Abstract:Here we present a robust fully automated method for relative radiometric co-registration. First, a new low dimensional feature-point descriptor, called the Expanded Haar-Like Filter (EHLF) descriptor, is introduced. The EHLF has many desirable properties like flexible design, fast computation, and multi-scale description, while also being insensitive to variations in image quality. Next, two spatial matching schemes are proposed for increasing the percentage of correctly matched feature points. The first is based on a global affine model and the second utilizes dynamic local template fuzzy distance matching. Finally, precise pixel-to-pixel invariant feature points are extracted from a diversity of image locations centered at matched local extrema points. Experimental results show that for high-resolution multi-temporal imagery, the EHLF descriptor can obtain matched feature points with accuracies equivalent to that using a higher dimensional descriptor. In addition, the EHLF descriptor produces a larger number of correctly matched feature points. The spatial matching methods significantly improve feature-point matching, especially for image pairs with large geometric distortions. Radiometric co-registration quality based on the pixel-based invariant features was tested using four different evaluation datasets, and the results demonstrate that the proposed approach produces the lowest normalized root-mean-square error compared with six other automated methods. The proposed method depends on successful extraction of feature points, which may not be available for the scenes that are fully undeveloped (e.g., forest areas). Nonetheless, Monte Carlo simulations show that 30 to 50 correctly matched feature points will provide relatively stable radiometric calibration coefficients.

Abstract:Introduction: Pediatric polytrauma is a complex condition with unique characteristics and requirements for early clinical care. This study aimed to analyze the injury patterns, early clinical care, and outcomes of pediatric polytrauma patients in a Level I trauma center. The focus was on evaluation between different age groups and the recognition of injuries as potential factors influencing outcomes. Methods: A prospective cohort study model of pediatric polytrauma patients (ISS ≥ 16) was conducted over a 13-year period, stratified by age groups (Group A: 0–5 years; Group B: 6–10 years; Group C: 11–15 years; and Group D: 16–18 years). A comparison of the groups was conducted to examine variations in early clinical care, trauma mechanisms, distribution of affected body regions (as per AIS and ISS criteria), and trauma-related mortality. Additionally, factors contributing to mortality were evaluated. Results: The median age of patients was 16 years, with a male predominance (64.7%). The Injury Severity Score (ISS) varied across age groups, with no significant difference. The 30-day mortality rate was 19.0%, with no significant age-related differences. Trauma mechanisms varied across age groups, with motor vehicle accidents being the most common mechanism in all age groups except 0–5 years, where falls were prevalent. Analysis of injury patterns by AIS body regions indicated that head trauma was a significant predictor of mortality (Hazard Ratio 2.894, p < 0.001), while chest, abdominal, and extremity trauma showed no significant association with mortality. Multiple regression analysis identified the ISS and preclinical GCS as valid predictors of mortality (p < 0.001 and p = 0.006, respectively). Conclusions: While age-related differences in injury severity and clinical interventions were limited, head trauma emerged as a critical predictor of mortality. Early recognition and management of head injuries are crucial in improving outcomes. Additionally, the ISS and preclinical GCS were identified as valid predictors of mortality, emphasizing the importance of early assessment and resuscitation. A tailored approach to pediatric polytrauma care, considering both age and injury patterns, might contribute to survival benefits in this vulnerable population.

Abstract:Objectif Présenter une approche pour évaluer et prendre en charge initialement les patients victimes de brûlures dans un département d'urgence rural. Trois moyens mnémoniques sont proposés qui peuvent servir à la fois pour l'évaluation et pour la prise en charge initiales des patients brûlés en milieu rural. Qualité des données Les lignes directrices actuelles et régionales compilées par un chirurgien plastique ont été passées en revue dans le but d'élaborer une approche systématique pour le traitement des patients souffrant de brûlures. Une recension a aussi été effectuée dans PubMed et d'autres bases de données pour trouver des ouvrages scientifiques récents sur les soins d'urgence aux victimes de brûlures. Message principal Les blessures par brûlure sont souvent le motif d'une visite au département d'urgence. Par ailleurs, les soins aux patients souffrant de telles blessures peuvent varier considérablement selon la région géographique, la formation des cliniciens et les ressources de l'hôpital. Nous traitons de la classification des brûlures, des lignes directrices sur la réanimation liquidienne, des pansements, des soins des plaies, des critères justifiant un transfert, de même que de la prise en charge de la douleur. Conclusion Le recours à une approche systématique peut contribuer à améliorer les issues des blessures par brûlure pour les patients et procurer aux cliniciens un référentiel par étapes pour la prise en charge des patients victimes de brûlures en milieu rural.

