Abstract:Background: Continuous understanding of the epidemiology and evolution of multidrug resistant TB (MDR-TB) is key to develop the appropriate strategies for preventing its epidemic. The present study aimed to investigate the longitudinal trend of its rates, transmissions and transition to extensively drug resistant TB(XDR-TB), in an attempt to adjust the TB control activities in eastern rural China. Methods: The data presented here span 8 years (2003-2010) and consisted of three population-based investigation of drug resistant TB in terms of socio-demographic and clinical characteristics of drug resistant TB cases as well as the microbiological and molecular features of their infected Mtb strains, respectively in one 19 years’ DOTS covered eastern rural county: Deqing (DQ) and the other 9 years’ DOTS covered Guanyun (GY). Results: Within 9 years’ study, 901 TB patients were identified at local TB dispensaries in two counties, with 254 resistant to at least one of 1st line drugs. Of the 135 MDR-TB isolates, 6 were XDR-TB. Rate of drug resistant TB remained stable, even reduced in some period both in two counties, while the rate of MDR-TB and MDR-TB plus resistance to fluoroquinolones increased significantly respectively from 14.5% and 5.1% in 2003/2004 to 21.3% and 9.7% in 2009/2010. The IS6110 RFLP genotyping revealed the 21 clusters involving 48(28.4%) drug resistant TB cases in DQ and 28 clusters involving 59(32.1%) drug resistant TB cases in GY. The clustering proportion increased among the drug resistant TB cases especially for MDR-TB over the study period, reaching 52%(DQ) and 48%(GY) in 2010. And 2 XDR-TB isolates was identified within only 2 clusters 0f drug resistant Mtb in DQ. Designated as subtypes of Beijing family, totally 3 clusters (11 drug resistant TB cases) typical with katG315Thr mutation, persisted throughout the study period in two counties. Conclusion: A high increased recent transmission of MDR-TB might implicate it necessary to improve the effectiveness on diagnosis and treatment of MDR-TB. Despite an increasing trend of MDR plus the resistant to fluoroquinolones, the low proportion of XDR-TB among MDR-TB might indicate it neccessary to cautiously introduce some of 2 line drugs in MDR-TB treatment in China.
Epidemiology and Drug Resistance Patterns of Mycobacterium tuberculosis in High-Burden Area in Western Siberia, Russia

Abstract:Russia is a high-burden area for multidrug-resistant tuberculosis (MDR-TB). Here, we studied the epidemiological situation and drug resistance patterns of Mycobacterium tuberculosis in the Omsk region in Western Siberia. M. tuberculosis isolates (n = 851) were recovered from newly diagnosed TB patients in 2021. The isolates were tested by bacteriological and molecular methods, and long-term epidemiological data were analyzed. The TB incidence dec, this is not variablereased from 93.9 in 2012 to 48.1 in 2021, per 100,000 population, but the primary MDR-TB rate increased from 19.2% to 26.4%. The destructive forms of tuberculosis accounted for 37.8% of all cases, while 35.5% of patients were smear-positive. Of all isolates tested, 55.2% were culture-positive, of which 94.5% were further tested for phenotypic drug resistance and associated mutations. More than half (53.4%) of isolates were drug-resistant, 13.9% were monoresistant and 67.9% were MDR. Among MDR isolates, 40.4% were pre-XDR, and 19.2% were XDR. The spectrum of drug resistance included second-line drugs (new-generation fluoroquinolones, linezolid), which significantly increase the risk of an adverse outcome in patients. In conclusion, our results highlight the critical importance of monitoring drug resistance in circulating M. tuberculosis strains emerging due to ineffective treatment and active transmission.

Molecular epidemiology of multidrug resistant tuberculosis in Mongolia and Eastern Siberia: two independent dissemination processes for dominant strains

