Abstract:Objective To explore the clinical effects of internal fixation with plate by the improved anterolateral approach to treatment of terminal femoral fractures.Methods Internal fixation with plate by improved anterolateral approach was applied to 78 patients with complex comminuted bone fractures of terminal femora from August 2008 to January 2012.Results During the follow-up of 10 months to 2 years,73 patients reached complete healing without screw relaxation,plate rupture and malformation(e.g.limb shortening and knee varus or eversion);the other 5 ones who suffered from nonunion reached complete healing by the second stage bone grafting.The excellent and good rate of knee joint function was up to 91 %.Conclusion With simple procedure,reliable fixation,integral protection of knee extensor device,mini-operative damage and propitiousness to functional recovery of knee joint,the internal fixation with plate by the improved anterolateral approach is a relatively good therapeutic approach to the complex comminuted bone fractures of terminal femora.
Finite Element Analysis of Three Internal Fixations for the Posteromedial Split Fracture Fragment in Tibial Plateau Fractures

Abstract:Posteromedial tibial plateau fracture is one of the most challenging traumatic fractures. We aimed to compare and explain the biomechanical advantages and disadvantages of different internal fixation methods for the treatment of the posteromedial split fracture fragment in tibial plateau fractures. Finite element models of the tibial plateau fracture were constructed. Three different internal fixations were developed to treat the posteromedial split fracture fragment in tibial plateau fractures finite element models: (a) the novel anatomic locking plate fixation model, (b) the common anatomic locking plate fixation model, and (c) the reconstruction plate fixation model. We applied the same loading conditions to each model: 320 N axial compression. Under the same condition, the von Mises stress (VMS) and displacement distribution of the three internal fixations and tibia plateau were studied. The stress values of the novel anatomic locking plate are lower than the common anatomic locking plate and the reconstruction plate. Additionally, the novel anatomic locking plate fixation system exhibits smaller maximum displacement. In conclusion, our study indicated that the novel anatomic locking plate resulted in a lower stress distribution in the plate and screws, and better stability than the common anatomic locking plate and the reconstruction plate for the posteromedial split fracture fragment in tibial plateau fractures fixation under the same loading conditions. Thus, for the posteromedial split fracture fragment in tibial plateau fractures, the use of the novel anatomic locking plate internal fixation is recommended.

Abstract:There is no consensus about the optimal internal fixation selection for treatment of posterolateral tibial plateau fracture. This study described a novel plate through an anterolateral approach for posterolateral tibial plateau fractures (PTPFs). We evaluated the biomechanical performance of a novel plate and two conventional internal implants and investigated the anatomic feasibility of the novel plate. The fracture models were randomly assigned into six groups: Groups A–C were the model groups of posterolateral split fracture, fixed with the posterior buttress plate, the lateral locking plate, and the novel plate, respectively. Groups D–E were the model groups of posterolateral depression fracture, fixed with the posterior buttress plate, the lateral locking plate, and the novel plate, respectively. We evaluated the biomechanical performance of six model groups by the biomechanical testing and finite element analysis. Progressively increasing axial compressive loads were applied to each synthetic fracture model by using a customized indentor under 250–750 N loads. Meanwhile, we dissected 12 fresh frozen knee specimens and fixed them with the novel plate through the anterolateral approach. We recorded the adjacency of the novel plate to important anatomic structures. Biomechanical testing showed that the novel plate had the least displacement, followed by the posterior buttress plate, and the lateral plate had the most displacement in posterolateral split fracture. There was no significant difference in the displacement between the novel plate and the lateral plate at different loads in posterolateral depression fractures. And the posterior buttress plate showed the most displacement. In the finite element analysis, the maximum stress values of Groups A, B, and C were 383.76, 414.63, and 305.07 MPa under the load of 750 N, respectively. The maximum stress values of Groups D, E, and F were 474.28, 436.31, and 413.4 MPa under the load of 750 N, respectively. In the anatomic study, the placement of the novel plate had a low risk of damage to the important anatomic structures of knee posterolateral corner. The novel plate could be a great choice for the treatment of PTPFs due to better biomechanical performance and easy manipulation.

