Abstract:Abstract Total skin electron therapy (TSET) has been used to treat mycosis fungoides since the 1950s. Practitioners of TSET rely on relatively crude, phantom-based point measurements for commissioning and treatment plan dosimetry. Using Monte Carlo simulation techniques, this study presents whole-body dosimetry for a patient receiving rotational, dual-field TSET. The Monte Carlo codes, BEAMnrc/DOSXYZnrc, were used to simulate 6 MeV electron beams to calculate skin dose from TSET. Simulations were validated with experimental measurements. The rotational dual-field technique uses extended source-to-surface distance with an acrylic beam degrader between the patient and incident beams. Simulations incorporated patient positioning: standing on a platform that rotates during radiation delivery. Resultant patient doses were analyzed as a function of skin depth-dose coverage and evaluated using dose-volume-histograms. Good agreement was obtained between simulations and measurements. For a cylinder with a 30 cm diameter, the depths that dose fell to 50% of the surface dose was 0.66 cm, 1.15 cm and 1.42 cm for thicknesses of 9 mm, 3 mm and without an acrylic scatter plate, respectively. The results are insensitive to cylinder diameter. Relatively uniform skin surface dose was obtained for skin in the torso area although large dose variations (>25%) were found in other areas resulting from partial beam shielding of the extremities. To achieve 95% mean dose to the first 5 mm of skin depth, the mean dose to skin depth of 5–10 mm and depth of 10–15 mm from the skin surface was 74% (57%) and 50% (25%) of the prescribed dose when using a 3 mm (9 mm) thickness scatter plate, respectively. As a result of this investigation on patient skin dose distributions we changed our patient treatments to use a 3 mm instead of a 9 mm thickness Acrylic scatter plate for clinically preferred skin depth dose coverage.

Impact Of Interfractional Motion On Hypofractionated Pencil Beam Scanning Proton Therapy And Vmat Delivery For Prostate Cancer

Abstract:PurposeHypofractionated radiotherapy of prostate cancer is gaining clinical acceptance given its potential increase in therapeutic ratio and evidence for noninferiority and lack of added late toxicities compared to conventional fractionation. However, concerns have been raised that smaller number of fractions might lead to larger dosimetric influence by interfractional motion. We aim to compare the effect of these variations on hypofractionated pencil beam scanning (PBS) proton therapy and volumetric modulated arc therapy (VMAT) for localized prostate cancer.MethodsWeekly CT images were acquired for 6 patients participating in a randomized clinical trial. PBS plans featuring bilateral (BL) and a combination of lateral and anterior-oblique beams (AOL), and VMAT plans were created. All patients were treated to a conventional 79.2Gy total dose in 44 fractions. For this study, hypofractionated dose to the prostate gland was 51.6Gy in 12 fractions or 36.25Gy in 5 fractions, and 32.8, and 23.1Gy to proximal seminal vesicles, respectively. Patients were simulated with endorectal balloons to aid gland immobilization. Three fiducial markers were implanted for setup guidance. All plans were recomputed on the weekly CT images after aligning with the simulation CT. The entire set of 9 CT images was used for dose recalculation for 12-fraction and only 5 used for the 5-fraction case. Adaptive range adjustments were applied to anterior-oblique beams assuming clinical availability of invivo range verification. Fractional doses were summed using deformable dose accumulation to approximate the delivered dose. Biologically equivalent dose to 2Gy(EQD2) was calculated assuming / of 1.5Gy for prostate and 3Gy for bladder and rectum.ResultsThe median delivered prostate D-98 was 0.13/0.14/0.13Gy(EQD2) smaller than planned for PBS-BL, 0.13/0.27/0.17Gy(EQD2) for PBS-AOL and 0.59/0.66/0.59Gy(EQD2) for VMAT, for 44/12/5 fractions, respectively. The largest D-98 reduction was 1.5 and 3.5Gy(EQD2) for CTV1 and CTV2, respectively. Target dose degradation was comparable for all fractionation schemes within each modality. The maximum increase in rectum D-2 was 0.98Gy(EQD2) for a 5-fraction PBS case.ConclusionsThe robustness of PBS and VMAT were comparable for all patients for the studied fractionations. The delivered target dose generally remained within clinical tolerance and the deviations were relatively minor for both fractionation schemes. The delivered OAR dose stayed in compliance with the RTOG hypofractionation constraints for all cases.
Development of a 3D-printed phantom for total skin electron therapy dose assessment

