Abstract:Abstract Latina and Vietnamese women are disproportionately burdened by human papillomavirus (HPV)-associated cervical cancer and underutilize the HPV vaccine, which is an effective cancer prevention measure. To inform the adaptation of a National Cancer Institute’s evidence-based cancer control program, HPV Vaccine Decision Narratives, and because of the rapidly changing information concerning consumption patterns of young adults, we elicited preferences and characteristics associated with women’s interest in various health information sources, specifically for HPV vaccination. We conducted 50 interviews with young Latina and Vietnamese women at two Planned Parenthood health centers in Southern California. Interview questions were guided by the Channel Complementarity Theory and focused on understanding the multiple communication channels women turn to for health, and the characteristics that motivate their use. Our results showed that Latina and Vietnamese women turn to many sources, from online and social media to school health classes, mothers, and doctors. Specific characteristics that motivate women’s online use of health information included immediacy and access, convenience and credibility. When receiving HPV vaccine information, privacy, avoiding information overload, interpersonal engagement, and receiving health information from trusted sources was important. Our study advances the prevention literature by elevating the role of platform delivery considerations and emphasizing preferences to effectively reach Latina and Vietnamese women, who are disproportionately burdened by HPV cancers and are less aware of HPV vaccine prevention. These results can be used to further inform the dissemination of this cancer control program. Adaptation of the program should include changes to its delivery, such that trusted sources are used, and dissemination is coordinated to send a consistent message across multiple communication methods.
Applying Multiple Data Collection Tools to Quantify Human Papillomavirus Vaccine Communication on Twitter

Abstract:Background: Human papillomavirus (HPV) is the most common sexually transmitted infection in the United States. There are several vaccines that protect against strains of HPV most associated with cervical and other cancers. Thus, HPV vaccination has become an important component of adolescent preventive health care. As media evolves, more information about HPV vaccination is shifting to social media platforms such as Twitter. Health information consumed on social media may be especially influential for segments of society such as younger populations, as well as ethnic and racial minorities. Objective: The objectives of our study were to quantify HPV vaccine communication on Twitter, and to develop a novel methodology to improve the collection and analysis of Twitter data. Methods: We collected Twitter data using 10 keywords related to HPV vaccination from August 1, 2014 to July 31, 2015. Prospective data collection used the Twitter Search API and retrospective data collection used Twitter Firehose. Using a codebook to characterize tweet sentiment and content, we coded a subsample of tweets by hand to develop classification models to code the entire sample using machine learning procedures. We also documented the words in the 140-character tweet text most associated with each keyword. We used chi-square tests, analysis of variance, and nonparametric equality of medians to test for significant differences in tweet characteristic by sentiment. Results: A total of 193,379 English-language tweets were collected, classified, and analyzed. Associated words varied with each keyword, with more positive and preventive words associated with "HPV vaccine" and more negative words associated with name-brand vaccines. Positive sentiment was the largest type of sentiment in the sample, with 75,393 positive tweets (38.99% of the sample), followed by negative sentiment with 48,940 tweets (25.31% of the sample). Positive and neutral tweets constituted the largest percentage of tweets mentioning prevention or protection (20,425/75,393, 27.09% and 6477/25,110, 25.79%, respectively), compared with only 11.5% of negative tweets (5647/48,940; P<.001). Nearly one-half (22,726/48,940, 46.44%) of negative tweets mentioned side effects, compared with only 17.14% (12,921/75,393) of positive tweets and 15.08% of neutral tweets (3787/25,110; P<.001). Conclusions: Examining social media to detect health trends, as well as to communicate important health information, is a growing area of research in public health. Understanding the content and implications of conversations that form around HPV vaccination on social media can aid health organizations and health-focused Twitter users in creating a meaningful exchange of ideas and in having a significant impact on vaccine uptake. This area of research is inherently interdisciplinary, and this study supports this movement by applying public health, health communication, and data science approaches to extend methodologies across fields.
Characterizing HPV Vaccine Sentiments and Content on Instagram