Abstract:Abstract Background: Dealing with high-energy fractures of the tibial plateau remains a challenge despite advances in implants, surgical approaches, and imaging methods. Posterior buttress plate is most commonly used implant but the fixation stability is still a challenge. Recently, a newly designed tibial locking plate was introduced that aims to provide better fixation strength for tibial plateau split fracture. This study compared the biomechanical strength of three different posteromedial tibial plateau split fracture fixation methods. Methods: The tibial plateau fractures were simulated using a human tibiae model. Each fracture model was virtually implanted with one of the three following constructs, proximal medial tibial plate (PMT), proximal posterior medial tibial plate (PPMT), and posterior T-shaped buttress plate (TBP). Posteromedial fragment vertical subsidence was measured under 2000 N joint contact force. The maximum Equivalent stress on the bone plate and bone screw and the construct stiffness were determined. Results: The proximal medial tibial plate (PMT) allowed the least posteromedial fragment subsidence and produced higher construct stiffness than each of the other two constructs. However, the proximal posterior medial tibial plate (PPMT) showed higher stiffness than the T-shaped buttress plate (TBP). The maximum Equivalent stress was the smallest for the proximal medial tibial plate (PMT). Conclusion: This study showed that the proximal medial tibial locking plate or proximal posterior medial tibial locking plate were biomechanically more stable fixation methods for posteromedial split tibial plateau fractures.

Abstract:Objective: The treatment for posterolateral tibial plateau fractures (PTPF) have been subjects of controversy. We conducted a study to improve the fixation of PTPF through a lateral approach. Methods: We utilized 40 synthetic tibias and categorized the fracture models into five groups based on the locking compression plate (LCP) and T-distal radius plate (TPP) via various forms of fixation with screws through the posterolateral (PL) fracture fragments. I: Two-screw fixation using two locking screws (LPTL). II: Two-screw fixation with both variable angle locking screws (LPTV). III: One-screw fixation with one locking screw (LPOL). IV: One-screw fixation with one locking screw and two anteroposterior lag screws (LPOLTL). V: a distal radius plate with three locking screws (TPP). Biomechanical tests were conducted to observe the axial compression displacement of the PL fracture fragments at force levels of 250 N, 500 N, and 750 N, as well as to determine the failure load and the axial stiffness for each respective group. Results: Under a 750 N load condition, the displacements within the five experimental groups exhibited the following trend: V < II < I< IV < III. However, there were no significant differences between Group V and Group II, Group I and Group IV ( p > 0.05), and only Group III demonstrated a displacement exceeding 3 mm. The failure load and the axial stiffness exhibited the same trend. Conversely, statistical significance was identified among the remaining group compared with Group III ( p < 0.05). Regarding the finite element analysis, the maximum displacements for the five models under the load of 750 N exhibited the following trend: V < II < I< IV < III. The following trends were observed in maximum von Mises stresses for these models under the load of 750 N: V < II < IV< I < III. Conclusion: It is crucial to address the inadequate mechanical strength associated with single screw fixation of LCP for fixing PL fractures in a clinical setting. The biomechanical strength of two-screw fixation surpasses that of single-screw fixation. Introducing variable-angle screws can further enhance the fixation range. Furthermore, the addition of two lag screws threaded from anterior to posterior can compensate the mechanical stability, when PL fracture is fixed with single screw in clinic.

