Abstract:Objective: To investigate the changes in surface electromyography(sEMG) activity of masticatory muscles before and after occlusal rehabilitation in patients without occlusal support.Methods: 24 patients without occlusal support were selected and conducted with regularly occlusal rehabilitation.The sEMG of masseter muscle(MM) and anterior temporalis muscle(TA) at mandibular postural position(MPP) and during clenching at intercuspal position(ICP) were separately recorded in different stages.The changes of the mean amplitude(MA) of sEMG,the asymmetry indexes(AsI) of bilateral MM,TA and total masticatory muscles(AsMM,AsTA,Astot),activity indexes(AcI) of MM/TA in each stage were analyzed.Results: 1.The changes in sEMG activity before and after restoration at MPP: MA and AsI of MM had no significant difference in different stages(P > 0.05).MA and AsI of TA after using 3 months were significantly higher than using early stage.AcI of MM/TA was negative at every stage(P < 0.05).2.The changes in sEMG activity before and after restoration at MPP: MA of MM and TA was significantly increased after using three months(P <0.05).Each AsI was close to first wearing(P>0.05),but significantly lower than 1 week and 1 month after restoration(P<0.05).AcI of MM/TA were close to 0.Conclusion: The sEMG of masticatory muscles changed at different stages of prothetic rehabilitation in patients without occlusal support.sEMG can provide reference for evaluation of effect of occlusal rehabilitation.

Abstract:Statement of problem Transcutaneous electrical nerve stimulation (TENS) has been used in several clinical areas. However, the effect of TENS on the masticatory muscles of young individuals with normal occlusion remains unclear. Purpose The purpose of the study was to assess the effect of TENS on the surface electromyographic (sEMG) activity of masticatory muscles in a young population with normal occlusion. Material and methods Twenty residents (5 men and 15 women, mean 24.27 ±2.59 years) of Dalian Stomatological Hospital were enrolled as the study participants. A trained operator collected the required information from the participants. The experiment was divided into 3 stages: pre-TENS acquisition, TENS application, and post-TENS acquisition. The pre-TENS stage was performed using surface electromyography (sEMG) (Myotronics Inc) to acquire the potential values of masticatory muscles in the following 3 states 5 times each: resting, intercuspal occlusion (ICO), and maximum voluntary clench (clenching). The potential values of the anterior of temporalis (TA), the masseter (MM), the sternocleidomastoid (SCM), and the anterior digastric (DA) muscles were collected in the resting state, and TA and MM were collected in the ICO and clenching states. During the TENS application phase, a TENS Unit device (J5 Myomonitor) (J5) was used on each participant for 45 minutes. The post-TENS acquisition phase involved the same procedure as the pre-TENS phase. The experimental data were recorded, and the normality of each group was analyzed using the Shapiro-Wilk test in a statistical software program (IBM SPSS Statistics, v26.0). The paired-sample t test was used to compare the differences in the mean values of sEMG and the asymmetry index (As); the independent-sample t test was used to compare the activity index (Ac) and torque index (To) (α=.05). Results Significant differences were observed in the mean potential values of TA, MM, LSCM, and RDA before and after TENS in the resting state and RTA, LMM, and RMM before and after TENS in the clenching state ( P <.05). Moreover, although As DA values showed a significant difference ( P =.027) before and after TENS in the resting state, the differences in As values for the other muscles in the resting state were statistically similar. Furthermore, in each state, the mean values of Ac and To after TENS showed no significant differences before and after TENS ( P >.05). Conclusions The resting EMG values of the TA and MM differed significantly before and after TENS. After TENS, the resting EMG activity decreased, whereas the functional EMG activity tended to increase.

The inaccuracy of cystatin C and creatinine-based equations in predicting GFR in orthotopic liver transplant recipients.