Abstract:Background. With its growing popularity, inclusion of image and text, and user-friendly interface, Instagram is uniquely positioned for exploring health behaviors and sources and types of informational exposure related to the human papillomavirus (HPV) vaccine. Aims. To characterize public Instagram posts about the HPV vaccine and quantify the impact of sentiment and context on engagement via likes. Method. Using Netlytic, 3,378 publicly available English-language posts were collected using the search terms “#HPV,” “#HPVVaccine,” and “#Gardasil.” We randomly selected 1,200 posts to content analyze. Our final analytic sample included 360 posts after excluding posts whose links were no longer active ( n = 221) or that were not relevant ( n = 619). Results. A higher proportion of posts were pro-vaccine (55.8%) than anti-vaccine (42.2%). Pro-HPV vaccination posts were liked significantly less than anti-vaccination posts (24 vs. 86 likes; p < .001). More posts contained actionable information/resources (63.9%) than personal narrative elements (36.1%). Less than one in three posts (30.0%) came from health-related sources. Discussion. Pro-vaccine posts were more prevalent on Instagram, and anti-vaccine posts had higher engagement and typically included misleading information about the HPV vaccine. Personal narratives skewed toward anti-vaccine sentiments and most were produced by individual users. Pro-vaccine narratives portrayed individuals who received the vaccine, but provided limited details on vaccine experiences, starkly contrasting with the depth of details in anti-vaccine personal narrative posts. Conclusion. On Instagram, individuals and organizations have an opportunity to promote HPV vaccination by continuing to provide informational resources in addition to creating more narrative-style posts.

Designing and Implementing an Educational Social Media Campaign to Increase HPV Vaccine Awareness among Men on a Large College Campus

Abstract:Background: The human papillomavirus (HPV) causes an estimated 11,600 males to contract HPV-associated cancers in the U.S. every year. Purpose: We describe the generalized results of a social media marketing campaign. The aim was to identify predictors of self-reported HPV vaccine uptake and target these predictors in a health education campaign for males on a large college campus. Methods: We conducted an observational cohort study that recorded surveys from 90 college males then analyzed these results to inform the implementation of a campus-wide social media campaign. The survey had good internal consistency (alpha = 0.83) and assessed the Health Belief Model and Theory of Planned Behavior. Results: The campaign reached an estimated 2,200 students with in-person presentations, 12,000 students with two student newspaper articles, and over 17,000 views of its three-part social media video series, which was shared 197 times by students and organizations at the university. After the campaign, our cohort survey demonstrated three of five health beliefs targeted had significantly improved: perceived susceptibility (p < .01), perceived severity (p < .05), and self-efficacy (p < .01). Discussion: Our study reports the generalized findings of an evidence-based, reproducible approach to implementing an educational campaign on a college campus. Translation to Health: Education Practice: This model for social media marketing and student engagement can be replicated by other university health centers for education of males about vaccines and other chronic diseases.
Mining Twitter to Assess the Determinants of Health Behavior Toward Human Papillomavirus Vaccination in the United States.

Abstract:Objectives: The study sought to test the feasibility of using Twitter data to assess determinants of consumers' health behavior toward human papillomavirus (HPV) vaccination informed by the Integrated Behavior Model (IBM). Materials and Methods: We used 3 Twitter datasets spanning from 2014 to 2018. We preprocessed and geocoded the tweets, and then built a rule-based model that classified each tweet into either promotional information or consumers' discussions. We applied topic modeling to discover major themes and subsequently explored the associations between the topics learned from consumers' discussions and the responses of HPV-related questions in the Health Information National Trends Survey (HINTS). Results: We collected 2 846 495 tweets and analyzed 335 681 geocoded tweets. Through topic modeling, we identified 122 high-quality topics. The most discussed consumer topic is "cervical cancer screening"; while in promotional tweets, the most popular topic is to increase awareness of "HPV causes cancer." A total of 87 of the 122 topics are correlated between promotional information and consumers' discussions. Guided by IBM, we examined the alignment between our Twitter findings and the results obtained from HINTS. Thirty-five topics can be mapped to HINTS questions by keywords, 112 topics can be mapped to IBM constructs, and 45 topics have statistically significant correlations with HINTS responses in terms of geographic distributions. Conclusions: Mining Twitter to assess consumers' health behaviors can not only obtain results comparable to surveys, but also yield additional insights via a theory-driven approach. Limitations exist; nevertheless, these encouraging results impel us to develop innovative ways of leveraging social media in the changing health communication landscape.