Abstract:Mongolia and Russia are among the countries with the high tuberculosis (TB) burden. The prevalence of tuberculosis, including multidrug-resistant tuberculosis (MDR), in Eastern Siberia bordering Mongolia is significantly higher than in the European part of Russia. In addition, unlike Mongolia, Eastern Siberia is characterized by a high prevalence of HIV infection. The cross-border spread of socially significant infections in these countries seems to occur due to their wide-range cooperation and cultural exchange. Whereas the HIV infection has no epidemiological significance for Mongolia at the moment, tuberculosis, however, has a similar prevalence on both sides of the border. The aim was to evaluate the cross-border MDR M. tuberculosis distribution in Mongolia and Eastern Siberia by using molecular genetic data. Materials and methods. A total of 1045 M. tuberculosis strains isolated in Mongolia (291) and the three regions of Eastern Siberia (754) were studied by using the MIRU-VNTR-24 loci genotyping. The CC2/W148 and CC1 subtypes were identified by the specific deletion in the kdpD gene and SNP in the pks17 gene at position 1887060, respectively. Phylogenetic analysis of MIRU-VNTR patterns was carried out by generating UPGMA tree and maximum likelihood tree. Results. The Beijing genotype was found in 75.3% (219/291) and 69.0% (520/754) from Mongolian and East Siberian collection, respectively. Common minor genotypes were LAM (11.0% and 15.1%), T (10.3% and 4.5%), and Haarlem (1.4% and 2.4%) found in Mongolia and Eastern Siberia, respectively. The genotypes S (1.3%) and Ural (5.0%) were found solely in the Russia-derived samples. The main epidemic Beijing subtypes in each country belonged to different clonal complexes (CC): the majority of Mongolian Beijing strains displayed profiles 342-32, 3819-32, 1773-32 MLVA types and belonged to the CC4 subtype; Russian Beijing strains mainly belonged to the CC1 (43.7% — 227/520) and CC2/W148 (34.8% — 181/520) subtypes. The MDR level and distribution patterns differed significantly between Mongolia and Eastern Siberia. Modeling of Beijing strain expansion evidences about extremely subtle contribution of the M. tuberculosis cross-border transmission between Mongolia and Russia. The phylogenetic reconstruction of Beijing CC4 subtype evolution in Mongolia suggests that its distribution is primarily associated with China and other countries of the Western Pacific Region. Three main phylogenetic branches of CC4 subtype were traced, which probably spread throughout Mongolia in the 11—12th centuries. It may be assumed that spreading of the epidemic Beijing CC4 subtype might occur in two stages: early period — emergence of ancestral CC4 variants in Mongolia or their introduction from China (they are homologous to the strains preserved in the Chinese population), later period — dissemination due to the active exchange of M. tuberculosis with countries of Southeast Asia, but not Russia. Conclusion. Using MIRU-VNTR-24 genotyping as well as classification according to specific single nucleotide polymorphisms specific to certain Beijing subtypes, it allowed to describe separate patterns of the epidemic variants spread in Mongolia and Russia. It has been demonstrated that emergence and spread of MDR-TB in Mongolia are entirely iatrogenic in nature, while the epidemic subtypes of the Beijing genotype (subtypes CC1 and CC2/W148) contribute markedly into the MDR-TB spreading in Eastern Siberia.

Abstract:Background: Kemerovo region has a high burden of tuberculosis with incidence rates exceeding the national level in two times. The tuberculosis epidemic process is determined by the circulating variants of M. tuberculosis as well. Screening of epidemically significant variants of the pathogen in areas with a high burden of tuberculosis underlies epidemiological diagnosis and is necessary for the development of effective prevention measures, however, the population structure of M. tuberculosis in the Kemerovo region - Kuzbass is poorly understood. Aims: to investigate the genetic characteristics of M. tuberculosis, phenotypic drug resistance and identification of the main genotypes circulating in the Kemerovo region. Materials.and methods: The MIRU-VNTR genotyping of 163 M. tuberculosis strains isolated from patients with tuberculosis in the Kemerovo region in March-October 2022 was carried out. Cultivation of M. tuberculosis, testing of drug susceptibility, and isolation of genomic DNA were carried out by standard methods. Genotypic identification was performed using MIRU-VNTR (24 loci) typing. In parallel, express genotyping was carried out: identification of isolates of the Beijing genotype (by RD105/207) and nonBeijing; subtyping Beijing with PCR RT tests for detection of Central Asian Russian and B0/W148; identification of the non-Beijing group by PCR RT tests for LAM, S, Ural. Results: The isolates of the Beijing (67.5%) were found to dominate both among newly diagnosed (64.4%) and previously treated patients (88.5%). MIRU-VNTR typing revealed 75 profiles, of which 94-32 (35.3%) and 100-32 (15.7%) were the most numerous and belonged to the Beijing genotype. Overall, 39.9% and 20.9% of isolates, respectively, were assigned to the Beijing Central Asian Russian and B0/W148 epidemic clusters, which differed significantly in MDR levels (50.8% and 85.3%, respectively, p=0.005). The second most common were strains of the genetic family of the Euro-American lineage (L4) (31.9%): LAM (6.7%) Ural (7.4%) Haarlem (4.9%) and L4-unclassified (12.9%), MDR among of these minor genotypes was significantly lower than among Beijing, and amounted to 11.5% (p0.001). Strains from HIV-TB patients (56.4% of the total sample) carried an MDR profile more often (54.8%) than in TB cases without HIV infection (35.2%) (p=0.005), which may be due to higher levels of Beijing in the HIV-TB group (75.0% vs. 57.7%; p = 0.026). Complete comparability of the SNP analysis (self-developed tests) to identify the main genotypes and epidemically significant subtypes of Beijing, which made it possible to characterize 75.5% of the sample by the express method, was shown. Conclusions: The molecular genetic screening carried out in the Kemerovo region revealed the heterogeneity of the M. tuberculosis population, which was dominated by strains of the Beijing genotype, in terms of the frequency of subtypes comparable with other territories of the Siberian Federal District.

