Abstract:The vertebral artery (VA) V3 segment, or suboccipital segment, is located between the C2 transverse process and the dura mater at the level of the foramen magnum (FM) (1–4). This chapter focuses on the two basic surgical approaches, the anterolateral and the posterolateral ones, which allow reaching this segment. Before explaining the approaches, the relevant anatomy is detailed. Knowledge of the anatomy of the craniocervical junction is a mandatory prerequisite. Finally, dedicated indications are discussed.
Exploring the atlantic part of the vertebral artery in the South Indian population and its implications in spine surgery

Abstract:Study design: A descriptive, quantitative cross-sectional study of the atlantic part (V3) of the vertebral artery (VA). Purpose: This study aimed to bridge the research gap in the morphometry of the V3 segment of the VA in the South Indian population. Overview of literature: The microsurgical anatomy of this segment of the VA has been explored in various populations, and a thorough understanding of the anatomy and course of the VA, particularly the V3 segment, is essential to prevent iatrogenic complications. Several computed tomography studies but a few cadaveric studies have explored the V3 segment of the VA in the South Indian population. Methods: This study examined 40 VAs from 20 embalmed cadavers that were obtained from the voluntary donation program, and Institutional Ethical Clearance was obtained before the study. The length, diameter, and angle of the vertical, horizontal, and exit parts of the V3 segment of the VA were documented after its exposure. Results: The mean lengths of the right and left VAs on each part were nearly similar, except for the mean length of the horizontal part (right: 38.937 mm, left: 40.237 mm) and total length of the V3 segment (right: 66.870 mm, left: 70.350 mm). Conclusions: These morphometric parameters are essential to spine surgeons who intend to operate on a small part (vertical, horizontal, or exit part) of the V3 segment of the VA. The mean values of the parameters obtained in this study give average measurements or safe limits to surgeons for safe surgical procedures such as the occipital condyle screw technique and C1 laminectomy.
Prevalence of anatomical variations at the suboccipital (V3) segment of the vertebral artery: a systematic review

Abstract:Abstract Background and objective A recent meta-analysis on the incidence of iatrogenic injury to the VA has revealed that patients with variant anatomy are more prone to iatrogenic injury. Therefore, this review is designed to investigate the incidence of variations in the suboccipital component of the vertebral artery in different population groups according to the available literature. Methods This systematic review was conducted according to PRISMA guidelines (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The review is based on a comprehensive and extensive search of PubMed, Google Scholar, and ResearchGate. The following search terms were used: “vertebral artery” AND “suboccipital segment” AND “anomalies/anatomical variations of the V3 segment.” Reference lists of all extracted articles were also extensively searched for references to any further relevant publications. Results A total of 17 papers met the inclusion criteria. The 17 studies corresponded to a total of 10,820 patients. A persistent first intersegmental artery was registered in 1.8% (197 out of 10,820) of the patients. Extradural PICA origin was observed in 1.6% (175 out of 10,820) of the patients. Fenestration was detected in 0.7% (72 out of 10,820) of the patients. Conclusion The authors summarize the incidence of vascular variation at the suboccipital segment of the VA in different population groups across the Asian, European, American, and African continents. Awareness of the extent of possible anatomical variation will help interpret radiographs, which will enhance the identification of vascular pathologies and reduce the risk of iatrogenic injury.

Abstract:Objective To explore the morphological characteristics and anatomic relationships of vertebral atery in the suboccipital area in order to avoiding the injury of it and its branches during the far lateral approach. Methods Mimicking the far lateral approach,10 adult cadaveric heads with connected neck fixed with 10% formalin were dissected in this study.The suboccipital segment of vertebral artery and related structures were explored and measured under×4.5 loupes magnification.Results The suboccipital segment of vertebral artery was enclosed by either venous plexus or venous sinus.Its length and curve were various significantly.Most of its branches were originated from its medial surface.Conclusions The transverse process of the atlas,the former branch of C2 nerve and the venous plexus enclosing vertebral artery are important landmarks for identification of the vertebral artery during the far lateral approach.Dissecting the vertebral artery from lateral to medial direction can avoid injuring its branches during the far lateral approach.