Abstract:Background Despite the expansion in human papillomavirus (HPV) vaccination recommendations, vaccine uptake and completion remain low among Black individuals. An 8‐week Facebook intervention (#HPVvaxtalks) was developed to increase knowledge and awareness of HPV risk factors, risk perceptions, and vaccination intention and uptake. This article details the formative phase of experts' feedback and participants' satisfaction on the components of the #HPVvaxtalks for content useability, acceptability, relevance, and visual appeal to inform further refinements. Method A convenience sample of 5 experts and 13 young Black adults were invited to provide feedback on #HPVvaxtalks intervention content. The experts reviewed and provided feedback via a survey and open‐ended questions. After incorporating suggestions from experts' feedback, 13 participants viewed #HPVvaxtalks posts, took part in a virtual focus group, and completed a survey. Qualitative data were thematically analyzed and integrated with survey results. Results Experts and participants mean age were 41.9 ± 2.3 and 21.2 ± 1.9 years, respectively. Experts and participants reported positive ratings for intervention posts, including useability, acceptability, relevance, and visual appeal. Data provided areas for improvements of #HPVvaxtalks. Conclusion Experts indicated strong content validity and participants showed satisfaction with #HPVvaxtalks content. Ensuring acceptability, relevance, and appeal of the intervention for the target population is an integral part of intervention development.

Leveraging influencers to increase HPV vaccination intention: The impact of message framing and health regulatory fit using repeated measures

Abstract:Through a series of two online experiments that incorporate a repeated measures design, we examine how message framing (loss versus gain) within Instagram influencers' posts interact with consumers' health regulatory orientation (promotion versus prevention) to impact HPV vaccination intention between two data collection points. Findings indicate that among those who are more prevention oriented, exposure to a loss-framed influencer advertisement was effective at increasing intention to receive an HPV vaccine relative to those that were exposed to gain-framed influencer advertisement. Based on these findings we offer theoretical and managerial implications.

Abstract:Background The human papillomavirus (HPV) vaccine is a major advancement in cancer prevention and this primary prevention tool has the potential to reduce and eliminate HPV-associated cancers; however, the safety and efficacy of vaccines in general and the HPV vaccine specifically have come under attack, particularly through the spread of misinformation on social media. The popular social media platform Instagram represents a significant source of exposure to health (mis)information; 1 in 3 US adults use Instagram. Objective The objective of this analysis was to characterize pro- and anti-HPV vaccine networks on Instagram, and to describe misinformation within the anti-HPV vaccine network. Methods From April 2018 to December 2018, we collected publicly available English-language Instagram posts containing hashtags #HPV, #HPVVaccine, or #Gardasil using Netlytic software (n=16,607). We randomly selected 10% of the sample and content analyzed relevant posts (n=580) for text, image, and social media features as well as holistic attributes (eg, sentiments, personal stories). Among antivaccine posts, we organized elements of misinformation within four broad dimensions: 1) misinformation theoretical domains, 2) vaccine debate topics, 3) evidence base, and 4) health beliefs. We conducted univariate, bivariate, and network analyses on the subsample of posts to quantify the role and position of individual posts in the network. Results Compared to provaccine posts (324/580, 55.9%), antivaccine posts (256/580, 44.1%) were more likely to originate from individuals (64.1% antivaccine vs 25.0% provaccine; P<.001) and include personal narratives (37.1% vs 25.6%; P=.003). In the antivaccine network, core misinformation characteristics included mentioning #Gardasil, purporting to reveal a lie (ie, concealment), conspiracy theories, unsubstantiated claims, and risk of vaccine injury. Information/resource posts clustered around misinformation domains including falsification, nanopublications, and vaccine-preventable disease, whereas personal narrative posts clustered around different domains of misinformation, including concealment, injury, and conspiracy theories. The most liked post (6634 likes) in our full subsample was a positive personal narrative post, created by a non-health individual; the most liked post (5604 likes) in our antivaccine subsample was an informational post created by a health individual. Conclusions Identifying characteristics of misinformation related to HPV vaccine on social media will inform targeted interventions (eg, network opinion leaders) and help sow corrective information and stories tailored to different falsehoods.

Abstract:This article summarizes the findings from a systematic literature review to examine how social media may impact HPV vaccine uptake and HPV and HPV vaccine related awareness, knowledge, and attitudes. Study inclusion criteria was original data collection of at least one data point about social media and HPV and/or HPV vaccination, such that the study provided insight into how social media content may influence HPV and HPV vaccine related knowledge, attitudes, and/or behaviors. A total of 44 relevant articles were identified using the following databases: PubMed, PsycINFO, Communication Source, Sociological Abstracts, Business Source Elite, and the Educational Resources Information Center (ERIC). Most studies analyzed the valence, type, and frequency of social media content about HPV vaccination, and some found associations between potential exposure to negative, anti-vacc`11qine content and lower vaccination rates. Some studies that included primary human subject data collection found that engagement with HPV related social media content was associated with improved awareness and knowledge but not with increased vaccine uptake. The literature overall is lacking in systematic and rigorous research examining the effects of social media on HPV related knowledge, attitudes, and behaviors and needs further examination as social media increasingly becomes a source of health information.