Abstract:A total of 397 strains of Mycobacterium tuberculosis isolated from newly diagnosed patients with pulmonary tuberculosis in the Omsk oblast in 2019–2020 were genotyped. The prevalence of the Beijing genotype strains (70.8%), in particular, of two modern sublineage clusters: Central Asian/Russian (46.1%) and B0/W148 (19.1%), was established. Strains of the ancient sublineage of the Beijing genotype were represented by clusters 1071-32 and 14717-15, totaling 4.8%. Compared with other genotypes, the strains of B0/W148-cluster and the ancient Beijing sublineage were more often characterized by multidrug resistance (MDR): 93.4 and 94.7%, respectively ( P < 0.0001). Among members of other genetic families (LAM, Ural, T, Haarlem), drug-sensitive strains predominated (75.0%). The circulation of MDR Beijing strains requires molecular epidemiological surveillance in view of their possible wider spread.

Abstract:Introduction. Against the background of improvement of the main epidemiological indicators (morbidity and mortality) for tuberculosis in the Arkhangelsk region, the proportion of newly diagnosed tuberculosis patients with multidrug-resistant pathogen (MDR-TB) increased from 18.7% in 2002 to 33.8% in 2018. The purpose of this study was the genotypic characterization of Mycobacterium tuberculosis strains obtained from newly diagnosed tuberculosis patients in the Arkhangelsk region in 2018. Materials and methods. 89 M. tuberculosis strains isolated in 2018 from newly diagnosed tuberculosis patients were studied. Beijing genotype, its clusters B0/W148 and Central Asian/Russian were determined by PCR detection of the specific markers: IS6110 insertions in the dnaA-dnaN region, mutations in codons 48 of the mutT4 gene (CGG GGG) and 58 of the mutT2 gene (GGA CGA), IS6110 insertions in the Rv2664 region-Rv2665 and Rv1359-Rv1360, substitutions G A in the sigE gene. Non-Beijing strains were spoligotyped. Results. Drug resistance was detected in 41.6% (37/89), MDR — in 33.7% of strains. In 90% (27/30) of MDR strains, resistance to rifampicin and isoniazid was due to rpoB Ser531Leu and katG Ser315Thr mutations. Following M. tuberculosis genotypes were identified: Beijing (67.4%), T (14.6%), Ural (4.5%), Haarlem (4.5%), LAM (2.3%) and CAS1-Delhi (1.1%). Among the Beijing strains, clusters Central-Asian/Russian (60%; 36/60) and B0/W148 (30%; 18/60) prevailed. The majority of MDR strains belonged to the Beijing family (93.3%; 28/30), of which 64.3% (18/28) and 21.4% (6/28) belonged to clusters B0/W148 and Central-Asian/Russian, respectively. Conclusion. In heterogeneous population of the causative agent of tuberculosis in the Arkhangelsk region, the most common strains were those of the Beijing genotype; in 2018 its share increased to 67.4% (40.4% in 1998–1999). Among MDR strains, the proportion of Beijing reached 93.3%, of which more than half (64.3%) belonged to the epidemiologically and clinically significant in Russia cluster B0/W148.
Small-scale stable clusters of elevated tuberculosis incidence in Moscow, 2000-2015: Discovery and spatiotemporal analysis

Abstract:Objectives: To find residential areas with high incidence rate of tuberculosis in Moscow using spatio-temporal analysis of incidence data. Methods: We analyzed the spatial patterns of residence locations of smear or culture positive patients with pulmonary tuberculosis in Moscow. To identify clusters with high local incidence rates, the neighborhoods of detected cases were studied. We assessed the spatial and temporal stability of clusters. Results: For 19033 cases diagnosed with smear or culture positive pulmonary tuberculosis among residents of Moscow in 2000-2015 we identified 18 small-scale clusters of increased incidence rate responsible for 3% of all registered cases identified on a territory inhabited by only 1% of the population. Locations of clusters were sufficiently stable in space throughout the whole period. The local incidence rate inside clusters was significantly (3-4 times) higher than the city average during the whole observation period. The presence of clusters was associated with the incidence rate in the surrounding area. Socio-demographic characteristics of patients in clusters were not significantly different from the average characteristics of patients in the city. Conclusions: The detected small-scale clusters of increased incidence may be used to target active case finding for tuberculosis. The causes and mechanisms of cluster formation and stability need further study.
Impact of COVID-19 Pandemic on Pre-Treatment Delays, Detection, and Clinical Characteristics of Tuberculosis Patients in Ningxia Hui Autonomous Region, China