Abstract:Purpose: Total skin electron therapy (TSET) is a complex radiotherapy technique, posing challenges in commissioning and quality assurance (QA), especially due to significant variability in patient body shapes. Previous studies have correlated dose with factors such as obesity index, height, and gender. However, current treatment planning systems cannot simulate TSET plans, necessitating heavy reliance on QA methods using standardized anthropomorphic phantoms and in-vivo dosimetry. Given the relatively few studies on rotational techniques, comprehensive data in commissioning could streamline the process. Methods: Developing a full-body phantom would enable a more thorough TSET commissioning process, including testing for position-specific dose distributions and comprehensive measurements across all body surfaces, unlike the typical torso-only phantoms. This was created using digital modeling software, fabricated using 3D-printing FDM technology, and filled with tissue-equivalent gelatine. The phantom was positioned at an SSD of 340 cm and irradiated with a standard rotational TSET plan using the 6E HDTSE mode on a Varian TrueBeam linac at gantry angles of ± 18° from the horizontal. The dose was measured at over 50 points across the surface using Gafchromic EBT3 film. Results: Dose distributions were generally consistent with existing literature values from in-vivo dosimetry, with several position-specific differences identified, including the hands and scalp compared to conventional positions. Hotspots were observed for the mid-dorsum of the foot and nose, with areas under 80% of the dose identified as the soles of the feet, perineum, vertex of the scalp, top of the shoulder, and palm of the hand. Additionally, analysis using an interpolated dose heatmap found that 90% of the pixel area received a dose within 10% of the prescribed dose, indicating good uniformity with the commissioned technique. Conclusions: With high agreement with the current literature, a 3D-printed phantom proves effective for measuring doses in areas typically unmeasurable in TSET commissioning.

Abstract:Current Total Skin Electron Therapy (TSET) Stanford technique for cutaneous lymphoma, established in the 70's, involves a unique irradiation setup, i.e. patient's position and beam arrangement, for all patients with ensuing great variability in dose distribution and difficult dose optimization. A Geant4-based simulation has been developed to explore the possibility of personalizing the dose to each patient's anatomy. To achieve this optimization of the treatment method, this project enrolls different aspects of the clinical and computational techniques: starting with the knowledge of the experimental parameters involving TSET practice, passing through an innovative approach to model the patient's anatomy, a precise description of the electron beam and a validated configuration of the physics models handling the interactions of the electrons and of secondary particles. The Geant4-based simulation models the patient as a tessellated solid derived from the optical scan of her/his body, realistically reproduces the irradiation environment in detail and calculates the energy deposition corresponding to each facet of the patient's scanned surface. The resulting three-dimensional dose distribution constitutes the basis for the personalization of the medical treatement as appropriate to each patient's specific characteristics.

Abstract:AIM: To give a technical description and present the dosimetric proporties of the total skin electron beam technique implemented at Heidelberg University Hospital.BACKGROUND: Techniques used for total skin electron beam irradiation were developed as early as in the 1960s to 1980s and have, since then, hardly changed. However, new measurements of the established methods allow deeper insight into the dose distributions and reasons for possible deviations from uniform dose.MATERIALS AND METHODS: The TSEI technique applied at Heidelberg University Hospital since 1992 consists of irradiating the patient with a superposition of two beams of low energy electrons at gantry angles of 72° and 108° while he is rotating in a standing position on a turntable at 370 cm distance from the accelerator. The energy of the electron beam is degraded to 3.9 MeV by passing through an attenuator of 6 mm of Perspex. A recent re-measurement of the dose distribution is presented using modern dosimetry tools like a linear array of ionization chambers in combination with established methods like thermoluminescent detectors and film dosimetry.RESULTS: The measurements show a strong dependence of dose uniformity on details of the setup like gantry angles.CONCLUSIONS: Dose uniformity of -4/+8% to the majority of the patient's skin can be achieved, however, for the described rotational technique overdoses up to more than 20% in small regions seem unavoidable.

