Abstract:Objective To investigate the clinical characteristics and surgical management of middle ear cholesteato-ma in children. Methods 27 cases of children with middle ear cholesteatoma were enrolled in this study;Clinical symp-toms, signs and surgical experience were reviewed. Of 27 patients, 2 were performed transmeatal atticotomy and tympano-plasty, 25 were performed canal-wall-down modified radical mastoidectomy, and 5 were performed first-stage ossicular chain reconstruction. Results Otorrhea, ear odour and hearing loss were the most common symptoms;Mastoid, sinus tympani and attic were involved in 25 cases;Mesotympanum, hypotympanum and eustachian tube were in-volved in 20 cases;88.9%of patients had ossicular chain destruction;Facial nerve canal was eroded in 7 cases;During the follow-up of one to five years, there was no one with significant complications as facial palsy and intracranial complica-tions, no recurrent cholesteatoma was observed. Conclusions The middle ear cholesteatoma in children show more aggres-sive in complicated condition, appropriate surgical approach should be selected according to the severity of the lesion, opened technique is benificial to complete removal of lesions and prevent recurrence in severe patient.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Background: Congenital cholesteatoma is defined as a white mass behind an intact eardrum without a history of otitis media or previous otologic procedures. Congenital cholesteatoma is a relatively rare disease that accounts for about 2% to 5% of all cholesteatomas. However, the actual incidence rate of congenital cholesteatoma may be underestimated. Conductive hearing loss is the most common presenting symptom. The current study aims to describe the clinical characteristics and management of patients with congenital cholesteatoma and promote awareness of the disease in unilateral or asymmetric conductive hearing loss patients. Methods: In this study, we report a case series of 3 patients including 1 child, 1 adolescent, and 1 young adult, managed in our department between June and August 2023, and present a summary of the literature. Results: Congenital cholesteatoma is primarily a pediatric disease, but it has also been reported in adults. Two cases presented with unilateral secretory otitis media, and 1 case presented with asymmetric unilateral conductive hearing loss. Two patients of Potsic stage III congenital middle ear cholesteatomas underwent transcanal endoscopic ear surgery, and 1 patient of Potsic stage IV underwent conventional microscopic approach canal wall-up mastoidectomy combined with endoscopy. Conclusions: In children or young adults with persistent unilateral or asymmetric conductive hearing loss, congenital middle ear cholesteatoma should be considered. Congenital cholesteatoma cannot be ruled out in children with unilateral secretory otitis media.

Metastatic versus primary oncocytic papillary adenocarcinoma of the endometrium: a report of a case and review of the literature.

Abstract:We report a case of an oncocytic papillary adenocarcinoma of the endometrium in an 89-year-old female with vaginal bleeding. Imaging studies revealed lesions in the uterus, kidneys, pancreas, gluteus, and an enlarged portacaval lymph node. Diagnostic workup included an endometrial biopsy which showed malignant, oncocytic cells in a predominantly papillary pattern. These cells stained positive for epithelial markers (pan-cytokeratin, CK7, epithelial membrane antigen) and weakly for estrogen receptor. The cells were negative for cytokeratin 903, CAM 5.2, progesterone receptor, CD10, RCC Marker, CA-125, c-kit, and vimentin. Consultation with experts in Gynecologic and Genitourinary pathology returned a diagnosis of "adenocarcinoma compatible with metastatic renal cell carcinoma"--an intriguing possibility worthy of further exploration. To our knowledge, there are no reports in the literature of metastatic oncocytic papillary renal cell carcinoma to the endometrium. The clinical and pathologic features of oncocytic papillary endometrial lesions, including primary and metastatic processes, are reviewed.
[Analyses of the Clinical Characteristics of Unilateral Conductive Hearing Loss with Intact Tympanic Membrane].