Abstract:Background: To contain the pandemic of COVID-19, China has implemented a series of public health interventions that impacted the tuberculosis control substantially, but these impacts may vary greatly depending on the severity of the local COVID-19 epidemic. The impact of COVID-19 on TB control in Ningxia Hui Autonomous Region is little known. Methods: Based on the national TB Information Management System (TBIMS), this study accessed the actual impact of COVID-19 on TB by comparing TB notifications, pre-treatment delays, and clinical characteristics of TB cases between 2020 COVID-19 period and 2017–2019 baseline. The data were divided into three periods based on the response started to fight against COVID-19 in Ningxia Hui Autonomous Region, including the control period (10 weeks before the pandemic), intensive period (10 weeks during the Ningxia Hui Autonomous Region lockdown), and regular (10 additional weeks after Ningxia Hui Autonomous Region reopen). Results: TB notification dropped sharply in the first week of the intensive period but took significantly longer to return to the previous level in 2020 compared with the 2017–2019 baseline. Totally, the TB notification rates decreased by more than 60% in the intensive period of COVID-19 compared with the average level of 2017–2019. The sputum smear-positive rate of TB patients diagnosed in intensive period of COVID-19 was significantly higher than that in the corresponding periods of 2017–2019 ( P < 0.001). The rate of cavity on X-ray inspection of TB cases diagnosed in the intensive period of COVID-19 was significantly higher than that in period 2 of 2017–2019 (23.5 vs. 15.4%, P = 0.004). The patients' delay in the intensive period was significantly longer than that before the pandemic ( P = 0.047). Conclusions: The TB notification in Ningxia was impacted dramatically by the pandemic of COVID-19. To compensate for the large numbers of missed diagnosis as well as delayed diagnosis during the intensive period of COVID-19, an urgent restoration of normal TB services, and further emphasis on enhanced active case finding and scale-up of household contact tracing and screening for TB-related symptoms or manifestation, will be essential.

Prevalence of Comorbid Tuberculosis Amongst COVID‐19 Patients: A Rapid Review and Meta‐analysis