Abstract:Purpose: To quantify interfractional anatomical variations and their dosimetric impact during the course of fractionated proton therapy (PT) of prostate cancer and to assess the robustness of the current treatment planning techniques.Methods: Simulation and daily in-room CT scans from ten prostate carcinoma patients were analyzed. PT treatment plans (78 Gy in 39 fractions of 2 Gy) were created on the simulation CT, delivering 25 fractions to PTV1 (expanded from prostate and seminal vesicles), followed by 14 boost fractions to PTV2 (expanded from prostate). Plans were subsequently applied to daily CT, with beams aligned to the prostate center in the sagittal plane. For five patients having a sufficiently large daily imaging volume, structure contours were manually drawn, and plans were evaluated for all CT sets. For the other five patients, the plans were evaluated for six selected fractions. The daily CT was matched to the simulation CT through deformable registration. The registration accuracy was validated for each fraction, and the three patients with a large number of accurately registered fractions were used for dose accumulation.Results: In individual fractions, the coverage of the prostate, seminal vesicles, and PTV1 was generally maintained at the corresponding prescription dose. For PTV2, the volume covered by the fractional prescription dose of 2 Gy (i.e., V2) was, on average, reduced by less than 3% compared to the simulation plan. Among the 225 (39 x 5 + 6 x 5) fractions examined, 15 showed a V2 reduction larger than 5%, of which ten were caused by a large variation in rectal gas, and five were due to a prostate shift in the craniocaudal direction. The fractional dose to the anterior rectal wall was found to increase for one patient who had large rectal gas volume in 25 of the 39 fractions, and another who experienced significant prostate volume reduction during the treatment. The fractional bladder dose generally increased with decreasing fullness. In the total accumulated dose for the three patients after excluding a few fractions with inaccurate registration due to a large amount of rectal gas (a condition inconsistent with RTOG protocol), 98.5%, 96.6%, and 98.2% of the PTV2 received the prescription dose of 78 Gy. The V75 and V70 of the anterior rectal wall and bladder both remained within tolerance.Conclusions: The results confirm that the PT planning techniques and dose constraints used at our institution ensure that target coverage to the prescription dose is maintained in the presence of interfractional anatomical variations. Dose coverage in individual fractions can be compromised, and normal tissue dose increased, due to deviations in the bladder and rectal volume compared to the simulation plans or progressive changes in the prostate volume during the treatment. A Clinical Monte Carlo Dosimetric Evaluation in Accelerated Partial Breast Electronic Brachytherapy

