Abstract:Summary Bone marrow cell proliferation was studied in Syrian hamsters treated with high doses of cortisone. Mitotic index after metaphase arrest of dividing cells was used to evaluate cell proliferation changes. Vinblastine was employed as the metaphasearresting agent. Within a few hours following the administration of a high dose of cortisone there was marked reduction of bone marrow cell proliferation. The lowest value of mitotic index was observed at the 12th hour. At the 24th hour the mitotic index increased somewhat above the normal and then returned to normal. The scoring of mitoses separately in the myeloid and erythroid series showed that mitotic activity was depressed in both series during the first 12–18 hours after administration of high doses of cortisone.
Performance of Machine Learning Algorithms for Predicting Disease Activity in Inflammatory Bowel Disease

Abstract:This study aimed to explore the effectiveness of predicting disease activity in patients with inflammatory bowel disease (IBD), using machine learning (ML) models. A retrospective research was undertaken on IBD patients who were admitted into the First Affiliated Hospital of Wenzhou Medical University between September 2011 and September 2019. At first, data were randomly split into a 3:1 ratio of training to test set. The least absolute shrinkage and selection operator (LASSO) algorithm was applied to reduce the dimension of variables. These variables were used to generate seven ML algorithms, namely random forests (RFs), adaptive boosting (AdaBoost), K-nearest neighbors (KNNs), support vector machines (SVMs), naïve Bayes (NB), ridge regression, and eXtreme gradient boosting (XGBoost) to train to predict disease activity in IBD patients. SHapley Additive exPlanation (SHAP) analysis was performed to rank variable importance. A total of 876 participants with IBD, consisting of 275 ulcerative colitis (UC) and 601 Crohn's disease (CD), were retrospectively enrolled in the study. Thirty-three variables were obtained from the clinical characteristics and laboratory tests of the participants. Finally, after LASSO analysis, 11 and 5 variables were screened out to construct ML models for CD and UC, respectively. All seven ML models performed well in predicting disease activity in the CD and UC test sets. Among these ML models, SVM was more effective in predicting disease activity in the CD group, whose AUC reached 0.975, sensitivity 0.947, specificity 0.920, and accuracy 0.933. AdaBoost performed best for the UC group, with an AUC of 0.911, sensitivity 0.844, specificity 0.875, and accuracy 0.855. ML algorithms were available and capable of predicting disease activity in IBD patients. Based on clinical and laboratory variables, ML algorithms demonstrate great promise in guiding physicians' decision-making.

Abstract:Although identifying high-cost inpatients with ischemic heart disease (IHD) at the point of admission is helpful for timely intervention and reducing costs, it is a difficult task due to the limited data available at such an early stage of treatment. The aim of this study was to tackle this challenge by integrating network analytics with machine learning. In this study, we first constructed two networks including Phenotypic Comorbidity Network and Distance-based Disease-Cost Network based on the hospital discharge records (HDR) of Chengdu, China during 2015–2017. Based on the two networks, three novel network features were generated to capture the potential relationships between comorbidities and high healthcare costs. Six machine learning models (Logistic Regression, Decision Tree, Neural Network, Random Forest, XGBoost, and LightGBM) with different input features (network and non-network features) were developed to compare their performance using HDR of 323,907 admissions for IHD between 2018 and 2019. Our results showed that LightGBM with the input feature set including network and non-network features achieved the best area under the receiver-operating characteristic curve (AUC) of 0.900. Network features were the most significant features accounting for the majority (47.4%) of feature importance. Models using network features provided more accurate prediction compared with those using only non-network features. In conclusion, the proposed approach combining network analysis and machine learning is useful for predicting high-cost inpatients with IHD at an early stage of admission, which highlights the potential value of developing and disseminating network-based machine learning in healthcare field.