Abstract:Tuberculosis is reported to be associated with the severity and mortality amongst patients with coronavirus disease 2019 (COVID-19).1 However, the reported prevalence of comorbid tuberculosis amongst COVID-19 patients varied greatly across the published studies. For example, the low prevalence of comorbid tuberculosis amongst COVID-19 patients was reported in Tian et al's study (0.4%)2 and Yan et al's study (0.2%)3 and the relatively high prevalence of comorbid tuberculosis amongst COVID-19 patients was reported in Hu et al's study (4.5%)4 and Zhang et al's study (9.0%).5 Therefore, it is needed to quantitatively estimate the pooled prevalence of comorbid tuberculosis amongst COVID-19 patients using a meta-analysis. A systematic literature search was conducted independently by two investigators in PubMed, Web of Science, EMBASE, Springer, Elsevier-ScienceDirect, Wiley Online Library, Scopus and Cochrane Library databases to select all eligible studies which were published from 1 January 2020 to 31 August 2021. The following keywords were used: "prevalence" or "incidence" or "rate" or "characteristics" and "tuberculosis" and "COVID-19" or "SARS-CoV-2" or "2019-nCoV" or "severe acute respiratory syndrome coronavirus 2" or "coronavirus disease 2019" or "2019 novel coronavirus". All peer-reviewed papers which were published in the English language were eligibly included if they provided the incidence rate of comorbid tuberculosis amongst COVID-19 patients. Accordingly, we excluded case reports, review articles, duplicate publications, errata, comments and preprints. Reference lists of the retrieved articles were also screened to identify additional studies. This rapid systematic review and meta-analysis was performed following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA).6 The pooled prevalence of comorbid tuberculosis amongst COVID-19 patients was estimated using a random effects meta-analysis model.7, 8 The statistical heterogeneity across studies was assessed by I2 statistic and Cochrane Q test.9, 10 The risk of publication bias was evaluated by Begg's rank correlation test.11, 12 All statistical analyses were carried out using the package "meta" on R version 3.6.3 (R Foundation for Statistical Computing). A two-tailed P value <.05 was regarded statistically significant. A total of 114 eligible studies2-5, 13-122 with 405 596 COVID-19 patients were included in this meta-analysis. Amongst the included studies, 77 studies2-5, 13-85 were reported in Asia, 15 studies90-100 were conducted in Africa, nine studies101-109 were conducted in South America, five studies110-114 were performed in North America, seven studies115-121 were conducted in Europe and one study122 was from multicountries. The baseline characteristics of the included studies are summarised in Table 1. Overall, our findings demonstrated that the prevalence of comorbid tuberculosis amongst COVID-19 patients in this pooled meta-analysis was 1.6% with 95% confidence interval (CI): 1.3%–1.9% (Figure 1). The regional tuberculosis prevalence in COVID-19 patients was estimated as follows: Africa (4.1%, 95% CI: 0.0%–5.8%), Asia (1.1%, 95% CI: 0.9%–1.3%), South America (0.5%, 95% CI: 0.0%–1.0%), Europe (0.4%, 95% CI: 0.0%–0.6%) and North America (0.2%, 95% CI: 0.0%–0.4%), which revealed great variability in the prevalence of comorbid tuberculosis amongst COVID-19 patients in different regions. Begg's test revealed that there was potential publication bias (P = .0017). In conclusion, our findings demonstrated that the prevalence of comorbid tuberculosis amongst COVID-19 patients was 1.6%, which varied greatly in different regions. Abstract:Many countries have reported increase of TB incidence during the COVID-19 pandemic period, which demands dire attention as it may threaten global effort to end TB transmission. Services, are among many others, were disrupted by the COVID-19 pandemic during the years 2020 and 2021; but its impact on the TB transmission is not well understood. This retrospective population-based molecular and epidemiological cohort study aims to determine the pattern of TB transmission in Kuala Lumpur (an area with high population density, moderate TB burden and high rates of COVID-19 cases) for the cohort of Pulmonary TB (PTB) cases notified from 2020 until 2021 and factors associated with clustering or clear epidemiologic linkage. This study will be carried out from 2022 until 2024. The study will utilise comparative phylogenetic analysis to determine the degree of relatedness between different isolates, based on the genomes similarities, and overlay this with epidemiological, clinical and social network data to enhance understanding of the social-behavioural dynamics of TB transmission. Mycobacterium tuberculosis complex (MTBC) cultures will be genotyped using Mycobacterial Interspersed Repetitive Unit Variable Number Tandem Repeats (MIRU-VNTR) and whole-genome sequence (WGS) for MTBC cluster isolates. Epidemiologic and genomic data will be overlaid on a social network constructed by means of interviews with patients, by using Social Network Analysis questionnaire, to determine the origins and transmission dynamics of the outbreak. The finding of this study would aid in the identification of TB transmission events, facilitating active case finding, TB screening, TB contact tracing, and the mapping of social contacts during critical period. This will contribute to building an effective preventive and preparedness strategy to interrupt TB transmission in Malaysia, tailored to the characteristics of the local population.
Prevalence, Strains, and Drug Susceptibility of Nontuberculous Mycobacteria Isolates from Tuberculous Suspects

Abstract:Background: The prevalence of nontuberculous mycobacteria (NTM) infection has been increasing globally. Many cases of NTM infection are misdiagnosed as Mycobacterium tuberculous (MTB) because of similar clinicoradiological features. Objectives: To determine the burden and characteristics of NTM infection, this study was done to evaluate clinical isolates collected from tuberculous (TB) suspects in a population from Northwest China. Methods: From January to December 2020, the clinical samples of 9,142 TB suspects were collected for the PCR-fluorescent probe and mycobacterial culture. The PCR-fluorescent probe-positive nucleic acid samples were further subjected to a DNA microarray for confirmation and species identification. Drug susceptibility testing (DST) was also carried out using the micropore plate method (MicroDSTTM) on isolates from NTM patients. Results: Of 9,412 TB suspects, 85 cases (0.9%) were clinically diagnosed with NTM infection according to the American Thoracic Society (ATS) guidelines. For the laboratory samples, a total of 169 NTM strains, identified by molecular biology methods, were classified into 10 species. The most common species were M. chelonae/ M. abscessus (64/169, 37.7%) and M. intracellulare (40/169, 23.7%). All strains showed the highest resistance to imipenem/cilastatin (85/85, 100%) and the highest susceptibility to linezolid (4/85, 4.7%). In comparison with the rapidly growing mycobacteria (RGM) group, the slowly growing mycobacteria (SGM) group showed a lower resistance and a shorter hospital inpatient stay (t = 6.66, P < 0.001 and t = 2.40, P = 0.020, respectively). Conclusions: Mycobacterium chelonae/M. abscessus and M. intracellulare were the most frequently detected NTM pathogens in Northwest China. The differences in drug sensitivity and clinical characteristics were giant for different strains. Timely identification and accurate DST play important roles in NTM management.
Changes in the incidence, clinical features and outcomes of tuberculosis during COVID-19 pandemic

