Abstract:OBJECTIVE: To evaluate the relative risk of pulmonary disease among patients with psoriasis, psoriatic arthritis, and inflammatory bowel disease treated with methotrexate.DATA SOURCES: PubMed, Cochrane central register of controlled trials, and Embase to 9 January 2014.STUDY SELECTION: Double blind randomised controlled trials of methotrexate versus placebo or active comparator agents in adults with psoriatic arthritis, psoriasis, or inflammatory bowel disease. Studies with fewer than 50 participants or of less than 12 weeks' duration were excluded.DATA SYNTHESIS: Two investigators independently searched both databases. All authors reviewed selected studies. We compared relative risk differences using the Mantel-Haenszel random effects method to assess total respiratory adverse events, infectious respiratory adverse events, non-infectious respiratory adverse events, interstitial lung disease, and death.RESULTS: Seven studies met our inclusion criteria, six with placebo as the comparator. Heterogeneity across the studies was not significant (I(2)=0%), allowing combination of trial results. 504 respiratory adverse events were documented in 1630 participants. Methotrexate was not associated with an increased risk of adverse respiratory events (relative risk 1.03, 95% confidence interval 0.90 to 1.17), respiratory infections (1.02, 0.88 to 1.19), or non-infectious respiratory events (1.07, 0.58 to 1.96). No pulmonary deaths occurred.CONCLUSIONS: Findings suggested that there was no increased risk of lung disease in methotrexate treated patients with non-malignant inflammatory diseases. Given the limitations of the study, however, we cannot exclude a small but clinically important risk.
Influence of C677T Polymorphism of 5,10-Methylenetetrahydrofolate Reductase on Methotrexate Toxicity

Abstract:OBJECTIVE To investigate the relationship between the mutation of 5,10-methylenetetrahydro-folate reductase(MTHFR) gene 677 C→T and the adverse drug reaction of methotrexate(MTX) in patients with rheumatoid arthritis((RA).)METHODS A total of 210 patients with RA were divided into MTX group,MTX+sulphasalazine(SASP) group,MTX+ Leflunomide(LEF) group and LEF+SASP group.100 healthy subjects were chosed as control group. PCR-restrictive fragment length polymorphism(RFLP) analysis was conducted to test mutation.The clinical measurements,laboratory measurements,the Disease Activity Score(DAS) were evaluated during 48 weeks.And the cases of adverse drug reaction(ADR) were also collected.RESULTS No difference was tained the three with genotypes of MTHFR.The frequencies of adverse drug reaction of patients with heterozygous(CT),and homozygous(TT) mutation in MTX,MTX+SASP,MTX+LEF was more than that without mutation(CC).Patients with heterozygous or homozygous mutation in the MTHFR gene induced a significantly higher plasma homocysteine at week 48,compared with those without mutation.But there was no statistical significance observed amonc CT,TT and CC when in LEF+SASP group.CONCLUSION MTHFR gene C677T mutation is probably one of the genetic risk factors for adverse drug reaction of MTX.
Pesticide Exposure As a Risk Factor for Myelodysplastic Syndromes: a Meta-Analysis Based on 1,942 Cases and 5,359 Controls.

Abstract:Objective Pesticide exposure has been linked to increased risk of cancer at several sites, but its association with risk of myelodysplastic syndromes (MDS) is still unclear. A meta-analysis of studies published through April, 2014 was performed to investigate the association of pesticide exposure with the risk of MDS. Methods Studies were identified by searching the Web of Science, Cochrane Library and PubMed databases. Summary odds ratios (ORs) with corresponding 95% confidence intervals (CIs) were calculated using random- or fixed-effect models. Results This meta-analysis included 11 case-control studies, all of which demonstrated a correlation between pesticide exposure and a statistically significant increased risk of MDS (OR = 1.95, 95% CI 1.23–3.09). In subgroup analyses, patients with pesticide exposure had increased risk of developing MDS if they were living in the Europe or Asia and had refractory anemia (RA) or RA with ringed sideroblasts (RARS). Moreover, in the analysis by specific pesticides, increased risk was associated with exposure to insecticides (OR = 1.71, 95% CI 1.22–2.40) but not exposure to herbicides or fungicides. Conclusion This meta-analysis supports the hypothesis that exposure to pesticides increases the risk of developing MDS. Further prospective cohort studies are warranted to verify the association and guide clinical practice in MDS prevention.
Risk of malignant melanoma and non-melanoma skin cancer in rheumatoid arthritis patients initiating methotrexate versus hydroxychloroquine: a cohort study

