Abstract:To improve the clinicopathological understanding of blastic plasmacytoid dendritic cell neoplasm (BPDCN).A total of 7 BPDCN patients were recruited from 2009 to 2013. And their clinicopathological and immunohistological features, treatment and prognosis were retrospectively analyzed.There were 6 males and 1 female. The median age range was 60 (6-72) years at the time of diagnosis. The skin lesion at the time of diagnosis presented as generalized (n = 6) or solitary (n = 1) plaques or nodules. Histologic examination showed a diffuse or nodular infiltrate composed of intermediate-sized cells in dermis sparing epidermis area. The tumor cells contained round to ovoid nucleus, finely dispersed chromatin and 1 or 2 small-sized nucleoli. Most neoplastic cells were positive for CD4, CD56 and CD123 while negative for CD3, CD20, CD30, CD34, CD79a, MPO and EBER. Two patients received chemotherapy.One had no response while another relapsed rapidly. Two patients had a spontaneous regression of skin lesions and one died 2 years later. And no systematic infiltration was detected at diagnosis.BPDCN is a rare hematopoietic malignancy with specific immunophenotypes.It primarily affects elderly males and frequently presents initially as cutaneous involvement.It may be easily misdiagnosed and has a rapid course and a poor prognosis.

A case of metastatic cutaneous plasmacytoma

Abstract:A case of metastatic cutaneous plasmacytoma is reported.A 50-year-old male presented with multiple red nodules on the left chest for 1 month.He was diagnosed as multiple myeloma in department of hematology half a year ago.Physical examination showed multiple round nodules with clear bouudaries and smooth surface on the left chest.Pathological examination showed nodular infiltration of plasma cell-like cells.Immunohistochemical staining showed positive for CD38,CD138 and λchain.The diagnosis of metastatic cutaneous plasmacytoma was made.The skin lesions resolved after CMDT chemotherapy.

Abstract:Aims Persistent pruritic eruptions (PPEs), presenting with dyskeratotic keratinocytes histologically, are characteristic skin rash in patients with adult-onset Still's disease (AOSD). The lesions may be histologically similar to other entities that present with dyskeratosis. In the present study, we compared the histopathological features between PPEs and other entities presenting with dyskeratosis. Methods and results To investigate whether histopathological findings can be used to discriminate among PPEs and other entities presenting with dyskeratotic keratinocytes, cutaneous histopathological changes of PPEs associated with AOSD (n = 26) were compared with those of systemic lupus erythematosus (SLE) (n = 16), dermatomyositis (n = 19), and drug eruption (n = 16). Dyskeratosis was observed in the upper one-third of the epidermal layer in all 26 PPEs. The rate of dyskeratosis for PPEs was higher than that for SLE (18.8%) and dermatomyositis (15.8%). In drug eruptions, the dyskeratotic cells were distributed in all levels of the epidermis. Variable densities of neutrophils were found in the dermis in all PPEs. Conclusions Although this was a retrospective study conducted at a single centre, presentation of dyskeratotic keratinocytes in the upper one-third of the epidermal layer is a distinctive histopathological reactive pattern of PPEs. This pattern may be a useful histopathological marker for early diagnosis of AOSD.
Unveiling the Tempest: Dermal Plasmacytoid Dendritic Cell Proliferation as the Harbinger of Acute Myeloid Leukemia