Abstract:Electronic brachytherapy has been recently utilized in accelerated partial breast irradiation. Deficient tissue inhomogeneity correction in the dose calculation draws physicians' attentions. In this study, a clinical Monte Carlo based dose evaluation was assessed to demonstrate these dosimetric impacts. The dosimetric characteristics of Xoft S700 Axxent X-ray source (Xoft, Fremont, CA) were first evaluated using EGS4 Monte Carlo code to verify the calculation model and source parameter accuracy. A whole body female photographic phantom with defined structures derived from National Library of Medicine's Visible Human Project utilizing all clinical parameters was then used in the dose calculation. A 5cm diameter spherical balloon was inserted in the right breast of the phantom. The volume expanded 1cm from the balloon surface was designed to define as planning target volume (PTV). The prescribed dose per fraction was 3.4 Gy to 5 user-selected points (anterior, posterior, right lateral, superior and inferior) of the PTV. Dose, dose volume histograms of the PTV and normal tissues, isodose distributions in axial, sagittal and coronal planes crossing the center of the balloon were evaluated. Using measured 50 kVp source spectrum provided by Xoft Inc, the simulated radial dose function agrees well with the published data within 3%. After corrected for vender addressed 6% balloon attenuation, the PTV dose reported from the commercial treatment planning system is within 1% compared to our calculation result. The simulation shows 100% PTV covered by 90.6% of the prescription dose with excellent dose uniformity. When accounted for the tissue heterogeneity, changes in the isodose distribution were clearly observed. The two dimensional dose distributions show high skin and rib doses due to photoelectric effect with reasonably acceptable doses to all other surrounding normal tissues. Our preliminary EGS4 Monte Carlo simulation study demonstrates the dosimetric impact in the electronic brachytherapy, especially on the surrounding normal tissues, when tissue inhomogeneity consideration is taken into account.

Evaluation of the cumulative Cherenkov converted dose on TSET patients with multiple Cherenkov cameras

Abstract:Cherenkov images can be used for the quality assurance of dose homogeneity in total skin electron therapy (TSET). For the dose mapping purpose, this study reconstructed the patient model from 3D scans using registration algorithms and computer animation techniques. The Cherenkov light emission of the patient's surface was extracted from multi-view Cherenkov images, converted into dose distribution, and projected onto the patient's 3D model, allowing for dose cumulation and evaluation. The projected result from multiple Cherenkov cameras provides additional information about Cherenkov emission on the sides of the patients, which improves the agreement between the Cherenkov converted dose and the OSLD measurements.

Abstract:Purpose To verify the practicality and safety of a treatment chair with six degrees of freedom (6DTC) through demonstrating the efficacy of the workflow in clinical settings and analyzing the obtained technical data, including intra-fraction patient movement during the use of the 6DTC. Materials and Methods A clinical study was designed and conducted to test the clinical treatment workflow and the safety of the 6DTC. Based on the demonstrated dosimetric advantages, fifteen patients with head and neck tumors were selected and treated with the 6DTC. The positional error at the first beam position (PE-B1) and the second beam position (PE-B2) were analyzed and compared with the results from daily quality assurance (QA) procedures of the 6DTC and imaging system performed each day before clinical treatment. The intra-fraction patient movement was derived from the total patient alignment positional error and the QA data based on a Gaussian distribution formulism. Results The QA results showed sub-millimeter mechanical accuracy of the 6DTC over the course of the clinical study. For 150 patient treatment fractions, the mean deviations between PE-B1 and PE-B2 were 0.13mm (SD 0.88mm), 0.25mm (SD 1.17mm), -0.57mm (SD 0.85mm), 0.02° (SD 0.35°), 0.00° (SD 0.37°), and -0.02° (SD 0.37°) in the x, y, z (translational), and u, v, w (rotational) directions, respectively. The calculated intra-fraction patient movement was -0.08mm (SD 0.56mm), 0.71mm (SD 1.12mm), -0.52mm (SD 0.84mm), 0.10° (SD 0.32°), 0.09° (SD 0.36°), and -0.04° (SD 0.36°) in the x, y, z, u, v, w directions, respectively. Conclusions The performance stability of the 6DTC was satisfactory. The position accuracy and intra-fraction patient movement in an upright posture with the 6DTC were verified and found adequate for clinical implementation.

