Abstract:Objectives To evaluate student tobacco control advocacy behavioural capacity using longitudinal trace data. Methods A tobacco control advocacy curriculum was developed and implemented at schools of public health (SPH) or departments of public health in seven universities in China. Participants comprised undergraduate students studying the public health curriculum in these 13 Universities. A standardised assessment tool was used to evaluate their tobacco control advocacy behavioural capacity. Repeated measures analysis of variance, paired t tests and paired χ2 tests were used to determine differences between dependent variables across time. Multivariate analysis of variance (MANOVA) and multivariate logistic regression were used to assess treatment effects between intervention and control sites. Results Respective totals of 426 students in the intervention group and 338 in the control group were available for the evaluation. Approximately 90% of respondents were aged 21 years or older and 56% were women. Findings show that the capacity building program significantly improved public health student advocacy behavioural capacity, including advocacy attitude, interest, motivation and anti-secondhand smoke behaviours. The curriculum did not impact student smoking behaviour. Conclusions This study provides sufficient evidence to support the implementation of tobacco control advocacy training at Chinese schools of public health.
Smoking Prevention and Control among Students of Middle Schools in Shanghai

Abstract:OBJECTIVE:To appraise the effectiveness of a school-based smoking control program and explore the effective smoking prevention and control intervention model in middle schools.METHODS:Through two phase cluster sampling, students in grade 1 and grade 2 of two schools were assigned to intervention group and control group. Comprehensive smoking intervention was implemented in the intervention group for one year. The assessment was carried out through three phases: baseline, after intervention, and 6-month after intervention.RESULTS:After intervention, the proportion of attempt smoking rate in the recent one year decreased from 9.7% to 4.2% and the heavy passive smoking rate declined from 15.5% to 12.2%. In addition, some knowledge, attitudes and skills related to tobacco prevention among the students in the intervention group were significantly improved and most effects can be sustained at 6-month follow up survey.CONCLUSION:It is concluded that tobacco control in middle school was effective and feasible and it should be closely integrated with tobacco control in the community.

Abstract:OBJECTIVE:To evaluate the effectiveness of a school-based smoking prevention and control intervention program among elementary school students.METHODS:Through two phase cluster sampling, 566 pupils in grade 4 and grade 5 of two schools were assigned to intervention group and control group. One year comprehensive smoking intervention was conducted in the intervention group. The assessment was carried out through three questionnaires: pre- and post-intervention, 6-month after intervention.RESULTS:After one year intervention, pupils in the intervention group significantly improved their knowledge and attitudes related to tobacco use. The rate of attempting smoking decreased form 7.8% to 2.6% and the rate of passive smoking from 53.6% to 41.8%. The difference between the intervention and control groups was statistically significant. However, several index started to decline at 6-month follow up survey.CONCLUSION:The result demonstrated the effectiveness and feasibility of tobacco control in elementary school and the positive effect must be developed.

Abstract:Objective Cigarette smoking is highly prevalent, despite being a primary preventable cause of disease and mortality. This study examined the effect of a Transtheoretical Model (TTM)-based psychoeducational intervention for smoking cessation (SC) on knowledge, SC-related parameters, and progression through the TTM stages of change among rural smokers. Methods This quasi-experimental study recruited 200 smokers from an outpatient clinic. The comparison group was recruited before the experimental group to address possible self-selection bias. Structured questionnaires were administered pre- and post-intervention (three months) and at follow-up (six months). Results A generalised estimation equation model indicated that the TTM-based intervention significantly increased participants' SC-knowledge and improved progression through TTM stages by the six-month follow-up. No significant group differences were found in self-efficacy and nicotine dependence scores or daily cigarette consumption. Conclusions A TTM-based intervention enhances SC-knowledge and fosters progress through change stages. However, it does not directly impact nicotine dependence or cigarette consumption. Outpatient settings may employ TTM-based programmes for SC education and motivation. Detecting anticipated effects may require a longer intervention duration exceeding six months. Practice implications Such TTM-based programmes may facilitate SC-knowledge and motivation in outpatient settings. Further research to comprehend patients' context and experiences during the stages of change is required.