Abstract:Plasmacytoid dendritic cell neoplasms are rare neoplasms originating from plasmacytoid dendritic cells (pDCs). They are subclassified into 2 types: blastic plasmacytoid dendritic cell neoplasm and mature plasmacytoid dendritic cell proliferation. Neoplastic expansion of pDCs has also been found to be associated with myeloid neoplasia. We present the diagnostically challenging case of a 62-year-old woman who presented to the emergency department with numerous hemorrhagic nodules and papules on the face and extensor surfaces near the elbows and neutropenic fevers. The patient had a history notable for lupus erythematosus and a recently performed excisional lymph node biopsy involved by a "plasmacytoid dendritic cell proliferation." A punch biopsy was performed, which showed a robust dermal infiltrate of atypical intermediate-sized mononuclear cells. The infiltrate was positive for CD4, CD43, and CD123. CD3 and CD8 highlighted background T cells. The infiltrate was negative for CD10, CD34, CD56, CD68, CD117, myeloperoxidase, lysozyme, TdT, and TCL-1. The findings favored a diagnosis of cutaneous involvement of the plasmacytoid dendritic cell proliferation. Given the association with acute leukemias, a subsequent bone marrow biopsy was recommended. The bone marrow biopsy was performed, which showed increased blasts (68% on a 500 differential cell count). Furthermore, immunohistochemical stains were performed, which highlighted the blasts to be positive for CD34 and BEST (alpha-naphthyl butyrate esterase) cytochemical stain. This diagnosis was consistent with bone marrow involvement of acute myelomonocytic leukemia. Given the overlapping presenting symptoms (skin lesions, adenopathy, marrow involvement) of pDC neoplasms and myeloid neoplasia and the possibility of presenting concurrently, increased awareness is of pivotal importance to help prevent potential misdiagnosis, missed diagnosis, and prompt investigation of possible associated neoplasms.
[A Case Report of Cutaneous Plasmacytosis].

Abstract:A collection of plasma cells in the skin can represent a broad spectrum of disease entities. Secondary syphilis, primary cutaneous plasmacytoma, primary cutaneous plasmacytosis, cutaneous lymphoid hyperplasia and nodular amyloidosis are considered possible differential diagnoses. The primary cutaneous plasma cell disorders can range from malignant to benign plasma cell neoplasms. The malignant conditions are neoplastic diseases having monoclonal proliferations, rapid progression and fatal outcome while the benign plasma cell disorders usually show polyclonality, chronicity and benign process, including plasmacytosis. We present a case of cutaneous plasmacytosis. The patient was a 34-year-old man, presented with disseminated reddish-brown plaques and nodules on the right side of the hips, inguinal groove, and the thigh. Histopathologically, mature plasma cells perivascular infiltrates were observed mainly in the dermis. Polyclonality of infiltrating plasma cells with coexistence of both kappa and gamma chain-positive cells demonstrated with immunohistochemistry, as well as CD20+++, CD38++++, CD79a++++, CD138++, Ki67<30%. The diagnosis, cutaneous plasmacytosis, was established by the pertinent laboratory findings. Primary cutaneous plasmacytosis was an uncommon reactive lymphoplasmacytic disorder of uncertain etiology. Cutaneous plasmacytosis is a rare disease characterized by peculiar multiple eruptions and hyper gamma globulinemia. It has been mainly described in patients of Japanese descent, with only few reports in Caucasians and Chinese, although information concerning the disorder was limited to individual case reports. Cutaneous plasmacytosis is a rare disorder, which is characterized by multiple red to dark-brown nodules and plaques on the trunk and usually associated with polyclonal hyper gamma globulinaemia. Primary cutaneous plasmacytosis or cutaneous plasmacytosis was thought to be a reactive process with unknown etiology. Histologically, lesions contain dense perivascular infiltration of mature polyclonal plasma cells without any atypia, in the dermis and subcutaneous fat. The clinical course is chronic and benign without spontaneous remission. Available treatments for cutaneous plasmacytosis include psoralen ultraviolet A radiotherapy, systemic chemotherapy and intralesional steroid injection. The patient with cutaneous plasmacytosis in this report was treated with tacrolimus ointment and psoralen ultraviolet A.