Abstract:OBJECTIVE:To analyze the clinical characteristics of unilateral conductive hearing loss with intact tympanic membrane, and summarize the key diagnostic points, differential diagnosis and observe the effects of surgical treatment.METHODS:We reviewed data from 82 patients with unilateral conductive hearing loss with intact tympanic membranes who accepted the exploratory tympanotomy from April 2011 to September 2013. There were 41 males and 41 females, aged from 7 to 66( averaged 26.5±13.7)years, with a history of one month to 50 years. The history, clinical symptoms, audiological evaluation, high resolution temporal bone CT, the results of surgical exploration and hearing reconstruction were analyzed.RESULTS:The exploratory tympanotomy revealed 43 cases of congenital middle ear malformations (52.4%), 22 cases of otosclerosis (26.8%), eight cases of congenital cholesteatoma (9.8%), six cases of trauma induced conductive hearing loss (7.3%), three cases of congenital ossicular malformations with congenital cholesteatoma (3.7%). Progressive hearing loss was common in patients with otosclerosis and congenital cholesteatoma, and patients with congenital middle ear malformations described their hearing loss since childhood. High resolution temporal bone CT of congenital middle ear malformation, trauma induced conductive hearing loss, congenital cholesteatoma diagnosis rate was 40.0%, 50.0%, and 83.3% respectively. The preoperative air-conductive threshold of patients with absence of the oval window were increased to (66.9±1.1)dBHL, the preoperative bone-conductive threshold achieved (28.3±10.4)dBHL at 2 000 Hz. While patients with stapes fixation and that with ossicular chain discontinuity were (27.2±9.7)dBHL and (17.8±8.8)dBHL(P=0.000)respectively. Through the tympanic exploration with endaural incision under the microscope, different hearing reconstruction were applied according to different lesions. After the operation, the hearing level of 52 patients with return visit were improved, the mean air-conductive threshold were decreased from (60.0±11.4)dBHL to (32.2±12.1)dBHL(P=0.000); and the mean ABG were decreased from (43.2±12.0)dB to (16.3±9.4)dB(P=0.000).CONCLUSIONS:Congenital middle ear malformations, otosclerosis, congenital cholesteatoma are the most common causes in unilateral conductive hearing loss with an intact tympanic membrane. The diagnosis rate can be improved by analyzing the clinical features. Through exploratory tympanotomy and hearing reconstruction, we can clarify the diagnosis and achieve a satisfying hearing recover.

Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Abstract:Objective: We aimed to assess the clinical characteristics of extent patterns in congenital cholesteatoma, based on the Japan Otological Society (JOS) staging system. Methods: This was a retrospective chart review that included 80 ears of 80 patients with congenital cholesteatoma who underwent primary surgery at a tertiary academic medical center. The main characteristics and outcomes reviewed were sex, age, clinical background, surgical method, and stage classification according to two staging classifications: the criteria advocated by JOS and Potsic staging system. Results: The age at the time of surgery ranged from 1 to 35 years (average 8.4 years), and there were 54 men and 26 women. According to the JOS staging system, 12 ears were Stage Ia (15%), 7 ears were Stage Ib (9%), 1 ear was Stage Ic (1%), 59 ears were Stage II (74%), and 1 ear was Stage III (1%). In the study of postoperative residual recurrence, there were 4 cases after the primary operation and 3 cases after the staged operation. All 3 ears with residual disease after planned surgery were cholesteatomas that extended to all the tympanomastoid space. Conclusion: We consider the JOS staging system to be more suitable, in terms of anatomical classification and surgical procedure selection for comparison between Europe, the United States, and Asia. Specifically, it was advantageous that the PTAM classification and the S classification are associated with surgical procedure selection and postoperative course.

