Abstract:The prognosis of congenital heart disease (CHD) has improved, and most patients now reach adulthood. Owing to residual disease and comorbidities, it is recommended that adult CHD (ACHD) patients transition to adult care for lifelong monitoring and treatment. However, this transition period can be challenging for CHD patients owing to obstacles such as independence from their parents and establishing self-management. To achieve a seamless shift from pediatric to adult care and ensure continuity, it is important to educate and motivate patients appropriately, and an established transition system is needed that involves collaboration between CHD specialists and healthcare providers across medical specialties. The present review describes the epidemiology of ACHD and notable points in patient care as the background of transition. The concepts and an overview of transition systems, educational systems, and potential lapses in the care of their relevant causes are then provided with supporting evidence.

Abstract:Background: Congenital heart anomalies are the most common type of organ malformation, affecting approximately 1% of all newborn infants. More than 90% of these children now survive into adulthood. They need to be cared for by specialists for adults with congenital heart disease (ACHD), as well as by family physicians, internists, and cardiologists who are adequately versed in the basic management of persons with this lifelong condition. Methods: This review is based on pertinent publications retrieved by a selective literature search, including guidelines and consensus statements from Germany and abroad. Results: Cardiovascular malformations cover a very wide spectrum, and the evidence base for the treatment of older patients with these conditions is scant. Congestive heart failure, arrhythmias, and the sequelae of pulmonary arterial hypertension are the main contributors to cardiac morbidity and mortality. Preg - nancy counseling, endocarditis prophylaxis, vaccinations, and psychosocial aspects must be targeted to each individual patient. Neither the affected patients nor their family physicians are yet adequately acquainted with the recently created care structures for this patient group. Conclusion: The care of ACHD is a multidisciplinary task that requires basic care by primary care physicians as well as the involvement of specialized cardiologists in order to ensure optimal individualized treatment.
Prevalence and Outcome of Adults with Congenital Heart Disease and Coronary Artery Disease

Abstract:BACKGROUND Little is known about the prevalence of outcome in patients with coronary artery disease (CAD) among adults with congenital heart disease (ACHD). The objectives of this study were to determine rates of ACHD and factors associated with outcomes among adults with CAD in a contemporary cohort within the China. METHODS Using pragmatic data from the Prospective Registry of the Current Status of Care for Patients with Coronary Artery Disease database, between January and May 2022, we stratified all CAD patients with comorbid ACHD or not. Using logistic regression method, we explored the risk factors associated with lower risk of mortality and stroke. We constructed a matching sub-cohort as sensitive analysis. RESULTS Of 10,517 evaluable patients 189 (1.8%) were with a diagnosis of ACHD, and were more female, more frequent with a history of atrial fibrillation, had a lower hemoglobin level at admission but less likely to have hypertension or smoking history than those without ACHD. In-hospital mortality before discharge was higher in patients with ACHD compared with no ACHD in the whole population (1.06% vs. 0.28%, p=0.05) and matched population (1.06% vs. 0.13%, p=0.04). Acute myocardial infarction (54.8%) represented the most common cause of death among all cohort, whereas 100% deaths among ACHD were due to ischemic stroke (2 cases). CONCLUSIONS ACHD was associated with a higher rate of mortality, which stroke complications contribute most. Future studies are needed to improve the acknowledge of underlying stroke mechanisms and prognosis in this special population.

Abstract:Congenital heart disease affects about 1% of human births. Some defects are incompatible with life, if left unrepaired, while others only lead to symptoms in adulthood. Before reconstructive surgery and interventional techniques became available, most children with congenital heart defects died before reaching adulthood, while today over 90% survive. However, these patients are not cured and require lifelong management to prevent and treat heart failure, arrhythmias, pulmonary hypertension, infections, respiratory disease, hepatic and renal dysfunction, as well as symptoms of exercise intolerance and psychosocial problems. Congenital heart disease is divided into low complexity, moderate complexity, and high complexity lesions. The most common complications are atrial and ventricular arrhythmias, heart failure, thrombus formation, and endocarditis among others. Female individuals reaching adulthood need pregnancy counselling, and most of the patients require treatment with cardiovascular drugs, pacemakers, as well as electrophysiological studies with ablation, re-do surgery, and/or structural interventions.