Abstract:Objective: This study aims to analyze the biomechanical characteristics of posterolateral plateau fractures fixed by a novel anatomical plate using finite element analysis. Methods: A three-dimensional digital model of the full length of right tibiofibula was obtained by CT scanning. A posterolateral tibial plateau fracture model was then created. The acquired fracture model was assembled with 4 groups of internal fixations: Group A, novel anatomical plate; Group B, straight buttress plate; Group C, oblique T-shaped locking plate; Group D, two lag screws. Axial loads of 500, 1,000 and 1,500 N perpendicular to the horizontal plane were used to simulate the stress on the lateral plateau of a 65 kg person standing, walking and fast running. Results: Vertical displacements of the posterolateral fragments in each of the four groups gradually increased under loads from 500 N to 1,500 N. The maximum displacement of the fracture fragment in four groups were all located on the lateral side of the proximal part, and the displacement gradually decreased from the proximal part to the distal end. The maximum displacement values under the axial load of 1,500 N was in the following order: novel anatomical plate (1.2365 mm) < oblique T-shaped locking plate (1.314 mm) < two lag screws (1.3747 mm) < straight buttress plate (1.3932 mm). As the axial load increased, the stress value of the different internal fixation models gradually increased. The stress behavior of the same internal fixation model under different loads was similar. The maximum stress value under the axial load of 1,500 N was in the following order: novel anatomical plate (114.63 MPa) < oblique T-shaped locking plate (277.17 MPa) < two lag screws (236.75 MPa) < straight buttress plate (136.2 MPa). Conclusion: The patients with posterolateral plateau fractures fixed with a novel anatomical plate in standing, walking and fast running can achieve satisfactory biomechanical results, which lays the foundation for future applications. At the same time, clinical fracture types are often diverse and accompanied by damage to the soft tissue. Therefore, the ideal surgical approach and appropriate internal fixation must be selected based on the patient's injury condition.

Abstract:The posterolateral tibial plateau fracture is a special type of intra-articular fracture, for which there is no simple, safe, and effective standardized procedure. In this paper, we evaluate the clinical efficacy and the advantages of the treatment of posterolateral tibial plateau fracture by using our designed proximal lateral tibial rim plate for the posterolateral condyle of the tibial plateau via the space above the fibula head. Thirty-eight patients with posterolateral tibial plateau fractures from June 2018 to June 2021 were retrospectively analyzed. CT scans were used to classify the degree of injury in the included patients. All of them were fixed with reduction using an approach above the fibula head combined with a homemade anatomical plate. The regular postoperative review was performed to instruct functional knee exercises. Postoperative complications were observed and follow-up visits were performed to assess the functional outcome. A total of 38 patients with posterolateral tibial plateau fractures, 13 males and 25 females were included in the study. All patients were followed up for 13–26 months, with a mean of 15.3 months. There were no postoperative complications such as numbness of the limb, knee joint instability, etc. X-ray review showed that the fractures were all healed, and the healing time was 10–16 weeks, with an average of 12.1 weeks; none of the internal fixation loosening and loss of articular surface occurred during the follow-up period. At the last follow-up, according to the HSS knee function score criteria, the scores were 79–98, with an average of 91.3. The HSS score presented excellent in 34 cases (89%) and good in 4 cases (11%). The Rasmussen score was graded as excellent in 29 cases (76%) and good in 9 cases (24%). In conclusion, The treatment of posterolateral tibial plateau fractures by an approach above the fibula head has the advantages of simplicity and safety, small trauma, and no risk of vascular and nerve injuries, and the anatomical proximal lateral tibial rim plate can play a direct and effective supporting role for the bone fragments of the posterolateral condyle, and the combination of both of them has obvious advantages in the treatment of posterolateral condylar fracture of the tibial plateau, and it is a method worth borrowing and popularizing.

Biomechanical evaluation of different fixation methods for fracture dislocation involving the proximal tibia

Abstract:Background: Fracture dislocations of the knee involving the proximal tibia are difficult to treat because of the operative approach that maybe required and the instability of the construct. The optimal fixation method of these fractures remains controversial, improper fixations usually lead to an unsatisfactory prognosis. The purpose of this study was to compare the mechanical strength of four different fixation constructs for this kind of fracture. Methods: Twenty-eight Synbone simulated fracture models were created, reduced, and instrumented with one of four constructs: medial single plate, medial double plates, bilateral double plates, or lateral locked plate. Biomechanical testing was done to determine the postfixation construct stiffness, the maximum load to failure, and the medial condylar displacement for each of the four constructs. Findings: The medial double plates construct was shown to provide the highest overall construct stiffness and the least medial condylar displacement than each of the other three constructs (both P=0.000); The lateral locked plate construct was shown to provide the lowest overall construct stiffness and the largest medial condylar displacement than each of the other three constructs (both P=0.000); There was no significant difference measured between the medial single plate construct and the bilateral double plates construct for overall construct stiffness and medial condylar displacement. Interpretation: This study suggests that the medial double plates technique is biomechanically the best fixation method for fracture dislocations involving the proximal tibia, while lateral locked plate technique is not indicated to this kind of fracture due to its insufficient stability.
Extended Anterolateral Approach for Thetreatment of Tibial Plateau Fractures with Posterolateral Bone Fragment

