knowledge on advanced assessments, neurobiological underpinnings, and interventional endeavors targeting cognitive impairment of schizophrenia. Disturbances of neurocognition and social cognition of schizophrenia have been shown to be associated with both genetic and environmental factors. Among them, the genetic contribution is supported by Bae et al. (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1373288/full) who report poor performance on a test of facial emotion-recognition both in schizophrenia patients and their first-degree relatives. In this line, Deng et al (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1362674/full) found an association between increased body mass index and neurocognition in first-episode drug-naive patients, also suggesting genetic susceptibility of cognitive impairment. On the other hand, the influence of socio-psychological (environmental) factors was reported by Peng et al (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1343188/full) who observed an additive adverse effect of childhood trauma on cognition in patients with schizophrenia. Importantly, the association between cognitive symptoms and social function in individuals with at-risk mental states, as reported by Montemagni et al (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1381133/full), underscore that cognitive impairment leads to poor functional outcome across stages of schizophrenia. Neurobiological mechanisms of cognitive impairment of schizophrenia have been an issue of intensive investigations based on data from a variety of research modalities. For example, Liang et al. (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1288955/full) employed resting-state functional magnetic resonance imaging and found increased levels of low-frequency fluctuation, an indicator of spontaneous and intrinsic neuronal activity, in childhood and adolescence-onset schizophrenia. Specifically, a relationship was noted between this imaging parameter and the performance on the tests of some cognitive domains. Another line of neurophysiological markers of cognitive function comes from magneto-electrical paradigms, e.g., electroencephalography and magnetoencephalography (MEG). By using MEG, Okazaki et al.( https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2023.1217307/full) reported altered auditory change-detection responses in patients with schizophrenia as a neural substrate for misattribution of inner experience to external agents. Based on the membrane phospholipid hypothesis, Higuchi et al. (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1361997/full) found associations between cognitive function and altered unsaturated fatty acid composition in the membrane of erythrocytes in antipsychotic-free schizophrenia patients. Substantial efforts have been directed to the development of effective treatments of cognitive impairment of schizophrenia, with pharmacotherapy, non-invasive brain stimulation (NIBS) and cognitive rehabilitation as representative approaches. Accordingly, Li et al (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2024.1369532/full) presented data indicating domain-specific gender differences in cognitive improvements in patients with first-episode schizophrenia treated with antipsychotic drugs for two months. As a safe and feasible NIBS method, transcranial direct current stimulation was used by Yamada et al. (https://www.frontiersin.org/journals/psychiatry/articles/10.3389/fpsyt.2023.1243859/full) who argued the advantage of stimulation of prefrontal cortical areas for enhancing neurocognition and higher levels of functional outcome, while disturbances of social cognition may be alleviated specifically when temporal brain regions are stimulated. Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Accumulating evidence has shown that cognitive deficits are prevalent among patients with major depressive disorder (MDD). However, the pattern and extent of cognitive deficits in patients with first-episode and recurrent depression remain unclear.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Objective measures of cognitive function were assessed in 433 first-episode of depression (FED) patients, 206 recurrent major depression (RMD) patients during the acute phase and 287 healthy controls (HCs). Five domains of cognition (speed of processing, attention, verbal learning, visual learning and execution function) were assessed by seven neuropsychological tests. Neuropsychological data were transformed into T scores according to the Chinese norm.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Both FED and RMD patients displayed deficits of mild-to-medium severity across all cognitive domains and their subtests, relative to HCs (Cohen's <em>d</em> = 0.12-0.62). Among MDD patients, RMD patients exhibited poorer verbal learning (Cohen's <em>d</em> = 0.17) and visual learning (Cohen's <em>d</em> = 0.21) than FED patients.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>MDD patients in the acute phase exhibited broad cognitive impairments, and there were significant differences in verbal and visual learning between FED and RMD patients. Our findings indicate that more attention should be paid to the domains of verbal and visual learning in future screening assessments and targeted interventions.</p>

Abstract:Background: Cognitive deficits are a key source of disability in individuals with major depressive disorder (MDD) and worsen with disease progression. Despite their clinical relevance, the underlying mechanisms of cognitive deficits remain poorly elucidated, hampering effective treatment strategies. Emerging evidence suggests that alterations in white matter microstructure might contribute to cognitive dysfunction in MDD. We aimed to investigate the complex association between changes in white matter integrity, cognitive decline, and disease course in MDD in a comprehensive longitudinal dataset. Methods: In the naturalistic, observational, prospective, case-control Marburg-Münster Affective Disorders Cohort Study, individuals aged 18-65 years and of Caucasian ancestry were recruited from local psychiatric hospitals in Münster and Marburg, Germany, and newspaper advertisements. Individuals diagnosed with MDD and individuals without any history of psychiatric disorder (ie, healthy controls) were included in this subsample analysis. Participants had diffusion-weighted imaging, a battery of neuropsychological tests, and detailed clinical data collected at baseline and at 2 years of follow-up. We used linear mixed-effect models to compare changes in cognitive performance and white matter integrity between participants with MDD and healthy controls. Diffusion-weighted imaging analyses were conducted using tract-based spatial statistics. To correct for multiple comparisons, threshold free cluster enhancement (TFCE) was used to correct α-values at the family-wise error rate (FWE; ptfce-FWE). Effect sizes were estimated by conditional, partial R2 values (sr2) following the Nakagawa and Schielzeth method to quantify explained variance. The association between changes in cognitive performance and changes in white matter integrity was analysed. Finally, we examined whether the depressive disease course between assessments predicted cognitive performance at follow-up and whether white matter integrity mediated this association. People with lived experience were not involved in the research and writing process. Findings: 881 participants were selected for our study, of whom 418 (47%) had MDD (mean age 36·8 years [SD 13·4], 274 [66%] were female, and 144 [34%] were male) and 463 (53%) were healthy controls (mean age 35·6 years [13·5], 295 [64%] were female, and 168 [36%] were male). Baseline assessments were done between Sept 11, 2014, and June 3, 2019, and after a mean follow-up of 2·20 years (SD 0·19), follow-up assessments were done between Oct 6, 2016, and May 31, 2021. Participants with MDD had lower cognitive performance than did healthy controls (p<0·0001, sr2=0·056), regardless of timepoint. Analyses of diffusion-weighted imaging indicated a significant diagnosis × time interaction with a steeper decline in white matter integrity of the superior longitudinal fasciculus over time in participants with MDD than in healthy controls (ptfce-FWE=0·026, sr2=0·002). Furthermore, cognitive decline was robustly associated with the decline in white matter integrity over time across both groups (ptfce-FWE<0·0001, sr2=0·004). In participants with MDD, changes in white matter integrity (p=0·0040, β=0·071) and adverse depressive disease course (p=0·0022, β=-0·073) independently predicted lower cognitive performance at follow-up. Interpretation: Alterations of white matter integrity occurred over time to a greater extent in participants with MDD than in healthy controls, and decline in white matter integrity was associated with a decline in cognitive performance across groups. Our findings emphasise the crucial role of white matter microstructure and disease progression in depression-related cognitive dysfunction, making both priority targets for future treatment development.