Abstract:BACKGROUND As survival with an orthotopic liver transplant (OLT) improves, the incidence of chronic kidney disease in OLT recipients increases. Measurement of kidney function using creatinine-based estimates is often inaccurate, while cystatin C may overcome the biases that effect creatinine. The aim of this study was to assess the accuracy of creatinine- and cystatin C-based equations to estimate kidney function in long-term OLT recipients. METHODS This was a cross-sectional study performed on OLT recipients within a single liver transplant centre where creatinine (n = 41) and cystatin C (n = 30) were measured and glomerular filtration rate (GFR) estimated using the Modification of Diet and Renal Disease (MDRD), Cockcroft-Gault (CG), Hoek, Larsson, Filler and Le Bricon equations. Comparison was made with the nuclear GFR (nGFR) (n = 41) measured through 51-Cr EDTA clearance. RESULTS The mean age of recipients was 56 +/- 13 years, and they were 6.5 +/- 4.7 years post-OLT. Fifty-six percent of recipients had a nGFR < or =60 mL/min/1.73 m(2). nGFR correlated significantly with all predictive equations (P < 0.001). The MDRD, CG and Le Bricon equations had the smallest degree of bias (-7.6, -7.3 and 3.4 mL/min/1.73 m(2), respectively), with 22%, 22% and 27% of estimates, respectively, being within 10% of nGFR measurements. In OLT recipients with nGFR < or =60 mL/min/1.73 m(2), the degree of bias of both the creatinine-base MDRD and cystatin-based Hoek equations was within 2 mL/min/1.73 m(2) difference between the measured and estimated GFR, but 41% and 36% of estimates were within 10% of the nGFR measurement. CONCLUSIONS Therefore, the degree of inaccuracy in cystatin C- and creatinine-based predictive equations brings into question their clinical utility in OLT recipients. We have no evidence that cystatin C is superior to creatinine in this population.
Effect of increased maxillo-mandibular relationship on isometric strength in TMD patients with loss of vertical dimension of occlusion

Abstract:The effect on isometric strength of the shoulders and limbs while biting in habitual occlusion, on a bite-elevating appliance and on a placebo appliance was analyzed. Twenty female volunteer patients, presenting with temporomandibular pain dysfunction syndrome and obvious loss of vertical dimension, served as subjects. All were weaker to the manual application of the isometric Deltoid Press (IDP) when biting, as opposed to maintaining the mandible in an unsupported rest position. Two intraoral appliances were fabricated for each subject: a bite-elevating appliance (BEA) set by a functional criterion of peak strength to the IDP and a placebo appliance which did not interfere with occlusion but was "set" with a mock IDP procedure. Testing was carried out by the Neuromuscular Research Testing Laboratory of the Neurology Department of Tufts New England Medical Center. Testing was independent of the dentist who fabricated and set the appliances. A standard neuromuscular test with the Maximal Voluntary Isometric Contraction apparatus was used to assess strength of right and left shoulder, elbow and knee flexion and extension as is routinely performed with all neuromuscular disease patients. Twelve strength tests were carried out for each of three conditions: 1. Baseline-biting in habitual occlusion; 2. Elevated-biting on the BEA; and 3. Placebo-biting with the placebo appliance inserted. The order of conditions 2 and 3 was counterbalanced without knowledge of the subjects. Twelve repeated measures ANOVAs (each subject as their own control) were conducted for each of the 12 strength measures. All F-tests indicated a significant main effect for treatment differences (p < 0.0001). Mean strength biting on the BEA was consistantly greater (p < 0.001) than Baseline or Placebo strength. Baseline and Placebo condition were equivalent. These findings confirmed previous observations at this TMD Center: individuals with loss of vertical dimension of occlusion respond to a bite raising appliance by increased isometric-strength.
Long-term evaluation of masseter muscle activity, dimensions, and elasticity after orthognathic surgery in skeletal class III patients