Predicting 30-day Hospital Readmission with Publicly Available Administrative Database. A Conditional Logistic Regression Modeling Approach

Abstract:Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on "Big Data and Analytics in Healthcare". Background: Hospital readmissions raise healthcare costs and cause significant distress to providers and patients. It is, therefore, of great interest to healthcare organizations to predict what patients are at risk to be readmitted to their hospitals. However, current logistic regression based risk prediction models have limited prediction power when applied to hospital administrative data. Meanwhile, although decision trees and random forests have been applied, they tend to be too complex to understand among the hospital practitioners. Objectives: Explore the use of conditional logistic regression to increase the prediction accuracy. Methods: We analyzed an HCUP statewide inpatient discharge record dataset, which includes patient demographics, clinical and care utilization data from California. We extracted records of heart failure Medicare beneficiaries who had inpatient experience during an 11-month period. We corrected the data imbalance issue with under-sampling. In our study, we first applied standard logistic regression and decision tree to obtain influential variables and derive practically meaning decision rules. We then stratified the original data set accordingly and applied logistic regression on each data stratum. We further explored the effect of interacting variables in the logistic regression modeling. We conducted cross validation to assess the overall prediction performance of conditional logistic regression (CLR) and compared it with standard classification models. Results: The developed CLR models outperformed several standard classification models (e.g., straightforward logistic regression, stepwise logistic regression, random forest, support vector machine). For example, the best CLR model improved the classification accuracy by nearly 20% over the straightforward logistic regression model. Furthermore, the developed CLR models tend to achieve better sensitivity of more than 10% over the standard classification models, which can be translated to correct labeling of additional 400 - 500 readmissions for heart failure patients in the state of California over a year. Lastly, several key predictor identified from the HCUP data include the disposition location from discharge, the number of chronic conditions, and the number of acute procedures. Conclusions: It would be beneficial to apply simple decision rules obtained from the decision tree in an ad-hoc manner to guide the cohort stratification. It could be potentially beneficial to explore the effect of pairwise interactions between influential predictors when building the logistic regression models for different data strata. Judicious use of the ad-hoc CLR models developed offers insights into future development of prediction models for hospital readmissions, which can lead to better intuition in identifying high-risk patients and developing effective post-discharge care strategies. Lastly, this paper is expected to raise the awareness of collecting data on additional markers and developing necessary database infrastructure for larger-scale exploratory studies on readmission risk prediction.
Machine learning prediction of hospitalization costs for coronary artery bypass grafting operations

Abstract:Background With the steady rise in health care expenditures, the examination of factors that may influence the costs of care has garnered much attention. Although machine learning models have previously been applied in health economics, their application within cardiac surgery remains limited. We evaluated several machine learning algorithms to model hospitalization costs for coronary artery bypass grafting. Methods All adult hospitalizations for isolated coronary artery bypass grafting were identified in the 2016 to 2020 Nationwide Readmissions Database. Machine learning models were trained to predict expenditures and compared with traditional linear regression. Given the significance of postoperative length of stay, we additionally developed models excluding postoperative length of stay to uncover other drivers of costs. To facilitate comparison, machine learning classification models were also trained to predict patients in the highest decile of costs. Significant factors associated with high cost were identified using SHapley Additive exPlanations beeswarm plots. Results Among 444,740 hospitalizations included for analysis, the median cost of hospitalization in coronary artery bypass grafting patients was $43,103. eXtreme Gradient Boosting most accurately predicted hospitalization costs, with R 2 = 0.519 over the validation set. The top predictive features in the eXtreme Gradient Boosting model included elective procedure status, prolonged mechanical ventilation, new-onset respiratory failure or myocardial infarction, and postoperative length of stay. After removing postoperative length of stay, eXtreme Gradient Boosting remained the most accurate model ( R 2 = 0.38). Prolonged ventilation, respiratory failure, and elective status remained important predictive parameters. Conclusion Machine learning models appear to accurately model total hospitalization costs for coronary artery bypass grafting. Future work is warranted to uncover other drivers of costs and improve the value of care in cardiac surgery.