Abstract:BACKGROUND Heart failure (HF) is one of the many cardiovascular epidemics that plague the world. It has a prevalence of about 40 million people globally and about 1.5% of the United States population. HF has been documented to be the leading cause of death in adults with congenital heart diseases (CHD), described in 26% of all deaths in a national registry of >8000 adults with CHD. This study dives deep into the in-hospital outcomes of patients admitted for HF with a history of CHD. METHOD This was a retrospective study done using patient records from the 2016-2020 National In-patient Database (NIS). We identified patients who were admitted for HF with a history of CHD, evaluated their associated socio-demographic and comorbid factors using (International Classification of Diseases- 10) ICD-10 codes. We excluded patients under 18 years of age. We utilized the chi-square test to compare baseline characteristics between our populations with and without a history of CHD and further performed multivariate logistic regression to identify outcomes. RESULTS The study analyzed a total of 1,372,419 patients with HF, out of which 10,315 individuals (1.3%) had a past medical history of CHD. The results revealed that patients with a history of CHD were significantly younger, with a mean age of 60 years compared to 70 years in those without CHD. Our population with a history of CHD had a higher rates of acute kidney injuries (32.2% vs 25.9%, p<0.0001), prior history of a cardiovascular accident (1.6% vs 0.4%, p<0.0001), liver disease (16.7% vs 6.6%, p<0.0001), electrolyte abnormalities (39.8% vs 32%, p< 0.0001), Anemia (33.7% vs 31.3%, p=0.017) and had a Charlson comorbidity index ≥3 (p<0.0001). Although the prevalence of HF in patients with CHD has decreased in recent years (from 4.9% - 1% between 2016 - 2020), in-hospital mortality rates remain high [adjusted Odd Ratio (aOR): 1.32, CI 1.0 - 1.66, p=0.023] The results of our study demonstrated that the population with a history of CHD had increased rates of cardiogenic shock (aOR: 1.32, CI: 1.1 - 1.7, p=0.005), respiratory failure (aOR: 1.3, CI:1.2 - 1.4, p<0.001), and a higher likelihood of using Mechanical Circulatory support devices (aOR: 2.7, CI: 1.03 - 3.58, p<0.001). Moreover, the CHD group had a longer mean length of hospital stay (9.7 days vs 5.4 days, p<0.0001) and incurred higher total hospital costs ($153,551.1 vs $53,169.8, p<0.0001). CONCLUSION Individuals with HF who also have a history of CHD experience poorer outcomes compared to those without CHD due to a range of intrinsic and extrinsic factors, such as the presence of structural abnormalities or additional cardiovascular conditions. Therefore, it is crucial to focus on preserving myocardial function throughout the individual's lifespan to mitigate the negative impact of these factors.