Abstract:A report by the House of Commons Health Committee into the influence of the UK pharmaceutical industry has recommended that companies should publish the results of all their trials and follow guidelines for responsible publication practice.1 Cynics may argue that such reports rarely have much influence over powerful commercial companies and that governments usually ignore their recommendations unless they support existing policies. But this call for greater transparency and responsibility echoes similar calls which, taken together, form a crescendo that is hard to ignore. Earlier events that may have focused the minds of drug companies and made them more receptive to the latest recommendations include the fine of $240 million plus $152 million damages awarded against Pfizer/Warner-Lambert in May 2004 for promoting off-label use of its anticonvulsant gabapentin.2 Promotional activities criticized by the US court included having a publication strategy that encouraged the production and dissemination of anecdotal reports of dubious scientific value and the use of ghost-writers to prepare favourable articles which appeared under the names of opinion leaders. Less than a month later, the New York Attorney General accused GlaxoSmithKline of failing to publish safety data about its antidepressant paroxetine. This case was settled out of court but the company agreed to post results from all its clinical trials on its website.3 Several other companies have followed suit,4 and the Pharmaceutical Research Manufacturers of America (the US industry association) has set up a website for this purpose.5 Later in 2004, the International Committee of Medical Journal Editors announced that, from mid-2005, journals edited by the 12 committee members would consider papers for publication only if the trial had been registered.6 Other journals have now adopted similar policies.7 In the USA, registration of trials in serious and life-threatening conditions has been required by the Food and Drug Administration for a few years, but the system has not been policed particularly thoroughly and it is not clear what happens if companies do not comply or provide incomplete information. In contrast, the journal editors' announcement not only included a sanction that companies would respect (i.e. the threat of not being able to publish in several prestigious journals) but has also been followed by clarification about the type of information required and warnings that journals will check companies' compliance.8 Support for trial registration and complete publication is also found in the latest revision of the Declaration of Helsinki which calls for the design of studies to be made available prospectively and for both negative and positive results to be published.9 Picking up these themes, the House of Commons report presents cogent arguments why publication bias, and, in particular, the suppression of negative findings, is harmful and supports demands for trial registration. The question of ghost-writing has also received attention in both medical journals and newspapers for some time.10,11 Once again, the House of Commons Committee has added its voice to the debate by calling on companies to behave responsibly. The report refers to the guidelines issued recently by the European Medical Writers Association which seek to abolish ghost-writing by encouraging proper acknowledgement of medical writers' contributions to articles and their source of funding.12 One problem with the discussion over ghost-writing has been confusion over exactly what is meant by the term. In this respect, the House of Commons report is helpful in recognizing that it is acceptable for professional medical writers to help doctors write up their research but in condemning the practice of preparing articles with insufficient input from named authors and of covering-up the role of writers or funding companies. This distinction between the legitimate role of professional writers and authorship abuses by pharmaceutical companies has also been emphasized in a recent position statement from the World Association of Medical Editors (WAME).13,14 The statement recommends that journals should make clear in their information for authors that medical writers can be legitimate contributors but that their roles and affiliations should be acknowledged. The issues raised by the involvement of professional writers are also covered in the Good Publication Practice (GPP) guidelines which are strongly endorsed by the parliamentary Committee's report.15 These guidelines were developed in response to concerns that drug companies were skewing the medical literature by failing to publish unfavourable findings and by publishing positive results repeatedly. The GPP guidelines therefore call on companies to endeavour to publish results of all trials of marketed products. This suggestion is taken up by the report which calls on companies to make results of all trials available 'at launch as a condition of the marketing licence'. If the UK regulatory authorities act on this it might ensure the implementation of such codes which have, until now, been voluntary. However, European drug regulation is highly centralized, and it is unlikely that the UK would take unilateral action especially over a measure that might be resented by the industry. The Committee also suggests that research ethics committees should 'require applicants to prove' that a proposed trial 'does not duplicate previous research' and to ensure 'that results will be published in full'. However, similar ideas were proposed over a decade ago,16 and have been aired more recently,17 suggesting that ethics committees are unable or unwilling to take on this extra burden. While many of the recommendations relating to trial registration and publication simply echo or amplify statements from other groups, the Committee took a more radical approach to the drug licensing process. It was particularly critical of the Medicines and Healthcare products Regulatory Agency (MHRA—the UK drug licensing authority), calling for a 'fundamental review' in view of its (unspecified) 'failings'. The report urges the MHRA to publish material submitted by companies to support licensing applications. This would be an important change from the current situation where such information is considered confidential. However, the secrecy of drug licensing and the lack of information available to prescribers about new products has been criticized for many years by other commentators,18 so it will be interesting to see whether this House of Commons Committee wields more influence. Some of the reforms proposed by the Committee could constrain prescribing freedom, since it favours a system of limiting the prescribing of new products to hospital specialists and calls for increased monitoring of nurse and pharmacist prescribing. The report concludes that 'it is extraordinary that there are stricter controls on hospital specialists prescribing than on GPs' and recommends that 'tougher restrictions' should be placed 'on what non-specialists can prescribe' with 'greater vigilance to guard against excessive or inappropriate prescribing'. If these recommendations are followed, the UK might move to a system like that in the Republic of Ireland, where certain drugs may be prescribed only by specialists. I suspect that, if such prescribing restrictions are viewed as ways to reduce spending on new products (which tend to be costlier than old ones) the recommendations might win support from the Department of Health. However, they may be resisted by doctors' organizations. The report could also strengthen the role of the National Institute for Clinical Excellence (NICE) which would presumably be responsible for reassessing products after their probationary period and recommending whether their licenses should be renewed and prescribing restrictions lifted. This need for support in implementation underlies the inherent toothlessness of House of Commons Committees, whose recommendations tend to be adopted only if they find powerful champions. I therefore suspect that the Committee's calls for reforming the MHRA will be resisted unless a major drug safety scandal forces action. However, if the Department of Health and NICE back the Committee's proposals for restricting the prescribing of new drugs, this could come into force. In the case of the calls for increased transparency for clinical trial results and for trial registration, several powerful groups are already forcing companies in this direction—the report simply adds to the groundswell. Drug companies are increasingly realizing that irresponsible publication practices can generate negative publicity, and are therefore responding to pressure from journal editors and regulatory authorities to clean up their act. They are also starting to realize that lack of diligence over publications could affect their profits, which usually provides the most powerful motive for change and might mean that some of the Committee's recommendations are enacted. HPV vaccine misinformation on social media: A multi-method qualitative analysis of comments across three platforms