Abstract:The antithrombotic armamentarium for the treatment of acute coronary syndromes has expanded so rapidly over the past decade that this evolution has challenged the pace at which large-scale clinical trials can be completed for new indications and cardiac society guidelines can be updated. For example, there are presently 60 different antiplatelet and anticoagulant combinations for drugs with Class I or II indications for the treatment of ST-segment elevation myocardial infarction (STEMI). Considering clopidogrel in particular, the current European Society of Cardiology (ESC) guidelines1 recommend an oral loading dose of at least 300 mg for patients undergoing primary percutaneous coronary intervention (PCI) (Class I, Level of Evidence C). In the acute phase of STEMI, a clopidogrel maintenance dose of 75 mg daily is recommended (I, A) and, while the optimal duration of clopidogrel has not been determined, therapy for 12 months in all STEMI patients is recommended (IIa, C). Similarly, the current American College of Cardiology/American Heart Association (ACC/AHA) guidelines recommend clopidogrel for patients with STEMI regardless of whether they undergo reperfusion with fibrinolytic therapy (I, A).2 These updates are based on the Clopidogrel and Metoprolol in Myocardial Infarction Trial/Second Chinese Cardiac Study (COMMIT/CCS-2) and the Clopidogrel as Adjunctive Reperfusion Therapy (CLARITY)-Thrombolysis in Myocardial Infarction (TIMI) 28 trials, which did not employ PCI as the primary reperfusion strategy.3,4 No prior large-scale trial or registry has demonstrated a mortality-lowering benefit from high-dose clopidogrel among primary PCI patients, though some relevant data are emerging.5,6 Witkowski et al. 7 report their findings among 7193 patients undergoing primary PCI during 2003 and included in a multicentre Polish STEMI registry. Congenital cholesteatoma clinical and surgical management

Abstract:Objectives: Congenital cholesteatoma (CC) is accompanied by hearing loss and an intact tympanic membrane. However, the hearing loss is usually associated with otitis media, and the diagnosis of CC is frequently delayed in patients with an intact tympanic membrane. This study aimed to describe the clinical characteristics, management and outcomes of patients with CC. Methods: We reviewed patients with cholesteatoma from January 2011 to May 2020 and selected those meeting the congenital cholesteatoma criteria. The primary outcome measures included presenting symptoms, surgical findings, stage of disease, recurrence rate and hearing outcomes. Results: We reviewed 1646 medical files of cholesteatoma patients and identified 18 patients with congenital cholesteatoma, the mean age at operation was 8.13 ± 1.36 years (range 3-18). The unilateral hearing loss included moderate 13 patients (72.2%), severe 4 patients (22.2%), and slight 1 (5.6%). There were 14 cases of conductive hearing loss (77.8%) and 4 cases of mixed hearing loss (22.2%). The mean course of disease was 1.41 ± 0.05 years (range 0.4-3). The surgical management was oto-endoscope exploratory tympanotomy in 1（5.6%）, canal wall up mastoidectomy in 12 (66.7%) and canal wall down in 5 (27.8%), with 17 (94.4%) ossicular replacements. Seventeen (94.4%) patients presented with Potsic stage III-IV. Recurrence occurred in 5.6% of patients in stage III and 11.1% of patients in stage IV. After surgery, patients achieved normal voice tone hearing. Conclusions: To diagnose it early, congenital cholesteatoma should be considered as a possible aetiology for hearing loss patients with an intact tympanic membrane. In our study, most patients were diagnosed at III and IV stage. This highlights the need to promote awareness of the disease among primary physicians in the community healthcare system. Surgical management with removal of the cholesteatoma and reconstruction of the tympanum and ossicular chain is an effective treatment.