Abstract:Congenital heart disease (CHD) has become the most common birth defect and is now the first leading cause of infant mortality due to congenital malformations in China. According to the 2012 Birth Defects Report , CHD has been the first leading birth defect in China since a decade ago. Patients with CHD have reached 2 million in China, and newly diagnosed patients were estimated to be 150 000–2 00 000 annually.1 China has the largest population in the world and with 15% of all children worldwide. In 2016, there were 17.9 million newborns.2 After adoption of the universal two-child policy since 2015, the incidence of CHD may be increased as a result of high portion of women with high maternal age. Though with such high incidence of CHD and large population of patients with CHD, no country-level data registry has been established. Current epidemiology of CHD is regional and not accurate, lacking high evidence. According to the available data, we think that the incidence of CHD is varied across the country. The data of 2011 showed that incidence of CHD diagnosed during perinatal period is 6.8–14.39%. Some regional surveillance data showed that the prevalence of CHD was 3.81% in Jiuquan City from 2002 to 2003, 8.04% in Beijing City during 2007–2012, and 9.04% in Guangdong Province during 2006–2015.3 Concerning the disease types, ventricular septal defect, atrial septal defect, and patent ductus arteriosus account for 75%–80% of all CHD. The nationwide epidemiological study on CHD remains difficult in China because of no unified diagnosis techniques and guidelines among different regions, no standard surveillance in the migration population, and lacking knowledge of collecting big data for analysis in the population. Abstract:Background: In the United States, there are over 1.5 million adults living with congenital heart disease (CHD). The Congenital Heart Initiative (CHI) is a digital, online patient empowered registry that was created to advance multicenter research and improve clinical care by gathering patient-reported outcomes (PROs) in adults with CHD. Methods: After a two year design process, the CHI was created and launched nationally on December 7, 2020 using a human centered design approach. Demographics and validated survey tools on quality of life, mental health, physical activity and health care utilization were collected at baseline and every 4 months. Data were collected virtually and stored on Health Insurance Portability and Accountability Act (HIPAA)?compliant cloud-based servers with restricted access. Results: By December 31, 2023, the CHI had enrolled 4558 participants (56% female) with an average age of 39 years + 14, representing all 50 states. Approximately 88% of participants have completed at least one e-Visit as of December 31, 2023. The most prevalent CHD anatomy included tetralogy of Fallot (883, 22%), transposition of great arteries (452, 11%), and coarctation of the aorta (429, 11%). Approximately 88% of participants reported at least one co-morbidity, with arrhythmia (1310, 29%) and mood disorder (1339, 29%) as the most common cardiac and non-cardiac co-morbidity, respectively. Among female participants, 45% (n=1147) reported having had a pregnancy with 38% (n=967) resulting in biological children. Participants with complex CHD were less likely to meet recommended physical activity guidelines (X2 (2, n = 917) = 15.9, p < 0.001), a factor that was more pronounced amongst female participants. Overall health-related quality of life was rated as good or better by 84% of participants with no difference by CHD complexity. Conclusion: CHI is the largest ongoing registry of adults living with congenital heart disease in the US and includes patients with a wide variety of CHD subtypes. Many patients report mood disorders, but most report good or very good health-reported quality of life. The CHI is poised to facilitate multicenter research with the goal of improving clinical outcomes for all adults with CHD.

Abstract:Aim: Out-of-hospital cardiac arrest (OHCA) has a poor prognosis in children; however, the annual patterns of prognosis and treatment have not been fully investigated. Methods: From the Japanese Association for Acute Medicine OHCA registry, a multicenter prospective observational registry in Japan, we identified pediatric patients (zero to 17 years old) between June 2014 and December 2019. The primary outcome was one-month survival. We investigated the annual patterns in patient characteristics, treatment, and one-month prognosis. Results: During the study period, 1188 patients were eligible for analysis. For all years, the zero-year-old group accounted for a large percentage of the total population (between 30% and 40%). There were significant increases in the rates of bystander-initiated cardiopulmonary resuscitation (CPR; from 50.6% to 62.3%, p = 0.003), dispatcher instructions (from 44.7% to 65.7%, p = 0.001), and adrenaline administration (from 2.4% to 6.9%, p = 0.014) over time, whereas the rate of advanced airway management decreased significantly (from 17.7% to 8.8%, p = 0.003). The odds ratios for one-month survival adjusted for potential resuscitation factors also did not change significantly (from 7.1% to 10.3%, adjusted odds ratio for one-year increment = 0.98, confidence interval: 0.86-1.11). Conclusion: Despite an increase in the rate of bystander-initiated CPR and pre-hospital adrenaline administration, there was no significant change in one-month survival.