Abstract:Objectives: To characterise the incidence rate of skin cancer associated with methotrexate and hydroxychloroquine in older adults with rheumatoid arthritis (RA). Methods: RA patients aged ≥65 years who initiated methotrexate or hydroxychloroquine as their first disease modifying antirheumatic drugs (DMARDs). The primary outcome was new occurrence of any skin cancer (i.e. malignant melanoma or non-melanoma skin cancer; NMSC) based on validated algorithms (positive predictive value >83%). Secondary outcomes were malignant melanoma, NMSC, basal cell carcinoma (BCC), and squamous cell carcinoma (SCC). We estimated the incidence rates (IRs) and hazard ratios (HRs) for each outcome in the 1:1 propensity score (PS)-matched methotrexate and hydroxychloroquine groups. Results: We included 24,577 PS-matched pairs of methotrexate and hydroxychloroquine initiators. Compared with hydroxychloroquine (IR 25.20/1,000 person-years), methotrexate initiators (IR 26.21/1,000 person-years) had a similar risk of any skin cancer [HR 1.03 -(95%CI 0.92, 1.14)] over a mean follow-up of 388 days. The HR (95%CI) associated with methotrexate was 1.39 (0.87, 2.21) for malignant melanoma, 1.01(0.90, 1.12) for NMSC, 1.37 (1.13, 1.66) for BCC, and 0.79 (0.63, 0.99) for SCC compared with hydroxychloroquine. Conclusions: In this large cohort of older RA patients initiating methotrexate or hydroxychloroquine as their first DMARD, we found no difference in the risk of skin cancer including malignant melanoma and NMSC. However, for specific components of NMSC, methotrexate initiators had higher risk of BCC but lower risk of SCC compared with hydroxychloroquine initiators.

Abstract:BackgroundRheumatoid arthritis(RA) sufferers have a higher mortality risk than the healthy population, and methotrexate (MTX) as a base drug for RA treatment is believed to affect patients mortality. Systematic analyses of MTX and RA mortality are lacking and it is still confused about the role of MTX on the long-term prognosis of RA.MethodsWe performed a systematic review and meta-analysis to identify any influence of MTX on mortality among RA patients. Hazard ratio(HR) for all-cause mortality were pooled in a meta-analysis, and HR for mortality from RA with cardiovascular diseases (RA-CVD) and mortality from RA associated interstitial lung diseases (RA-ILD) were also pooled and analyzed.ResultsFifteen studies were eventually included. Meta-analysis of data from 15 studies on overall mortality showed that MTX significantly reduced mortality in patients with RA (HR = 0.59, 95%CI 0.50–0.71, P < 0.001), MTX was independently associated with decreased RA-CVD-induced mortality (HR = 0.72, 95%CI 0.53–0.97, P = 0.031). In the meanwhile, MTX was also significantly reduced mortality in RA-ILD (HR = 0.44, 95%CI 0.20–0.95, P = 0.037).ConclusionMTX can significantly decrease the overall mortality for RA patients, specifically, RA-CVD- and RA-ILD-induced mortality were reduced.