Abstract:Objective: To evaluate the association between smoke-free regulations in public places and secondhand smoke exposure and related beliefs, awareness, attitudes, and behavior among urban residents in China. Methods: We selected one city (Hangzhou) as the intervention city and another (Jiaxing) as the comparison. A structured self-administered questionnaire was used for data collection, and implemented at two time points across a 20-month interval. Both unadjusted and adjusted logistic methods were considered in analyses. Multiple regression procedures were performed in examining variation between final and baseline measures. Results: Smoke-free regulations in the intervention city were associated with a significant decline in personal secondhand smoke exposure in government buildings, buses or taxis, and restaurants, but there was no change in such exposure in healthcare facilities and schools. In terms of personal smoking beliefs, awareness, attitudes, and practices, the only significant change was in giving quitting advice to proximal family members. Conclusions: There was a statistically significant association between implementation of smoke-free regulations in a city and inhibition of secondhand tobacco smoking exposure in public places. However, any such impact was limited. Effective tobacco control in China will require a combination of strong public health education and enforcement of regulations.

Abstract:OBJECTIVESTo examine the prevalence of smoking, second-hand smoke exposure, and attitudes toward tobacco control and cessation training among university students in China.METHODSWe administered the Global Health Professions Student Survey (GHPSS) to students from 50 universities offering medical/ health professional (MHP) programs and received 11,954 responses. Non-MHP students, MHP students, and third-year MHP students comprised the sample. Descriptive statistics were calculated for weighted prevalence and 95% confidence intervals. Key factors of interest and attitudes toward medical smoking cessation were examined using logistic regression.RESULTSSmoking and exposure to second-hand smoke was higher among non-MHP (15.9%, 31.9%) than MHP (7.0%, 21.2%) students. For third-year MHP students, the rates were 6.9% and 19.7%, respectively. Students held positive attitudes toward smoking bans in public places and cessation services. However, few received formal training in smoking cessation, and 37.0% agreed that light cigarettes are less harmful to health. Positive attitudes toward cessation services were related to several factors.CONCLUSIONSThis study is the first comprehensive survey of students in China providing direction for building capacity in tobacco control and smoking cessation among students in health professional programs.

Abstract:Objectives. To inform a prevention strategy, this study determined the prevalence of and attitudes toward smoking among Chinese secondary school students. Methods. Self-completion questionnaires were administered to 13- to 18-year-olds attending school in Zhejiang Province, eastern China. Results. Of the 6674 respondents, 15.9% (25.7% of the boys, 5.4% of the girls) were ever smokers. Only 0.3% were regular smokers. Of the ever smokers, 41.9% had smoked before 10 years of age and 7.9% before 5 years of age. Parental smoking was the strongest predictor of smoking. Conclusions. The very low prevalence of regular smoking found in this age group suggests that prevention programs in school may be beneficial. Parents should be encouraged to adopt more responsible attitudes toward smoking in the home.

Abstract:Objectives To examine the association of exposure to secondhand smoke (SHS) and tobacco advertising with unassisted smoking cessation among Chinese male smokers. Methods A questionnaire was administered to 6500 male adult smokers from six cities in China selected in a cross-sectional multistage sampling design. The survey collected self-reported demographic characteristics, smoking and cessation status, SHS exposure and tobacco advertising exposure, with 5782 valid questionnaires included in this study. Multiple logistic regression analysis was used to assess the association of unassisted smoking cessation with exposure to SHS and tobacco advertising. Results 42.1% of smokers who made unassisted quit attempts achieved abstinence (95% CI 32.5% to 51.7%). SHS (adjusted OR (aOR) 0.36; 95% CI 0.18 to 0.71; p=0.033) and tobacco advertising exposure (aOR 0.63; 95% CI 0.37 to 0.95; p=0.039) were negatively associated with unassisted smoking cessation. Conclusion The vast majority of Chinese male smokers rely on unassisted methods to quit smoking. Success prevalence is high, which is very beneficial to health. This study suggests that exposure to secondhand smoking and tobacco advertising hinders the success of unassisted cessation. These findings speak to the need for environmental tobacco control measures to promote unassisted smoking cessation among Chinese adult male smokers.

Abstract:Objective: To examine smoking cessation among urban-based Chinese. Methods: Multi-stage random sampling was used to obtain a sample from 21 cities in China. Two logistic regression models were established to identify factors influencing quit intention and smoking cessation. Results: Prevalence of smoking cessation was 10.1%; 45.5% of smokers intended to quit. Women and professionals had higher cessation rates than men and nonprofessionals. Rates of quit intention were highest among managers and clerks, and lowest among those who used gifted tobacco, smoked alone, and reported addiction to nicotine. Conclusion: Individual and city level factors are associated with quit intention and smoking cessation among urban-dwelling Chinese smokers. This information should guide smoking cessation programs and inform health policy.