Abstract:Objective The purpose of this study was to characterize similarities and differences in HPV vaccine misinformation narratives present in the comment sections of top-performing initial creator posts across three social media platforms. Methods A qualitative multi-method design was used to analyze comments collected from social media posts. A sample of 2996 comments were used for thematic analysis (identifying similar themes) and content analysis (identifying differences in comment type, opinion, and misinformation status). Results Misinformation was pervasive in comment sections. Cross-cutting misinformation themes included adverse reactions, unnecessary vaccine, conspiracy theories, and mistrust of authority. The proportion of comments related to these themes varied by platform. Initial creator posts crafted to be perceived as educational or with an anti-vaccine opinion had a higher proportion of misinformation in the comment sections. Facebook had the highest proportion of misinformation comments. Conclusion Differences in the proportion of cross-cutting themes in the comment sections across platforms suggests the need for targeted communication strategies to counter misinformation narratives and support vaccine uptake. Innovation This study is innovative due to its characterization of misinformation themes across three social media platforms using multiple qualitative methods to assess similarities and differences and focusing on conversations occurring within the comment sections.

Abstract:The reported study compared the impact of four influence strategies (agency assignment, enhanced active choice, deviance regulation marking, and temporal framing) on English- and Spanish-speaking parents' reported intention to vaccinate their children for HPV. An online experiment was conducted to examine the impact of the strategies. In a fractional factorial design, participating parents (N = 1663) were exposed to combinations of influence strategies in text messages presented as reminders they might receive from a healthcare provider about their child's eligibility for the vaccine series. The results indicated small but significant impacts of agency assignment, enhanced active choice, and deviance regulation marking on parents' reported vaccination intentions. The study adds to the research literature on HPV vaccination communication in two important respects. First, it demonstrated how incorporating evidence-based influence strategies into reminder messages can increase parents' vaccination intentions, an important precursor and predictor of actual vaccine uptake. Second, it sets an important precedent by examining the effects of influence strategies on vaccination intentions across different languages.