Abstract:Chronic myelomonocytic leukemia (CMML) is a hematopoietic stem cell neoplasm exhibiting both myelodysplastic and myeloproliferative features. Cutaneous involvement by CMML is critical to recognize as it typically is a harbinger of disease progression and an increased incidence of transformation to acute myeloid leukemia. Cutaneous lesions of CMML exhibit heterogeneous histopathologic features that can be challenging to recognize as CMML. We describe a 67-year-old man with a 3-year history of CMML who had been managed on single-agent azacitidine with stable disease before developing splenomegaly and acute onset skin lesions. Examination of these skin lesions revealed a dense infiltrate of histiocytic cells morphologically resembling Langerhans type cells (lacking frank histopathologic atypia), and with the immunophenotype of an indeterminate cell histiocytosis (S100+ CD1a+ and langerin-). Given the history of CMML, next-generation sequencing studies were performed on the skin biopsy. These revealed a KRAS (p.G12R) mutation identical to that seen in the CMML 3years prior, establishing a clonal relationship between the 2 processes. This case expands the spectrum for and underscores the protean nature of cutaneous involvement by CMML and underscores the importance of heightened vigilance when evaluating skin lesions of CMML patients.
Mature plasmacytoid dendritic cell proliferation in association with mixed-phenotype acute leukemia, T/myeloid

Abstract:A 21-year-old woman presented with facial rash, leukocytosis (25.4 K/μL), anemia (7.8 g/dL), thrombocytopenia (40 K/μL), and circulating blasts. Bone marrow was hypercellular with ∼20% myeloid blasts expressing CD34, CD117, CD33, lysozyme, and myeloperoxidase (MPO, subset) and lacking CD3, along with proliferation of plasmacytoid dendritic cells (pDCs) without CD56 and TCL1. Terminal deoxynucleotidyltransferase (TdT) was negative by flow cytometry. Cytogenetic and molecular studies revealed monosomy 7 without other common translocations and somatic variants. An excisional biopsy of the inguinal lymph node showed effacement of nodal architecture by blasts (panel A-B, hematoxylin and eosin stains, 20-40× objective) expressing myeloid and T-cell antigens at varying proportions, including CD3 (weaker than T cells), CD5 (weaker than T cells), CD34, CD117, MPO, and TdT (panel D-I, immunohistochemical stains, with hemotoxylin counterstains, 20× objective; panels G-H [insets], 60× objective). The pDC nodules with frequent apoptosis (panel A,C, 20-40× objective) had a similar phenotype to the marrow and showed expression of CD123 and CD68R (panel J-K, immunohistochemical stains, with hemotoxylin counterstains, 20× objective). Ki-67 proliferation index was 70% within the leukemic blasts and <10% within the pDC nodules (panel L, immunohistochemical stains, with hemotoxylin counterstains, 20× objective). The facial skin biopsy revealed infiltration by mature pDCs without leukemic blasts.

Abstract:Background Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, aggressive malignancy which is diagnostically challenging. Objectives To characterize clinical, histopathological and immunophenotypic features of BPDCN with cutaneous involvement. Materials & Methods Cases of BPDCN with skin lesions diagnosed between 2010 and 2020 were retrospectively studied. Results The age at diagnosis presented a bimodal pattern. The morphological features of skin lesions of BPDCN were heterogeneous, manifesting as disseminated, sporadic or solitary plaques, nodules or tumours. Histopathologically, tumour cells in 67.9% of lesions infiltrated in a diffuse and nodular pattern. Interstitial, lichenoid and perivascular patterns were observed in 14.3%, 7.1% and 10.7% lesions, respectively. Angioinvasion and necrosis were absent. All tumour cells were lymphoblastic-like with polymorphic and irregular nuclei except for one lesion revealing monomorphous immunoblastic-like cells. Immunohistochemically, CD4, CD56, CD123 and TCL-1 were positive in 82.1%, 92.9%, 96.0% and 100% cases of BPDCN, respectively. Simultaneous expression of the four markers were observed in 65.0% of stained lesions. Notably, E2-2 was positively expressed in the nuclei of tumour cells in all BPDCN lesions, but was negative in all myeloid sarcoma lesions. Moreover, peripheral blood flow cytometry of six BPDCN cases revealed varying degrees of tumour cells with heterogeneous expression of CD56. Conclusion The morphological and phenotypic features of cutaneous BPDCN are heterogeneous. E2-2 may serve as a useful marker to definitively diagnose BPDCN
P022 A case of cutaneous plasmacytosis occurring in a patient with pulmonary adenocarcinoma