Abstract:Objective: To evaluate changes in the masseter muscle after orthognathic surgery using electromyography (EMG), ultrasonography (US), and ultrasound elastography (USE) in individuals with skeletal class III anomaly over long-term follow-up and compare with a control group. Materials and methods: The study group included 29 patients with class III dentofacial deformities scheduled to undergo orthodontic treatment and orthognathic surgery. The control group included 20 individuals with dental class I occlusion. Assessment of the masseter muscles using EMG, US, and USE was performed before orthognathic surgery (T1) and at postoperative 3 months (T2) and 1 year (T3) in the study group, and at a single time point in the control group. All assessments were performed at rest and during maximum clenching. Masseter muscle activity, dimension, and hardness were analyzed. Results: Electromyographic activity of the masseter muscle during maximum clenching was increased at postoperative 1 year but did not reach control group values. On ultrasonography, the masseter muscle showed minimal changes in dimension at postoperative 1 year compared to preoperative values and remained below control group values. The postoperative increase in masseter muscle hardness at rest and during maximum clenching persisted at postoperative 1 year. Conclusion: The results of this study suggest that after orthognathic surgery, additional interventions and much longer follow-up are needed to ensure better muscle adaptation to the new occlusion and skeletal morphology. Clinical relevance: All assessment methods are useful for comprehensively evaluating changes in the masticatory muscles after orthognathic surgery.
The Diagnosis of Neuromuscular in Orthodontics Effects of TENS on The Antero-Posterior Maxilla-Mandibular Relationship

Abstract:Objective:This study was conducted in order to assess the changes in the occlusal position of the mandible after ULF (Ultra Low Frequency)- TENS relaxing procedure in subjects in pubertal growth phase with diagnosed Angle Class II division I and mandibular dentoalveolar retrusion. Materials and methods: This study was performed on 70 patients (48 females,22 males) with an Angle Class II division I, aged between 10 and 15 years old (mean age 12.26, SD 1.32) characterized by mandible dentoalveolar retrusion and optimal vertical facial dimension, diagnosed by clinicaland cephalometric evaluation. Diagnostic neuromuscular registrations were made for all subjects. The casts were mounted on articulator in habitual intercuspal position with a tooth-guided wax bite registration. Reference points were chosen at molar level. Subsequently the same casts were mounted in myocentric position and compared to the habitual intercuspal position, assessing the antero-posterior shift after TENS procedure. Statistics: Mean and standard deviation were calculated on the amount of shifting at the left molar reference point after TENS procedure. Analysis of variance was carried out in order to evaluate the influence of sex and age on the amount of molar shift.Results: Nine subjects showed, in the antero-posterior plane, a forward mandibular shift in neuromuscular myocentric position compared to habitual intercuspal position. Six subjects showed no differences between habitual and myocentric position in the antero-posterio plane. Four individuals showed a backward mandible shift after TENS indicating worsening of the II molar in the antero-posterior plane. Conclusion: This study suggests that TENS recorded occlusion in subjects with Class II division I with mandible dentoalveolar retrusion allows to visualize an unusual trend of growth.The advancement of the mandible were not taken into account. These results could offer new diagnosis and prognosis methods for Class II malocclusions.

Abstract:There are several bronchiolar diseases with different pathologic and clinical findings. Idiopathic BOOP is a distinct entity consisting of a flu-like illness, late inspiratory crackles, patchy infiltrates roentgenographically, and physiologically decreased vital capacity and diffusing capacity. Response to corticosteroid therapy is good and relapse does not occur if sufficient therapy is given. Bronchiolitis obliterans organizing pneumonia is an appropriate description of this entity. The term is specific because it includes bronchioles and alveoli and excludes disorders involving only alveoli such as organizing pneumonitis or organizing diffuse alveolar damage. The term is general enough to include a sufficient number of patients with a homogenous disorder. Furthermore, the entity can be described to clinicians and pathologists throughout the world in such a fashion that patient care and research can be standardized. Researchers from different centers studying the cause or utilizing treatment protocols are able to discuss a single BOOP entity rather than comparing results of a heterogeneous group of many different types of interstitial lung disorders. This will lead to breakthroughs in discovery of etiologic causes and new effective therapeutic regimens.