Abstract:The study examined the prevalence of unassisted smoking cessation among Chinese urban male smokers and factors important in the decision to quit. A cross-sectional survey employing multistage sampling involving 5,782 participants in six cities in China was conducted. Survey respondents reported their smoking cessation status and related individual and environmental variables. Among current smokers 1,112 or 35.0% (95% CI [31.0%, 40.8%]) had attempted to quit and of those who had made such an attempt 87.6% reported that they had done so without assistance. Of all former smokers (3,389), most (97.6%; 95% CI [96.7%, 98.5%]) quit without assistance. Logistic regression analysis showed those who engaged in physical exercise and who had more belief in their ability to quit were more than twice as likely to make a quit attempt and be successful than those in comparable reference groups. Exposure to tobacco advertising was negatively associated with both unassisted quit attempts and success. By contrast, exposure to anti-tobacco information was positively associated with unassisted quit attempts while household and workplace smoking restrictions were negatively associated with unassisted attempts to quit. Most attempts to quit smoking among Chinese males are unassisted. Unassisted attempts to quit smoking and success rates are highly influenced by the presence of environmental smoking restrictions, tobacco advertising, and exposure to anti-tobacco information. Smoking cessation programs and policies in China need to pay greater attention to the social and cultural norms, which perpetuate high levels of smoking.
Secondhand Smoke Exposure Reduction Intervention in Chinese Households of Young Children: A Randomized Controlled Trial

Abstract:OBJECTIVE:To assess whether a theory-based, community health worker-delivered intervention for household smokers will lead to reduced secondhand smoke exposure to children in Chinese families. METHODS:Smoking parents or caregivers who had a child aged 5 years or younger at home were randomized to the intervention group (n = 164) to receive smoking hygiene intervention or to the comparison group (n = 154). The intervention was delivered by trained community health workers. Outcomes were assessed at 2- and 6- month follow-up. RESULTS:Of the 318 families randomized, 98 (60%) of 164 intervention group and 82 (53%) of 154 of controls completed 6-month follow-up assessment. At the 6-month follow-up, 62% of intervention and 45% of comparison group households adopted complete smoking restrictions at home (P = .022); total exposure (mean number of cigarettes per week ± standard deviation) from all smokers at home in the past 7 days was significantly lower among children in the intervention (3.29 ± 9.06) than the comparison (7.41 ± 14.63) group (P = .021); and mean urine cotinine level (ng/mL) was significantly lower in the intervention (0.030 ± .065) than the comparison (0.087 ± .027) group, P < .001). Participants rating of the overall usefulness of the intervention was 4.8 + 0.8 (1 standard deviation) on the 5 point scale (1 not at all and 5 = very useful). CONCLUSIONS:The findings of this very first study in China showed that smoking hygiene intervention was effective in reducing children's exposure to secondhand smoke. These findings have implications for the development of primary health care-based secondhand smoke exposure reduction and family oriented smoking cessation interventions as China moves toward a smoke-free society.
[Evaluation on the Effects of Community-Based Anti-Smoking Intervention].

Abstract:To explore effective antismoking intervention measures, measures including flyers and booklets as major mass media, school-based health education and follow-up to smokers were carried out in Beijing, Shanghai and Changsha urban communities. The changes of smoking behavior in population were evaluated by quasi-trial designation. Results showed that smoking rates in adult male, female and both genders decreased by 5.54%, 2.61% and 2.94% respectively and were statistically significant in intervention community. Compared with control community, the net decrease of smoking rates in adult male, female and both were 4.90%, 3.37% and 3.51% respectively. The net increase of quitting rates in adult male, female and both were 4.35%, 17.19% and 6.30% respectively. The net changes in female and in both genders of smoking and quitting were all statistically significant. It was suggested that comprehensive intervention measure with health education as the core might be the priority for the effective community-based antismoking activity. The policy-making and behavior encouraging activities should be focused.
Assessing the Impact of a Health Education Anti-Smoking Program for Students: A Follow-Up Investigation