Abstract:Abstract A 43-year-old woman of Japanese origin presented with a 4-year history of a slow-spreading rash over her face, torso and legs. This consisted of symmetrical hyperpigmented nodular lesions, which were asymptomatic and persistent with no identifiable triggers. Apart from iron-deficiency anaemia, she was otherwise fit and well, with an unremarkable family history. Examination revealed a widespread hyperpigmented erythematous rash distributed symmetrically across the back, with multiple smaller lesions over the face, axilla, torso and legs. Routine biochemistry was unremarkable apart from low ferritin level. Electrophoresis and IgG subclass analysis showed a mildly raised IgM, but was otherwise unremarkable. Investigations for HIV, hepatitis, syphilis and chlamydia were unremarkable. Histopathology showed a mild to focally moderate perivascular and minimally interstitial mononuclear cell infiltrate in the superficial and deep dermis. The infiltrate consisted of lymphocytes, histiocytes and particularly large numbers of CD20+ plasma cells. There was no cytological atypia, with most of the plasma cells expressing IgG. Kappa and lambda in situ hybridization showed a polytypic population of plasma cells. Subsequent imaging studies detected a lung nodule, which was later surgically excised and confirmed to be a pulmonary adenocarcinoma. There was no evidence of significant lymphadenopathy to suggest Castleman disease or lymphoma. Cutaneous plasmacytosis (CP) is a rare skin disorder characterized by multiple reddish-brown nodules with polyclonal plasma-cell proliferation, usually accompanied by a polyclonal hypergammaglobulinaemia. CP is seen predominantly in Asian populations and commonly affects the trunk, particularly the axillae, but can also involve the face and extremities. The pathogenesis of CP remains poorly elucidated; however, existing evidence indicates that an aberrant production of interleukin-6 might be a cause. Histopathology typically shows perivascular and periadnexal mature, polytypic, plasma-cell-rich infiltrate in the superficial and deep dermis. Treatment modalities such as topical and systemic corticosteroids, topical tacrolimus, systemic chemotherapy, cyclophosphamide, psoralen plus ultraviolet A (PUVA) phototherapy and radiotherapy have been used, with varied response. In our patient, topical and systemic corticosteroids did not result in significant improvement, and further PUVA phototherapy has been planned.

Abstract:Primary cutaneous posttransplant lymphoproliferative disorders (PTLDs) after allogeneic hematopoietic stem cell transplant (allo-HSCT) are exceedingly rare, with only 6 published cases, all of them consisting in T-cell neoplasms. In this report, we present for the first time a donor-derived B-cell PTLD consisting in a primary, cutaneous, B-cell, marginal zone, lymphoproliferative disorder (PCMZLPD). The patient, a 37-year-old woman with a history of Hodgkin lymphoma received an allo-HSCT from her healthy, matched, related father, achieving complete host chimerism in the bone marrow and peripheral blood. However, 8 years after the allo-HSCT, she presented asymptomatic skin lesions consisting in oval, well-defined, slightly raised erythematous plaques, located on the arms, trunk, and legs. Skin biopsies of 2 lesions demonstrated a class-switched IgG+, EBV-, PCMZLPD, showing kappa light chain restriction and monoclonal rearrangement of the IgH gene. Microsatellite genotyping and 2-color fluorescence in situ hybridization (X and Y chromosomes) confirmed that the origin of the neoplastic cells was the donor graft. The lesions showed an indolent behavior, good response to topical corticosteroids, and no need for systemic treatment. Our case broadens the spectrum of PTLD, a diverse group of lymphoid and/or plasmacytic proliferations with variable clinical presentations and histopathological features.