Abstract:The aim of the present study was to investigate the short-term impact of an occlusal highspot on the occurrence of orofacial symptoms by collecting self-evaluation and using electromyography (EMG) evaluation. A rigid unilateral intercuspal occlusal highspot (A cast onlay of 0.5 mm) was placed on the right lower first molar of six adult volunteers (three males, three females), and remained for 6 days. Continuously all the induced orofacial symptoms were collected and the subjects scored the orofacial pain on a 10-cm visual analogue scale (VAS) during the placement of onlay. The surface EMG was recorded before the placement of onlay, during (on the 3rd and 6th day) and after the onlay was removed. Then the contractile symmetry of bilateral masseter (MAL, MAR) and anterior temporalis (TAL, TAR) was measured by using an asymmetry index. On the 3rd day of the placement of the occlusal highspot, all subjects complained of headache in right temporal region (mean-VAS +/- s.d. = 3.7 +/- 0.5); the activity of TAR at rest position of mandible increased significantly (P = 0.027). In addition, on the 3rd and 6th day with the highspot the EMG activity of the tested muscles during maximal voluntary contraction (MVC) was significantly reduced; the asymmetry index of bilateral anterior temporalis during MVC was increased significantly (P-3rd = 0.028; P-6th = 0.046). A unilateral occlusal highspot may make the ipsilateral anterior temporalis become tenser at rest position. Furthermore, the activity of bilateral anterior temporalis becomes more unsymmetrical during MVC although there are inter-individual differences between subjects. The changes in muscular activity may have some relationship with the occurrence of tension-type headache in temporal region.

Abstract:Introduction: Physical therapy (PT) methods applied in dentistry are increasingly discussed nowadays. Taking into account a rapidly growing number of temporomandibular disorders (TMDs) and orofacial pain patients, it is reasonable to determine which of the available physiotherapeutic (PT) methods are more effective than others, especially in terms of their possible analgesic and myorelaxant effects. Objective: To assess manual and physical factors influencing pain reduction or elimination and increased muscle tension in patients with TMD; yet the influence of the applied forms of PT on the range of motion (ROM) of temporomandibular joints (TMJ). Material and methods: A randomized, parallel-group, RCT, single-blind, equi-randomized (1:1) study was conducted in DC/TMD Group Ib patients (20-45 years of age). An experimental group (G1, n = 104) and a control group without TMD (G2, n = 104) were created according to CONSORT guidelines. Diagnostic measurements were performed in both groups (mass sEMG, temporomandibular joint range of motion-ROM, pain intensity - NRS). Group G1 was randomly divided (envelope method) into 4 therapeutic groups, in which therapy was carried out for 10 days: magnetostimulation (MS), magnetoledotherapy (MLE), magnetolaserotherapy (MLA), manual therapy (MT). Each time after the therapy, ROM and NRS measurements were performed, and after the 5th and 10th day sEMG. Results: Statistically significant differences were found in the sEMG values of the masseter muscles, TMJ ROM and the pain intensity in G1 and G2 (p < 0.00). The largest decrease in sEMG (% MVC) of the masseter muscle occurred in the subgroup in which the manual therapy (MT) procedures were applied, p < 0.000. There was no clinically significant difference in and between other subgroups. There was a distinct mandible ROM increase noted in the MT group, with minimal changes in the MLA and MLE groups and no changes in the MS group. There was a clear increase in the lateral mobility of both right and left TMJ in the MT group. There were no differences in the course of the study in the MS group, and slight increases in the MLA and MLE groups. In the case of pain measurements, the greatest decrease in pain intensity was observed in the MT subgroup. Conclusions: According to our results manual therapy is an effective form of treatment in patients with pain, increased masticatory muscle tension and limitation in mandible ROM. Dental physiotherapy should become an integral part of multimodal TMD patients' treatment.
Electromyographic evaluation of masseter and anterior temporalis muscles in rest position of edentulous patients with temporomandibular disorders, before and after using complete dentures with sliding plates