Abstract:Congenital heart disease (CHD) is the most common birth defect, affecting about 0.8% of live births. Advances in recent decades have allowed >85% of children with CHD to survive to adulthood, creating a growing population of adults with CHD. Little information exists regarding survival, demographics, late outcomes, and comorbidities in this emerging group, and multiple barriers impede research in adult CHD. The National Heart, Lung, and Blood Institute and the Adult Congenital Heart Association convened a multidisciplinary working group to identify high-impact research questions in adult CHD. This report summarizes the meeting discussions in the broad areas of CHD-related heart failure, vascular disease, and multisystem complications. High-priority subtopics identified included heart failure in tetralogy of Fallot, mechanical circulatory support/transplantation, sudden cardiac death, vascular outcomes in coarctation of the aorta, late outcomes in single-ventricle disease, cognitive and psychiatric issues, and pregnancy.
Impact of an Adult Congenital Heart Disease Order Set in a Pediatric Cardiac Intensive Care Unit

Abstract:Background: Patients with congenital heart disease (CHD) are increasingly surviving to adulthood. Given their age- and condition-specific needs, the optimal postoperative setting for these patients is not yet determined. This study aims to evaluate the utility of an adult-specific order set in improving clinical outcomes for these patients in a pediatric cardiovascular intensive care unit (ICU). Methods: Adults with CHD (ACHD) admitted to the cardiovascular ICU following cardiac surgery at a pediatric tertiary care center were identified for a retrospective cohort study. Health care delivery metrics and clinical outcomes of participants who received the adult-specific order set were compared with control patients admitted prior to the intervention. Categorical outcomes were compared using the χ2 test of independence or Fisher exact test, and continuous outcomes were assessed using the t test. Results: A total of 130 ACHD patients received the intervention, with no significant differences observed in clinical outcomes when compared with 89 controls. While there was additionally no reduction in hospital-related mortality (2.3% vs 7.8%, RR 0.3; P = .1), clinician ordering behaviors were better aligned with best-practices following the intervention. Among patients with moderate or greater anatomic complexity, a post hoc analysis demonstrated reduced in-hospital mortality from those who received the adult order set. Conclusions: The implementation of an adult-specific order set in a pediatric care setting did not improve clinical outcomes for all ACHD patients. Patients with moderate and greater anatomic complexity did see a mortality benefit, suggestive that targeted electronic tools may most benefit those who have the highest risk.
Adults With Congenital Heart Disease: Trends in Event-Free Survival Past Middle Age

Abstract:Background: The survival of children with congenital heart disease has increased substantially over the past decades, with 97% currently reaching adulthood. The total effect of advanced treatment on future mortality and morbidity in adult survivors with congenital heart disease (CHD) is less well described. Methods: We used data from the Swedish National Inpatient, Outpatient, and Cause of Death Register to identify patients with CHD who were born between 1950 and 1999 and were alive at 18 years of age. Ten controls identified from the Total Population Register were matched for year of birth and sex and with each patient with CHD. Follow-up was from 1968 and 18 years of age until death or at the end of the study (2017). Survival percentage with 95% CI for all-cause mortality were performed with Kaplan-Meier survival function. Cox proportional hazard regression models with hazard ratios (HRs) and 95% CI were used to estimate the risk of all-cause mortality. Results: We included 37 278 patients with adult CHD (ACHD) and 412 799 controls. Mean follow-up was 19.2 years (±13.6). Altogether, 1937 patients with ACHD (5.2%) and 6690 controls (1.6%) died, a death rate of 2.73 per 1000 person-years and 0.84 per 1000 person years, respectively. Mortality was 3.2 times higher (95% CI, 3.0-3.4; P<0.001) among patients with ACHD compared with matched controls. Up to the maximum of 50 years of follow-up, >75% of patients with ACHD were still alive. Mortality was highest among patients with conotruncal defects (HR, 10.13 [95% CI, 8.78-11.69]), but also significantly higher for the more benign lesions, with the lowest risk in patients with atrial septal defects (HR, 1.36 [95% CI, 1.19-1.55]). At least 75% of patients with ACHD alive at 18 years of age lived past middle age and became sexagenerians. Conclusions: In this large, nationwide, register-based cohort study of patients with ACHD surviving to 18 years of age, the risk of mortality up to 68 years of age was >3 times higher compared with matched controls without ACHD. Despite this, at least 75% of patients with CHD alive at 18 years of age lived past middle age and became sexagenerians. A notable risk decline in the mortality for patients with ACHD was seen for those born after 1975.