Abstract:Objective To investigate the differences and clinical effects of extended anterolateral approach in the treatment of simple/complex tibial plateau fractures with posterolateral bone fragment.Methods From January 2013 to December 2015,forty-six patients with posterolateral tibial plateau fractures treated in our hospital were included in our research.The posterolateral plateau fractures were reduced and fixed by the extended anterolateral approach.According to Schatzker fracture classification,patients with type Ⅱ fractures were treated as simple group and patients with type Ⅴ and type Ⅵ fractures were treated as complex group.According to tibial plateau three column classification,patients were divided into single column group,double column group and three column group.The operative time,bleeding volume,fracture healing time,postoperative Rasmussen score,knee mobility,reduction and fixation of fractures and postoperative complications were compared among the groups.Results All 46 patients were followed up for 23-45 months,with an average of 31.9 months.There were 24 cases in simple fracture group and 22 cases in complex fracture group.The mean operation time of simple group was 124±33.8 min,and that of complex group was 175±65.5 min,with significant difference (t=2.302,P=0.025);the mean bleeding volume of simple group was 118±93.5 ml,and that of complex group was 190± 149.4 ml,with significant difference (t=1.905,P=0.028).12 cases were treated with arthroscopy.Bone union was achieved in all patients at the last follow-up,and daily activities were not limited.The average healing time was 4.58 months in simple group and 5.54 months in complex group.The excellent rate of fracture reduction was 83.3％ in simple group and 27.3％ in complex group (x2=14.679,P=0.000).Posterolateral bone fragment was not completely fixed in 4 cases (16.7％) in simple group and 8 cases (36.4％) in complex group.The average Rasmussen function score was 26.8±2.1 and the range of motion of knee joint was 100°-120° (average 115.5°±6.2°) in simple group;The average Rasmussen function score was 23.5±3.4 points and the range of motion of knee joint was 95°-115° (average 106.3°±7.4°) in complex group,neither with no significant differences.According to the three-column fracture classification system,there were 6 cases in the single-column group,24 cases in the double-column group and 16 cases in the three-column group.There were significant differences in the operative time (F=5.039,P=0.000),bleeding volume (F=5.215,P=0.000) and the excellent rate of fracture reduction (x2=7.003,P=0.030) between these three groups.But there was no significant difference in the time of fracture healing,un-fixation rate of posterolateral bone fragment,knee joint Rasmussen score and knee jointactivity.The excellent rate of fracture reduction was 83.3％ in arthroscopy assisted cases and 47.1％ in arthroscopy un-assisted cases respectively (x2=4.749,P=0.002).Postoperative wound infection occurred in 1 patient.Conclusion Extended anterolateral approach could provide good reduction and fixation of tibial plateau fractures with posterolateral bone fragment with satisfactory clinical outcomes obtained in both simple and complex fractures.The operation was relatively simple and posterolateral bone fracture could get better reduction and fixation in simple tibial plateau fractures,and arthroscopy treatment was helpful to improve fracture reduction.

Abstract:Newborn mouse skin and human preputial skin were incubated for varying time periods in different media containing 1-C 14 -acetate. Fatty acids were isolated by conventional chromatographic procedures and their radioactivity monitored. Complete tissue culture medium and serum or an oxygen atmosphere enhanced acetate uptake compared to that in phosphate buffer. This increase affected saturated acids more than unsaturated. Insulin increased incorporation, especially into PUFA, but prostaglandin E 2 exerted the opposite effect in some experiments. Incorporation into all acids increased with time, although there was a tendency to fall off or even plateau except in the case of polyunsaturated (PUFA) fractions. Individual PUFA, including linoleic and linolenic acids, showed a linear increase at widely different rates. The labeling rate of arachidonic acid tended to increase after about five hours.

