Abstract:Catheter ablation (CA) is considered as one of the most effective methods technique for eradicating persistent and abnormal cardiac arrhythmias. Nevertheless, in some cases, these arrhythmias are not treated properly, resulting in their recurrences. If left untreated, they may result in complications such as strokes, heart failure, or death. Until recently, the primary techniques for diagnosing recurrent arrhythmias following CA were the findings predisposing to the changes caused by the arrhythmias on cardiac imaging and electrocardiograms during follow-up visits, or if patients reported having palpitations or chest discomfort after the ablation. However, these follow-ups may be time-consuming and costly, and they may not always determine the root cause of the recurrences. With the introduction of artificial intelligence (AI), these follow-up visits can be effectively shortened, and improved methods for predicting the likelihood of recurring arrhythmias after their ablation procedures can be developed. AI can be divided into two categories: machine learning (ML) and deep learning (DL), the latter of which is a subset of ML. ML and DL models have been used in several studies to demonstrate their ability to predict and identify cardiac arrhythmias using clinical variables, electrophysiological characteristics, and trends extracted from imaging data. AI has proven to be a valuable aid for cardiologists due to its ability to compute massive amounts of data and detect subtle changes in electric signals and cardiac images, which may potentially increase the risk of recurrent arrhythmias after CA. Despite the fact that these studies involving AI have generated promising outcomes comparable to or superior to human intervention, they have primarily focused on atrial fibrillation while atrial flutter (AFL) and atrial tachycardia (AT) were the subjects of relatively few AI studies. Therefore, the aim of this review is to investigate the interaction of AI algorithms, electrophysiological characteristics, imaging data, risk score calculators, and clinical variables in predicting cardiac arrhythmias following an ablation procedure. This review will also discuss the implementation of these algorithms to enable the detection and prediction of AFL and AT recurrences following CA.

Abstract:Cardiovascular health-related problem is a rapidly increasing integrated field concerning the processing and fetching the information from cardiovascular systems for early detection and treatment of cardiovascular diseases. Artificial Intelligence (AI) techniques, especially machine and deep learning techniques are more impactful and powerful tools for upgrading the capabilities of an application, and they have been applied to medical data for analysis and disease detection purposes. This paper, represents a comprehensive view of AI-based computational modeling with the abilities of powerful AI techniques that can play a crucial role in developing smart and enhanced systems in a real-world application. The paper outlines the broad overview of AI-based modeling that can be utilized in various application domains. An electrocardiogram (ECG) plays a major role in biomedical applications to record the heartbeat activity. Regular monitoring of ECG through wearable devices like the band, watches, etc. can be done for early detection of cardiovascular diseases. The competency of each method discussed is related to ECG classification approaches that have been compared in terms of some parameters like accuracy, sensitivity, specificity, positive predictivity, and F-score. The noise affects the ECG signal which may deteriorate the features of the respective signal that leading to improper treatment. De-noising has been done by pre-processing of the signal, which enables the prediction of the heart condition. After detecting the positions of the P wave, QRS wave, and T wave, feature extraction has been done. The efficiency of ECG classification with different computational methods was evaluated with proposed algorithms by using different databases like MIT-BIH, PTB, and MIT-BIH Atrial Fibrillation test. It has been observed from the literature that Convolutional Neural Networks (CNN) are best suited for the classification and detection of arrhythmia. The challenges of existing techniques to analyze the ECG signal for the classification and detection of arrhythmia are summarized.

Artificial Intelligence Interpretation of the Electrocardiogram: A State-of-the-Art Review