Abstract:Objectives: This study was performed with the purpose of investigating electromyographic (EMG) activity of the anterior temporalis and masseter muscles in edentulous individuals with temporomandibular disorder (TMD), before and after using sliding plates on complete dentures in the mandibular rest position. Background: Edentulous patients may present TMD, which is characterised by pain in temporomandibular joints, masticatory and neck muscles, uncoordinated and limited mandible movements, joint sounds and an altered occlusal relationship. It is imperative to offer treatment in order to re-establish stomatognathic system structures before submitting the individual to any definitive restorative treatment. Materials and methods: The patients were edentulous for at least 10 years. EMG recordings were made before the insertion of the dentures (0 months) and also after using the sliding plates at the fourth month, 9th month and 12th month, using computerised electromyography K6-I/ EMG Light Channel Surface. EMG evaluations of the muscles were performed under the following clinical conditions: rest position with dentures (R1), rest position without dentures (R2), rest position with dentures post-activity (chewing) (R3), rest position without dentures post-activity (chewing) (R4). Results: All patients obtained remission of muscular fatigue and reduced pain in stomatognathic system structures. Temporalis muscle showed significant increase in EMG activity compared with initial values (p < 0.01). Masseter muscles showed significantly lower mean values (p < 0.01) compared with initial values. Conclusion: The sliding plates allowed the process of neuromuscular deprogramming, contributing to muscular balance of the masticatory system, and are therefore indicated to be used before the fabrication of definitive complete dentures in patients with TMD.

Abstract:Purpose of study: This study aimed to evaluate masticatory muscle function by using electromyography and compare the final outcome of treatment of ADDWR by using centric and anterior repositioning splint. Patient and methods: this study was conducted on 20 patients with mean age of 29.5±3.4 complaining of anterior disc displacement with reduction of TMJ. Patients were randomly divided into two groups: I- centric splints, II- anterior repositioning splints. All patients were assessed by evaluating TMJ pain and muscle palpation, interincisal distance, joint sounds and visual analogue scale (VAS). This clinical examination was done before treatment and at 15, 30, 90, and 180 days after treatment. Also, electromyography evaluation of masseter and temporalis muscles was performed preoperative and at 3 and 6 months after treatment. Result: In both groups, joint pain, joint click, muscle pain and interincisal distance were improved significantly. Also, significant improvement in mean amplitude and duration of masseter and temporalis muscles was achieved. There was no significant statistical difference between both groups. Conclusion: The treatment of anterior disc displacement with reduction of TMJ with either of the two types of splint was acceptably successful and a significant reduction in the leading symptoms could be achieved. The use of occlusal splint is efficient to improve the electrical activity of the masseter and temporalis muscles.
The effect of clear aligner treatment on masticatory muscles (masseter, temporalis) activity in adults: a systematic review and meta-analysis

Abstract:Background: The use of clear aligners is becoming more common for aesthetic orthodontic treatment, but there are still concerns about how they affect mastication biomechanics in the short and long term. The clear aligners treatment (CAT) mechanism changes the position of the mandible and maxilla, especially impacting the masseter muscle. Surface electromyography (sEMG) proves to be a useful method to evaluate masticatory muscle activity (MMA). Objectives: To analyze the effect of clear aligners treatment on alterations in masticatory muscles (masseter, temporalis) using surface electromyography. Search methods: Five databases (PubMed, Web of Science, SCOPUS, Cochrane Library, and Google Scholar) were searched up to March 2024. Selection criteria: Studies in which MMA was evaluated after the installation of orthodontic clear aligners. Data collection and analysis: Screening, data extraction, and quality assessments were performed by four investigators independently. The data, which evaluated temporalis and masseter muscle characteristics during CAT using surface electromyography, was extracted, and the quality of the studies was evaluated. The risk of bias was assessed using the Newcastle-Ottawa Scale (NOS). Results: Six studies (two prospective cohort studies, three observational longitudinal studies, and one observational longitudinal case-control study) with low and moderate risk of bias were included in the qualitative synthesis. Six of these were also included in the meta-analysis. Our study investigated the dynamics of masseter and temporalis muscle activity during CAT. The results show that during maximal voluntary clenching, the masseter muscle demonstrated a significant initial increase (P < .05) followed by a subsequent non-significant decrease. It also showed that submaximal voluntary clenching consistently exhibited a significant reduction in muscle activity throughout the study period (P < .01). Assessment of muscle activity at the mandibular resting position revealed a variety of responses, with some participants showing a significant increase while others exhibited non-significant changes (P < .05, P > .05, respectively). However, the meta-analysis showed a non-significant difference in measuring masseter and temporalis muscles activity during CAT. Conclusions: Based on existing evidence, it is reasonable to conclude that CAT affected MMA. During maximal voluntary clenching, masseter muscle activity initially increased but later decreased, while temporalis muscle activity showed mixed results. Submaximal voluntary clenching revealed a consistent decrease in muscle activity over time. Mandibular resting position assessments showed both increases and no significant changes in muscle activity. However, the existing literature is insufficient to draw concrete conclusions; therefore, well-conducted further research is needed to confirm this statement. Registration: This systematic review and meta-analysis were registered in the International Prospective Register of Systematic Reviews (PROSPERO CRD42024522231).