Abstract:A woman in her 80s reported of generalised pruritus, which was treated with phototherapy and steroid administration. Two months after onset, lymph node biopsy revealed CD4+ angioimmunoblastic T-cell lymphoma with systemic superficial nodal involvement. Intractable prurigo was judged as T-cell lymphoma related. After effective chemotherapy (7 months later), skin nodules appeared multifocally, including on the lip, thumb and lower leg. The biopsied infiltrative lesion on the right lower leg microscopically revealed subcutaneous growth of atypical plasmablasts with nearly 100% Ki-67 labelling and Epstein-Barr virus (EBV)-encoded small nuclear RNA positivity. Plasmablastic lymphoma (CD45/CD19/CD38/CD138/MUM1+, CD20/CD79a/PAX5-) was suspected. Immunoglobulin light-chain restriction and nuclear expression of c-myc protein were undetectable, and the ulcers were spontaneously epithelialised by the cessation of steroid administration. After 10 months, non-progressive prurigos persisted on the extremities, but without regrowth of nodal T-cell lymphoma and cutaneous lymphoproliferative lesion. Reactive nature of the EBV-induced mucocutaneous plasmablastic growth (EBV-positive mucocutaneous ulcer, plasmablastic type) is discussed.

Abstract:Primary cutaneous aggressive epidermotropic cytotoxic CD8 positive T cell lymphoma, is an uncommon disease, with an aggressive clinical behavior. Differentiation with other types of cutaneous T-cell lymphoma (CTCL) that express a CD8+ cells, is based only on clinical grounds and in certain morphological characteristics, such as a marked epidermotropism with squamous cell necrosis. We report a 50-year-old male presenting with painless cutaneous lesions appearing in trunk, limbs, scalp and face, suggestive of cutaneous lymphoma. He was admitted to the hospital in bad conditions, with confluent papules and tumors, some of them ulcerated and with foul smelling honey-colored crusts, involving the complete body surface. Cutaneous biopsy demonstrated a CD8 positive epidermotropic cytotoxic T cell lymphoma. He was treated with chemotherapy with an excellent initial response, but cutaneous lesions reappeared after four cycles. He did not respond to rescue chemotherapy and died seven months after diagnosis.

Abstract:etr1-1is a dominant ethylene receptor gene in Arabidopsis and confers ethylene insensitivity. The truncated etr1-1(1-349) protein is capable of repressing ethylene responses while etr1(1-349) is not, lending support to a hypothesis that the dominant etr1-1(1-349) could convert wild-type receptors to an ethylene-insensitive state. Assuming that etr1-1(1-349) and etr1(1-349) would share a same signaling mechanism, we hypothesize that the etr1(1-349) protein is capable of repressing ethylene responses when not bound with ethylene. In this study, we show that both etr1(1-349) and etr1-1(1-349) are capable of receptor signal output which is primarily dependent of subfamily I receptors. The etr1(1-349) and etr1-1(1-349) clones were individually transformed to mutants and the resulting phenotypes were scored. Each of those transgenes restored the rosette growth and flower fertility of etr1-7 ers1-2 to a similar extent. In contrast, neither etr1(1-349) nor etr1-1(1-349) was capable of signal output in etr1-7 ers1-3. The ERS1 transcript was detectable in ers1-2 but not in ers1-3, implying that the ETR1 N-terminus signaling is subfamily I-dependent. Loss of the subfamily II receptor genes did not perturb the etr1-1(1-349)-mediated ethylene insensitivity. Possible roles of subfamily I receptors and disulfide linkages in the ETR1 receptor signal output mediated through the N-terminus are discussed.
Karyotypic analysis of bone marrow cells in pyodermic lesions associated with myelodysplastic syndrome