Abstract:Background: COVID-19 pandemic has disrupted tuberculosis (TB) services in many countries, but the impacts on sites of involvement, drug susceptibility, smear positivity and clinical outcomes, and clinical outcomes of co-infection with influenza and COVID-19 remain unclear. Methods: Descriptive epidemiological study using episode-based and patient unique data of tuberculosis from Hospital Authority's territory-wide electronic medical record database, comparing baseline (January 2015-December 2019) and COVID-19 period (January 2020-December 2022), followed by univariate and multivariate analyses. Effects of co-infection with influenza and COVID-19 were investigated. Results: The study included 10,473 episodes of laboratory-confirmed TB, with 6818 in baseline period and 3655 during COVID-19 period. During COVID-19 period, TB patients had a lower proportion of smear positivity (49.2 % vs 54.7 %, P < 0.001), and fewer cases of extrapulmonary TB (7.0 % vs 8.0 %, P = 0.078) and multidrug resistant TB (1.0 % vs 1.6 %, P = 0.020). Mortality was higher in TB patients with COVID-19 coinfection (OR 1.7, P = 0.003) and influenza coinfection (OR 2.6, P = 0.004). During COVID-19 period, there were higher rates of treatment delay (20.5 % vs 15.5 %, P < 0.001) and episodic death (15.1 % vs 13.3 %, P = 0.006). Factors associated with higher mortality included age ≥ 70 years (OR 7.24), treatment delay (OR 2.16), extrapulmonary TB (OR 2.13). smear positivity (OR 1.71) and Charlson comorbidity index score ≥ 3 (OR 1.37). Higher mortality was observed with co-infection by influenza (OR 1.18) and COVID-19 (OR 1.7). Conclusions: The epidemiology and outcomes of TB were changed during COVID-19 period. Mortality was higher during COVID-19 period and with co-infection by influenza and COVID-19.

Abstract:Objective The burden of both general and drug-resistant tuberculosis in rural areas is higher than that in urban areas in China. To characterize the genetic structure and transmission risk of Mycobacterium tuberculosis in rural China, we used whole genome sequencing to analyze clinical strains collected from patients in two counties of Yichang for three consecutive years. Methods From 2018 to 2020, sputum samples were collected for cultures from patients with suspected tuberculosis in Yidu and Zigui county, and DNA was extracted from the positive strains for genome sequencing. The online SAM-TB platform was used to identify the genotypes and drug resistance-related mutations of each strain, establish a phylogenetic tree, and calculated the genetic distances between pairwise strains. Twelve single nucleotide polymorphisms (SNPs) were used as thresholds to identify transmission clusters. The risk of related factors was estimated by univariable and multivariable logistic regression. Results A total of 161 out of the collected 231 positive strains were enrolled for analysis, excluding non-tuberculous mycobacterium and duplicate strains from the same patient. These strains belonged to Lineage 2 (92, 57.1%) and Lineage 4 (69, 42.9%), respectively. A total of 49 (30.4%) strains were detected with known drug resistance-related mutations, including 6 (3.7%) multidrug-resistant-TB (MDR-TB) strains and 11 (6.8%) RIF-resistant INH-susceptible TB (Rr-TB) strains. Six of the MDR/Rr-TB (35.3%) were also resistant to fluoroquinolones, which made them pre-extensively drug-resistant TB (pre-XDR-TB). There were another seven strains with mono-resistance to fluoroquinolones and one strain with resistance to both INH and fluoroquinolones, making the overall rate of fluoroquinolones resistance 8.7% (14/161). A total of 50 strains (31.1%) were identified as transmission clusters. Patients under 45 years old (adjusted odds ratio 3.46 [95% confidential intervals 1.28–9.35]), treatment-naive patients (6.14 [1.39–27.07]) and patients infected by lineage 4 strains (2.22 [1.00–4.91]) had a higher risk of transmission. Conclusion The drug resistance of tuberculosis in rural China, especially to the second-line drug fluoroquinolones, is relatively serious. The standardized treatment for patients and the clinical use of fluoroquinolones warrant attention. At the same time, the recent transmission risk of tuberculosis is high, and rapid diagnosis and treatment management at the primary care needs to be strengthened.