Abstract:The high risk of cardiovascular disease is well recognized in rheumatoid arthritis. Type 2 diabetes also attributes to this increase in risk. Rheumatoid arthritis is a chronic inflammatory condition, which aggravates insulin resistance, placing the patients at a higher risk of type 2 diabetes and subsequent cardiovascular outcomes. Methotrexate treatment, as a gold standard anti-inflammatory drug in the treatment of rheumatoid arthritis has shown beneficial effects on cardiovascular health. However, its impact on type 2 diabetes is still unknown.To assess the strength of the association between exposure to methotrexate and the rate of development of type 2 diabetes in rheumatoid arthritis patients.All rheumatoid arthritis studies reporting the use of methotrexate as an exposure and type 2 diabetes as an outcome were searched until March 2020 using MEDLINE, Cochrane and Scopus databases. Studies were included if the diagnosis of rheumatoid arthritis was made according to current guidelines or by a rheumatologist, and if there was information about methotrexate exposure and the type 2 diabetes outcome. The author and an independent assessor evaluated the articles for eligibility. Meta-analyses combined relative risk estimates from each study where raw counts were available.Sixteen studies reporting sufficient data for inclusion in the meta-analyses were identified. Methotrexate showed a promising effect on the risk of type 2 diabetes as this risk decreased in rheumatoid arthritis patients using methotrexate (Relative risk 0.48, 95% CI 0.16, 1.43).Rheumatoid arthritis patients on methotrexate treatment had a lower risk of developing type 2 diabetes compared to rheumatoid arthritis patients not exposed to methotrexate. This finding highlights the need for future, randomized control trials to confirm the beneficial effect of methotrexate on type 2 diabetes in the rheumatoid arthritis population.

Abstract:Methotrexate (MTX) is a commonly used anti-metabolite for the treatment of inflammatory bowel diseases (IBD) and autoimmune diseases such as rheumatoid arthritis (RA) and psoriasis. It has been debated for its potential hepatotoxic effects following long-term use, with multiple guidelines recommending strict serologic monitoring as well as liver biopsies for signs of liver injury. This is the first meta-analysis assessing the risk of hepatotoxicity in patients undergoing prolonged methotrexate therapy compared to a non-methotrexate (non-MTX) group.

Abstract:Abstract Background Ustekinumab has shown efficacy in treating psoriasis, psoriatic arthritis, and inflammatory bowel disease and is generally safe in the short term; however, data on its long-term safety, especially in terms of malignant tumors, are lacking, which has aroused great public concern. Methods The PubMed, Web of Science, Embase, and Cochrane Library databases were searched from their establishment to July 2021 for randomized controlled trials (RCTs) reporting malignant tumors in adults with autoimmune diseases. Peto odds ratios (PORs) were calculated as effect sizes to assess the risk of malignant tumors. Results Fifteen RCTs involving 5835 patients were included in the study. The meta-analysis revealed that the risk of malignant tumors was not significantly different between patients treated with ustekinumab or placebo (POR=0.88, 95% confidence interval [CI] [0.37, 2.11], P=0.78). Similarly, no statistically significant difference existed in the incidence of non-melanoma skin cancers (POR=0.77, 95% CI [0.23, 2.61], P=0.68). No heterogeneity was observed in the comparisons. Conclusion Compared with placebo, ustekinumab did not increase the risk of malignant tumors or non-melanoma skin cancers in patients with autoimmune diseases, indicating the safety of this biological agent.
[Coronary angioplasty in cardiac transplantation].

Abstract:Coronary artery disease is a common complication of cardiac transplantation threatening long-term survival and its management is not well defined. Percutaneous transluminal coronary angioplasty has been proposed as a palliative and exceptional technique in some patients with limited lesions. Of 145 patients undergoing cardiac transplantation between April 1983 and January 1994, 7 underwent 10 coronary angioplasty procedures. The majority was performed in asymptomatic patients, 4 for angigraphic abnormalities alone and 3 for documented painless ischaemia. Primary angiographic success was obtained in 90% (9 out of 10) of lesions. Complication included one acute occlusion and one arteriovenous fistula. Three patients died, 1, 8 and 10 months after angioplasty. After an average of 24 months' follow-up, 3 (out of 7) patients had no cardiac events (myocardial infarction, second angioplasty, second transplantation or death). A multicentre prospective controlled trial is necessary to assess the impact of this procedure on graft and patient survival.