Abstract:The vertebral artery (VA) is classically divided into V1, V2, V3, and V4 segments. With consideration of the unique anatomy of the vertebral artery, the exposure of different cervical spine pathologies in close proximity of this artery, at each segment, has remained a risky and a challenging issue in spite of progress in surgery. Vertebral artery V2 segment exposure, control and transposition via anterolateral approach may be advised as a part of total spondylectomy for a vertebral tumor. It may also be considered for tumors located in the foramen with extension to the anterior aspect of the neck, such as giant dumbbell schwannomas. Obviously, for this purpose, a thorough knowledge of the surgeon about the anatomy of the vertebral artery in the subaxial cervical spine is necessary.Herein, the steps of V2 segment exposure with description of the tips and the tricks of the procedure will be described in two illustrative cases.

Abstract:Objective To study the microsurgical anatomy marks and parameters for thefar lateral suboccipital approach and to protect the vital structure in operations. Methods Through the far lateral suboccipital approach, 10 adult cadveric heads were anatomized. Under the microscopy, the involving muscles, bony structures, vessels and nerves were observed and measured anatomically. Results The distance from asteria to asteria was (21. 68 ± 1. 88) mm on the left and (22. 34 ± 2. 62) mm on the right. The distance from anterior asteria to mas-toidale was (38. 56 ± 3. 48) mm on the left and (39. 14 ± 2. 24) mm on the right. The distance from asteria to root of zygoma was (55. 72 ± 3. 64) mm on the left and (56. 16 ± 2. 72) mm on the right. Conclusion The suboccipital triangle and C2 nerve were the significant marks which can identify the vertebral artery. The bone anatomic landmarks in the far lateral suboccipital approach included anterior asteria, aste-ria, mastoidale and root of zygoma. These marks contributed the successful implementation of the far lateral suboccipital approach surgery.

Abstract:Vertebral artery-posterior inferior cerebellar artery (VA-PICA) aneurysm is challenging owing to its variable anatomy and interference of lower cranial nerves. A recurrent ruptured VA-PICA aneurysm was successfully treated by occipital (OA)-PICA bypass with aneurysm trapping (Video 1). Informed consent was obtained from the patient. A ruptured right VA aneurysm was found in a 47-year-old man and treated by endovascular treatment. Follow-up angiography at 1 month revealed aneurysm recurrence with migration of endovascular materials. The patient was placed in a park-bench position for the far-lateral approach. A J-shaped incision was used for craniotomy. A 1-piece myocutaneous flap was raised, and the OA was skeletonized in a distal-to-proximal fashion. After craniotomy, the VA-PICA aneurysm with endovascular material inside was located under the vagoaccessory triangle. The tonsillomedullary segment or P3 of the PICA was robust and long enough as the recipient candidate for anastomosis. A linear arteriotomy was performed on the recipient artery, and an end-to-side OA-PICA bypass was achieved. Occlusion of the aneurysm was achieved by placing permanent clips on the VA proximal and distal to the aneurysm and the PICA next to the aneurysm sac without occluding the perforator. The postoperative course was uneventful with bypass patency. We believe this case demonstrates the surgical tenet of OA-PICA bypass for VA-PICA aneurysm.