Abstract:Background:Although the incidence of pediatric patients in emergency services is as low as 5–10%, trauma remains one of the leading causes of death during childhood. Only a few reports exist about the quality of the initial treatment of pediatric trauma patients. Therefore, we tested the hypothesis of whether prehospital treatment and emergency management in pediatric trauma patients is similar to the treatment that is provided for adult patients.Materials and Methods:We performed a retrospective data analysis of the German Trauma Registry of the DGU from January 1993 to December 2007. Exclusion criteria were missing information about injury severity and/or age and patients older than 50 years. All pediatric patients were subdivided into five groups (infants 0–1 year, toddlers 2–5 years, children 6–9 years, pupils 10–13 years, teenagers 14–17 years) with regard to their age and were compared with the adult cohort (18–50 years). From 24,396 patients, 2,961 were below 18 years of age, thus, about 12% of the whole population of injured patients below the age of 50 years.Results:66.4% of infants sustained relevant head injuries (Abbreviated Injury Scale [AIS] ≥3), and this rate declined with increasing age. The mean Injury Severity Score (ISS) increased from 21.0 (±11.6) in the group of infants to 26.7 (±13.9) in the adult cohort. In all groups, the majority of patients were male. The injury pattern differed according to age, with predominant traumatic brain injury (TBI) in infants. During the preclinical treatment, infants were less often intubated and this was contrasted by a higher rate of cardiopulmonary resuscitation in this group (infants 16.2%, toddlers 6.8%, adults 3.1%). Diagnostic multislice computed tomography (CT) examination was less often performed in infants as compared to the other groups (infants 57.1%, toddlers 77.2%, adults 77.8%). Mortality and quality indicators such as timelines show no significant differences between children and adults.Conclusion:We observed typical age-dependent differences regarding the injury pattern and severity and differences referring to the preclinical and initial treatment. With respect to the high rate of serious TBI in the infants and toddlers age groups, a more focused education and training of emergency physicians and paramedics should be considered.

Fusing language information from diverse data sources for phonotactic language recognition

Abstract:The baseline approach in building phonotactic language recognition systems is to characterize each language by a single phonotactic model generated from all the available languagespecific training data. When several data sources are available for a given target language, system performance can be improved using language source-dependent phonotactic models. In this case, the common practice is to fuse language source information (i.e., the phonotactic scores for each language/source) early (at the input) to the backend. This paper proposes to postpone the fusion to the end (at the output) of the backend. In this case, the language recognition score can be estimated from well-calibrated language source scores. Experiments were conducted using the NIST LRE 2007 and the NIST LRE 2009 evaluation data sets with the 30s condition. On the NIST LRE 2007 eval data, a Cavg of 0.9% is obtained for the closed-set task and 2.5% for the open-set task. Compared to the common practice of early fusion, these results represent relative improvements of 18% and 11%, for the closed-set and open-set tasks, respectively. Initial tests on the NIST LRE 2009 eval data gave no improvement on the closedset task. Moreover, the Cllr measure indicates that language recognition scores estimated by the proposed approach are better calibrated than the common practice (early fusion).
Child head injury criteria investigation through numerical simulation of real world trauma

Abstract:Finite element modelling has been used for decades in the study of adult head injury biomechanics and determination of injury criteria. Interest is recently growing in investigation on pediatric head injury which requires elaboration of biofidelic models that take into account child's head particularities in terms of size, geometry, and mechanical properties. In this study, a finite element model of a 3-year-old child head is proposed. The model is reconstructed from real CT scan images and mechanical properties are extracted from available data in the literature. A large number of real accidents (25 falls) are reconstructed with proposed model using different brain constitutive relationships in order to investigate their influence on model response. Mechanical output parameters (HIC, pressure, shearing stress) are calculated from these simulations. Statistical analysis was performed in order to evaluate predictive capability of the parameters. Von Mises stress appears to be clearly the most predictive parameters, allowing clear distinction between injured and non-injured cases. To the authors' knowledge, this study proposes for the first time a statistically based neurological injury criterion for a pediatric population using finite element modelling.