Abstract:Purpose of review: Artificial intelligence (AI) is transforming electrocardiography (ECG) interpretation. AI diagnostics can reach beyond human capabilities, facilitate automated access to nuanced ECG interpretation, and expand the scope of cardiovascular screening in the population. AI can be applied to the standard 12-lead resting ECG and single-lead ECGs in external monitors, implantable devices, and direct-to-consumer smart devices. We summarize the current state of the literature on AI-ECG. Recent findings: Rhythm classification was the first application of AI-ECG. Subsequently, AI-ECG models have been developed for screening structural heart disease including hypertrophic cardiomyopathy, cardiac amyloidosis, aortic stenosis, pulmonary hypertension, and left ventricular systolic dysfunction. Further, AI models can predict future events like development of systolic heart failure and atrial fibrillation. AI-ECG exhibits potential in acute cardiac events and non-cardiac applications, including acute pulmonary embolism, electrolyte abnormalities, monitoring drugs therapy, sleep apnea, and predicting all-cause mortality. Many AI models in the domain of cardiac monitors and smart watches have received Food and Drug Administration (FDA) clearance for rhythm classification, while others for identification of cardiac amyloidosis, pulmonary hypertension and left ventricular dysfunction have received breakthrough device designation. As AI-ECG models continue to be developed, in addition to regulatory oversight and monetization challenges, thoughtful clinical implementation to streamline workflows, avoiding information overload and overwhelming of healthcare systems with false positive results is necessary. Research to demonstrate and validate improvement in healthcare efficiency and improved patient outcomes would be required before widespread adoption of any AI-ECG model.

Abstract:Contemporary methods used to interpret the electrocardiogram (ECG) signal for diagnosis or monitoring are based on expert knowledge and rule-centered algorithms. In recent years, with the advancement of artificial intelligence, more and more researchers are using deep learning (ML) and deep learning (DL) with ECG data to detect different types of cardiac issues as well as other health problems such as respiration rate, sleep apnea, and blood pressure, etc. This study presents an extensive literature review based on research performed in the last few years where ML and DL have been applied with ECG data for many diagnoses. However, the review found that, in published work, the results showed promise. However, some significant limitations kept that technique from implementation in reality and being used for medical decisions; examples of such limitations are imbalanced and the absence of standardized dataset for evaluation, lack of interpretability of the model, inconsistency of performance while using a new dataset, security, and privacy of health data and lack of collaboration with physicians, etc. AI using ECG data accompanied by modern wearable biosensor technologies has the potential to allow for health monitoring and early diagnosis within reach of larger populations. However, researchers should focus on resolving the limitations.

Abstract:Vibrio species release a lipase which shares many properties with mammalian lecithin-cholesterol acyltransferase. We have studied the action of the enzyme on phospholipid monolayers. At similar surface pressures, reaction velocities were higher with monolayers of dilauroylphosphatidylcholine than with the corresponding phosphatidylglycerol or phosphatidylethanolamine. The dependence of reaction velocity on molecular density was very similar for phosphatidylcholine and phosphatidylethanolamine monolayers. Lag times were shortest with phosphatidylglycerol at low molecular densities, but maximum velocity was reached at considerably lower densities than with the other two lipids. We have found [Hilton, S., McCubbin, N. D., Kay, C., & Buckley, J.T. (1990) Biochemistry 29, 9072-9078] that nicking of the enzyme with trypsin or other proteases results in an increase in its activity against lipids in membranes. Here we show that trypsin treatment results in a large change in the surface activity of the lipase, allowing it to penetrate monolayers at pressures higher than 40 mN.m-1.

Abstract:Purpose of Review Development of artificial intelligence (AI) models, particularly the application of machine learning (ML) and deep learning (DL) architectures to enhance the current diagnostic armoury for the detection of cardiovascular diseases (CVD) has expanded exponentially over the past five years. We review the current AI landscape in CVD and the impact of applying AI models to the electrocardiogram (ECG). Recent Findings Researchers continue to explore innovative methods utilising ML and DL to automate ECG diagnosis and gain insights into underlying cardiovascular physiology. The results from numerous studies including two randomised control trials (EAGLE and BEAGLE) demonstrates the wide-ranging potential for the application of AI models to both 12-lead and single lead ECG recordings. This allows a fixed timepoint recording to effectively be extrapolated into a continuous monitoring device to predict outcomes for CVD diseases such as left ventricular systolic dysfunction (LVSD), atrial fibrillation (AF), hypertrophic obstructive cardiomyopathy (HOCM), cardiac amyloidosis, valvular heart disease, pulmonary hypertension and channelopathies. A significant proportion of models developed utilise DL architectures such as convolutional neural networks (CNN). The area under the curve (AUC) for published studies range from 0.66–0.99 with sensitivities and specificities between 26.29—95% and 79.5—96.6% respectively. These results suggest a possible role for AI-ECG as a screening tool for cardiovascular pathology. Summary Initial research is promising and demonstrates the potential transformative impact of AI to streamline workflow, aid clinical decision making and prognosticate future cardiovascular outcomes. Future randomised controlled trials are needed to provide an evidence base for the implementation of AI tools into the clinician's workflow.