Abstract:ABSTRACT Streptococcus pneumoniae is a leading cause of invasive bacterial disease. This is the first study to examine the expression of S. pneumoniae genes in vivo by using whole-genome microarrays available from The Institute for Genomic Research. Total RNA was collected from pneumococci isolated from infected blood, infected cerebrospinal fluid, and bacteria attached to a pharyngeal epithelial cell line in vitro. Microarray analysis of pneumococcal genes expressed in these models identified body site-specific patterns of expression for virulence factors, transporters, transcription factors, translation-associated proteins, metabolism, and genes with unknown function. Contributions to virulence predicted for several unknown genes with enhanced expression in vivo were confirmed by insertion duplication mutagenesis and challenge of mice with the mutants. Finally, we cross-referenced our results with previous studies that used signature-tagged mutagenesis and differential fluorescence induction to identify genes that are potentially required by a broad range of pneumococcal strains for invasive disease.
Far-lateral approach without C1 laminectomy for microsurgical treatment of vertebral artery and proximal posterior inferior cerebellar artery aneurysms: Experience from 48 patients

Abstract:Background: In the endovascular era, most of vertebral artery (VA) and posterior inferior cerebellar artery (PICA) aneurysms were mainly treated with endovascular procedures. This study aimed to demonstrate the microsurgical treatment via the far-lateral approach without C1 laminectomy and its clinical outcomes. Methods: Forty-eight patients with VA and proximal PICA aneurysms treated by microsurgery through the far-lateral approach without C1 laminectomy, between January 2016 and June 2021, were retrospectively evaluated. Results: Most patients (87.5%) presented with subarachnoid hemorrhage. Grading at presentation was poor in 41.7%. The rates of VA dissecting aneurysms, saccular aneurysms of the VA-PICA junction, and true PICA saccular aneurysms were 54.2, 18.7, and 14.6%, respectively. All aneurysms were located above the lower margin of the foramen magnum. The far-lateral approach without C1 laminectomy was successfully used in all patients without residual aneurysms. Various surgical strategies were applied depending on the characteristics of the aneurysm. Good outcomes 3 months postoperatively were achieved in 77.1% and 89.3% for the overall and good-grade groups, respectively. Conclusions: Microsurgery is a safe and effective treatment of VA and proximal PICA aneurysms. Moreover, the far-lateral approach without C1 laminectomy was adequate and effective for aneurysms located above the lower border of the foramen magnum.

Abstract:Vertebral artery （VA）Aneurysms involving the origin of the posterior inferior cerebellar artery (PICA) ,occasionally, induce cerebellum and brainstem infarction due to intraluminal thrombus and calcific VA stenosis. At times, vessel occlusion and revascularization is necessary for successful obliteration of these aneurysms.2 The occipital artery (OA) is often the preferred donor graft for lesions of the posterior fossa. Although most OA-PICA bypasses can be performed using the p3 segment as the recipient site for an end-to-side anastomosis, a more feasible alternative to conventional OA-p3 PICA bypass in cases of high-riding caudal loops , aberrant anatomy or p3 multiple perforators is to free the p1 PICA, transpose it away from the lower cranial nerves, and perform an end-to-end OA-p1 PICA bypass instead. This video captures the dissection of the OA using an orientational anterograde harvesting technique and the end-to-end anastomosis of the OA to the PICA at the p1 segment. This was performed in a 56-year-old man who presented with posterior circulation ischemia from a fusiform aneurysm with calcific vertebral artery stenosis located at the origin of the right PICA. The patient tolerated the procedure well and suffered no major complications related to the operation. He did experience some mild, posterior neck rigidity at the time of his 6-month follow-up, likely due to nerve injury that occurred while harvesting the OA. Overall, the patient remains in good neurologic status 1 year after the operation. The operation proved the feasibility of end-to-end bypass in OA-p1 PICA.