Abstract:Management of severely injured patients remains a challenge, characterised by a number of advances in clinical practice over the last decades. This evolution refers to all different phases of patient treatment from prehospital to the long-term rehabilitation of the survivors. The spectrum of injuries and their severity is quite extensive, which dictates a clear understanding of the existing nomenclature. What is defined nowadays as polytrauma or major trauma, together with other essential terms used in the orthopaedic trauma literature, is described in this instructional review. Furthermore, an analysis of contemporary management strategies (early total care (ETG), damage control orthopaedics (DCO), early appropriate care (EAC), safe definitive surgery (SDS), prompt individualised safe management (PRISM) and musculoskeletal temporary surgery (MuST)) advocated over the last two decades is presented. A focused description of new methods and techniques that have been introduced in clinical practice recently in all different phases of trauma management will also be presented. As the understanding of trauma pathophysiology and subsequently the clinical practice continuously evolves, as the means of scientific interaction and exchange of knowledge improves dramatically, observing different standards between different healthcare systems and geographic regions remains problematic. Positive impact on the survivorship rates and decrease in disability can only be achieved with teamwork training on technical and non-technical skills, as well as with efficient use of the available resources.

Abstract:Surgical management of complex adult spinal deformities is of high risk, with a substantial risk of operative mortality. Current evidence shows that potential risk and morbidity resulting from surgery for complex spinal deformity may be minimized through risk-factor optimization. The multidisciplinary team care model includes neurosurgeons, orthopaedic surgeons, physiatrists, anesthesiologists, hospitalists, psychologists, physical therapists, specialized physician assistants, and nurses. The multidisciplinary care model mimics previously described integrated care pathways designed to offer a structured means of providing a comprehensive preoperative medical evaluation and evidence-based multimodal perioperative care. The role of each team member is illustrated in the case of a 66-year-old male patient with previous incomplete spinal cord injury, now presenting with Charcot spinal arthropathy and progressive vertebral-body destruction resulting in lumbar kyphosis.
Finite element modelling of paediatric head impact: global validation against experimental data

Abstract:Biomechanics of the human head has been widely studied for several decades. At a mechanical level, the use of engineering and finite element (FE) methods has allowed injury mechanisms to be investigated using biofidelic FE models. These models are generally validated using experimental data then used to simulate real-world head trauma in order to derive numerical tolerance limits, leading to efficient injury predicting tools. Due to ethical issues, experimental tests on the paediatric population remain prohibitive so direct validations of numerical models cannot be performed. However injury biomechanics on paediatric population is emerging with experimental tests on the paediatric cadavers or tests on biological tissue and the development of finite element models. The present paper proposes a new finite element model of a newborn head, simulating its main features, with material properties from the literature. Global validation of the model against experimental data in terms of skull deflection is performed and the model is used to simulate paediatric skull fracture coming from real-world head trauma.

Abstract:Multiply injured patients with fractures are co-managed by acute care surgeons and orthopaedic surgeons. In most centers, orthopaedic surgeons definitively manage fractures, but preliminary management, including washouts, splinting, reductions, and external fixations, may be performed by selected acute care surgeons. The acute care surgeon should have a working knowledge of orthopaedic terminology to communicate with colleagues effectively. They should have an understanding of the composition of bone, periosteum, and cartilage, and their reaction when there is an injury. Fractures are usually fixed urgently, but some multiply injured patients are better served with a damage control strategy. Extremity compartment syndrome should be suspected in all critically injured patients with or without fractures and a low threshold for compartment pressure measurements or empiric fasciotomy maintained. Acute care surgeons performing rib fracture fixation and other chest wall injury reconstructions should follow the principles of open fracture reduction and stabilization.