Abstract:Purpose: It has been long known that patients treated with radiation carry a risk of developing second cancers in their lifetimes as a result of their treatments. In addition to hosting a symposium on “Secondary Cancer Risks for Emerging Radiation Treatments,” the AAPM has recently formed Task Group 158 that will provide recommendations on the dosimetry protocols for quantifying scattered radiation dose outside the treatment volume. Measurements are useful in providing a relative comparison of different treatment modalities or beam characteristics. On the other hand, more precise organ dose equivalent values need to be calculated from Monte Carlo simulations and anatomically realistic computational phantoms. This study outlines a computational platform that integrates anatomically‐refined virtual patient models with a detailed model of a medical accelerator head to calculate scattered doses to patients treated with radiation. Method and Materials: A detailed model of a 6‐MV Varian Clinac 2100C was developed in MCNPX. Several virtual patient models have been integrated with the accelerator head model in MCNPX. Various conventional and IMRT treatment plans have been considered. Results: Average organ doses are significantly different than point‐wise measurement values. The differences can be attributed to various sources of scattered radiation that deposits dose in the organ. Large dose gradients will be present throughout organs at short and intermediate distances from the tumor volume, while organs at longer distances will have a more uniform dose distribution. Conclusion: A computational platform to calculate organ dose from scattered radiation during radiation treatments has been demonstrated. Results indicate that average organ/tissue doses are significantly different than point‐wise measurements. Organ doses calculated in this manner will likely provide more accurate organ dose information for epidemiologic studies and for assessing the risk during the treatment planning.
A Fractionated X-ray Total Body Irradiation Technique with Patients Lying on Side and in Vivo Dosimetry Analysis

Abstract:Objective To investigate an X-ray total body irradiation (TBI) technique using anterior-posterior opposed fields with patients at the side-lying position,and to analyze the real-time in vivo dosimetry results.Methods The accelerator with 10 MV X-rays of Varian Trilogy was used for the TBI with the extended source to skin distance of 390 cm.The percent depth dose,off axis factors and absolute dose output were measured.The dose accuracy and homogeneity was monitored real-time using multichannel diode dosimeter for 10 patients.The monitored sites included forehead,mandible,suprasternal fossae,xiphoid,umbilicus,pelvis,middle of thigh,knee,middle of leg and ankle.The patients were irradiated at the side-lying position,with the prescription dose of 1 200 cGy/6 f during 3 days,the middle line dose rate of 5.0 cGy/min.Solid water was used for the compensation of the dose homogeneity.Results The off axis dose homogeneity was less than ± 5.0％ for the TBI geometry.The absolute dose output was 0.072 1 cGy/MU at the maximum dose point.The total body irradiation was finished smoothly for the 10 patients lying on side.The deviation of monitored total dose from the total prescription dose was within -4.9％ to 6.7％ for the 10 monitored sites.The monitored dose homogeneity was less than 5.0％.Conclusions The fractionated anterior-posterior opposed parallel TBI can be finished smoothly with patients side-lying.Accurate and homogenous dose distribution can be obtained using real-time dose monitoring and compensation with solid water.

Abstract:Objective: This study developed a three-dimensional visualization method for presenting the geometric pattern of the relative dose generated by Monte Carlo (MC) simulation and validated the simulated dose values against the physical measurement. Methods: In fluoroscopy and interventional radiology examinations, relatively high level of occupational dose is delivered to healthcare workers due to prolonged radiation exposure in imaging procedures. As the spatial difference in dose is never negligible, the scatter radiation distribution under fluoroscopic exposures is thus worth investigating. MC simulation is a sophisticated statistical method, which has been widely applied for modeling scatter radiation in general X-ray examination room. However, the application and validation of MC simulation under fluoroscopy setting have not been found yet. In this study, a stack of tissue equivalent slabs were used as the object under fluoroscopic irradiation. EGSnrc-based DOSXYZnrc code was applied to simulate the scatter dose distribution and the physical measurement of radiation dose was performed using an ionization chamber radiation detector. Results: At 19 representative locations taking into account the work area and radiosensitive organs of healthcare workers, the trend compliance of the simulated with the measured dose values was examined using the correlation analysis. A significant monotonic association between the MC simulation and physical measurement of dose values was identified ( r s =0.822 and P <.01). At the same horizontal distance from the irradiation axis, it was observed that the air dose was relatively higher at the level of gonad region than at the eye level. Conclusions: The proposed visualization approach illustrates a three dimensional dose simulation in local display density, which arouses the awareness about prolonged radiation exposure in clinical environment. The reliability and validity of the simulation were examined.
Quantification of Dosimetry Improvement With or Without Patient Surface Guidance