Abstract:Objective: We performed a systematic microanatomical study of the occipital artery (OA) and its branches to describe the course, diameter, and branches of this vessel as well as the presence and pattern of anastomotic channels between the OA and the vertebral artery. Methods: Twelve occipital arteries (6 adult cadaveric heads injected with colored latex) were studied using the surgical microscope. Particular attention was given to the course and branching pattern of the artery as well as the presence and type of anastomotic channels between the occipital artery and vertebral artery. Results: Based upon anatomical considerations, the course of the occipital artery was divided into three segments. The first, or digastric segment, extends from the origin to the exit off the occipital groove of the mastoid process. The second segment, or suboccipital, extends from the occipital groove to the superior nuchal line. The third, or terminal segment, corresponds to the subgaleal segment just above the superior nuchal line up to the vertex. Two main descending branches of the second segment or suboccipital were identified. The superficial descending branch (SDB) runs between the splenium capitis and semispinalis capitis while the deep descending branch (DDB) enters the suboccipital triangle. Anastomotic vessels between one of these two descending branches and branches of the vertebral artery were found in 11 out of the 12 OAs dissected (91%). Conclusion: Detailed knowledge of the OA anatomy is helpful in choosing this vessel as a donor for extra-intracranial bypasses. More importantly, knowledge and understanding of the type and pattern of anastomoses between the OA and the vertebral artery are critical to avoid disastrous complications (i.e., posterior circulation stroke) during embolization of vascular or neoplastic processes fed by distal OA branches.
The efficacy of temporary clamping of V3 with a suboccipital far-lateral approach in microvascular decompression for Hemifacial spasm associated with the vertebral artery

Abstract:Hemifacial spasm (HFS) is often caused by compression of the vertebral artery (VA) directly or indirectly as a result of other intervening vessels, so VA-associated HFS is difficult to treat. Recently, we have achieved good surgical outcomes using a far lateral approach and temporary clamping of V3 for VA-associated HFS. Herein, we present our method with an accompanying surgical video. From April 2018 to March 2019, 5 patients with VA-associated HFS underwent surgery, and pre-and postoperative symptoms and postoperative complications were evaluated. In the procedure, the suboccipital muscles were dissected and reflected layer by layer, and the extracranial VA (V3) was secured within the suboccipital triangle. A lateral suboccipital craniotomy followed by far lateral drilling was made to widen the surgical field from the caudolateral side. After reducing the VA flow pressure by temporary clamping of V3, the VA was transposed using a Teflon sling via two triangular space above and below the lower cranial nerves (LCNs). Causative vessels included direct VA compression in two cases and intervening vessels in three cases. The symptoms disappeared in four cases and improved satisfactorily in one case. One patient had mild hearing loss (approximately 10 dB) and hoarseness, but both improved 9 months after surgery. There was no postoperative cerebrospinal fluid leakage in any cases. A wide surgical field via the far lateral approach and the temporary clamping of V3 contributed to thorough observation of the REZ and safe and complete VA transposition.
Use of Proximal Suboccipital Segment of Occipital Artery as Donor for Posterior Circulation Revascularization Surgery: Technical Note

Abstract:Background: The distal suboccipital segment of the occipital artery is commonly used as a donor vessel for aneurysms of the posterior circulation requiring revascularization techniques. Isolating this segment of vessel can be challenging due to its course in and out of the various layers of the suboccipital musculature. Methods: We describe the use of the proximal suboccipital segment of the occipital artery within the occipital groove at the lateral skull base as a donor segment for an interposition graft in revascularization surgery. We present a case detailing the use of this technique for treatment of a dissecting PICA aneurysm and photographs from cadaveric dissections demonstrating relevant anatomy. Results: Our patient is a 62-year-old man who presented with a high-grade subarachnoid hemorrhage from a proximal dissecting-type aneurysm of the posterior inferior cerebellar artery (PICA). Endovascular options were limited to coil sacrifice of the parent vessel, and thus the patient was taken for a far lateral craniotomy, trapping of the aneurysm, and revascularization of the PICA territory via a saphenous vein interposition graft sewn proximally and distally to the occipital artery within the occipital groove and a cortical segment of the affected PICA, respectively. Postoperative imaging demonstrated filling of the PICA territory via the graft, and the patient ultimately recovered from his subarachnoid hemorrhage. Conclusions: The use of the proximal suboccipital segment of the occipital artery is a viable option for a donor in posterior fossa revascularization surgery.
Aberrant drainage of posterior condylar emissary vein and abnormal orifice of hypoglossal canal: surgical implications in the transcondylar fossa approach for VA-PICA junction aneurysm