The surface electromyography (EMG) of masseter (MM), anterior temporalis (TA) and posterior temporalis (TP) at rest position, during clenching and chewing were separately recorded in the stage of pre-treatment, 1 month after temporary restoration, 1 month after permanent restoration and long-term observation (mean 3.5 years). The Asymmetry Indexes (AI) of bilateral muscles, Activity Indexes of MM/TA, MM/TP and TA/TP were compared in each stage.RESULTS:(1) After occlusal rehabilitation, all the AIs of bilateral MM, TA and TP at rest position, during clenching and chewing showed obvious decrements. (2) In every stage of occlusal rehabilitation, all of the Activity Indexes of MM/TA at rest position, during clenching and chewing had no obvious changes. (3) The Activity Indexes of MM/TP and TA/TP during clenching and chewing significantly increased after occlusal rehabilitation.CONCLUSION:The treatment of occlusal rehabilitation can significantly improve the bilateral symmetry of masticatory muscles in patients with extensive tooth wear, and also change the coordinated EMG activity of different masticatory muscles.

Abstract:Introduction: This study evaluated the masseter muscle changes after surgical-orthodontic treatment in patients with a skeletal Class III malocclusion using automatic segmentation. Methods: Images of 120 patients with skeletal Class III malocclusion were obtained and reconstructed at T0 (pretreatment), T1 (presurgery), and T2 (6-12-month postsurgery). The patients were divided into symmetrical and asymmetrical groups. The volume, major axis length, maximum cross-sectional area, horizontal cross-sectional area 5 mm above the mandibular foramen (CSAF), and orientation were calculated automatically. Results: In the asymmetrical group, the volume and major axis length on the deviated side were lower than on the nondeviated side at T0, T1, and T2 ( P < 0.05). There were no significant differences in maximum cross-sectional area and CSAF bilaterally. The orientation was coronally more vertical and sagittally more forward on the deviated side (both P < 0.001). In the symmetrical group, there were no significant bilateral differences at T0, T1, and T2. The volume, major axis length, and CSAF decreased, and the coronal orientation was more vertical on the nondeviated side at T2 than at T0 in both groups ( P < 0.05). The coronal plane orientation was more inclined on the deviated side at T2 than at T0 in the asymmetrical group ( P < 0.05). Conclusions: The smaller volume on the deviated side at T2 indicates the need for myofunctional training after surgery. The masseter muscle volume and the cross-sectional area did not recover to the preorthodontic levels. Studies with longer follow-up durations are needed to confirm these findings.