Abstract:Objective: High-dose methotrexate (HDMTX) is a mainstay therapeutic agent for the treatment of diverse hematological malignancies, and it plays a significant role in interindividual variability regarding the pharmacokinetics and toxicity. The genetic association of HDMTX has been widely investigated, but the conflicting results have complicated the clinical utility. Therefore, this systematic review aims to determine the role of gene variants within the HDMTX pathway and to fill the gap between knowledge and clinical practice. Methods: Databases including EMBASE, PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), and the Clinical were searched from inception to November 2020. We included twelve single-nucleotide polymorphisms (SNPs) within the HDMTX pathway, involving RFC1, SLCO1B1, ABCB1, FPGS, GGH, MTHFR, DHFR, TYMS, and ATIC. Meta-analysis was conducted by using Cochrane Collaboration Review Manager software 5.3. The odds ratios (ORs) or hazard ratios (HRs) with 95% confidence interval (95% CI) were analyzed to evaluate the associations between SNPs and clinical outcomes. This study was performed according to the PRISMA guideline. Results: In total, 34 studies with 4102 subjects were identified for the association analysis. Nine SNPs involving MTHFR, RFC1, ABCB1, SLCO1B1, TYMS, FPGS, and ATIC genes were investigated, while none of studies reported the polymorphisms of GGH and DHFR yet. Two SNPs were statistically associated with the increased risk of HDMTX toxicity: MTHFR 677C > T and hepatotoxicity (dominant, OR=1.52, 95% CI=1.03-2.23; recessive, OR=1.68, 95% CI=1.10-2.55; allelic, OR=1.41, 95% CI=1.01-1.97), mucositis (dominant, OR=2.11, 95% CI=1.31-3.41; allelic, OR=1.91, 95% CI=1.28-2.85), and renal toxicity (recessive, OR=3.54, 95% CI=1.81-6.90; allelic, OR=1.89, 95% CI=1.18-3.02); ABCB1 3435C > T and hepatotoxicity (dominant, OR=3.80, 95% CI=1.68-8.61), whereas a tendency toward the decreased risk of HDMTX toxicity was present in three SNPs: TYMS 2R > 3R and mucositis (dominant, OR=0.66, 95% CI=0.47-0.94); RFC1 80A > G and hepatotoxicity (recessive, OR=0.35, 95% CI=0.16-0.76); and MTHFR 1298A > C and renal toxicity (allelic, OR=0.41, 95% CI=0.18-0.97). Since the data of prognosis outcomes was substantially lacking, current studies were underpowered to investigate the genetic association. Conclusions: We conclude that genotyping of MTHFR and/or ABCB1 polymorphisms prior to treatment, MTHFR 677C > T particularly, is likely to be potentially useful with the aim of tailoring HDMTX therapy and thus reducing toxicity in patients with hematological malignancies.
Patients With IBD Receiving Methotrexate Are at Higher Risk of Liver Injury Compared With Patients With Non-IBD Diseases: A Meta-Analysis and Systematic Review

Abstract:Background: Methotrexate is well-known in treating inflammatory bowel disease (IBD), rheumatoid arthritis (RA), psoriasis (Ps), and psoriatic arthritis (PsA). Several reports have indicated a higher incidence of methotrexate (MTX)-related liver adverse events in patients with IBD. We aim to investigate the risk of liver injury in patients with IBD and those with non-IBD diseases treated with MTX. Methods: We searched PubMed, Embase, and the Cochrane Library for articles that reported liver adverse events in patients with IBD, RA, and Ps/PsA, receiving MTX therapy. Additional articles were obtained by screening the references of recent meta-analysis and reviews. Raw data from included articles were pooled to calculate the cumulative incidence of total liver injury (TLI), MTX discontinuation (MTX-D), and liver fibrosis (LF). RR (relative risk) was calculated to compare the difference between patients with IBD and those with non-IBD diseases. Results: A total of 326 articles with 128,876 patients were included. The patients with IBD had higher incidence of TLI [11.2 vs. 9.2%; relative risk (RR) = 1.22; P = 0.224] and MTX-D (2.6 vs. 1.8%; RR, 1.48; P = 0.089) than patients with non-IBD diseases. Due to the publication bias, trim-and-fill was performed. Afterwards, the patients with IBD showed significantly higher risk of TLI (11.2 vs. 3%; RR = 3.76; p < 0.001), MTX-D (3.3 vs. 0.7%; RR = 5; p < 0.001) and LF (3.1 vs. 0.1%; RR = 38.62; P = 0.001) compared with patients with non-IBD diseases. Conclusion: IBD is associated with a higher risk of MTX-related liver injury. The mechanism of MTX-induced hepatotoxicity might be different in IBD and non-IBD diseases, and needs to be verified in future research.