Abstract:Background: The posterior condylar emissary vein (PCEV) and posterior condylar canal (PCC) are anatomical landmarks for identifying important structures like jugular tubercle and occipital condyle in surgical approach to the foramen magnum and condylar fossa. Several anatomical variations have been described. Drainage into the jugular bulb is found to be commonest. Method: A 70-year-old patient with unruptured vertebral artery-posterior inferior cerebellar artery (PICA) junction aneurysm-underwent surgical clipping via transcondylar fossa approach. Result: Preoperative computed tomography demonstrated an abnormal communication existed between the left-sided PCC and hypoglossal canal (HC). The PCEV was identified draining into a dilated venous channel/pouch at the "hip" of sigmoid sinus (junction of sigmoid sinus and jugular bulb). Intra-operatively, an occipital artery-PICA bypass was performed. The PCEV was skeletonized, coagulated, and divided to achieve hemostasis. The lateral and cranial drilling around PCC was successful at safeguarding the underlying contents of HC (in medial and caudal extent). Conclusion: Preoperative angiography and detailed morphometric analysis of the PCC were helpful in planning surgical approach-identifying and controlling the PCEV, and skeletonization of the PCC without compromising the hypoglossal nerve and anterior condylar emissary vein.

Abstract:The lateral spinal artery (LSA) represents the most rostral extent of the posterolateral arterial axis of the spinal cord and supplies both the posterior and lateral aspects of the spinal cord. 1 The LSA originates from either the intradural segment of vertebral artery or the lateral segment of the posterior inferior cerebellar artery. 1,2 It terminates at the level of C5 because it anastomoses with the posterior spinal artery. Although the LSA is in communication with the subarachnoid space, subarachnoid hemorrhage (SAH) due to an LSA aneurysm is rare. 3-6 Moreover, accurate diagnosis of vascular pathologies involving the LSA is challenging because the LSA is typically too small to be visualized with conventional angiography. 2 Here, we present the case of a 74-year-old woman who presented to our institution with headache, nausea, and confusion due to posterior fossa SAH without any visible vascular malformation on initial diagnostic cerebral angiography. A right frontal external ventricular drain was placed for communicating hydrocephalus. Immediate cerebral angiogram was within normal limits. Two-week follow-up angiogram demonstrated a fusiform right LSA aneurysm. The endovascular treatment was not an option because of the risk of occlusion of proximal perforators of the right posterior spinal artery, resulting in a long segment ischemia. The patient underwent a right far lateral transcondylar approach with C1 hemilaminotomy and excision of the LSA aneurysm. The postoperative course was uneventful. The patient was neurologically intact. Six-month follow-up angiogram showed no evidence of residual aneurysm. LSA pathologies should be kept in mind with posterior fossa SAH. Surgical excision is an effective treatment option in carefully selected patients. The patient consented to the procedure.