Abstract:In this follow-up study, we aimed to assess the effectiveness of the “I do not smoke, I exercise” anti-smoking preventive health education program. The program was based on the theory of planned behavior supplemented with life skills teaching and targeted at high school students. The intervention comprised ten one-hour online sessions, administered by physical education instructors. The study cohort comprised 222 students (109 boys, 113 girls) from 11 secondary schools, with an average age of 16.42 ± 1.36 years. Data collection involved pre- and post-intervention self-assessment questionnaires. The examined variables included attitudes towards smoking, intention to smoke, subjective norm, perceived behavioral control (PBC), knowledge about smoking, smoking behavior, exercise behavior, attitudes toward the program’s implementation, and satisfaction with the program. A separate paired samples t-test revealed a significant improvement in students’ knowledge about smoking (t217 = −5.605, p < 0.001, d = 0.38) and perceived behavioral control (t220 = −2.166, p < 0.05, d = 0.15) following the intervention. However, no significant changes were observed in the remaining variables. In addition, students’ overall satisfaction with the implementation of the present health education program was high (M = 5.72 ± 1.39). These findings suggest that the health education smoking prevention intervention incorporates techniques and strategies that influence the perceived behavioral control variable, emphasizing students’ strong interest in educationally theorized programs integrating technology into their design. Future studies should consider further examination of tobacco control strategies within the high school context.

Abstract:Background: Little is known about the smoking behaviors and household smoking practices among rural population in developing countries, and among the Chinese rural residents.The purpose of this study is to explore the perception of a tailored health education intervention, smoking hygiene intervention (SHI), to address secondhand smoke (SHS) exposure reduction and promote smoking cessation in rural Chinese households.Method: We conducted three focus group discussions (FGDs) of 18 residents in rural Taizhou.FGDs were audiotaped, transcribed and analyzed thematically.The FGDs guided the tailoring of all intervention materials for use by rural Chinese population in a subsequent randomized controlled trial.Results: Participants considered the proposed SHI intervention to reduce children's SHS exposure and promote smoking cessation in adults, in general, is appropriate for rural Chinese.Participants provided suggestions on the content and design of the intervention, which are relevant to improve the acceptability and usability of the SHI intervention.These are summarized under three major themes: (1) perceptions about smoking and SHS exposure, (2) perceptions about smoking cessation and SHS exposure reduction intervention, and (3) design considerations (i.e.contents and delivery) of the SHI.Conclusion: This qualitative study in rural Chinese household members provides insights about the need and delivery of a community-based intervention to reduce children's SHS exposure and promote smoking cessation in the households of rural China.The findings informed the development of a revised SHI model for use by the rural Chinese household members, which is now being evaluated in an ongoing randomized controlled trial.
Reducing Environmental Tobacco Smoke Exposure of Preschool Children: A Randomized Controlled Trial of Class-Based Health Education and Smoking Cessation Counseling for Caregivers

Abstract:OBJECTIVES: To assess counseling to caregivers and classroom health education interventions to reduce environmental tobacco smoke exposure of children aged 5-6 years in China.METHODS: In a randomized controlled trial in two preschools in Changsha, China, 65 children aged 5-6 years old and their smoker caregivers (65) were randomly assigned to intervention (n = 33) and control (no intervention) groups (n = 32). In the intervention group, caregivers received self-help materials and smoking cessation counseling from a trained counselor, while their children were given classroom-based participatory health education. Children's urinary cotinine level and the point prevalence of caregiver quitting were measured at baseline and after 6 months.RESULTS: At the 6-month follow-up, children's urinary cotinine was significantly lower (Z = -3.136; p = 0.002) and caregivers' 7-day quit rate was significantly higher (34.4% versus 0%) (p < 0.001; adjusted OR = 1.13; 95% CI: 1.02-1.26) in the intervention than control group.CONCLUSIONS: Helping caregivers quitting smoke combined with classroom-based health education was effective in reducing children's environmental tobacco smoke exposure. Larger-scale trials are warranted.

Abstract:Objectives To examine the effectiveness of a media literacy‐based smoking prevention program based on Ajzen's theory of planned behavior in female adolescents. Methods This quasi‐experimental study was conducted with female high school students aged 16–17 years in Seoul, Republic of Korea. The program provided eight sessions over 4 weeks. Quantitative data were collected before and after online surveys in an intervention (n = 21) and control (n = 21) groups, and analyzed using mixed analysis of variance. Qualitative data on participation experiences was collected by requesting the participants to answer open‐ended questions once a week during the intervention and performing co‐occurrence analysis of specific terms in the responses was conducted through text mining. Results Although the program decreased smoking intention and increased smoking media literacy in the intervention group, there were no significant differences between the groups. Qualitative results obtained from the intervention group showed cognitive and behavioral changes in the perception of the harmfulness of e‐cigarettes in the media and the expression of a willingness to overcome the temptation to smoke. Conclusions Our findings show that the enhancement of smoking media literacy, specifically by correcting misconceptions regarding e‐cigarettes promoted by the new media, contributes smoking prevention in female adolescents. It supports calls for an expanded role of public health professionals in health education at the school level.