Abstract:Abstract Background The incidence of inflammatory bowel disease (IBD) continues to rise rapidly among Canadian children. The care of children results in higher direct healthcare costs than adults with IBD. It is imperative that we identify individuals who will become the highest-cost users of the health system in order to intervene early and decrease the individual- and system-level burden of IBD. Purpose To develop a predictive-model for high-cost health system users and (2) identify factors associated with high-cost healthcare use. Method Incident cases of IBD diagnosed ≤17y residing in Ontario and enrolled in the Canadian Children IBD Network (CIDsCaNN) between Dec 31 2013 and Jan 31 2019 were linked deterministically using health card number to health administrative data. Using a validated algorithm, direct healthcare costs accumulated between the 31st and 365th day after diagnosis were calculated using data from CIDsCaNN (medications) and health administrative data (health system encounters, including surgery). A predictive model was created to determine high-cost (≤25th percentile) and medium-cost (26th to 75th) users, compared to low-cost users (>75th) using ordinal logistic regression. Potential predictive variables were determined a priori based on clinical significance and magnitude of univariable association, based on sample size-informed degrees of freedom. Variables from CIDsCaNN data included: IBD type, age at diagnosis, sex, first line of therapy (steroids, aminosalicylates, exclusive enteral nutrition; yes or no, not mutually exclusive), disease activity (severe vs. moderate vs. none/mild based on PUCAI [UC] or wPCDAI [Crohn’s]). Predictive variables from the health administrative data included: rural/urban residence, hospitalization at diagnosis, intestinal resection or colectomy within 3 months of diagnosis, emergency department visit ±1 month of diagnosis, and a mental health encounter within the first year following diagnosis. Anti-TNF treatment was excluded from models due to the strong correlation with the outcome (direct costs). Overall model fit was estimated with a c-statistic. Result(s) Among the 487 (57% Crohn’s) children included in the study, the mean (sd) direct costs accumulated between the 31st and 365th days following IBD diagnosis was $14,451 (14,665). The mean cost among high-cost users was $33,533 (16,530); medium-cost users, $11,038 (5322); low-cost users, $2530 (831). The predictive model identified high-cost users of the health system with acceptable model fit (c-statistic 0.69). The relative contribution of individual variables, as measured by odds ratio (OR), is reported in the Table. Image Conclusion(s) The direct healthcare costs of pediatric IBD are substantial. Children with IBD who become high-cost users of the health system were identifiable using characteristics at diagnosis (e.g., need for mental health care, emergency visits, older age). Abstract:11084 Background: Integra Connect previously created in-patient (IP) admission prediction model based on OCM data. One of the challenges for practices in a value-based care (VBC) program is to provide continuous care-coordination during and after an IP admission. Our objective is to show how adopting combination machine learning (ML) models can predict IP length of stay (LOS). Potential benefits include an overall reduction in LOS which could minimize the risk of hospital acquired conditions. Methods: ML models were trained on 5 major cancer types (Lung Cancer, Multiple Myeloma, Lymphoma, Small Intestine / Colorectal Cancer, High-risk Breast Cancer') from 7 OCM practices of PP4-PP9 data, excluding surgery IP admission. The top 5 cancer types accounted for ~50% of total in-patient admissions and ~50% of total LOS in days. The IP admissions were divided into 4 major cohorts in terms of LOS in days (1-3: class 1, 4-8: class 2, 9-15: class 3, and more than 15: class 4). To reduce the overall LOS, we adopted two ML models: 1) To classify LOS, a multi-classification model (Model1, an eXtreme Gradient Boosted Trees Classifier) and 2) To predict the LOS for classes 1-3, a regression-based model (Model2, an eXtreme Gradient Boosted Trees Regressor with Early Stopping). Class 4 cohort had a wide range (16- 90+ days) for LOS and therefore was excluded from Model2. The other three cohorts belonged to Q1, IQR, and Q4, respectively. Model1 was trained on 4,280 randomly selected sample IP admissions to balance each class and Model2 was trained on 19,636 IP admissions, both with 102 features. Results: The models were tested on PP10-PP11 claims excluding 0 days LOS. Model1 predicted 296 (3,945) IP admissions in class 4 of which 88 were true positives. Assuming at least a 10% reduction in associated total LOS translated to lower LOS of 1-3% for 5 (7) practices. The remaining 3,649 IP admissions were used to predict LOS by Model2. The difference between actual LOS and Predicted LOS was found as 18%-24%. To address model prediction errors, we defined the target LOS to be predicted LOS with an upper bound of at least 10%. This led to reductions of 6% -16% across practices. Finally, combining the results of both ML models we determined that the potential to lower LOS was at least 6% and at most 19%. Conclusions: Our ML models identified opportunities to reduce LOS across multiple OCM cancer practices for 5 major cancer types. We also identified a cohort of patients with critical condition (class 4) that is vital for practice transformation initiatives. These identified reduction in LOS for oncology patients provides cost reduction and quality improvement opportunities in VBC programs. In the future, more opportunities to lower LOS will be explored with a re-admission prediction model.