Abstract:Electrocardiography (ECG), improved by artificial intelligence (AI), has become a potential technique for the precise diagnosis and treatment of cardiovascular disorders. The conventional ECG is a frequently used, inexpensive, and easily accessible test that offers important information about the physiological and anatomical state of the heart. However, the ECG can be interpreted differently by humans depending on the interpreter's level of training and experience, which could make diagnosis more difficult. Using AI, especially deep learning convolutional neural networks (CNNs), to look at single, continuous, and intermittent ECG leads that has led to fully automated AI models that can interpret the ECG like a human, possibly more accurately and consistently. These AI algorithms are effective non-invasive biomarkers for cardiovascular illnesses because they can identify subtle patterns and signals in the ECG that may not be readily apparent to human interpreters. The use of AI in ECG analysis has several benefits, including the quick and precise detection of problems like arrhythmias, silent cardiac illnesses, and left ventricular failure. It has the potential to help doctors with interpretation, diagnosis, risk assessment, and illness management. Aside from that, AI-enhanced ECGs have been demonstrated to boost the identification of heart failure and other cardiovascular disorders, particularly in emergency department settings, allowing for quicker and more precise treatment options. The use of AI in cardiology, however, has several limitations and obstacles, despite its potential. The effective implementation of AI-powered ECG analysis is limited by issues such as systematic bias. Biases based on age, gender, and race result from unbalanced datasets. A model's performance is impacted when diverse demographics are inadequately represented. Potentially disregarded age-related ECG variations may result from skewed age data in training sets. ECG patterns are affected by physiological differences between the sexes; a dataset that is inclined toward one sex may compromise the accuracy of the others. Genetic variations influence ECG readings, so racial diversity in datasets is significant. Furthermore, issues such as inadequate generalization, regulatory barriers, and interpretability concerns contribute to deployment difficulties. The lack of robustness in models when applied to disparate populations frequently hinders their practical applicability. The exhaustive validation required by regulatory requirements causes a delay in deployment. Difficult models that are not interpretable erode the confidence of clinicians. Diverse dataset curation, bias mitigation strategies, continuous validation across populations, and collaborative efforts for regulatory approval are essential for the successful deployment of AI ECG in clinical settings and must be undertaken to address these issues. To guarantee a safe and successful deployment in clinical practice, the use of AI in cardiology must be done with a thorough understanding of the algorithms and their limits. In summary, AI-enhanced electrocardiography has enormous potential to improve the management of cardiovascular illness by delivering precise and timely diagnostic insights, aiding clinicians, and enhancing patient outcomes. Further study and development are required to fully realize AI's promise for improving cardiology practices and patient care as technology continues to advance.

Pre-Processing techniques and artificial intelligence algorithms for electrocardiogram (ECG) signals analysis: A comprehensive review

Abstract:Electrocardiogram (ECG) are the physiological signals and a standard test to measure the heart's electrical activity that depicts the movement of cardiac muscles. A review study has been conducted on ECG signals analysis with the help of artificial intelligence (AI) methods over the last ten years i.e., 2012–22. Primarily, the method of ECG analysis by software systems was divided into classical signal processing (e.g. spectrograms or filters), machine learning (ML) and deep learning (DL), including recursive models, transformers and hybrid. Secondly, the data sources and benchmark datasets were depicted. Authors grouped resources by ECG acquisition methods into hospital-based portable machines and wearable devices. Authors also included new trends like advanced pre-processing, data augmentation, simulations and agent-based modeling. The study found improvement in ECG examination perfection made each year through ML, DL, hybrid models, and transformers. Convolutional neural networks and hybrid models were more targeted and proved efficient. The transformer model extended the accuracy from 90% to 98%. The Physio-Net library helps acquire ECG signals, including the popular benchmark databases such as MIT-BIH, PTB, and challenging datasets. Similarly, wearable devices have been established as a appropriate option for monitoring patient health without the time and place limitations and are also helpful for AI model calibration with so far accuracy of 82%–83% on Samsung smartwatch. In the pre-processing signals, spectrogram generation through Fourier and wavelet transformations are erected leading approaches promoting on average accuracy of 90%–95%. Likewise, data enhancement using geometrical techniques is well-considered; however, extraction and concatenation-based methods need attention. As the what-if analysis in healthcare or cardiac issues can be performed using a complex simulation, the study reviews agent-based modeling and simulation approaches for cardiovascular risk event assessment.