Abstract:Abstract Background There has been controversial for the treatment of the posterolateral tibial plateau fractures (PTPF). This study aimed to evaluate clinic outcomes of the lateral locking compression plate (LCP) postposition, analyze the feasibility of LCP postposition through anatomical measurement, and address the potential problems of LCP postposition through the biomechanical assessment. Methods 39 patients with PTPF undergoing LCP fixation between June 2019 and June 2022 were retrospectively evaluated. All cases were divided into two group: Group A (15 cases) employed plate transverse arm postpositioning with posterolateral (PL) fracture fixation using two raft screws, while Group B (24 cases) utilized non-postpositioning with fixation by a single raft screw. Surgical duration, intraoperative blood loss, the change of lateral tibial plateau angle (LTPA), lateral tibial plateau posterior slope angle (LPSA) and fracture collapse between immediate postoperative and last follow up, range of motion (ROM), HSS knee score, and Lysholm knee score were recorded. CT measurements of the fibular head superior space and LCP transverse arm were taken in 50 healthy adult knees to assess postposition feasibility. Finally, three fracture models were established using finite element analysis: Model A with plate postposition and PL split fracture fixed by two raft screws of transverse arm, Model B with plate non-postposition and PL split fracture fixed by one raft screw, and Model C with plate non-postposition and PL split fracture fixed by one raft screw and anterior–posterior tension screws. Loadings of 250N, 500N, and 750N were applied for the analysis of the displacement degree, von Mises stress distribution. Results Results indicate comparable operative duration and intraoperative hemorrhage between groups. Complications were minimal in both groups. Group A demonstrated superior outcomes in terms of radiographic parameters, functional scores, and fracture collapse prevention. CT measurements revealed compatibility in 72% of healthy knees with the postpositioning technique. Finite element analysis indicated favorable biomechanical stability. Conclusion Not all patients with PTPF were applicable to the management of the plate postposition and two raft screws fixation, even though this technique exerted good biomechanical stability and achieved satisfactory clinic outcomes. When the PL fracture was fixed by only raft screw through LCP owing to various reasons, two anterior–posterior tension screws might be necessitated to maintain the fracture stability.

Abstract:Purpose: The extra-articular proximal tibial fractures continue to have high malunion rates despite development in intramedullary nailing (IMN) technology. Combined plate and IMN fixation can increase mechanical stability. The purpose of this study was to investigate combined plate and IMN for the treatment of extra-articular proximal tibial fracture using a biomechanical model. Methods: A 10-mm defective osteotomy was created in the fourth-generation composite tibia to simulate extra-articular proximal tibial fractures (AO/OTA 41A2). The fractures were stabilized with IMN alone (IMN group), IMN with supplementary medial plate (M-IMN group), and IMN with supplementary lateral plate (L-IMN group). The biomechanical properties of each specimen were tested under axial compression loading, bending stress, and cyclic loading. The maximum displacement of the fragments and implant-bone construct failure was recorded. Results: The maximum displacement of the M-IMN group was significantly less than either the L-IMN or IMN group in both axial compression loading and bending stress ( p < 0.05 for both comparisons). All specimens in the three groups survived in 10,000 cyclic loading without hardware deformation. The maximum stiffness of failure was similar between the M-IMN and L-IMN groups, but the IMN group was statistically lower than either the L-IMN or the IMN group ( p < 0.05). Conclusion: The results indicated that combined medial plate and IMN fixation could effectively increase the mechanical stability of proximal tibial fractures.