Abstract:Objective: This study was undertaken to determine the characteristics of congenital cholesteatoma (CC) in Japan, via a nationwide survey using staging and classification criteria for middle ear cholesteatoma, as proposed by the Japan Otological Society (JOS). Methods: A nationwide survey regarding middle ear cholesteatoma treated in 2015 was performed. There were 1787 registrations from 74 facilities, among which, CC accounted for 12.9% (231 cases) of all middle ear cholesteatoma cases. The extent of the disease was classified according to the classification and staging of cholesteatoma proposed by JOS in 2015. Results: The age of the patients ranged from 1 to 55 years (mean, 8.2 years; median, 6 years; 149 ears of males; 82 ears of females). Among these cases, 105 (45.5%) were classified as stage I, 121 (52.4%) as stage II, and 5 (2.1%) as stage III, with no cases in stage IV. In the sub classification of stage I, 35 (33%), 43 (41%) and 27 (26%) ears were classified as stages I-a, I-b and I-c, respectively. Stage I-b (cholesteatoma confined to the posterior half of the tympanic cavity) was more frequent than stage I-a (the anterior half of tympanic cavity). Pre-operative hearing level of air-conduction was 20.5dB for stage I-a, 34.5dB for stage I-b, 30.5dB for stage I-c, 38.6dB for stage II, and 59.0dB for stage III. The rate of missing stapes superstructure increased as the disease progressed. In the same way, the mastoid development lowered as the stage of the disease progressed. There were significant differences in the age at operation, pre-operative hearing level, status of stapes, and major ossiculoplasty, between stage l-a and l-b. Conclusions: The current condition of CC in Japan was been evaluated; in particular, it was observed that the cases limited to the posterior part of tympanic cavity were not rare. The JOS stage classification based on the progression of the disease seemed simple and efficient.

The authors present cholesteatoma definition from the viewpoint of V. F. Undrits, L. T. Levin, V. T. Palchun, I. Friedmann, H. Schuknecht and other domestic and foreign authors. The article describes the modern concepts of its macroscopic and microscopic structure, giving coverage to the currently existing classifications. The authors consider the theories of the etiology and pathogenesis of congenital and acquired middle ear cholesteatoma.
Formulation Development of Efavirenz Tablets Employing HP Β Cyclodextrin-PVP K30-SLS: A Factorial Study

Abstract:Efavirenz, a widely prescribed anti-retroviral drug belongs to class IΙ under BCS and exhibit low and variable oral bioavailability due to its poor aqueous solubility. Its oral absorption is dissolution rate limited and it requires enhancement in the solubility and dissolution rate for increasing its oral bioavailability. The objective of the study is to evaluate the feasibility of formulating efavirenz-HPβCDPVP K30/SLS inclusion complexes into tablets and to evaluate the effects of HPβCD, PVP K30 and SLS on the dissolution rate and dissolution efficiency of efavirenz tablets in a 23 factorial study. A comparative evaluation of wet granulation and direct compression methods was made for the preparation of tablets employing drug-HPβCD-PVP K30/SLS inclusion complexes. Drug-HPβCD-PVP K30/SLS inclusion complexes were prepared by kneading method. Tablets each containing 50 mg of efavirenz were prepared by wet granulation and direct compression methods employing various HPβCD complexes as per 23 factorial design and the tablets were evaluated for dissolution rate and other physical properties. Efavirenz tablets formulated employing dug-HPβCD-PVP K30/ SLS inclusion complexes and prepared by direct compression method disintegrated rapidly when compared to those made by wet granulation method. Efavirenz dissolution was rapid and higher from the tablets formulated employing drug- HPβCD- PVP K30 /or SLS inclusion complexes when compared to the tablets containing efavirenz alone in both wet granulation and direct compression methods. The individual as well as combined effects of the three factors involved i.e., HPβCD (factor A), PVP K30 (factor B) and SLS (factor C) were highly significant (P < 0.01) in enhancing the dissolution rate (K1) and dissolution efficiency (DE 30) of efavirenz in both wet granulation and direct compression methods. Among the three factors HPβCD (factor A) gave highest enhancement in the dissolution rate (K1) and dissolution efficiency (DE 30) of efavirenz tablets in both wet granulation and direct compression methods. SLS (factor C) alone gave low dissolution rate in both wet granulation and direct compression methods. Overall direct compression method gave higher dissolution rates (K1) and dissolution efficiency (DE 30) values than the wet granulation method in all the cases. Combination of HPβCD with either PVP K30 or SLS or both gave a significantly higher dissolution rate (K1) of efavirenz in both wet granulation and direct compression methods. Hence a combination of HPβCD with either PVP K30 or SLS or both is recommended to enhance the dissolution rate and efficiency of efavirenz tablets.