Abstract:Introduction: Methotrexate (MTX) is a folate antagonist used as an immunosuppressant in a number of conditions, including rheumatoid arthritis (RA). Low-dose MTX (MTX-LD) is associated with a risk of haematological, hepatic, gastrointestinal and pulmonary toxicity, which may up until now have limited its use. State of the art: In RA, data from retrospective cohorts have reported a possible excess risk of methotrexate toxicity in cases of underlying interstitial lung disease (ILD). However, recent prospective and retrospective multicentre studies have found no such increased risk, and have reassuringly concluded that MTX-LD can be prescribed in cases of RA-associated ILD (RA-ILD). Perspectives and conclusions: Current recommendations are not to delay the introduction of MTX in patients with RA at risk of developing ILD or in the presence of RA-ILD with mild to moderate respiratory impairment.
Safety and Efficacy of Methotrexate for Chinese Adults with Psoriasis with and Without Psoriatic Arthritis

Abstract:IMPORTANCE It is necessary to determine whether psoriasis responds to methotrexate in the same manner in patients with and without psoriatic arthritis. OBJECTIVE To evaluate the effectiveness and safety of methotrexate in treating patients with psoriasis with and without psoriatic arthritis. DESIGN, SETTING, AND PARTICIPANTS In this prospective, single-arm, interventional study, a total of 235 patients with psoriasis, 107 without psoriatic arthritis and 128 with psoriatic arthritis who were receiving methotrexate therapy from April 1, 2015, to December 31, 2017, were recruited from the outpatient department of a hospital at a large Chinese university. There were no significant demographic or clinical differences between the subgroups with the exception of diabetes. INTERVENTIONS A 12-week course of low-dosage oral methotrexate (7.5-15mg weekly). MAIN OUTCOMES AND MEASURES Changes in disease severity, adverse events, blood cell counts, and liver and renal function. RESULTS A total of 235 patients with psoriasis (166 male [66.0%]; mean [SD] age, 49.6 [15.1] years) received methotrexate treatment for 12 weeks. The 90% reduction from baseline Psoriasis Area Severity Index response was significantly lower in patients with psoriatic arthritis than in patients without psoriatic arthritis at week 8 (4 of 128 [3.1%] vs 12 of 107 [11.2%]; P =.02) and week 12 (19 of 128 [14.8%] vs 27 of 107 [25.2%]; P =.049). Furthermore, the incidence of adverse events, including dizziness (12 of 128 [9.4%] vs 1 of 107 [0.9%]; P =.007), gastrointestinal symptoms (32 of 128 [25.0%] vs 13 of 107 [12.1%]; P =.01), and hepatoxicity (34 of 128 [26.6%] vs 16 of 107 [15.0%]; P =.04), was significantly higher in patients with psoriatic arthritis than in patients without psoriatic arthritis. Methotrexateinduced elevation of alanine aminotransferase levels was associated with body mass index (mean [SD] body mass index, 26 [4] in patients with [P =.04] vs 26 [4] in those without [P =.005] psoriatic arthritis) and smoking (17 of 34 [50.0%] in patients with [P =.02] vs 9 of 16 [56.3%] in those without [P =.04] psoriatic arthritis). CONCLUSIONS AND RELEVANCE In this study, methotrexate was well tolerated and effective in treating psoriasis. It was more effective, with fewer adverse effects, in patients with psoriasis who did not have psoriatic arthritis than in patients who presented with both psoriasis and psoriatic arthritis. Therefore, methotrexate can be recommended as first-line treatment for psoriasis without arthritis.