Abstract:Purpose: Noncoplanar beams and arcs are routinely used to improve dosimetry for intracranial cases, but their application for extracranial cases has been hampered by the risk of collision. This has led to conservative beam selection whose impact on plan dosimetry has not been previously studied. Methods and materials: A full-body 3-dimensional patient surface was acquired using optical cameras for a single lung patient at the time of computed tomography simulation. Eight stereotactic body radiation therapy (SBRT) plans were created for the patient, with varying degrees of noncoplanarity and deliverability. The plans included volumetric modulated arc therapy and intensity modulated radiation therapy (IMRT) plans ranging from simple, coplanar arcs to multiple noncoplanar arcs and IMRT beams. A total of 70 fields were created across the 8 plans, of which 21 fields were undeliverable with a 5-cm buffer. Organs-at-risk (OARs) metrics including R50, Dmax 2 cm from the PTV, lung V20, and chest wall V30 were evaluated. Five expert SBRT dosimetrists from 5 institutions evaluated field deliverability, with or without the guidance of the clearance map. Results: In the dosimetry evaluation, a clear trend in increasing dosimetric compactness and OAR sparing is observed with increasing plan noncoplanarity. R50, Dmax 2 cm, lung V20, and chest wall V30 decreased 41%, 39%, 43%, and 57%, respectively, from plan 1 (2 coplanar partial arcs) to plan 8 (19 noncoplanar IMRT beams). In the observer tests, the expert dosimetrists' ability to accurately discern beam deliverability because of collision significantly increases with the clearance map. The errors in predicting colliding fields were eliminated using the whole-body surface and clearance map, and the user was able to select fields based on plan quality and patient comfort instead of being overly conservative. Conclusion: The study shows that incorporating a personalized, whole-body clearance map in the treatment planning workflow can facilitate the adoption of noncoplanar beams or arcs that benefit the SBRT plan dosimetry.
In Silico Studies of the Impact of Rotational Errors on Translation Shifts and Dose Distribution in Image-Guided Radiotherapy.

Abstract:Objective: To compare the 6-dimensional errors of different immobilization devices and body regions based on 3-dimensional cone beam computed tomography for image-guided radiotherapy and to further quantitatively evaluate the impact of rotational corrections on translational shifts and dose distribution based on anthropomorphic phantoms. Materials and Methods: Two hundred ninety patients with cone beam computed tomographies from 3835 fractions were retrospectively analyzed for brain, head & neck, chest, abdomen, pelvis, and breast cases. A phantom experiment was conducted to investigate the impact of rotational errors on translational shifts using cone beam computed tomography and the registration system. For the dosimetry study, pitch rotations were simulated by adjusting the breast bracket by ±2.5°. Roll and yaw rotations were simulated by rotating the gantry and couch in the planning system by ±3.0°, respectively. The original plan for the breast region was designed in the computed tomography image space without rotation. With the same planning parameters, the original plan was transplanted into the image space with different rotations for dose recalculation. The effect of these errors on the breast target and organs at risk was assessed by dose-volume histograms. Results: Most of the mean rotational errors in the breast region were >1°. A single uncorrected yaw of 3° caused a change of 2.9 mm in longitudinal translation. A phantom study for the breast region demonstrated that when the pitch rotations were −2.5° and 2.5° and roll and yaw were both 3°, the reductions in the planning target volumes-V 50 Gy were 20.07% and 29.58% of the original values, respectively. When the pitch rotation was +2.5°, the left lung V 5 Gy and heart Dmean were 7.49% and 165.76 Gy larger, respectively, than the original values. Conclusions: Uncorrected rotations may cause changes in the values and directions of translational shifts. Rotational corrections may improve the patient setup and dose distribution accuracy.