Machine Learning-Based Prediction for High Health Care Utilizers by Using a Multi-Institutional Diabetes Registry: Model Training and Evaluation

Abstract:Background: The cost of health care in many countries is increasing rapidly. There is a growing interest in using machine learning for predicting high health care utilizers for population health initiatives. Previous studies have focused on individuals who contribute to the highest financial burden. However, this group is small and represents a limited opportunity for long-term cost reduction. Objective: We developed a collection of models that predict future health care utilization at various thresholds. Methods: We utilized data from a multi-institutional diabetes database from the year 2019 to develop binary classification models. These models predict health care utilization in the subsequent year across 6 different outcomes: patients having a length of stay of ≥7, ≥14, and ≥30 days and emergency department attendance of ≥3, ≥5, and ≥10 visits. To address class imbalance, random and synthetic minority oversampling techniques were employed. The models were then applied to unseen data from 2020 and 2021 to predict health care utilization in the following year. A portfolio of performance metrics, with priority on area under the receiver operating characteristic curve, sensitivity, and positive predictive value, was used for comparison. Explainability analyses were conducted on the best performing models. Results: When trained with random oversampling, 4 models, that is, logistic regression, multivariate adaptive regression splines, boosted trees, and multilayer perceptron consistently achieved high area under the receiver operating characteristic curve (>0.80) and sensitivity (>0.60) across training-validation and test data sets. Correcting for class imbalance proved critical for model performance. Important predictors for all outcomes included age, number of emergency department visits in the present year, chronic kidney disease stage, inpatient bed days in the present year, and mean hemoglobin A1c levels. Explainability analyses using partial dependence plots demonstrated that for the best performing models, the learned patterns were consistent with real-world knowledge, thereby supporting the validity of the models. Conclusions: We successfully developed machine learning models capable of predicting high service level utilization with strong performance and valid explainability. These models can be integrated into wider diabetes-related population health initiatives.
Applying Machine Learning Models Derived From Administrative Claims Data to Predict Medication Nonadherence in Patients Self-Administering Biologic Medications for Inflammatory Bowel Disease

Abstract:Background: Adherence to self-administered biologic therapies is important to induce remission and prevent adverse clinical outcomes in Inflammatory bowel disease (IBD). This study aimed to use administrative claims data and machine learning methods to predict nonadherence in an academic medical center test population. Methods: A model-training dataset of beneficiaries with IBD and the first unique dispense of a self-administered biologic between June 30, 2016 and June 30, 2019 was extracted from the Commercial Claims and Encounters and Medicare Supplemental Administrative Claims Database. Known correlates of medication nonadherence were identified in the dataset. Nonadherence to biologic therapies was defined as a proportion of days covered ratio <80% at 1 year. A similar dataset was obtained from a tertiary academic medical center's electronic medical record data for use in model testing. A total of 48 machine learning models were trained and assessed utilizing the area under the receiver operating characteristic curve as the primary measure of predictive validity. Results: The training dataset included 6998 beneficiaries (n = 2680 nonadherent, 38.3%) while the testing dataset included 285 patients (n = 134 nonadherent, 47.0%). When applied to test data, the highest performing models had an area under the receiver operating characteristic curve of 0.55, indicating poor predictive performance. The majority of models trained had low sensitivity and high specificity. Conclusions: Administrative claims-trained models were unable to predict biologic medication nonadherence in patients with IBD. Future research may benefit from datasets with enriched demographic and clinical data in training predictive models.