Abstract:There is limited and conflicting evidence regarding the impact of orthodontic appliances on masticatory muscle activity (MMA), jaw function, and oral parafunctional behaviours. Investigations in this field frequently utilise surface electromyography (sEMG) to analyse changes in muscle contractile activity. Fixed orthodontics appliances have been associated with significant reductions in MMA post archwire activation. Conversely, elevations in masseter and digastric muscle activity have been reported in response to removable functional appliances. Research into the effect of clear aligner therapy (CAT) on MMA is still in its infancy with few studies published. There is a lack of agreement on whether MMA increases or diminishes in response to CAT though there is evidence this relationship may change over time due to muscle adaptation. The impact of CAT on temporomandibular disorder (TMD) symptoms is unclear, particularly how pre-existing risk factors for TMD may alter and individual's response.

Abstract:Non-invasive approach is gaining an increasing recognition in the TMD patients management. It is therefore reasonable to conduct RCTs evaluating the effectiveness of both physical and manual physiotherapy interventions. The aim of this study was to evaluate the short-term efficacy of selected physiotherapeutic interventions and their effect on the bioelectrical function of the masseter muscle in patients with pain and limited TMJ mobility. The study was conducted on a group of 186 women (T) with the Ib disorder diagnosed in DC/TMD. The control group consisted of 104 women without diagnosed TMDs. Diagnostic procedures were performed in both groups. The G1 group was randomly divided into 7 therapeutic groups in which the therapy was carried out for 10 days: magnetostimulation (T1), magnetoledotherapy (T2), magnetolaserotherapy (T3), manual therapy- positional release and therapeutic exercises (T4), manual therapy - massage and therapeutic exercises (T5), manual therapy - PIR and therapeutic exercises (T6), self therapy - therapeutic exercises (T7). In the T4 and T5 groups, the treatments led to complete resolution of pain after the 10th day of therapy and to the largest minimal clinically significant difference in the MMO and LM parameter. GEE model for PC1 values using treatment method and time point showed that T4, T5 and T6 treatments had the strongest effect on the parameters studied. Therefore, it may be concluded that SEMG testing is a helpful indicator to assess the therapeutic effectiveness of physiotherapeutic interventions. Therefore, we wanted to assess the effectiveness of physiotherapy interventions in TMD patients using SEMG. Purpose: Evaluation of the short-term efficacy of selected physiotherapeutic interventions and their effect on the bioelectrical function of the masseter muscle in patients with pain and limited TMJ mobility. Material and methods: The study was conducted on a group of 186 women (T) with the Ib disorder diagnosed in DC/TMD (Ib - myofascial pain with restricted mobility). The control group consisted of 104 women without diagnosed TMDs (normal reference values for TMJ ROM and masseter muscle SEMG bioelectric activity). Diagnostic procedures were performed in both groups (SEMG of the masseter muscles at baseline and during exercise, measurement of TMJ mobility, assessment of pain intensity - NRS scale). The G1 group was randomly divided into 7 therapeutic groups in which the therapy was carried out for 10 days: magnetostimulation (T1), magnetoledotherapy (T2), magnetolaserotherapy (T3), manual therapy- positional release and therapeutic exercises (T4), manual therapy - massage and therapeutic exercises (T5), manual therapy - PIR and therapeutic exercises (T6), self-therapy - therapeutic exercises (T7). Each time after therapy, the intensity of pain and TMJ mobility were assessed. Sealed, opaque envelopes were used for randomization. After 5 and 10 days of therapy, bilateral SEMG signals of the masseter muscles were acquired. PC1 factor analysis was performed. A score of 99% in the PC1 parameter, demonstrates the clinical relevance of electromyography (MVC). Results: Synergism of physical factors will lead to a higher MID on the NRS scale. Evaluating the MID of the therapeutic interventions used showed a better therapeutic effect of manual interventions over physical and self-therapy. In the T4 and T5 groups, the treatments led to complete resolution of pain after the 10th day of therapy and to the largest minimal clinically significant difference in the MMO and LM parameter. GEE model for PC1 values using treatment method and time point showed that T4, T5 and T6 treatments had the strongest effect on the parameters studied. Conclusions: 1. Exercise SEMG testing is a helpful indicator to assess the therapeutic effectiveness of physiotherapy interventions. 2. Manual therapy treatments are superior to physical treatments in their relaxation and analgesic efficacy and should therefore be prescribed as a first line non-invasive intervention for TMD pain patients.