Abstract:In the mountainous, rural regions of eastern China, tuberculosis (TB) remains a formidable challenge; however, the long-term molecular epidemiological surveillance in these regions is limited. This study aimed to investigate molecular and spatial epidemiology of TB in two mountainous, rural counties of Zhejiang Province, China, from 2015 to 2021, to elucidate the recent transmission and drug-resistance profiles. The predominant Lineage 2 (L2) Beijing family accounted for 80.1% of total 532 sequenced Mycobacterium tuberculosis (Mtb) strains, showing consistent prevalence over seven years. Gene mutations associated with drug resistance were identified in 19.4% (103/532) of strains, including 47 rifampicin or isoniazid-resistant strains, eight multi-drug-resistant (MDR) strains, and five pre-extensively drug-resistant (pre-XDR) strains. Genomic clustering revealed 53 distinct clusters with an overall transmission clustering rate of 23.9% (127/532). Patients with a history of retreatment and those infected with L2 strains had a higher risk of recent transmission. Spatial and epidemiological analysis unveiled significant transmission hotspots, especially in densely populated urban areas, involving various public places such as medical institutions, farmlands, markets, and cardrooms. The study emphasizes the pivotal role of Beijing strains and urban-based TB transmission in the western mountainous regions in Zhejiang, highlighting the urgent requirement for specific interventions to mitigate the impact of TB in these unique communities.

We conducted a ten-year study on the transmission of M. tuberculosis in a population in eastern China. Patients and Methods: We selected Lianyungang, an eastern city in China, as the study site. Patients diagnosed with active pulmonary TB from 2011 to 2020 were enrolled as the study subjects. We isolated and sequenced 2252 M. tuberculosis . Strains with pairwise genetic distances of less than 12 single nucleotide polymorphisms were defined as genomic clusters and which were considered recent transmissions. Kernel density estimation and K-function analysis were applied to explore the spatial distribution of recently transmitted strains. Results: After excluding non-tuberculous mycobacteria and duplicated samples, 2114 strains were included in the final analysis. These strains comprised lineage 2 (1593, 75.35%) and 4 (521, 24.65%). There were 672 clustered strains, with a recent transmission rate of 31.79%. The logistic regression model showed that the risk of recent transmission was high in students [adjusted odds ratio (aOR): 2.68, 95% confidence interval (CI): 1.63– 4.49, P < 0.001] and people infected with L2.2.1 strains (aOR: 1.59, 95% CI: 1.20– 2.12). Higher spatial aggregation of TB transmission has been concentrated in Haizhou, Donghai, and Guanyun for the past 10 years. Three outbreaks affecting 46 patients were spatially spaced, with 11 to 23 persons each. Different groups exhibited varying geographic distances between the initial and later cases. Conclusion: There are areas with a high risk of transmission for M. tuberculosis in the research site, and the risk varies among different populations. Accurate prevention strategies targeted at specific regions and key populations can help curb the prevalence of TB. Abstract:Background High burden of drug-resistant (DR) tuberculosis (TB) is a significant threat to national TB control programs all over the world and in the Russian Federation. Different Mycobacterium tuberculosis (MTB) genotypes are hypothesized to have specific characteristics affecting TB control programs. For example, Beijing strains are supposed to have higher mutation rates compared to strains of other genotypes and subsequently higher capability to develop drug-resistance. Results Clinical MTB isolates from HIV- and HIV+ patients from four regions of Russia were analyzed for genotypes and mutations conferring resistance to Isoniazid, Rifampicin, Ethambutol, aminoglycosides, and fluoroquinolones. Analysis of genotypes and polymorphism of genomic loci according to the HIV status of the patients – sources of MTB isolates were performed. Studied MTB isolates from HIV- TB patients belonged to 15 genotypes and from HIV + TB patients – to 6 genotypes. Beijing clinical isolates dominated in HIV- (64,7%) and HIV+ (74,4%) groups. Other isolates were of LAM (including LAM1 and LAM9), Ural, and 4 minor groups of genotypes (including 5 subclones T). The spectrum of genotypes in the HIV- group was broader than in the HIV+ group. PR of B0/W148 Beijing was significantly lower than of other Beijing genotypes in susceptible and MDR-XDR isolates. Rates of isolates belonging to non-Beijing genotypes were higher than Beijing in susceptible isolates from HIV- patients. Conclusions Beijing genotype isolates prevailed in clinical isolates of all drug susceptibility profiles both from HIV- and HIV+ patients, although B0/W148 Beijing genotype did not dominate in this study. Genome loci and mutations polymorphisms were more pronounced in clinical isolates from HIV- patients, than from HIV+.