Abstract:The selection of internal fixation as the primary fixation modality for the patient is one of the challenges for the surgeon treating the patient in question. A model of the lateral tibial plateau fracture was established. Three different configurations of internal fixators namely L bone plate, T bone plate, and screw-washer were analyzed. Three stages after surgery were simulated to assess the displacement of bone plates, screws, washers, and the stress shielding ratio in the fracture area.At three stages after surgery, the T bone plate showed better stability for patients during rehabilitation compared with the remaining two schemes, and the screw-washer scheme was the least stable due to the larger internal fixation displacement and stress shielding ratio in the fracture area. In contrast, the L bone plate scheme showed better stability in the early stages after surgery but was second only to the screw-washer scheme in the middle and late stages after surgery. The T bone plate showed better stability and became a new selection for surgeons to treat related patients. At three stages after surgery, the T bone plate has better biomechanical stability compared to the L bone plate and screw-washer schemes.

Abstract:Abstract Different fixation modalities are available for fixation of posterior malleolar fractures (PMFs), but the best method is still unclear. The purpose of this study was to carry out a comparative biomechanical analysis of three commonly used fixation constructs for PMFs using experimental and finite element analysis (FEA). 15 human cadaveric ankle specimens were randomly divided into three groups. Specimens in group-A were fixed with two anteroposterior (AP) lag screws, group-B with two posteroanterior (PA) lag screws, and for group-C, a posterior plate was used. Each model was subjected to axial load. Outcomes included loads for 0.5 mm, 1 mm, 1.5 mm, and 2 mm vertical displacements of posterior fragments were noted. 3D FE models were reconstructed from computed tomography (CT) images and subjected to vertical loads. The model’s stress, fracture step-off, and resultant strains in implants were also studied in 3D FE models. Significantly higher amounts of mean compressive loads were observed to cause the same amount of vertical displacements in plate group (265 ± 60.21 N, 796 ± 57.27 N, 901.18 ± 8.88 N, 977.26 ± 13.04 N) than AP (102.7 ± 16.78 N, 169.5 ± 19.91 N, 225.32 ± 15.92 N, 269.32 ± 17.29 N) and PA (199.88 ± 31.43 N, 362.80 ± 28.46 N, 431.3 ± 28.12 N, 541.86 ± 36.05 N) lag screws respectively (P < 0.05). Simulated micro-motion analysis demonstrated that fracture step-off values in plate group (0.03 ± 0.001 mm, 0.06 ± 0.003 mm and 0.13 ± 0.010 mm) were the lowest among the three groups (P < 0.001). The cancellous bone showed the highest amount of stress in AP and PA lag groups respectively, whereas the lowest stress was noted in the plate-group. This biomechanical study concluded that posterior plating is biomechanically the most stable fixation construct for PMFs fixation. AP and PA lag screws with higher bone stress and fracture step-off values have a high tendency of bone cut-through and loss of fixation respectively.

Abstract:BackgroundProximal tibia fractures are one of the most familiar fractures. Surgical approaches are usually needed for anatomical reduction. However, no single treatment method has been widely established as the standard care. Our present study aims to compare the stress and stability of intramedullary nails (IMN) fixation and double locking plate (DLP) fixation in the treatment of extra-articular proximal tibial fractures.MethodsA three-dimensional (3D) finite element model of the extra-articular proximal tibial fracture, whose 2-cm bone gap began 7 cm from the tibial plateau articular surface, was created fixed by different fixation implants. The axial compressive load on an adult knee during single-limb stance was imitated by an axial force of 2500 N with a distribution of 60% to the medial compartment, while the distal end was fixed effectively. The equivalent von Mises stress and displacement of the model was used as the output measures for analysis.ResultsThe maximal equivalent von Mises stress value of the system in the IMN model was 293.23 MPa, which was higher comparing against that in the DLP fixation model (147.04 MPa). And the mean stress of the model in the IMN model (9.25 MPa) was higher than that of the DLP fixation system in terms of equivalent von Mises stress (EVMS) (P < 0.0001). The maximal value of displacement (sum) in the IMN system was 8.82 mm, which was lower than that in the DLP fixation system (9.48 mm).ConclusionsThis study demonstrated that the stability provided by the locking plate fixation system was superior to the intramedullary nails fixation system and served as an alternative fixation for the extra-articular proximal tibial fractures of young patients.