Abstract:The European Academy of Otology and Neurotology in collaboration with the Japanese Otological Society (EAONO/JOS) recently produced a joint consensus document outlining the definitions, classification and staging of middle ear cholesteatoma. The goals were to provide terminologies in the description of cholesteatoma, classify cholesteatoma into distinct categories to facilitate the comparison of surgical outcomes and to provide a staging system that reflects the severity, difficulty of complete removal and restoration of normal function. Cholesteatoma is considered a benign, expanding and destructive epithelial lesion of the temporal bone that is the result of a multifactorial process. If undetected and left treated, cholesteatoma may lead to significant complications including hearing loss, temporal bone destruction and cranial invasion. Recent advances in imaging modalities have allowed for high sensitivity and specificity in identifying the presence of cholesteatoma. Despite these advances, deficiencies exist around the world with access to health care facilities meaning cholesteatoma remains a serious and challenging entity to manage whether found within the pediatric or adult population. Proper diagnosis and management of each form of cholesteatoma is achieved by a thorough understanding of the etiology, classification, clinical presentation and histology, thereby facilitating prevention, early detection and appropriate treatment.

Abstract:OBJECTIVE:To explore the clinical characteristics, diagnosis and surgical management of petrous bone cholesteatoma.METHOD:The data of 12 patients who underwent surgery for petrous bone cholesteatoma(PBC) were retrospectively analyzed with respects to the clinical characteristics, diagnosis and management.RESULT:Seven cases were characterized by a long otitis media history, severe hearing loss, and facial paralysis. In contrast, 5 cases were characterized with the symptoms of facial paralysis, hearing loss and vertigo attack and the absence of early otorrhea history. Trans labyrinth approach was chosen for 2 cases,while trans labyrinth-cochlear approach for 10. Cochlea was involved in 10 cases, while internal auditory canal in 9 and semicircular canal in 11. Otoscope was used in 1 case. Four patients were treated by partial resection of petrous apex and ear canal closure with good results. After years of follow-up, three recurrence cases were operated for a second time. Simultaneous facial nerve anastomosis or decompression was performed. The function of facial nerve recovered from V-VI to Ill-IV (House and Brackmann grading) in 6 anastomosis cases and from IV-V to II- IlI in 3 cases of decompression.CONCLUSION:Petrous bone cholesteatoma was characterized by severe hearing loss, and facial paralysis. Surgical approaches are decided according to the extent of lesion and hearing status. Our study indicated that otoscope would help to ensure a radical removal of the pathology in cases with deep involvement and restricted vision. Partial resection of petrous bone and ear canal closure could be effective solution for challenging cerebrospinal fluid otorrhea with large dural defects and protecting vital neurovascular structures.

Results: The mean patient age ± standard deviation was 38.4±19.6 years. The patients comprised 19 men and 25 women. According to the JOS classification, 18 ears (40.9%) were classified as stage I, 22 (50.0%) as stage II, and 4 (9.1%) as stage III. According to the EAONO/JOS joint consensus statements, 14 ears (31.8%) were classified as stage I, 26 (59.1%) as stage II, and 4 (9.1%) as stage III. Fourteen ears (31.8%) demonstrated involvement of the sinus tympani. Four ears (9.1%) that were originally categorized as stage I cholesteatoma by the JOS criteria showed sinus tympani invasion and were subsequently categorized as stage II according to the EAONO/JOS criteria. Conclusion: We determined the clinical characteristics of pars tensa cholesteatoma based on the novel and well-defined classification criteria. Further studies including long-term outcomes are necessary to demonstrate the clinical relevance of the discrepancy between the two criteria with respect to involvement of the sinus tympani.