Abstract:The electronic stethoscope combined with artificial intelligence (AI) technology has realized the digital acquisition of heart sounds and intelligent identification of congenital heart disease, which provides objective basis for heart sound auscultation and improves the accuracy of congenital heart disease diagnosis. At the present stage, the AI based cardiac auscultation technique mainly focuses on the research of AI algorithms, and the researchers have designed and summarized a variety of effective algorithms based on the characteristics of cardiac audio data, among which the mel-frequency cepstral coefficients (MFCC) is the most effective one, and widely used in the cardiac auscultation. However, the current cardiac sound analysis techniques are based on specific data sets, and have not been validated in clinic, so the performance of algorithms need to be further verified. The lack of heart sound data, especially the high-quality, standardized, publicly available heart sound database with disease labeling, further restricts the development of heart sound diagnostic analysis and its application in screening. Therefore, expert consensus is necessary in establishing an authoritative heart sound database and standardizing the heart sound auscultation screening process for congenital heart disease. This paper provides an overview of the research and application status of auscultation algorithm and hardware equipment based on AI in auscultation screening of congenital heart disease, and puts forward the problems to be solved in clinical application of AI auscultation screening technology.
Electrocardiogram-based artificial intelligence for the diagnosis of heart failure: a systematic review and meta-analysis.

Abstract:BACKGROUND:The electrocardiogram (ECG) is an inexpensive and easily accessible investigation for the diagnosis of cardiovascular diseases including heart failure (HF). The application of artificial intelligence (AI) has contributed to clinical practice in terms of aiding diagnosis, prognosis, risk stratification and guiding clinical management. The aim of this study is to systematically review and perform a meta-analysis of published studies on the application of AI for HF detection based on the ECG. METHODS:We searched Embase, PubMed and Web of Science databases to identify literature using AI for HF detection based on ECG data. The quality of included studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) criteria. Random-effects models were used for calculating the effect estimates and hierarchical receiver operating characteristic curves were plotted. Subgroup analysis was performed. Heterogeneity and the risk of bias were also assessed. RESULTS:A total of 11 studies including 104,737 subjects were included. The area under the curve for HF diagnosis was 0.986, with a corresponding pooled sensitivity of 0.95 (95% CI: 0.86-0.98), specificity of 0.98 (95% CI: 0.95-0.99) and diagnostic odds ratio of 831.51 (95% CI: 127.85-5407.74). In the patient selection domain of QUADAS-2, eight studies were designated as high risk. CONCLUSIONS:According to the available evidence, the incorporation of AI can aid the diagnosis of HF. However, there is heterogeneity among machine learning algorithms and improvements are required in terms of quality and study design.

Abstract:Myocardial infarction (MI) results in heart muscle injury due to receiving insufficient blood flow. MI is the most common cause of mortality in middle-aged and elderly individuals worldwide. To diagnose MI, clinicians need to interpret electrocardiography (ECG) signals, which requires expertise and is subject to observer bias. Artificial intelligence-based methods can be utilized to screen for or diagnose MI automatically using ECG signals. In this work, we conducted a comprehensive assessment of artificial intelligence-based approaches for MI detection based on ECG and some other biophysical signals, including machine learning (ML) and deep learning (DL) models. The performance of traditional ML methods relies on handcrafted features and manual selection of ECG signals, whereas DL models can automate these tasks. The review observed that deep convolutional neural networks (DCNNs) yielded excellent classification performance for MI diagnosis, which explains why they have become prevalent in recent years. To our knowledge, this is the first comprehensive survey of artificial intelligence techniques employed for MI diagnosis using ECG and some other biophysical signals.