Abstract:Objectives: To determine the genetic diversity of Mycobacterium tuberculosis (M. tuberculosis) strains in a Chinese population predominately infected with strains of the W-Beijing family.Methods: A cross-sectional study was conducted in three counties of eastern China. M. tuberculosis strains were collected at TB clinics, and patients were interviewed by trained physicians at the time of TB diagnosis. RD105 and RD181 were used to identify W-Beijing and modern W-Beijing strains, respectively, while seven-locus variable numbers of tandem repeat-mycobacterial interspersed repetitive unit (VNTR-MIRU) analysis was employed to differentiate the genotypes of these strains.Results: Of 441 strains studied, 394 (89.3%) were identified as W-Beijing family strains; of them, 299 were modern W-Beijing strains. VNTR-MIRU identified 409 genotypes from 426 strains, including 395 unique patterns and 14 clusters. Ancestral W-Beijing strains were more likely to be clustered (OR = 1.32, 95% CI: 0.58-2.97) compared to modern W-Beijing strains. The proportions of clustered strains were 14.6%, 4.2% and 0% at sites Funing (FN), Deqing (DQ) and Yinzhou (YZ), respectively. Of the seven MIRU loci, VNTR3820 was found to have the highest discriminatory power and allelic diversity.Conclusions: VNTR-MIRU typing appears to be a reliable method for analyzing M. tuberculosis transmission in relatively closed populations. The low clustering proportions indicate that endogenous relapse may be a main source of TB cases in eastern China. Furthermore, our results indicate that migration has played may play an important role in the recent transmission of the W-Beijing family of M. tuberculosis. Spatiotemporal analysis of regional and age differences in tuberculosis prevalence in mainland China

Abstract:Globally, tuberculosis is a leading cause of infectious disease deaths. China ranks third among the 30 high‐burden countries for tuberculosis and accounts for approximately 7.4% of the cases reported worldwide. Since very few studies have investigated the age difference in tuberculosis prevalence in mainland China, therefore, the preliminary characterisation of age differences in tuberculosis patients is not well understood. The data of reported sputum smear‐positive, tuberculosis and sputum smear‐negative cases in 340 prefectures from mainland China were extracted from the China Information System for Disease Control and Prevention from January 2009 to December 2018. Multiple statistical analysis and GIS techniques were used to investigate the temporal trend and identify the spatial distribution of sputum smear‐positive, tuberculosis and sputum smear‐negative cases in the study area. The results showed that the incidence of sputum smear‐positive and tuberculosis has dropped to a stable level, while sputum smear‐negative exhibited a rising trend. Additionally, sputum smear‐positive, tuberculosis and sputum smear‐negative are still highly prevalent in northwestern and southwestern regions of China. Interestingly, the young adult group (20–50 age) and elder group (>50 age) are more susceptible to being infected with tuberculosis, while lower infection levels were recorded in the juvenile group (<20 age). The present study investigated the temporal–spatial distribution of sputum smear‐positive, tuberculosis and sputum smear‐negative cases in mainland China before the COVID‐19 pandemic breakout, which would help the government agency establish an effective mechanism of tuberculosis prevention in high‐risk periods and high‐risk areas in the study region.

Abstract:ABSTRACT Tuberculosis (TB) is more prevalent in rural than urban areas in China, and delineating TB transmission patterns in rural populations could improve TB control. We conducted a prospective population-based study of culture-positive pulmonary TB patients diagnosed between July 1, 2009 and December 31, 2020 in two rural counties in China. Genomic clusters were defined with a threshold distance of 12-single-nucleotide-polymorphisms, based on whole-genome sequencing. Risk factors for clustering were identified by logistic regression. Transmission links were sought through epidemiological investigation of genomic-clustered patients. Of 1517 and 751 culture-positive pulmonary TB patients in Wusheng and Wuchang counties, respectively, 1289 and 699 strains were sequenced. Overall, 624 (31.4%, 624/1988) patients were grouped into 225 genomic clusters. Epidemiological links were confirmed in 41.8% (196/469) of clustered isolates, including family (32.7%, 64/196) and social contacts (67.3%, 132/196). Social contacts were generally with relatives, within the community or in shared aggregated settings outside the community, but the proportion of clustered contacts in each category differed between the two sites. The time interval between diagnosis of student cases and contacts was significantly shorter than family and social contacts, probably due to enhanced student contact screening. Transmission of multidrug-resistant (MDR) strains was likely responsible for 81.4% (83/102) of MDR-TB cases, with minimal acquisition of additional resistance mutations. A large proportion of TB transmission in rural China occurred among social contacts, suggesting that active screening and aggressive contact tracing could benefit TB control, but contact screening should be tailored to local patterns of social interactions.