Abstract:Background: Orthodontic and surgical technical advances in recent years have resulted in treatment opportunities for a whole range of craniofacial skeletal disorders either in the adolescent or adult patient. In the growing child, these can include myofunctional orthodontic appliance therapy or distraction osteogenesis procedures, while in the adult, the mainstay approach revolves around orthognathic surgery. The literature agrees that for a change in craniofacial morphology to remain stable, the muscles acting upon the facial skeleton must be capable of adaptation in their structure and, therefore, their function. Failure of the muscles to adapt to the change in their length or orientation will place undesirable forces on the muscle attachments leading to potential instability of the skeleton. Adaptation can occur through various processes including those within the neuromuscular feedback mechanism, through changes within muscle structure or through altered muscle physiology, and through changes at the muscle/bone interface. It is now accepted that because there is no single method of assessing masticatory function, several measures should be taken, and whenever possible, simultaneously. Methods: This investigation was designed to apply several, newly developed and more sophisticated methods of measuring muscle structure and function to a situation where adaptation of muscle is pivotal to the success of a therapeutic approach. Patients attending the combined orthodontic/orthognathic surgery clinic at the Clitrofa - Centro Médico, Dentário e Cirúrgico, in Trofa, Portugal, were screened. Ten patients scheduled for a bimaxillary osteotomy involving a combination of maxillary Le Fort I impaction procedure coupled with a sagittal split advancement of the mandible were selected to form the study group. The patients have MRI of the masseter muscle to evaluate the masseter muscle volume and fibre orientation changes. This exam was taken before surgery (T0), 6 to 12 months after surgery (T1), and 3 years after surgery (T2), by two independent observers, according to the protocol jointly developed between the Eastman Dental Institute - University of London and the MRI Centre - Department of Radiology at John Radcliffe Hospital - University of Oxford. Results: Significant differences (p < 0.05) have been identified between Time 0 (pre-op) and Time 1 (6-12 months post-op) regarding the masseter area (mm2). The differences against Time 0 (pre-op) seem to disappear at Time 2 (3 years post-op). Conclusions: MRI therefore seems to be a valid tool for measuring differences in the masseter muscle area and volume associated with high-severity occlusal deformities, although showing not to be as efficient in detecting the same differences in cases of low-severity occlusal deformities.

Abstract:Objective: Temporomandibular joint disorders (TMD) are a type of disorder that affects the temporomandibular joint, muscles, and nerves leading to persistent facial pain. Transcutaneous electrical nerve stimulation (TENS) therapy is an alternative treatment for pain relief in TMD patients. The aim of the study was to assess the condylar position changes and pain levels occurring in TMD patients after TENS therapy. Materials and methodology: According to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) criteria, a total of seven patients with TMD were included in this study. Routine diagnostic investigations were taken for each patient along with joint vibration analysis (JVA), electromyography (EMG), and a cone beam computed tomography (CBCT) at rest position. Using the Dolphin software (version 11.95), the temporomandibular joint spaces were measured on the CBCT for the right and left sides individually for each patient. These patients were then subjected to TENS therapy, and an EMG reading was taken post-TENS therapy to evaluate the effect on muscle strain. A CBCT was taken after the TENS therapy to assess its effect on the condylar position. The statistical significance of the pre- and post-temporomandibular joint spaces and the EMG readings post-TENS therapy were assessed using paired t-tests, respectively. Results: There was a change in the anterior, middle, and posterior dimensions observed radiographically, before and after TENS. However, the results were found to be statistically significant only in the anterior region on the right joint space (P = 0.03) and posterior region of the left joint space (P = 0.04). Conclusion: TENS therapy has been found to bring about a change in the condylar position and pain intensities by relieving inadvertent muscle activity in the temporomandibular region. Therefore, further studies with larger sample sizes and control are required to fully ascertain the role of inadvertent muscle activity on the TMJ.