Abstract:This case report presents the management of a severe crush injury in a 7-year-old male patient. The injury occurred following entrapment in a concrete mixer, resulting in extensive soft tissue trauma and vascular compromise. Prompt surgical intervention, including meticulous debridement and vascular reconstruction, was undertaken to optimize outcomes. Postoperative care involved serial debridement, negative pressure wound therapy, and subsequent skin grafting. Reconstructive procedures aimed to enhance hand function, yielding satisfactory outcomes at the three-month follow-up. This case underscores the importance of a multidisciplinary approach in managing pediatric crush injuries and highlights the role of collaborative care in optimizing outcomes.

Abstract:Objective To outline an approach to the assessment and initial management of patients with burns in the rural emergency department setting. Three mnemonics are presented that can be used for both the assessment and the initial management of patients with burns in rural settings. Quality of evidence Current and local guidelines compiled by a plastic surgeon were reviewed to develop a systematic approach to the treatment of patients with burns. PubMed and other databases were also searched for current literature on emergency care of patients with burns. Main message Burn injuries are a common reason for presentation to the emergency department. However, the care of patients with these injuries can vary substantially depending on geographic location, provider training, and hospital resources. Classification of burns, fluid resuscitation guidelines, dressings and wound care, indications for referral, and pain management are discussed. Conclusion Using a systematic approach may help improve burn injury outcomes for patients and provide practitioners with a step-by-step framework for the management of patients with burns in rural settings.

Abstract:Background: Pediatric extremity vascular injuries constitute a rare yet serious entity that can lead to serious complications especially if left untreated or become late diagnosed. In our scoping review, we sought to evaluate different characteristics and outcomes of pediatric and combined adult trauma centers (ATCs) in the management of pediatric extremity vascular injury. Methods: We sought to analyze various characteristics and parameters that differentiate a dedicated pediatric and a combined pediatric ATC in terms of effectiveness and quality of care in the acute setting and to describe special features and characteristics of an acute vascular disease that constitute pediatric population unique from the aspect of diagnosis and management. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping reviews guidelines to conduct the study. Results: The search identified 8,815 records in title using MeSH terms from PubMed/MEDLINE database among which 12 studies reporting a total of 2,124 pediatric patients with vascular extremity injuries were included for analysis. Incidence of pediatric extremity vascular injury was 0.5%. Upper extremity injuries were the most frequent presenting in 63% of cases followed by lower extremity injuries in 37% of cases. Blunt injuries were marginally more common than penetrating injuries (58% vs. 42%). In-hospital mortality and morbidity ranged from 13.2% to 0.9% and 13% to 30%, respectively. Limb-salvage rates were high, ranging from 92% to 99%. Furthermore, there are no clearly defined clinical guidelines involving the mode of imaging and diagnosis, the surgical specialties involved and the competency of nursing or medical staff overall. Conclusions: Dedicated children trauma centers theoretically represent the optimal path for acute pediatric trauma admission, especially in complex trauma necessitating vascular reconstruction. However, in the current setting of rapidly increasing health costs and economic crisis worldwide, regional or resource-related factors make this option rather unavailable. In any case, it is imperative the clinicians have a high index of suspicion when confronting with these types of injuries because early diagnosis is highly related with reduced morbidity and superior outcomes.

Abstract:PURPOSE: The purpose of this paper is to discuss risk and prognostic factors for concussion outcomes, review comprehensive approaches to assessment, and describe a new method for conceptualizing treatment for sport-related concussion using clinical experience.METHODS: Based on the current literature of sport-related concussion and clinical experience, an approach for conceptualizing concussion care using clinical trajectories and targeted treatments was developed.RESULTS: A comprehensive approach to assessment and targeted treatments for sport-related concussion was developed using specific clinical trajectories.CONCLUSION: Sport-related concussions are heterogeneous and require an individualized clinical approach. The use of a comprehensive approach for assessing specific clinical trajectories following a sport-related concussion will help clinicians better conceptualize this injury. Clinicians can then match targeted treatment pathways to specific clinical trajectories to accelerate safe return to play for athletes following a sport-related concussion.