Abstract:When not treated properly, Rheumatoid arthritis (RA) progresses in its chronic course, joint damage increases, and it can even lead to systemic involvement. Methotrexate (MTX), is the most preferred drug in RA treatment. However, there is no agreement among physicians on the dose of methotrexate. The aim of this study was to determine the effects and side effects of high dose methotrexate use in the initial combined therapy on the activity of disease among active RA patients. Newly diagnosed 45 RA patients were divided into two groups randomly. First group received 12,5 mg/week MTX im for four weeks and continued orally. Second group received 25 mg/week MTX im for four weeks and continued with 12,5 mg/week orally. Beside the MTX treatment, 2 gr/day SLZ and 5 mg/day prednisolone were given to both groups. At three different stages of the treatment patients were evaluated in terms of DAS28, MHAQ, VAS, morning stiffness, ESR and CRP. Patients were 34 female and 11 male. Mean disease duration was 14.7 months. In 1st group, mean DAS28 was 6.51 at the beginning, and in the end of the 3rd month it decreased to 2.75. In second group, mean DAS28 was 6.05 at the beginning, and in the end of the 3rd month it decreased to 3.56. Tender joint count in second group was 3.17 at the 3rd month, whereas it was 1.64 at the first group. In the 3rd month, at the high dose MTX group, VAS, DAS28 showed statistically significant improvement when compared to parameters of other group. CRP, morning stiffness, swollen joint count, and MHAQ scores at the 25mg/week MTX group were better at the 1st and 3rd months; however, there was no statistically significant difference between two groups in terms of those parameters. Three patients had nausea, fatigue, vomiting. At the 3rd month, at first group treatment was changed in one patient only. At second group, treatment was changed in 4 patients. In one patient, nausea and vomiting developed and ALT increased in another patient. Although scores of 1st month didn't show a significant difference between two groups, scores of 3rd month in the high dose MTX group exhibited a marked improvement in all criteria except morning stiffness. Improvements in DAS28 and number of sensitive joints were statistically significant. ACR 20,50,70 and 90 improvements of 3rd month in the high dose MTX group were higher than those of first group. In 2010, a new classification criteria for RA was developed, which makes early diagnosis of RA possible. The treatment includes the use of MTX plus combined DMARDs at the early stages of active disease. Findings of this study demonstrated that usage of high dose MTX at an early stage is more effective in controlling the disease activity and increases the quality of life in RA. Abstract:OBJECTIVES: Patients with RA have an increased prevalence of cardiovascular disease (CVD). This is due to traditional risk factors and the effects of chronic inflammation. MTX is the first-choice DMARD in RA. We performed a systematic literature review to determine whether MTX affects the risk of CVD in patients with RA.METHODS: We searched Medline, Embase, Cochrane database, database of abstracts of reviews of effects, health technology assessment and Science Citation Index from 1980 to 2008. Conference proceedings (British Society of Rheumatology, ACR and EULAR) were searched from 2005 to 2008. Papers were included if they assessed the relationship between MTX use and CVD in patients with RA. Two reviewers independently assessed each title and abstract for relevance and quality.RESULTS: A total of 2420 abstracts were identified, of which 18 fulfilled the inclusion criteria. Two studies assessed the relationship between MTX use and CVD mortality, one demonstrated a significant reduction in CVD mortality and the second a trend towards reduction. Five studies considered all-cause CVD morbidity. Four demonstrated a significant reduction in CVD morbidity and the fifth a trend towards reduction. MTX use in the year prior to the development of RA decreased the risk of CVD for 3-4 years. Four studies considered myocardial infarction, one demonstrated a decreased risk and three a trend towards decreased risk with MTX use.CONCLUSION: The current evidence suggests that MTX use is associated with a reduced risk of CVD events in patients with RA. This suggests that reducing the inflammation in RA using MTX not only improves disease-specific outcomes but may also reduce collateral damage such as atherosclerosis.

A dose-dependent beneficial effect of methotrexate on the risk of interstitial lung disease in rheumatoid arthritis patients

Abstract:Objectives The aim of the study was to assess the influence of different factors, including treatment, on the risk of ILD in the course of RA. Methods A total of 109 RA patients were included in the analysis. High-resolution computed tomography (HRCT) of chest was obtained in each patient. Patients were classified as having ILD (ILD group) or not (N-ILD group). The ILD was graded using the semi-quantitative Warrick scale of fibrosis. Warrick extent score (WES) and Warrick severity score (WSS) were calculated separately for each patient, then combined to obtain a global score (WGS). Results In univariate analysis the presence of ILD was associated positively with age ( P = 5x10 -6 ) and negatively with MTX treatment ( P = 0.0013), mean MTX dose per year of treatment ( P = 0.003) and number of DMARDs used ( P = 0.046). On multivariate analysis only age and treatment with MTX were independently associated with the presence of ILD. WGS was significantly lower in patients treated with MTX in a dose of ≥15 mg/week (MTX≥15 group) as compared to patients treated with lower doses of MTX (0<MTX<15 group) or not treated with MTX (N-MTX group) ( P = 0.04 and P = 0.037, respectively). The ILD prevalence was higher in N-MTX group than in 0<MTX<15 group ( P = 0.0036) and MTX≥15 group (0.0007). The difference in ILD prevalence between MTX≥15 and 0<MTX<15 groups was not significant, but the latter group had higher WES ( P = 0.044) and trended to have higher WSS and WGS. Consclusions We found a beneficial effect of MTX on RA-ILD. Importantly, this effect seems to be dose dependent.