Abstract:Background: With the advancement in machine learning (ML) and artificial neural networks as well as the development of portable electrocardiogram devices, artificial intelligence (AI) has been increasing in popularity over the years. In this study, we aimed to provide an overview of the research regarding the utilization of AI techniques to improve the diagnosis of arrhythmia.Methods: We extracted data published 2004 to 2021 from Web of Science database. The online analytic platform, Literature Metrology (http://bibliometric.com), was used to analyze publication trends, including information about journals, authors, institutions, collaborations between countries, citations, and keywords.Results: Keywords, such as deep learning, electrocardiogram (ECG), and convolutional neural network, have been increasing in frequency over the years. The analysis outcomes demonstrated that topics associated with AI, robotic prosthesis, and big data analysis for arrhythmia have become increasingly popular since 2016. Our study also found that atrial fibrillation (AF) and ventricular arrhythmia were the two ECG signal sharing the most interest.Conclusions: The utility of deep learning in diagnostics and the prognostication of arrhythmia has been gaining traction over the years, covering areas from electrocardiogram detection to atrial arrhythmogenesis model construction. Our study revealed the trend of topics from 2004 to 2021, which may help researchers to monitor future trends.

Abstract:The application of artificial intelligence (AI) to the electrocardiogram (ECG), a ubiquitous and standardized test, is an example of the ongoing transformative effect of AI on cardiovascular medicine. Although the ECG has long offered valuable insights into cardiac and non-cardiac health and disease, its interpretation requires considerable human expertise. Advanced AI methods, such as deep-learning convolutional neural networks, have enabled rapid, human-like interpretation of the ECG, while signals and patterns largely unrecognizable to human interpreters can be detected by multilayer AI networks with precision, making the ECG a powerful, non-invasive biomarker. Large sets of digital ECGs linked to rich clinical data have been used to develop AI models for the detection of left ventricular dysfunction, silent (previously undocumented and asymptomatic) atrial fibrillation and hypertrophic cardiomyopathy, as well as the determination of a person's age, sex and race, among other phenotypes. The clinical and population-level implications of AI-based ECG phenotyping continue to emerge, particularly with the rapid rise in the availability of mobile and wearable ECG technologies. In this Review, we summarize the current and future state of the AI-enhanced ECG in the detection of cardiovascular disease in at-risk populations, discuss its implications for clinical decision-making in patients with cardiovascular disease and critically appraise potential limitations and unknowns.

Abstract:The artificial intelligence (AI) system has achieved expert-level performance in electrocardiogram (ECG) signal analysis. However, in underdeveloped countries or regions where the healthcare information system is imperfect, only paper ECGs can be provided. Analysis of real-world ECG images (photos or scans of paper ECGs) remains challenging due to complex environments or interference. In this study, we present an AI system developed to detect and screen cardiac abnormalities (CAs) from real-world ECG images. The system was evaluated on a large dataset of 52,357 patients from multiple regions and populations across the world. On the detection task, the AI system obtained area under the receiver operating curve (AUC) of 0.996 (hold-out test), 0.994 (external test 1), 0.984 (external test 2), and 0.979 (external test 3), respectively. Meanwhile, the detection results of AI system showed a strong correlation with the diagnosis of cardiologists (cardiologist 1 (R=0.794, p<1e-3), cardiologist 2 (R=0.812, p<1e-3)). On the screening task, the AI system achieved AUCs of 0.894 (hold-out test) and 0.850 (external test). The screening performance of the AI system was better than that of the cardiologists (AI system (0.846) vs. cardiologist 1 (0.520) vs. cardiologist 2 (0.480)). Our study demonstrates the feasibility of an accurate, objective, easy-to-use, fast, and low-cost AI system for CA detection and screening. The system has the potential to be used by healthcare professionals, caregivers, and general users to assess CAs based on real-world ECG images.