Investigation of Tobacco Control in Primary and Secondary Schools in Huairou District of Beijing City

Abstract:[Objective]To understand the smoking status among primary and secondary school students and the understanding of tobacco related knowledge,to find effective tobacco control method for different schools,and provide support data and scientific evidence for government to develop related policies and measures.[Methods] 840 students of 4 primary schools,684 students of 4 secondary schools,430students of 3 high schools,and 252 students of a vocational school in Huairou District were selected.The proportion of boys to girls should be in line with the 1:1 as far as possible.Anonymous questionnaire survey of closed type was conducted.Based on the different learning stages,questionnaire were divided into junior primary level(grades 1 to 3),senior primary level(grades 4 to 6),and secondary level.[Results]Attempting smoking rate of junior primary school students was 15.21%,that of senior primary school students was 0.10,smoking rate was 0.49%;attempting smoking rate of secondary school students was 29.51%,smoking rate was 17.20%.[Conclusion]Tobacco control of secondary school students should be the focus of tobacco control in schools of our region.
A COMPUTATIONAL FLUID DYNAMICS AND AN EXPERIMENTAL APPROACH TO THE EFFECTS OF PUSH VALVES ON SIEVE TRAYS

Abstract:In this study to improve hydraulic performance of sieve tray, push valves have been utilized on the sieve tray deck. The hydraulic effects have been investigated using computational fluid dynamics simulation and experimentation. The computational fluid dynamics (CFD) model used 1.213 m diameter Solari and Bell [1] sieve tray. In the CFD model it is found that the ratio of the push valves' open area to the total hole area is approximately 14.31% which is considered as a design parameter. The experimentally investigated sieve tray had 7.039 % hole area and 32 push valves. The ratio of push valves' open area to the total hole area was 14.59%. The dry pressure drop, total pressure drop and weeping were measured. The experimental results of this modified sieve tray and the sieve tray without push valves were compared with each other. Results show that better hydrodynamic behavior of the modified push valve sieve tray than conventional sieve tray.
Effects of Comprehensive Interventions in Community on Smoking, Chronic Bronchitis, and Asthma in Rural Areas of Beijing

Abstract:OBJECTIVE:To study the effects of comprehensive interventions in community on smoking, chronic bronchitis, and asthma in rural areas of Beijing. METHODS:Twenty-three villages in rural areas of Beijing were randomly divided into interventional (13 villages) and control villages (10 villages) in 1992. Comprehensive interventions including education of former-smokers and improvement of living environment were carried out in the interventional villages, and none was done in the control villages. In April 2000, surveys on smoking, chronic bronchitis, and asthma were carried out among 34,436 participants aged 15 or more in the interventional and control villages. During the same period, knowledge on prevention from chronic obstructive pulmonary diseases (COPD), living environments, and smoking were assessed among 1658 high-risk individuals of COPD at baseline and following-up period. RESULTS:The scores of knowledge and improvement on living environments in the interventional villages were significantly higher than those in control villages (P < 0.001). The decrease rate of smoking and current smoking rate in the interventional villages were significantly higher than in the control villages (0.4% vs -0.8%, P < 0.001; 2.4% vs 1.3%, P < 0.001) in men, while not different in women (P > 0.05). Among never smokers at baseline, the accumulated incidence of smoking among people aged 15 to 24 from 1993 to 2000 was significantly lower in the interventional villages than in the control villages in men (18.9% vs 23.7%, P = 0.005) and in women (0% vs 0.7%, P = 0.005). Daily cigarettes smoked by smokers in the interventional villages were less than in control villages in both men (14.8 +/- 7.0 vs 17.2 +/- 8.2 cigs daily, P < 0.001) and women (12.8 +/- 6.9 vs 13.4 +/- 7.2 cigs daily, P = 0.088). The increase of prevalence of chronic bronchitis in the interventional villages was less than in the control villages (men: 0.9% vs 1.3%, P = 0.012; women: 0.1% vs 0.3%, P = 0.003). After the age factor is adjusted, odds ratio (OR) for accumulated incidence of chronic bronchitis from 1993 to 2000 in the interventional villages were 0.80 (95%CI: 0.60-1.07) for men, 0.76 (95%CI: 0.45-1.28) in women. The OR for asthma was not significant in both men and women. CONCLUSIONS:Comprehensive interventions in community may improve knowledge of COPD prevention and living environments, decrease the smoking rate, cigarettes smoked per day, and incidence of chronic bronchitis, but have no significant effects on asthma.