Abstract:Inflammatory bowel disease (IBD) is a chronic, incurable disease involving the gastrointestinal tract. It is characterized by complex, unclear pathogenesis, increased prevalence worldwide, and a wide spectrum of extraintestinal manifestations and comorbidities. Recognition of IBD remains challenging and delays in disease diagnosis still poses a significant clinical problem as it negatively impacts disease outcome. The main diagnostic tool in IBD continues to be invasive endoscopy. We aimed to create an IBD machine learning prediction model based on routinely performed blood, urine, and fecal tests. Based on historical patients’ data (702 medical records: 319 records from 180 patients with ulcerative colitis (UC) and 383 records from 192 patients with Crohn’s disease (CD)), and using a few simple machine learning classificators, we optimized necessary hyperparameters in order to get reliable few-features prediction models separately for CD and UC. Most robust classificators belonging to the random forest family obtained 97% and 91% mean average precision for CD and UC, respectively. For comparison, the commonly used one-parameter approach based on the C-reactive protein (CRP) level demonstrated only 81% and 61% average precision for CD and UC, respectively. Results of our study suggest that machine learning prediction models based on basic blood, urine, and fecal markers may with high accuracy support the diagnosis of IBD. However, the test requires validation in a prospective cohort.

Comparison of Machine Learning Algorithms for Predicting Hospital Readmissions and Worsening Heart Failure Events in Patients With Heart Failure With Reduced Ejection Fraction: Modeling Study

Abstract:Background: Heart failure (HF) is highly prevalent in the United States. Approximately one-third to one-half of HF cases are categorized as HF with reduced ejection fraction (HFrEF). Patients with HFrEF are at risk of worsening HF, have a high risk of adverse outcomes, and experience higher health care use and costs. Therefore, it is crucial to identify patients with HFrEF who are at high risk of subsequent events after HF hospitalization. Objective: Machine learning (ML) has been used to predict HF-related outcomes. The objective of this study was to compare different ML prediction models and feature construction methods to predict 30-, 90-, and 365-day hospital readmissions and worsening HF events (WHFEs). Methods: We used the Veradigm PINNACLE outpatient registry linked to Symphony Health's Integrated Dataverse data from July 1, 2013, to September 30, 2017. Adults with a confirmed diagnosis of HFrEF and HF-related hospitalization were included. WHFEs were defined as HF-related hospitalizations or outpatient intravenous diuretic use within 1 year of the first HF hospitalization. We used different approaches to construct ML features from clinical codes, including frequencies of clinical classification software (CCS) categories, Bidirectional Encoder Representations From Transformers (BERT) trained with CCS sequences (BERT + CCS), BERT trained on raw clinical codes (BERT + raw), and prespecified features based on clinical knowledge. A multilayer perceptron neural network, extreme gradient boosting (XGBoost), random forest, and logistic regression prediction models were applied and compared. Results: A total of 30,687 adult patients with HFrEF were included in the analysis; 11.41% (3184/27,917) of adults experienced a hospital readmission within 30 days of their first HF hospitalization, and nearly half (9231/21,562, 42.81%) of the patients experienced at least 1 WHFE within 1 year after HF hospitalization. The prediction models and feature combinations with the best area under the receiver operating characteristic curve (AUC) for each outcome were XGBoost with CCS frequency (AUC=0.595) for 30-day readmission, random forest with CCS frequency (AUC=0.630) for 90-day readmission, XGBoost with CCS frequency (AUC=0.649) for 365-day readmission, and XGBoost with CCS frequency (AUC=0.640) for WHFEs. Our ML models could discriminate between readmission and WHFE among patients with HFrEF. Our model performance was mediocre, especially for the 30-day readmission events, most likely owing to limitations of the data, including an imbalance between positive and negative cases and high missing rates of many clinical variables and outcome definitions. Conclusions: We predicted readmissions and WHFEs after HF hospitalizations in patients with HFrEF. Features identified by data-driven approaches may be comparable with those identified by clinical domain knowledge. Future work may be warranted to validate and improve the models using more longitudinal electronic health records that are complete, are comprehensive, and have a longer follow-up time.