Abstract:Introduction Human papillomavirus (HPV) vaccination rates are lower than other recommended adolescent vaccines. Cancer survivor narratives are used to promote cancer prevention and control, but little is known about their impact on adolescent HPV vaccination. Objective This pilot study explored the feasibility and effects of a video education intervention using a cancer survivor narrative to improve parents’ attitudes toward and intentions to get the HPV vaccine. Methods This study utilized a one-group design; participants completed a pre-intervention survey, watched the video before attending their sons’ wellness visits, and completed a post-intervention survey within one week of their appointment. Using the narrative persuasion framework, we developed a 4-minute video of a local HPV-related cancer survivor to promote the HPV vaccine as cancer prevention. We recruited 37 participants between June and October 2020. Participants were parents of males ages 9-17 who had not yet initiated HPV vaccination. Results After the video, more parents agreed that HPV vaccination is safe (pre: 66% vs. post: 82%; P = .045) and that their child’s chances of getting HPV-related cancer in the future are high (pre: 24% vs. post: 46%; P = .014). Overall, 91% of parents felt the cancer survivor story helped them understand the risks of HPV cancers, and 52% said the story influenced their decision to start HPV vaccination for their child. Conclusions Our findings suggest that cancer survivor narratives influence parents’ vaccine opinions and understanding of their child’s risk of HPV infection, leading to increased parental intent to get the HPV vaccine for their adolescent males.

Abstract:Objective To illustrate an evidence-, theory- and person-based approach to codesign the COMMUNICATE films that support parental decision-making about the human papillomavirus (HPV) vaccine for their teenagers. Design Codesign study. Setting Localities covered by two immunisation teams in London and the south-west of England. Methods The intervention planning phase involved combining evidence from a literature review with qualitative interview data to identify barriers and facilitators to HPV vaccine uptake, as well as design features that should be incorporated within the COMMUNICATE films. The intervention development phase involved identifying guiding principles for the COMMUNICATE films, mapping behaviour change techniques onto the behaviour change wheel and codesigning the COMMUNICATE films. Feedback from users informed modifications to maximise acceptability and feasibility and to support behaviour change. Results The primary and secondary evidence highlighted important content to include within the COMMUNICATE films: emphasise the benefits of the HPV vaccine, provide transparent information about the safety profile and side effects and emphasise the universality and commonality of HPV infection. A series of scripts were used to guide 4 film shoots to create the content in multiple community languages with 16 participants, including vaccine-hesitant, ethnically diverse parents and professionals. Overall, participants were positive about the films. Potential messengers and ways the films could be distributed, identified by parents, include local social media networks or text messages from general practices. The need for information about the HPV vaccine to be shared by schools ahead of consent being sought was also raised. Conclusions By using an integrated approach to intervention development, this study has begun to address the need for an intervention to support vaccine-hesitant, ethnically diverse parents' decision-making about the HPV vaccination programme. A future study to codesign, implement and evaluate a communication strategy for the COMMUNICATE films is planned.

Abstract:Instagram is one of the most widely used social-media platforms and is becoming a growing forum for discussing health-related topics (HIV, cancer, vaping, alcohol, self-harm behavior, etc.). However, also anti-vaccine activists often upload posts with hashtags displaying antivaccine intentions on social media. Considering that the most of Instagram users have an age stackable to the age indicated for Human Papillomavirus (HPV) vaccination, the aim of our review was to investigate Instagrammers' attitude toward HPV vaccination. In October–November 2020 we performed a search for original peer-reviewed papers in the electronic database PubMed (MEDLINE). The key search terms were "Instagram AND Vaccine", "Instagram AND HPV Vaccine", "Instagram AND HPV", "Instagram AND Papillomavirus". We searched for studies published without any temporal limit, reporting information useful to understand Instagrammers' attitudes or feelings about HPV vaccination. We considered eligible for the review articles reporting clear data on: i) number of analyzed post; ii) study period; iii) a deep and clear analysis of posts referred to HPV vaccine; iv) other interesting data related to HPV/HPV vaccine. Descriptive studies written in English, French, Spanish, Italian have been considered eligible. The literature search yielded 21 publications but 2 manuscripts met the inclusion criteria. The studies involved 510 Instagram posts with a mean number of likes ranging from 27.7 to 50. The most of them were PRO-vaccine (up to 55.8%); nevertheless, ANTI-vaccine posts had significantly more likes than PRO-vaccine posts (all p < .05). Anti-vaccine posts were less likely to mention prevention and cancer, compared to pro-vaccine posts. Posts with only female figures tended to be pro-vaccine. Concluding, the importance of Instagram as a new source of information for patients should not be underestimated. Healthcare professionals should be aware of this tool and should intervene in order to ensure a proper and correct information.
User-Centered Design for Developing Interventions to Improve Clinician Recommendation of Human Papillomavirus Vaccination