Abstract:Leveraging artificial intelligence (AI) for the analysis of electrocardiograms (ECGs) has the potential to transform diagnosis and estimate the prognosis of not only cardiac but, increasingly, noncardiac conditions. In this review, we summarize clinical studies and AI-enhanced ECG-based clinical applications in the early detection, diagnosis, and estimating prognosis of cardiovascular diseases in the past 5 years (2019-2023). With advancements in deep learning and the rapid increased use of ECG technologies, a large number of clinical studies have been published. However, most of these studies are single-centre, retrospective, proof-of-concept studies that lack external validation. Prospective studies that progress from development toward deployment in clinical settings account for < 15% of the studies. Successful implementations of ECG-based AI applications that have received approval from the Food and Drug Administration have been developed through commercial collaborations, with approximately half of them being for mobile or wearable devices. The field is in its early stages, and overcoming several obstacles is essential, such as prospective validation in multicentre large data sets, addressing technical issues, bias, privacy, data security, model generalizability, and global scalability. This review concludes with a discussion of these challenges and potential solutions. By providing a holistic view of the state of AI in ECG analysis, this review aims to set a foundation for future research directions, emphasizing the need for comprehensive, clinically integrated, and globally deployable AI solutions in cardiovascular disease management.
The Determinants of State Policy Innovativeness

Abstract:What determines an American state’s propensity for innovativeness, or their willingness to adopt new policies sooner or later relative to other states? Most studies focus almost exclusively on one policy area at a time at the expense of a broader understanding of innovativeness as a characteristic of states. Our study therefore departs from this tradition by simultaneously studying policy innovation across almost 100 different policies. We exploit this leverage to study the broad determinants of state innovativeness via a pooled event history analysis of all policies. This allows us to determine the internal factors that influence innovativeness in general and to more flexibly study external diffusion patterns via contiguity, geographic proximity in general, and ideological proximity.
Low-risk lifestyle behaviors and all-cause mortality: findings from the National Health and Nutrition Examination Survey III Mortality Study.

Abstract:OBJECTIVES We examined the relationship between 4 low-risk behaviors-never smoked, healthy diet, adequate physical activity, and moderate alcohol consumption-and mortality in a representative sample of people in the United States. METHODS We used data from 16958 participants aged 17 years and older in the National Health and Nutrition Examination Survey III Mortality Study from 1988 to 2006. RESULTS The number of low-risk behaviors was inversely related to the risk for mortality. Compared with participants who had no low-risk behaviors, those who had all 4 experienced reduced all-cause mortality (adjusted hazard ratio [AHR]=0.37; 95% confidence interval [CI]=0.28, 0.49), mortality from malignant neoplasms (AHR=0.34; 95% CI=0.20, 0.56), major cardiovascular disease (AHR=0.35; 95% CI=0.24, 0.50), and other causes (AHR=0.43; 95% CI=0.25, 0.74). The rate advancement periods, representing the equivalent risk from a certain number of years of chronological age, for participants who had all 4 high-risk behaviors compared with those who had none were 11.1 years for all-cause mortality, 14.4 years for malignant neoplasms, 9.9 years for major cardiovascular disease, and 10.6 years for other causes. CONCLUSIONS Low-risk lifestyle factors exert a powerful and beneficial effect on mortality.

Abstract:Abnormal cardiac functionality produces irregular heart rhythms which are commonly known as arrhythmias. In some conditions, arrhythmias are treated as very dangerous which may lead to sudden cardiac arrest. The incidence and prevalence of cardiac anomalies seeks early detection of arrhythmias using automated classification techniques. In the past, numerous automated arrhythmia detection techniques have been developed that are based on electrocardiogram (ECG) signal analysis. Focusing on the prospective research in this field, this article reports a comprehensive review of existing techniques that are obtained using search engines such as IEEE explore, Google scholar and science direct. Based on the review, the existing techniques are broadly categorized into two types: machine-learning and deep-learning-based techniques. In this study, it is noticed that the performance of the machine-learning-based arrhythmia detection techniques depend on pre-processing of ECG signal, R-peaks detection, features extraction and classification tools while the deep-learning-based techniques do not require the features extraction step. Generally, the existing techniques utilize Massachusetts Institute of Technology-Beth Israel Hospital arrhythmia database to evaluate the classification performance. The classification performance of automated techniques also depends on ECG data used for training and testing of the classifier. It is expected that the performance should be evaluated using a variety of ECG signals including the cases of inter-patient and intra-patient paradigm. The existing techniques also require to deal with the class-imbalance problem. In addition to this, a specific partition-ratio between training and testing datasets should be maintained for fair comparison of performance of different techniques.