Abstract:Abstract Background Personalised medicine approaches are eagerly awaited to facilitate individualisation of medical care for patients with inflammatory bowel disease (IBD). Multiple approaches have already been explored in attempts to stratify patients into different prognostic trajectories. In this study we aimed to use machine learning algorithms to cluster patients based on their routinely collected electronic health records in an unsupervised approach. Methods We interrogated the rich electronic health records (EHR) of 619 patients with a diagnosis of IBD requiring an advanced therapy and attending the IBD service at King’s College Hospital (KCH), London, UK. Data for a seven year period (June 2016-May 2023) were included with variables that captured diagnosis, treatments, service utilisation and investigations. Ethical approval was provided following application to the KERRI committee (KCH committee for accessing anonymised HER). In a preliminary analysis, we employed a multi-step approach to analyse patient data, identify significant features through k-prototypes clustering and a combination of parametric and non-parametric statistical tests. We examined different cluster solutions up to six clusters. Results We included 38 quantitative and 32 qualitative variables for 350 patients with a diagnosis of CD and 269 with a diagnosis of UC. Missing data were addressed by imputation by k-NN, mode value and zero. In the context of the unsupervised learning approach, when k=3, key differentiating features included faecal calprotectin levels, body mass index, mean platelet volume, marital status, taking advanced therapy for less than 3 years, and service access (telephone or clinic appointments). Conclusion This first pass analysis suggests that patients who are relatively early in their journey with at least moderately active IBD take up more recourses of the IBD service. This associates also with their social circumstances. Further work will explore these interactions in more detail.

Abstract:Abstract Background Treatment of IBD has improved with the introduction of biologics and small molecules, yet this has come with a considerable increase in healthcare costs. Due to the increasing cost burden, reliable nationwide epidemiological data on the prevalence of IBD is necessary to inform health policy makers, especially as the prevalence of IBD is forecasted to double between 2010 and 2030. We aimed to develop a model for identifying prevalent IBD cases in administrative data and to determine prevalence of IBD in the Netherlands. Methods Data on hospital care came from the Dutch National Hospital Care Basic Registration (Landelijke Basisregistratie Ziekenhuiszorg). This database contains data on all hospital admissions (since 1991), outpatient clinic visits (since 2017), and dispensations of biologics and small molecules (since 2015) of all hospitals in the Netherlands. Data on pathology reports were retrieved from the nationwide network and registry of histo- and cytopathology in the Netherlands (PALGA), this database contains coded pathology reports of all Dutch hospitals since 1991. These datasets were combined with a reference cohort with a verified IBD diagnosis (yes/no) and demographics for all patients. Models were trained to optimise the F-score and evaluated using five-times repeated ten-fold cross-validation. The best performing model from cross-validation was applied to assess IBD prevalence in the Netherlands. Results The reference cohort consisted of 10,155 patients, of which 3,381 were diagnosed with IBD. All models performed well in the cross-validation procedure, with F-scores of 0.870 and higher. The use of more flexible models led to improved performance, with gradient boosted trees performing best in the cross-validation procedure (Table 1). When applying the gradient boosted trees model to the general population, a prevalence of 691 per 100,000 was found for IBD in the Netherlands on 31-12-2020. Cases are unevenly distributed throughout the Netherlands (Figure 1), with the highest incidence in the south (Middle Limburg: 936 per 100,000 inhabitants) and the lowest in the northwest (Amsterdam: 545). Conclusion Prevalent IBD cases can be identified from administrative data using a gradient boosted trees model. Using this model, we have shown that prevalence of IBD in the Netherlands is increasing. However, while prior studies have predicted a growth in prevalence of 50% over the last 10 years, we found that IBD prevalence has only increased by 12% in that time range. The lower increase in prevalence may be predictive of a transition to the fourth epidemiological stage of IBD, aptly named Prevalence Equilibrium.