Abstract:Introduction: Human papillomavirus (HPV) is the most common sexually transmitted infection in the US and is associated with multiple types of cancer. Although effective HPV vaccines have been available since 2006, coverage rates in the US remain much lower than with other adolescent vaccinations. Prior research has shown that a strong recommendation from a clinician is a critical determinant in HPV vaccine uptake and coverage. However, few published studies to date have specifically addressed the issue of helping clinicians communicate more effectively with their patients about the HPV vaccine. Objective: To develop one or more novel interventions for helping clinicians make strong and effective recommendations for HPV vaccination. Methods: Using principles of user-centered design, we conducted qualitative interviews, interviews with persons from analogous industries, and a data synthesis workshop with multiple stakeholders. Results: Five potential intervention strategies targeted at health care clinicians, youth, and their parents were developed. The two most popular choices to pursue were a values-based communication strategy and a puberty education workbook. Conclusion: User-centered design is a useful strategy for developing potential interventions to improve the rate and success of clinicians recommending the HPV vaccine. Further research is needed to test the effectiveness and acceptability of these interventions in clinical settings.
Digital Storytelling Intervention to Promote Human Papillomavirus Vaccination Among At-Risk Asian Immigrant Populations: Pilot Intervention Study

Abstract:Background: The high morbidity, mortality, and economic burden attributed to cancer-causing human papillomavirus (HPV) calls for researchers to address this public health concern through HPV vaccination. Despite disparities in HPV-associated cancers in Korean Americans and Vietnamese Americans, their vaccination rates remain low. Evidence points to the importance of developing culturally and linguistically congruent interventions to improve HPV vaccination rates. Digital storytelling (a specific form of cultural narrative) shows promise as an effective culture-centric health promotion strategy. Objective: The aim of this quasi-experimental single-group study was to assess the feasibility, acceptability, and preliminary effects of a culturally and linguistically congruent digital storytelling intervention on Korean American and Vietnamese American mothers' attitudes and intention in vaccinating their children against HPV. We also examined if the association between attitudes and intention differed by their child's sex (boy vs girl) and by ethnicity (Korean American vs Vietnamese American). Methods: Participants were recruited via multiple avenues (eg, ethnic minority community organizations, social media, and flyers posted in local Asian supermarkets and nail salons). Web-based, valid, and reliable measures were administered to collect data preintervention and postintervention. Descriptive statistics, paired and independent sample t tests, the chi-square test, and the McNemar test were used to describe the distributions of variables and to examine the differences between subgroups and changes in key variables over time. Logistic regression models were used to examine associations of mothers' HPV- and vaccine-related attitudes with vaccination intention and to explore if the association between attitudes and vaccination intention differed by the target children's sex or ethnicity. Results: In our sample of 50 Korean American mothers (mean age 42.8, SD 4.8 years) and 114 Vietnamese American mothers (mean age 41.5, SD 5.4 years), 36% (18/50) of Korean American and 51% (58/114) of Vietnamese American mothers reported that their children received free or reduced-price lunches at school. After the intervention, mothers' attitudes toward HPV and the vaccine (t163=2.49, P=.01) and intention to vaccinate their children improved significantly (X21=18.38, P<.001). The measure of mothers' negative attitudes toward HPV and the vaccine was significantly associated with higher vaccination intention (odds ratio 0.27, 95% CI 0.14-0.51; P<.001), adjusting for background variables (sociodemographic characteristics) and other HPV-related variables (family cancer history, prior HPV education, and HPV communication with health care providers). Findings did not suggest that a child's sex or ethnicity moderated the association between attitudes and vaccination intention. Conclusions: This remotely delivered intervention using digital stories was feasible and acceptable, and showed preliminary effects on promoting Korean American and Vietnamese American mothers' intention to vaccinate their children against HPV. Future research that uses a randomized controlled trial design with a larger and more diverse sample and includes children's vaccination status will help understand the effect of the intervention.