Abstract:Background: Recalcitrant pyodermic lesions and neutrophilic dermatoses are often associated with subclinical myelodysplastic syndrome (MDS). In this case series, we assessed the diagnostic importance of karyotypic analysis of bone marrow cells in 4 patients with MDS-associated pyodermic eruptions treated at our university hospital. Karyotypic analysis was performed in bone marrow cells and peripheral blood lymphocytes obtained. Serum levels of granulocyte colony-stimulating factor were measured. Observations: Four patients with pyodermic eruptions or neutrophilic dermatosis had chromosomal abnormalities in bone marrow cells, including del(20)(q11;q13.3) in 2 patients, trisomy 8 in 1 patient, and t(11;22)(q23;q11) in 1 patient. Three patients without morphologic findings suggestive of MDS were diagnosed as having refractory anemia. One female patient had refractory anemia with ringed sideroblasts associated with del(20). Two patients with refractory anemia had a normal karyotype in peripheral blood lymphocytes. Two patients with elevated serum levels of granulocyte colony-stimulating factor had more active or widespread cutaneous diseases. Conclusions: Karyotypic analysis of bone marrow cells, but not of peripheral blood lymphocytes, is essential in proving a diagnosis of MDS-associated pyodermic lesions. The overexpression of granulocyte colony-stimulating factor, which may compensate for impaired hematopoiesis in patients with MDS, seems to be a key cytokine leading to neutrophilic infiltration.

Abstract:A 57-year-old man was admitted to the hospital for a 7-year progressively spreading plaques involving the entire body surface, and multiple irregularly sized red nodules and infiltrated patches on the face, trunk and limbs. Histopathological examination showed pleomorphic tumor cells diffusely dis-persed throughout the dennis, giving an appearance of low proliferation. Some cells with cytoplasmic pro-cesses appeared multiangular in shape, lmmunohistochemically, tumor cells were negative for CDla or S-100, but positive for CD45, FXIIIa, CDl4, MHC- Ⅱ, CD68 and lysozyme with extracellular interstitial expression. Ultrastructurally, the cells exhibited cytoplasmic processes and irregularly sized nuclei; no Birbeck granules were observed. Vesicules of low electron-density were seen diffusely in cytoplasm and extracellular matrix. The case is herein diagnosed as cutaneous non-Langerhans cell histiocytosis, which presents with a chronically invasive clinical course. These cells may develop from immature dermal dendritic cells.
Lymphomatoid Papulosis Type D in a Mestizo-Ancestry Man

Abstract:Lymphomatoid papulosis (LyP) belongs to the CD30 + skin lymphoproliferative disorders; it is defined as a chronic, recurrent, self-healing eruption of papules and small nodules with the histopathologic features of a cutaneous T-cell lymphoma. It is classified according to histopathology into subtypes A to F and with chromosomal rearrangement 6p25.3. Type D is characterized by epidermotropism of atypical CD8 + and CD30 + lymphocytes, small to medium size, forming papules and nodules with erosion and necrosis, which represents a formidable challenge in the differential diagnosis with aggressive cutaneous cytotoxic lymphomas. We present the clinical case of a 22-year-old man with subacute dermatosis, who underwent a skin biopsy with a report of LyP. Immunohistochemistry showed negative CD4, CD5, granzyme-B markers and positive CD3, CD30, CD8, CD56, and (T-cell intracellular antigen 1) TIA-1 markers, concluding the diagnosis of type D LyP. The course of the disease is recurrent; however, the prognosis is good with a 10-year survival of 100%. We present the case of a mestizo-ancestry patient who developed a type-D LyP, and, to the best of our knowledge, there are no publications of type D LyP from Latin-American authors or about mestizo-ancestry (or hispanic) patients; therefore, we consider of relevance to inform about these findings.

Abstract:Primary myelodysplastic syndromes are acquired disorders of hematopoietic stem cells occurring in older persons and characterized by bone marrow failure and a substantial risk of progression into acute myeloid leukemia [1]. These disorders show impressive clinical heterogeneity, ranging from indolent conditions spanning years—like the myelo-dysplastic syndrome with isolated 5q deletion or refractory anemia with ringed sideroblasts—to forms with a rapid evolution into acute myeloid leukemia. The World Health Organization (WHO) classiﬁcation of myeloid neoplasms [2] provides clinicians with a very useful tool for deﬁning the different subtypes of myelodysplastic syndromes. This clas-siﬁcation is not only useful for diagnostic purposes but also has a relevant prognostic value, and it can therefore be used to facilitate clinical decision-making in the individual patient [3,4].