Abstract:Study Design. Observational study with 3-dimensional computed tomography angiography analysis. Objective. To examine the course of the vertebral artery (VA) at the craniovertebral junction (CVJ) in individuals with occipitalization of the atlas. Summary of Background Data. The anatomy of the VA at the CVJ should be completely understood to decrease the risk of iatrogenic injury. Although quantitative anatomic studies have focused on the normal VA, the anomalous VA with occipitalization of the atlas has not been fully explored. Methods. A consecutive series of 36 cases with occipitalization of the atlas underwent 3-dimensional computed tomography angiography. Seventy-two vertebral arteries were analyzed. In this setting, the safety of placing lateral mass screws (LMS) was studied. Results. Four different pathways of the VA at the CVJ with occipitalization of the atlas were found. Type I, wherein the VA enters the spinal canal below the C1 posterior arch, and the course of the VA is below the occipitalized C1 lateral mass (8.3% of 72 vertebral arteries); Type II, the VA enters the spinal canal below the C1 posterior arch, and the course of the VA is on the posterior surface of the occipitalized C1 lateral mass, or makes a curve on it (25%); Type III, wherein the VA ascends externally laterally after leaving the axis transverse foramen, enters an osseous foramen created between the atlas and occipital bone, then into the cranium (61.1%); and Type IV, in which the VA is absent (5.6%). Conclusion. Four types of VA with occipitalization of the atlas are confirmed. Type-I and type-IV VA have relatively low risks for C1 LMS perforation. Type-II and type-III anomalies will probably increase the risk of VA injury during C1 LMS placement. Abstract:Dielectric measurements have been carried out using an impedance analyzer for two ferroelectric liquid crystals R-4' (1-methoxycarbonyl-1-ethoxy) phenyl 4-(4-octyloxy phenyl) benzoate (1MC1EPOPB) and R-4' (1-butoxycarbonyl-1-ethoxy) phenyl 4-(4-octyloxy phenyl) benzoate (1BC1EPOPB). The two types have large spontaneous polarization, +1700 /spl mu/C/m/sup 2/ for 1MC1EPOPB and +2400 /spl mu/C/m/sup 2/ for 1BC1EPOPB. The permittivity and dielectric loss have been measured at different temperatures in the range 343.0 K to 383.0 K for 1MC1EPOPB and 318.0 K to 353.0 K for 1BC1EPOPB in the frequency range 2 Hz to 2 MHz. Both of the ferroelectric liquid crystals, 1MC1EPOPB and 1BC1EPOPB show a new phase smectic X along with smectic C* and smectic A phases. The work reported in this paper is new and is very useful in understanding their application in switching devices.
Clinical characteristics, endovascular choices, and surgical outcomes of intracranial vertebral artery dissecting aneurysms: a consecutive series of 196 patients

Abstract:Objective: The authors aimed to evaluate the clinical features, endovascular strategy selection, and outcomes of vertebral artery (VA) dissecting aneurysms (VADAs) near the origin of the posterior inferior cerebellar artery (PICA). Methods: Clinical data were obtained from 196 patients with 202 VADAs who had been surgically treated at the authors' hospital between 2005 and 2021. The patients' medical records, imaging data, and follow-up outcomes were retrospectively analyzed. Results: All 196 patients (148 male, 48 female; mean age 53 years) underwent endovascular therapy for VADA. The most common chief complaints were headache (56.6%), dizziness (40.8%), and other occasional symptoms (18.4%). Twenty-six patients experienced posterior circulation ischemia, and 25 patients experienced subarachnoid hemorrhage (SAH). On DSA images, 37 aneurysms were found in the dominant VA, and 165 aneurysms were found on the nondominant side. Eighty-eight VADAs had stenosis. Regarding the VADA location, there were 59 distal, 51 proximal, 23 ventral, 64 PICA, and 5 obliteration types. Single-stent implantation (10.9%), overlapping stent implantation (23.8%), flow diverter (FD) implantation (11.9%), single stent-assisted coil (SAC) embolization (31.7%), overlapping SAC embolization (12.9%), parent artery occlusion (PAO) (8.4%), and FD-assisted coil embolization (0.5%) were chosen. The follow-up rate was 82.7%. The modified Rankin Scale (mRS) score was 0.5 ± 1.1. Ninety-six patients underwent DSA. The complete occlusion rates were 100%, 81.4%, 56.7%, and 76.5% in the PAO group, SAC reconstruction group, stent implantation group, and FD implantation group, respectively. Conclusions: Young patients showed a higher probability of SAH, and elderly patients showed a higher probability of posterior circulation infarction. The surgical plan selection should be based on the SAH history, VA dominant side, and PICA origin location.