Abstract:Background: Human papillomavirus (HPV) can cause several types of cancers and genital warts. A vaccine is available to prevent HPV infections, and several efforts have been made to increase HPV education and, eventually, vaccination. Although previous studies have focused on the development of messages to educate children about HPV and the existence of the HPV vaccine, limited research is available on how to help children better communicate with their parents and health care professionals about the HPV vaccination. In addition, limited research is available on the target audience of this study (Italian children). Objective: This manuscript describes a study assessing the feasibility of using an evidence-based animated video and a web-based game to help children (aged 11-12 years) participate in discussions about their health-in particular when such conversations center around the HPV vaccination-and improve several HPV-related outcomes. The study also compares the effects of these 2 educational multimedia materials on children's knowledge and perceptions of HPV prevention. Methods: A mixed methods approach consisting of focus group discussions and an experiment with children (N=35) was used to understand children's experiences with, and perceptions of, the animated video and the game and to measure possible improvements resulting from their interaction with these materials. Results: Both the animated video and a web-based game increased children's knowledge and positive perceptions about HPV and HPV vaccination. Any single message was not more effective than the others. The children discussed aspects of the features and characters they liked and those that need improvements. Conclusions: This study shows that both materials were effective for improving children's education about the HPV vaccine and for helping them to feel more comfortable and willing to communicate with their parents and health care professionals about their health. Several elements emerged that will allow further improvements in the design and development of the messages used in this study as well as the creation of future campaigns.
Use of Deep Learning to Analyze Social Media Discussions About the Human Papillomavirus Vaccine

Abstract:Importance: Human papillomavirus (HPV) vaccine hesitancy or refusal is common among parents of adolescents. An understanding of public perceptions from the perspective of behavior change theories can facilitate effective and targeted vaccine promotion strategies. Objective: To develop and validate deep learning models for understanding public perceptions of HPV vaccines from the perspective of behavior change theories using data from social media. Design, setting, and participants: This retrospective cohort study, conducted from April to August 2019, included longitudinal and geographic analyses of public perceptions regarding HPV vaccines, using sampled HPV vaccine-related Twitter discussions collected from January 2014 to October 2018. Main outcomes and measures: The prevalence of social media discussions related to the construct of health belief model (HBM) and theory of planned behavior (TPB), categorized by deep learning algorithms. Locally estimated scatterplot smoothing (LOESS) revealed trends of constructs. Social media users' US state-level home location information was extracted from their profiles, and geographic analyses were performed to identify the clustering of public perceptions of the HPV vaccine. Results: A total of 1 431 463 English-language posts from 486 116 unique usernames were collected. Deep learning algorithms achieved F-1 scores ranging from 0.6805 (95% CI, 0.6516-0.7094) to 0.9421 (95% CI, 0.9380-0.9462) in mapping discussions to the constructs of behavior change theories. LOESS revealed trends in constructs; for example, prevalence of perceived barriers, a construct of HBM, deceased from its apex in July 2015 (56.2%) to its lowest prevalence in October 2018 (28.4%; difference, 27.8%; P < .001); Positive attitudes toward the HPV vaccine, a construct of TPB, increased from early 2017 (30.7%) to 41.9% at the end of the study (difference, 11.2%; P < .001), while negative attitudes decreased from 42.3% to 31.3% (difference, 11.0%; P < .001) during the same period. Interstate variations in public perceptions of the HPV vaccine were also identified; for example, the states of Ohio and Maine showed a relatively high prevalence of perceived barriers (11 531 of 17 106 [67.4%] and 1157 of 1684 [68.7%]) and negative attitudes (9655 of 17 197 [56.1%] and 1080 of 1793 [60.2%]). Conclusions and relevance: This cohort study provided a good understanding of public perceptions on social media and evolving trends in terms of multiple dimensions. The interstate variations of public perceptions could be associated with the rise of local antivaccine sentiment. The methods described in this study represent an early contribution to using existing empirically and theoretically based frameworks that describe human decision-making in conjunction with more intelligent deep learning algorithms. Furthermore, these data demonstrate the ability to collect large-scale HPV vaccine perception and intention data that can inform public health communication and education programs designed to improve immunization rates at the community, state, or even national level.