Abstract:Methotrexate (MTX) is an essential anti-rheumatic drug used to treat rheumatoid arthritis (RA). Prevention or management of adverse reactions, including interstitial lung disease (ILD), hepatotoxicity, myelosuppression, and infection, remains fundamental for safe MTX therapy. Using the Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) (JAPIC AERS), we performed disproportionality analyses of adverse events related to MTX use and the impact of concomitant medications. Upon analyzing all reported cases in FAERS between 1997 and 2019, the crude reporting odds ratios (cRORs; 95% confidence intervals) for ILD, hepatotoxicity, myelosuppression, and tuberculosis (TB) in relation to MTX use were 4.00 (3.83–4.17), 1.99 (1.96–2.02), 3.66 (3.58–3.74), and 7.97 (7.65–8.3), respectively. Combining MTX with folic acid (FA) or tumor necrosis factor-alpha inhibitors (TNFis) tended to reduce cRORs for these adverse events (except for TB). Multiple logistic regression analysis in patients with RA was conducted to calculate adjusted reporting odds ratios (aRORs) for age, sex, and MTX treatment patterns (MTX alone and combined with FA and TNFi). Higher age (except for hepatotoxicity) and male sex were significantly associated with adverse events. Combining FA or TNFi with MTX reduced aRORs for MTX-related hepatotoxicity and myelosuppression; in contrast, the effect of FA was not obvious in ILD or TB. Although studies assessing spontaneous reporting systems have limitations such as reporting bias, data from our logistic regression analysis demonstrated that adding FA to MTX-based therapy could help reduce the dose-dependent adverse events of MTX, thereby providing clinical evidence that supports the beneficial effect of FA. This study also demonstrated the usefulness of FAERS in comparing adverse events based on treatment patterns.

Abstract:Former findings suggested that MTX-based combination therapies show improved efficacy and safety compared to monotherapy[1]. Several studies[2]have been conducted analysis on MTX-based combination therapies, but there is a lack of systematic analysis involving MTX+TCM (Traditional Chinese medicine) therapies.To assess the effectiveness and adverse events of Methotrexate-based (MTX-based) combination therapies(especially MTX + TCM therapies) in rheumatoid arthritis (RA) treatment.A comprehensive search of PubMed, Embase, the Cochrane Library, China National Knowledge Infrastructure (CNKI), the Database for Chinese Technical Periodicals and Wanfang data was done without language restriction. A systematic review and network meta-analysis included randomized controlled trials of MTX-based combination therapies and methotrexate monotherapy. The ability of different regimens to reduce rheumatoid factors and inflammatory indicators and the probability of adverse reactions were compared. This study has been registered with PROSPERO under the number CRD42022265799.A total of 37 studies, including 3119 patients, were eligible for inclusion. Meta-analysis results showed that methotrexate combined with other DMARDs or Traditional Chinese Medicine (TCM) could reduce serum Rheumatoid Factor (RF) and C-reactive protein (CRP) levels to some extent, but the therapies combined with TCM were more effective and had a lower rate of adverse effects.The results of RF reducing ability were ranked in ascending order: MTX+LEF+TCM > MTX+SSZ+HCQ > MTX+LEF >MTX+TCM >MTX+HCQ >MTX.While the results of CRP reducing ability were ranked in ascending order: MTX+LEF+TCM>MTX+SSZ+HCQ>MTX+LEF>MTX+TCM>MTX+HCQ.Overall, MTX-based combination therapies are an effective treatment, and the combination of TCM and MTX-based combination therapies is more effective and safer. For treating rheumatoid arthritis or patients with an inadequate response to MTX, MTX-based combination therapies remain an alternative, cost-effective option.