Abstract:Introduction Heart Failure (HF) may induce bowel hypoperfusion, leading to hypoxia of the villa of the bowel wall and the occurrence of Clostridioides difficile infection (CDI). However, the risk factors for the development of CDI in HF patients have yet to be fully illustrated, especially because of a lack of evidence from real-world data. Methods: Clinical data and survival situations of HF patients with CDI admitted to ICU were extracted from the Medical Information Mart for Intensive Care (MIMIC)-IV database. For developing a model that can predict 28-day all-cause mortality in HF patients with CDI, the Recursive Feature Elimination with Cross-Validation (RFE-CV) method was used for feature selection. And nine machine learning (ML) algorithms, including logistic regression (LR), decision tree (DT), bayesian, adaptive boosting (AdaBoost), random forest (RF), gradient boosting decision tree (GBDT), XGBoost, light gradient boosting machine (LightGBM), and categorical boosting (CatBoost) were applied for model construction. After training and hyperparameter optimization of the models through grid search 5-fold cross-validation, the performance of models was evaluated by the area under curve (AUC), accuracy, sensitivity, specificity, precision, negative predictive value, and F1 score. Furthermore, the SHapley Additive exPlanations (SHAP) method was used to interpret the optimal model. Results: A total of 526 HF patients with CDI were included in the study, of whom 99 cases (18.8%) experienced death within 28 days. 18 of the 57 variables were selected for the model construction algorithm for model construction. Among the ML models considered, the RF model emerged as the optimal model achieving the accuracy, F1-score, and AUC values of 0.821, 0.596, and 0.864 respectively. The net benefit of the model surpassed other models at 16%~22% threshold probabilities based on decision curve analysis. According to the importance of features in the RF model, red blood cell distribution width, blood urea nitrogen, Simplified Acute Physiology Score II, Sequential Organ Failure Assessment, and white blood cell count were highlighted as the five most influential variables. Conclusions: We developed ML models to predict 28-day all-cause mortality in HF patients associated with CDI in the ICU, which are more effective than the conventional logistic regression model. The RF model has the best performance among all the ML models employed. It may be useful to help clinicians identify high-risk HF patients with CDI.

A double-acting cylinder has a rod thereof connected to the first section and a housing thereof connected to the second section. A first pulley is rotatably mounted on a forward end of the cylinder housing and a second pulley, having a larger diameter than the first pulley, is rotatably mounted on a rearward end of the cylinder housing. A first end of a cable is secured to the first section and is sequentially entrained over the first pulley, secured to the third section, entrained over the second pulley and has a second end thereof secured to a forward end of the first section. Thus, extension of the cylinder will function to extend the second section relative to the first section and simultaneously extend the third section at twice the linear speed as that of the second section. The fourth section of the boom is adapted to have a rearward end thereof connected to a forward end of the third section to substantially increase the overall working length thereof.

Abstract:Hospital readmissions are expensive and reflect the inadequacies in healthcare system. In the United States alone, treatment of readmitted diabetic patients exceeds 250 million dollars per year. Early identification of patients facing a high risk of readmission can enable healthcare providers to to conduct additional investigations and possibly prevent future readmissions. This not only improves the quality of care but also reduces the medical expenses on readmission. Machine learning methods have been leveraged on public health data to build a system for identifying diabetic patients facing a high risk of future readmission. Number of inpatient visits, discharge disposition and admission type were identified as strong predictors of readmission. Further, it was found that the number of laboratory tests and discharge disposition together predict whether the patient will be readmitted shortly after being discharged from the hospital (i.e. <30 days) or after a longer period of time (i.e. >30 days). These insights can help healthcare providers to improve inpatient diabetic care. Finally, the cost analysis suggests that \$252.76 million can be saved across 98,053 diabetic patient encounters by incorporating the proposed cost sensitive analysis model.

Abstract:Introduction: Despite significant therapeutic advancements, Atherosclerotic Cardiovascular Disease (ASCVD) patients require frequent hospitalization. Machine learning (ML) algorithms present an opportunity to develop improved and more generalizable prediction models for 30-day hospital readmission due to ASCVD. Objectives: The current study aims to leverage electronic health record (EHR) data to predict 30-day hospital readmission in high-risk ASCVD patients. Methods: This study utilized the MIMIC III database (a publicly available large, single-centre critical care database of 46,520 patients) comprising of deidentified ~60,000 ICU admissions. Adult patients with the first admission due to ASCVD event and whose length of stay (LOS) in the ICU was >48 hours were included. To develop a prediction model, features representing groups of diagnosis data along with demographics and length of stay in the hospital were used. The performance of ML models was evaluated using the area under the receiver operating characteristic curves (AUCs). Results: Our cohort consisted of 22,666 admissions (mean age 70.1 years) due to an ASCVD event. Out of the 9022 readmissions within 180 days, 60.5% were observed within 30 days of discharge. Among the 17 prospective predictors age, LOS and first hospitalization due to coronary atherosclerosis, heart failure, and haemorrhage issues were the most important factors to predict readmission within 30 days (Fig A). Random Forest was the best performing model with an AUC of 0.66 (Fig B). Conclusions: This model can be helpful in predicting the readmission of high-risk patients after the first ASCVD event which might address the huge unmet need of aiding healthcare resource planning, better patient care, and prevention of rehospitalization and death by intensifying treatment interventions in those patients and taking appropriate decisions to discharge patients based on their age, LOS and ASCVD events resulting in first hopitalization.