Abstract:Organization of B chromosomes in the Korean field mouse Apodemus peninsulae was analyzed. We painted its metaphase chromosomes with whole and partial chromosome paints generated by microdissection and DOP-PCR. The results of the painting indicated that all B chromosomes contained a large amount of repeated DNA sequences. The repeats could be classified in terms of their homology and predominant location. Pericentromeric repeats of B chromosomes were present in many copies in pericentromeric C-blocks of all autosomes and in non-centromeric C-blocks of the sex chromosomes. B arm specific type 1 repeats comprised the main body of the arms of almost all B chromosomes and were present in the arms of A chromosomes as interspersed sequences. B arm-specific type 2 repeats were found at the ends of some B chromosomes that did not undergo compaction at the interphase– metaphase transition and remained uncondensed. On the basis of comparative analysis of localization of B chromosome repeats in the chromosomes of two related species, A. peninsulae and A. agrarius, we suggest a hypothesis of B chromosome origin and evolution in the genus Apodemus.
Peripartum Management of Pregnant Women With Congenital Heart Disease

Abstract:Background: The average maternal age at delivery, and thus the associated maternal risk are increasing including in women with congenital heart disease (CHD). A comprehensive management approach is therefore required for pregnant women with CHD. The present study aimed to investigate the factors determining peripartum safety in women with CHD.Methods and Results:We retrospectively collected multicenter data for 217 pregnant women with CHD (age at delivery: 31.4±5.6 years; NYHA classifications I and II: 88.9% and 7.4%, respectively). CHD severity was classified according to the American College of Cardiology/American Heart Association guidelines as simple (n=116), moderate complexity (n=69), or great complexity (n=32). Cardiovascular (CV) events (heart failure: n=24, arrhythmia: n=9) occurred in 30 women during the peripartum period. Moderate or great complexity CHD was associated with more CV events during gestation than simple CHD. CV events occurred earlier in women with moderate or great complexity compared with simple CHD. Number of deliveries (multiparity), NYHA functional class, and severity of CHD were predictors of CV events. Conclusions: This study identified not only the severity of CHD according to the ACC/AHA and NYHA classifications, but also the number of deliveries, as important predictive factors of CV events in women with CHD. This information should be made available to women with CHD and medical personnel to promote safe deliveries.

Abstract:Background: Congenital heart diseases (CHDs) are conditions that involve structural problems to the heart's structure existing at birth, with an incidence of approximately 8 per 1,000 live births globally. CHD is one of the leading causes of maternal, fetal, and neonatal morbidity and mortality worldwide. The present study sought to examine the clinical profiles of CHD patients and provide important implications for therapeutic interventions. Methods: This was a retrospective, observational, cohort study. The medical records of all CHDs patients aged between 0 and 18 years were collected from July 1, 2021 to June 30, 2022. Clinical profiles and demographic data were collected from cardiology and pediatric department registers for analysis. Results: Of the 265 children with CHDs, 201 were diagnosed with acyanotic CHD (ACHD), while 64 children had cyanotic CHD (CCHD). Based on the eleventh revision of the International Classification of Diseases (ICD-11), "congenital anomaly of a ventricle or the ventricular septum" was the most common CHD. The most common symptom was failure to thrive, accounting for 18.5% of all CHD cases. The most frequent symptom in ACHD was murmur (93.53%) and sweating (80.60%), whereas the most common symptom in CCHD was sweating (95.31%) and cyanosis (84.38%). Conclusions: This study retrospectively analyzed CHD clinical characteristics from children receiving care at the seventh center, which forms a proper basis for appropriate clinical treatments and further studies.