Abstract:Purpose The purpose of this study is to characterize the dosimetric properties of a commercial brass GRID collimator for high energy photon beams including 15 and 10 MV. Then, the difference in dosimetric parameters of GRID beams among different energies and linacs was evaluated. Method A water tank scanning system was used to acquire the dosimetric parameters, including the percentage depth dose (PDD), beam profiles, peak to valley dose ratios (PVDRs), and output factors (OFs). The profiles at various depths were measured at 100 cm source to surface distance (SSD), and field sizes of 10 × 10 cm2 and 20 × 20 cm2 on three linacs. The PVDRs and OFs were measured and compared with the treatment planning system (TPS) calculations. Results Compared with the open beam data, there were noticeable changes in PDDs of GRID fields across all the energies. The GRID fields demonstrated a maximal of 3 mm shift in dmax (Truebeam STX, 15MV, 10 × 10 cm2). The PVDR decreased as beam energy increases. The difference in PVDRs between Trilogy and Truebeam STx using 6MV and 15MV was 1.5% ± 4.0% and 2.1% ± 4.3%, respectively. However, two Truebeam linacs demonstrated less than 2% difference in PVDRs. The OF of the GRID field was dependent on the energy and field size. The measured PDDs, PVDRs, and OFs agreed with the TPS calculations within 3% difference. The TPS calculations agreed with the measurements when using 1 mm calculation resolution. Conclusion The dosimetric characteristics of high‐energy GRID fields, especially PVDR, significantly differ from those of low‐energy GRID fields. Two Truebeam machines are interchangeable for GRID therapy, while a pronounced difference was observed between Truebeam and Trilogy. A series of empirical equations and reference look‐up tables for GRID therapy can be generated to facilitate clinical applications.

Abstract:Background Electron beams are used at extended distances ranging between 300 to 700 cm to uniformly cover the entirety of the patient's skin for total skin electron therapy (TSET). Even with electron beams utilizing the high dose rate total skin electron (HDTSe) mode from the Varian 23iX or TrueBeam accelerators, the dose rate is only 2500 cGy/min at source‐to‐surface distance (SSD) = 100 cm. At extended distances, the decrease in dose rate leads to long beam delivery times that can limit or even prevent the use of the treatment for patients who, in their weakened condition, may be unable to stand on their own for extended periods of time. Previously, to increase dose rate, a customized 6 MeV electron beam was created by removing the x‐ray target, flattening filter, beam monitor chamber, and so forth. from the beam path (Chen, et at IJROBP 59, 2004) for TSET. Using this scattering‐foil free (SFF) electron beam requires the treatment distance be extended to 700 cm to achieve dose uniformity from the single beam. This room size requirement has limited the widespread use of the 6 MeV‐SFF beam. Purpose This study explores an application of a dual‐field technique with a 6 MeV‐SFF beam to provide broad and uniform electron fields to reduce the treatment distances in order to overcome treatment room size limitations. Methods The EGSnrc system was used to generate incident beams. Gantry angles between 6 MeV‐SFF dual‐fields were optimized to achieve the similar patient skin dose distribution resulting from a standard 6 MeV‐HDTSe dual‐field configuration. The patient skin dose comparisons were performed based on the patient treatment setup geometries using dose‐volume‐histograms. Results Similar dose coverage can be achieved between 6 MeV‐SFF and 6 MeV‐HDTSe beams by reducing gantry angles between dual‐field geometries by 8° and 7° at treatment distances of 400 and 500 cm, respectively. To achieve 95% mean dose to the first 5 mm of skin depth in the torso area, the mean dose to depths of 5–10 mm and 10–15 mm below the skin surface was 74% (74%) and 49% (50%) of the prescribed dose when using 6 MeV‐SFF (6 MeV‐HDTSe) beam, respectively. Conclusions The 6 MeV‐SFF electron beam is feasible to provide similar TSET skin dose coverage at SSD ≥ 400 cm using a dual‐field technique. The dose rate of the 6 MeV‐SFF beam is about 4 times that of current available 6 MeV‐HDTSe beams at treatment distances of 400–500 cm, which significantly shortens the treatment beam‐on time and makes TSET available to patients in weakened conditions.