Abstract:Background: Improved survival of patients after acute coronary syndromes, population growth, and overall life expectancy rise have led to a significant increase in the proportion of patients with stable coronary artery disease (CAD), creating a significant load on the entire healthcare system. The disease often progresses with the development of many complications while significantly increasing the likelihood of hospitalization. Developing and applying a machine learning model for predicting hospitalizations of patients with CAD to an inpatient medical facility will allow for close monitoring of high-risk patients, early preventive interventions, and optimized medical care. Aims: Development and external validation of personalized models for predicting the preventable hospitalizations of patients with stable CAD and its complications using ML algorithms and data of real-world clinical practice. Methods: 135,873 depersonalized electronic health records of 49,103 patients with stable CAD were included in the study. Anthropometric measurements, physical examination results, laboratory, instrumental, anamnestic, and socio-demographic data, widely used in routine medical practice, were considered as potential predictors, a total of 73 features. Logistic regression, decision tree-based methods including gradient boosting (AdaBoost, LightGBM, XGBoost, CatBoost) and bagging (RandomForest and ExtraTrees), discriminant analysis (LinearDiscriminant, QuadraticDiscriminant), and naive Bayes classifier were compared. External validation was performed on the data of a separate region. Results: The best results and stability to external validation data were shown by the CatBoost model with an AUC of 0.875 (95% CI 0.865-0.885) for the internal testing and 0.872 (95% CI 0.856-0.886) for the external validation. The best model showed good performance evaluated through AUROC, Brier score and standardized net benefit (for the target NPV threshold) for the validation dataset that was only slightly similar to the train data. Conclusion: The metrics of the best model were superior to previously published studies. The results of external validation demonstrated the relative stability of the model to new data from another region that confirms the possibility of the model's application in real clinical practice.

Abstract:Background Existing readmission reduction solutions tend to focus on complementing inpatient care with enhanced care transition and postdischarge interventions. These solutions are initiated near or after discharge, when clinicians’ impact on inpatient care is ending. Preventive intervention during hospitalization is an underexplored area that holds potential for reducing readmission risk. However, it is challenging to predict readmission risk at the early stage of hospitalization because few data are available. Objective The objective of this study was to build an early prediction model of unplanned 30-day hospital readmission using a large and diverse sample. We were also interested in identifying novel readmission risk factors and protective factors. Methods We extracted the medical records of 96,550 patients in 205 participating Cerner client hospitals across four US census regions in 2016 from the Health Facts database. The model was built with index admission data that can become available within 24 hours and data from previous encounters up to 1 year before the index admission. The candidate models were evaluated for performance, timeliness, and generalizability. Multivariate logistic regression analysis was used to identify readmission risk factors and protective factors. Results We developed six candidate readmission models with different machine learning algorithms. The best performing model of extreme gradient boosting (XGBoost) achieved an area under the receiver operating characteristic curve of 0.753 on the development data set and 0.742 on the validation data set. By multivariate logistic regression analysis, we identified 14 risk factors and 2 protective factors of readmission that have never been reported. Conclusions The performance of our model is better than that of the most widely used models in US health care settings. This model can help clinicians identify readmission risk at the early stage of hospitalization so that they can pay extra attention during the care process of high-risk patients. The 14 novel risk factors and 2 novel protective factors can aid understanding of the factors associated with readmission.