Abstract:OBJECTIVE To develop a staging system for congenital cholesteatoma in predicting the likelihood of residual disease. DESIGN Retrospective analysis of data from a case series, to identify predictors of residual disease. SETTING Tertiary care pediatric hospital. PARTICIPANTS Children undergoing surgical removal of congenital cholesteatoma. There were 156 patients, with 160 cholesteatomas; 4 children had bilateral disease. INTERVENTIONS Each case was scored as to quadrants of the middle ear involved, ossicular involvement, and mastoid extension. MAIN OUTCOME MEASURE Surgically confirmed residual disease at any time after the initial procedure. RESULTS Four stages were defined as follows: stage I, disease confined to a single quadrant; stage II, cholesteatoma in multiple quadrants, but without ossicular involvement or mastoid extension; stage III, ossicular involvement without mastoid extension; and stage IV, mastoid disease. There was a strong association between stage and residual disease, ranging from a 13% risk in stage I to 67% in stage IV. CONCLUSIONS This simple staging system may be particularly useful in standardizing the reporting of congenital cholesteatoma and in adjusting for severity in evaluating outcomes. It also provides information that is useful in counseling parents.

Abstract:Among 1,041 women fitted with intra-uterine contraceptive devices during a period of 21,445 months there were 51 pregnancies: a rate of 2.9 per hundred women years. Pregnancy occurred most commonly in the young and nulliparous. The importance of the patient checking her device is stressed. Pregnancy occurs most commonly soon after fitting, but may occur at any time. In more than half the cases pregnancy occurred with the device in the uterus. Though most pregnancies were uterine there was a high incidence of ectopic pregnancies. Many pregnancies ended in vaginal delivery, but since the Abortion Act 1967 fewer probably now go to term.

Abstract:BACKGROUND: The role of imaging in cholesteatoma continues to evolve with excellent bony details provided by high-resolution computed tomography and high soft tissue identification for cholesteatoma by diffusion-weighted magnetic resonance imaging. The fusion of highresolution computed tomography and diffusion-weighted magnetic resonance imaging combines the advantages of both imaging techniques.METHODS: A random sample of 40 consecutive patients with chronic suppurative otitis media with cholesteatoma was included in this study. Both high-resolution computed tomography of the petrous bone and non-echoplanar diffusion-weighted magnetic resonance imaging were performed. This was followed by their fusion. Patients were classified according to The European Academy of Otology and Neurotology, in cooperation with the Japanese Otological Society Joint Consensus Statement on the Definitions, Classification, and Staging of Middle Ear Cholesteatoma. All patients were operated, and the technique was tailored according to the data obtained from the preoperative fusion of computed tomography and diffusion-weighted magnetic resonance imaging and the intraoperative findings.RESULTS: Patients were equally divided between males and females with a mean age of 26.8 years of which 52.5% were left-sided ears. The fusion of high-resolution computed tomography and diffusion-weighted magnetic resonance imaging had a 100% sensitivity and 88.9% specificity regarding The European Academy of Otology and Neurotology, in cooperation with the Japanese Otological Society classification. On the other hand, it showed 100% specificity and 100% sensitivity for all middle ear subsites except sinus tympani which obtained 55.56% sensitivity and 100% specificity. In all patients with preoperative fusion showing cholesteatoma not reaching the mastoid antrum (30%), exclusive endoscopic approach was employed, and no postauricular incision was needed.CONCLUSION: The fusion of high-resolution computed tomography and diffusion-weighted magnetic resonance imaging images is an accurate tool for localizing cholesteatoma in various middle ear cleft subsites. This makes it a valuable tool for cholesteatoma classification and staging and surgical planning preoperatively.