Abstract:Background The requirement for precise and effective delivery of the actual dose to the patient grows along with the complexity of breast cancer radiotherapy. Dosimetry during treatment has become a crucial component of guaranteeing the efficacy and security. Purpose To propose a dosimetry method during breast cancer radiotherapy based on body surface changes. Methods A total of 29 left breast cancer radiotherapy cases were retroactively retrieved from an earlier database for analysis. Non‐rigid image registration and dose recalculation of the planning computed tomography (CT) referring to the Cone‐beam computed tomography were performed to obtain dose changes. The study used 3D point cloud feature extraction to characterize body surface changes. Based on the correlation proof, a mapping model is developed between body surface changes and dose changes using neural network framework. The MSE metrics, the Euclidean distances of feature points and the 3D gamma pass rate metric were used to assess the prediction accuracy. Results A strong correlation exist between body surface changes and dose changes (first canonical correlation coefficient = 0.950). For the dose deformation field and dose amplitude difference in the test set, the MSE of the predicted and actual values were 0.136 pixels and 0.229 cGy, respectively. After deforming the planning dose into a deformed one, the feature points' Euclidean distance between it and the recalculated dose changes from 9.267 ± 1.879 mm to 0.456 ± 0.374 mm. The 3D gamma pass rate of 90% or higher for the 2 mm/2% criteria were achieved by 80.8% of all cases, with a minimum pass rate of 75.9% and a maximum pass rate of 99.6%. Pass rate for the 3 mm/2% criteria ranged from 87.8% to 99.8%, with 92.3% of the cases having a pass rate of 90% or higher. Conclusions This study provides a dosimetry method that is non‐invasive, real‐time, and requires no additional dose for breast cancer radiotherapy.

Abstract:Purpose To investigate bolus design and VMAT optimization settings for total scalp irradiation. Methods Three silicone bolus designs (flat, hat, and custom) from .decimal were evaluated for adherence to five anthropomorphic head phantoms. Flat bolus was cut from a silicone sheet. Generic hat bolus resembles an elongated swim cap while custom bolus is manufactured by injecting silicone into a 3D printed mold. Bolus placement time was recorded. Air gaps between bolus and scalp were quantified on CT images. The dosimetric effect of air gaps on target coverage was evaluated in a treatment planning study where the scalp was planned to 60 Gy in 30 fractions. A noncoplanar VMAT technique based on gEUD penalties was investigated that explored the full range of gEUD alpha values to determine which settings achieve sufficient target coverage while minimizing brain dose. Abstract:The rate of dose accumulation within a given area of a target volume tends to vary significantly for non-isocentric delivery systems such as Cyberknife stereotactic body radiotherapy. In this study, we investigated whether intra-target temporal dose distributions produce significant variations in the biological equivalent dose. For the study, time courses of ten patients were reconstructed and calculation of a biologically equivalent uniform dose (EUD) was performed using a formula derived from the linear quadratic model (α/β = 3 for prostate cancer cells). The calculated EUD values obtained for the actual patient treatments were then compared with theoretical EUD values for delivering the same physical dose distribution except that the whole target being irradiated continuously (e.g. large-field 'dose-bathing' type of delivery). For all the case, the EUDs for the actual treatment delivery were found to correlate strongly with the EUDs for the large-field delivery: a linear correlation coefficient of R² = 0.98 was obtained and the average EUD for the actual Cyberknife delivery was somewhat higher (5.0 ± 4.7%) than that for the large-field delivery. However, no statistical significance was detected between the two types of delivery (p = 0.21). We concluded that non-isocentric small-field Cyberknife delivery produced consistent biological dosing that tracked well with the constant-dose-rate, large-field-type delivery for prostate stereotactic body radiotherapy.