Abstract:Purpose: Proximal tibial fractures are common, but the current available internal fixation strategies remain debatable, especially for comminuted fractures. This study aimed to compare the biomechanical stability of three internal fixation strategies for extra-articular comminuted proximal tibial fractures. Methods: A total of 90 synthetic tibiae models of simulated proximal tibial fractures with segmental bone defects were randomly divided into three groups: Single lateral plating (LP), double plating (DP) and intramedullary nailing (IN). Based on the different number of fixed screws, the above three groups were further divided into nine subgroups and subjected to axial compression, cyclic loading and static torsional testing. Results: The subgroup of intramedullary nailing with five proximal interlocking screws showed the highest axial stiffness of 384.36 ± 35.00 N/mm. The LP group obtained the lowest axial stiffness performance with a value of 96.59 ± 16.14 N/mm. As expected, the DP group offered significantly greater biomechanical stability than the LP group, with mean static axial stiffness and mean torque increasing by approximately 200% and 50%, respectively. According to static torsional experiments, the maximum torque of the DP subgroup was 3,308.32 ± 286.21 N mm, which outperformed all other groups in terms of torsional characteristics. Conclusion: Utilizing more than four distal screws did not provide improved biomechanical stability in the LP or DP groups, while a substantial increase in the biomechanical stability of DP was obtained when an additional medial plate was used. For the intramedullary nailing group, increasing the number of proximal interlocking screws could significantly improve biomechanical stability, and the intramedullary nailing with three proximal interlocking screws had similar static and cyclic stiffness as the DP group. The intramedullary nailing with five proximal screws had better axial stability, whereas DP had better torsional stability.

Abstract:Schatzker type IV tibial plateau fractures occur on the inner side of the tibial plateau and often involve the posterior lateral aspect. Schatzker type V tibial plateau fractures are bilateral plateau fractures, often resulting from high-energy trauma such as car accidents or falls. [ 1 ] In most cases, complex tibial plateau fractures are often associated with damage to the soft tissues, nerves, and blood vessels around the knee joint, and sometimes they may even involve dislocation of the knee joint. The tibial plateau is a critical bone structure in the human body, supporting the bending and extension functions of the knee joint. [ 2 ] As a major weight-bearing joint, when a partial tibial plateau fracture occurs in the knee joint, it severely limits its function, causing inconvenience in daily life, and may even lead to lower limb disability, having a profound impact on the patient's physical and psychological well-being. [ 3 ] When dealing with complex tibial plateau fractures, the key lies in accurately repositioning the damaged joint and providing stable internal fixation. This not only effectively corrects the alignment of the lower limb but also restores the integrity of the joint surface. Therefore, such surgeries not only require doctors to possess a high level of surgical skills but also demand the correct selection of appropriate internal fixation materials. Although traditional bilateral support plate fixation surgery provides good results in stabilizing Schatzker IV and Schatzker V tibial plateau fractures, during bilateral support plate fixation, the periosteum at the fracture site experiences additional pressure. This excessive compression may not only lead to periosteal damage but also affect the blood supply to the fracture site, thereby delaying the healing process of the fracture. [ 4 ] Compared to the limitations of traditional double-support steel plate treatment, locked plate fixation has shown significant advantages in the treatment of complex tibial plateau fractures. These advantages include providing stable fixation, employing minimally invasive techniques, and featuring point contact, all of which contribute to maintaining the stability of the biomechanical structure. This ensures solid fixation of the fracture site and ensures good blood supply, thereby promoting rapid fracture healing. Especially for patients with comminuted and extra-articular tibial plateau fractures, locked plate fixation has become the preferred method for treating such fractures. [ 5 ] In light of this, the aim of this study is to analyze the therapeutic effects of internal and external dual-plate fixation surgery on patients with posterior lateral tibial plateau fractures and investigate its safety. It is hoped that this study will provide empirical evidence for future research on the treatment methods of complex tibial plateau fractures.
